동맥압에 기초한 심기능측정 [1일당]

Abstract

평가배경 동맥압에 기초한 심기능 측정은 혈역학적 모니터가 필요한 환자의 상태를 평가하기 위해 기존에 거치된 동맥 카테터에 센서와 모니터를 연결하여 연속적으로 심박출량을 감시하는 기술이다. 2008년 비급여로 등재된 기술로 내부 모니터링 결과 의료기술재평가를 통해 안전성 및 효과성에 대한 최신 근거 마련의 필요성이 확인되어 재평가대상 안건 주제로 발굴되었으며, 우선순위 심의를 통해 재평가 대상으로 선정되었다. 2022년 제11차 의료기술재평가위원회(2022.11.11.)에서는 동맥압에 기초한 심기능 측정과 열희석법 보정후 동맥압에 기초한 심기능 측정의 평가계획서 및 통합 소위원회 구성안을 심의한 후 임상적 안전성 및 효과성을 평가하였다.

평가방법 혈역학적 모니터가 필요한 환자에서 동맥압에 기초한 심기능 측정이 임상적으로 안전하고 효과적인지 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “동맥압에 기초한 심기능 측정, 열희석법 보정후 동맥압에 기초한 심기능 측정 통합 소위원회(이하 ‘소위원회’)”의 논의를 거쳐 확정하였다. 소위원회 구성은 순환기내과 2인, 흉부외과 2인, 호흡기내과(중환자의학) 2인, 마취통증의학과 2인, 근거기반의학 1인의 전문가 총 9인으로 구성하였다. 평가의 핵심질문은 ‘동맥압에 기초한 심기능 측정은 혈역학적 모니터가 필요한 환자에서 임상적으로 안전하고 효과적인가?’이다. 안전성은 검사 관련 이상반응 또는 부작용, 효과성은 의료결과의 영향, 참고표준검사와의 차이, 경제성은 비용절감, 비용 효과성 등으로 평가하였다. 체계적 문헌고찰은 위의 핵심질문을 토대로 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials) 및 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, 사이언스온)를 이용하여 문헌을 검색하였으며, 문헌 선정기준 및 배제기준 적용을 통한 문헌선택과 비뚤림위험 평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치가 있을 경우 회의를 통해 합의하였다. 문헌의 비뚤림위험 평가는 연구유형에 따라 무작위배정 비교임상시험(randomized controlled trial, 이하 ‘RCT’)는 Cochrane의 Risk of Bias (RoB), 그 외에 비무작위 연구(non randomized study, 이하 ‘NRS’)(비무작위 비교임상연구, 코호트 연구)는 Risk of bias Assessment Tool for Nonrandomized Studies (RoBANS) 2.0 국문판, 진단법평가 문헌의 비뚤림위험 평가는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)를 사용하였다. 자료분석에서 의료결과의 영향은 사망률, 전체 임상사건발생, 세부 임상사건 발생(장기 또는 주요 사건 중심), 입원기간, 중환자실 재원기간 인공호흡기 유지기간을 포함하여 양적 분석이 가능할 경우 메타분석을 수행하였으며, 불가능할 경우 질적으로 검토하였다. 참고표준검사와의 차이는 bias, 일치한계(limits of agreements, 이하 ‘LOA’), 백분율 오차(percentage error, 이하 ‘PE’)에 대해 질적 기술하였다. 이 중 bias는 심박출량(cardiac ouput, 이하 ‘CO’) 및 심장박출지수(cardiac index, 이하 ‘CI’)로 구분하여 메타분석을 통해 통합 평균 차이(mean difference, 이하 ‘MD’)를 확인하였다. 결론 도출은 동맥압에 기초한 심기능 측정에 대한 분석결과를 우선적으로 제시하고, 임상적 안전성 및 효과성에 대한 결론은 합의를 통하여 결정하였다. 이러한 평가결과를 토대로 권고등급을 결정하였다.

