자율신경계이상검사(기립성 혈압검사)

Abstract

평가배경 기립성 혈압검사(Orthostatic blood pressure test)는 5분 이상 눕혀 놓은 상태에서 혈압과 맥박을 측정한 후 일어선 상태에서 1분과 3분 후에 혈압과 맥박을 각각 측정 및 비교하여 자율신경계 기능을 평가하는 검사이다. 기립성 혈압검사는 2006년에 등재 비급여로 고시되었으며, 이후 건강보험심사평가원 예비급여부에서 비급여의 급여화 추진을 위하여 본 검사를 포함한 자율신경계이상검사 7개 항목(교감신경피부반응검사, 자율신경계이상검사(기립성 혈압검사, 발살바법, 지속적 근긴장에 따른 혈압검사, 심박변이도검사, 피부전도반응검사), 정량적 발한 축삭 반사검사)에 대한 재평가를 한국보건의료연구원에 의뢰하였다(2022.1.). 이에 따라 2022년 제2차 의료기술재평가위원회(2022.2.18.)에서는 해당 기술들의 평가계획서 및 통합 소위원회 구성안을 심의한 후 임상적 안전성 및 효과성을 평가하였다.

평가방법 이번 평가는 자율신경계 기능장애 의심환자 및 질환자에서 자율신경계기능을 평가하기 위한 자율신경계이상검사(기립성 혈압검사)(이하 ‘기립성 혈압검사’)가 임상적으로 안전하고 효과적인지 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “통합 자율신경계이상검사 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 신경과 3인, 내분비대사내과 2인, 순환기내과 2인, 정신건강의학과 2인, 근거기반의학 2인의 전문가 총 11인으로 구성하였다. 평가의 핵심질문은 ‘자율신경계 기능장애 의심환자 및 질환자를 대상으로 자율신경계기능을 평가하기 위한 기립성 혈압검사는 임상적으로 안전하고 효과적인가?’이다. 안전성은 검사 관련 이상반응 또는 부작용, 효과성은 진단정확성, 비교검사와의 결과 비교, 질환과의 관련성, 의료결과에의 영향으로 평가하였다. 체계적 문헌고찰은 위의 핵심질문을 토대로 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, 사이언스온) 및 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials)를 이용하여 문헌검색하였다. 문헌 선정기준 및 배제기준 적용을 통한 문헌선택과 자료추출, 비뚤림위험평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 문헌의 비뚤림 위험평가는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)을 사용하였다. 자료분석은 진단정확성의 경우 대상환자 및 진단기준이 다양하고 문헌별로 2×2 표 값이 추출되지 않아 질적으로 기술하였다. 기립경사테이블검사와의 비교결과는 양적으로 합성이 불가능하여 질적으로 기술하였다. 질환과의 관련성은 환자대조군(환자군과 해당질환이 없는 그룹으로 구분하여 분석한 경우), 질환대조군(각각 다른 질환군끼리 비교한 경우), 자율신경계병증 유무, 질환 세부 유형별, 질환중증도별로 구분하여 오즈비를 이용한 숲그림을 통해 결과 방향의 경향성을 확인하였다.

평가결과 체계적 문헌고찰 결과 선택문헌은 총 22편(진단법평가연구 8편, 진단적 코호트 연구 3편, 환자대조군 연구 11편)이었다. 비뚤림위험을 평가한 결과 환자선택 영역 및 적용성에 대한 우려는 각각 31.8%, 22.7%에서 ‘높음’, 중재검사 영역 및 적용성에 대한 우려는 각각 27.3%, 40.9%에서 ‘불확실’로 평가하였다. 참고표준검사 영역에서 비뚤림 위험은 40.9%에서 ‘높음’, 적용성에 대한 우려는 50.0%에서 ‘불확실’로 평가하였다. 연구진행과 시점에 대한 비뚤림 위험은 13.6%에서 ‘높음’으로 평가하였다.

