방사성 요오드 치료 후 타액선염 및 구강건조증 환자에서 타액선 내시경술

Abstract

평가 배경 동 기술은 2021년 의료기술재평가 주제 수요조사를 통하여 재평가 안건으로 발굴되었으며, 우선순위 심의를 거쳐 2021년 제4차 의료기술재평가위원회(‘21.4.9.)에서 재평가 대상으로 최종 선정되었다. 제5차 의료기술재평가위원회(‘21.5.27.)에서는 의료기술재평가 과정에서 일반 국민이 판단하는 동 기술의 가치 및 선호를 반영하고자 국민참여단을 통한 사회적 가치평가 시범안건으로 선정하였다. 본 평가의 목적은“ 방사성 요오드 치료 후 타액선염 및 구강건조증 환자에서 타액선 내시경술”의 임상적 안전성 및 효과성 등에 대한 의·과학적 근거를 제공하여 의료기술의 적정사용 등 정책적 의사결정을 지원하는 것이다.

평가 방법 ‘방사성 요오드 치료 후 타액선염 및 구강건조증 환자에서 타액선 내시경술’의 임상적 안전성 및 효과성 등에 대하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “방사성 요오드 치료 후 타액선염 및 구강건조증 환자에서 타액선 내시경술 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 이비인후과 2인, 구강악안면외과 1인, 구강내과 1인, 내분비대사내과 1인, 근거기반의학 1인의 전문가로 구성하였다. 평가의 핵심질문은 “방사성 요오드 치료 후 타액선염 및 구강건조증 발생 환자에서 타액선 내시경술은 안전하고 유효한가?”이었고 안전성은 시술 관련 합병증을, 효과성은 증상개선, 기능개선, 삶의 질, 시술실패율로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선택하였다. RoBANS를 사용한 문헌의 비뚤림 위험 평가와 자료추출은 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 논의하여 합의하였다. 자료분석은 질적 검토(qualitative review) 및 정량적 분석(quantitative analysis)을 수행하였다.

평가 결과 본 평가에 포함된 문헌은 총 12편(중재군 대상자 수 총 181명)으로, 비교군 연구 1편(타액선 내시경술 미시행군과 비교 1편), 단일군 연구(전후 비교 4편, 증례 7편) 11편이 포함되었다. 연구에 포함된 대부분의 환자들은 보존적 치료(마사지, 타액 분비 촉진제, 온찜질, 인공 타액 및 수분공급 등)를 받았으나, 이러한 치료에 반응이 없었던 환자들이었다. 또한 타액선 내시경술은 타액선 관의 협착이 발견된 경우에는 필요시 관 확장술이 시행되었으며, 관내의 염증이 심한 경우에는 세척술 및 스테로이드를 섞어 관내에 주입하는 등 연구들마다 다양한 보조치료가 함께 수행된 것으로 확인되었다.

안전성 안전성 결과를 보고한 연구는 총 10편이었다. 4편의 연구에서는 대다수 환자에서 시술 후 경미하거나 일시적인 통증 및 타액선 부종 발생을 보고하였으며, 수술 후 감염 사례가 1편에서 보고되었다. 그 외 6편의 연구에서는 시술 관련 합병증 사례는 발생하지 않은 것으로 보고하였다.

효과성 효과성 결과를 보고한 연구는 총 11편이었다. 타액선 내시경술 미시행군(대조군)과 비교된 연구(1편)에서는 두 군 모두 시술 전후 기능지표 개선에 유의한 차이는 확인되지 않았고, 시술 후 3개월 시점에서의 구강건조증 관련 삶의 질은 중재군이 대조군에 비해 유의하게 개선되었다. 단일군 연구 10편 중 3편에서 보고한 주관적인 증상점수는 타액선 내시경술 전에 비해 시술 후 3개월 시점에서 유의하게 개선되었다. 타액선 내시경술을 시행한 환자에서 증상이 개선된 환자의 비율을 보고한 9편 연구에서 개선(부분 개선+완전개선)된 환자의 비율은 75-100%이었다. 기능지표 중 타액 분비율은 2편에서 보고되었으며, 시술 후 3, 6개월 시점에서 측정된 자극성 타액 분비율은 2편 모두 시술 전후 유의한 차이가 없었고, 비자극성 타액 분비율은 1편에서만 유의한 차이가 있는 것으로 보고되었다. 타액선 영상 검사결과를 보고한 2편 중 1편에서는 타액선 섭취율 및 분비분획률에 유의한 개선이 있었으나, 다른 1편에서는 세부 지표별로 결과가 상이하였다(타액선 섭취율은 유의한 감소를 보였으나, 최대분비율은 유의한 차이가 없었음). 삶의 질을 보고한 1편의 연구에서 시술 후 3개월 시점에서 구강건조증 관 삶의 질 점수는 평균 13.5% 개선되었으며, 개선된 환자의 비율은 66.7% (6/9명)이었다. 시술실패율을 보고한 5편에서의 시술실패율은 10(1/10)-50% (3/6)이었으며 이 때 시술 실패는 관의 협착 등으로 인하여 관 확장에 실패한 경우를 의미하였다.