평가결과 체계적 문헌고찰 결과 선택된 문헌은 총 129편이었다. 연구유형은 RCT 32편, NRS 20편, 진단적 코호트 연구 75편, 경제성 연구 4편으로 이 중 2편은 임상적 효과성과 경제성 모두 보고하였다. 의료결과의 영향을 보고한 연구대상자는 대부분 수술 환자로 소화기 수술(31%)과 심장수술(13%)이 가장 많은 비율을 차지하였다. 비뚤림위험 평가결과 RCT는 무작위배정 순서 생성 영역에서 비뚤림위험 ‘불확실’이 41%, 배정 순서 은폐 영역에서 비뚤림위험 ‘불확실’이 25%, 불완전한 결과 자료 영역에서 비뚤림위험 ‘높음’이 6.3%, 기타 비뚤림위험 ‘높음’이 12.5%로 평가하였다. NRS는 대상군 비교가능성 영역에서 비뚤림위험 ‘높음’이 30%, 교란변수 영역에서 비뚤림위험 ‘높음’이 5%, ‘불확실’이 55%, 불완전한 결과 자료 영역에서 비뚤림위험 ‘높음’이 10%로 평가하였다. 진단적 코호트 연구는 연구대상 선정 영역에서 비뚤림위험 ‘불확실’이 14.7%였으며, 그 외 나머지 항목은 비뚤림위험을 ‘낮음’으로 평가하였다.

안전성 동맥압에 기초한 심기능 측정 검사에 대해 안전성을 보고한 문헌은 8편으로 장비와 관련된 합병증이나 심각한 이상반응은 없었다.

효과성 동맥압에 기초한 심기능 측정 검사의 효과성은 의료결과의 영향과 참고표준검사와의 차이로 평가하였다. 의료결과의 영향은 52편에서 사망률, 임상사건 발생(전체, 신장, 간 및 상처감염 관련), 입원기간, 중환자실 재원기간, 인공호흡기 유지기간을 주요 지표로 평가하였다. 동맥압에 기초한 심기능 측정 검사를 이용하여 수액을 조절한 그룹(이하 ‘중재군’)과 기존 검사를 이용하여 수액을 조절한 그룹(이하 ‘비교군’) 간 비교시 사망률은 RCT에서는 유의한 차이가 없었으나 NRS (6편, 총 734명)는 중재군에서 유의하게 낮았다(통합 OR 0.30, 95% CI 0.09, 0.94, I2 = 0%). 전체 임상사건 발생은 RCT에서는 유의한 차이가 없었으나 NRS (3편, 총 484명)는 중재군이 유의하게 낮았다(통합 OR 0.61, 95% CI 0.41, 0.90, I2 = 27%). 그 외에 신장, 간 관련 임상사건 발생 및 상처 감염은 RCT, NRS 모두 군간 유의한 차이가 없었다. 입원기간(18편, 총 2,304명), 중환자실 재원기간(16편, 총 1,816명), 인공호흡기 유지기간(9편, 총 673명)은 RCT에서 중재군이 유의하게 짧았으나 NRS에서는 군간 유의한 차이가 없었다. 참고표준검사와의 차이는 75편에서 폐동맥 카테터를 이용한 심기능 측정(열희석법 이용)과의 차이를 통해 확인하였다. 메타분석 결과 bias는 CO 기준 36편(총 1,006명)에서 통합 MD –0.15 (95% CI –0.38, 0.09, I2 = 48%), CI 기준 5편(총 110명)에서 통합 MD –0.06 (95% CI –0.62, 0.51, I2 = 85%)로 연구간 이질성은 CO는 중등도, CI는 높게 나타났다. LOA는 CO 기준으로 34편(총 865명)에서 95% 신뢰구간 하한값은 –6.7 L/min ~ -0.41 L/min, 상한값은 –2.89 L/min ~ 7.1 L/min이었으며, CI 기준으로 10편(총 259명)에서 95%CI 하한값 –4.56 L/min ~ 0.05 L/min/m2, 상한값 –1.2 L/min ~ 4.1 L/min/m2이었다. PE는 48편(총 1,260명)에서 전체 평균값은 50.7%, 중앙값은 53.5%로 범위는 20% ~ 82.5%이었다.

