전립선비대증에서 쏘팔메토 추출물(Serenoa repens)의 효과 및 안전성

Abstract

평가 배경 쏘팔메토 추출물(Saw palmetto; 성분명: Serenoa repens)은 톱야자나무의 열매 추출물로, 배뇨증상 개선을 목적으로 시중에서 많이 판매되고 있다. 쏘팔메토 추출물은 언론 및 광고 등에 노출이 많아 국민들의 관련 정보 요구도가 높아 한국보건의료연구원 의료기술평가 국민참여단 보건의료정보 여과활동 검증주제로 발굴되었다. 본 평가의 목적은 전립선비대증 환자에서 쏘팔메토 추출물의 안전성 및 효과성에 대한 과학적 근거검토를 통해 인포데믹스(infodemics)에 대응한 대국민 정보를 제공하는 것이다.

평가 방법 쏘팔메토 추출물(Serenoa repens)의 안전성 및 효과성에 대한 과학적 근거검토를 위해 선행 체계적 문헌고찰 문헌을 검토(Overview of systematic review)하였다. 모든 평가방법, 결론, 대국민 정보문 등은 “쏘팔메토 추출물 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 비뇨의학과 3인, 신장내과 1인, 근거기반의학 1인 등 총 5인으로 구성하였다. 핵심질문은 “전립선비대증 환자에서 쏘팔메토 추출물은 임상적으로 안전하고 효과적인가?”이었고, 보고된 부작용 등을 안전성 지표로 하였고, 전립선 증상점수, 최대 요속, 야뇨, 잔뇨량, 전립선 크기 등을 효과성 지표로 결과를 확인하였다. 체계적 문헌고찰(Systematic Review, SR) 문헌은 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자 간 합의를 통해 최종 논문을 결정하였다. 체계적 문헌고찰 문헌의 방법론적 질평가는 AMSTAR 2 (A Measurement tool to assess systematic reviews) 도구를 사용하여 2명의 평가자가 독립적으로 수행하였고, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 체계적 문헌고찰 문헌에서 보고한 결과를 바탕으로 결론 및 대국민 정보문을 작성하고, 이에 대한 대한비뇨의학회의 의견을 회신받았다.

평가 결과 전립선비대증에서의 쏘팔메토 추출물의 안전성ㆍ효과성을 검토하기 위해 선택된 체계적 문헌고찰 문헌은 총 7편이었다. 각 SR 문헌별로 포함된 문헌은 4~40편이었고, 무작위배정 임상시험연구(Randomized controlled trial, RCT) 연구만을 대상으로 한 SR은 5편이었고, Agbabiaka 등(2009)은 연구설계 제한없이 증례연구까지 포함하였고, Vela-Navarrete 등(2018)은 RCT와 관찰연구를 포함하여 일차 문헌을 평가하였다. 또한, 쏘팔메토의 제품 중 하나인 Permixon의 결과를 보고한 문헌은 3편이었다(Tacklind 등, 2012; Novara 등, 2016; Vela-Navarrete 등, 2018). 각 체계적 문헌고찰의 전반적 신뢰도는 ‘높음’이 1편(Trivisonno 등, 2021)이었고, ‘중등도’ 1편(Russo 등, 2021), ‘낮음’ 4편(Tacklind 등, 2012; Novara 등, 2016; Vela-Navarrete 등, 2018; Cai 등, 2020), ‘매우 낮음’이 1편(Agbabiaka 등, 2009)이었다.

