Abstract

평가배경 Iodine-123 Fluoro-propyl-carbomethoxy-iodophenyl-tropane(이하 ‘I-123 FP-CIT’) 뇌 단일광자단층촬영(Single photon Emmision Computed Tomography, 이하 ‘SPECT’)은 파킨슨병 (Parkinson's disease)이 의심되는 환자를 대상으로 질환감별 목적으로 I-123 FP-CIT 방사성 동위원소를 이용하여 뇌 부위를 SPECT 검사하는 기술이다. 본 기술은 2012년 제6차 신의료기술평가위원회(2012.6.15.)에서 안전성 및 유효성이 있는 기술로 심의(보건복지부 고시 제2012-112호, 2012.9.3.)되어 이후 비급여로 등재되었고(2013.7.1.), 2014년에 선별급여 80%로 변경된 이후(2014.9.1.), 2018년 선별급여 50%로 전환되었다(2018.1.1.). 본 기술에 대해 내부모니터링 결과 선별급여대상 적용 후 일정 기간이 경과하여(선별급여 50% 적용 3년 6개월 경과, 신의료기술평가 이후 8년 경과) 의료기술재평가가 필요하다고 판단하였다. 본 기술은 임상자문 및 우선순위심의를 거쳐 의료기술재평가 대상으로 선정되어 2021년 제9차 의료기술재평가위원회(2021.9.10.)에서 평가계획을 심의한 후 재평가를 수행하였다.

평가방법 I-123 FP-CIT 뇌 SPECT에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “I-123 FP-CIT 뇌 SPECT 소위원회(이하 ‘소위원회’)”의 심의를 거쳐 확정하였다. 소위원회는 구성은 신경과 2인, 핵의학과 2인, 신경외과 1인, 정신건강의학과 1인, 근거기반의학 1인의 전문가 총 7인으로 구성하였다. 본 기술의 건강보험심사평가원 고시항목 상세내용 중 실시목적에는 ‘파킨슨 증상을 나타내는 환자 중 도파민 신경세포의 손상을 동반하는 파킨슨증과 도파민 신경세포의 손상을 동반하지 않는 파킨슨증의 감별’ 및 ‘도파민 신경세포의 손상을 동반하는 파킨슨증과 본태성진전증의 감별’로 제시되어 있다. 이를 근거로 핵심질문은 ‘I-123 FP-CIT 뇌 SPECT 검사는 파킨슨병 의심 환자를 대상으로 도파민 신경세포의 손상을 동반하는 파킨슨증과 도파민 신경세포의 손상을 동반하지 않는 파킨슨증을 감별하는데 임상적으로 안전하고 효과적인가?’ 및 ‘I-123 FP-CIT 뇌 SPECT 검사는 도파민 신경세포의 손상을 동반하는 파킨슨증과 본태성진전증의 감별하는데 임상적으로 안전하고 효과적인가?’로 정한 바 있다. 이후 문헌을 검색하고 선택배제기준에 따라 선별한 결과 핵심질문에 부합한 선택문헌은 확인되지 않았다. 특히, 임상진단으로는 도파민 신경세포의 손상을 확인할 수 없기 때문에 실제 도파민 신경세포의 손상여부를 임상에서 입증하기는 어려울 수 있다는 소위원회의 의견이 있었다. 다만, I-123 FP-CIT 뇌 SPECT와 관련된 문헌들에서 대부분 연구대상은 모두 도파민 신경세포의 손상과는 상관없이 임상진단을 기준으로 환자군은 파킨슨병을 중심으로, 대조군은 파킨슨병을 제외한 파킨슨증 환자로 도파민 신경세포의 손상 여부를 나타내는 검사 결과가 연구대상에서 파킨슨병을 정확하게 감별해줄 수 있는지를 보여주는 의료결과로 확인되었다. 이에 따라 소위원회에서는 현재의 문헌적 수준에서 도출할 수 있는 의료결과에 초점을 두고 핵심질문을 ‘I-123 FP-CIT 뇌 SPECT은 파킨슨병 의심환자를 대상으로 파킨슨병과 파킨슨병을 제외한 파킨슨증을 감별하는데 임상적으로 안전하고 효과적인가?’ 및 ‘I-123 FP-CIT 뇌 SPECT은 파킨슨병 의심환자를 대상으로 파킨슨병과 본태성진전증을 감별하는데 임상적으로 안전하고 효과적인가?’로 수정하여 체계적 문헌고찰을 수행하였다. 의료결과는 안전성, 효과성 및 경제성에 대해 확인하였다. 위 핵심질문을 토대로 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, 사이언스온) 및 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials)을 이용하여 문헌을 검색하였다. 문헌 선정기준 및 배제기준 적용을 통한 문헌선택과 비뚤림위험 평가는 모두 2명의 검토자가 독립적으로 수행하였고, 자료추출은 한 명의 검토자가 우선적으로 추출한 후 다른 검토자가 추출한 결과를 독립적으로 검토하고 오류가 있는지 확인하였다. 문헌의 비뚤림위험 평가는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)을 사용하였다. 자료분석은 정량적 분석이 불가능하여 질적으로 기술하였다.

