비침습적 무통증 신호요법

Abstract

평가배경 비침습적 무통증 신호요법(Scrambler therapy)은 기존 통증치료로 관리되지 않는 만성 통증, 암성 통증 및 난치성 통증 환자를 대상으로 통증 경감을 목적으로 주요 통증 신경경로에 전극을 부착하여 미세전류와 함께 무통증 정보를 신경으로 전달하여 무통증 인지치료를 하는 시술이다. 본 의료기술은 2012년 제11차 신의료기술평가위원회(2012.11.23.)에서 안전성 및 유효성이 있는 기술로 인정되어 ‘비침습적 무통증 신호요법’라는 기술명으로 신의료기술 안전성 유효성 평가결과 고시 되었고(보건복지부 고시 제2013-30호, 2013.2.21.), 2014년에 등재비급여가 되었다(2014.6.1.). 2021년 주제 수요조사를 통해 의료기술재평가 대상으로 제안된 기술로 우선순위평가를 통해 재평가 대상으로 선정되었으며 2021년 제3차 의료기술재평가위원회(2021.3.12.)에서 평가계획서 및 소위원회 구성(안)을 심의, 2021년 제12차 의료기술재평가위원회(2021.12.10.)에서 최종심의하였다. 본 평가의 목적은 비침습적 무통증 신호요법의 임상적 안전성 및 효과성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하는 것이다.

평가 목적 및 방법 비침습적 무통증 신호요법에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “비침습적 무통증 신호요법 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회는 구성은 재활의학과 1인, 정형외과 1인, 혈액종양내과 1인, 마취통증의학과 1인, 신경과 1인, 신경외과 1인, 근거기반의학 1인의 전문가 총 7인으로 구성하였다. 평가의 핵심질문은 i) 비침습적 무통증 신호요법은 기존 통증치료로 관리되지 않는 만성 통증 환자에서 통증을 경감시키는데 있어 임상적으로 안전하고 유효한가? ii) 비침습적 무통증 신호요법은 기존 통증치료로 관리되지 않는 만성 통증 환자에서 통증을 경감시키는데 있어 임상적으로 안전하고 유효한가? iii) 비침습적 무통증 신호요법은 기존 통증치료로 관리되지 않는 난치성 통증 환자에서 통증을 경감시키는데 있어 임상적으로 안전하고 유효한가?로 시술대상자별로 구분하여 제시하였다. 안전성은 시술 관련 부작용 및 이상반응, 효과성은 통증감소정도, 진통제 사용 감소정도, 삶의 질 영향 정도로 평가하였다. 위의 핵심질문을 토대로 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, 과학기술정보통합서비스) 및 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials)을 이용하여 문헌을 검색하였다. 문헌 선정기준 및 배제기준 적용을 통한 문헌선택, 자료추출, 비뚤림위험 평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 문헌의 비뚤림위험 평가는 무작위배정 비교임상연구의 경우 Cochrane의 Risk of Bias (RoB)를 사용하였고, 전후연구의 경우 Risk of Bias for Nonrandomized Studies (RoBANS Ver.2)를 사용하였다. 자료분석은 정량적 분석(quantitative analysis)이 가능한 경우는 메타분석을 수행하였다. 메타분석은 연속형 변수는 통합 표준화된 평균차이(standardized means difference, SMD), 이분형 변수는 비교군 연구의 경우 통합 오즈비, 단일군 연구의 경우 통합 비율을 95% 신뢰구간(confidence interval, CI)과 함께 제시하였다. 정량적 분석이 불가능한 경우는 정성적으로 분석하였다(qualitative review). 체계적 문헌고찰 결과의 근거수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.

