레이저를 이용한 손발톱진균증 치료술

Abstract

평가 배경 레이저를 이용한 손발톱진균증 치료술은 손발톱진균증 환자에서 레이저를 조사하여 손발톱진균증의 증상을 개선하는 시술이다. 본 평가대상은 2015년 신의료기술평가(보건복지부 고시 제2015-6호)를 받은 이후, 비급여로 등재(보건복지부 고시 제2016-169호)되어 현재 사용되고 있는 의료기술로, 내부 모니터링을 통해 의료기술재평가 주제로 발굴되었다. 2021년 제10차 의료기술재평가위원회(2021.10.15.)에서는 체계적 문헌고찰을 통해 ‘레이저를 이용한 손발톱진균증 치료술’의 안전성 및 효과성을 재평가하며, 동 안건에 대하여 권고등급 결정을 수행하는 것으로 평가계획서 및 소위원회 구성(안)을 심의하였다.

평가 방법 레이저를 이용한 손발톱진균증 치료술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “레이저를 이용한 손발톱진균증 치료술 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 동 평가의 핵심질문은 ‘손발톱 진균증 환자에서 ‘레이저 치료’는 임상적으로 안전하고 효과적인가?’이다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias (RoB)를 사용하였으며, 자료추출은 미리 정해놓은 자료추출 양식을 활용하였고 비뚤림위험 평가와 자료추출은 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석(quantitative analysis) 및 정성적(qualitative review) 분석을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.

평가 결과 레이저를 이용한 손발톱진균증 치료술의 안전성과 효과성은 총 14편(국내 2편, 국외 12편)이었고, 중재군 대상 환자수는 총 145명이었다. 연구유형별로 무작위배정 비교임상연구 13편, 비무작위배정 비교임상연구 1편이다.

안전성 레이저를 이용한 손발톱진균증 치료술의 안전성이 보고된 13편의 문헌의 결과를 바탕으로 레이저 시술 관련 부작용으로 평가하였다. 5편의 문헌에서 레이저 시술과 관련한 통증의 발생을 보고하였는데 주로 경미한 통증이었고, 1편의 문헌에서만 경미한 통증 이외에 중등도 통증의 발생도 보고하였다. 3편의 문헌에서 레이저 시술 수행 시, 일부 환자에서 피부의 열감을 보고하였다. 1편의 문헌에서 레이저 시술군에서 심각한 부작용 6건의 발생을 보고하였으나 상세내역을 보고하지 않았다. 손발톱 밑부분에 혈종 발생 1건도 보고하였다. 대부분의 문헌에서 보고한 부작용 및 이상반응은 경미한 통증, 열감 등이었다. 전반적으로 선택문헌에서 레이저 시술과 관련되어 중등도 이상으로 간주할 만한 부작용 보고는 없어 소위원회는 레이저를 이용한 손발톱진균증 치료술은 안전한 기술로 판단하였다.

효과성 레이저를 이용한 손발톱진균증 치료술의 효과성은 14편의 문헌을 바탕으로 평가하였다. 진균학적 치료율, 임상적 개선(반응, 향상, 중증도의 변화, 정상 손발톱의 성장 등), 재발, 환자 만족도(삶의 질) 등으로 구분하여 평가하였다.

레이저 치료와 경구 항진균제 치료 비교 레이저 치료와 경구 항진균제를 비교한 문헌은 총 3편이었는데, 마지막 추적관찰 시점을 기준으로 진균학적 치료율을 메타분석 한 결과, 두 군 간에 효과 차이가 없었다(Risk ratio 1.17, 95% CI 0.84~1.63, I²=80%). Q-switched 1064nm Nd:YAG 레이저 치료술의 진균학적 치료율을 보고한 1편에서는 레이저 치료술의 진균학적 치료율이 경구 항진균제보다 통계적으로 유의하게 높았다(68% vs. 32%, p<0.001). Long-pulsed 1064nm Nd:YAG 레이저를 사용한 치료술의 효과를 평가한 2편의 문헌에 대하여 8주, 16주, 24주 추적관찰 시점의 자료를 통합하여 진균학적 치료율의 양상을 분석한 결과, 치료 초기에는 경구 항진균제의 효과가 유의하게 높게 나타났으나, 시간이 지날수록 군간 효과 차이가 줄어드는 양상이었고 마지막 24주 시점에는 레이저 치료와 경구 항진균제의 효과 차이가 없었다(24주 시점: Risk ratio 1.01, 95% CI 0.89~1.13, I²=0%). 이는 레이저 치료의 효과가 시간이 지나면서 서서히 나타나기 때문으로 소위원회는 판단하였다.

