자율신경계이상검사(발살바법)

Abstract

평가배경 발살바법(Valsalva maneuver)은 자율신경계 기능정도를 평가하기 위해 40mmHg의 압력으로 15초동안 압력계를 불다가 이완시키면서 심박동 및 혈압의 변화를 측정하는 검사이다. 발살바법은 2006년에 등재 비급여로 고시되었으며, 이후 건강보험심사평가원 예비급여부에서 비급여의 급여화 추진을 위하여 본 검사를 포함한 자율신경계 이상검사 7개 항목(교감신경피부반응검사, 자율신경계 이상검사(기립성 혈압검사, 발살바법, 지속적 근긴장에 따른 혈압검사, 심박변이도검사, 피부전도반응검사), 정량적 발한 축삭 반사검사)에 대한 재평가를 한국보건의료연구원에 의뢰하였다(2022.1.). 이에 따라 2022년 제2차 의료기술재평가위원회(2022.2.18.)에서는 해당 기술들의 평가계획서 및 통합 소위원회 구성안을 심의한 후 임상적 안전성 및 효과성을 평가하였다.

평가방법 이번 평가는 자율신경계 기능장애 의심환자 및 질환자를 대상으로 자율신경계 이상검사(발살바법)(이하 ‘발살바법’)이 임상적으로 안전하고 효과적인지 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “통합 자율신경계 이상검사 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 신경과 3인, 내분비대사내과 2인, 순환기내과 2인, 정신건강의학과 2인, 근거기반의학 2인의 전문가 총 11인으로 구성하였다. 평가의 핵심질문은 ‘자율신경계 기능장애 의심환자 및 질환자에서 자율신경계 기능을 평가하기 위한 발살바법은 임상적으로 안전하고 안전하고 효과적인가?’이다. 안전성은 검사 관련 이상반응 또는 부작용, 효과성은 진단정확성, 질환과의 관련성, 의료결과의 영향으로 평가하였다. 체계적 문헌고찰은 위의 핵심질문을 토대로 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials) 및 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, 과학기술정보통합서비스)를 이용하여 문헌검색하였으며, 문헌 선정기준 및 배제기준 적용을 통한 문헌선택과 자료추출, 비뚤림위험 평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 문헌의 비뚤림위험 평가는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)를 사용하였다. 자료분석에서 진단정확성은 대상환자 및 진단기준이 다양하고 문헌별 2×2 표 값이 추출되지 않아 질적으로 기술하였으며, 질환과의 관련성은 환자대조군(환자군과 해당 질환이 없는 군으로 구분하여 분석한 경우), 질환대조군(각기 다른 질환군끼리 비교한 경우), 자율신경계병증 유무, 질환 세부 유형별, 질환 중증도별 오즈비를 이용한 숲그림으로 결과의 경향성을 확인하였다.

평가결과 체계적 문헌고찰 결과, 선택문헌은 총 46편(진단법평가연구 10편, 환자대조군 연구 36편)으로 이 중 국내 연구는 4편이었다. 비뚤림위험 평가결과 환자 선택영역은 비뚤림위험이 56.5%에서 ‘높음’, 중재검사영역은 비뚤림위험이 50.0%에서 ‘불확실’, 참고표준검사영역은 비뚤림위험이 13.0%에서 ‘불확실’, 연구진행과 시점영역은 비뚤림위험이 13%에서 ‘높음’으로 평가하였다.

안전성 발살바법의 안전성은 검사 관련 이상반응 및 합병증으로 평가하고자 하였으나, 이를 보고한 문헌은 없었다. 발살바법의 안전성을 확인하기 위해 참고문헌을 확인한 결과 발살바법은 비교적 안전한 기술로 모든 환자에서 시행할 수 있으나 망막병증 및 안구 내 수정체 이식을 수행한 환자는 발살법으로 인해 안압 및 복강내압이 상승할 수 있으므로 검사를 피해야 한다고 보고하였다. 또한 발살바법으로 인한 실신, 흉통, 부정맥이 나타날 수 있어 관상동맥질환, 심장판막질환 및 선천성 심장질환이 있는 환자에서 이 검사를 할 때 주의가 필요하다고 하였다.

