하악골 재건술

Abstract

평가 배경 ‘하악골 재건술’은 하악골의 결손부위를 재건하는 목적으로 행해지는 수술이다. 현재 국내에서는 ‘외상, 낭종, 종양, 선천성 기형 등에 의하여 하악골의 연속성이 결여된 경우’에 대하여 재건술을 행하는 경우 보험급여 대상인 기술이다. 동 의료기술에 대한 재평가 제안은 대한구강악안면외과학회에서 이루어졌고, 2021년 제5차 의료기술재평가위원회(2021.05.14.)에서 하악골 병적 골절에 관한 하악골 재건술에 대한 재평가를 통해 동 기술의 안전성 및 효과성에 대한 의과학적 근거를 평가하는 것으로 심의하였다.

평가 방법 하악골 재건술의 안전성 및 효과성을 재평가하기 위하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 구강악안면외과 2인, 성형외과 2인, 근거기반의학 1인 등 5인으로 구성된 하악골 재건술 재평가 소위원회(이하 소위원회)의 심의를 거쳐 확정하였다. 체계적 문헌고찰의 핵심질문은 ‘골괴사로 인하여 하악골에 병적 골절이 발생한 환자들에 대한 하악골 재건술은 임상적으로 안전하고 효과적인가?’이다. 본 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료 분석은 정성적 분석을 적용하였다. 2021년 제12차 의료기술재평가위원회(2021.12.10.)에서는 소위원회의 검토 결과를 고려하여 최종 권고등급을 결정하였다.

평가 결과 하악골 재건술 재평가에 선택된 문헌은 단일군 연구 총 19편(환자군 연구 11편, 증례 보고 8편)이었으며, 약물 관련 악골괴사 23명, 방사선 골괴사 98명, 골수염 골괴사 21명 등 총 142명의 환자들에 대한 사례들이 포함되었다.

안전성 안전성은 12편의 문헌의 결과를 바탕으로 수술 관련 부작용 및 이상반응으로 평가하였다. 일부 문헌에서 보고한 부작용 및 이상반응이 수술과 직접적인 관련이 있는지 판단하기 어려운 경우들이 있었고, 이상반응에 대한 정의 및 서술도 문헌마다 다르게 나타났다. 6편의 문헌에서 하악 부위 절개 및 봉합의 과정을 거치면서, 구강 및 악안면 부위의 상처, 상처치유 지연, 피부 열개, 피부 손실, 절개부위 혈종, 누공, 감각신경부위 손상 등의 부작용에 대하여 보고하였다. 5편의 문헌에서 하악골 부위 이식을 위하여 재건용 금속판을 이용하거나, 골이식(bone graft), 피판(flap: 혈관이 포함된 조직) 이식 등의 과정이 이루어지면서 이와 관련된 피판 부적응 및 소실, 공여부 합병증 등의 부작용 사례들을 보고하였다. 3편의 문헌에서 재건판 노출, 나사 풀림 등과 같은 하드웨어 사용으로 인한 문제를 보고하였다. 이식편 부작용을 보고한 3편의 문헌들 중, 1편의 문헌에서는 5명(24%)의 환자에서 피판 실패(flap failure)를 보고하였으나, 심각한 공여부 합병증은 없었다고 보고하였다. 2편의 문헌에서도 피판 관련 합병증은 없다고 보고하였다. 1편의 문헌에서 심각한 부작용이 0건이었다고 보고하였다. 전반적으로 합병증이 없었다고 보고한 문헌이 4편이었다.