평가 결과 체계적 문헌고찰 결과 선택된 문헌은 총 27편이었으며, 안전성 및 진단정확도, 의료결과에의 영향을 보고하였다. 경제성에 대해 보고한 문헌은 확인되지 않았다. 비뚤림위험 평가결과 선택 비뚤림 위험이 65%에서 높음으로 평가되었으며, 중재검사와의 수행 또는 해석과정에서 비뚤림을 초래할 가능성은 55%에서 불확실로 평가되었다.

안전성 안전성은 검사관련 이상반응 및 방사성 의약품에 대한 부작용으로 평가하고자 하였고, 이를 보고한 문헌은 총 5편이었다. 검사와 관련하여 주로 보고된 이상반응은 두통, 감기유사증상, 어지러움(현기증), 주사부위 출혈(혈종), 낙상으로, 가장 많이 발생한 이상반응은 두통으로 1편의 문헌에서 전체 검사자 중 약 8%에서 두통으로 발생을 보고하였다. 그 밖에 문헌에서도 중재검사와 관련있는 심각한 이상반응은 발생하지 않았다고 보고하였다. 이에 소위원회에서는 I-123 FP-CIT 뇌 SPECT 검사로 발생한 이상반응이 대부분 경미한 수준으로 동 기술을 안전한 기술로 판단하였다.

효과성 효과성은 진단정확성, 참고표준기준(임상진단)과의 일치도, 의료결과에의 영향으로 평가하였으며, 이 중 참고표준검사(임상진단)과의 일치도를 보고한 문헌은 확인되지 않았다. 진단정확성은 총 18편에서 보고하였으며, 파킨슨병과 파킨슨병이 아닌 파킨슨증과의 감별에 대한 진단정확도를 보고한 문헌은 16편, 파킨슨병과 본태성진전증 감별에 대한 진단정확도 문헌은 4편이었으며, 이 중 2편은 파킨슨병과 파킨슨병이 아닌 파킨슨증 감별 및 본태성진전증 감별에 대한 진단정확성을 모두 보고하였다. 파킨슨병과 파킨슨병이 아닌 기타 질환과의 감별에 대한 진단정확성은 분석방식에 따라 반정량적 분석을 이용한 문헌 9편, 육안적 분석 8편, 정량적 방법 1편으로 총 16편에서 결과를 제시하였다. 육안적 분석을 통해 제시된 9편의 문헌에서 진단정확도 결과는 민감도 0.33~1.00, 특이도 0.06~1.00 이었다. 반정량 분석을 통해 제시된 8편의 문헌에서 진단정확도 결과는 민감도 0.53~0.95, 특이도 0.15~1.00 이었고. 정량적 분석을 통해 제시된 1편의 문헌에서 민감도 0.59, 특이도 0.93, 양성예측도 0.80, 음성예측도 0.83 이었다. 파킨슨병 환자와 본태성진전증 환자 감별에 대한 진단정확도는 육안적 분석, 정량적 분석을 기준으로 평가되었으며, 총 4편에서 결과를 제시하였다. 육안적분석을 기준으로 제시된 진단정확도는 민감도 0.70~0.82, 특이도 0.72~0.95 이었고, 정량적분석을 기준으로 제시된 민감도는 0.91, 특이도는 1.00 이었다. 의료결과에의 영향을 보고한 문헌 5편 중 대부분의 문헌에서 I-123 FP-CIT 뇌 SPECT 검사를 통해 진단이 변경되거나 치료계획이 변경된 경우(약물 종류나 용량의 변경)의 비율을 제시하였다. 1편의 문헌에서는 동 검사 후 의무기록이 65%에서 약물 변경 등 치료방식이 변경되었다고 보고하였고, 다른 1편에서는 검사 후 49.2%의 환자에서 치료계획의 변화가 있었다고 보고하였다. 이에 소위원회에서는 I-123 FP-CIT 뇌 SPECT 검사가 임상적으로 파킨슨병과 약물 유도성 파킨슨증감별에 있어서는 유용하게 활용되고 있으며 현재 문헌에서 보고된 진단정확성이 수용가능한 수준으로 효과적인 기술로 판단하였다. 다만 파킨슨병 중에는 도파민 신경세포의 손상에 대한 증거가 확인되지 않는 파킨슨병이 있는 경우까지 고려했을 때, 본 검사에서의 위음성 결과를 해석하기 어려운 점이 있어 근거가 제한적이라고 판단하였다.

