Abstract

평가 배경 초음파 유도하 혈관경화요법초음파 유도하 혈관경화요법(duplex ultrasound guided sclerotherapy 또는 ultrasound-guided foam sclerotherapy, UGFS)은 하지정맥류 환자(깊은 곳에 위치한 정맥류나 기존의 방법으로는 치료가 어려운 정맥류)를 대상으로 초음파 유도 하에 혈관경화제를 정맥류가 있는 부위의 정맥에 주입하여 하지정맥류를 치료하는 기술이다.

동 기술은 신의료기술평가제도 확립 전에 행위비급여로 등재된 의료기술로, 보건의료 자원의 효율적 사용 도모와 의료기술의 최적 사용을 유도하기 위해 최신 근거를 평가할 필요가 있어 내부 모니터링을 통해 재평가 항목으로 제안되었으며, 2022년 제3차 의료기술재평가위원회(2022.03.11.)에서 의료기술재평가 안건으로 선정되어 재평가를 수행하였다.

평가 방법 초음파 유도하 혈관경화요법의 안전성 및 효과성의 평가를 위해 체계적 문헌고찰을 수행하였으며, 모든 평가방법은 평가목적을 고려하여 “초음파 유도하 혈관경화요법 평가 소위원회(이하 ‘소위원회’)”의 심의를 거쳐 확정하였다.

평가의 핵심질문은 하지정맥류 환자에서 초음파 유도하 혈관경화요법은 임상적으로 안전하고, 효과적인가이며, 안전성 측면으로는 시술 관련 합병증 및 부작용을, 효과성 측면으로 표적 정맥의 폐쇄율, VCSS (Venous Clinical Severity Score), 삶의 질(AVVQ [Averdeen Varicose Vein Questionnaire], CIVIQ [Chronic Venous Insufficiency Quality of life questionnaire] 등)을 확인하였다. 해당 소위원회에서는 초음파 유도하 혈관경화요법 관련 무작위배정 임상시험 연구(randomized controlled trial, RCT)가 충분하다고 판단하여, 연구유형을 RCT로 제한하였다.

체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림 위험 평가는 코크란의 Risk of Bias (RoB)를 사용하여 평가하였다. 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 수행하였으며, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 자료 분석은 기본적으로 비교중재의 유형에 따라 구분하여 수행하였으며, 정량적 분석이 가능할 경우 메타분석을 수행하고 불가능한 경우 질적 검토를 수행하였다. 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법론을 이용하여 근거수준을 평가하였으며, 본 평가는 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.

평가 결과 체계적 문헌고찰을 수행한 결과, 핵심질문을 충족하는 22편(연구기준 RCT 14개)이 선정되었다. 비교중재의 유형별로 ‘수술’과 비교한 연구가 11개, ‘고주파 치료’와 비교한 연구가 3개, ‘레이저 치료’와 비교한 연구가 5개 포함되었다.

안전성 초음파 유도하 혈관경화요법의 안전성은 시술 관련 합병증 및 부작용을 검토하였으며, 안전성 결과는 비교중재의 유형에 따라 제시하였다.

수술과 비교한 연구 11개에서 모두 시술 관련 합병증 및 부작용을 보고하였다. 중증 합병증은 대부분의 연구에서 발생하지 않았으며, 2개의 연구(Brittenden et al., 2014; Rasmussen et al., 2011)에서 중증 합병증이 매우 드물게 발생한 것으로 보고하였으나, 이는 수술과 유사한 수준이었다. 그 외 합병증으로, 색소침착, 정맥염, 시술 후 만져지는 덩어리(palpable lump)는 수술에 비해 초음파 유도하 혈관경화요법에서 더 많이 발생하였으며, 상처 감염, 감각이상은 수술군에서 유의하게 더 많이 발생한 것으로 보고되었다.

고주파 치료와 비교한 2개 연구에서 시술 관련 합병증 및 부작용을 보고하였다. 1편의 연구에서 중증 합병증이 발생하지 않았으나, 다른 1편에서는 심부정맥혈전증과 폐색전증이 1명의 환자에서 발생한 것으로 보고되었다. 그 외 합병증의 경우, 1편의 연구에서 고주파 치료에 비해 초음파 유도하 혈관경화요법에서 정맥염이 다소 많이 발생한 것으로 보고되었다.