효과성 발살바법의 효과성은 진단정확성과 질환과의 관련성으로 구분하여 제시하였으며, 의료결과에의 영향을 보고한 연구는 없었다. 발살바법의 진단정확성은 10편에서 민감도 0.54 ~ 1.00, 특이도 0.36 ~ 0.98이었다. 자율신경기능장애 진단 연구는 총 6편으로 당뇨 환자나 병원에 입원 중이 노인 환자 대상으로 민감도 0.3 ~ 0.76, 특이도 0.35 ~ 0.98이었다. 질환 구분을 위한 연구는 총 4편으로 민감도 0.64 ~ 1.00, 특이도 0.54 ~ 0.92였으며, 다계통 위축증(파킨슨 타입)과 파킨슨 병, 마카도 조셉병, 당뇨병 환자 등이 포함되었다. 이 중 다계통 위축증(파킨스 타입)과 파킨슨 병 구분에 대한 진단정확도가 가장 높았다(민감도 0.91, 특이도 0.92). 질환과의 관련성은 자율신경계 기능장애 유무별, 환자 및 질환 대조군별, 질환 세부 유형별, 질환 증상 유무별, 질환중증도 간 발살바법의 비정상 비율을 비교하였을 때 특정 질환에서 유의하게 높거나 유의한 차이가 없었다. 연구대상자는 만성 일차성 자율신경계부전, 복막투석환자, 혈압패턴 비강하군, 당뇨병 환자, 다계통 위축증, 파킨슨 병등 다양하였다. 자율신경계 기능장애 유무별로는 자율 신경계 기능장애가 있는 그룹에서 발살바법의 비정상 비율이 유의하게 높았으며, 환자 및 질환 대조군별로 환자나 특정 질환에서 발살바법의 비정상 비율이 높거나 군간 유사하였다. 질환 세부 유형별는 특정 유형에서 발살바법의 비정상 비율이 더 높은 경향을 보이거나 세부 유형별 차이가 없었다. 질환 증상 유무별로는 말초신경병증이나 저혈당 증상, ST 분절 하강이 있는 군이 없는 군에 비해 발살바비의 비정상 비율이 더 높았고 질환 중증도가 심할수록 발살바비의 비정상 비율이 높은 경향을 보였다. 이에 소위원회에서는 일부 연구에서 발살바법의 낮은 민감도는 임상에서 여러 자율신경계 검사를 종합해서 판단하는 점을 고려했을 때 현재 문헌에서 제시된 진단정확성 수준은 수용가능하고, 발살바법과 자율신경계 기능장애 유무나 질환 중증도 간의 관련성이 확인되어 발살바법이 임상진단에 도움이 되는 기술이라는 의견, 임상진단에 도움이 되나 효과성에 대한 결론을 내리기에 근거가 제한적이라는 의견 및 단독검사로는 근거가 부족한 상태로 통합지표를 포함한 더 많은 연구결과가 필요하다는 의견 등 이견이 있었다. 추가적으로 임상에서는 박동간 혈압감시기(beat to beat blood pressure monitoring)를 이용할 때 검사의 정확도가 더 높아질 수 있으며, 가이드라인 및 합의문에 자율신경계 기능 평가방법으로 발살바법이 포함되어있다는 의견이 있었다.

결론 통합 자율신경계 이상검사 소위원회는 평가 결과에 근거하여 다음과 같이 제언하였다. 발살바법은 자율신경계 기능장애 의심환자 및 질환자를 대상으로 자율신경계 기능 평가목적으로 사용시 비교적 안전한 기술로 모든 환자에서 시행할 수 있으나 망막병증 및 안구 내 수정체 이식 환자에서는 발살바법으로 인해 안압 및 복강내압이 상승할 수 있으므로 이 검사를 피해야 하며, 관상동맥질환, 심장판막질환 및 선천성 심장질환이 있는 환자는 발살바법으로 인한 실신, 흉통, 부정맥이 보고되고 있어 이 검사를 할 때 주의가 필요하다고 판단하였다. 발살바법의 효과성은 i) 자율신경계 기능장애 질환은 여러 검사들을 종합해서 판단하는 점을 고려했을 때 임상진단에 도움이 되는 기술, ii) 임상진단에 도움이 되나 효과성에 대한 결론을 내리기에 근거가 제한적인 기술, iii) 유용한 검사이나 단독검사로는 근거가 부족한 상태로 통합 지표를 포함한 더 많은 연구결과가 필요한 기술, iv) 발살바법의 표준화가 필요하며, 특히 발살바가 환자의 협조에 의해 진행되는 검사라 충분한 노력이 반영된 것인지 검증할 수 있는 기준이 필요한 기술이라는 의견 등 이견이 있었다. 2023년 제1차 의료기술재평가위원회(2023.1.13.)에서는 발살바법에 대한 여러 전문가 간 이견을 포함한 소위원회 결론을 검토하여 다음과 같이 심의 의결하고 권고등급을 결정하였다. 발살바법은 비교적 안전한 기술로 모든 환자에서 시행할 수 있으나 망막병증 및 안구 내 수정체 이식환자에서는 발살바법으로 인해 안압 및 복강내압이 상승할 수 있으므로 이 검사를 피해야 하며, 관상동맥질환, 심장판막질환 및 선천성 심장질환이 있는 환자는 발살바법으로 인한 실신, 흉통 및 부정맥이 보고되고 있어 이 검사를 할 때 주의가 필요하다고 판단하였다. 효과성은 임상에서 자율신경계 기능장애 평가시 여러 자율신경계 검사들을 종합해서 판단하는 점을 고려했을 때 임상진단에 도움이 되는 기술로 판단하였다. 이에 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 자율신경계 기능장애 의심환자 및 질환자를 대상으로 자율신경계 기능 평가목적으로 사용시 발살바법을 ‘조건부 권고함’으로 심의하였다.