레이저 치료와 국소 항진균제 치료 비교 레이저 치료와 국소 항진균제를 비교한 총 3편 문헌의 3개월 시점 진균학적 치료율을 메타분석한 결과, 두 군간 효과 차이가 없었다(Risk ratio 1.22, 95% CI 0.35~4.17, I²= 62%).

레이저 병용요법(레이저 + 항진균제)와 항진균제 단독요법 비교 항진균제 요법에 레이저 치료술을 추가하여 실시한 경우의 효과를 평가한 5편 문헌(경구제에 추가 1편, 국소제에 추가 4편) 모두에서 레이저 병용요법의 진균학적 치료율이 단독요법보다 우수한 효과를 보고하였는데, 이 중 3편에서는 통계적으로 유의한 차이가 있음을 보고하였다. 임상적 반응률을 보고한 4편의 문헌 중, 3편에서는 레이저 병용요법이 항진균제 단독요법보다 유의하게 우수한 효과를 보고하였고, 나머지 1편의 연구에서는 동일한 효과를 보고하였다. 손발톱진균증의 중증도 변화를 보고한 2편 문헌 중, 1편의 문헌에서는 레이저 병용요법이 항진균제 단독요법보다 통계적으로 유의하게 중증도가 개선되었다고 보고하였다. 1편의 문헌에서 손발톱 성장변화를 측정한 결과, 레이저 병용요법의 무병변 영역변화가 국소 항진균제 단독요법보다 더 큰 것으로 나타났으나, 통계적으로 유의한 차이는 없었다. 1편의 문헌에서 레이저 병용요법군의 환자 만족도가 국소 항진균제 단독요법군보다 통계적으로 유의하게 높았다.

레이저 치료와 무처치(sham 포함)의 비교 진균학적 치료율을 보고한 문헌은 총 4편 중, 3편의 문헌에서 레이저군의 진균학적 치료율이 무처치군에 비해 높았으나 군간 통계적으로 유의한 차이가 없었다. 1편에서는 레이저군의 임상적 반응률이 무처치군보다 높았으나, 군간 통계적으로 유의한 차이가 없었다. 중증도 변화를 보고한 3편 문헌 모두에서 군 간에 통계적으로 유의한 차이가 없었다. 1편의 문헌에서 레이저 치료를 수행한 경우가 그렇지 않은 경우에 비해 통계적으로 유의하게 손발톱이 성장한 것으로 보고하였다. 1편의 문헌에서 레이저군의 재발률이 무처치군보다 높았다고 보고하였으나, 군간 통계적으로 유의한 차이가 없었다.

결론 및 제언 따라서, 문헌적 근거를 토대로 소위원회에서는 다음과 같이 제언하였다. 동 기술의 안전성과 관련하여 레이저를 이용한 손발톱진균증 치료술과 직접적인 관련성이 있는 부작용 및 합병증은 대부분 통증, 작열감 등과 같은 경미한 항목들로 중등도 이상의 부작용 사례가 드문 안전한 기술로 판단하였다. 동 기술의 효과성과 관련하여 진균학적 치료율 및 여러 임상적 지표에서 기존의 항진균제 요법에 레이저 치료술을 추가하는 경우에는 단독 항진균제 요법보다 효과적인 기술로 판단하였다. 항진균제 치료와의 일대일 비교에서는 레이저 치료술의 효과가 우월함을 입증하지는 못하였으나, 군간 효과 차이가 없어, 기존 항진균제 치료법보다 열등하지는 않은 기술로 소위원회는 평가하였다. 평가에 선정된 문헌들의 대상자 수가 적고, 문헌별 환자 상태 및 중증도의 차이가 존재하여 문헌마다 치료율의 범위가 넓었고, 장기 추적관찰한 연구가 없어 선정된 문헌의 결과만으로 결론을 도출하는 것에는 한계가 있다는 의견이었다. 임상 전문가들의 치료경험에 기반하여 경구 항진균제를 사용할 수 없는 환자들에서 제한적 사용에 유용성이 있다고 제언하였다. 2022년 제5차 의료기술재평가위원회(2022.05.13.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “레이저를 이용한 손발톱진균증 치료술”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 손발톱진균증 환자에서 레이저를 이용한 치료술을 수행하는 것에 대해 “조건부 권고함”으로 심의하였다(권고등급: 조건부 권고함).

