과립구 흡착요법

Abstract

평가 배경 과립구 흡착요법은 중증의 활동기 궤양성 대장염환자를 대상으로 체외 순환형 혈액정화기를 이용하여 말초혈중의 과립구 및 단구를 선택적으로 흡착하는 기술이며, 신의료기술평가제도가 확립되기 전인 2006년 비급여로 등재되었다. 2021년 의료기술재평가 내부 모니터링을 통해 발굴된 기술로 우선순위심의를 통해 재평가대상으로 선정하여 2021년 제4차 의료기술재평가위원회(2021.04.09.)에서 평가계획서 및 소위원회 구성(안)을 심의하였으며, 2021년 제 11차 의료기술재평가위원회(2021.11.12.)에서 최종심의를 수행하였다. 본 평가의 목적은 과립구 흡착요법의 임상적 안전성 및 유효성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하는 것이다.

평가 방법 과립구 흡착요법의 안전성 및 유효성을 평가하기 위하여 업데이트 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 소화기내과 3인, 외과 1인, 소아청소년과(소아소화기) 1인, 근거기반의학 1인 등 총 6인으로 구성된 과립구 흡착요법 재평가 소위원회(이하 소위원회)의 심의를 거쳐 확정하였다. 핵심질문은 ‘중증의 활동기 궤양성 대장염 환자에서 과립구 흡착요법은 임상적으로 안전하고 유효한가?’이다. 본 핵심질문을 토대로 검색된 체계적 문헌고찰 문헌들을 문헌선정 및 배제기준에 따라 적합한 체계적 문헌고찰 문헌을 선택하였으며 AMSTAR(A MeaSurement Tool to Assess systematic Reviews)를 이용하여 비뚤림위험 평가를 수행하였다. 선택된 체계적 문헌고찰 문헌의 검색일을 기준으로 1년 전부터 현재까지 간행된 문헌을 검색하였고, 선택된 체계적 문헌고찰 문헌의 분석대상으로 포함된 원저들(original articles)과 중복되지 않는 원저들을 추가적으로 선택하였다. 선택된 체계적 문헌고찰 문헌에 포함된 원저 연구들과 추가로 포함된 원저들 모두에 대하여 Cochrane의 Risk of Bias를 이용하여 비뚤림위험 평가를 수행하였고, 자료추출을 진행하였다. 문헌 검토 및 선정, 비뚤림위험 평가, 자료 추출 등에 대한 모든 과정은 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 최종 자료분석은 정성적 분석을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.

평가 결과 최종 선택된 체계적 문헌고찰 문헌(Kiss 등, 2021)에 포함된 11편의 원저 문헌들과 추가로 선정된 문헌 3편을 포함하여 총 14편의 무작위 배정 임상시험연구(Randomized controlled trial, RCT)가 선정되어, 해당 연구들을 기준으로 평가를 수행하였다. 중재군, 비교군 모두 기존의 약물치료 유지하면서 중재군에만 과립구 흡착요법 수행하는 경우이고, 비교군은 기존 약물 치료만을 유지하거나, 가짜치료(sham)이 적용된 경우였다. 문헌의 비뚤림위험 수준은 대체로 낮은 것으로 평가되었으나, 일부 문헌에서 참가자와 평가자의 눈가림이 진행되지 않은 경우가 있었으며, 기기 제조업체의 연구비를 지원받은 일부 연구들에게서 높은 수준으로 평가받은 항목들이 존재했다.

안전성 안전성 관련 지표에 대한 결과값은 14편 중 13편의 문헌에서 보고하고 있었으나, 과립구 흡착요법의 수행과 직접적으로 관련된 안전성 지표를 별도로 구분하여 보고하지 않는 경우가 대부분이었다. 동 기술과 가짜치료(sham)를 비교한 1편의 대규모 연구에서 보고된 두통, 저혈압 등의 다빈도 부작용은 항목별로 결과의 방향성이 상이하였으며, 2개 항목(두통, 복부통증)을 제외하고 중재군과 비교군간 통계적으로 유의한 차이가 없었다. 해당 문헌에서 보고한 부작용 지표들이 시술 과정과의 관련성이 불분명하다고 소위원회는 판단하였다. 13편의 문헌에서 중재시술과 관련되었다고 보고한 부작용들은 대부분 경미한 부작용(예. 두통, 오한, 메스꺼움(nausea), 복부 통증, 어지러움, 상열감(flushing), 피부가려움 등)에 해당하였다. 1개 문헌에서 중등도 이상의 부작용으로 아나필락시스 1건을 보고하였으며, 다른 1개 문헌에서 시술 부위의 혈종 발생 1건이 보고되었다. 시술 과정 중 일시적 저혈압의 발생 4건을 보고한 1개 문헌도 있었다. 전반적으로 선택문헌에서 보고한 부작용 및 이상반응들 중, 과립구 흡착요법과 관련된 중등도 이상의 부작용으로 주의 깊게 고려할만한 부작용은 없는 것으로 소위원회는 판단하였다.