평가 결과 비침습적 무통증 신호요법의 안전성과 효과성은 총 30편(국내 3편, 국외 27편)에 근거하여 평가하였다. 연구유형별로는 무작위배정 비교임상연구 10편, 전후연구 19편, 증례연구 1편이었다. 비교시술별로는 경피적 신경전기자극치료(transcutaneous electrical nerve stimulation, TENS)와 비교 4편, 모의대조군과 비교 4편, 약물치료와 비교된 연구 2편이었다. TENS와 비교한 4편 중 1편에서는 두 군 모두 관절가동술을 수행하였으며, 약물치료와의 비교연구 2편은 중재군은 약물치료와 비침습적 무통증 신호요법을 병행한 그룹이었으며, 비교군은 약물치료만 수행한 그룹이었다. 비뚤림위험은 무작위배정 비교임상연구에서 연구자 및 연구참여자의 눈가림 및 결과평가에 대한 눈가림, 불완전한 자료, 선택적 보고에 대해 40~50%에서 비뚤림위험이 ‘높음’으로 평가되었다. 전후 연구에서는 대상군 선정, 교란변수, 노출측정, 불완전한 결과자료에 대해 5~15%에서 비뚤림위험이 ‘높음’으로 평가되었다. 안전성 결과는 총 30편 중 2편에서 중재시술과 관련하여 경미한 반상출혈, 접촉성 피부염, 발작(seizure)의 이상반응이 4.2~6.3%로 확인되었다. 이 중 발작은 1편에서 16명 중 1명(6.3%)에서 보고되었으며, 해당 환자는 치료 6일 후에 뇌전이 진단을 받았고 치료로 기인된 부작용은 아닌 것으로 보고하였다. 이에 소위원회에서는 발작 1건을 제외하고는 대부분 경미한 부작용으로 발작 역시 치료와 관련성이 없는 것으로 보고되었고 소요장비의 식품의약품안전처 허가사항 내 사용시 주의사항에 의사와 상의해서 사용해야 할 적응증이 별도로 명시된 점을 고려했을 때 해당사항을 잘 준수한다면 동 기술은 안전한 기술로 판단된다는 의견이었다. 효과성 결과는 연구유형별로 구분하여 제시하되, 무작위배정 비교임상연구의 경우 비교시술별로 구분하여 지표별로 제시하였다. 효과성 지표는 통증감소정도, 진통제 사용감소정도, 삶의 질 영향정도로 구분하여 제시하였으며, 통증감소정도는 통증전후 점수 비교, 50%이상 통증감소한 환자비율에 대한 결과를 주요지표로 제시하였다. 비교군 연구 결과는 다음과 같다. 모의대조군과 비교시 통증감소정도는 2편에서 중재군이 비교군에 비해 통증이 유의하게 더 감소하였으며, 2편은 군간 유의한 차이를 보이지 않았다. 메타분석결과 군간 유의한 차이를 보이지 않았으며 이질성은 높게 나타났다(통합 SMD –1.17, 95% CI -2.81, 0.47, I2=93%). 진통제 사용 감소정도는 1편에서 1개월 추적관찰시점에서 군간 유의한 차이를 보이지 않았다. 삶의 질 영향정도는 2편 중 1편에서 우울 영역에서만 중재군에서 시술 전후 유의한 감소를 보였고 비교군은 시술 전후 유의한 변화를 보이지 않았으며, 나머지 1편에서는 군간 유의한 차이를 보이지 않았다. TENS와 비교시 통증감소정도는 3편에서 중재군이 더 유의하게 통증이 감소한 것으로 보고되었으나 1편에서는 군간 유의한 차이를 보이지 않았다. 메타분석결과 통합 SMD는 –0.90(95% CI -1.35, -0.44, I2=12%)로 비침습적 무통증 신호요법이 유의하게 더 감소한 것으로 나타났다. 50%이상 통증 감소한 환자 비율을 보고한 2편에서는 모두 군간 유의한 차이를 보이지 않았으나 메타분석 결과 통합오즈비는 3.40(95% CI 1.24, 9.30, I2=0%)으로 중재군에서 50%이상 통증 감소한 환자비율이 유의하게 높게 나타났다. 비침습적 무통증 신호요법과 약물치료 병용군과 약물치료 단독군을 비교한 2편에서 모두 중재군이 비교군에 비해 추적관찰시점에서 더 낮은 통증점수를 나타냈다. 메타분석결과 통합 SMD는 –2.62(95% CI -4.42, -0.82, I2=93%)로 중재군에서 더 많이 통증이 감소된 것으로 나타났으며, 이질성은 높게 나타났다. 진통제 사용 감소정도는 1편에서 진통제(트라마돌) 사용량은 시술 후 군간 유의한 차이를 보이지 않았으며, 마약성 진통제(몰핀) 사용량은 시술 후 중재군이 유의하게 더 낮았다. 전후연구에 대한 효과성 결과는 통증감소정도, 진통제 사용 감소정도 및 삶의 질 영향정도로 구분하여 제시하였다. 시술 전후 통증점수를 비교한 연구 14편에서 통증이 5.16~9.22에서 0.11~4.3으로 시술 후 통증이 유의하게 감소되었으며, 메타분석 결과 통합 SMD는 –5.35(95% CI -7.17, -3.53, I2=64%)로 시술 후 통증이 유의하게 낮아진 것으로 확인되었다. 50%이상 통증감소된 환자비율은 7편에서 38.1~93.3%로 보고되었으며, 메타분석결과 통합 비율은 0.63(95% CI 0.46, 0.81)으로 이질성이 높게 나타났다(I2=96%). 진통제 사용은 2편에서 시술 전·후 1~2주에 진통제 사용이 유의한 감소를 보이지 않았으며, 진통제 관련 약물감소비율(1편)은 71.4~89.3%, 진통제 사용의 50% 감소를 보인 환자 비율(1편)은 5.9%, 진통제 중단한 환자 비율(1편)은 81.8%로 확인되었다. 삶의 질은 1편에서 시술 2주후 신체, 심리, 사회, 환경 영역 모두 유의한 개선을 보였으며, 2편에서는 중간 이상의 만족도를 보인 환자 비율이 각각 50%, 82%로 보고되었다. 수면시간은 1편에서 시술 후 유의하게 증가하였다. 근거수준은 비뚤림위험 평가 및 이질성 등을 고려했을 때 매우 낮음으로 확인되었다. 이에 소위원회에서는 비침습적 무통증 신호요법과 약물치료 병용군과 약물치료 단독군을 비교한 결과에서 중재군에서 마약성 진통제 사용이 더 감소한 것으로 확인되어 본 기술이 마약성 진통제 사용을 감소시키는데 도움을 줄 수 있다고 판단하였다. 또한 본 기술의 적응증인 만성 통증, 난치성 통증 또는 암성통증 환자에서 완전한 통증감소를 위한 치료방법이 존재하지 않기 때문에 일부 비교연구 및 대부분의 전후연구에서 확인된 유의한 통증감소 결과는 임상적으로 의미가 있다고 판단하였다. 다만 모의대조군과 비교한 연구에서 통증감소에 대한 유의한 차이를 보이지 않아 효과성에 대한 근거는 아직 부족하다고 볼 수 있으며, 비뚤림위험이 전반적으로 높고 근거수준도 낮은 제한점이 있다는 의견이 있었다.

