요관용 금속스텐트

Abstract

평가 배경 요관 스텐트는 요관 폐쇄 환자에서 신장에서 방광까지 막힌 요관 내에 설치해 소변 배출이 잘 되도록 도와주는 역할로 널리 쓰이고 있다. 기존 스텐트는 스텐트 기능 부전에 취약하여 정기적으로 교체해야 하는 문제점이 있으며 이를 보완하기 위해 개발된 치료재료가 외압 저항성이 큰 금속스텐트(metallic ureteral stent)이다. 우리나라에서는 2015년 11월 1일부터 요관용 금속스텐트를 선별급여 80%로 도입하였고, 2021년 5월 1일부터 본인부담률을 50%로 조정하였으며, 2차 요양급여 적합성 평가 시기는 2025년 11월로 예정되어 있다. 요관용 금속스텐트의 효과성과 장기 안전성에 대한 근거가 필요하여 환자중심 의료기술 최적화 연구사업에 전향적 연구주제로 제안되었으나 후향적 연구를 통해 근거평가가 상대적으로 신속하게 수행가능한 재평가과제가 더 적절할 것이라는 사업운영위원회의 검토의견에 따라 해당 주제가 한국보건의료연구원 의료기술재평가위원회로 이관되어 심의하에 재평가를 수행하게 되었다. 본 평가의 목적은 요관 폐쇄(요관이 좁아지거나 막힘) 환자에서 요관용 금속스텐트 치료재료의 임상적 안전성 및 효과성, 경제성 등에 대한 과학적 근거를 제공하고 의료기술의 적정 사용 등 정책적 의사결정을 지원하고자 하였다.

평가 방법 요관용 금속스텐트의 안전성 및 효과성을 평가하기 위해 체계적 문헌고찰, 성과 분석, 비용 및 재정영향 분석을 수행하였다. 모든 평가 방법은 평가 목적을 고려하여 “요관용 금속스텐트 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 비뇨의학과 3인, 신장내과 1인, 영상의학과 1인, 근거기반의학 1인 등 총 6인으로 구성하였다. 체계적 문헌고찰의 핵심질문은 “요관용 금속스텐트는 요관 폐쇄 환자에서 임상적으로 안전하고 효과적인가?”이었고, 안전성은 요관용 스텐트 이동, 오작동, 요관 손상, 요로 감염, 혈뇨 등을 결과지표로 하였고, 효과성은 스텐트 개통률, 성공률, 유지지간 등을 결과지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자 간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias (RoB)및 Risk of bias Assessment tool for Non-randomised Study (RoBANS) 2.0을 사용하여, 최종 선택 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 암종 단위로 결과제시 및 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다. 성과 분석은 국내의 요관용 금속스텐트의 현황 및 임상적 효과를 확인하기 위해 건강보험심사평가원 맞춤형 연구자료(과제번호 M20211007549)를 활용하였다. 대상자는 2015년 1월 ~ 2021년 5월까지 요관용 스텐트 재료를 사용한 성인 환자이며, 요관용 스텐트를 삽입한 에피소드 단위로 분석대상자를 선정하였다. 에피소드는 요관용 스텐트 치료재료가 있는 동일 요양개시일, 동일 요양기관에서 시행된 명세서를 기준으로 조작적으로 정의하였다. 이를 기준으로 연도별 이용현황과 환자 및 에피소드 단위 기저특성을 파악하고 요관용 금속스텐트 임상적 효과지표로 평균 유지기간, 교체발생 위험, 사망발생 위험, 시술 30일내 비뇨기 감염발생, 재입원 등을 분석하였다. 분석 대상의 기저 특성은 빈도와 백분율, 평균, 표준 편차 등으로 제시하고, 군간 차이 검정을 위해 범주형 변수는 카이제곱검정(Chi-square test), 연속형 변수는 t검정(t-test) 또는 분산 분석(Analysis of variance, ANOVA)을 수행하였다. 요관용 스텐트 종류에 따른 결과지표 발생위험 분석은 콕스비례위험모형을 적용하여 위험비(Hazard ratio, HR)를 추정하거나 로지스틱 회귀분석을 적용하여 오즈비(Odds ratio, OR)를 추정하였고, 95% 신뢰구간(Confidence interval, CI) 및 유의확률(p-value)을 함께 제시하였다. 비용 및 재정영향 분석은 성과분석시 구축된 건강보험 청구자료의 후향적 코호트를 활용하여 요관용 금속스텐트로 인한 비용절감 효과 및 향후 5년 건강보험 재정변화를 추계하였다. 요관용 금속스텐트로 인한 연간 소요비용은 환자 1인당 평균 시술비용과 1년 이내 추적관찰에 소요된 비용으로 정의하였으며, 재정영향분석은 성과분석의 결과를 반영하여 요관용 금속스텐트 점유율 및 본인부담률 변화로 인한 건강보험 재정변화를 추정하였다. 또한 요관용 스텐트 시술 관련 비용이 아닌 다른 의료비용이 포함될 가능성이 높은 사망자의 비용을 제외한 경우에 대해 민감도 분석을 시행하였다.