레이저 치료와 비교한 5개 연구에서 시술 관련 합병증 및 부작용을 보고하였다. 중증 합병증은 2개 연구에서 발생하지 않았으나, 2개의 연구(Brittenden et al., 2014; Rasmussen et al., 2011)에서는 중증 합병증이 매우 드물게 발생한 것으로 보고하였다. 그 외 합병증의 경우, 색소침착, 정맥염, 시술 후 만져지는 덩어리는 레이저 치료에 비해 초음파 유도하 혈관경화요법에서 더 많이 발생하였으며, 감각이상, 혈종은 수술군에서 유의하게 더 많이 발생한 것으로 보고되었다.

안전성 결과를 종합하면, 초음파 유도하 혈관경화요법과 관련하여 중증 합병증은 대부분 발생하지 않았으며, 일부 연구에서 드물게 발생한 것으로 보고되었다. 그 외 합병증은 중재(수술, 열 치료, 최소침습적 치료)의 특성에 따라 예상 가능한 합병증으로, 소위원회에서는 초음파 유도하 혈관경화요법은 비수술적 형태의 적극적 치료법으로 안전한 의료기술이라는 의견이었다.

효과성 초음파 유도하 혈관경화요법의 효과성은 정맥 폐쇄율, 증상 완화 측면에서 VCSS, 통증, 삶의 질(AVVQ, CIVIQ 등)을 확인하였으며, 효과성 결과는 비교중재의 유형에 따라 제시하였다.

첫째, 초음파 유도하 혈관경화요법과 수술을 비교한 연구는 RCT 11개이었다. 폐쇄율을 보고한 8개 연구결과를 메타 분석한 결과 단기, 중기, 장기 시점에서 모두 수술에 비해 중재군의 폐쇄율이 유의하게 낮았다. 정맥류의 중증도 관련 지표인 VCSS를 보고한 6개 연구 중 4개 연구에서 시점에 관계없이 두 군 간 차이가 유의하지 않았다. 다만, 나머지 1개의 대규모 RCT 결과(Shadid et al., 2012)에 따르면, 2년 추적관찰시점의 VCSS는 두 군 간 유의한 차이가 없었으나, 8년 시점까지 장기 추적관찰 시 중재군에서 VCSS 점수가 악화되어 수술군에 비해 통계적으로 유의하게 점수가 높았다. 삶의 질은 질환 특이적 삶의 질 측정도구(AVVQ, CIVIQ)와 일반적 건강 관련 삶의 질 측정도구(EQ-5D [EuroQol-5 Dimension], SF-36 [Short Form-36])를 사용하여 평가하였다. 하지정맥류 환자의 질환 특이적 삶의 질 측정 도구로, AVVQ는 6개 연구에서 해당 결과를 보고하였다. 6개 연구 중 3개 연구에서 두 군 간 차이가 유의하지 않았으며 다른 2개 연구(Brittenden et al., 2014; Liu et al., 2011)의 경우 단기 및 장기(5년) 시점에서 중재군에 비해 수술군에서 질환 특이적 삶의 질이 좋은 것으로 보고되었다. 나머지 1개 연구(Bountouroglou et al., 2006)의 경우 단기 추적관찰 시점(3개월)의 삶의 질은 중재군이 수술군에 비해 좋았으나, 5년 시점까지 장기 추적관찰 시 중재군에 비해 수술군의 삶의 질이 통계적으로 유의하게 향상된 것으로 보고하였다. 만성 정맥부전 환자의 삶의 질 측정 도구인 CIVIQ를 보고한 1개 연구에서 중재군은 시간이 경과함에 따라 매년 삶의 질 점수가 나빠졌던 반면, 수술군에서는 매년 삶의 질 점수가 좋아졌으나, 비교군 간 통계적으로 유의한 차이는 확인되지 않은 것으로 보고하였다. 일반적 건강 관련 삶의 질 도구를 이용하여 해당 결과를 보고한 4개 연구로, 이 중 3개 연구에서 두 군 간 유의한 차이가 없었으며, 나머지 1개 연구에서는 세 군을 비교한 연구로 중재군과 수술군 간 통계적 유의성은 보고되지 않았다.