유효성 과립구 흡착요법의 유효성은 소위원회의 논의를 바탕으로 주요 지표(관해/반응), 기타 지표(삶의 질, 생체 반응지표)등으로 평가하였다. 관해율을 보고한 모든 연구(11편)에서 중재군의 관해율이 비교군(과립구 수행요법 미수행)의 관해율보다 높게 나타났으나, 통계적으로 유의하게 높은 경우는 2편에 불과하였다. 2편의 연구는 모두 2008년에 출판된 연구이며, 최근 10년 내에 출판된 연구들 중 유의미한 결과를 보고한 경우는 없었으며, 특히 미국/유럽/일본에서 진행된 대규모 연구 1편의 결과에서도 통계적으로 유의한 차이를 보이지 않았다. 반응률을 보고한 모든 연구(9편)에서 중재군의 반응률은 비교군(과립구 수행요법 미수행)의 반응률보다 높게 나타났으나, 통계적으로 유의하게 높은 경우는 3편이었다. 유의한 차이를 보고한 3편 연구 중 2편은 각 군의 환자수가 10명 이하의 소규모 연구였다. 미국/유럽/일본에서 진행된 대규모 연구에서도 통계적으로 유의한 차이를 보이지 않았다. 생체 반응지표들의 치료 전후 값을 비교한 1편의 연구에서 동 시술의 치료 후 통계적으로 유의하게 개선된 생체 반응지표를 보고하였다. 1편의 연구에서 동 시술과 비교군 간의 삶의 질의 변화를 비교하였는데, 통계적으로 유의한 차이가 없었다. 유효성 지표 관련하여 연구마다 결과지표 정의 방식에 차이가 있어 직접 비교에 문제가 있는 경우들도 있었고, 일부 연구들의 경우 표준화되지 않은 방식으로 유효성 지표들을 정의하여 사용하기도 하였다. 일부 긍정적인 효과를 보고한 연구들도 있으나, 대규모 연구의 결과를 포함하여 통계적으로 유의하지 않은 결과를 제시하는 자료가 더 많았다.

결론 및 제언 과립구 흡착요법 재평가 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 궤양성 대장염 환자에서 과립구 흡착요법의 안전성은 시술과 직접적인 관련성이 있는 부작용 및 합병증 대부분이 경미한 항목이며, 중등도 이상의 부작용 사례가 드문 안전한 기술로 판단하였다. 유효성 측면에서는 궤양성 대장염 치료에서 중요한 변수인 관해, 반응 면에서 대규모 환자자료에 기반한 연구결과를 포함하여 상당수의 연구에서 과립구 흡착요법 수행군과 미수행군간의 효과차이가 없어 동 기술을 유효하지 않은 의료기술로 판단하였다. 아울러 본 기술이 도입된 2000년대 중반 이후로 궤양성 대장염 치료에 이용가능한 치료법이 다양해져 현시점에서 동 기술의 임상적 유용성은 기술 도입시기보다 낮아 임상에서 활용되지 않는다는 의견이었다. 2021년 제11차 의료기술재평가위원회(2021.11.12.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “과립구 흡착요법”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 과립구 흡착요법의 안전성은 시술과 직접적인 관련성이 있는 부작용 및 합병증 대부분이 경미한 항목이며, 중등도 이상의 부작용 사례가 드문 안전한 기술로 판단하였다. 유효성 측면에서는 궤양성 대장염 치료에서 중요한 변수인 관해, 반응 면에서 대규모 환자자료에 기반한 연구결과를 포함하여 상당수의 연구에서 과립구 흡착요법 수행군과 미수행군 간의 효과차이가 없어 동 기술을 유효하지 않은 의료기술로 판단하였다. 이에 따라 의료기술재평가위원회는 중증의 활동기 궤양성 대장염 환자치료에 과립구 흡착요법을 수행하는 것에 대해 권고하지 않음으로 심의하였다.

Granulocyte/Monocyte Adsorption Apheresis

Background Granulocyte/monocyte adsorption apheresis (GCAP) is the technology that uses an extracorporeal apheresis instrument to selectively adsorb granulocytes and monocytes from the peripheral blood of patients with active ulcerative colitis. This technology was registered as a non-covered item in 2006, i.e., prior to the establishment of the new health technology assessment system. As it was a technology identified through internal monitoring for the 2021 health technology reassessment, GCAP was selected as a subject of reassessment through a priority review. In 2021, the fourth session of the Committee of Health Technology Reassessment (April 9, 2021) reviewed the assessment plan and the proposed composition of a subcommittee. Subsequently, the final review was conducted by the eleventh session of the Committee of Health Technology Reassessment (November 12, 2021). The purpose of this assessment was to provide scientific evidence for the clinical safety and effectiveness of GCAP and support policy decision making for various issues such as the appropriate use of this health technology.

