전립선동맥색전술

Abstract

평가배경 전립선 동맥색전술은 전립선비대증 환자에서 전립선으로 들어가는 혈관에 폐색제를 주입하여 혈관을 막아 하부요로증상을 개선하는 시술이다. 전립선 동맥색전술(Prostate Arterial Embolization, PAE) 평가는 신의료기술평가를 받은 후 2019년 선별급여로 등재되었다(보건복지부 고시 제2019-80호, 2019.5.1.). 해당기술은 선별급여 항목의 적합성 평가 주기를 고려하여 내부 모니터링으로 발굴되었고, 2022년 제11차 의료기술재평가위원회(2022.11.12.)에서 의료기술재평가 안건으로 선정되어 재평가를 수행하였다.

평가방법 전립선 비대증 환자에서 전립선 동맥색전술의 임상적 안전성 및 효과성을 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “전립선 동맥색전술 평가 소위원회(이하, 소위원회)”논의를 거쳐 확정하였다. 소위원회 구성은 영상의학과 2인, 비뇨의학과 2인, 근거기반의학 1인의 전문가 총 5인으로 구성하였다. 본 평가의 핵심질문은 ‘전립선 비대증 환자에서 전립선 동맥색전술은 비교시술 대비 안전하고 효과적인가?’이었다. 동 기술의 비교시술은 개복 전립선 적출술, 경요도적 전립선 절제술, 광선택적 전립선 기화술, 홀뮴레이저를 이용한 전립선 광적출술, 수증기를 이용한 경요도 전립선 절제술, 이식형 결찰사를 이용한 전립선 결찰술, 거짓치료와 약물치료를 포함하였다. 안전성 지표는 부작용 및 합병증, 효과성 지표는 증상점수, 삶의 질, 요역동학적 지표, 전립선 용적, 전립선 특이항원, 카테터 관련 효과성, 재수술, 만족도 혹은 수술 성공률, 수술시간, 입원기간으로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 3개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가에서 무작위대조군 비교임상시험(Radnomized Controlled Trials, RCT) 연구는 Cochrane의 Risk of Bias (RoB), 비무작위 대조군 비교임상시험(Non-Ranomzed Studies, NRS) 연구는 Risk of Bias Assessment tool for Non-randomised Study (RoBANS)를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 하였다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석(quantitative analysis)과 정성적(qualitative review) 분석을 적용하였다. 체계적 문헌고찰 결과의 근거수준은 Grading of Recommendations Assessment Development and Evaluation (GRADE) 접근 방법으로 평가하였다. 전립선 비대증 환자에 대한 전립선동맥색전술을 비교한 결과는 연구단위로 기술하였다.

평가결과 전립선 비대증 환자에서 전립선 동맥색전술의 안전성, 효과성은 총 15편(총 13개 연구(RCT 5개, NRS 8개))에 근거하여 평가하였다. 평가결과는 연구유형별, 비교기술별로 안정성과 효과성을 정리하였다. 비교기술별 선택연구는 개복 전립선 적출술 NRS 2개, 전립선 경요도 절제술 NRS 8개(RCT 5개와 NRS 3개), 광선택적 전립선 기화술 NRS 1개, 홀뮴레이저를 이용한 전립선 광적출술 NRS 2개, 수증기 이용 경요도 요법 NRS 1개, 이식형 결찰사를 이용한 전립선 결찰술 NRS 1개, 거짓치료와 약물치료 각각 RCT 1개로 총 8개와 비교하였다. 비뚤림 위험은 RoB로 평가한 RCT연구에서는 참여자, 연구자에 대한 눈가림과 결과평가자에 대한 눈가림, 불충분한 결과자료에서 일부 비뚤림위험이 높았고, RoBANS ver. 2.0으로 평가한 NRS연구에서는 대상군 비교가능성, 대상군 선정에서 절반 이상이 비뚤림위험이 높거나 불확실하여 전반적으로 중등도의 위험으로 평가하였다.