둘째, 초음파 유도하 혈관경화요법과 고주파 치료를 비교한 연구는 RCT 3개이었다. 2개 RCT 연구에서 폐쇄율을 보고하였다. 1년 이하의 단기 폐쇄율을 보고한 2개 연구를 메타분석 결과한 두 군 간 통계적으로 유의한 차이가 없었다. 중기 및 장기 시점의 폐쇄율을 보고한 1개 연구(Rasmussen et al., 2011)에서 중재군이 고주파 치료군에 비해 통계적으로 유의하게 낮았다. VCSS는 2개 연구에서 보고하였으며, 두 군 간 유의한 차이는 확인되지 않았다. 하지정맥류 환자의 질환 특이적 삶의 질 측정 도구 AVVQ는 2개 연구에서 보고하였으며, 두 군 간 유의한 차이는 확인되지 않았다. 일반적 건강 관련 삶의 질 도구를 이용하여 해당 결과를 보고한 2개 연구 중 1개 연구에서 비교군 간 유의한 차이가 없었다. 다른 1개 연구에서는 네 군 간 비교 결과 유의한 차이가 있다고 보고하였으나 사후검정 결과는 제시하지 않았으며, 평균 점수가 유사한 수준으로 확인되었다.

셋째, 초음파 유도하 혈관경화요법과 레이저 치료를 비교한 연구는 RCT 5개이었다. 폐쇄율은 5개 RCT에서 모두 보고하였다. 1년 이하의 단기 폐쇄율을 보고한 5개 연구 결과를 메타분석한 결과 두 군 간 통계적으로 유의한 차이가 없었다. 중기 폐쇄율(2개 연구)과 장기 폐쇄율(4개 연구)은 메타분석 결과 중재군이 레이저 치료군에 비해 폐쇄율이 유의하게 낮았다. VCSS를 보고한 3개 연구에서 모두 두 군 간 차이는 유의하지 않았다. 질환 특이적 삶의 질 관련하여, AVVQ를 보고한 4개 연구 중 3개 연구에서 두 군 간 유의한 차이가 없었다. 나머지 1개 연구에서 5년 시점에 중재군에 비해 레이저 치료군에서의 질환 특이적 삶의 질이 유의하게 좋은 것으로 보고되었다. 1개 연구에서 CIVIQ를 보고하였으며, 두 군 간 차이가 유의하지 않다. 일반적 건강 관련 삶의 질 도구를 이용하여 해당 결과를 보고한 연구는 3개로, 2개 연구에서 비교군 간 대부분의 하위 영역에서 통계적으로 유의한 차이가 없었다. 나머지 1개 연구에서는 네 군 간 비교 결과 유의한 차이가 있다고 보고하였으나 사후검정 결과는 제시하지 않았으며, 평균 점수가 유사한 수준으로 확인되었다. 효과성 결과를 종합하면, 초음파 유도하 혈관경화요법에서의 폐쇄율은 수술에 비해 단기/중기/장기 시점에서 모두 유의하게 낮았으며, 정맥 내 열 치료인 고주파 치료 또는 레이저 치료와 비교 시에는 단기 시점의 폐쇄율은 차이가 없었으나, 1년을 초과하는 중기/장기 시점에는 유의하게 낮았다. 증상 완화 측면에서 VCSS는 비교중재에 상관없이 대부분의 연구에서 군 간 유의한 차이가 없는 것으로 확인되었다. 삶의 질은 대부분의 연구에서 두 군 간 차이가 없었다.

결론 및 제언 소위원회에서는 현재 문헌적 근거를 바탕으로 다음과 같이 결과 및 의견을 제시하였다.