결론 및 제언 소위원회는 “방사성 요오드 치료 후 타액선염 및 구강건조증 환자에서 타액선 내시경술”에 대하여 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 동 시술로 인한 관 천공, 신경 손상, 출혈 등의 중대한 합병증이 보고되지 않았고, 보고된 합병증 사례는 일시적이거나 경미한 수준에 해당되어 소위원회는 해당 시술이 안전성 측면에서 임상적으로 문제가 되지 않을 것으로 판단하였다. 주관적인 증상 및 삶의 질 측면에서 개선 효과가 있는 것으로 나타나 동 기술을 방사성 요오드 치료 후 기존의 치료방법에 반응하지 않고 증상이 지속되는 타액선염 및 구강건조증 환자들에서 증상개선을 위한 치료 대안으로 기대해 볼 수 있겠으나, 소위원회는 본 평가에 선택된 연구들의 대상자 수가 적고 객관적인 지표에서 유의한 개선효과가 확인되지 않고, 협착이 심한 환자에서 시술실패율이 높게 보고된 점과 함께 현재 근거수준이 낮고 전반적으로 근거가 부족한 상황으로 향후 동 기술과 관련한 전향적인 연구가 더 필요하다고 판단하였다. 2022년 제1차 의료기술재평가위원회(2022.1.14.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “방사성 요오드 치료 후 타액선염 및 구강건조증 환자에서 타액선 내시경술”에 대해 시술 대상이 암 환자에서 치료로 발생된 합병증 질환이고, 기존 치료에 반응하지 않는 환자로 대체 치료가 부족한 점을 종합적으로 고려하여 다음과 같이 심의하였다. 방사성 요오드 치료 후 타액선염 및 구강건조증 환자에서 타액선 내시경술은 기존의 보존적ㆍ내과적 치료에 반응하지 않는 방사성 요오드 치료 후 타액선염 및 구강건조증 환자에서 증상 완화를 목적으로 수행시 안전하고 효과적인 기술로 판단되나 그 근거가 아직 제한적이므로 ‘조건부 권고함’으로 심의하였다.

Background This technology was identified as an item for health technology reassessment (HRT) through a survey on technologies that required health technology reassessment. Following priority review, the technology was finally selected as the item subject to HRT at the 4th Health Technology Reassessment Committee (April 9, 2021) in 2021. At the 5th Health Technology Reassessment Committee (May 27, 2021), the technology was selected as a pilot item for social value evaluation through Citizen Participation Group in order to reflect the value and preference of this technology judged by general citizens of South Korea in the process of HRT. The purpose of this assessment is to provide medical and scientific evidence for the clinical safety and effectiveness of “Sialoendoscopy for Radioactive Iodine Therapy-induced Sialadenitis and Xerostomia” to support policy decision-making such as the optimal utilization of the health technology.

Methods A systematic review of the related literature was conducted for assessment of the clinical safety and effectiveness of “Sialoendoscopy for Radioactive Iodine Therapy–induced Sialadenitis and Xerostomia”. All assessment methods were finalized after review by the “Sialoendoscopy for Radioactive Iodine Therapy–induced Sialadenitis and Xerostomia Sub-committee (Hereinafter Sub-committee) in consideration of the purpose of the assessment. The Sub-committee consisted of experts in the relevant fields as follows: two in Otolaryngology, one in Oral and maxillofacial surgery, one in Oral medicine, one in Endocrinology and Metabolism, and one in Evidence-based Medicine The key question of the assessment was “Is sialoendoscopy safe and effective for patients with sialadenitis and xerostomia after they underwent radioiodine therapy?” Safety was evaluated with post-procedural complications as outcome measures and effectiveness was evaluated based on improvement in symptoms, functional improvement, quality of life and procedure failure rate. Based on the key question, systematic review was conducted with literature search on three international and five domestic databases, and the articles were selected based on independent screening by two reviewers according to the inclusion/exclusion criteria for the related literature. The assessment of the risk of bias of the literature using the tool of Risk of Bias Assessment for Nonrandomized Studies (RoBANS) and data extraction were conducted independently by two reviewers and in case of disagreement, the two reviewers held discussions to draw consensus on the matter. For methods of data analysis, quantitative analysis and qualitative review were applied.

