증식치료[척추부위]

Abstract

평가 배경 ‘증식치료-척추 부위(Prolotherapy-spine)’는 만성적 근골격계 통증이 있는 부위의 인대나 건이 뼈에 부착하는 부위에 증식물질을 주사하여 건, 인대를 강화시켜 통증이 소실되거나 완화되는 것을 유도하는 치료로 만성통증 환자의 동통완화 목적으로 실시하는 기술이다. 동 기술은 2004년 1월1일부터 100분의100 본인부담으로 적용되었다가 2006년 1월1일자로 행위비급여로 전환되어 사용되어왔다. 건강보험심사평가원에서는 동 기술의 급여적용 타당성 검토를 위한 근거확인을 위하여 본 원에 의료기술재평가를 의뢰하였고(2021.03.23.), 이에 전문적·심층적 검토를 통해 임상적 안전성 및 효과성을 확인할 필요가 있는 안건으로 2021년 제6차 의료기술재평가위원회(2021.06.11.)에서 의료기술재평가 안건으로 선정되어 재평가를 수행하였다.

평가 방법 증식치료에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “증식치료[척추 부위]에 대한 안전성 및 효과성 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 평가의 핵심질문은 “증식치료는 척추질환 환자의 통증완화와 기능개선에 있어 임상적으로 안전하고 효과적인가?”이었고, 안전성은 시술 관련 부작용 및 이상반응 지표로, 효과성은 통증경감, 기능개선, 삶의 질 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림 위험 평가는 연구유형별로 Cochrane의 Risk of Bias (RoB)와 RoBANS를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치 발생 시에는 제3자와 함께 논의하여 합의하였다. 자료분석은 질적 검토(qualitative review) 방법을 적용하였으며, 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.

평가 결과 증식치료[척추 부위]의 안전성과 효과성은 총 18편 무작위배정비교임상시험 6편, 비무작위배정비교임상시험 1편, 단면연구 1편, 증례연구 9편, 증례보고 1편)에 근거하여 평가하였다. 총 18편의 문헌에 포함된 분석 대상자수는 총 1,085명(비교연구의 중재군 265명, 비교군 251명, 단일군 연구 569명)이었다.대상자들의 대부분은 요통 등의 통증이 기존의 보존적 치료에서 조절되지 못한 경우였으며, 비교중재는 스테로이드 주사, 위치료군(생리식염수 또는 국소 마취제 투여) 등이었다.

안전성 총 18편의 문헌 중 증식치료 관련 부작용 및 이상반응 여부를 보고한 연구는 총 9편이었으며, 그 외 연구들은 관련 내용을 다루지 않았다. 위치료군(placebo treatment)과 비교한 4편에서 통증, 강직, 두통, 오심 등의 사례가 보고되었으나 경미한 수준의 것으로 확인되었고, 소수 환자에서 안면신경마비, 신장암, 우울증, 생리흐름 증가 및 폐경 후 점상질출혈 등이 보고되었다. 이 중 군 간 발생여부를 비교한 1편(Ongley et al., 1987)에서 중재군과 비교군 간 유의한 차이는 없는 것으로 나타났다. 다른 1편(Klein et al., 1993)에서는 치료 1일째 발생한 과민반응(상세내용 설명없음)에 대해 일회성의 코르티코스테로이드 투약(중재군과 비교군 각각 8명과 5명)을 실시한 것으로 나타났다. 증식치료 수행횟수별로 비교한 1편의 단면연구과 단일군 연구 4편에서는 심각한 부작용 및 이상반응 발생이 없는 것으로 보고하였다.

효과성 동 시술의 효과성은 통증경감 정도, 기능개선여부, 삶의 질 지표로 평가하였다. 통증경감 정도는 총 16편의 연구에서 VAS, NRS, 통증그리드 및 다이어그램, 통증감소율 등의 다양한 지표로 보고하였다. 위치료군과 비교한 연구는 6편이었으며, 전 편에서 양 군 모두에서 통증경감을 확인하였다(3편: 통계적으로 유의함, 2편: 통계적으로 유의하지 않음, 1편: p값 제시되지 않음). 두 군 비교 시 1편의 연구에서는 중재군의 통증경감 정도가 유의하게 큰 것으로 보고되었으며, 이 외 5편의 연구에서는 군 간 유의한 차이를 보이지 않았다. 스테로이드 주사와 비교한 1편에서는 모든 군 내 유의한 변화를 보였으나, 군 간 비교 시에는 유의한 차이가 없었다. 증식치료 10회와 3.5회 시행군을 비교한 1편의 단면연구에서는 중재군에서만 유의한 경감효과를 보였다. 단일군 연구 8편 전편에서 증식치료 전에 비해 치료 후 통증경감 효과가 있는 것으로 보고하였다. 기능개선여부는 Oswestry Disability Index (ODI), Roland-Moris를 비롯한 장애점수, 기능제한일수, 장애감소율 등으로 다양한 지표로 총 12편의 비교연구 결과로 평가하였다. 스테로이드 주사와 비교한 1편은 양 군 모두에서 유의한 개선을 보고하였으나, 군 간 비교 시에는 유의한 차이가 없었다. 위치료군과 비교한 5편의 문헌 중 3편은 양 군 모두 군 내 기능이 유의하게 개선되었으나, 군 간 비교 시에는 유의한 차이를 나타내지 않았다. 다른 1편(Ongley et al., 1987)은 양 군 모두 장애정도가 감소한 것으로 나타났으나(p값 제시없음), 군 간 비교 시 중재군에서 더 유의한 효과를 보고하였다. 동 연구에서 시술 후 6개월 시점에서 장애정도가 50% 이상 감소했다고 응답한 비율과 장애점수가 0인 대상자 수는 중재군에서 월등하게 높은 것으로 나타났다. 나머지 1편(Dechow et al., 1999)의 연구는 군 내, 군 간 모두 장애 정도에 대한 설문 상 유의한 차이를 보이지 않았고, 요추굴곡평가는 하향 추세를 보이지만 통계적으로 유의미한 수준은 아닌 것으로 나타났다. 단일군 연구 6편에서는 시술 전에 비하여 증식치료 후 기능이 개선된 것으로 보고하였다. 삶의 질에 관련해서는 시술 관련 환자만족도에 대해 증식치료와 위치료군을 비교한 2편의 비교연구에서는 군 간 유의한 차이가 없었고, 단일군 연구 1편에서는 시술 전에 비해 1년 후 삶의 질이 향상되었다.