경제성 경제성 결과지표를 보고한 문헌은 총 4편으로 연구국가는 중국, 프랑스, 이탈리아, 체코 각각 1편씩이었다. 이 중 분석적 결정모델을 이용한 연구는 1편으로 병원 사망률 및 주합병증 회피 관련 ICER를 확인한 결과 동맥압에 기초한 심기능 측정을 수행한 중재군이 기존 모니터링을 수행한 비교군에 비해 더 비용효과적인 것으로 보고하였다. 중재군과 비교군 간 병원 비용을 비교한 3편으로 이 중 1편은 전체 수술 후 관리에 소요되는 총 비용의 절감 양상이 확인되어 중재검사 및 수술 중 모니터링에 필요한 투자 비용을 상쇄할 수 있는 경향을 보여준다고 보고하였다. 나머지 2편에서는 중재군과 비교군 간 병원 비용에 군간 유의한 차이가 없었다.

결론 동맥압에 기초한 심기능 측정 소위원회는 현재 평가결과에 근거하여 다음과 같이 제언하였다. 동맥압에 기초한 심기능 측정검사는 혈역학적 모니터가 필요한 환자를 대상으로 환자 상태를 평가하기 위해 사용 시 안전성에 문제가 없어 안전한 기술로 판단하였다. 효과성은 선택문헌 대부분에서 수술 중 혈역학적으로 불안정할 수 있는 환자에서 동 검사가 사망률과 전체 임상사건 발생을 감소시키고 입원기간, 중환자실 재원기간 및 인공호흡기 유지기간을 단축시키는 데 도움이 되는 것으로 보고하여, 혈역학적 모니터가 필요한 수술환자에게 효과적인 의료기술로 판단하였다. 경제성은 선행연구의 임상상황과 국내 임상상황이 달라 해당 연구결과만으로 동맥압에 기초한 심기능 측정에 대한 경제성을 확인하기는 어렵다고 판단하였다. 2023년 제8차 의료기술재평가위원회(2023.8.11.)에서는 소위원회 결론을 검토하여 다음과 같이 심의 의결하고 권고등급을 결정하였다. 소위원회는 동맥압에 기초한 심기능 측정을 안전한 기술로 판단하였고 선택문헌 대부분에서 수술 중 혈역학적으로 불안정할 수 있는 환자에서 동 검사가 사망률과 전체 임상사건 발생을 감소시키고 입원기간, 중환자실 재원기간 및 인공호흡기 유지기간을 단축시키는 데 도움이 되는 것으로 보고하여, 혈역학적 모니터가 필요한 수술 환자에게 효과적인 의료기술로 판단하였다. 이에 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 혈역학적 모니터가 필요한 수술 환자에게 심기능 측정 및 감시를 목적으로 사용 시 동맥압에 기초한 심기능 측정을 ‘권고함’으로 심의하였다.

Arterial pressure-based cardiac function monitoring

Background Arterial pressure-based cardiac function monitoring is used for continuous monitoring of cardiac output in patients requiring hemodynamic monitoring and involves connecting a sensor and monitor to an existing arterial catheter. In 2008, this technology was classified as a non-reimbursable item. Based on internal evaluations, it was determined that updated evidence was needed to confirm the safety and effectiveness of this technology through a health technology re-assessment. Accordingly, it was selected for a priority review and ultimately chosen for re-assessment. During the 11th Health Technology Re-Assessment Committee meeting on November 11, 2022, the assessment plan for arterial pressure-based cardiac function monitoring, both with and without calibration by thermodilution, as well as the proposed composition of the combined subcommittee was evaluated, and the clinical safety and effectiveness of the technique were assessed.