안전성 쏘팔메토 추출물과 위약의 전체 부작용 발생을 보고한 결과, 3편 모두 두 군간 차이가 없었다. 또한, 쏘팔메토 추출물과 알파 차단제(α-blockers, 예. Tamsulosin)의 부작용 발생을 비교한 결과에서도 2편 모두 두 군간 차이가 없었다. 성기능 관련 부작용 발생에서 사정 장애 발생은 쏘팔메토군이 Tamsulosin군에 비해 2편 모두 통계적으로 유의하게 낮았고(Odds ratio (OR) 0.08, 95% CI 0.02~0.26; 쏘팔메토군 발생률 0%, p=0.03), 성욕 감소 발생, 발기부전은 Tamsulosin과 5-알파 환원효소 차단제(5-Alpha reductase inhibitors, 5-ARIs, 예. Finasteride)와 비교한 결과에서 1편에서는 통계적으로 유의하게 발생이 낮았고, 다른 문헌에서는 차이가 없다고 보고하였다. 두통, 어지럼증, 체위성 저혈압 등의 발생은 쏘팔메토군과 위약, Tamsulosin, Finasteride를 비교한 결과에서 두 군간 통계적 유의한 차이를 보고한 문헌은 없었다. 위장 장애, 설사, 오심/구토 등 위장관련 부작용 발생도 쏘팔메토군과 위약, Finasteride를 비교한 결과에서 두 군간 통계적 유의한 차이를 보고한 문헌은 없었다. 무기력증, 피로, 구강 건조, 비염 발생도 쏘팔메토 추출물과 Tamsulosin과 비교한 결과에서 두 군간 통계적으로 차이가 없었다고 보고하였다.

효과성 전립선 증상점수 개선에서 쏘팔메토 추출물을 위약과 비교한 13건의 합성결과 일부(3건) 통계적으로 유의한 결과가 있긴 하였으나 전체적으로 유의미한 개선효과에 대한 근거가 없었고, Tamsulosin과 비교한 23건의 합성한 결과 중 5건에서 Tamsulosin군이 쏘팔메토군에 비해 통계적으로 유의한 개선 효과가 있었고, Finasteride와 비교에서는 4건의 합성결과 모두 통계적으로 유의한 차이가 없었다. 최대 요속은 쏘팔메토 추출물과 위약을 비교한 총 18건 중 6건의 합성결과가 통계적으로 유의하게 쏘팔메토군이 개선효과가 있었다(가중 평균 차이(Weighted mean difference, WMD) 1.16~3.37 ml/sec). 단일 제품 결과를 제외할 경우 두 군의 최대 요속 차이는 1.16~2.41 ml/sec로임상적으로 의미있는 개선효과가 없었고, 쏘팔메토 제품 중 Permixon 단일 제품과 위약군 간의 최대 요속 차이는 2.75~3.37 ml/sec로 통계적ㆍ임상적 개선 효과가 있었다. Tamsulosin (α-blockers)과 Finasteride (5-ARIs) 비교 결과는 통계적으로 유의한 차이가 없었다. 아뇨 개선에서 쏘팔메토 추출물은 위약 대비 통계적으로 유의한 차이가 있었고(WMD –0.31~-0.79회/밤), Tamsulosin군이 Permixion에 비해 야뇨 개선 효과가 있다고 보고하였다(WMD 0.91회/밤). Finasteride와 비교한 결과에서는 두 군간 통계적으로 유의한 차이가 없었다. 전립선 크기 개선에서 쏘팔메토 추출물은 위약 대비 통계적으로 유의한 차이가 없었고, Tamsulosin과의 비교한 문헌 중 1편에서는 쏘팔메토군에 비해 통계적으로 유의한 전립선 크기 감소효과가 있었고(WMD 0.29 cc), 다른 2편에서는 두 군간 통계적으로 유의한 차이가 없었다. Finasteride는 쏘팔메토에 비해 통계적으로 유의한 전립선 크기 개선효과가 있었다(WMD 4.8 cc). 잔뇨량 개선에서 쏘팔메토 추출물은 위약 대비 통계적으로 유의한 차이가 없었고, Tamsulosin과 비교에서 1편은 쏘팔메토군에 비해 Tamsulosin군이 통계적으로 유의한 개선 효과가 있었고(WMD 4.27 ㎖), 다른 1편에서는 두 군간 통계적으로 유의한 차이가 없어 쏘팔메토의 잔뇨량 개선 효과를 확인할 수 없었다. 삶의 질은 2~6개월 시점에서는 위약에 비해 쏘팔메토 단독군에서 통계적으로 유의하게 삶의 질이 개선되었고(WMD –0.15점), 12~17개월 시점에서는 두 군간 통계적으로 유의한 차이가 없었다. 쏘팔메토 복합물은 위약과 차이가 없었으며, Tamsulosin과 비교에서도 삶의 질은 두 군간 통계적으로 유의한 차이가 없었다. 쏘팔메토와 Tamsulosin 군간 전립선 특이항원(Prostate Specific Antigen, PSA) 비교한 결과 두 군간 차이가 없었고, 5-ARIs군은 Permixon군에 비해 통계적으로 유의하게 PSA 수치가 감소하였다고 보고하였다(WMD 0.97 ng/mL). 증상개선에 대해 환자 자가보고와 임상의 평가결과를 보고한 문헌은 1편이었다. Tacklind 등(2012)은 쏘팔메토와 위약 두 군간 임상의의 증상개선 평가에는 차이가 없었고, 환자들의 자가보고 결과는 쏘팔메토군이 위약에 비해 통계적으로 유의하게 증상이 개선되었다고 보고하였다(1.83배).