결론 I-123 FP-CIT 뇌 SPECT 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. I-123 FP-CIT 뇌 SPECT은 파킨슨병 의심환자를 대상으로 도파민 신경세포의 손상을 동반하는 파킨슨증과 도파민 신경세포의 손상을 동반하지 않는 파킨슨증을 감별하는 목적으로 수행하는 검사로서 안전한 기술이나, 효과성 및 경제성은 관련 문헌적 근거를 확인할 수 없어 판단할 수 없다는 의견이었다. 또한 도파민 신경세포의 손상을 동반하는 파킨슨증과 본태성진전증을 감별하는 목적으로도 안전한 기술이나, 효과성 및 경제성은 관련 문헌적 근거를 확인할 수 없어 판단할 수 없다는 의견이었다. I-123 FP-CIT 뇌 SPECT은 파킨슨병 의심환자를 대상으로 파킨슨병과 파킨슨병을 제외한 파킨슨증을 감별하는 목적으로 수행 시 안전한 검사이며, 효과적인 기술이나 근거가 제한적이라는 의견이었으며, 경제성은 관련된 문헌적 근거를 확인할 수 없어 판단할 수 없다는 의견이었다. 또한 파킨슨병과 본태성진전증을 감별하는 목적으로 수행 시 안전한 검사이며, 효과적인 기술이나 근거가 제한적이라는 의견이었으며, 경제성은 관련된 문헌적 근거를 확인할 수 없어 판단할 수 없다는 의견이었다. 2022년 제6차 의료기술재평가위원회(2022.6.10.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “I-123 FP-CIT 뇌 SPECT”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 I-123 FP-CIT 뇌 SPECT에 대해 파킨슨병 의심환자를 대상으로 도파민 신경세포의 손상을 동반하는 파킨슨증과 도파민 신경세포의 손상을 동반하지 않는 파킨슨증을 감별하는 목적으로 수행하는 검사로서 안전한 기술이나, 효과성 및 경제성은 관련 문헌이 없어 판단할 근거가 확인되지 않아 ‘불충분’으로 심의하였고, 도파민 신경세포의 손상을 동반하는 파킨슨증과 본태성진전증을 감별하는 목적으로도 안전한 기술이나, 효과성 및 경제성은 관련 문헌이 없어 판단할 근거가 확인되지 않아 ‘불충분’으로 심의하였다. 또한 의료기술재평가위원회는 I-123 FP-CIT 뇌 SPECT은 파킨슨병 의심 환자를 대상으로 파킨슨병과 파킨슨병을 제외한 파킨슨증을 감별하는 목적으로 수행시 안전하고 효과적인 기술이나 문헌적 근거가 제한적이어서 ‘조건부 권고함’으로 심의하였고, 파킨슨병과 본태성진전증을 감별하는 목적으로 수행 시 안전한 검사이나, 임상적 유용성 및 문헌적 근거가 제한적으로 ‘불충분’으로 심의하였다.