평가 결과

1. 체계적 문헌고찰

1.1 안전성 및 효과성 임상적 안전성과 효과성 관련 최종 선택 문헌은 총 6편이었고, 무작위배정 임상연구(Randomized controlled trial, RCT)는 1편, 코호트 연구 5편이었다. RCT 연구는 한국에서 수행된 연구로 악성 요관 폐쇄 환자를 대상으로 피복형 요관용 금속스텐트(상품명 Urexel)와 중합체스텐트(Double J stent)를 비교하였고, 추적관찰 기간은 12개월이었다. RCT 연구의 비뚤림위험 평가결과, 참여자, 연구자 눈가림, 결과평가 눈가림, 불충분한 결과자료, 선택적 보고는 비뚤림위험이 낮았고, 무작위순서, 배정순서 은폐, 민간연구비 지원은 언급이 없어 불확실로 평가하였다. 코호트 연구는 악성 요관 폐쇄 환자 대상 문헌이 4편으로 가장 많았고, 금속스텐트 종류별로는 Resonance®(Cook Medical)을 사용한 연구가 3편으로 가장 많았다. 문헌에서의 추적관찰 기간은 1~72개월이었다. 비뚤림위험 평가결과, 대상군 비교가능성과 대상군 선정, 노출측정, 불완전한 결과자료는 1편 이상의 문헌에서 비뚤림위험이 ‘높음’으로 평가되었고, 선택적 결과보고, 민간 연구비 지원은 대체로 비뚤림위험이 낮았고, 교란변수는 언급이 없어 5편 모두 불확실하였다.

안전성 요관용 금속스텐트 관련 시술관련 부작용은 RCT 연구에서는 모두 발생하지 않았고, 코호트 연구에서는 시술관련 부작용이 1편에서는 중합체스텐트보다 높았으나, 다른 한편에서는 낮았다. 스텐트 이동은 RCT 연구에서는 요관용 금속스텐트에서 0.1% 발생하였고, 중합체스텐트는 발생하지 않았으며, 스텐트 오작동은 금속스텐트 0.4%, 중합체스텐트군 0.7%에서 발생했다. 코호트 연구에서도 금속스텐트군의 스텐트 이동 발생률이 0.1% 미만이었고, 이 외 요관 및 신장 손상, 혈뇨, 요로 감염, 통증 등은 발생빈도가 1% 미만이거나 중합체스텐트와 비교해 차이가 없었다.

효과성 개통률은 RCT 연구에서 금속스텐트군이 중합체스텐트군에 비해 3개월, 6개월, 9개월, 12개월 시점에서 모두 높았고, 12개월 시점의 금속스텐트군의 개통률이 통계적으로 유의하게 높았다. 코호트 연구의 메타분석 결과에서는 7일, 1개월, 3개월, 6개월 시점 개통률은 두 군간 차이가 없었고, 12개월 시점의 금속스텐트의 개통률은 중합체스텐트에 비해 통계적으로 유의하게 오즈비가 1.4배 더 높았다(12개월 개통률 오즈비(Odds ratio, OR) 1.41, 95% 신뢰구간(Confidence interval, CI) 1.01~1.98, I2= 0%). RCT 연구에서는 기술적 성공률과 스텐트 성공률이 두 군에서 모두 100% 였고, 코호트 연구는 두 군간 차이가 없었다(12개월 스텐트 성공률 OR 1.09, 95% CI 0.72~1.64, I2= 0%). 요관용 스텐트 유지기간은 RCT 문헌에서 금속스텐트 239일, 중합체스텐트 80일이었고, 코호트 연구에서는 금속스텐트 380일, 중합체스텐트 183일로 금속스텐트군에서 더 길었다. 코호트 연구 1편에서 금속스텐트군에서의 삶의 질이 중합체스텐트군에 비해 통계적으로 유의하게 더 높은 것으로 보고하였다.