결론 비침습적 무통증 신호요법 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 비침습적 무통증 신호요법은 기존 통증치료로 관리되지 않는 만성 통증, 암성 통증 및 난치성 통증 환자를 대상으로 통증 경감을 목적으로 수행시 안전한 기술로는 판단되며, 효과성은 i) 통증조절에 어려움이 있는 대상자 특징을 고려했을 때 일부 연구에서 통증감소 및 진통제 사용감소와 같은 긍정적인 효과가 확인되어 임상적 유용성을 기대할 수 있어 유효한 기술로 판단하였다. 다만 현재의 낮은 근거수준을 고려했을 때 추후 눈가림 등의 비뚤림위험을 낮출 수 있게 잘 설계된 비교 연구를 통한 지속적인 근거축적이 더 필요하다는 의견과 ii) 통증 감소 및 진통제 사용감소와 같은 임상적 유용성은 기대되나 효과성을 입증하기에는 그 근거가 부족하다는 의견 및 iii) 현재 문헌적 수준의 질이 낮고 이질성이 높으며, 비뚤림위험이 높게 평가되어 근거수준은 낮으나, 통증조절에 어려움이 있는 대상자 특징을 고려했을 때 일부 연구에서 통증감소 및 진통제 사용감소와 같은 긍정적인 효과가 확인되어 유효한 기술로 판단한다는 의견으로 전문가간 이견이 있었다. 2021년 제12차 의료기술재평가위원회(2021.12.10.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “비침습적 무통증 신호요법”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 비침습적 무통증 신호요법의 소위원회 의견 중 기존 통증치료로 관리되지 않는 만성 통증, 암성 통증 및 난치성 통증 환자를 대상으로 통증 경감을 목적으로 수행시 안전한 기술로는 판단되며, 효과성은 통증조절에 어려움이 있는 대상자 특징을 고려했을 때 일부 연구에서 통증감소 및 진통제 사용감소와 같은 긍정적인 효과가 확인되어 임상적 유용성을 기대할 수 있는 유효한 기술로 판단되나 현재의 낮은 근거수준을 고려했을 때 추후 눈가림 등의 비뚤림위험을 낮출 수 있게 잘 설계된 비교 연구를 통한 지속적인 근거축적이 더 필요하다는 의견에 동의하였다. 추가적으로 장기효과에 대한 연구가 더 필요하고 비용효과성에 대한 검토가 필요하다고 판단하였다. 이에 따라 의료기술재평가위원회는 기존 통증치료로 관리되지 않는(불응성) 만성 통증, 암성 통증 및 난치성 통증 환자를 대상으로 비침습적 무통증 신호요법을 통증 경감 목적으로 수행시 안전하고 유효한 의료기술로 판단되나 그 근거가 아직 제한적이고 장기효과 및 비용-효과성에 대한 추가적인 근거가 더 필요하여 비침습적 무통증 신호요법을 조건부 권고로 심의하였다.