안전성 전립선동맥색전술 대비 비교시술의 안전성은 부작용 및 합병증으로 평가하였다. 전립선 동맥색전술과 개복 전립선 적출술를 비교한 연구는 후향적 코호트 연구 2개였다. 전체 부작용 및 합병증은 Clavien–Dindo 3단계를 합성한 결과 두 군간 유의한 차이는 없었다(Risk Ratio (RR) 0.18, 95% Confidence Interval (CI) 0.02~ 1.44, p=0.11, I2=0%). Clavien–Dindo 1-2 단계는 전립선 동맥색전술이 개복 전립선 적출술보다 발생률이 낮았다(RR 0.46, 95% CI 0.24~ 0.89, p=0.02, I2=25%). 개별 부작용 및 합병증은 출혈관련 합병증, 배뇨관련 합병증, 감염, 성기능 관련 안전성으로 성기능 점수(국제발기기능측정기준(International index of erectile function, IIEF)-5가 보고되었으나 두 군을 비교한 결과 대부분의 지표에서 유의한 차이를 검증하지 않았다. 전립선 동맥색전술과 경요도적 전립선 절제술을 비교한 연구는 총 8개이었고, 이 중 RCT는 5개, NRS는 3개이었다. 전체 부작용 및 합병증은 RCT 3개, NRS 1개에서 보고하였다. RCT 3개 중 2개 연구를 합성한 결과 Clavien–Dindo 3단계(RR 0.58, 95% CI 0.21~ 1.60, p=0.29, I2 = 0%)와 1-2단계(RR 0.01, 95% CI 0.93~1.10, p=0.79, I2=0%) 모두에서 두 군간 유의한 차이가 없었다. NRS에서는 1개에서 12개월 시점 Clavien–Dindo 3단계 합병증은 전립선 동맥색전술에서는 발생하지 않았고, 경요도적 전립선 절제술에서 2% (1/48명) 발생하였다. 1~2단계 합병증은 전립선 동맥색전술 33% (5/15명), 경요도적 전립선 절제술 25% (14/48명)이 발생하였지만, 두 군간 유의한 차이는 언급하지 않았다. 개별 부작용 및 합병증은 출혈관련 합병증, 감염, 배뇨관련 합병증, 통증, Transurethral resection (TUR)증후군 혹은 색전 증후군, 성기능 관련 안전성, 형광 투시법 관련 안전성, 재수술 및 재입원, 기타 등을 보고하였고, 두 군간 통계적 유의성을 보고한 연구는 없었다. 형광투시법 관련 안전성은 방사선량과 형광 투시시간으로 평가하였고, 동 기술의 방사선량이 수용가능한 범위라고 언급하거나, 투시시간에 따른 부작용이 없었다고 하였다. 전립선 동맥색전술과 광선택적 전립선 기화술을 보고한 연구는 NRS 1개였다. Clavien–Dindo로 보고한 3단계 합병증은 중재군에서는 발생하지 않았고, 1-2단계 합병증은 전립선 동맥색전술 33% (5/15명), 광선택적 전립선 기화술은 19% (12/62명)가 발생하였다. 전립선 동맥색전술과 홀뮴레이저를 이용한 전립선 광적출술을 보고한 연구는 NRS 2개였고, 전체 합병증 및 부작용을 발생을 합성한 결과 두 군간 유의한 차이가 없었다(RR 0.99, 95% CI 0.69~1.43, p=0.98, I2=0%). 개별 합병증은 출혈관련 합병증, 배뇨관련 합병증, 패혈증, 성기능 관련 안전성, 형광투시법 관련 합병증을 보고하였다. 이 중 요실금은 전립선 동맥색전술과 홀뮴레이저를 이용한 전립선 광적출술에는 발생하지 않았고, 전립선 광적출술에서는 9.3% (46/490명) 발생하여 전립선 동맥색전술 보다 홀뮴레이저를 이용한 전립선 광적출술에서 유의하게 많이 발생하였다(p<0.001). 전립선 동맥색전술과 이식형 결찰사를 이용한 전립선 결찰술은 NRS 1개이었고, 요통, 혈뇨, 요로감염, 불특정 복통, 재입원을 보고하였으나 두 군간 유의한 차이는 없었다. 전립선 동맥색전술과 수증기 이용 경요도 전립선 절제술을 비교한 연구는 NRS 1개이었다. 색전술 후 증후군은 PAE군에서 27% (3/11명) 발생하였다. 혈뇨, 요로감염은 전립선 동맥색전술에서는 언급이 없었고, 수증기 이용 경요도 전립선 절제술에서는 각각 9% (1/11명)씩 발생하였다. 전립선 동맥색전술과 거짓치료와 비교한 연구는 RCT 1개였다. 전체 합병증은 중재군 35% (14/40명), 거짓치료군은 32.5% (12/40명)가 발생하였고 두 군간 유의한 차이는 언급이 없었다. 이외 개별합병증으로 출혈관련 합병증, 감염, 배뇨관련 합병증, 성기능 점수(IIEF), 형광투시법 관련 안전성이 보고되었으나 유의한 차이를 보고하지 않았다. 전립선 동맥색전술과 약물치료와 비교한 연구는 RCT 1개였다. 전체 합병증은 중재군 51건, 약물치료군은 42건 발생하였다. 개별 합병증은 출혈관련 합병증, 감염, 배뇨관련 합병증, 통증, 색전증후군, 성기능 관련 안전성, 형광투시법 관련 안전성 등을 보고하였고, 이 중 성기능은 9개월 시점에 전립선 동맥색전술이 약물치료 보다 더 장애가 적었다(p=0.0235). 형광투시법 관련 안전성으로는 과도한 방사선 조사량으로 보고하였는데 전립선 동맥색전술에서 2.2% (1/45명) 발생하였다.

