신장암에 실시하는 고주파 열치료술-개복술하, 복강경하-고주파

Abstract

평가 배경 신장암의 고주파 열치료술은 신장암 환자에서 고주파를 이용하여 조직을 절개 및 응고시킴으로써 신장기능의 저하를 최소화하면서 환자의 생존율 및 삶의 질을 향상시키기 위한 목적으로 실시하는 기술이다. 동 기술은 접근방법별로 개복술하, 복강경하, 경피적 3가지 방법이 신의료기술평가제도가 확립되기 전 비급여로 등재된 후 2011년 행위급여로 전환되어 현재까지 이용되고 있다. 동 기술은 2019년 유관기관 수요조사 접수 및 2022년 내부 모니터링을 통해 재평가 안건으로 발굴되었으며, 2022년 제8차 의료기술재평가위원회(2022.8.12.)에서 전문적·심층적 검토를 통해 동 기술의 임상적 안전성 및 효과성 평가가 필요한 안건으로 심의하였다. 이에 본 평가는 체계적 문헌고찰을 통해 신장암의 고주파 열치료술의 임상적 안전성 및 효과성의 근거를 확인하여 합리적 의료자원 활용을 지원하고자 수행하였다.

평가 방법 동 기술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “신장암의 고주파 열치료술 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 비뇨의학과 2인, 영상의학과 2인, 신장내과 1인, 종양내과 1인, 근거기반의학 1인의 전문가 7인으로 구성하였다. 평가의 핵심질문은 “신장암 환자에서 개복술하 고주파 열치료술은 안전하고 효과적인가?”와 “신장암 환자에서 복강경하 고주파 열치료술은 안전하고 효과적인가?”이며 안전성은 합병증 및 이상반응 지표로, 효과성은 일차지표로 종양학적 결과, 신기능 변화, 환자만족도 또는 삶의 질, 이차지표로 소요시간, 혈액소실 정도, 재원기간을 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 선택된 문헌은 7편의 후향적 코호트연구로 비뚤림 위험 평가는 Risk of Bias Assessment Tool for Nonrandomized Studies (RoBANS) 2.0을 이용하여 평가하였다. 자료분석은 양적분석이 가능한 부분에 대하여 메타분석을 수행하였고, 불가능한 영역에 대하여 정성적(qualitative review) 분석을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.

평가 결과 신장암의 개복술하 고주파 열치료술은 선택된 문헌이 없었고, 복강경하 고주파 열치료술 평가에 포함된 문헌은 총 7편(중재군 336명, 비교군 377명)의 후향적 코호트연구로 모두 신장 부분절제술과의 비교를 다루었다. 안전성 안전성은 합병증 및 이상반응 여부를 통해 평가하였고, 총 7편의 문헌에 대한 메타분석 결과 전체 합병증 발생률의 위험비(risk ratio)는 1.05로 군간 유사한 수준이었다(95% confidence interval, CI 0.62~1.78, I2=0%, p=0.56). 중대한 합병증과 경미한 합병증을 구분하여 제시한 문헌들에 대한 메타분석 결과에서도 발생 위험도는 군간 유의한 차이가 없는 것으로 나타났다. 효과성 효과성에 대한 일차지표는 종양학적 결과, 신기능 변화 여부, 환자만족도(또는 삶의 질), 이차지표는 시술/수술 소요시간, 혈액소실량, 재원기간, 재입원율을 살펴보기로 하였다. 일차지표의 첫 번째 지표인 종양학적 결과에서는 잔존을 다룬 2편에서 모두 잔존 사례가 없었고, 재발을 다룬 6편의 메타분석 결과, 재발 발생 위험비는 1.93 (95% CI 0.48~7.74)으로 통계적 유의성은 없었다. 전이를 다룬 2편에서 1편에서만 중재군에서 1례의 전이 사례가 보고되었고, 생존을 보고한 2편에서는 5년 시점의 전체 생존율, 암특이 생존율, 무재발 생존율, 무전이 생존율, 무질병 생존율 등의 세부 지표에서 군간 유의한 차이를 보이지 않았다. 두 번째 신기능 관련 지표에서 사구체여과율 및 혈청 크레아티닌 등의 지표를 다룬 연구는 5편으로 군간 유의한 차이가 없거나, 군별로 시술 전후 유의한 차이가 발생하지 않아 신기능 보존 정도가 유사한 것으로 나타났다. 기타 신기능을 나타낸 지표로 만성 신질환 신규 발생률은 두 군 모두 0% 및 동측 신장스캔 기능 감소 정도에서도 군간 유의한 차이가 없었다. 세 번째 지표인 환자만족도 또는 삶의 질을 다룬 연구는 없었다. 효과성의 이차지표인 시술/수술 소요시간의 경우, 총 5편 중 4편에서 중재군의 유의한 경감을 보고하였고, 나머지 1편은 유의한 차이가 없었다. 혈액소실량은 6편 전편에서 중재군의 혈액소실 정도가 유의하게 적은 것으로 나타났고, 재원기간은 4편에서 중재군에서 유의한 경감을, 나머지 1편에서는 군간 유의한 차이를 나타내지 않았다. 경제성 진료데이터를 기반으로 총 소요비용을 비교한 2편의 연구(미국)에서 모두 복강경하 RFA가 부분 절제술에 비해 비용절감효과가 있는 치료옵션이라고 제시하였다.