Scrambler Therapy

Background Scrambler therapy is a non-invasive analgesic cognitive therapy that uses electrodes attached to major pain nerve pathways to deliver microcurrents and analgesic information to the nerves to reduce pain in patients with chronic, cancer, or intractable pain that cannot be managed with conventional pain therapy. This health technology was determined as a safe and effective technology by the 11th New Health Technology Assessment Committee in 2012 (November 23, 2012), and the results of the safety and effectiveness assessments of the new health technology, “scrambler therapy,” were announced (Ministry of Health and Welfare Notice No. 2013-30; February 21, 2013). Subsequently, scrambler therapy was registered as a non-reimbursable coverage in 2014 (June 1, 2014). The technique was selected for reassessment based on determination of the priority of the technology to be evluated in 2021. The protocol was approved by the 3rd Health Technology Reassessment Committee of 2021(March 12, 2021) and a relevant subcommittee was designated. The final review was conducted at the twelfth session of the Committee of Health Technology Reassessment (December 10, 2021). The purpose of this assessment was to provide scientific evidence for the clinical safety and effectiveness of scrambler therapy and to support policy decision-making for issues, such as the appropriate use of this health technology.

Purpose and Methods A systematic literature review was conducted to assess the safety and effectiveness of scrambler therapy. All assessment methods were finalized after a review by the “scrambler therapy subcommittee” (hereinafter “Subcommittee”) with consideration of the purpose of the assessment. The subcommittee comprised seven experts—one each from rehabilitation medicine, orthopedic surgery, hematology and oncology, anesthesiology and pain medicine, neurology, neurosurgery, and evidence-based medicine. The key questions in the assessment were: i) “Is scrambler therapy clinically safe and effective for reducing pain in patients with chronic pain that cannot be managed with conventional pain therapy?”; ii) “Is scrambler therapy clinically safe and effective for reducing cancer pain that cannot be managed with conventional pain therapy?”; and iii) “Is scrambler therapy clinically safe and effective for reducing pain in patients with intractable pain that cannot be managed with conventional pain therapy?” Safety was assessed based on procedure-related side effects and adverse events (AEs), while effectiveness was assessed based on level of pain reduction, reduction in analgesic use, and degree of impact on patient quality of life (QoL). Articles were searched in five domestic (KoreaMed, KMBASE, KISS, RISS, SCIENCE ON) and three international (Ovid MEDLINE, Ovid EMBASE, and Cochrane Central Register of Controlled Trials) databases. Two assessors independently selected the articles based on the inclusion and exclusion criteria and also performed data extraction and risk of bias (RoB) assessment. Any disagreements were resolved by discussion with a third person to reach a consensus. For the RoB assessment, the Cochrane RoB tool was used for randomized controlled trials (RCTs), while the RoB for Non-randomized Studies was used for before-and-after studies. For the data analysis, a meta-analysis was performed if a quantitative analysis was possible. In the meta-analysis, continuous variables were presented as pooled standardized mean differences (SMDs), while dichotomous variables were presented as pooled odds ratios (ORs) for comparative study and pooled proportion for single study with 95% confidence intervals (CIs). A qualitative review was performed if quantitative analysis was not possible. The level of evidence of systematic literature reviewe were assessed using the Grading of Recommendations Assessment, Development, and Evaluation approach.