1.2 경제성 비용 등 경제성 관련 결과를 보고한 문헌은 총 7편이었고, 평균 시술비용(스텐트 비용, 마취 등 수술실 비용, 검사비 등)과 스텐트 유지관리를 위한 연간비용을 추계하고 중합체스텐트 대비 비용절감액을 보고하였다. 7편 모두에서 중합체스텐트군에 비해 금속스텐트군의 유지기간이 길고 교체빈도가 적어 금속스텐트군에서 환자 1인당 연간비용 절감효과(43%~59.5%)를 보고하였다.

2. 성과분석 요관용 스텐트 후향적 코호트의 분석대상 에피소드는 총 171,147건(67,584명)이었고, 금속스텐트 에피소드는 361건(0.2%), 중합체스텐트 에피소드는 170,786건(99.8%)이었다. 요관용 금속스텐트의 연도별 사용현황 결과, 사용환자가 연간 100명 미만으로 매우 적고, 요관용 스텐트 삽입시 환자당 총 진료비는 2021년 기준 430만 원 정도였다. 금속스텐트만 사용한 환자는 41명이었고, 중합체스텐트만 사용한 환자는 67,266명, 금속스텐트와 중합체를 모두 경험한 환자는 277명이었다. 환자단위 기저 특성결과, 금속스텐트를 시술받은 환자는 중합체스텐트 시술환자에 비해 연령 및 동반질환의 중증도가 더 높았다. 분석대상자의 평균 추적관찰 기간은 3년이었고, 금속스텐트만 사용한 환자는 환자 1인당 평균 1.1회의 삽입시술과 1.68개의 금속스텐트를 사용하였고, 중합체스텐트만 사용한 환자는 평균 2.5회의 삽입시술과 3.3개의 스텐트를 사용하였다. 에피소드 단위 기저특성 결과에서는 금속스텐트와 중합체스텐트 모두 상급종합병원에서 주로 시행되었고(60% 이상), 금속스텐트는 요관경하 삽입이 49%로 가장 많았고, 중합체스텐트는 방광경하 삽입이 65%로 가장 많았다. 재원기간은 금속스텐트는 평균 8.48일, 중합체스텐트는 6.88일이었다. 1회 삽입시 평균 진료비 총액은 금속스텐트 423만 1천 원, 중합체스텐트 334만 2천 원이었다. 금속스텐트는 중합체스텐트에 비해 평균 유지기간이 평균 4.4개월(132.8일)이 더 길며, 교체빈도도 약 30% 정도 더 낮았으며, 교체빈도 및 유지기간을 고려한 Cox비례위험 모형분석에서도 교체발생 위험이 0.52배(95% CI 0.31~0.86)로 통계적으로 유의하게 낮았다. 하위그룹 분석결과, 80세 이상군, 암유병자가 아닌 군에서 금속스텐트가 중합체스텐트에 비해 스텐트 교체 발생 위험이 통계적으로 유의하게 낮았다. 사망, 30일내 비뇨기 감염, 재입원, 응급실 이용 발생은 기저특성을 보정한 결과에서 두 군간 통계적으로 유의한 차이가 없었다.

3. 비용 및 재정영향 분석 요관용 금속스텐트와 중합체스텐트의 환자 1인당 연간비용을 비교한 결과, 금속스텐트군에서의 비용이 중합체스테트군에 비해 약 80만원(7.5%)정도 낮았고, 사망자 비용을 제외한 민감도 분석에서는 152만원(22.5%) 낮았다. 또한, 일반화 추정 방정식 모형(Generalized estimating equation, GEE)적용 결과 금속스텐트군과 중합체스텐트군간 1인당 소요비용 차이가 통계적으로 유의하였다. 재정영향 분석에서 현재의 선별급여 본인부담률 50%과 금속스텐트 점유율 0.4%를 유지할 경우 향후 5년간 총 건강보험 소요재정은 7,750억 1,600만원이었고 금속스텐트의 점유율 증가(시나리오 1~6)할 경우 건강보험재정이 절감될 것으로 추계되었다. 현재 금속스텐트 점유율 0.4%가 10%로 증가할 경우 현 기준에서 추정된 5년 건강보험 재정규모에서 0.72%(▽56억 300만 원) 감소할 것으로 예상되며, 점유율이 매년 10%씩 증가할 경우에는 2.35%( ▽181억 9,900만 원)감소할 것으로 추계되었다. 민감도 분석에서는 금속스텐트 점유율이 10%인 경우 2.16%(145억 3,800만 원), 매년 10% 증가하면 7.02%(472억 2,400만 원) 건강보험재정이 절감되는 것으로 추정되었다.