초음파 유도하 혈관경화요법과 관련하여 중증 합병증은 거의 발생하지 않았고, 일부 연구에서만 드물게 발생하였다. 이에 소위원회에서는 초음파 유도하 혈관경화요법은 하지정맥류 치료법 중 비수술적 형태의 적극적 치료법으로 안전한 의료기술이라는 의견이었다.

초음파 유도하 혈관경화요법은 주요 의료결과인 폐쇄율이 수술적 치료에 비해 유의하게 낮고, 정맥 내 열 치료인 고주파 치료 또는 레이저 치료와 비교 시 단기 시점에만 유의한 차이가 없었고, 1년을 초과하는 중기/장기 시점에는 유의하게 낮았다. 이에 대해 소위원회에서는 초음파 유도하 혈관경화요법의 5년 장기 폐쇄율이 23~74.3% 수준으로 보고되고 있어, 해당 기술의 효과성은 임상에 적용 가능한 수준이라는 의견이었다. 초음파 유도하 혈관경화요법은 최소침습적 중재로, 수술적 치료 등에 비해 높은 환자의 편의성을 보여주고 있다. 국내외 임상 가이드라인 및 현행 보험 상황에서도 초음파 유도하 혈관경화요법을 일부 환자를 대상으로 하여 제한적으로 권고하고 있어 소위원회에서는 이러한 상황을 고려하여 초음파 유도하 혈관경화요법의 안전성은 수용 가능하고, 깊은 곳에 위치한 정맥류나 기존의 방법(수술적 치료 혹은 정맥 내 열 치료)으로는 치료가 어려운 정맥류 환자를 대상으로 시행할 수 있는 효과적인 치료법으로 판단하였다.

2022년 제10차 의료기술재평가위원회(2022.10.14.)에서는 의료기술재평가사업 관리지침 제4조 제10항에 의거 “초음파 유도하 혈관경화요법”에 대하여 소위원회 검토 결과에 근거하여 다음과 같이 심의하였다.

의료기술재평가위원회에서는 초음파 유도하 혈관경화요법은 깊은 곳에 위치한 정맥류나 기존의 방법(수술적 치료 혹은 정맥 내 열 치료)으로는 치료가 어려운 정맥류 환자를 대상으로 시행할 수 있는 안전하고, 효과적인 치료법으로 판단하여 ‘조건부 권고함’으로 심의하였다(권고등급: 조건부 권고함).

Background Ultrasound-guided foam sclerotherapy (UGFS), also known as duplex ultrasound-guided sclerotherapy (DGS), is a health technology used to treat varicose veins, and it involves intravenously injecting a sclerosing agent into an area with varicose veins under ultrasound guidance in patients with deep varicose veins or varicose veins that were difficult to treat using conventional methods.

This health technology was listed as a non-benefit service item prior to establishment of the new health technology assessment system; it was identified as a reassessment item through internal monitoring based on the need to assess the latest evidence for promoting the efficient use of healthcare resources and promoting the optimal use of health technology. Accordingly, reassessment was performed based on its selection as a health technology reassessment agenda item during the third session of the Committee of Health Technology Reassessment in 2022 (March 11, 2022).

Methods A systematic review was performed to assess the safety and effectiveness of UGFS. All assessment methods were finalized after review by the “UGFS assessment subcommittee” (hereinafter referred to as “subcommittee”) with consideration of the purpose of the assessment.

The key question of the assessment was as follows: “Is UGFS clinically safe and effective in patients with varicose veins?” Safety was assessed based on procedure-related complications and adverse events (AEs), whereas effectiveness was assessed based on target vein occlusion rate, venous clinical severity score (VCSS), and quality of life (QoL; Aberdeen Varicose Vein Questionnaire [AVVQ] and chronic venous insufficiency quality of life questionnaire [CIVIQ]). The subcommittee limited the study type to randomized controlled trials (RCTs) based on the determination that sufficient number of UGFS-related RCTs were available.