안전성 기립성 혈압검사의 안전성은 검사 관련 이상반응 및 합병증으로 평가하고자 하였으나, 이를 보고한 문헌은 없었다. 기립성 혈압검사의 안전성을 확인하기 위해 참고문헌을 검토한 결과, 본 기술은 알려진 금기사항이 없는 안전한 기술로, 기립상태가 불안정하거나 낙상의 위험이 있는 대상자에서는 검사시 기립을 유지하기 위해 도움이 필요하며, 안전문제나 임상사건 관리에 대해 교육을 받은 자격이 있는 인력이 감독해야 한다고 보고하였다.

효과성 기립성 혈압검사의 효과성은 진단정확성, 기립성 혈압검사와 기립경사테이블검사와의 진단정확성 및 결과 비교, 질환과의 관련성으로 구분하여 제시하였으며, 의료결과의 영향을 보고한 연구는 없었다. 기립성 혈압검사의 자율신경계병증, 기립성 저혈압 등에 대한 진단정확성 결과는 8편에서 전체 민감도 0.20~0.69, 특이도 0.66~1.00으로 보고하였다. 진단 목적별로 자율신경계병증의 진단정확성(4편)은 민감도 0.1 ~ 0.53, 특이도 0.84 ~ 1.0, 기립성 저혈압의 진단정확성(3편)은 민감도 0.21 ~ 0.61, 특이도 0.66 ~ 1.0이었다. 파킨슨병과 다계통 위축증 파킨슨 타입과의 감별진단(1편)은 민감도 0.64, 특이도 0.66이었다. 기립성 혈압검사와 기립경사테이블검사를 비교한 연구는 3편으로 체위기립성 빈맥증후군에 대한 진단정확도(1편) 및 실신 환자의 혈압결과(2편)을 비교하였다. 1편은 기립성 못견딤증 환자 대상으로 체위기립성 빈맥증후군 진단시 기립성 혈압검사와 기립경사테이블 검사의 진단정확도가 유사하였으며, 1편은 기립성 혈압검사가 실신의 진단적 검사로 기립경사테이블검사 만큼 잠재성이 있다고 보고하였다. 나머지 1편은 실신 종류(심장기능억제형, 혈관미주신경성)에 따라 기립성 혈압검사와 기립경사테이블검사의 심박동수, 수축기 혈압 및 이완기 혈압 변화를 비교했으며, 기립성 혈압검사가 실신 종류에 따른 차이를 보여주지는 못했다. 질환과의 관련성은 11편에서 자율신경계 기능장애 유무별, 환자 및 질환 대조군별, 질환 세부 유형별, 질환 증상 유무별로 기립성 혈압검사의 비정상 비율을 비교하였을 때 특정 질환에서 유의하게 높거나 유의한 차이가 없었다. 연구대상자는 당뇨병환자, 겸상적혈구 빈혈 환자, 만성 신부전 환자, 내당능 장애 환자, 파킨슨병 및 다계통 위축증 등 다양하였다. 자율신경계병증 유무에 따른 기립성 혈압검사의 비정상 비율차이(4편)는 자율신경계병증이 있는 그룹에서 기립성 혈압검사의 비정상 비율이 유의하게 높거나 유의한 차이가 없었다. 파킨슨병과 다계통 위축증 구분과 관련하여 2편에서 다계통 위축증에서 기립성 혈압검사의 비정상 비율이 유의하게 높거나 높은 경향을 보였으나 그 외 질환에서는 유의한 차이가 없었다. 질환 세부 유형별로는 특정 유형에서 기립성 혈압검사의 비정상 비율이 더 높은 경향을 보이거나 세부 유형별 차이가 없었다. 질환 중증도와의 관련성은 1편에서 자율신경계병증이 심할수록 기립성 혈압검사의 비정상 비율이 유의하게 높았다. 이에 소위원회에서는 기립성 혈압검사는 임상에서 많이 사용하고 있지는 않으며, 초기 저혈압 진단에 도움이 되는 검사이지만 기립경사테이블검사보다 민감도가 떨어지는 것으로 알려져 있다는 의견이 있었다. 일부 연구에서 자율신경계병증 진단과 관련해 기립성 혈압검사의 민감도가 낮은 것은 자율신경계의 손상 정도가 심각해야 기립성 혈압검사에서 비정상으로 나올 수 있기 때문에 민감도가 낮게 나올 수 있으며, 문헌에서 질병 초기에 자율신경계 기능장애를 보이는 다계통 위축증이나 당뇨병 환자, 자율신경계병증군과 기립성 혈압검사의 비정상 비율과 관련성이 있고 실신을 진단하는 검사로써 기립성 혈압검사가 기립경사테이블검사 정도의 가능성이 있어 임상적 유용성이 있다고 판단하였다. 추가적으로 임상에서는 박동간 혈압감시기(beat to beat blood pressure monitoring)를 이용할 때 검사의 정확도가 더 높아질 수 있으며, 여러 가이드라인 및 치료지침에 실신평가 과정내에 기립성 혈압검사 내용이 포함되어있다는 의견을 제시하였다.