결론 및 제언 신장암의 개복술하 고주파 열치료술을 비교기술과 비교평가할 만한 연구가 없었다. 신장암의 복강경하 고주파 열치료술의 안전성과 효과성은 후향적 코호트연구 7편을 통해 평가하였다. 신장암의 복강경하 고주파 열치료술은 수술적 요법과 비교하여 합병증 및 이상반응 발생률이 유사한 수준이므로 안전한 의료기술로 평가하였다. 동 기술은 수술적 요법과 비교하여 종양학적 결과에서 유의한 차이가 없었고, 신기능 보존 정도도 유사하였으며, 이차지표인 수술 소요시간 및 혈액소실량, 재원기간은 수술적 치료에 비해 더 효과적이거나 유사하였다(근거의 신뢰수준, Very Low). 소위원회는 신장암의 국소 치료법인 복강경하 고주파 열치료술이 제한된 근거 상에서 표준 수술치료인 부분 신절제술과 효과가 유사하다는 근거를 바탕으로 선택문헌의 주 연구대상자인 ‘4cm 미만의 작은 신장암’ 환자에서 동 기술을 효과적인 기술로 평가하였다. 2023년 제8차 의료기술재평가위원회(2023.08.11.)에서는 소위원회 검토 결과와 관련하여 논의하였고, 의료기술재평가사업 관리지침 제4조제10항에 의거 “신장암의 개복술하 고주파 열치료술”과 “신장암의 복강경하 고주파 열치료술”에 대해 다음과 같이 심의하였다. 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상 상황에서 수술적 절제술의 적응증이 되지 않는 환자에서 신장암의 개복술하 고주파 열치료술을 ‘불충분’으로 심의하였다(권고등급: 불충분). 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상 상황에서 수술적 절제술의 적응증이 되지 않는 환자에서 신장암의 복강경하 고주파 열치료술을 ‘조건부 권고함’으로 심의하였다(권고등급: 조건부 권고함).

Radiofrequency ablation for renal cancer - open surgery and laparoscopic approaches

Background Radiofrequency ablation (RFA) for renal cancer is used to excise and coagulate tissue using radiofrequency to increase patient survival rates and quality of life while minimizing kidney function deterioration. This technology, applied using three approaches (open, laparoscopic, and percutaneous surgeries), had been listed as a non-reimbursable practice prior to the implementation of the new health technology assessment system and was subsequently converted to a reimbursable procedure in 2011.

The technology was identified as a candidate for re-assessment through a 2019 needs assessment survey by relevant agencies and internal monitoring in 2022. Subsequently, the eighth 2022 Health Technology Reassessment Committee (August 12, 2022) determined the need for an in-depth review of its clinical safety and effectiveness. Therefore, the aim of this assessment was to review the evidence on the clinical safety and effectiveness of RFA for renal cancer to support rational healthcare resource utilization.

Method The safety and effectiveness of the technique were assessed through a systematic review. All assessment methods were finalized after discussions with the “Subcommittee for RFA for renal cancer” (hereinafter “subcommittee”) in consideration of the objective of the assessment. The subcommittee consisted of seven experts, including two urologists, two radiologists, one nephrologist, one oncologist, and one evidence-based medicine expert.

The key questions for the assessment were “Is open surgical RFA safe and effective for patients with renal cancer?” and “Is laparoscopic RFA safe and effective for patients with renal cancer?” Safety was assessed based on complications and adverse events and effectiveness was assessed based on oncological outcomes, changes in renal function, and patient satisfaction or quality of life as primary outcomes and duration, blood loss, and length of stay (LOS) as secondary outcomes.