결론 및 제언 소위원회에서는 현재 문헌적 근거를 바탕으로 다음과 같이 결론을 제시하였다. 척추 부위 질환을 가진 환자를 대상으로 한 증식치료는 중재시술에서 보고된 시술 관련 부작용 및 이상반응이 일시적이고 경미한 수준으로 비교중재와 비교 시에도 통계적으로 유의한 차이가 없었고, 소수 보고된 사례도 동 시술과 직접적인 관련성이 있는 것으로 보기 어렵다는 점 등을 들어 동 시술은 타 보존적 치료와 유사한 수준의 안전성을 가진 것으로 평가하였다. 효과성은 주요 지표인 통증경감, 기능개선에 있어 증식치료 수행 후 유의한 개선을 보고하거나 증상개선의 경향을 확인할 수 있다는 점에서 동 시술은 기존의 보존적 치료에 반응하지 않는 척추질환 환자의 통증경감 및 기능개선에 효과가 있을 수 있음을 시사한다고 보았다. 그러나 스테로이드 주사 및 위치료군과의 비교에서 군 간 유의한 차이를 보이지 않으며, 평가에 포함된 근거의 대부분이 중등도 비뚤림 위험이 있는 연구들로부터 도출된 낮은(low) 근거수준이라는 점을 감안할 때 증식치료의 효과에 대하여 결론을 내리기에 근거가 부족하다는 의견이었다. 그리고 증식치료의 시행방법이 연구별로 상이하여 시술방법의 표준화에 대한 향후 추가 연구가 필요하다는 의견을 제시하였다. 2022년 제1차 의료기술재평가위원회(2022.01.14.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “증식치료[척추 부위]”에 대해 다음과 같이 심의하였다. 척추 부위 질환을 가진 환자에서 증식치료는 안전하지만 효과성에 대한 결론을 내리기에 근거가 부족하여 증식치료[척추부위]를 근거 불충분으로 심의하였다(권고등급: 불충분)

Prolotherapy (Spine)

Background Prolotherapy of the spine is used for the relief of chronic musculoskeletal pain by strengthening tendons and ligaments through the injection of proliferative substances that attach to the tendon, ligament, or bone in sites with pain. For this technology, 100/100 co-payment was applied starting from January 1, 2004, although it was subsequently changed to a non-benefit item as of January 1, 2006. The Health Insurance Review and Assessment Service contracted our institute (March 23, 2021) to conduct a health technology reassessment to verify the evidence for reviewing the validity of expanding benefits to include this technology. Accordingly, a reassessment was conducted based on the selection of this technology as an agenda item for health technology reassessment by the sixth session of the Committee of Health Technology Reassessment in 2021 (June 11, 2021) to verify the clinical safety and effectiveness of this technology through a professional, in-depth review.

Methods A systematic literature review was conducted to assess the safety and effectiveness of prolotherapy. All assessment methods were finalized after a review by the “Prolotherapy [spine] safety and effectiveness assessment subcommittee” (hereinafter “Subcommittee”). The key question in the assessment was “Is prolotherapy clinically safe and effective for alleviating pain and improving function in patients with spinal disease?” Safety was assessed based on adverse events (AEs) and adverse reactions (ARs) related to the procedure, while effectiveness was assessed according to pain reduction, functional improvement, and quality of life (QoL) indicators. For the systematic literature review, three international and five domestic databases were searched based on the key question. Two reviewers independently selected the articles based on the inclusion and exclusion criteria. Risk of bias (RoB) assessment of the selected articles was performed independently by two reviewers using the Cochrane RoB tool or Risk of Bias for Nonrandomized Studies depending on the study type to reach a consensus on their findings. Data extraction was performed by two reviewers, using a predetermined format. Any disagreements were resolved after reaching a consensus through discussion with a third person. A qualitative review was used for the data analysis, and the level of evidence of the systematic review results was assessed using the Grading of Recommendations Assessment, Development and Evaluation approach.