Method A systematic review was performed to assess the safety and effectiveness of arterial pressure-based cardiac function monitoring in patients requiring hemodynamic monitoring. All assessment methods were finalized by the “Subcommittee for the Assessment of Arterial Pressure-based Cardiac Function Monitoring and Arterial Pressure-based Cardiac Function Monitoring after Calibration by Thermodilution” (hereinafter, Subcommittee) after thorough discussion. The Subcommittee consisted of nine experts, namely two cardiologists, two thoracic surgeons, two pulmonologists (critical care), two anesthesiologists, and one evidence-based medicine expert. The key question of this assessment was “Is arterial pressure-based cardiac function monitoring clinically safe and effective in patients requiring hemodynamic monitoring?” Safety was assessed based on test-related adverse events or side effects, and effectiveness was evaluated based on the impact on clinical outcomes and differences from the reference standard. Economic evaluation was also performed, focusing on cost savings and cost-effectiveness. The systematic literature review was based on the above key question and involved searches in three international databases (i.e., Ovid MEDLINE, Ovid EMBASE, and Cochrane Central Register of Controlled Trials) and five Korean databases (i.e., KoreaMed, Korea medical database, Koreanstudies information service system, Research information sharing service, and ScienceON). Literature selection based on predefined inclusion and exclusion criteria and assessment of risk of bias were conducted independently by two reviewers. Any disagreements were resolved through consensus discussion. Risk of bias was evaluated according to the study design: randomized controlled trials (RCTs) were assessed using the Cochrane Risk of Bias (RoB) tool, non-randomized studies (NRS) (including non-randomized comparative clinical studies and cohort studies) were assessed using the Korean version of the Risk of Bias Assessment Tool for Nonrandomized Studies (RoBANS 2.0), and diagnostic accuracy studies were evaluated using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). Impact on clinical outcomes was assessed in terms of mortality, overall incidence of clinical events, incidence of specific clinical events (focused on organs or major events), length of hospital stay (LOS), length of intensive care unit (ICU) stay, and duration of mechanical ventilation. Quantitative analysis was conducted through meta-analysis where possible; otherwise, a qualitative review was performed. Differences from the reference standard were qualitatively described in terms of bias, limits of agreement (LOA), and percentage error (PE). Bias was further analyzed through meta-analysis to determine the mean difference (MD) for cardiac output (CO) and cardiac index. Conclusions were primarily based on the results for arterial pressure-based cardiac function monitoring, and the final conclusions regarding clinical safety and effectiveness were determined through consensus. Strength of recommendation was determined based on these results.

Results A total of 129 studies were included in the systematic review. By study design, these studies included 32 RCTs, 75 diagnostic cohort studies, and 4 economic evaluation studies. The majority of participants in studies reporting the impact on clinical outcomes were surgical patients, with gastrointestinal surgery (31%) and cardiac surgery (13%) being the most common studied procedures. The risk of bias assessment yielded the following results: for RCTs, the risk of bias was "uncertain" in 41% of studies for random sequence generation, "uncertain" in 25% for allocation concealment, "high" in 6.3% for incomplete outcome data, and "high" in 12.5% for other sources of bias. For NRS, the risk of bias was "high" in 30% of studies for comparability of study groups, "high" in 5% and "uncertain" in 55% for confounding variables, and "high" in 10% for incomplete outcome data. For diagnostic cohort studies, the risk of bias was "uncertain" in 14.7% of studies for selection of study participants and "low" for the other items. Safety Eight studies reported data on the safety of arterial pressure-based cardiac function monitoring, with no cases of equipment-related complications and serious adverse events reported.