결론 통합 자율신경계이상검사 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 기립성 혈압검사는 자율신경계 기능장애 의심 환자 및 질환자를 대상으로 자율신경계기능 평가목적으로 사용시 알려진 금기사항이 없는 안전한 기술이나 기립상태가 불안정하거나 낙상의 위험이 있는 대상자에서는 기립을 유지하기 위해 도움이 필요하며, 안전문제나 임상사건 관리에 대해 교육을 받은 자격이 있는 인력이 감독해야 한다고 판단하였다. 기립성 혈압검사의 효과성은 i) 임상에서 여러 자율신경계검사를 종합해서 판단한다는 점을 고려했을 때 기립성 혈압검사의 진단정확성 수준은 수용가능하여 임상진단에 도움이 되는 기술, ii) 효과성에 대한 결론을 내리기에는 근거가 제한적인 기술, iii) 검사 인력 및 과정에 대한 규정이 필요하며, 검사방법의 표준화가 되지 않아 근거가 부족한 기술, iv) 임상진단에 유용한 검사이나 단독검사로는 근거가 부족한 상태로 통합 지표를 포함한 더 많은 연구결과가 필요한 기술로 전문가 간 이견이 있었다. 2023년 제1차 의료기술재평가위원회(2023.1.13.)에서는 기립성 혈압검사에 대한 여러 전문가 간 이견을 포함한 소위원회 결론을 검토하여 다음과 같이 심의 의결하고 권고등급을 결정하였다. 기립성 혈압검사는 알려진 금기사항이 없는 안전한 기술이나 기립상태가 불안정하거나 낙상의 위험이 있는 대상자에서는 기립을 유지하기 위해 도움이 필요하며, 안전문제나 임상사건 관리에 대해 교육을 받은 자격이 있는 인력이 감독해야 한다고 판단하였다. 효과성은 임상에서 자율신경계 기능장애 평가시 여러 자율신경계검사들을 종합해서 판단하는 점을 고려했을 때 임상진단에 도움이 되는 기술로 판단하였다. 이에 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 자율신경계 기능장애 의심 환자 및 질환자를 대상으로 자율신경계기능 평가목적으로 사용시 기립성 혈압검사를 ‘조건부 권고함’으로 심의하였다.

Background The orthostatic blood pressure (BP) test is designed to assess autonomic nervous system function by comparing BP and heart rate (HR) measured after the subject has been lying down for at least 5 minutes and measuring them again at 1 and 3 minutes after standing. Orthostatic BP test was listed as a registered non-reimbursable coverage in 2006, and subsequently, the Preliminary Coverage Division of the Health Insurance Review and Assessment Service (HIRA) requested the National Evidence-based Healthcare Collaborating Agency to conduct a reassessment of seven autonomic neuropathy test items, including sympathetic skin response test, autonomic neuropathy [orthostatic blood pressure test, Valsalva maneuver, sustained handgrip test, heart rate variability test, and skin conductance test], and quantitative sudomotor axon reflex test for expansion of non-reimbursable coverage into insurance coverage (January 2022). Accordingly, the reassessment of clinical safety and effectiveness was conducted after a review of the reassessment protocol and the integrated subcommittee composition of the orthostatic BP test at the 2nd Health Technology Reassessment Committee in 2022 (February 18, 2022).