효과성 전립선동맥색전술 대비 대조군의 효과성은 증상점수, 삶의 질, 요역동학적 지표, 전립선 용적, 전립선 특이항원, 카테터 관련 효과성, 재수술, 만족도 혹은 수술 성공률, 수술시간, 입원기간으로 평가하였다. 전립선 동맥색전술과 개복 전립선 적출술을 비교한 효과성은 NRS 1개에서 증상점수, 삶의 질, 요역동학적 지표, 전립선 특이항원, 카테터 관련 효과성, 재수술, 수술시간, 입원기간을 보고하였다. 이 중 증상점수, 삶의 질은 중재군이 대조군에 비해 효과가 낮았다(p<0.05). 요역동학적 지표는 전립선 동맥색전술에 비해 개복 전립선 적출술에서 더 개선된 효과가 나타났고, 전립선 특이항원은 중재군이 대조군에 비해 통계적으로 유의하게 더 높았다(p<0.05). 카테터 관련 효과성 중 카테터 사용기간(일)은 중재군 평균 0.03±0.16일, 대조군 평균 6.1±2.97일로 확인되어 중재군이 대조군보다 유의하게 짧았다(p<0.01). 전립선 동맥색전술과 경요도적 전립선 절제술을 비교한 연구는 8개 연구가 있었고, 이 중 5개는 RCT, 3개는 NRS이었다. 증상점수와 삶의 질은 RCT 5개 연구 중 4개 연구를 합성한 결과 두 군간 유의한 차이는 없었지만(증상점수: Mean difference (MD) 2.94점, 95% CI –1.35-7.22, p=0.18, I2= 81%, 삶의 질: MD 0.40점, 95% CI–0.26~1.06, p=0.24, I2= 92%), 이질성이 높아 해석에 주의가 필요하다. 이외 합성이 불가능한 1개의 연구에서 증상점수 변화량은 중재군이 대조군에 비해 더 적었다(p<0.05). 요역동학적 지표는 최고요속, 잔뇨량, 배뇨근압으로 평가하였다. 이 중 최고요속을 보고한 RCT 5개를 합성한 결과 전립선 동맥색전술이 경요도적 전립선 절제술보다 더 느리게 나타났고(MD - 6.54m/s, 95% CI –9.45~ -3.64, p<0.00001, I2= 83%), 잔뇨량은 RCT 5개에서 전립선 동맥색전술보다 경요도적 전립선 절제술에서 더 적게 나타났지만(MD - 25.95ml, 95% CI 4.67~47.23, p=0.02, I2= 95%), 이질성이 높아 해석에 주의가 필요하다. 전립선 특이항원은 RCT 4개에서 보고한 최장기 시점을 중심으로 합성한 결과 전립선 동맥색전술보다 경요도적 전립선 절제술에서 더 낮았다(MD 0.46 ng/ml, 95% CI 0.25~0.67,p<0.0001, I2= 0%). 이외 배뇨근압(RCT 1개), 최고 요속(NRS 1개), 잔뇨량(NRS 1개)은 두 군간 차이를 보고하지 않았다. 1개 NRS에서 카테터 관련 효과성은 비약물치료환자에서 카테터 비의존 생존율이 중재군 13.3% (2/15명) 보다 대조군 60.4% (29/48명)에서 생존율이 더 높았다(p<0.002). 수술 후 카테터 사용기간은 PAE군 평균 1.3±1.4일, TURP군 3.3±1.4일로 PAE군이 TURP군에 비해 유의하게 짧았다(p=0.001). 재수술은 RCT 1개에서 전립선 동맥색전술 29% (10/34명)가 발생하였고, 경요도적 전립선 절제술은 언급이 없었다. NRS 2개 중 1개에서는 전립선 동맥색전술 재수술 위험이 2년 시점에 28.5%, 경요도적 전립선 절제술은 3.4%로 중재군이 대조군보다 더 재수술 위험이 높았다. 만족도는 RCT연구 1개에서 1개월 시점에 중재군(평균 88.9±16.5점)이 대조군(평균 65.9±12.2점)보다 더 높았다(p<0.001). 전립선 동맥색전술과 광선택적 전립선 기화술의 효과성 지표는 NRS 1개에서 보고하였다. 