Methods An updated systematic literature review was conducted to assess the safety and effectiveness of GCAP. All assessment methods were finalized after a review by the “GCAP reassessment subcommittee” (hereinafter “Subcommittee”), considering the purpose of the assessment. The Subcommittee comprised six experts – three from gastroenterology and one each from surgery, pediatrics (pediatric gastroenterology), and evidence-based medicine. The key question in the assessment was “Is GCAP clinically safe and effective for patients with severe active ulcerative colitis?” Articles selected based on the key question were screened according to the inclusion and exclusion criteria for the systematic literature review, and a risk of bias (RoB) assessment was performed using the AMSTAR (A MeaSurement Tool to Assess systematic Reviews) criteria. Articles that were published within one year from the search date were searched and selected for the systematic literature review, and original articles that were not duplicates of the systematic review articles included in the analysis set were additionally selected. The RoB of the original articles selected for the systematic literature review and the original articles additionally selected were assessed using the Cochrane RoB tool, followed by data extraction. All processes, including literature review and selection, RoB assessment, and data extraction, were performed independently by two reviewers. In the event of a disagreement, a consensus was reached through a discussion with a third person. A qualitative analysis was used for the final data analysis. The level of evidence of the systematic review articles was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach.

Assessment results The assessment was based on 14 randomized controlled trials (RCT) described in 11 original articles selected for the systematic literature review (KISS, etc., 2021) and three additionally selected original articles. The intervention was the application of GCAP while maintaining existing drug therapy, whereas the control group received no additional treatment or sham treatment while maintaining the existing drug therapy. The level of RoB in the articles was assessed to be mostly low. However, participant and/or assessor blinding was not applied in some articles, and some studies that were funded by the device manufacturer reported high scores for certain items.

Safety Outcomes regarding safety-related indicators were reported in 13 out of 14 articles; however, in most cases, safety indicators directly related to GCAP were not reported separately. Common adverse events (AEs), such as headache and hypertension, reported in one large-scale study that compared GCAP and sham treatment showed different directionalities in the outcomes depending on the item. The results also showed no statistically significant differences between the intervention and control groups, except for two items (headache and abdominal pain). The Subcommittee determined that the association between the AEs reported in the article and the intervention group was unclear. Intervention-related AEs reported in the 13 articles were mostly mild AEs (e.g., headache, chills, nausea, abdominal pain, dizziness, flushing, itchy skin, etc.). One article reported a case of anaphylaxis as a moderate-to-severe AE, whereas another article reported a case of hematoma at the intervention site. Further, one article reported four cases of transient hypotension during the intervention period. The Subcommittee determined that there were no moderate-to-severe AEs related to GCAP that require careful consideration among the AEs and reactions reported in the selected articles.

Effectiveness The effectiveness of GCAP was assessed by key indicators (remission/response) and other indicators (quality of life and bio-response indicators) based on a discussion by the Subcommittee. In the studies that reported on remission rate (n=11), the remission rate was higher in the intervention group than that in the control group (no GCAP), but only two studies showed statistically significant differences. However, both studies were published in 2008, and no studies have been published within the past 10 years that reported significant results. In particular, even a large-scale study conducted in the US, EU, and Japan reported no statistically significant difference. In all the studies that reported on response rate (n=9), the response rate was higher in the intervention group than that in the control group (no GCAP), but only three studies showed statistically significant differences. Of these three studies, two studies were small-scale studies with ≤10 patients in each group. Even a large-scale study conducted in the US, EU, and Japan reported no statistically significant difference. In one study that compared the pre- and post-intervention values of bio-response indicators, statistically significant improvement was reported after the intervention using GCAP. In another study, changes in quality of life were compared between the GCAP and control groups, and the results were not statistically significant. With respect to the effectiveness indicators, direct comparison was difficult because the studies used different methods for defining the outcome indicators, whereas some studies used effectiveness indicators that had been defined using non-standardized methods. Although some studies reported positive effects, there were more studies that reported results that were not statistically significant, including the large-scale study.

Conclusions and recommendations The Subcommittee recommends the following based on the current assessment results: Regarding the safety of GCAP in patients with ulcerative colitis, AEs and complications that directly related to GCAP were mild. Accordingly, GCAP is determined to be a safe technology that rarely causes moderate-to-severe AEs. Regarding its effectiveness, most studies, including a study based on large-scale patient data, showed no significant difference between the GCAP and control groups in the remission and response rates, which are key variables in the treatment of ulcerative colitis. Accordingly, GCAP is determined to be a health technology that is not effective for ulcerative colitis. In addition, various treatment modalities for ulcerative colitis have become available since the mid-2000s when GCAP was first introduced, and the clinical effectiveness of GCAP presently is lower than that when the technology was introduced. Therefore, GCAP is not used in clinical practice. During the eleventh session of the Committee of Health Technology Reassessment in 2021 (November 12, 2021), the following was determined regarding GCAP based on the Subcommittee findings in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project: The Committee of Health Technology Reassessment has determined that AEs and complications directly related to GCAP were mild; thus, GCAP is a safe technology that rarely causes moderate-to-severe AEs. With respect to effectiveness, most studies, including a study based on large-scale patient data, showed no significant differences between the GCAP and control groups in the remission and response rates, which are key variables for treatment of ulcerative colitis,. Accordingly, GCAP is determined to be a health technology that is not effective for ulcerative colitis. In conclusion, the Committee of Health Technology Reassessment does not recommend the use of GCAP for treating patients with severe active ulcerative colitis.
