자율신경계이상검사[지속적 근긴장에 따른 혈압검사]

Abstract

평가배경 자율신경계이상검사 [지속적 근긴장에 따른 혈압검사]는 심혈관계 반사를 이용한 자율신경계검사 중 하나로서, 악력계(handgrip dynamometer)를 사용하여 처음 수초 간은 강하게, 이후 5분까지 최대 수축의 30%가 되도록 유지한 후 매분마다 혈압을 측정하며, 반응의 단위로 시작 전 이완기 혈압과 놓기 전 이완기 혈압의 차이를 사용한다. 지속적 근긴장에 따른 혈압검사는 2006년에 등재 비급여로 고시되었으며, 이후 건강보험심사평가원 예비급여부에서 비급여의 급여화 추진을 위하여 본 검사를 포함한 자율신경계이상검사 7개 항목(교감신경피부반응검사, 자율신경계이상검사(기립성 혈압검사, 발살바법, 지속적 근긴장에 따른 혈압검사, 심박변이도검사, 피부전도반응검사), 정량적 발한 축삭 반사검사)에 대한 재평가를 한국보건의료연구원에 의뢰하였다(2022.1.). 이에 따라 2022년 제2차 의료기술재평가위원회(2022.2.18.)에서는 해당 기술들의 평가계획서 및 통합 소위원회 구성안을 심의한 후 임상적 안전성 및 효과성을 평가하였다.

평가방법 본 평가는 자율신경계기능장애 의심환자 및 질환자를 대상으로 지속적 근긴장에 따른 혈압검사가 임상적으로 안전하고 효과적인지 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “통합 자율신경계이상검사 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 신경과 3인, 내분비대사내과 2인, 순환기내과 2인, 정신건강의학과 2인, 근거기반의학 2인의 전문가 총 11인으로 구성하였다. 체계적 문헌고찰의 핵심질문은 “자율신경계기능장애 의심환자 및 질환자를 대상으로 자율신경계기능을 평가하기 위한 지속적 근긴장에 따른 혈압검사는 임상적으로 안전하고 효과적인가?”이다. 안전성은 검사 관련 이상반응 또는 부작용, 효과성은 진단정확성, 질환과의 관련성, 의료결과의 영향으로 평가하였다. 체계적 문헌고찰은 위의 핵심질문을 토대로 국외 데이터베이스 3개(Ovid MEDLINE, Ovid Embase, Cochrane Central Register of Controlled Trials) 및 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, ScienceON)를 이용하여 문헌을 검색하였으며, 문헌 선정기준 및 배제기준 적용을 통한 문헌선택과 자료추출, 비뚤림위험 평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 문헌의 비뚤림위험 평가는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)를 사용하였다. 자료분석에서 진단정확성은 대상환자 및 진단기준이 다양하고 문헌별 2×2 표 값이 추출되지 않아 질적으로 기술하였으며, 질환과의 관련성은 질환 하위그룹별, 자율신경계이상 유무별, 자율신경계이상 중증도별 비교에서 모두 환자군과 대조군 간 비정상 검사결과 비율에 대한 메타분석으로 결과의 경향성을 확인하였다.

평가결과 본 평가에 최종 선택된 문헌은 총 11편으로 진단적 코호트 연구 4편, 환자대조군 연구 7편이었다. 비뚤림위험 평가결과, 환자선택 영역에서 비뚤림위험은 ‘높음’이 36.4%, 중재검사 영역에서 비뚤림위험은 ‘불확실’이 63.6%였다. 참고표준검사 영역에서 비뚤림위험은 ‘높음’이 54.5%, 연구진행과 시점 영역에서 비뚤림위험은 ‘낮음’이 100%였다. 적용성에 대한 우려는 환자선택 영역에서 ‘불확실’이 100%, 중재검사 영역에서는 ‘불확실’이 45.5%, 참고표준검사 영역에서는 ‘불확실’이 63.6%였다.

안전성 지속적 근긴장에 따른 혈압검사의 안전성은 검사 관련 이상반응 또는 부작용으로 평가하고자 하였으나, 해당 지표를 보고한 문헌은 없었다. 소위원회는 동 검사는 비침습적인 검사로 안전성은 문제가 없다고 판단하였다.