Methods In this assessment, a systematic review was performed to assess the clinical safety and effectiveness of the autonomic neuropathy test [orthostatic BP test] (hereinafter referred to as the “orthostatic BP test”) for assessing autonomic nervous system function in patients with or suspected of having autonomic dysfunction. All assessment methods were finalized after the review by the “Integrated subcommittee for autonomic neuropathy tests” (hereinafter referred to as the “subcommittee”) with consideration of the purpose of the assessment. The subcommittee consisted of 11 experts: three from neurology and two each from endocrinology and metabolism, cardiology, psychiatry, and evidence-based medicine. The key question of the assessment was, “Is the orthostatic BP test clinically safe and effective for assessing the autonomic nervous system function in patients with or suspected of having autonomic dysfunction?” Safety was assessed based on test-related adverse events (AEs) or complications, while effectiveness was assessed based on diagnostic accuracy, comparison to comparator, association with the disease, and impact on clinical outcomes. In the systematic review, five Korean databases (KoreaMed, KMbase, DBpia, KISS, KERIS, and ScienceON) and three international databases (Ovid MEDLINE, Ovid EMBASE, and Cochrane Central Register of Controlled Trials) were searched based on the key question. Article selection based on inclusion/exclusion criteria, data extraction, and risk of bias (RoB) assessment were performed independently by two reviewers, and in cases of disagreement, a consensus was reached through discussions with a third person. RoB assessment was performed using Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). In the data analysis, the results for diagnostic accuracy were described qualitatively because the included patients were various and quantitative synthesis was impossible. For comparison with the head-up tilt table test, the results were also qualitatively described since quantitative synthesis was not possible. With respect to association with the disease, trends in the results were identified based on a forest plot using odds ratios for case-control studies (comparison between group with and without disease or comparison between different disease groups), presence/absence of autonomic neuropathy, disease subtypes, and disease severity.

Results A total of 22 articles (8 diagnostic test accuracy studies, 3 diagnostic cohort studies, and 11 case-control studies) were included in the systematic review. In the RoB assessment, there were “high” in the patient selection and concerns about applicability domains with 31.8% and 22.7%, respectively, and “uncertain” in the index test and concerns about applicability domains with 27.3% and 40.9%, respectively. There were “high” in 40.9% and “uncertain” in 50.0% in the reference standard, while it was “high” in 13.6% in the flow and timing.

Safety Safety of the orthostatic BP test was assessed based on test-related AEs and complications, but no articles reporting on these factors. The articles reviewed for the safety assessment of orthostatic BP tests reported that orthostatic BP tests are a safe technology with no known contraindications, but they require assistance to maintain a standing posture during the test in patients with unstable standing posture or a risk of falls and supervision by qualified personnel who have been trained to handle safety issues or clinical events.