결론 및 제언 요관용 금속스텐트의 임상적 안전성과 효과성에 대한 체계적 문헌고찰 결과, 요관용 금속스텐트는 시술 관련 부작용 발생이 많지 않아 안전한 기술로 평가하였고 장기간 요관의 개통을 유지하는데 효과적인 기술로 평가하였다. 건강보험 청구자료를 활용한 성과 분석 결과, 요관용 금속스텐트는 중합체스텐트에 비해 고령, 중증도가 높은 환자에서의 시술 비율이 높고, 유지기간이 평균 4.4개월(132.8일)이 더 길고, 교체빈도 및 유지기간을 고려한 Cox 비례위험모형결과에서도 교체발생 위험이 0.52배로 통계적으로 유의하게 낮았다. 비용 및 재정영향 결과, 요관용 금속스텐트는 중합체스텐트에 비해 환자당 연간 비용이 7.5% (587천 원) 감소하였고, 민감도 분석으로 사망자 비용을 제외했을 때 22.5% (1,524천 원) 낮았다. 재정영향 분석에서 금속스텐트 점유율이 증가하면 현재보다 건강보험재정이 절감될 것으로 예상되며, 금속스텐트 점유율이 향후 5년간 10%으로 지속될 때는 건강보험재정이 현재보다 약 0.72%(56억 3백만원) 감소할 것으로, 5년간 매년 10%씩 증가할 때는 현재 건강보험재정에서 2.35%(181억 99백만원) 절감효과가 있을 것으로 예상되었다. 종합적으로 요관용 금속스텐트는 중합체 스텐트와 비교해 안전성 및 효과성은 유사하나, 금속 스텐트의 유지기간이 길어 교치빈도 감소효과가 있는 것으로 평가되었다. 스텐트 교체빈도의 감소로 환자 1인당 연간 의료비용은 낮고(7.5%), 요관용 금속스텐트 사용이 현재보다 증가할 경우 건강보험 재정절감 효과 역시 기대할 수 있는 것으로 평가되었다. 본 재평가는 체계적 문헌고찰을 통해 요관용 금속스텐트의 안전성 및 효과성을 평가한 것외 국내 건강보험 청구자료를 통해 국내 환자에서의 치료성과 및 소요비용을 확인한 것에 의의가 있다. 하지만, 현재 자료제공기간의 제한 등으로 인해 현재 급여기준인 선별급여 50%인 상황에서의 자료를 활용하지 못한 한계점이 있다. 따라서 소위원회는 요관용 금속스텐트의 선별급여 적합성 평가시기(2025년)에 맞춰 본 재평가 결과를 바탕으로 임상적 유용성을 재확인할 필요가 있다고 제언하였다. 2022년 제9차 의료기술재평가위원회(2022.9.2.)에서는 소위원회 검토 결과를 논의하여 의료기술재평가사업 관리지침 제4조 제10항에 의거 “요관용 금속스텐트”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 안전성 및 효과성에 대한 문헌적 근거와 비용 등을 종합적으로 고려하여 요관 폐쇄 환자에서 요관용 금속스텐트를 ‘권고함’으로 심의하였다.