효과성 지속적 근긴장에 따른 혈압검사의 효과성은 진단정확성, 질환과의 관련성으로 구분하였고, 의료결과의 영향을 보고한 연구는 없었다. 진단정확성은 총 4편에서 보고하였다. 개별 연구의 결과에 따르면, 노인의 자율신경계이상 진단에서 동 검사의 민감도는 60.6%, 특이도는 70.3%였고, 만성신부전에서 자율신경계이상 진단의 민감도는 86%, 특이도는 80%였다. 당뇨병에서 심혈관계 자율신경병증 진단의 민감도는 24.6%, 특이도는 79.4%였다. 다계통위축(multiple system atrophy with predominant parkinsonism, MSA-P)과 파킨슨병을 감별하는 데 있어 동 검사의 민감도는 85%, 특이도는 94%였다. 환자군과 대조군 간 지속적 근긴장에 따른 혈압검사의 비정상 검사결과 비율은 총 7편에서 보고하였다. 메타분석 결과, 질환 하위그룹별, 자율신경계이상 유무별, 자율신경계이상 중증도별 비교에서 모두 대조군보다 환자군에서 비정상 검사결과 비율이 유의하게 높았다. 이에 소위원회에서는 지속적 근긴장에 따른 혈압검사가 문헌에서는 의미있는 연구결과들이 확인되나 실제로 임상에서는 거의 사용하지 않고, 검사방법이 표준화되지 않아 검사방법에 따라 결과가 달라질 수 있어 임상적 유용성이 제한적이라고 판단하였다.

결론 통합 자율신경계이상검사 소위원회는 현재 평가결과에 근거하여 다음과 같이 제언하였다. 지속적 근긴장에 따른 혈압검사는 자율신경계기능장애 의심환자 및 질환자를 대상으로 자율신경계기능평가를 목적으로 사용시 안전한 기술이나, 임상에서 거의 사용하지 않고 검사방법이 표준화되어 있지 않아 임상적 유용성이 제한적이라고 판단하였다. 2023년 제1차 의료기술재평가위원회(2023.1.13.)에서는 자율신경계이상검사 [지속적 근긴장에 따른 혈압검사]에 대한 소위원회 결론을 검토하여 다음과 같이 심의 의결하고 권고등급을 결정하였다. 자율신경계이상검사 [지속적 근긴장에 따른 혈압검사]는 비침습적 검사로 안전한 기술로 판단하였으며, 임상에서 거의 사용하지 않고 검사방법이 표준화되어 있지 않아 임상적 유용성이 제한적이라고 판단하였다. 이에 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황이나 가치에 따라 자율신경계기능장애 의심환자 및 질환자를 대상으로 자율신경계기능평가를 목적으로 사용시 자율신경계이상검사 [지속적 근긴장에 따른 혈압검사]를 ‘권고하지 않음’으로 심의하였다.

Background The autonomic neuropathy test [sustained handgrip test] is one of the autonomic nervous system tests that uses the cardiovascular reflex. In this test, the subject is instructed to grip the handgrip dynamometer at full strength for the first few seconds and maintain 30% of maximum contraction for up to five minutes. Then, blood pressure (BP) is measured each minute, and the difference between the diastolic BP before starting and systolic BP before releasing is calculated as the unit of response. Sustained handgrip test was listed as a registered non-benefit item in 2006, and subsequently, the Preliminary Coverage Division of the Health Insurance Review and Assessment Service (HIRA) commissioned the National Evidence-based Healthcare Collaborating Agency to conduct a reassessment of seven autonomic neuropathy test items, including sympathetic skin response test, autonomic neuropathy [orthostatic blood pressure test, Valsalva maneuver, sustained handgrip test, heart rate variability test, and skin conductance test], and quantitative sudomotor axon reflex test for expansion of insurance coverage to include non-benefit items (January 2022). Accordingly, the reassessment of clinical safety and effectiveness was conducted after a review of the reassessment plan and the proposed integration subcommittee composition of the applicable technologies during the 2nd session of the Committee of Health Technology Reassessment in 2022 (February 18, 2022).