Assessment Results The safety and effectiveness of prolotherapy-spine were assessed based on a total of 18 articles on various studies, including six randomized controlled trials (RCTs), one non-RCT, one cross-sectional study, nine case studies, and one case report. The total number of patients analyzed in the 18 articles was 1,085 (265 in the intervention group and 251 in the control group in comparative studies and 569 in single-group studies). Most patients had pain, such as low back pain, that could not be controlled with conventional conservative therapy. The comparator interventions used included steroid injections and placebo treatment (saline or local anesthesia).

Safety Of the 18 articles selected, nine reported prolotherapy-related AEs and ARs. Other studies did not discuss relevant matters. Four articles that compared prolotherapy with placebo treatment reported mild pain, stiffness, headache, and nausea. In addition, facial nerve palsy, renal cancer, depression, increased menstrual flow, and postmenopausal vaginal spotting were reported in a small number of patients. In one study that compared the incidence between the intervention and control groups (Ongley et al., 1987), the results showed no significant difference between the two groups. In another study (Klein et al., 1993), the results showed that corticosteroids were administered one time (in eight and five patients in the intervention and control groups, respectively) for hypersensitivity reactions (details not given) that occurred on the first day of the intervention. Meanwhile, one cross-sectional study and four single-group studies that compared the number of prolotherapy sessions reported no serious AEs or ARs.

Effectiveness The effectiveness of this technology was assessed based on the level of pain reduction, functional improvement, and QoL indicators. A total of 16 studies reported levels of pain reduction based on various indicators, including the visual analog scale, numerical rating scale, pain grid and diagram, and pain reduction rate. In six studies that compared prolotherapy with placebo treatment, pain reduction was observed in both groups in all studies (statistically significant in three studies; non-statistically significant in two studies; and no p-value given in one study). In comparisons of the two groups, one study reported a significantly greater pain reduction in the intervention group, whereas the other five studies showed no significant differences between the two groups. In one study that compared prolotherapy with steroid injection, all groups showed significant changes, with no significant differences between the groups. One cross-sectional study comparing 10 and 3.5 sessions of prolotherapy observed a significant reduction effect in the intervention group alone. All eight single-group studies reported pain reduction after prolotherapy compared with before prolotherapy. Functional improvement was assessed based on outcomes in 12 comparative studies that used various indicators, including the Oswestry Disability Index, Roland-Morris disability score, number of days with functional impairment, and disability reduction rate. One study comparing prolotherapy with steroid injection reported significant improvement in both groups, with no significant difference between the groups. Of the five articles that compared prolotherapy with sham treatment, three reported significant improvement in both groups, with no significant difference between the groups. In another study (Ongley et al., 1987), both groups showed reductions in the degrees of disability (p-value not given), with a more significant effect in the intervention group. In the same study, the percentage of patients who reported a ≥50% reduction in disabilities or had a disability score of 0 at 6 months after the intervention was overwhelmingly higher in the intervention group. In the remaining study (Dechow et al., 1999), the results showed no significant within- and between-group differences in disability questionnaire scores; moreover, while lumbar flexion assessment results showed a downward trend, the difference was not statistically significant. Six single-group studies reported functional improvement after prolotherapy compared with before the intervention. Regarding QoL, two studies that compared intervention-related patient satisfaction between the prolotherapy and placebo groups reported no significant difference between the groups. Meanwhile, one single-group study reported improved QoL at 1 year after the intervention.

Conclusions and Recommendations The Subcommittee reached the following conclusions based on the evidence in current literature. Prolotherapy applied to patients with spinal disease showed transient and mild prolotherapy-related AEs and ARs, with no statistically significant difference to comparator interventions, while few reported cases were not directly related to this technology. Accordingly, this technology was assessed to have a level of safety that is similar to other conservative therapies. Regarding pain reduction and functional improvement, which are major indicators of effectiveness, significant improvement after prolotherapy or improving trends in symptoms was reported, which suggested that this technology could be effective in reducing pain and improving function in patients with spinal disease who do not respond to conventional conservative therapy. However, there was no significant difference between the groups in the comparisons with steroid injection and placebo, while the level of evidence was low due to the studies with moderate RoB that were included in the assessment. Based on these points, it was determined that there is a lack of evidence to make conclusions about the effectiveness of prolotherapy. Moreover, studies used different methods in performing prolotherapy; thus, additional future studies are needed on the standardization of methods for applying prolotherapy. During the first session of the Committee of Health Technology Reassessment in 2022 (January 14, 2022), the following determination was made on prolotherapy-spine based on the Subcommittee findings under Article 4-10 of the Guidelines for Management of Medical Technology Reassessment Project: Prolotherapy of the spine is safe for patients with spine disease, although the evidence is insufficient to conclude its effectiveness (Grade of recommendation: Insufficient).