Background Ureteral stents, placed in the ureter in cases with blockages from the kidney to the bladder, are widely used to help facilitate urine excretion in patients with ureteral obstruction. However, conventional stents need to be replaced regularly due to their vulnerability to stent dysfunction. To overcome this problem, metallic ureteral stents have been developed with therapeutic materials with high resistance to external pressure. In Korea, metallic ureteral stents were adopted as a selective benefit (80%) item from November 1, 2015, and subsequently, the co-payment rate was adjusted to 50% from May 1, 2021. The second appropriateness determination of the healthcare benefit is scheduled for November 2025. Based on the need for evidence of its effectiveness and long-term safety, metallic ureteral stents were proposed as a topic for prospective study in the Patient-Centered Medical Technology Optimization Research Project. However, the review opinion of the project steering committee was that a reassessment that could be performed more quickly might be more appropriate than an assessment of evidence through a prospective study. Accordingly, the task was transferred to the Committee of Health Technology Reassessment of the National Evidence-based Healthcare Collaborating Agency for review and reassessment. The purpose of this assessment was to provide scientific evidence of the clinical safety and effectiveness, and economic feasibility, of metallic ureteral stents as therapeutic material in patients with ureteral obstruction (ureteral narrowing or blockage). This would support policy decision-making, including the appropriate use of this health technology.

Methods The safety and effectiveness of metallic ureteral stents were analyzed using a systematic review, performance analysis, and cost and financial impact analyses. The assessment methods were finalized after review by the “metallic ureteral stent subcommittee” (hereinafter referred to as the “subcommittee”) after considering the purpose of the assessment. The subcommittee comprised six experts—three from urology and one each from nephrology, radiology, and evidence-based medicine. The key question in the assessment was “Are metallic ureteral stents clinically safe and effective in patients with ureteral obstruction?” Safety was assessed using ureteral stent migration or malfunction, ureteral injury, urinary tract infection, and hematuria as outcome indicators, while effectiveness was assessed using the stent patency rate, success rate, and retention time as outcome indicators. In the systematic review, three international and five domestic databases were searched, based on the key question, and two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria. Two reviewers independently performed risk of bias (RoB) assessment using the Cochrane RoB and the Risk of Bias Assessment for Non-randomized Studies (RoBANS) 2.0 tools. In the event of any disagreement, the reviewers discussed the matter to reach a consensus. All data were extracted at the study level. The systematic review results were assessed for level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method. The grade of recommendation was based on the assessment results. For the performance analysis, customized research data from the Health Insurance Review and Assessment Service (HIRA; project number: M20211007549) were used to identify the status of metallic ureteral stent use in Korea and their clinical effectiveness. The patient population consisted of adult patients who used ureteral stents between January 2015 and May 2021 and the patients were selected by episodes of ureteral stent insertion. Episodes were operationally defined, based on statements indicating the use of a ureteral stent with the same start of care date and the same care institution. Annual usage status and baseline characteristics by patient and episode were identified, while mean retention time, risk of stent replacement, risk of mortality, urinary infection within 30 days of the procedure, and readmission were analyzed as clinical effectiveness indicators. The baseline characteristics of the patients were presented as frequencies, percentages, means, and standard deviations (SDs). For the analysis of differences between groups, the chi-squared test was used for categorical variables and the t-test of analysis of variance (ANOVA) was used for continuous variables. For analysis of the risk of outcome indicators according to the type of ureteral stent, the Cox proportional hazards model was used to estimate the hazard ratio (HR), or logistic regression analysis was used to estimate the odds ratio (OR). The results were presented with 95% confidence intervals (CI) and significance probabilities (P-values). For cost and financial impact analyses, retrospective cohorts from health insurance claims data established during the performance analysis were used to estimate the cost-saving effect of using metallic ureteral stents and changes in health insurance financing over the next five years. Annual expenditure associated with metallic ureteral stents was defined as the average procedure cost per patient and the cost incurred during follow-up of ≤ 1 year. For the financial impact analysis, performance analysis results were reflected to estimate the market share of metallic ureteral stents and changes in health insurance financing due to changes in co-payment rates. Furthermore, sensitivity analysis was performed on cases, excluding mortality-related costs, which were highly likely to include healthcare costs other than ureteral stent procedure-related costs.