For the systematic review, three international and five domestic databases were searched based on the key question. Two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria. In case of any disagreement, the final articles were selected through consensus among the evaluators. Risk of bias (RoB) assessment was performed using the Cochrane Risk of Bias tool for the selected literature. Two evaluators independently assessed the selected literature, and in cases of disagreement, a consensus was reached through discussion. Data analysis was performed according to the type of comparator intervention. A meta-analysis was performed if quantitative analysis was possible; otherwise, a qualitative review was performed. The results of the systematic review were assessed for level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method. The Committee of Health Technology Reassessment issued the grade of recommendation after a final review with consideration of the subcommittee’s review opinions.

Results Based on the systematic review, a total of 22 articles (14 RCTs) that satisfied the key question were selected. With respect to type of comparator intervention, 11, 3, and 5 RCTs that compared UGFS with surgery, radiofrequency ablation (RFA), and endovenous laser ablation (EVLA), respectively, were selected.

Safety The safety of UGFS was assessed based on procedure-related complications and AEs, and the safety outcomes were presented according to the types of comparator interventions.

Procedure-related complications and AEs were reported in all the 11 studies that compared UGFS with surgery. While severe complications were rare in most studies and occurred in two studies (Brittenden et al., 2014; Rasmussen et al., 2011) at similar levels to that associated with surgery. Other complications, such as pigmentation, phlebitis, and a palpable lump after the procedure, were more common in the UGFS group than in the surgery group, whereas wound infection and paresthesia were significantly more common in the surgery group.

Two studies on the comparison between UGFS and RFA reported procedure-related complications and AEs. One study reported no severe complications, whereas the other reported deep venous thrombosis and pulmonary embolism in one patient. With respect to other complications, one study reported that phlebitis occurred slightly more frequently in the UGFS group than in the RFA group.

Five studies comparing UGFS and EVLA reported procedure-related complications and AEs. Severe complications did not occur in two studies but were reported to be very rare in two other studies (Brittenden et al., 2014; Rasmussen et al., 2011). With respect to other complications, pigmentation, phlebitis, and a palpable lump after the procedure were more common in the UGFS group than in the EVLA group, whereas paresthesia and hematoma were significantly more common in the surgery group.

In summary, with respect to the safety outcomes, severe complications were mostly not observed with ultrasound-guided sclerotherapy and were only reported rarely in some studies. Other complications were expected based on the nature of the intervention (surgery, heat therapy, and minimally invasive therapy). Accordingly, the subcommittee determined that UGFS is a safe health technology that can be used as a non-surgical active therapy option.

Effectiveness The effectiveness of UGFS was assessed based on the vein occlusion rate, VCSS score for symptom improvement, and QoL (AVVQ and CIVIQ), and the effectiveness outcomes were presented according to the types of comparator interventions.

First, 11 RCTs comparing UGFS and surgery were identified. A meta-analysis of eight RCTs that reported on the occlusion rate showed that this rate was significantly lower in the intervention group than in the surgery group at all time points (short-term, mid-term, and long-term). Among six studies that used the VCSS, an indicator of the severity of varicose veins, four studies reported no significant difference between the two groups, regardless of time. Meanwhile, one large-scale RCT (Shadid et al., 2012) reported that while no significant difference was noted in the VCSS scores between the two groups at the 2-year follow-up, long-term follow-up (up to eight years) showed that VCSS scores in the intervention group decreased and were significantly different from those in the surgery group. QoL was assessed using disease-specific QoL scales (AVVQ and CIVIQ) and general health-related QoL scales (EuroQol-5 Dimension [EQ-5D] and Short Form-36 [SF-36]). Six studies reported results on disease-specific QoL in patients with varicose veins using the AVVQ tool. Of the six studies, three studies reported no significant difference between the two groups. Two other studies (Brittenden et al., 2014; Liu et al., 2011) reported higher disease-specific QoL in the surgery group than in the intervention group at short-term and long-term (5 years) follow-ups. The remaining one study (Bountouroglou et al., 2006) reported higher QoL in the intervention group than in the surgery group at short-term follow-up (3 months); however, compared with the intervention group, the surgery group showed significant improvement in QoL at follow-up up to 5 years. One study that used the CIVIQ to measure QoL in patients with chronic venous insufficiency reported that the intervention group showed a decrease in QoL scores over time, whereas the surgery group showed an increase in QoL scores each year. However, no significant difference was noted between the two groups. Six studies reported results based on the application of the general health-related QoL scale. Of these, three studies reported no significant difference between the two groups. The remaining one study compared three groups and did not report statistical significance for comparisons between the intervention and surgery groups.