효과성 지표는 입원기간(일), 카테터 관련 효과성으로 카테터 제거 성공 환자수, 카테터 비의존(중재 후 카테터 제거) 생존율을 보고하였는데 이들 모두 통계적으로 유의한 차이는 언급이 없었다. 전립선 동맥색전술과 홀뮴레이저를 이용한 전립선 광적출술을 비교한 연구는 NRS 2개이었다. 증상점수는 12개월 시점에 중재군 평균 9.55점, 대조군 평균 2.93점으로 증상개선의 효과는 중재군이 대조군보다 적었다(p<0.001). 삶의 질은 12개월 시점에 전립선 동맥색전술(평균 0.57점)이 홀뮴레이저를 이용한 전립선 광적출술(평균 2.25점) 보다 더 높았다(P<0.001). 최고요속은 12개월 시점에 전립선 동맥색전술(평균 15m/s)이 홀뮴레이저를 이용한 전립선 광적출술(평균 24m/s)보다 더 낮았다(p<0.001). 전립선 용적은 중재군과 대조군간 유의한 차이가 없었고, 전립선 특이항원은 12개월 시점에 전립선 동맥색전술(4.32ml)이 홀뮴레이저를 이용한 전립선 광적출술(2ml)보다 더 높았다(p<0.001). 카테터 관련 효과성 중 카테터 사용기간(일)은 수술 후 PAE군 평균 4.73±7.03일, HoLEP군 평균 1.36±1.39일 발생하여 PAE군 보다 HoLEP군에서 유의하게 더 짧은 기간 사용하였다(p<0.001). 이외 카테터 제거 성공률, 카테터 비의존 생존율의 통계적 유의성에 대한 언급하지 않았다. 전립선 동맥색전술과 이식형 결찰사를 이용한 전립선 결찰술의 효과성을 비교한 NRS 1개에서는 재수술을 보고하였다. 재수술의 위험률은 2년 시점에 전립선 동맥색전술 28.5% (95% CI 23.7-34.2%)에서 이식형 결찰사를 이용한 전립선 결찰술 8.5% (95% CI 8.5% 5.6-12.9%) 보다 더 높게 나타났다. 전립선 동맥색전술과 수증기 이용 경요도 절제술의 효과성을 비교한 연구는 NRS연구 1개로 잔뇨량, 카테터 관련 효과성, 수술시간(분)으로 평가하였다. 카테터 제거 성공률은 12개월 시점에 전립선 동맥색전술(45.4%(5/11명))이 수증기 이용 경요도 절제술(100%(11/11명))보다 성공률이 더 낮았다(p=0.01). 수술시간은 PAE군은 중간값 148분(범위 123~161분), 수증기 이용 경요도 전립선 절제술군 중간값 8분(범위 5~13분)으로 PAE군이 수증기 이용 경요도 전립선 절제술군보다 통계적으로 유의하게 길었다(p<0.001). 잔뇨량에서는 통계적으로 유의한 차이는 없었다. 전립선 동맥색전술과 거짓치료를 비교한 연구는 RCT연구 1개였다. 효과성 지표 중 증상점수, 삶의 질, 요역동학적 지표, 전립선 특이항원, 전립선 용적, 수술시간으로 평가하였다. 증상점수는 6개월 시점에 평가한 결과 전립선 동맥색전술이 거짓치료에 비해 더 증상점수가 낮았다(p<0.0001). 삶의 질은 결과 6개월 시점에 전립선 동맥색전술에서 거짓치료 대비 삶의 질이 유의하게 높았다(p<0.0001). 요역동학적 지표 중 최고요속은 전립선 동맥색전술이 거짓치료 보다 더 빨랐고(p=0.005), 잔뇨량은 6개월 시점에서 중재군이 대조군보다 더 적게 나타났다(p=0.03). 이외 전립선 특이항원과 전립선 용적은 두 군간 유의한 차이가 없었다. 전립선 동맥색전술과 약물치료를 비교한 연구는 RCT 1개였다. 효과성 지표로 증상점수. 삶의 질, 요역동학적 지표, 전립선 특이항원, 전립선 용적, 입원기간을 보고하였다. 그 결과 증상점수와 삶의 질에서 9개월 시점에 전립선 동맥색전술이 약물치료에 비해 개선에 효과가 있었고(p<0.05), 이외 다른 지표에서는 두 군간 유의한 차이가 없었다.

