포경수술

Abstract

평가 배경 포경수술은 젖혀지지 않는 포피 일부를 절제하여 귀두가 완전히 드러나도록 만드는 수술로, 치료보다 비치료적(종교적, 문화적 등) 목적으로 많이 수행되고 있으며 수술을 둘러싼 다양한 찬반 논쟁이 존재하고 있다. 국내에서는 질병을 동반하는 것에 한하여 건강보험에서 급여로 인정되며, 질병을 동반하지 아니한 단순포경(phimosis)은 의학적 치료 목적이 아닌 것으로 선택비급여에 해당한다. 해당 의료기술은 국민 생활밀착형 의료기술로 올바른 의료정보를 국민에게 제공하기 위하여 내부 모니터링을 통해 발굴된 주제로, 대상선별 임상자문회의 및 우선순위 심의에서 재평가 적합성과 필요성을 인정받고 2022년 제3차 의료기술재평가위원회(2022.3.11.)에서 평가계획서를 심의받아 재평가를 수행하였다.

평가 방법 본 평가는 포경수술과 관련된 체계적 문헌고찰 문헌이 다수 확인되어, 체계적 문헌고찰의 검토(Overview of Systematic Reviews)를 통해 최근에 출판된 체계적 문헌고찰의 결과를 정리하고 포경수술의 안전성 및 효과성에 대한 정보를 종합적으로 제공하기로 하였다. 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 수행하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 체계적 문헌고찰 연구의 방법론적 비뚤림 위험평가는 AMSTAR 2 (A Measurement tool to assess systematic reviews) 도구를 이용하여 수행하였으며, 자료추출은 미리 정해놓은 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였다. 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 선정된 체계적 문헌고찰들의 결과를 중심으로 결론 및 대국민 정보문을 작성하였으며, 이는 대한비뇨의학회의 의견 수렴 절차를 거쳤다.

평가 결과 최종적으로 포경수술과 관련된 25편의 최신의 체계적 문헌고찰 문헌이 선정되었다.

안전성 포경수술의 안전성과 관련된 10편의 체계적 문헌고찰이 확인되었으나, 근거수준(AMSTAR 2)이 대부분 ‘매우 낮음’으로 나타나 연구결과의 전반적 신뢰도가 낮은 것으로 보았다. 4편의 체계적 문헌고찰에서 보고한 포경수술 행위와 직접 관련된 시술 관련 합병증으로 유착, 외요도구 협착, 감염, 출혈 등이 대표적이었으며 이러한 경미한 이상반응이 드물게(약 1~5%) 발생하는 것으로 확인되었다. 7편의 체계적 문헌고찰에서는 포경수술 후 발생할 수 있는 성기능 및 성적 쾌락과 관련된 부작용을 주요하게 보고하였다. 포경수술 후 해당 부작용은 발생하지 않거나 경미한 것으로 나타났으며, 포경수술 여부와 해당 부작용과 관련성은 없는 것으로 확인되었다. 그 외 다른 부작용과 관련된 근거는 거의 확인하기 어려웠다.

효과성 포경수술의 효과성과 관련된 18편의 체계적 문헌고찰이 확인되었으나, 근거수준(AMSTAR 2)이 대부분 ‘매우 낮음’으로 나타나 연구결과의 전반적 신뢰도가 낮은 것으로 보았다. 4편의 체계적 문헌고찰에서는 모두 일관성 있게 포경수술을 받은 남성에서 HIV 감염 위험이 유의하게 낮은 것으로 나타났다. 3편의 체계적 문헌고찰에서는 모두 일관성 있게 포경수술을 받은 남성에서 HPV 감염 위험이 유의하게 낮았으나, HPV acquisition 및 HPV clearance와는 관련성이 없는 것으로 나타났다. 7편의 체계적 문헌고찰에서는 그 외 다양한 성매개감염병의 효과를 확인하였고, 생식기 사마귀 발생 위험을 보고하고 있는 문헌들에서는 모두 일관되게 포경수술과 생식기 사마귀는 관련성이 없는 것으로 나타났다. 그러나 그 외 다양한 성매개감염병에 있어서는 일부 질환에서 효과가 있다고 하거나 포경수술과 관련성이 없다고 한 연구결과가 혼재되어 있었다. 3편의 체계적 문헌고찰에서는 모두 일관성 있게 포경수술을 받은 남성에서 요로감염 발생 위험이 낮은 것으로 나타났다. 6편의 체계적 문헌고찰에서 암 예방의 효과를 확인하였다. 포경수술과 전립선암의 관련성을 확인한 대부분의 연구에서는 포경수술을 받은 남성에서 전립선암 발생 위험이 낮은 것으로 보고되었으나, 최신의 연구(Howe 등, 2020)에서는 관련성이 없다고하여 그 결과에 차이가 있었다. 포경수술은 음경암 발생을 예방하는 것으로 보고되었으며, 특히 침윤성 음경암 발생 위험이 유의하게 낮은 것으로 나타났다. 남성 포경수술과 여성 관련 건강결과를 보고한 3편의 체계적 문헌고찰 중, 최근 연구 2편에서는 남성 포경수술이 여성의 자궁경부암 발생 위험을 낮추는 것으로 보고되었으나, 그 외 여성 질환(HSV-2, HIV acquisition, 매독 등)에 있어서는 연구결과가 혼재되어 있거나 관련성이 없는 것으로 나타났다.