Second, three RCTs comparing UGFS and RFA were identified, of which, two RCTs reported on the occlusion rate. A meta-analysis of two RCTs that reported on the short-term (≤1 year) occlusion rate revealed no significant difference between the two groups. In one study that reported on mid-term and long-term occlusion rates (Rasmussen et al., 2011), the intervention group showed significantly lower occlusion rates than did the RFA group. Two studies used the VCSS and reported no significant difference between the two groups. Two studies used the AVVQ and reported no significant difference between the two groups. Of the two studies that reported results based on the general health-related QoL scale, one study reported no significant difference between the two groups, whereas the other study reported significant differences between the four groups that were compared but did not provide post-hoc test results; the mean scores were found to be similar among the groups.

Third, five RCTs comparing UGFS and EVLA were identified, all of which reported on occlusion rates. A meta-analysis of five studies that reported the short-term (≤1 year) occlusion rates revealed no significant difference between the two groups. A meta-analysis of two RCTs on the mid-term occlusion rate and four RCTs on the long-term occlusion rate showed that the occlusion rate was significantly lower in the intervention group than in the EVLA group. All three studies that used the VCSS reported no significant difference between the two groups. Among the four studies that used AVVQ, three studies reported no significant difference between the two groups. The remaining one study reported that the disease-specific QoL was significantly higher in the EVLA group than in the intervention group at 5-year follow-up. One study used the CIVIQ and reported no significant difference between the two groups. Of the three studies that used the general health-related QoL scale, two studies reported no significant differences in most of the subdomains between the two groups. The remaining study reported significant differences among four groups that were compared but did not provide post-hoc test results; the mean scores were found to be similar among the groups. In summary, with reference to the effectiveness outcomes, UGFS was associated with significantly lower occlusion rates at all time points (short-, mid-, and long-term) than surgery. Although the short-term occlusion rate associated with UGFS was not different from that associated with RFA or EVLA, UGFS showed significantly lower occlusion rates at mid-term and long-term time points (>1 year). With respect to symptom alleviation, no significant differences were noted in the VCSS scores in most studies, regardless of the comparator intervention used. Moreover, no significant difference was noted in QoL scores between the two groups in most studies.

Conclusions and recommendations Based on the evidence currently available in literature, the subcommittee presented the following findings and opinions.

According to 11 studies comparing UGFS, severe complications were rare and only reported in some studies. Accordingly, the subcommittee determined that UGFS is a safe health technology that can be used as a non-surgical active therapy option for treating varicose veins.

Compared to surgery, UGFS was associated with a significantly lower occlusion rate (a key health outcome). The short-term occlusion rate associated with UGFS was not significantly different compared with that associated with endovenous treatments such as RFA and EVLA; however, the occlusion rates at mid/long-term time points (>1 year) were significantly lower with UGFS. Accordingly, based on the 5-year long-term occlusion rate (reported to range from 23% to 74.3%), the subcommittee opined that the effectiveness of UGFS is at a clinically acceptable level. Compared with surgery, UGFS is a minimally invasive procedure that offers greater convenience to patients. The current domestic and international clinical guidelines and current insurance guidelines also recommend UGFS for use on a limited basis for some patients. Considering these factors, the subcommittee determined that UGFS has an acceptable level of safety and can be used as an effective treatment modality for patients with deep varicose veins or varicose veins that were difficult to treat using conventional methods such as surgery or endovenous heat therapy.

During the 10th session of the Committee of Health Technology Reassessment in 2022 (October 14, 2022), the following conclusion was made regarding UGFS based on the subcommittee findings in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project.

The Committee of Health Technology Reassessment determined that UGFS is a safe and effective health technology and “conditionally recommends” the use of UGFS for patients with deep varicose veins or varicose veins that were difficult to treat using conventional methods such as surgery or endovenous heat therapy (grade of recommendation: conditionally recommended).