효과성 효과성은 16편의 문헌의 결과를 바탕으로 하악골 재건술 수술과정의 성공과 관련이 있는 지표들, 재건수술을 통한 증상 치유, 하악골의 기능회복, 외모개선 기능 등에 관한 내용을 평가하였다. 결과지표에 대한 정의 및 서술도 문헌마다 다르게 나타났으며, 문헌에서 보고한 사유대로 서술하였다. 7편의 문헌에서 하악골 재건술의 수행과정과 관련하여 유합여부, 이식재의 적용, 수술 진행상의 성공여부 등을 평가하였다. 유합에 대해 언급한 2편의 문헌들 중, 1편에서는 11명 전원에게서 제대로 유합되었다고 보고하였고, 다른 1편의 문헌에서는 15명의 환자(83%)에게서 잘 유합되었음을 보고하였다. 이식과정과 관련하여 증례보고 1편에서 1명 환자에 대한 비골 분절, 하악 절제가 계획대로 잘 이루어졌다고 보고하였다. 이식 후의 결과에 대해 보고한 6편의 문헌에 포함된 31명의 모든 환자에서 이식된 조직이 성공적으로 적용되어 잘 기능하고 있다고 보고하였다. 9편의 문헌에서 증상의 해소(재발 여부, 치유 여부) 등에 대해 보고를 하였다. 증상 재발과 관련하여 보고한 5편의 문헌 중, 1편은 11명 환자 모두에서 약물관련 골괴사 재발이 없었다고 하였고, 1편의 문헌 내에 37명의 재건술 수행 환자들에서 수술 후 병적 골절이 없었고, 방사선 골괴사의 재발이 없었다고 보고하였다. 2명(10%)에서 방사선 골괴사의 재발을 보고한 문헌이 1편 있었다. 2편의 문헌 내에 19명의 환자에서 수술 후 골수염의 재발이 없었다고 보고하였다. 문헌 4편의 대상 환자 11명 모두 수술 부위가 잘 치유되어 통증이 해소되었음을 보고하였다. 7편의 문헌에서 하악의 기능회복과 관련된 내용인 저작 기능, 말하기 기능 등에 대해 평가하였다. 5편 문헌 내에 58명의 환자에서 수술 후 저작 기능 향상으로 식이 능력이 개선되었다고 보고하였다. 1편의 문헌 내에 5명의 환자 중, 1명에서 말하기 능력이 향상되었다고 보고하였다. 4편의 문헌 내에 12명의 환자에서 재건술 후 안면 형태 회복을 통한 외모 개선에 관하여 보고하였다. 전반적으로 효과성 지표 관련 결과를 보고한 모든 문헌에서 추적관찰 기간동안 긍정적인 결과를 보고하였다.

결론 및 제언 하악골 재건술 재평가 소위원회는 평가 결과에 근거하여 다음과 같이 제언하였다. 소위원회는 선택문헌에서 보고한 부작용 및 이상반응들 대부분은 하악골 재건술 과정과 관련된 부작용인 절개 부위의 상처치유 지연, 이식재 적용의 문제 등의 사례들로 흔히 알려진 부작용이었다. 중등도 이상의 부작용이나 주의 깊게 고려할만한 부작용 보고는 없어서, 하악골 괴사로 인하여 병적 골절이 일어난 환자들에 대하여 하악골 재건술은 안전한 수술법이라고 판단하였다. 그럼에도 하악골 재건술은 전신마취, 절개, 이식, 재건, 봉합의 과정을 거치는 고난도의 외과수술인 만큼 악안면 부위 수술의 훈련을 받은 의료전문가에 의해 수행되고, 부작용의 가능성에 신속하게 대비할 수 있는 환경에서 수술을 받고 입원을 진행해야 부작용 및 합병증의 발생을 예방할 수 있다는 점을 고려해야 한다고 소위원회는 제언하였다. 효과성 측면에서는 하악골 재건술의 수행과정과 관련한 이식재의 적용, 수술 부위의 유합, 수술 진행상의 성공, 증상 해소, 기능 향상, 안면형태 회복을 통한 외모개선 등에서 추적관찰 기간동안 긍정적인 결과를 보고한 문헌들이 상당수 존재하여, 하악골 재건술은 골괴사로 인한 하악골 병적골절로 인하여 하악골의 결손이 생겼을 때, 이를 재건하여 하악골의 주요 기능을 회복시킬 수 있는 유효한 기술이라고 판단하였다. 2021년 제12차 의료기술재평가위원회(2021.12.10.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조 제10항에 의거 “하악골 재건술”에 대해 다음과 같이 심의하였다. 하악골 재건술은 골괴사로 인하여 하악골에 병적 골절이 발생한 환자에서 사용을 ‘권고함’으로 심의하였다.

Mandibular reconstruction

Background Mandibular reconstruction is a surgical procedure aimed at reconstructing defects in the mandible. At present, in Korea, this procedure is covered by insurance when performed in cases with loss of mandibular continuity due to trauma, cyst, tumor, or congenital malformation. The Korean Association of Oral and Maxillofacial Surgeons proposed reassessment of this technique. In the fifth session of the Committee of Health Technology Reassessment in 2021 (May 14, 2021), the evidence on the safety and effectiveness of mandibular reconstruction for pathologic mandibular fractures was reassessed.

Methods A systematic literature review was performed to reassess the safety and effectiveness of mandibular reconstruction. All assessment methods were finalized after a review by a subcommittee for reassessment of mandibular reconstruction (hereinafter “subcommittee”), based on the purpose of the assessment. The subcommittee was composed of five experts: two in oral and maxillofacial surgery, two in plastic surgery, and one in evidence-based medicine. The key question in the systematic literature review was “Is mandibular reconstruction clinically safe and effective for patients with pathologic mandibular fracture caused by osteonecrosis?” For the systematic literature review, three international and five domestic databases were searched based on the key question. Two reviewers independently selected the articles in accordance with the inclusion and exclusion criteria. Data extraction was performed by two reviewers, using a predetermined format. In the event of any disagreement, consensus was reached through discussion with a third person. Qualitative review was used for data analysis. In the twelfth session of the Committee of Health Technology Reassessment in 2021 (December 10, 2021), the final grade of recommendation was determined based on the subcommittee findings.