결론 및 제언 선행 체계적 문헌고찰 문헌들을 검토한 결과 전립선비대증에서 쏘팔메토 추출물복용과 관련하여 보고된 안전성 지표에는 사정 장애, 발기부전 등 성기능 관련 부작용과 두통, 어지럼증 및 설사, 위장 장애 등 위장관련 부작용 등이 있었고, 대부분 경미한 부작용으로 회복가능하고, 심각한 부작용 발생이 없었다. 전립선 비대증 환자에서 쏘팔메토 추출물은 위약과 비교시 최대 요속, 야뇨횟수에서 일부 개선효과를 확인할 수 있었으나 전립선 증상점수, 전립선 크기, 잔뇨량 등 대부분의 결과에서 개선효과가 없었다(요속: 18건 중 6건, WMD 1.16~3.37ml/sec 요속 증가, 야뇨: 4건 중 4건, WMD 0.31~0.79회/일 야뇨 횟수 감소). 또한, 기존 전립선비대증 치료약물인 알파 차단제(예. Tamsulosin) 및 5-알파 환원효소 차단제(예. Finasteride)와 비교에서도 야뇨 횟수감소에서는 알파 차단제가 쏘팔메토보다 더 효과적이었고(WMD 0.91회/일 감소), 전립선 크기감소는 5-알파 환원효소 차단제가 쏘팔메토보다 더 효과적이었다(WMD 0.29 cc). 이외 전립선 증상 점수, 최대 요속, 잔뇨량 등은 전립선 치료약물이 쏘팔메토 추출물보다 더 효과적이라는 결과와 효과의 차이가 없다는 결과들이 혼재하였다. 쏘팔메토 추출물 소위원회에서는 총 7편의 체계적 문헌고찰 문헌(일차문헌 4~40편 포함)을 검토한 결과, 전립선비대증에서 쏘팔메토 추출물은 안전성 측면에서는 큰 우려가 없으나 전립선비대증 개선 효과를 뒷받침할만한 근거는 없다고 평가하였다. 2022년 제3차 의료기술재평가위원회(2022.3.11.)에서는 소위원회 검토 결과와 대국민 정보문을 논의하여 의료기술재평가사업 관리지침 제4조 제10항에 의거 “쏘팔메토 추출물”에 대해 소위원회 결론 및 대국민 정보문에 대해 원안대로 심의 의결하였다.

Background Saw palmetto extract (generic name: Serenoa repens) is an extract from the berries of palm-like tree, which is sold commercially for the purpose of improving urinary symptoms. Based on the high public demand for information related to saw palmetto extract owing to high exposure through mass media and advertisements, saw palmetto extract was identified as a topic of public health information filtering activity for public involvement in health technology assessment by the National Evidence-based Healthcare Collaborating Agency. The objective of this assessment is to provide public information in response to infodemics through a review of the scientific evidence for the safety and effectiveness of saw palmetto extract in patients with benign prostatic hyperplasia (BPH).