Background “Laser Therapy for Onychomycosis” is a treatment method for improvement of the symptoms of onychomycosis by laser irradiation for patients with onychomycosis This technology was subjected New Health Technology Assessment (Notice No. 2015-6, Ministry of Health and Welfare) in 2015, and was listed as a non-benefit item for national health insurance coverage (Notice No. 2016-169, Ministry of Health and Welfare); the technology is currently applied in clinical practice. Through internal monitoring, it was determined that health technology reassessment (HTR) was necessary for this technology. At the 10th Health Technology Reassessment Committee (October 15, 2021) in 2021, the assessment protocol and organization of the Sub-committee (plan) were reviewed so that HTR was conducted on the safety and effectiveness of “Laser Therapy for Onychomycosis” to determine the grade of recommendation.

Methods A systematic review of the related literature was conducted for assessment of the safety and effectiveness of laser therapy for onychomycosis. All assessment methods were finalized after review by the Laser Therapy for Onychomycosis Sub-committee (Hereinafter Sub-committee) in consideration of the purpose of the assessment. The key question for this assessment was “Is laser therapy clinically safe and effective for patients with onychomycosis?” In the process of systematic review, based on the above key question, three international and five domestic databases were used for literature search and the articles were selected based on independent screening by two reviewers according to the inclusion/exclusion criteria for the related literature. The Cochrane Risk of Bias (RoB) tool was used for risk of bias assessment of the literature. For data extraction, a predefined format was used. Risk of bias assessment and data extraction were conducted independently by two reviewers, and in case of disagreement, the two reviewers held discussions to draw consensus on the matter. For methods of data analysis, quantitative analysis and qualitative review were applied. The evidence level of the results of systematic review was assessed using the approach of Grading of Recommendations Assessment, Development and Evaluation (GRADE).

Results A total of fourteen articles (two domestic studies, twelve international studies) were selected pertaining to safety and effectiveness of the laser therapy for onychomycosis, and the number of patients that underwent intervention was 145 in total. In terms of type of studies, thirteen articles were based on randomized controlled clinical trials and one article was based on non-randomized controlled studies.

Safety Based on the results of thirteen selected articles on the safety of laser therapy for onychomycosis, adverse events related to the laser treatment were evaluated. Five articles reported on the incidence of laser therapy-induced pain, most of which were mild pain, and only one article reported events of moderate pain in addition to mild pain. In three articles, some patients reported burning sensation in the skin in the process of laser therapy. In one article, six cases of serious adverse events were reported in the laser therapy group, but details of the adverse events were not reported. Also, there was a report of one case of hematoma at the nail bed. Most of the adverse events and reactions reported in the selected articles were mild pain and burning sensation. Overall, there were no reports of adverse events that could be considered as moderate or more severe in terms of their severity in the selected literature; thus, the Sub-committee determined that laser therapy for onychomycosis was a safe technique.

Effectiveness The effectiveness of laser therapy for onychomycosis was assessed based on the results reported in fourteen articles. The assessment was conducted based on categorization into mycological cure rate, improvement in clinical outcomes (clinical response, improvement, change in severity of the condition, growth of clear, unaffected nails, etc.), recurrence, and patient satisfaction (quality of life).

Comparison of laser therapy and oral antifungal agents There were a total of three articles that performed comparative analysis between laser therapy and oral antifungal agents. As a result of meta-analysis of mycological cure rate based on the last timepoint in the follow-up, there was no difference in effectiveness between the two groups (Risk ratio 1.17, 95% CI 0.84~1.63, I²=80%). In one article that presented the mycological cure rate of treatment using a Q-switched 1064nm Nd:YAG laser, the mycological cure rate of laser therapy was statistically significantly higher than that of oral antifungal agents (68% vs. 32%, p<0.001). In two articles that evaluated the effectiveness of laser therapy using a long-pulsed 1064nm Nd:YAG laser, as a result of analyzing the trend in the mycological cure rate through data at Week 8, Week 16, and Week 24 in the follow-up period, the effect of oral antifungal agents was significantly higher in the early stage of treatment, but over time, the difference between the two groups showed a decrease, and at Week 24, the final week, there was no difference in the effectiveness between laser therapy and oral antifungal agents (Week 24: Risk ratio 1.01, 95% CI 0.89~1.13, I²=0%). The Sub-committee reasoned that this was because the effect of laser therapy is presented slowly over time.