Results 1. Systematic review 1.1 Safety and effectiveness A total of six articles, including one randomized controlled trial (RCT) and five cohort studies, reported on clinical safety and effectiveness. One RCT, which was conducted in Korea, compared covered metallic ureteral stents (product name: Urexel) and polymeric stents (product name: Double J stent) in patients with malignant ureteral obstruction. The follow-up period in this RCT was 12 months. RoB assessment of this RCT showed that the RoB for participant-personnel blinding, outcome assessment blinding, incomplete outcome data, and selective reporting was low, while the RoB for randomization, allocation concealment, and private research funding was uncertain since these categories were not reported on. Among the cohort studies, most (n=4) reported on patients with malignant ureteral obstruction, and regarding the type of metallic stent used, most studies (n=3) used Resonance® (Cook Medical). The follow-up period in these studies was 1–72 months. RoB assessment of these studies showed that the RoB for comparability of cohorts, control selection, exposure measurement, and incomplete outcome data was reported to be “high” in at least one study, while the RoB for selective reporting and private research funding was mostly low. The RoB for confounding variables was assessed to be uncertain in all five studies since they were not mentioned.

1.1.1 Safety With respect to metallic ureteral stent procedure-related adverse events (AEs), the RCT reported no such AEs, while the rate of procedure-related AEs in the metallic stent group, compared to the polymeric stent group, was reported to be higher in one cohort study, but lower in another cohort study. In the RCT, stent migration was found in 0.1% of cases in the metallic stent group and 0% in the polymeric stent group, while stent malfunction was found in 0.4% of cases in the metallic stent group and 0.7% of cases in the polymeric stent group. In the cohort studies, the incidence of stent migration was < 0.1% in the metallic stent group, while the incidence of ureteral and kidney injury, hematuria, urinary tract infection, and pain were < 1% or showed no difference compared to the polymeric stent group.

1.1.2 Effectiveness In the RCT, the stent patency rate was higher in the metallic stent group than in the polymeric stent group at 3, 6, 9, and 12 months, and in particular, the stent patency rate at 12 months was statistically significantly higher in the metallic stent group. In the meta-analysis of the cohort studies, there were no differences in the patency rate at 7 days, and at 1, 3, and 6 months between the two groups. At 12 months, the metallic stent group showed a statistically significantly higher OR for patency rate, compared to the polymeric stent group (12-month patency rate OR: 1.41, 95% CI: 1.01–1.98, I2= 0%). In the RCT, the technical and stent success rates were 100% in both groups and in the cohort studies, there were no differences between the two groups (12-month stent success rate OR: 1.09, 95% CI: 0.72–1.64, I2= 0%). Ureteral stent retention time was 239 days in the metallic stent group and 80 days in the polymeric stent group in the RCT and 380 days in the metallic stent group and 183 days in the polymeric stent group in the cohort studies. One cohort study reported that quality of life was statistically significantly higher in the metallic stent group than in the polymeric stent group.

1.2 Economic feasibility Seven articles reported on economic feasibility-related outcomes, including cost. The mean procedure cost i.e., stent cost; surgery cost, including anesthesia; and test and examination costs; and the annual cost for maintaining the stent were estimated and the cost-savings relative to polymeric stents were reported. In all seven articles, the polymeric stent group showed longer retention and less frequent replacement than the metallic stent group, resulting in annual cost savings per patient of 43%–59.5%.

2. Performance analysis There was a total of 171,147 episodes (67,584 patients) of ureteral stent use in the retrospective cohorts being analyzed, of which, 361 episodes (0.2%) involved a metallic stent and 170,786 episodes (99.8%) involved a polymeric stent. Regarding the usage of metallic ureteral stents by year, the number of patients using these stents was low with < 100 patients per year, and the total cost per patient for ureteral stent insertion was approximately 4.3 million won, as of 2021. The number of patients who only used metallic stents was 41; only polymeric stents was 67,266; and both metallic and polymeric stents were 277. Regarding the baseline characteristics of the patients, the patients who received metallic stents were older, with more severe comorbidities, than patients who received polymeric stents. The mean follow-up period was 3 years. Patients who only used metallic stents received an average of 1.1 insertion procedures and used 1.68 stents, while patients who only used polymeric stents received an average of 2.5 insertion procedures and used 3.3 stents. Regarding baseline characteristics according to episodes, metallic and polymeric stent procedures were mostly performed in tertiary hospitals (≥ 60%), while metallic stent insertion was performed most often by ureteroscopic insertion (49%) and polymeric stent insertion was performed most often by cystoscopic insertion (65%). The mean length of hospital stay was 8.48 days in the metallic stent group and 6.88 days in the polymeric stent group. The mean cost per insertion was 4.231 million won in the metallic stent group and 3.342 million won in the polymeric stent group. The mean stent retention time was longer by 4.4 months (132.8 days) and the frequency of replacement was lower by approximately 30% in the metallic stent group than in the polymeric stent group. In the Cox proportional hazard model analysis considering the frequency of replacement and retention time, the HR of replacement was statistically significantly lower (HR: 0.52; 95% CI: 0.31–0.86). In the subgroup analyses, the HR of stent replacement was statistically significantly higher in the metallic stent group than in the polymeric stent group for all subgroups except the ≥ 80 years group and the non-cancer patient group. The baseline-adjusted results showed no statistically significant differences in mortality, urinary tract infection within 30 days, readmission, and emergency department admission between the two groups.