Methods Articles in previous systematic reviews (SRs) were reviewed (overview of systematic review) performed to review the scientific evidence for the safety and effectiveness of saw palmetto extract. All assessment methods, conclusions, and public information were confirmed through discussions by the “Saw palmetto extract subcommittee” (hereinafter referred to as “subcommittee”). The subcommittee was composed of five experts – three from urology and one each from nephrology and evidence-based medicine. The key question was “Is saw palmetto extract clinically safe and effective for patients with benign prostatic hyperplasia?” Reported adverse events (AEs) were used as the safety indicator, while International Prostate Symptom Score (IPSS), maximum flow rate, nocturnal enuresis, residual urine volume, and prostate volume were used effectiveness indicators. The articles for the SR were searched in three international and five domestic databases. Two assessors independently selected the articles in accordance with the inclusion and exclusion criteria. In the event of any disagreement, the assessors reached a consensus to select the final articles. Two assessors independently performed methodological quality assessment of SR using AMSTAR 2 (a measurement tool to assess systematic reviews). In the event of any disagreement, the assessors reached a consensus to derive consistent results. Based on the results reported in SR, the conclusions and public information were drafted, for which, feedback from the Korean Urological Association was received.

Results A total of seven SRs were selected for the review of safety and effectiveness of saw palmetto extract in patients with BPH. The number of articles included in each SR ranged between 4 and 40 articles, while there were five SRs that included on randomized controlled trials (RCTs). Agbabiaka et al. (2009) had no limitation on study design and included even case studies, while Vela-Navarrete et al. (2018) assessed primary sources with inclusion of RCTs and observational studies. In addition, there were three articles that reported on outcomes related to Permixon, a saw palmetto product (Tacklind et al., 2012; Novara et al., 2016; Vela-Navarrete et al., 2018). With respect to the overall reliability of each SR, the results showed one “high” (Trivisonno et al., 2021), one “moderate” (Russo et al., 2021), four “low” (Tacklind et al., 2012; Novara et al., 2016; Vela-Navarrete et al., 2018; Cai et al., 2020), and one “very low” (Agbabiaka et al., 2009).

Safety Three articles on overall AEs of saw palmetto extract and placebo reported no difference between the two groups. In addition, two articles on AEs of saw palmetto extract and α-blockers (e.g., Tamsulosin) also reported no difference between the two groups. In two articles on AEs related to sexual function, incidence of such AEs was statistically significantly lower in the saw palmetto group than the Tamsulosin group in both articles (Odds ratio (OR): 0.08; 95% CI: 0.02~0.26; Incidence in saw palmetto group: 0%; p=0.03). With respect to reduced libido and erectile dysfunction, statistically significantly lower incidence, as compared to Tamsulosin and 5-alpha reductase inhibitors (5-ARIs; e.g., Finasteride), was reported in one article, whereas other articles reported no difference. With respect to headache, dizziness, and orthostatic hypotension, there were no articles that reported statistically significant difference between saw palmetto group and placebo, Tamsulosin, or Finasteride group. Moreover, articles that compared incidence of gastrointestinal disorder, diarrhea, and nausea/vomiting also reported no statistically significant difference between saw palmetto and placebo or Finasteride group. Articles that compared lethargy, fatigue, dry mouth, and rhinitis also reported no statistically significant difference between saw palmetto and Tamsulosin groups.