Assessment results A total of 19 articles on single-group studies (11 case series and 8 case reports) were selected for the reassessment of mandibular reconstruction. The cases included 142 patients: 23 patients with medication-related osteonecrosis of the jaw, 98 with radiation osteonecrosis, and 21 with osteomyelitis/osteonecrosis.

Safety Safety was assessed based on procedure-related adverse events (AEs) and adverse reactions (ARs) reported in 12 articles. It was difficult to determine whether AEs and ARs reported in some articles were directly related to the procedure, while the definitions and descriptions of ARs differed between articles. Six articles reported on AEs related to incision and suturing of the mandibular region, including oral and maxillofacial wound, delayed wound healing, skin dehiscence, skin loss, incision site hematoma, fistula, and sensory nerve damage. Five articles reported on AEs related to the use of a metal plate for mandibular reconstruction, bone graft, or flap (vascularized flap) graft, including flap failure/loss and donor site complications. Three articles reported on equipment-related problems, including exposed reconstruction plate and loosening of screws. Of three articles that reported on graft-related AEs, one article reported flap failure in five patients (24%), but there were no serious donor site complications. The other two articles reported no flap-related complications. One article reported no serious AEs. Overall, there were four articles that reported no complications.

Effectiveness Effectiveness was assessed based on indicators related to the success of mandibular reconstruction surgery, symptomatic improvement through reconstructive surgery, recovery of mandibular function, and improvement in extraoral appearance reported in 16 articles. The definitions and descriptions of outcome indicators differed between articles, and were described by the reasons reported in the articles. Seven studies evaluated bone union, application of graft material, and the success of mandibular reconstruction surgery. Of the two articles that reported on bone union, one article reported good bone union in all 11 patients, while the other articles reported good bone union in 15 patients (83%). In one case of bone grafting, the desired nasal bone segmentation and mandibular resection were achieved. Six articles describing the post-grafting results indicated that the tissues were grafted successfully with adequate function in all 31 patients included. Nine articles reported on symptom alleviation (recurrence and healing). Among five articles that reported on symptom recurrence, one article reported no recurrence of medication-related osteonecrosis in all included 11 patients, while another article reported no postoperative pathologic fracture and no recurrence of radiation osteonecrosis in 37 patients who underwent reconstructive surgery. One article reported recurrence of radiation osteonecrosis in two patients (10%). Two articles reported no postoperative recurrence of osteomyelitis in 19 patients. All 11 patients included in four studies achieved pain relief following proper healing of the surgical site. Meanwhile, seven articles reported on the assessment of chewing and speaking, which are related to functional recovery of the mandible. Five articles reported that 58 patients showed improvement in their ability to eat owing to postoperative improvement in chewing function. One article reported that one out of five patients showed postoperative improvement in speaking ability. In four studies, 12 patients reported to have achieved improvement in facial appearance following the reconstructive surgery. Overall, all articles that reported on effectiveness indicators reported favorable outcomes during the follow-up.

Conclusions and recommendations The subcommittee recommends the following based on the current assessment results: The subcommittee determined that most AEs and ARs after mandibular reconstruction reported in the selected articles are common and include delayed wound healing at the incision site and problems associated with grafting. Since there was no report of any moderate-to-serious AEs or AEs that warrant careful consideration, it was determined that mandibular reconstruction is a safe procedure for patients with pathologic fracture due to mandibular necrosis. Despite this, the subcommittee recommended that mandibular reconstruction must be performed by trained maxillofacial surgeons due to a high degree of difficulty associated with the need for general anesthesia, incision, grafting, reconstruction, and suturing; moreover, the surgical process and hospitalization should allow a timely response and help prevent AEs and complications. With respect to effectiveness, many articles reported favorable outcomes during the follow-up, including graft placement, bone union at the site of surgery, success of surgery, symptom alleviation, functional improvement, and restoration of facial aesthetics. Accordingly, mandibular reconstruction was determined to effectively restore major functions of the mandible in patients with pathologic mandibular fracture due to osteonecrosis. During the twelfth session of the Committee of Health Technology Reassessment in 2021 (December 10, 2021), the following determination was made on mandibular reconstruction based on the subcommittee findings in accordance with Article 4-10 of the Guidelines for Management of Medical Technology Reassessment Project: Mandibular reconstruction is “recommended” for use in patients with pathologic mandibular fracture due to osteonecrosis.