Effectiveness Effectiveness was evaluated based on the impact on medical outcomes and differences from the reference standard. The impact on medical outcomes was assessed in 52 studies in terms of mortality, incidence of clinical events (overall, renal, hepatic, and wound infection-related), LOS, length of ICU stay, and duration of mechanical ventilation. In RCTs, no significant difference was noted in mortality between the group in which fluid levels were adjusted based on arterial pressure-based cardiac function monitoring (hereafter, "intervention group") and the group in which fluid levels were adjusted based on conventional methods (hereafter, "comparison group"). However, in NRS (6 studies, 734 patients), the intervention group had significantly lower mortality than the comparison group (pooled odds ratio (OR) 0.30, 95% confidence interval (CI) 0.09, 0.94, I2 = 0%). In RCTs, no significant difference was observed in the overall incidence of clinical events, but in NRS (3 studies, total of 484 patients), the intervention group had a significantly lower incidence than the comparison group (pooled OR 0.61, 95% CI 0.41, 0.90, I2 = 27%). No significant differences were noted in the incidence of renal and hepatic clinical events and wound infections between the groups in both RCTs and NRS. LOS (18 studies, 2,304 patients), length of ICU stay (16 studies, 1,816 patients), and duration of mechanical ventilation (9 studies, 673 patients) were significantly shorter in the intervention group than in the comparison group in RCTs but not significantly different between the groups in NRS. Differences from the reference standard were assessed in 75 studies, with cardiac function monitoring using a pulmonary arterial catheter (using thermodilution) as the reference standard. Meta-analysis results for bias were as follows: for CO (36 studies, 1,006 patients), the pooled MD was –0.15 (95% CI –0.38, 0.09, I2 = 48%); for cardiac index (5 studies, 110 patients), the pooled MD was –0.06 (95% CI –0.62, 0.51, I2 = 85%), indicating moderate heterogeneity for CO and high heterogeneity for CI. For LOA, the lower limit of the 95% CI was –6.7 L/min to –0.41 L/min and upper limit was –2.89 L/min to 7.1 L/min for CO (34 studies, 865 patients). For cardiac index (10 studies, 259 patients), the lower limit of the 95% CI was –4.56 L/min/m² to 0.05 L/min/m² and upper limit was –1.2 L/min/m² to 4.1 L/min/m². For PE (48 studies, 1,260 patients), the overall mean was 50.7%, with a median of 53.5% and range of 20%–82.5%.

Economic evaluation Four studies reported economic outcomes, with one study each from China, France, Italy, and the Czech Republic. One study used an analytical decision model to assess the incremental cost-effectiveness ratio related to hospital mortality and avoidance of major complications. This study reported that the intervention (arterial pressure-based cardiac function monitoring) was more cost-effective than conventional monitoring. Three studies compared hospital costs between the intervention and comparison groups. One of these studies suggested a trend toward reduced total costs associated with postoperative management, indicating that the initial investment costs for the intervention and intraoperative monitoring could be balanced out. However, the remaining two studies found no significant differences in hospital costs between the intervention and comparison groups.

Conclusion The Subcommittee presented the following recommendations based on the assessment results. The Subcommittee determined arterial pressure-based cardiac function monitoring as a safe technique for assessing the status of patients requiring hemodynamic monitoring. In most of the included studies, this technique was associated with reduced mortality and overall clinical events and shortened LOS, ICU stay, and mechanical ventilation duration in patients who may be hemodynamically unstable during surgery. Therefore, the Subcommittee determined that the technique is effective for health monitoring in surgical patients requiring hemodynamic monitoring. However, the Subcommittee could not conduct a solid economic evaluation for the technique due to contextual differences between the included study setting and the clinical setting in Korea. Based on the Subcommittee assessment results, the final decision was reached, and the strength of recommendation was determined at the eighth 2023 Health Technology Re-Assessment Committee (August 11, 2023) as follows: The Subcommittee determined arterial pressure-based cardiac function monitoring to be a safe technology. Most selected studies reported that this technique helps reduce mortality and overall clinical events and shorten LOS, ICU stay, and mechanical ventilation duration in patients who may be hemodynamically unstable during surgery. Therefore, the Subcommittee determined that arterial pressure-based cardiac function monitoring is an effective health technology for surgical patients requiring hemodynamic monitoring. Based on comprehensive consideration of the evidence for clinical safety and effectiveness and other evaluation criteria, the Health Technology Re-Assessment Committee deliberated and decided to recommend arterial pressure-based cardiac function monitoring for measuring and monitoring cardiac function in surgical patients requiring hemodynamic monitoring in Korea.
