Abstract

평가 배경 비침습적 압력요류검사[음경커프이용](Non-invasive Pressure Flow Study using Penile Cuff, PCT)은 하부요로증상(Lower Urinary Track Symptoms, LUTS) 환자 중 방광출구폐색(Bladder Outlet Obstruction, BOO)이 의심되는 남성을 대상으로 커프를 이용하여 방광압력과 요속을 측정하여 방광출구폐색 여부를 진단하는 기술이다. 본 기술은 2013년 제3차 신의료기술평가위원회(2013.3.22.)에서 안전성 및 유효성이 있는 기술로 심의되었고(보건복지부 고시 제2013-71호, 2013.5.2.), 이후 2015년 비급여로 등재되었다(보건복지부 고시 제2015-59호, 2015.4.15.). 본 기술은 내부 모니터링을 통해 발굴된 안건으로 비급여로 등재된 이후 일정 기간 이상(비급여 등재 5년 이상) 임상에서 사용된 현 시점에서 임상적 유용성에 대한 재평가 필요성이 임상자문 및 우선순위 심의를 통해 확인되었다. 2022년 제3차 의료기술재평가위원회(2022.3.11.)에서 평가계획서를 심의한 후 재평가를 수행하였다.

평가 방법 비침습적 압력요류검사[음경커프이용]에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “비침습적 압력요류검사[음경커프이용] 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 비뇨의학과 3인, 근거기반의학 1인의 전문가 총 4인으로 구성하였다. 평가의 핵심질문은 ‘비침습적 압력요류검사[음경커프이용]은 하부요로증상 환자 중 방광출구폐색이 의심되는 환자를 대상으로 방광출구폐색을 진단하는데 안전하고 효과적인가?’이었다. 위 핵심질문을 토대로 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, 사이언스온) 및 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials)을 이용하여 문헌을 검색하였다. 문헌 선정기준 및 배제기준 적용을 통한 문헌선택과 비뚤림위험 평가는 모두 2명의 검토자가 독립적으로 수행하였고, 자료추출은 한 명의 검토자가 우선적으로 추출한 후 다른 검토자가 추출한 결과를 독립적으로 검토하고 오류가 있는지 확인하였다. 문헌의 비뚤림위험 평가는 Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2)을 사용하였다. 정량적 분석이 가능한 부분은 양적분석(메타분석)을 수행하였다.

평가 결과 동 평가에 최종 선택된 문헌은 총 10편(국외 10편)이었다. 비뚤림위험 평가결과 중재검사 수행 영역의 비뚤림 위험이 65%에서 ‘불확실’로 평가되었으며, 연구진행과 시점 영역의 비뚤림 위험도 약 70%에서 ‘불확실’로 평가되었다.

안전성 안전성은 검사관련 이상반응을 보고한 총 5편 문헌을 기준으로 평가하였다. 4편에서는 검사와 관련한 이상반응이 발생하지 않았다고 보고하였고, 나머지 1편에서 연구참여자 중 2%에서 자가요도출혈 및 통증 발생을 보고하였으나 검사와의 관련성을 명시하지 않았다. 소위원회에서는 문헌에서 보고된 출혈 및 통증은 방광출구폐색의 중증도에 따라 동반될 수 있는 증상으로 동 검사와 직접적인 관련이 있다고 판단하기 어렵다는 의견으로 동 검사는 안전한 것으로 평가하였다.

효과성 효과성은 9편의 문헌에서 보고된 진단정확성으로 평가하였다. 해당 문헌들에서는 진단정확성 결과를 노모그램 방식별로 제시하였다. 변형된 ICS 노모그램(5편)을 이용한 경우 민감도 0.70~0.90, 특이도 0.66~1.00, Griffiths 노모그램(3편)을 이용한 경우 민감도 0.78~1.00, 특이도 0.63~0.84, Shaefer & URA 노모그램(1편)을 이용한 경우 민감도 0.74, 특이도 0.94이었다. 합성이 가능한 자료를 메타분석 한 결과, 변형된 ICS 노모그램(3편)은 통합민감도 0.79(I2=48.9%), 통합특이도 0.84(I2=90.5%) 이었고, Gfiffiths 노모그램(2편)은 통합민감도 0.95 (I2=68.9%), 통합특이도 0.70(I2=13.4%) 이었다. 소위원회에서는 문헌에서 보고된 진단정확성 결과는 동 검사가 방광출구폐색 진단시 의미있는 것으로 보이지만, 각 문헌별로 채택한 노모그램 방식 및 임계값이 문헌 간 이질성이 높아 효과성에 대한 결론을 내리기에 근거가 제한적이라고 판단하였다. 또한 소위원회는 본 평가결과와 함께 방광출구폐색 진단시 비침습적 검사를 권고하지 않는 유럽 비뇨기과학회 임상진료지침(2021) 등을 고려하여 동 검사의 임상 적용 가능성이 낮다는 의견을 제시하였다.

결론 및 제언 비침습적 압력요류검사[음경커프이용]의 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 비침습적 압력요류검사[음경커프 이용]는 하부요로증상 환자 중 방광출구폐색이 의심되는 환자를 대상으로 방광출구폐색을 진단하는데 안전한 검사이지만, 검사의 효과성을 판단하기에 근거가 제한적이라고 판단하였다. 소위원회에서는 각 문헌별로 채택한 노모그램 방식 및 임계값이 문헌 간 이질성이 높고, 유럽 비뇨기학회 임상진료지침(2021)에서 방광출구폐색 진단 시 비침습적 검사를 권고하지 않는 점 등을 함께 고려할 때 동 검사의 임상 적용가능성이 낮다는 의견을 제시하였다. 2022년 제11차 의료기술재평가위원회(2022.11.11.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “비침습적 압력요류검사[음경커프 이용]”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 비침습적 압력요류검사[음경커프이용]는 하부요로증상 환자 중 방광출구폐색이 의심되는 환자 대상으로 방광출구폐색을 진단하는데 안전한 검사이지만 임상적 효과성을 판단하기에 근거가 제한적이라고 판단한 소위원회 결론에 동의하였다. 이에 의료기술재평가위원회는 하부요로증상 환자 중 방광출구폐색 의심환자에서 방광출구폐색을 진단하는데 비침습적 압력요류검사[음경커프이용]를 ‘권고하지 않음’으로 심의하였다(권고등급: 권고하지 않음).

