Abstract

평가 배경 이명재훈련치료(Tinnitus Retraining Therapy)는 약물요법 등에 반응하지 않는 이명환자에게 습관화를 유도하고 부정적인 감정을 느끼지 않도록 하여 이명을 인식하지 못하게끔 하는 치료 방법이다. 동 기술은 신의료기술평가 제도 이전에 비급여로 등재된 기술(보건복지부 고시 제2002-68호, 2002년 9월 27일)로 보건복지부 급여화 추진 계획 분야를 중심으로 이루어진 내부 모니터링을 통해 재평가 대상 안건으로 발굴되었다. 2021년 제4차 의료기술재평가위원회(‘21.4.9.)에서 재평가 대상으로 최종 선정되었으며, 제5차 의료기술재평가위원회(‘21.5.14.)에서는 의료기술재평가 과정에서 일반 국민이 판단하는 동 기술을 가치 및 선호를 반영하고자 국민참여단을 통한 사회적 가치평가 시범안건으로 선정하였다. 본 평가에서는 ‘이명재훈련치료’의 임상적 안전성, 효과성 등에 대한 의과학적 근거를 제공하고 의료기술의 적정사용 등 정책적 의사결정의 근거를 마련하고자 한다.

평가 방법 이명재훈련치료에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “이명재훈련치료 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회는 이비인후과 2인, 신경과 1인, 재활의학과 1인, 직업환경의학과 1인, 근거기반의학 1인의 전문가 6인으로 구성하였다. 평가의 핵심질문은 “약물요법 등에 반응하지 않는 이명환자에서 이명재훈련치료의 임상적으로 안전하고 효과적인가?”이었다. 안전성 결과변수로는 부작용 및 합병증, 효과성의 결과변수로는 이명장애점수, 이명개선율, 삶의 질 지표로 결정하고 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias (RoB)를 사용하여 평가하였으며, 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다.

평가 결과 이명재훈련치료의 안전성과 효과성은 총 11편에 근거하여 평가하였다. 중재치료는 습관화 치료(Habituational Therapy), 인지행동치료 + 소리치료, 수정된 이명재훈련치료, 이명재훈련치료 +보청기를 이명재훈련치료(지시적 상담 + 소리치료)로 정의하였고, 포함된 비교기술은 상담, 인지치료, 이명차폐(tinnitus total masking), 무치료(no treatment)로 보고되었다. 본 평가에서는 이명대처훈련(Tinnitus coping training), 인지행동치료(Cognitive behavioral therapy), 수용전념치료(Acceptance and commitment therapy)를 인지치료로 정의하고 평가하였다.

안전성 이명재훈련치료의 안전성은 1편의 문헌에서 보고된 부작용 및 합병증으로 평가하였다. 이명재훈련치료를 시행한 군의 경우, 1건의 소리발생기(sound generator)로 인한 외이도의 부종 혹은 통증이 발생하였다고 보고하였다.