Results The safety and effectiveness of scrambler therapy were assessed based on a total of 30 articles (three domestic and 27 international articles). Regarding study designs, this assessment included 10 RCTs, 19 before-and-after studies, and one case series. The comparators included transcutaneous electrical nerve stimulation (TENS; n=4), sham control (n=4), and drug therapy (n=2). Of the four articles that compared scrambler therapy with TENS, one applied joint mobilization to both groups. In two articles that compared scrambler therapy with drug therapy, the intervention group received drug therapy combined with scrambler therapy while the control group received only drug therapy. In RCTs, the RoB was defined as “high,” with 40–50% for the blinding of researchers and participants, blinding of outcome assessment, incomplete data, and selective reporting. In before-and-after studies, the RoB was defined as “high” for 5–15% for the selection of control and confounding variables, exposure measurement, and incomplete outcome data. Regarding safety outcomes, two of the 30 articles reported AEs, including mild ecchymosis, contact dermatitis, and seizure, in 4.2–6.3% of patients. Of these, seizures occurred in one of 16 patients (6.3%) in one study. This patient was diagnosed with brain metastasis 6 days after the intervention; thus, it was determined that the AE was not attributable to the intervention. The Subcommittee considered that all reported AEs were mild, except for the one case with seizure, which was reportedly unrelated to the intervention. The precautions for use in the approval by the Ministry of Food and Drug Safety require consultation by a physician must regarding the indications for device use. Accordingly, the Subcommittee determined that scrambler therapy is a safe technology if these requirements are followed. Effectiveness outcomes were presented separately by study designs. For RCTs, the outcomes were presented separately by type of comparator. The effectiveness were presented separately for the degree of pain reduction, reduction in analgesics use, and degree of impact on QoL. For pain reduction, the comparison of pain scores before and after the procedure and the percentage of patients with 50% or more pain reduction were presented as main outcome.

The findings in articles according to groups were as follows: Compared with a sham control, two articles reported significantly greater pain reduction in the intervention group than in the control group, whereas two articles reported no significant difference between the two groups. The results of a meta-analysis showed no significant difference between the groups, while there was high heterogeneity (pooled SMD –1.17, 95% CI -2.81, 0.47, I2=93%). Regarding the reduction in analgesic use, one article reported no significant difference between the groups at 1-month follow-up. Concerning the degree of impact on patient QoL, compared with before the intervention, one of two articles reported a significant decrease in the score for the depression dimension after the intervention, with the control group showing no significant change after the intervention compared with before. The other article also reported no significant difference between the groups. When compared with TENS, three articles reported more significant pain reduction in the intervention group; however, one article showed no significant difference between the groups. The results of a meta-analysis showed a pooled SMD of –0.90 (95% CI -1.35, -0.44, I2=12%), indicating that scrambler therapy provided a more significant reduction. Two articles that reported the percentages of patients with ≥50% pain reduction observed no significant difference between the two groups. However, the results of the meta-analysis showed a pooled OR of 3.40 (95% CI 1.24, 9.30, I2=0%), indicating a significantly higher percentage of patients in the intervention group with ≥50% pain reduction. Two arthcile that compared the group with scrambler therapy and drug therapy and the group with drug therapy alone showed lower pain scores at follow-up in the intervention groups in both studies compared with the control group. The results of the meta-analysis showed a pooled SMD of –2.62 (95% CI -4.42, -0.82, I2=93%), indicating a greater pain reduction in the intervention group, although the heterogeneity was high. Regarding reduced analgesic use, one article reported no significant difference in the use of analgesics (tramadol) between the groups post-intervention, while the use of narcotic analgesics (morphine) was significantly lower in the intervention group post-intervention. In before-and-after studies, the effectiveness outcomes were presented separately according to the degree of pain reduction, reduction in analgesic use, and degree of impact on QoL. Fourteen articles comparing pre- and post-intervention pain scores reported a significant reduction in pain scores from 5.16–9.22 to 0.11–4.3 points. The results of the meta-analysis showed a pooled SMD of –5.35 (95% CI -7.17, -3.53, I2=64%), indicating a significant reduction in pain after the intervention. In seven articles, 38.1–93.3% of patients experienced a ≥50% pain reduction. The results of the meta-analysis showed a pooled proportion of 0.63 (95% CI 0.46, 0.81), indicating high heterogeneity (I2=96%). Two articles reported no significant reduction in analgesic use between before and 1–2 weeks after the intervention. Meanwhile, 71.4–89.3% of patients showed a reduction in the use of analgesic-related drugs (one article); 5.9% of patients showed a ≥50% reduction in analgesic use (one article); and 81.8% of patients discontinued using analgesics (one article). Regarding QoL, one article reported significant improvement in the physical, psychological, social, and environmental dimensions 2 weeks after the intervention, while two articles reported that 50% and 82% of patients, respectively, showed moderate or higher satisfaction. Meanwhile, one article reported a significant increase in sleeping hours after the intervention. The level of evidence was assessed to be very low based on the RoB assessment and heterogeneity results. The results of comparisons between the intervention (drug therapy + scrambler therapy) and control (drug therapy alone) groups showed greater reductions in the use of narcotic analgesics in the intervention group; thus, the Subcommittee determined that this technology can help reduce narcotic analgesic use. Moreover, because there is no treatment for complete pain reduction in patients with chronic, intractable, and cancer pain, which are the indications for this technology, the significant pain reductions found in some RCTs and most before-and-after studies were determined to be clinically significant. However, it was also determined that the evidence of effectiveness is still lacking, as studies performing comparisons with sham controls showed no significant differences. Moreover, the Subcommittee also considered that there are limitations, such as generally high RoB and low level of evidence.