결론 및 제언 소위원회에서는 전립선비대증 환자에서 전립선 동맥색전술에 대해 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 체계적 문헌고찰 결과 전립선 동맥색전술은 개복 전립선 적출술 대비 경미한 합병증이 적게 발생하였고, 일부 요폐와 색전증후군이 확인되었으나 대부분의 연구에서 비교시술 대비 부작용 및 합병중 발생에 차이가 없었다. 또한 시술 중 방사선 조사량도 인체에 위해를 끼칠 만한 수준이 아니므로 안전한 기술로 평가하였다. 효과성에서 전립선 동맥색전술은 증상점수와 삶의 질에 있어 거짓치료와 약물치료보다는 효과적이었으나, 개복 전립선 적출술과 홀뮴레이저를 이용한 전립선 광적출술 보다는 증상점수가 높았고, 경요도적 전립선 절제술과 비교시 증상점수가 높은 경향을 보이나 일부 연구에서 유의한 차이가 없는 결과도 있었다. 삶에 질에 있어서 동 기술은 개복 전립선 적출술에 비해 낮았고, 홀뮴레이저를 이용한 전립선 광적출술 보다는 높았으나 경요도적 전립선 절제술과 비교시 높거나 낮은 연구결과가 혼재하였다. 이에 소위원회는 전립선비대증 환자에서 전립선 동맥색전술은 안전하고, 환자 상태 및 선호를 고려하여 선택적으로 사용할 수 있는 기술로 평가하였다. 따라서 수술적 치료를 원하지 않거나 마취에 제한이 있는 환자에서 동 시술의 적용을 고려해볼 수 있다는 의견이었다. 2024년 제1차 의료기술재평가위원회(2024.1.12.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “전립선 동맥색전술”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 전립선비대증 환자를 대상으로 약물치료나 다른 비수술적·수술적 치료에도 개선효과가 없을 때 검토될 수 있는 치료방법으로 전립선 동맥색전술의 사용을 ‘조건부 권고함’으로 심의하였다.