효과성 이명재훈련치료의 효과성은 ‘상담 혹은 인지치료’, ‘상담’, ‘인지치료’, ‘이명차폐’, ‘무치료’와 비교하여 평가하였다. 결과변수는 소위원회의 논의를 통하여 이명장애점수, 이명개선율, 삶의 질 지표로 정하여 평가하였다. 이명재훈련치료와 상담 혹은 인지치료를 비교한 문헌 중 이명장애점수를 보고한 문헌은 총 9편이었다. 이명재훈련치료군과 상담 혹은 인지치료를 받은 군 간의 이명장애점수는 통계적으로 유의한 차이가 없는 것으로 나타났다(SMD –0.11, 95% CI –0.41~0.20, p=0.49, I2=74%). 이명개선율을 보고한 문헌은 총 5편으로 이명재훈련치료군과 상담 혹은 인지치료를 받은 군 간의 차이는 통계적으로 유의하지 않았다(RR 1.01, 95% CI 0.71~1.71, p=0.66, I2=53%). 이명재훈련치료와 상담 혹은 인지치료를 비교한 문헌 중 삶의 질 결과를 보고한 경우는 없었다. 이명재훈련치료와 상담군간의 이명장애점수를 보고한 총 8편의 문헌에서 이명재훈련치료군과 상담을 받은 군 간의 통계적으로 유의한 차이는 없는 것으로 나타났다(SMD –0.25, 95% CI –0.61~0.11, p=0.17, I2=75%). 이명개선율을 보고한 3편의 문헌에서 이명재훈련치료를 받은 환자군에서 상담을 받은 환자에 비해 이명개선율이 유의하게 높았다(RR 1.53, 95% CI 1.04~2.26, p=0.03, I2=0%). 이명재훈련치료와 비교한 문헌에서 삶의 질 결과를 보고한 경우는 없었다 이명재훈련치료와 인지치료와 비교하여 이명장애점수를 보고한 총 3편의 문헌에서 이명재훈련치료군과 인지치료를 받은 군 간의 차이가 통계적으로 유의하지 않았다(SMD –0.24, 95% CI –0.23~0.71, p=0.33, I2=65%). 이명개선율을 보고한 문헌 2편에서는 이명재훈련치료군과 인지치료를 받은 군 간의 유의한 차이는 없는 것으로 나타났다(RR 0.71, 95% CI –0.46~1.09, p=0.11, I2=0%). 또한 삶의 질을 비교한 1편의 문헌에서는 두 군간 유의성을 보고하지 않았다. 이명재훈련치료와 이명차폐를 비교하여 이명장애점수를 보고한 3편의 문헌에서 이명재훈련치료가 이명차폐보다 유의한 효과가 있는 것으로 나타났다(SMD –0.74, 95% CI –1.16 ~ –0.32, p=0.0006, I2=0%). 그러나 이명개선율을 보고한 2편의 문헌에서는 두 군간 유의한 차이가 없는 것으로 나타났다(RR 1.10, 95% CI 0.69~1.73, p=0.69, I2=0%). 이명재훈련치료와 이명차폐를 비교하여 삶의 질을 보고한 문헌은 없었다. 이명재훈련치료와 무치료를 비교한 1편의 문헌에서 이명재훈련치료군이 무치료군 보다 전·후 차이 값이 더 크게 나타나 이명에 더 효과적인 것으로 확인되었고(p=0.013), 다른 1편의 문헌에서는 두 군간 차이가가 있음을 보고하였다(p<0.001). 이명재훈련치료와 무치료를 비교하여 삶의 질을 보고한 문헌은 없었다. 근거수준 평가 결과, 이명재훈련치료 안전성의 근거수준은 ‘높음(High)’으로 평가되었고, 효과성의 근거수준은 ‘매우 낮음(Very Low)’에서 ‘중등도(Moderate)’로 평가되었다.

결론 및 제언 소위원회에서는 이명재훈련치료에 대하여 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 이명환자에서 시행한 이명재훈련치료 과정에서 사용한 소리발생기(sound generator)로 인하여 외이도의 부종 혹은 통증이 발생하였다고 보고하였는데, 이는 보청기, 이어폰 등을 사용할 때도 흔히 발생할 수 있는 부작용으로 소위원회는 이명재훈련치료를 안전한 기술로 평가하였다. 이명재훈련치료는 상담, 인지치료, 이명차폐와 같은 비교치료와 유사한 효과를 보이는 것으로 확인되어 소위원회는 이명재훈련치료를 이명의 다양한 치료방법 중 하나로 사용할 수 있는 효과적인 치료로 판단하였다. 2022년 제1차 의료기술재평가위원회(2022.1.14.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 ‘이명재훈련치료’에 대해 다음과 같이 심의하였다. 이명재훈련치료는 난치성 이명환자에 적용하는 기술로 이명의 다양한 치료방법 중 하나로 사용할 수 있는 안전하고 효과적인 치료로 판단하여 동 기술을 ‘조건부 권고함’으로 심의하였다.

Tinnitus Retraining Therapy

Background Tinnitus retraining therapy (TRT) is applied to prevent the perception of ringing in the ear by inducing habituation and negative emotions in patients with tinnitus who are non-responsive to drug treatment. This technology was registered as a non-coverage item before the implementation of the new medical technology (the Ministry of Health and Welfare Notice No. 2002-68; September 27, 2002), which was subsequently identified as a subject for reassessment through internal monitoring as a part of the coverage implementation plan by the Ministry of Health and Welfare. On April 9, 2021, the fourth session of the Committee of Health Technology Reassessment selected this technology as a subject for reassessment. On May 14, 2021, at the fifth session of the Committee of Health Technology Reassessment, this technology was selected as a pilot project for social value assessment through a public participation group to reflect its values and preferences, as determined by the general public, for the medical technology reassessment process. This assessment aimed to provide medical and scientific evidence for the clinical safety and efficacy of TRT and policy decision-making on various issues, such as the appropriate use of this medical technology.

Methods A systematic literature review was performed to assess the safety and efficacy of TRT. All assessment methods were finalized after the review by the “TRT subcommittee” (hereinafter referred to as “subcommittee”), with consideration of the purpose of the assessment. The subcommittee was composed of six experts—two from otolaryngology and one each from neurology, rehabilitation medicine, occupational and environmental medicine, and evidence-based medicine. The key question in the assessment was: “Is TRT clinically safe and effective for patients with tinnitus who are non-responsive to drug therapy?” The assessment was conducted to evaluate the safety outcome using adverse events (AEs) and complications as variables. The efficacy outcome variables included indicators, such as tinnitus handicap score, tinnitus improvement rate, and quality of life (QoL). For the systematic literature review, three international and five domestic databases were searched based on the key question. Two reviewers independently selected the articles in accordance with the inclusion and exclusion criteria. In the event of any disagreement, the reviewers reached a consensus to select the final articles. Cochrane’s risk of bias (RoB) tool was used to assess the RoB of the selected articles. Two assessors independently assessed the selected articles, and in the event of any disagreement, the assessors reached a consensus to derive matching results. All data were extracted by the unit of study. The review results were assessed for the level of evidence using the Grading of Recommendations Assessment, Development and Evaluation method, while the grade of recommendation was determined based on the assessment results.