Background The Valsalva maneuver is a test used to assess autonomic nervous system function in which the subject exhales into a manometer and maintains a pressure of 40 mmHg for 15 seconds, followed by measurement of changes in heart rate and blood pressure as it is inhaled. Valsalva maneuver was listed as a registered non-reimbursable coverage in 2006, and subsequently, the Preliminary Coverage Division of the Health Insurance Review and Assessment Service (HIRA) requested the National Evidence-based Healthcare Collaborating Agency to conduct a reassessment of seven autonomic neuropathy test items, including sympathetic skin response test, autonomic neuropathy [orthostatic blood pressure test, Valsalva maneuver, sustained handgrip test, heart rate variability test, and skin conductance test], and quantitative sudomotor axon reflex test for expansion of non-reimbursable coverage into insurance coverage item (January 2022). Accordingly, the reassessment of clinical safety and effectiveness was conducted after a review of the reassessment protocol and the integrated subcommittee composition of the autonomic neuropathy test [Valsalva maneuver] at the 2nd Health Technology Reassessment Committee in 2022 (February 18, 2022).

Methods In this assessment, a systematic review was performed to assess the clinical safety and effectiveness of the autonomic neuropathy test [Valsalva maneuver] (hereinafter referred to as “VM”) for assessing autonomic nervous system function in patients with or suspected of having autonomic dysfunction. All assessment methods were finalized after the review by the “Integrated subcommittee for autonomic neuropathy tests” (hereinafter referred to as the “subcommittee”) while considering the assessment's purpose. The subcommittee consisted of 11 experts: three from neurology and two each from endocrinology and metabolism, cardiology, psychiatry, and evidence-based medicine. The key question of the assessment was, “Is VM clinically safe and effective for assessing the autonomic nervous system function in patients with or suspected of having autonomic dysfunction?” Safety was assessed based on test-related adverse events (AEs) or complications, while effectiveness was assessed based on diagnostic accuracy, association with the disease, and impact on clinical outcomes. For the systematic review, three international databases (Ovid MEDLINE, Ovid EMBASE, and Cochrane Central Register of Controlled Trials) and five Korean databases (KoreaMed, KMbase, KISS, KERIS, and ScienceON) were searched based on the key question. Article selection based on inclusion/exclusion criteria, data extraction, and risk of bias (RoB) assessment were performed independently by two reviewers, and in cases of disagreement, a consensus was reached through discussions with a third person. RoB assessment was performed using Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). In the data analysis, the results for diagnostic accuracy were described qualitatively because the included patients were various and quantitative synthesis was impossible. Regarding the association with the disease, trends in the results were identified based on a forest plot using odds ratios for the case-control studies (comparison between group with and without disease or comparison between different disease groups), presence/absence of autonomic neuropathy, disease subtypes, and disease severity.

Results A total of 46 articles (10 diagnostic test accuracy studies and 36 case-control studies) were included in the systematic review, among which four studies were conducted in Korea. In the RoB assessment, there were “high” in 56.6% in the patient selection domain; “uncertain” in 50.0% in the index test domain; “uncertain” in 13.0% in the reference standard domain; and “high” in 13.0% in the flow and timing domain.

Safety The safety of VM was assessed based on test-related AEs and complications, but no articles reported on these factors. The articles reviewed for the safety assessment of VM reported that it is a relatively safe technology that can be applied to all patients, but patients with retinopathy or intraocular lens implantation should avoid VM as it may cause an increase in intraocular and intra-abdominal pressure. Also VM may also cause syncope, chest pain, and arrhythmias, and thus, caution is necessary for patients with pre-existing coronary artery disease, valvular disease, or congenital heart disease.