Background Iodine-123 fluoro-propyl-carbomethoxy-iodophenyl-tropane (I-123 FP-CIT) brain single-photon emission computed tomography (SPECT) is a technology in which SPECT is performed using the I-123 FP-CIT radioisotope. In patients suspected of having Parkinson’s disease (PD), I-123 FP-CIT SPECT is performed on the brain region to establish differential diagnoses. During the sixth session of the Committee of New Health Technology Assessment in 2012 (June 15, 2012), I-123 FP-CIT brain SPECT was reviewed as a safe and effective technology (Ministry of Health and Welfare Notice No. 2012-112; December 3, 2012) and subsequently registered as a non-benefit item (July 1, 2013). It was changed to a selective benefit (80%) item on September 1, 2014, and was subsequently converted to a selective benefit (50%) item on January 1, 2018. Based on internal monitoring, a health technology reassessment was deemed necessary because a certain period of time had elapsed (3 years and 6 months since the application of selective benefit 50% status and 8 years since a new health technology assessment). Accordingly, I-123 FP-CIT brain SPECT was selected as a health technology reassessment item by clinical consultation, and a priority review and reassessment were performed after the assessment plan was reviewed during the ninth session of the Committee of New Health Technology Reassessment in 2021 (September 10, 2021).

Methods A systematic review was performed to assess the safety and effectiveness of I-123 FP-CIT brain SPECT. All assessment methods were finalized after a review by the “I-123 FP-CIT brain SPECT subcommittee” (hereinafter referred to as the “subcommittee”) with consideration of the assessment’s purpose. The subcommittee was composed of seven experts: two each from neurology and nuclear medicine and one each from neurosurgery, psychiatry, and evidence-based medicine. In the Health Insurance Review and Assessment Service notice, the purpose of this technology is listed as “differentiation between parkinsonism with and without dopaminergic neuron damage in patients exhibiting symptoms of PD” and “differentiation between parkinsonism with dopaminergic neuron damage and essential tremor (ET).” Based on this, the key questions were, “Is I-123 FP-CIT brain SPECT clinically safe and effective in differentiating between parkinsonism with and without dopaminergic neuron damage in patients suspected of having PD?” and “Is I-123 FP-CIT brain SPECT clinically safe and effective in differentiating between parkinsonism with dopaminergic neuron damage and ET?” A subsequent literature search followed by screening based on the inclusion and exclusion criteria identified no articles that corresponded to the key questions. The subcommittee opined that because dopaminergic neuron damage cannot be clinically diagnosed, it may be difficult to demonstrate actual dopaminergic neuron damage in clinical settings. However, most articles related to I-123 FP-CIT brain SPECT examined health outcomes that showed whether test results on dopaminergic neuron damage status could accurately differentiate PD in participants, with the patient group defined as those with PD regardless of dopaminergic neuron damage and the control group comprising those who have parkinsonism other than PD. Accordingly, the subcommittee focused on health outcomes that could be derived from currently available literature and revised the key questions to “Is I-123 FP-CIT brain SPECT clinically safe and effective in differentiating between PD and parkinsonism other than PD in patients suspected of having PD?” and “Is I-123 FP-CIT brain SPECT clinically safe and effective in differentiating between PD and ET in patients suspected of having PD?” Subsequently, a literature review was performed based on the revised key questions. The health outcomes assessed were safety, effectiveness, and economic feasibility. Five domestic databases (KoreaMed, KMbase, KISS, KERIS, and ScienceOn) and three international databases (Ovid MEDLINE, Ovid EMBASE, and Cochrane Central Register of Controlled Trials) were searched based on the key questions. Two reviewers independently selected articles based on the inclusion and exclusion criteria and assessed the risk of bias (RoB) in the selected articles. Data extraction was initially performed by a single reviewer, and another reviewer independently reviewed the extracted results to check for errors. RoB assessment was performed using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). The data were qualitatively analyzed since a quantitative analysis was not possible.

Results For the systematic review, a total of 27 articles that reported on safety, diagnostic accuracy, and impact on health outcomes were selected. Articles reporting on economic feasibility were not found. On assessing RoB, the risk of selection bias was considered high in 65% of the articles, while the risk associated with the performance and interpretation of the interventional test was considered uncertain in 55% of the articles.

Safety Safety was designed to be assessed based on test-related adverse reactions (ARs) and radiopharmaceutical-related adverse events; five articles reported on these factors. Test-related ARs consisted mostly of headaches, cold-like symptoms, dizziness (vertigo), injection site bleeding (hematoma), and falls. The most common AR was headache, with one article reporting headache occurrence in approximately 8% of all study participants. Other articles reported that there were no serious ARs related to the interventional test. Accordingly, the subcommittee determined that the ARs that occurred due to I-123 FP-CIT brain SPECT were mostly mild and that, therefore, this health technology is safe.