Effectiveness Synthesis of 13 articles that compared saw palmetto extract and placebo for improvement in IPSS showed that statistically significant results were reported in some articles (n=3), but overall, evidence of significant improvement effect could not be found. In the synthesis of 23 articles that compared saw palmetto extract to Tamsulosin, five articles reported statistically significant improvement effect in Tamsulosin group, as compared to saw palmetto. Meanwhile, synthesis of four articles that compared saw palmetto and Finasteride showed no statistically significant difference between the groups. Among 18 articles that compared saw palmetto extract and placebo for maximum flow rate, synthesis results of six articles showed statistically significant improvement in saw palmetto group (weighted mean difference (WMD): 1.16–3.37 ml/sec). Excluding single product results , the difference in maximum flow rate between the two groups was 1.16–2.41 ml/sec, indicating no clinically significant improvement effect. Among the saw palmetto products, Permixon single product showed a statistically/clinically significant improvement effects with difference of 2.75–3.37 ml/sec in maximum flow rate, as compared to placebo. In the comparison to Tamsulosin (α-blockers) and Finasteride (5-ARIs), there were no statistically significant differences. With respect to improvement in nocturnal enuresis, saw palmetto extract showed a statistically significant difference (WMD: -0.31 – -0.79 times/night), as compared to placebo. Moreover, Tamsulosin group was reported to show greater improvement in nocturnal enuresis than Permixion group (WMD: 0.91 times/night). Comparison to Finasteride showed no statistically significant difference between the two groups. With respect to improvement in prostate volume, saw palmetto extract showed no statistically significant difference, as compared to placebo. Among the articles that compared saw palmetto extract to Tamsulosin, one article reported statistically significant reduction in prostate volume in Tamsulosin group, as compared to saw palmetto group (WMD: 0.29 cc), while two other articles reported no statistically significant difference between the two groups. Meanwhile, Finasteride showed statistically significant improvement in prostate volume, as compared to saw palmetto (WMD: 4.8 cc). With respect to improvement in residual urine volume, saw palmetto extract showed no statistically significant difference, as compared to placebo. Among the articles that compared saw palmetto extract to Tamsulosin, one article reported statistically significant improvement in Tamsulosin group, as compared to saw palmetto group (WMD: 4.27 ㎖), while one other article reported no statistically significant difference between the two groups. Effectiveness of saw palmetto in improving residual urine volume could not be found. With respect to quality of life (QoL), saw palmetto alone group showed statistically significant improvement in QoL at 2-6 months, as compared to placebo (WMD: –0.15 points), whereas there was no statistically significant difference in QoL between the two groups at 12–17 months. Saw palmetto complex showed no difference, as compared to placebo, while comparison with Tamsulosin also show no statistically significant difference in QoL between the two groups. Comparison of prostate specific antigen (PSA) between saw palmetto and Tamsulosin groups showed no difference between the two groups, whereas 5-ARI group showed statistically significant decrease in PSA level, as compared to Permixon group (WMD: 0.97 ng/mL). There was one article that reported on patient-reported and clinician-assessed improvement in symptoms. Tacklind et al. (2012) reported no difference in clinician-assessed outcomes between saw palmetto and placebo groups, but significant improvement in symptoms in saw palmetto group, as compared to placebo group, in patient-reported outcomes (1.83 times).

Conclusions and Recommendations Overview of SRs showed that safety indicators reported in relation to taking saw palmetto extract for BPH included AEs related to sexual function (i.e., ejaculatory disturbance, erectile dysfunction), headache, dizziness, and gastrointestinal AEs (i.e., diarrhea, gastrointestinal disorder). However, most were mild AEs, from which recovery was possible, while there were no serious AEs reported. In patients with BPH, saw palmetto extract showed some improvement effect on maximum flow rate and frequency of nocturnal enuresis, as compared to placebo (increase in flow rate: 6 out of 18 articles reported WMD of 1.16–3.37 ml/sec; reduced frequency of nocturnal enuresis: 4 out of 4 articles reported WMD of 0.31–0.79 times/day). However, saw palmetto extract showed no improvement effect on IPSS, prostate volume, and residual urine volume in most results. Moreover, comparisons to α-blockers (e.g., Tamsulosin) and 5-ARIs (e.g., Finasteride), which are conventional drugs for BPH, showed that α-blockers were more effective than saw palmetto for reducing the frequency of nocturnal enuresis (WMD: 0.91 times/day) and 5-ARIs were most effective than saw palmetto in reducing prostate volume (WMD: 0.29 cc). With respect to other indicators, such as IPSS, maximum flow rate, and residual urine volume, the results were mixed with some indicating that therapeutic drugs for BPH were more effective than saw palmetto extract or that there were no differences in effectiveness. Based on the review of seven SRs (including 4-40 primary sources), the subcommittee concluded that using saw palmetto extract for BPH has no significant safety concerns, but that there is no evidence to support its effectiveness for improving BPH. During the third session of the Committee of Health Technology Reassessment on March 11, 2022, subcommittee review findings and public information were discussed. Accordingly, the committee reviewed and deliberated on the original plan for the subcommittee conclusions and public information regarding “saw palmetto extract” in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project.