3. Cost and financial impact analyses Comparison of the annual cost per patient between metallic and polymeric stents showed that the cost was lower by approximately 800,000 won (7.5%) in the metallic stent group than in the polymeric stent group. Moreover, sensitivity analysis, excluding mortality-related costs, also showed that the cost was lower by 1.52 million won (22.5%). In addition, when generalized estimating equations were applied, the difference in cost per person between the metallic and polymeric stent groups was statistically significant. In the financial impact analysis, the results showed that if the current co-payment rate of 50% and metallic stent market share of 0.4% are maintained, the total health insurance expenditure for the next five years would be 775.016 billion won and health insurance expenditure would decrease if the metallic stent market share increased (scenarios 1–6). The current metallic stent market share is 0.4%, and if the market share increased to 10%, the estimated health insurance expenses for the next five years would decrease by 0.72% (▽ 5.603 billion won). If the market share increased by 10% each year, health insurance expenditure would decrease by 2.35% (▽ 18.199 billion won). In the sensitivity analysis, the results showed a decrease in health insurance expenditure by 2.16% (14.538 billion won) if the metallic stent market share was 10% and by 7.02% (47.224 billion won) if the market share increased by 10% each year.

Conclusions and recommendations Based on the systematic review of the clinical safety and effectiveness of metallic ureteral stents, metallic ureteral stents were assessed to be a safe health technology since there were very few procedure-related AEs. It was also assessed to be an effective health technology for maintaining ureteral patency. The performance analysis using health insurance claims data showed that metallic ureteral stents were used more often than polymeric stents in patients who were older and had more severe comorbidities. Moreover, metallic ureteral stents showed a longer retention time (mean: 4.4 months or 132.8 days) and the HR of replacement in the Cox proportional hazard model, considering the frequency of replacement and retention time, was also statistically significantly lower at 0.52. In the cost and financial impact analyses, the results showed that the annual cost per patient was lower by 7.5% (587,000 won) in the metallic ureteral stent group than in the polymeric stent group, while the sensitivity analysis, excluding mortality-related costs, also showed lower costs, by 22.5% (1.524 million won). In the financial impact analysis, the results showed that health insurance expenditure would decrease if the metallic stent market share increased. The results also showed that if the metallic stent market share was maintained at 10% for the next five years, the estimated health insurance expenditure would decrease by 0.72% (5.603 billion won) relative to the present time, and if the market share increased by 10% each year for the next five years, health insurance expenditure would decrease by 2.35% (18.199 billion won). In summary, metallic ureteral stents were assessed to have similar safety and effectiveness as polymeric stents, but metallic stents had longer retention time, which had the effect of reducing the frequency of replacement. Owing to the decrease in the frequency of stent replacement, annual healthcare costs per patient could be reduced (7.5%) and an increase in the use of metallic ureteral stents was also expected to reduce health insurance expenditure. This reassessment was significant as it not only assessed the safety and effectiveness of metallic ureteral stents by a systematic review but also analyzed the performance and expenditure in Korea by using Korean health insurance claims data. However, due to the limitations in the current data processing period, the reassessment did not use data for the current benefit standard, which is a co-payment of 50%. Accordingly, the subcommittee concluded that it is necessary to reconfirm the clinical usefulness of metallic ureteral stents based on the findings in this reassessment, in keeping with the appropriateness determination of selective benefits in 2025. During the ninth session of the Committee of Health Technology Reassessment, on September 2, 2022, the committee discussed the subcommittee findings and reached the following conclusion in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project. The Committee of Health Technology Reassessment “recommended” the use of metallic ureteral stents in patients with ureteral obstruction in consideration of evidence of its safety and effectiveness and cost factors.