Comparison of laser therapy and topical antifungal treatments As a result of meta-analysis of mycological cure rate at 3-month timepoint in a total of three articles comparing laser therapy and topical antifungal treatments, there was no difference in terms of effectiveness between the two groups (Risk ratio 1.22, 95% CI 0.35~4.17, I²= 62%).

Comparison of combination therapy (Laser + Antifungal agents) and monotherapy of antifungal agents In all of the five articles (one article for oral antifungal agent + four articles for topical antifungal treatments) evaluating the effects of additionally applying laser therapy to treatments with oral or topical antifungal agents, the mycological cure rate of the laser combination therapy was more effective than monotherapy, of which three articles reported that the difference in effectiveness was statistically significant. Of the four articles presenting the clinical response rate, three studies reported that laser combination therapy was significantly more effective than antifungals monotherapy, and the other one study reported the same effectiveness between the two. Among the two articles that reported changes in the severity of onychomycosis, one article reported that laser combination therapy was showed a statistically significant improvement in severity compared to monotherapy of antifungal agents. In one article, as a result of measuring nail growth changes, the change in the unaffected area was larger in the laser combination therapy than monotherapy with topical antifungal treatment, but the difference was not statistically significant. In one article, the patient satisfaction was statistically significantly higher in the laser combination therapy group than that of the group treated with topical antifungals alone.

Comparison of laser therapy and controls (including sham) In three out of four articles that presented the outcome in terms of mycological cure rate, the mycological cure rate in the laser therapy group was higher than that of the control group, but there were no statistically significant intergroup differences. In one article, the clinical response rate of the laser therapy group was higher than that of the control group, but the difference between the two groups was not statistically significant. In all three articles that reported changes in severity, there was no statistically significant difference between groups. In one article, statistically significant improvement in terms of nail growth was reported in laser therapy group than their counterparts. One article reported that the recurrence rate was higher in the laser therapy group than that of the control group, but there was no statistically significant intergroup difference.

Conclusion and Suggestions Therefore, based on the evidence from the selected articles, the Sub-committee suggested the following. Regarding the safety of this technology, most of the adverse events and complications directly related to laser therapy for onychomycosis were minor events such as pain and burning sensation, and thus, it was determined that the technology is safe with almost no events of moderate or higher severity. In relation to the effectiveness of this technology, in terms of mycological cure rate and other clinical outcome measures, when laser therapy was added to the conventional treatments with antifungal agents, the combined therapy was determined to be more effective than the conventional monotherapy using antifungal agents. As for one-to-one comparison between laser therapy and antifungal agent treatment, the superiority of the effectiveness of laser therapy was not proven from the literature evidence, but since there was no intergroup difference in terms of effectiveness, the Sub-committee evaluated that laser therapy was not inferior to the existing treatments using antifungal agents. Since the number of patients in the articles selected for the assessment was small, and there were differences in patient conditions and severity depending on the articles, the cure rate varied over a wide range for each article, and there was no study that performed long-term follow-up; thus, the opinions of the Sub-committee was that there was a limitation in drawing a conclusion based on the results reported in the selected articles. Based on the treatment experience of clinical experts, it is suggested that the technology is useful for limited use in patients who are unable to use oral antifungal agents. Based on the results of the review by the Sub-committee, at the 5th Health Technology Reassessment Committee (May 13, 2022) in 2022, the following is the result of the review for “Laser therapy for onychomycosis” in accordance with Article 4, Paragraph 10 of the Management Guidelines for Health Technology Reassessment Project. The review by the Health Technology Reassessment Committee determined the grade of recommendation for using laser therapy for onychomycosis as “conditional recommendation” (Grade of recommendation: conditional recommendation).