Background Non-invasive pressure flow study using a penile cuff (PCT) is a diagnostic technique used to assess bladder outlet obstruction (BOO) in male patients with lower urinary tract symptoms (LUTS). It involves measuring bladder pressure and urine flow using a cuff to determine the presence of BOO. This technology was recognized as safe and effective during the third session of the Committee of Health Technology Reassessment in 2013 (March 22, 2013) – Ministry of Health and Welfare Notice No. 2013-71; May 2, 2013. Subsequently, it was registered as a non-beneficial item in 2015 – Ministry of Health and Welfare Notice No. 2015-59; April 15, 2015. The need for reassessment of the clinical usefulness of this technology was identified through clinical consultation and priority review at present, as this technology has been used in clinical practice for longer than a certain period since being listed as a non-beneficial item (at least five years). It was identified by internal monitoring. Subsequently, reassessment was performed after the assessment plan was reviewed during the third session of the Committee of New Health Technology Reassessment in 2022 (March 11, 2022).

Methods A systematic review was performed to assess the safety and effectiveness of PCT. All assessment methods were finalized after review by the “PCT assessment subcommittee” (hereinafter referred to as “subcommittee”), with consideration of the assessment. The subcommittee comprised four experts: three urologists and one evidence-based medicine specialist. The key question in the assessment was “Is PCT safe and effective for diagnosing BOO in patients with LUTS suspected of having BOO?” Five domestic databases (KoreaMed, KMbase, KISS, KERIS, and ScienceOn) and three international databases (Ovid MEDLINE, Ovid EMBASE, and Cochrane Central Register of Controlled Trials) were searched based on the key question above. Two reviewers independently performed article selection based on the inclusion and exclusion criteria. A single reviewer performed data extraction, and a second reviewer independently assessed the extracted results to check for errors. Risk of bias (RoB) assessment was performed using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) tool. Quantitative analysis (meta-analysis) was performed for relevant variables.

Results A total of 10 articles (all non-Korean articles) were ultimately selected for the assessment. The RoB in the performance of the intervention was evaluated as "uncertain" in 65% of studies, while the RoB in study progression and timing was evaluated as "uncertain" in approximately 70% of studies.

Safety Safety evaluation was based on a total of five articles reporting adverse reactions (ARs) related to the procedure. Among these, four articles reported no occurrence of ARs related to the test, while the remaining article reported self-reported urethrorrhagia and pain in 2% of the participants, but the relationship with the test was not clearly specified. Accordingly, the subcommittee determined that this test is a safe health technology based on the opinion that bleeding and pain reported in the literature are symptoms that may appear depending on the severity of BOO and that it is difficult to determine the causal relationship with the test.

Effectiveness Effectiveness was evaluated based on the diagnostic accuracy reported in nine articles. The diagnostic accuracy results were presented according to different nomogram methods in the respective literature. In articles that used the modified International Continence Society (ICS) nomogram (n=5), the sensitivity was 0.70-0.90 and specificity was 0.66-1.00; in articles that used the Griffiths nomogram (n=3), the sensitivity was 0.78-1.00 and specificity was 0.63-0.84; and in the articles that used the Shaefer & URA nomogram (n=1), the sensitivity was 0.74 and specificity was 0.94. Meta-analysis of synthesizable data showed a pooled sensitivity of 0.79 (I2=48.9%) and a pooled specificity of 0.84 (I2=90.5%) in articles that used the modified ICS nomogram (n=3) and a pooled sensitivity of 0.95 (I2=68.9%) and pooled specificity of 0.70 (I2=13.4%) in articles that used the Griffiths nomogram (n=2). Accordingly, the subcommittee determined that the diagnostic accuracy reported in the literature may appear to be significant when this test is used for diagnosing BOO, but the evidence for concluding the effectiveness of this technology is limited due to high heterogeneity in the nomogram methods and threshold values used in each article. In addition, the subcommittee opined that this diagnostic technology has low clinical applicability in consideration of the clinical practice guidelines issued by the European Association of Urology (2021), which does not recommend non-invasive testing for diagnosing BOO.

Conclusions and recommendations Based on the current assessment results, the subcommittee made the following recommendations: PCT is safe for diagnosing BOO in patients with LUTS suspected of having BOO, but there is limited evidence for determining the effectiveness of this test. The subcommittee concluded that this health technology has low clinical applicability, given the high heterogeneity in the nomogram methods and threshold values used in each article, and the clinical practice guidelines issued by the European Association of Urology (2021) do not recommend non-invasive testing for the diagnosis of BOO.

During the eleventh session of the Committee of Health Technology Reassessment in 2022 (November 11, 2022), the following conclusions were made on “PCT” based on the subcommittee findings in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project. The Committee of Health Technology Reassessment concurred with the subcommittee's conclusion that PCT may be a safe health technology for diagnosing BOO in patients with LUTS suspected of having BOO, but there is limited evidence for determining the effectiveness of this test. Accordingly, the Committee of Health Technology Reassessment “does not recommend” PCT for diagnosing BOO in patients with LUTS suspected of having BOO (Grade of recommendation: not recommended).