Literature searches were conducted using three international and five Korean databases based on the key question. Two reviewers independently screened and selected the literature according to predetermined inclusion and exclusion criteria. Seven studies were selected for the assessment, all of which were retrospective cohort studies. Risk of bias was assessed using the Risk of Bias Assessment Tool for Nonrandomized Studies 2.0. Quantitative analyses were performed when meta-analysis was feasible and qualitative review was used otherwise. The level of evidence of the systematic review results was assessed using the Grading of Recommendations Assessment, Development, and Evaluation approach.

Results There were no studies on open surgical RFA for renal cancer, but seven studies reported data on laparoscopic RFA (336 patients in the intervention group and 377 patients in the comparison group). All seven studies were retrospective cohort studies comparing laparoscopic RFA with partial nephrectomy.

Safety Safety was assessed based on the presence of complications and adverse events. A meta-analysis of seven studies showed that the risk ratio for overall complication rates was 1.05, indicating a similar level between the groups (95% confidence interval [CI] 0.62–1.78, I2 = 0%, p = 0.56). A meta-analysis of studies reporting major and minor complications also showed no significant difference in the risk between the groups.

Effectiveness Effectiveness was assessed based on oncological outcomes, changes in renal function, and patient satisfaction (or quality of life) as primary outcomes and procedural/surgical duration, blood loss, LOS, and readmission rates as secondary outcomes.

Two studies reported data on residual tumors; there were no cases of residual tumors in both studies. A meta-analysis of six studies analyzing recurrence showed a risk ratio of 1.93 (95% CI 0.48–7.74), which was not significantly different between the groups. In the two studies on metastasis, one study reported one case of recurrence in the intervention group. Two studies that reported survival data showed no significant differences in overall survival, cancer-specific survival, recurrence-free survival, metastasis-free survival, or disease-free survival between the groups at 5 years post treatment.

Five studies reported data on glomerular filtration rate and serum creatinine levels; there were either no significant differences between the groups or no significant changes within groups pre- and post-procedure, indicating a similar degree of renal function preservation. No new chronic kidney disease cases were reported in either group, and the degree of decline in ipsilateral renal function, shown using a renal scan, did not significantly differ between the groups.

None of the studies reported data on patient satisfaction or quality of life.

Regarding the secondary outcomes, four of five studies reported significant reductions in procedure/surgery duration, whereas there was no significant difference in one study. Blood loss (six studies) was significantly lower in the intervention group than in the comparison group in all six studies. LOS (four studies) was significantly lower in the intervention group than in the comparison group in three studies but did not significantly differ between the groups in one study.

Cost-effectiveness Two US-based studies comparing total costs using clinical data showed that laparoscopic RFA was a more cost-effective treatment option than partial nephrectomy.

Conclusion and Recommendations No comparative studies on open surgical RFA were available.

The safety and effectiveness of laparoscopic RFA were assessed based on seven retrospective cohort studies. The subcommittee determined that laparoscopic RFA is a safe health technology as evidenced by similar rates of complications and adverse events as those associated with surgical treatment. Comparing RFA and surgical treatment, there were no significant differences in oncological outcomes and there was a similar degree of renal function preservation. The secondary outcomes, such as procedure duration, blood loss, and LOS, were either more favorable in the intervention group than in the surgical treatment group or similar between the groups (level of evidence: very low).

The Subcommittee determined that laparoscopic RFA is an effective technology for small renal cell carcinoma (< 4 cm) (the primary patient population in the included studies) based on limited evidence showing its effectiveness comparable to that of the standard surgical treatment (partial nephrectomy).

The eighth 2023 Health Technology Re-assessment Committee (August 11, 2023) discussed the findings of the subcommittee and, based on articles 10-4 of the Health Technology Reassessment Project Management Guidelines, drew the following conclusions for “Open surgical RFA for renal cancer” and “Laparoscopic RFA for renal cancer.”

The evidence regarding clinical safety and effectiveness and other evaluation criteria is insufficient to recommend open surgical RFA for renal cancer for patients who do not meet the indications for surgical resection in the Korean clinical setting (Grade of recommendation: Insufficient).

Based on the evidence for clinical safety and effectiveness and other evaluation criteria, laparoscopic RFA for renal cancer is conditionally recommended for patients who do not meet the indications for surgical resection in the Korean clinical setting (Grade of recommendation: Conditional).