Methods In this assessment, a systematic review was performed to assess the clinical safety and effectiveness of sustained handgrip test for assessing autonomic nervous system function in patients with or suspected of having autonomic dysfunction. All assessment methods were finalized through discussions with the “Integration subcommittee for autonomic neuropathy tests” (hereinafter referred to as the “subcommittee”) with consideration of the purpose of the assessment. The subcommittee consisted of 11 experts: three from neurology and two each from endocrinology and metabolism, cardiology, psychiatry, and evidence-based medicine. The key question of the systematic review was, “Is the sustained handgrip test clinically safe and effective for assessing the autonomic nervous system function in patients with or suspected of having autonomic dysfunction?” Safety was assessed based on test-related adverse events (AEs) or complications, while effectiveness was assessed based on diagnostic accuracy, association with the disease, and impact on health outcomes. In the systematic review, three international databases (Ovid MEDLINE, Ovid EMBASE, and Cochrane Central Register of Controlled Trials) and five domestic databases (KoreaMed, KMbase, KISS, KERIS, and ScienceON) were searched based on the key question. Article selection based on inclusion/exclusion criteria, data extraction, and risk of bias (RoB) assessment were performed independently by two reviewers, and in cases of disagreement, a consensus was reached through discussions with a third person. RoB assessment was performed using Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). In the data analysis, the results for diagnostic accuracy were qualitatively described due to variations in patient populations and diagnostic criteria, making it impossible to extract 2 × 2 tables of values for each article. Regarding association with the disease, trends in the results were identified based on meta-analysis of rates of abnormal test results between the patient and control groups according to comparisons by disease subgroups, presence/absence of autonomic neuropathy, and severity of autonomic neuropathy.

Results A total of 11 articles were found in this assessment: four diagnostic cohort studies and seven patient-control studies. Regarding the RoB assessment, “high” RoB was observed in 36.4% of the patient selection domain; “uncertain” in 63.6% of the index test domain; “high” in 54.5% of the reference standard domain; and “low” in 100% of the flow and timing domain. Meanwhile, concerns about applicability were “uncertain” in 100% of the patient selection domain, 45.5% of the index test domain, and 63.6% of the reference standard domain.

Safety Safety of sustained handgrip test was assessed based on test-related AEs and complications, but no articles reported on these indicators. The subcommittee determined that safety is not a concern, as this test is non-invasive.

Effectiveness The effectiveness outcomes were classified into diagnostic accuracy and association with the disease; however, no articles reported on the impact on health outcomes. Diagnostic accuracy was reported in a total of four articles. According to results reported in individual studies, the sensitivity and specificity of this test were 60.6% and 70.3%, respectively, for diagnosing autonomic neuropathy in the elderly, and 24.6% and 80%, respectively, for diagnosing autonomic neuropathy in patients with chronic renal failure. Moreover, the sensitivity and specificity of this test were 24.6% and 79.4%, respectively, for diagnosing cardiovascular autonomic neuropathy in patients with diabetes. Furthermore, the sensitivity and specificity of this test were 85% and 94% for differentiating between multiple system atrophy with predominant parkinsonism (MSA-P) and Parkinson’s disease. The rate of abnormal sustained handgrip test results between patient and control groups was reported in a total of seven articles. The meta-analysis results showed significantly higher rate of abnormal test results in the patient group, as compared to the control group, in comparisons by disease subgroups, presence/absence of autonomic neuropathy, and severity of autonomic neuropathy. Accordingly, despite identifying meaningful findings in the literature, the subcommittee determined that sustained handgrip test has limited clinical usefulness, as this method is almost never used in actual clinical practice and the potential variability in results due to the lack of standardized test methods.

Conclusions Based on the current assessment results, the subcommittee presented the following recommendations. Sustained handgrip test was determined to be a safe technology when used for the purpose of assessing autonomic nervous system function in patients with or suspected of having autonomic dysfunction. However, its clinical usefulness is limited because it is rarely used in actual clinical practice and the test method lacks standardization. During the first session of the Committee of Health Technology Reassessment in 2023 (January 13, 2023), the following determinations and grade of recommendation were made on autonomic neuropathy test [sustained handgrip test] based on the review of the subcommittee findings. Autonomic neuropathy test [sustained handgrip test] was determined to be a safe technology as a non-invasive test, but its clinical usefulness is determined to be limited because it is rarely used in actual clinical practice and the test method lacks standardization. Accordingly, the Committee of Health Technology Reassessment “does not recommend” the use of autonomic neuropathy test [sustained handgrip test] for the purpose of assessing autonomic nervous system function in patients with or suspected of having autonomic dysfunction according to the clinical situation or value in Korea based on comprehensive consideration of the evidence of its clinical safety and effectiveness and other assessment items.