Assessment Results The safety and efficacy of TRT were assessed based on 11 articles. To define the interventions for the assessment, habituation therapy, cognitive behavioral therapy (CBT) + sound therapy, modified TRT, and TRT + hearing aid were defined as TRT (directive counseling + sound therapy). The comparators included counseling, cognitive test, tinnitus total masking, and no treatment. Tinnitus coping training, CBT, and acceptance and commitment therapy were defined as cognitive therapy during the assessment.

Safety For the safety of TRT, AEs and complications reported in one of the articles were assessed. The article reported one case of swelling or pain in the external auditory canal due to the sound generator used in the TRT group.

Efficacy The efficacy of TRT was assessed by comparing it to “counseling or cognitive therapy,” “counseling,” “cognitive therapy,” “tinnitus total masking,” and “no treatment.” The outcome variables included the indicators, such as tinnitus handicap score, tinnitus improvement rate, and QoL, based on discussion by the subcommittee. Nine of the articles comparing TRT with counseling or cognitive therapy involved the tinnitus handicap score. The results showed no significant difference in the tinnitus handicap scores between the TRT and counseling or cognitive therapy groups (standardized mean difference [SMD] –0.11, 95% confidence interval [CI] –0.41~0.20, p=0.49, I2=74%). Five articles involving the tinnitus improvement rate reported no significant differences between the TRT and counseling or cognitive therapy groups (RR 1.01, 95% CI 0.71~1.71, p=0.66, I2=53%). None of the articles comparing TRT with counseling or cognitive therapy involved QoL outcomes. None of the eight articles comparing the tinnitus handicap scores between the TRT and counseling groups showed any significant difference between the two groups (SMD –0.25, 95% CI –0.61~0.11, p=0.17, I2=75%). Three of the articles involving the tinnitus improvement rate reported that the TRT group showed a significantly higher tinnitus improvement rate than the counseling group (relative risk [RR] 1.53, 95% CI 1.04~2.26, p=0.03, I2=0%). None of the articles comparing TRT have reported QoL outcomes. Three articles comparing the tinnitus handicap scores between the TRT and cognitive therapy groups reported no statistically significant difference between the two groups (SMD –0.24, 95% CI –0.23~0.71, p=0.33, I2=65%). Two articles involving the tinnitus improvement rate showed no significant difference between the TRT and cognitive therapy groups (RR 0.71, 95% CI –0.46~1.09, p=0.11, I2=0%). Moreover, one article comparing QoL reported no significant difference between the two groups. Three articles comparing the tinnitus handicap scores between TRT and tinnitus total masking showed a higher significant effect of TRT than of tinnitus total masking (SMD –0.74, 95% CI –1.16 ~ –0.32, p=0.0006, I2=0%). However, two articles involving the tinnitus improvement rate showed no significant difference between the two groups (RR 1.10, 95% CI 0.69~1.73, p=0.69, I2=0%). Meanwhile, none of the articles compared QoL between TRT and tinnitus total masking. One article comparing TRT and no treatment showed that TRT was more effective for tinnitus than no treatment, with a larger difference between before and after the intervention (p=0.013). Another article reported a difference between the two groups (p<0.001). Meanwhile, none of the articles compared QoL between TRT and no treatment. The level of evidence for the safety of TRT was assessed to be “high,” while the level of evidence for the efficacy of TRT was assessed to be “very low” to “moderate.”

Conclusions and Recommendations Based on the current assessment results, the subcommittee recommendations for TRT are as follows. It was reported that the sound generator used during TRT caused swelling or pain in the external auditory canal of patients with tinnitus, although this is a common AE due to earphone or hearing aid use. Accordingly, the subcommittee assessed the TRT technology to be safe. Moreover, TRT was found to have a similar effect as comparators, such as counseling, cognitive therapy, and tinnitus total masking. Accordingly, the subcommittee evaluated TRT as one of the effective treatment modalities available for tinnitus. During the first session of the Committee of Health Technology Reassessment on January 14, 2022, the following conclusion was made on TRT based on the subcommittee findings in accordance with Article 4-10 of the Guidelines for Management of Medical Technology Reassessment Project: Based on the safety and efficacy of TRT among various treatment modalities available to patients with intractable tinnitus, this technology is “conditionally recommended” for patients with tinnitus.