Conclusions and recommendations Based on the current assessment results, the Subcommittee recommends the following for scrambler therapy: Scrambler therapy is determined to be a safe technology when used for the purpose of pain reduction in patients with chronic, cancer, or intractable pain that cannot be managed with conventional pain therapy. Regarding the effectiveness of this therapy, expert opinions were conflicting opinions regarding the following: i) Considering the characteristics of patients with difficulty controlling pain, this technology was determined to be effective since clinical usefulness may be expected based on the identification of positive effects in some studies, such as reduced pain and analgesic use. However, considering the low level of evidence at present, it is necessary to continue to accumulate evidence through well-designed comparative studies that can lower RoB, such as blinding; ii) While clinical usefulness such as reduction in pain and analgesic use may be expected, the evidence is not enough to prove its effectiveness; and iii) Currently, published articles have low quality and high heterogeneity, while the level of evidence was assessed to be low with high RoB. However, considering the characteristics of patients who have difficulty controlling pain, this technology was determined to be effective since clinical usefulness may be expected based on the identification of positive effects in some studies, such as reduction in pain and analgesic use. During the 12th Health Technology Reassessment Committe in 2021 (December 10, 2021), the following determination was made on scrambler therapy based on the Subcommittee findings under Article 4 Paragraph 10 of the Health Technology Reassessment Guideline. The Committee of Health Technology Reassessment concurred with the opinion of the Subcommittee that “Scrambler therapy is determined to a safe technology when used for the purpose of pain reduction in patients with chronic, cancer, or intractable pain that cannot be managed with conventional pain therapy. Regarding its effectiveness, considering the characteristics of patients who have difficulty controlling pain, this technology was determined to be effective since clinical usefulness may be expected based on the identification of positive effects in some studies, such as reduction in pain and analgesic use. However, considering the low level of evidence at present, it is necessary to continue to accumulate evidence through well-designed comparative studies that can lower RoB, such as blinding.” The committee also determined that there is a need for additional studies on the long-term effects and a review of the cost-effectiveness of this therapy. The Health Technology Reassessment Committe determined that scrambler therapy is a safe and effective health technology when used for the purpose of pain reduction in patients with (refractory) chronic, cancer, or intractable pain that cannot be managed with conventional pain therapy. However, the evidence is limited and additional evidence is needed regarding its long-term effects and cost-effectiveness, so it was deliberated on the conditional recommendation.