Effectiveness The effectiveness outcomes were classified into diagnostic accuracy; comparisons of diagnostic accuracy and results between the orthostatic BP test and head-up tilt table test; and association with the disease. Meanwhile, no articles reported on the impact on clinical outcomes. Regarding the diagnostic accuracy of the orthostatic BP test for autonomic neuropathy and orthostatic hypotension, eight articles reported an overall sensitivity of 0.20–0.69 and specificity of 0.66–1.00. The diagnostic accuracy for diagnosing specific disease was sensitivity of 0.1-0.53 and specificity of 0.84-1.0 for autonomic neuropathy(n=4), and sensitivity of 0.21-0.61 and specificity of 0.66-1.0 for orthostatic hypotension(n=3). For the differential diagnosis between Parkinson's disease and multiple system atrophy (MSA)-parkinsonian subtype (n=1), sensitivity and specificity were reported to be 0.64 and 0.66, respectively. Three studies compared orthostatic BP test and head-up tilt table test, including those on diagnostic accuracy for postural orthostatic tachycardia syndrome (n=1) and comparison of BP measures in syncope patients. One study reported that the diagnostic accuracy was similar between orthostatic BP test and head-up tilt table test when diagnosing postural orthostatic tachycardia syndrome in patients with orthostatic intolerance, while another study reported that orthostatic BP test has just as much potential as head-up tilt table test as a diagnostic test for syncope. The remaining one study compared changes in HR, systolic BP, and diastolic BP between orthostatic BP test and head-up tilt table test according to the type of syncope (cardioinhibitory and vasovagal), in which, orthostatic BP test showed no difference according to the type of syncope. Regarding association with the disease, 11 articles reported on comparisons of the rates of abnormal orthostatic BP test results by presence/absence of autonomic dysfunction, case-control groups, disease subtypes, and presence/absence of disease symptoms. The results showed no significant increase or difference in any particular disease. The study populations varied and included patients with diabetes, sickle cell anemia, chronic renal failure, impaired glucose tolerance, Parkinson's disease, and MSA. In studies that compared the rates of abnormal orthostatic BP test results according to the presence/absence of autonomic neuropathy (n=4), the group with autonomic neuropathy showed significantly higher or no significant difference in the rates of abnormal orthostatic BP test results. In studies on differentiation between Parkinson’s disease and MSA (n=2), the group with MSA showed a significant higher tendency in the rates of abnormal orthostatic BP test results, but no significant differences were found in other diseases. Concerning disease subtypes, the rates of abnormal orthostatic BP test results tended to be higher in particular subtypes or showed no difference according to subtypes. In one study on the association with disease severity, the group with more severe autonomic neuropathy showed significantly higher rates of abnormal orthostatic BP test results. Accordingly, the subcommittee stated that while the orthostatic BP test is not widely used in clinical practice, it can be a helpful test for diagnosing early hypotension. However, it is known to have poorer sensitivity than the head-up tilt table test. The observed low sensitivity of orthostatic BP test for diagnosing autonomic neuropathy in some studies can be attributed to the fact that orthostatic BP test results would only appear abnormal in cases with severe damage to the autonomic nervous system. The association was also confirmed between patients with MSA or diabetes who show autonomic dysfunction in the early stage of the disease and the abnormal rate of orthostatic BP test. And the ability of the orthostatic BP test and head-up tilt table test to diagnose syncope was reported to be similar, so the orthostatic BP test was judged to have clinical effectiveness. Additionally, the subcommittee also stated that using beat-to-beat BP monitoring in clinical practice can increase the accuracy of the test and that orthostatic BP test is included in syncope assessment process in various guidelines.

Conclusions Based on the current assessment results, the subcommittee presented the following recommendations. Orthostatic BP test was determined to be a safe technology with no known contraindications when used for the purpose of assessing autonomic nervous system function in patients with or suspected of having autonomic dysfunction, but it requires assistance to maintain a standing posture during the test in patients with unstable standing posture or risk of falls and supervision by qualified personnel who have been trained to handle safety issues or clinical events. Varying expert opinions on the effectiveness of orthostatic BP test included: i) Orthostatic BP test is a technology that can be helpful for clinical diagnosis with acceptable level of diagnostic accuracy, considering that clinical diagnosis is based on combination of various autonomic nerve system tests; ii) this technology has limited evidence for making any conclusions about its effectiveness; iii) Orthostatic BP requires regulations regarding testing personnel and process, as it lacks evidence due to absence of standardization of test methods; and iv) despite being a useful test for clinical diagnosis, it is a technology that requires more study results, including composite indicators, since it lacks evidence as a stand-alone test. During the first session of the Committee of Health Technology Reassessment in 2023 (January 13, 2023), the following determinations and grade of recommendation were made on orthostatic BP test based on a review of the subcommittee findings, including various expert opinions. Orthostatic BP test was determined to be a safe technology with no known contraindications, but it requires assistance to maintain a standing posture during the test in patients with unstable standing posture or risk of falls, and supervision by qualified personnel who have been trained to handle safety issues or clinical events. Regarding effectiveness, this technology can be helpful for clinical diagnosis considering that assessment of autonomic dysfunction is based on combination of various autonomic nervous system tests in clinical practice. The Health Technology Reassessment Committee comprehensively considered the evidence of clinical safety and effectiveness and other evaluation items, and decided to recommend a “conditional recommendation” when orthostatic BP test is used for the purpose of evaluating autonomic nervous system function in clinical situations in Korea.