Effectiveness The effectiveness outcomes were classified into diagnostic accuracy and association with the disease, while no articles reported on the impact on clinical outcomes. With respect to the diagnostic accuracy of VM, a sensitivity of 0.54–1.00 and specificity of 0.36–0.98 were reported in 10 articles. In six studies on the diagnosis of autonomic dysfunction, sensitivity of 0.3–0.76 and specificity of 0.35–0.98 were reported for diabetic patients or elderly inpatients. Sensitivity of 0.64–1.00 and specificity of 0.54–0.92 were reported in four studies on differentiation between diseases, which included multiple system atrophy (MSA) [with predominant parkinsonism; MSA-P], Parkinson’s disease, Machado-Joseph disease, and diabetes. Of these, differentiation between MSA-P and Parkinson’s disease showed the highest diagnostic accuracy, with a sensitivity of 0.91 and specificity of 0.92. With respect to association with the disease, comparisons of the rates of abnormal VM results by presence/absence of autonomic dysfunction, case-control groups, disease subtypes, presence/absence of autonomic disease symptoms, and disease severity showed no significant increase or difference in any particular disease. The study populations varied and included patients with chronic primary autonomic failure, non-dipping blood pressure, diabetes, MSA, Parkinson’s disease, and peritoneal dialysis patients. In the comparison by presence/absence of autonomic dysfunction, the group with autonomic dysfunction showed significantly higher rates of abnormal VM results. In comparisons of case-control groups, the rates of abnormal VM results were significantly higher or similar between groups in particular diseases. For disease subtypes, the rates of abnormal VM results tended to be higher in particular diseases or showed no difference according to subtypes. For the presence/absence of disease symptoms, the groups with peripheral neuropathy, hypoglycemia, or ST segment depression showed higher rates of abnormal VM results than the groups without such symptoms. Meanwhile, the rates of abnormal VM results tended to be higher in groups with higher disease severity. Accordingly, the subcommittee opined that although the sensitivity of VM was low in some studies, the level of diagnostic accuracy reported in the currently available literature was acceptable considering that determinations in clinical practice are made based on a combination of various autonomic nerve system tests. Moreover, the subcommittee determined that VM can be helpful for clinical diagnosis, as it exhibited an association between VM and the presence/absence of autonomic dysfunction or disease severity. Conversely, despite being helpful for clinical diagnosis, evidence is limited to draw any conclusions about its effectiveness, and more studies that include composite indicators are needed since evidence as a stand-alone test is lacking. Additionally, the subcommittee also opined that using beat-to-beat BP monitoring in clinical practice can increase the accuracy of the test and that inclusion of VM in autonomic nervous system assessment methods is mentioned in various guidelines and consensus statements.

Conclusions Based on the assessment results, the subcommittee presented the following recommendations The use of VM for the purpose of assessing autonomic nervous system function in patients with or suspected of having autonomic dysfunction was determined to be a relatively safe technology that can be applied to all patients, but patients with retinopathy or intraocular lens implantation should avoid VM as it may cause an increase in intraocular and intra-abdominal pressure. Also VM may also cause syncope, chest pain, and arrhythmias. Therefore, caution is necessary for patients with pre-existing coronary artery disease, valvular disease, or congenital heart disease. Varying opinions on the effectiveness of VM include: i) VM is a technology that can be helpful for clinical diagnosis considering that determination in clinical practice is made based on combination of various autonomic nerve system tests; ii) despite being helpful for clinical diagnosis, this technology has limited evidence for making any conclusions about its effectiveness; iii) despite being a useful test, VM requires more study results, including composite indicators, since it lacks evidence as a stand-alone test; and iv) standardization of VM is necessary, and in particular, this technology also requires standards for testing whether enough effort was reflected in the test since VM is conducted with cooperation from the patient. During the first session of the Committee of Health Technology Reassessment in 2023 (January 13, 2023), the following determinations and grades of recommendation were made on the VM based on a review of the subcommittee findings, including various expert opinions. VM is a relatively safe technology that can be applied to all patients, but patients with retinopathy or intraocular lens implantation should avoid VM as it may cause an increase in intraocular and intra-abdominal pressure. Also, VM may cause syncope, chest pain, and arrhythmias; thus, caution is necessary when this test is used on patients with pe-existing coronary artery disease, valvular disease, or congenital heart disease. Regarding effectiveness, this technology was determined to be helpful for clinical diagnosis, considering that assessment of autonomic dysfunction is based on combination of various autonomic nervous system tests in clinical practice. The Health Technology Reassessment Committee comprehensively considered the evidence of clinical safety and effectiveness and other evaluation items, and decided to recommend a “conditional recommendation” when VM is used for the purpose of evaluating autonomic nervous system function in clinical situations in Korea.