Effectiveness Effectiveness was to be assessed based on diagnostic accuracy, agreement with reference standards (clinical diagnosis), and impact on health outcomes. However, no articles reported on agreement with reference standards (clinical diagnosis). A total of 18 articles reported on diagnostic accuracy, of which 16 reported on diagnostic accuracy regarding the differentiation between PD and parkinsonism other than PD and four reported on diagnostic accuracy regarding the differentiation between PD and ET. Of these, two reported on diagnostic accuracy regarding the differentiation between PD and parkinsonism other than PD and between PD and ET. The 16 articles that reported on the diagnostic accuracy regarding the differentiation between PD and diseases other than PD varied in their statistical analysis: nine articles used semi-quantitative analysis, eight articles used visual assessment, and one article used quantitative analysis. In the nine articles that used visual assessment, the diagnostic accuracy results had a sensitivity of 0.33–1.00 and a specificity of 0.06–1.00. In the eight articles that used semi-quantitative analysis, the diagnostic accuracy results had a sensitivity of 0.53–0.95 and a specificity of 0.15–1.00. In the one article that used quantitative analysis, the results had a sensitivity of 0.59, a specificity of 0.93, a positive predictive value of 0.80, and a negative predictive value of 0.83. The diagnostic accuracy regarding the differentiation between PD and ET was assessed by visual and quantitative analyses in the four articles. Diagnostic accuracy had a sensitivity of 0.70–0.82 and a specificity of 0.72–0.95 in visual analysis results, and a sensitivity of 0.91 and a specificity of 1.00 in quantitative analysis results. Five articles reported on the impact on health outcomes, with most articles presenting the percentage of cases in which the diagnosis or treatment plan (change in drug type or dosage) was changed based on the results of the I-123 FP-CIT brain SPECT. One article reported that 65% of medical records contained changes in the treatment method, such as a change in medication, after I-123 FP-CIT brain SPECT, while another reported that the treatment plan changed in 49.2% of patients after I-123 FP-CIT brain SPECT. Accordingly, the subcommittee determined that I-123 FP-CIT brain SPECT is an effective health technology as it is clinically useful for differentiating PD and drug-induced parkinsonism and its currently reported diagnostic accuracy is at an acceptable level. However, considering that there are some cases of PD in which the evidence of dopaminergic neuron damage cannot be confirmed, the evidence for this test was determined to be limited due to difficulties in interpreting false-negative results.

Conclusions Based on the current assessment results, the subcommittee recommended the following: For differentiating between parkinsonism with and without dopaminergic neuron damage in patients suspected of having PD, I-123 FP-CIT brain SPECT is safe, but its effectiveness and economic feasibility cannot be determined due to a lack of evidence in the literature. For differentiating between parkinsonism with dopaminergic neuron damage and ET, I-123 FP-CIT brain SPECT is safe, but its effectiveness and economic feasibility cannot be determined due to a lack of evidence in the literature. For differentiating between PD and parkinsonism other than PD in patients suspected of having PD, I-123 FP-CIT brain SPECT may be safe (as there is limited evidence), but its economic feasibility cannot be determined due to a lack of evidence in the literature. For differentiating between PD and ET, I-123 FP-CIT brain SPECT may be safe and effective (as there is limited evidence), but its economic feasibility cannot be determined due to a lack of evidence in the literature. During the sixth session of the Committee of Health Technology Reassessment in 2022 (June 10, 2022), the committee made the following conclusions regarding “I-123 FP-CIT brain SPECT” based on the subcommittee’s findings in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project: I-123 FP-CIT brain SPECT is a safe health technology when used for differentiating between parkinsonism with and without dopaminergic neuron damage in patients suspected of having PD, but the evidence of its effectiveness and economic feasibility is “insufficient” due to a lack of relevant literature. I-123 FP-CIT brain SPECT is a safe technology when used to differentiate between parkinsonism with dopaminergic neuron damage and ET, but the evidence of its effectiveness and economic feasibility is “insufficient” due to a lack of relevant literature. I-123 FP-CIT brain SPECT is a safe and effective health technology when used for differentiating between PD and parkinsonism other than PD in patients suspected of having PD, however, it is “conditionally recommended” due to a lack of evidence in the literature. I-123 FP-CIT brain SPECT is a safe test when used for differentiating between PD and ET, however, it is determined to be “insufficient” due to its limited clinical usefulness and evidence in the literature.