결론 및 제언 포경수술 소위원회는 본 평가결과에 근거하여 다음과 같이 제언하였다. 총 25편의 체계적 문헌고찰을 중심으로 현재까지의 근거를 종합해 본 결과, 포경수술은 합병증 및 부작용에 있어 큰 우려가 없는 안전한 의료기술로 평가하였다. 효과성 측면에서 포경수술은 인체면역결핍바이러스(HIV), 사람유두종바이러스(HPV), 요로감염, 음경암, 자궁경부암 예방에 효과가 있는 것으로 확인되었으나, 그 외 성매개감염병, 전립선암, 그 외 여성 질환과 관련된 효과를 입증하기는 어려웠다. 그러나 본 평가결과는 근거수준(AMSTAR 2)이 대부분 ‘매우 낮음’으로 체계적 문헌고찰로 도출된 근거가 정확하고 포괄적이라고 보기 어려운 제한점을 가지고 있어 해석에 주의가 필요하겠다. 2022년 제10차 의료기술재평가위원회(2022.10.14.)에서는 소위원회 검토 결과와 대국민 정보문에 대해 논의하였으며, 의료기술재평가사업 관리지침 제4조 제10항에 의거 “포경수술”에 대해 소위원회 결론은 원안대로 심의 의결하고 대국민 정보문을 일부 수정하여 심의 의결하였다.

Background Male circumcision is a procedure used to excise non-retracting foreskin to fully expose the glans. Male circumcisions are performed more often for non-therapeutic (e.g., religious, cultural) purposes than therapeutic purposes and there are debates for and against this procedure. In Korea, male circumcision is recognized as a health insurance benefit item when the case involves a disease, whereas phimosis that does not involve a disease is considered a selective benefit service performed for non-medical purposes. This procedure was identified as a life-oriented health technology by internal monitoring undertaken to provide proper health information to the public. The appropriateness and need for reassessment of this health technology was recognized by subject selection clinical consultation meetings and priority reviews. Subsequently, reassessment was performed after the assessment plan was reviewed during the third session of the Committee of Health Technology Reassessment on March 11, 2022.

Methods Several systematic review (SR) articles related to male circumcision were identified and this assessment aimed to comprehensively review the safety and effectiveness of male circumcision and synthesize the results reported in the latest published SR articles through an overview of SRs. Three international and five domestic databases were searched and two reviewers independently screened and selected the articles based on the inclusion and exclusion criteria. Methodological risk of bias (RoB) assessment was performed using the AMSTAR 2 (version 2 of a measurement tool to assess systematic reviews) and data extraction was independently performed by two reviewers using a predetermined format. Disagreements were resolved through discussion with a third person to reach a consensus. The conclusions and public information were drafted based on the results from the selected SRs, which underwent opinion convergence by the Korean Urological Association.

Results A total of 25 of the latest SR articles related to male circumcision were ultimately selected.

Safety A total of 10 SRs related to the safety of male circumcision were identified, but the level of evidence (AMSTAR 2) in most of these SRs was assessed to be “very low,” meaning that the overall reliability of the study results was considered to be low. Prime examples of procedure-related complications directly associated with male circumcision, reported in four SRs, were adhesion, meatal stenosis, infection, and bleeding, and these mild adverse events (AEs) occurred rarely (in approximately 1%–5% of cases). Seven SRs reported mostly on AEs related to sexual function and pleasure that may occur after male circumcision. These AEs either did not occur or were mild cases, while associations between male circumcision and such AEs could not be found. Evidence related to other AEs was also difficult to find.

Effectiveness A total of 18 SRs relating to the effectiveness of male circumcision were identified, but the level of evidence (AMSTAR 2) in most of these SRs was assessed to be “very low,” meaning that the overall reliability of the study results was considered to be low. Four SRs consistently reported that the risk of human immunodeficiency virus (HIV) infection was significantly lower in circumcised males. Three SRs consistently reported that the risk of HIV infection was significantly lower in circumcised males, but that male circumcision had no association with human papillomavirus (HPV) acquisition and clearance. Seven SRs reported the effect of various other sexually transmitted diseases (STDs). Articles on the risk of genital warts consistently reported that there was no association between male circumcision and genital warts. However, results were mixed, indicating that male circumcision had an effect on some STDs or that there was no association between male circumcision and other STDs. Three SRs consistently reported that the risk of urinary tract infection (UTI) was lower in circumcised males. Six SRs reported the cancer prevention effect of male circumcision. Most studies that investigated the association between male circumcision and prostate cancer reported a lower risk of prostate cancer in circumcised males, whereas a recent study (Howe et al., 2020) reported no association. It has been reported that male circumcision can prevent penile cancer and it was found to significantly reduce the risk of invasive penile cancer. Among three SRs that reported on male circumcision and female health outcomes, two recent studies reported that male circumcision can reduce the risk of cervical cancer. Regarding other female diseases, such as HSV-2, HIV acquisition, and syphilis; studies have reported conflicting results or no association.

Conclusions and recommendations The subcommittee recommended the following, based on the assessment results. Based on the evidence found in a total of 25 SRs, male circumcision is a safe health technology with no major concerns regarding complications and AEs. With respect to its effectiveness, male circumcision was effective in preventing HIV and HPV infection, UTIs, penile cancer, and cervical cancer, but its effectiveness for other STDs, prostate cancer, and other female diseases, remains unknown. However, the level of evidence (AMSTAR 2) of most of the assessment results was found to be “very low,” and thus, caution is needed in interpreting the results since it is difficult to consider the evidence derived from the SRs to be accurate and comprehensive. During the tenth session of the Committee of Health Technology Reassessment on October 14, 2022, the subcommittee findings and public information were discussed. Accordingly, the committee made the decision to accept the subcommittee’s conclusions and partially revise the public information regarding male circumcision in accordance with Article 4-10 of the Guidelines for the Management of Health Technology Reassessment Project.