Results A total of twelve articles were included in this assessment (a total of 181 patients in the intervention group), consisting of one comparative study (one article comparing the intervention group with a control group (the group without undergoing sialoendoscopy)), and eleven single-arm studies (four articles of pre-post study, seven articles of case reports). Most of the patients included in these studies underwent conservative treatments (salivary gland massages, sialagogues, warm compression, artificial saliva, and adequate hydration) but their symptoms were unresponsive to these treatments. In addition, if stenosis of the salivary duct was found, ductal dilatation was conducted where necessary, and in case of severe intraductal inflammation, ductal washout or intraductal corticosteroid injection were conducted, confirming that sialoendoscopy was conducted in combination with various adjunct therapies depending on the studies.

Safety A total of ten studies reported on outcomes of safety. In four studies, mild or transient pain and swelling of salivary glands after the procedure were reported in the majority of patients, and postoperative infection was reported in one study. In the other six studies, no post-procedural complications were reported.

Effectiveness A total of eleven studies reported on outcomes of effectiveness. In the study (one article) that compared the intervention group with the group without sialoendoscopy (control group), there was no significant difference in the improvement of functional outcome measures before and after the procedure in both groups. At 3 months after the procedure, a significant improvement in the xerostomia-related quality of life was reported in the intervention group compared to that of the control group. The scores of subjective evaluation of symptoms reported in three out of ten single-group studies were significantly improved three months after the procedure compared to the scores before sialoendoscopy. In nine studies that reported the proportion of patients with symptomatic improvement in patients who underwent sialoendoscopy, the proportion of patients that reported resolution (partial resolution + complete resolution) was 75-100%. Among the functional outcome measures, saliva secretion was reported in two studies; stimulated saliva secretion measured at three and six months after the procedure did not show a significant difference before and after the procedure in both studies, and unstimulated saliva secretion showed a significant difference in only one study. In one of the two articles that reported the findings of salivary gland scintigraphy, there was a significant improvement in the uptake ratio and excretion fraction, but in the other article, the findings were different depending on the detailed outcome measures (uptake ratio showed a significant decrease, while maximum secretion showed no significant difference). In one study reporting quality of life, the scores of xerostomia-related quality of life improved by an average of 13.5% at three months after the procedure, and the percentage of patients with improvement was 66.7% (6/9 patients). In the five articles that reported the procedure failure rate, the values ranged from 10(1/10) to 50% (3/6). In this case, procedure failure indicated cases of failure in ductal dilatation due to factors such as salivary duct stenoses.

Conclusion and Suggestions The Sub-committee made the following suggestions for “Sialoendoscopy for Radioactive Iodine Therapy-induced Sialadenitis and Xerostomia” based on the results of the present HRT. No major complications such as perforation of the ducts, nerve injury, bleeding, etc. were reported due to the procedure, and the reported cases of complications were temporary or at a minor level; therefore, the Sub-committee determined that the procedure would not cause problems in terms of clinical safety. As the technology has been shown to have an effect of improvement in terms of subjective evaluation of symptoms and quality of life, this treatment method may be expected as an alternative method for improvement of symptoms in patients with sialadenitis/xerostomia following radioiodine therapy who are unresponsive to conservative therapies and experience persistent symptoms. However, the Sub-committee considered that since the number of samples used in selected studies is small, significant effect of improvement was not confirmed in objective outcome measures, and that the procedure failure rate was high for patients with severe stenosis, the current level of evidence is low and there is a lack of evidence in general. Therefore, it was determined that further prospective studies are required in relation to the technology in the future. Based on the results of the review by the Sub-committee, at the 1st Health Technology Reassessment Committee (2022.14.) in 2022, the following is the result of the review for “Sialoendoscopy for Radioactive Iodine Therapy-induced Sialadenitis and Xerostomia” in accordance with Article 4, Paragraph 10 of the Management Guidelines for Health Technology Reassessment Project, in comprehensive consideration that the target conditions are complications from treatments of cancer patients, the patients were unresponsive to conventional treatments and that there is a lack of alternative therapies for these patients. The review by the Health Technology Reassessment Committee determined that “sialoendoscopy for radioactive iodine therapy–induced sialadenitis and xerostomia” is considered a safe and effective technology for improving symptoms in patients with sialadenitis and xerostomia following radioiodine therapy whose symptoms are unresponsive to conventional conservative treatments and medical management, but due to the limitation of the evidence, the Grade of Recommendation was “Conditional Recommendation.”