Prostate Arterial Embolization

Background Prostate arterial embolization (PAE) is a procedure used to alleviate lower urinary tract symptoms in patients with benign prostatic hyperplasia (BPH). It involves injecting embolic agents into the arteries supplying the prostate to obstruct blood flow. PAE was listed as a selectively reimbursable procedure in 2019, following a new health technology assessment (Ministry of Health and Welfare Bulletin 2019–80, May 1, 2019). The procedure was recognized as a candidate for reassessment through internal monitoring and selected for reassessment at the 11th 2022 Health Technology Reassessment Committee (November 12, 2022). Reassessment was conducted accordingly.

Method A systematic literature review was conducted to assess the clinical safety and effectiveness of PAE in patients with BPH. All assessment methods were finalized after deliberations by the Subcommittee for Prostate arterial embolization (hereinafter referred to as the "Subcommittee"). The Subcommittee consisted of five experts, including two radiologists, two urologists, and one expert in evidence-based medicine. The key question was “Is PAE safe and effective compared to alternative procedures for patients with BPH?” The alternative comparative interventions included open prostatectomy, transurethral resection of the prostate (TURP), photoselective vaporization of the prostate (PVP), holmium laser enucleation of the prostate (HoLEP), transurethral microwave therapy (TUMT), prostatic urethral lift (PUL), sham treatment, and pharmacological therapy. Safety was assessed based on adverse events and complications, while effectiveness was assessed using symptom scores, quality of life, urodynamic indices, prostate volume, prostate-specific antigen (PSA) levels, catheter-related outcomes, reoperation rates, satisfaction or success rates, operation duration, and length of hospital stay (LOS). Based on the key questions, literature searches of three international databases and five Korean databases were conducted. Two reviewers independently selected studies based on inclusion and exclusion criteria. The risk of bias was independently assessed by two reviewers using the Cochrane Risk of Bias (RoB) tool for randomized controlled trials (RCTs) and the Risk of Bias Assessment Tool for Non-randomized Studies (RoBANS) for non-randomized studies (NRSs), and consensus was reached. Data extraction was conducted independently by two reviewers using predefined extraction forms, and disagreements were resolved through discussion with a third reviewer. Data were analyzed through quantitative analysis and qualitative review. The level of evidence from the systematic literature review was assessed using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach. The results of PAE for BPH were presented on a per-study basis.

Results The safety and effectiveness of PAE in patients with BPH was assessed based on 15 articles (13 studies; 5 RCTs, 8 NRSs). The results were presented by study type and comparative procedure. The comparative procedures included 2 NRSs on open prostatectomy, 8 studies on TURP (including 5 RCTs and 3 NRS), 1 NRS on PVP, 2 NRS for HoLEP, 1 NRS on TUMPT, 1 NRS on PUL, and 1 RCT each for sham treatment and pharmacological therapy. For RCTs, a high risk of bias was noted in areas such as blinding of participants, researchers, and outcome assessors, as well as incomplete outcome data (assessed using RoB). Among NRSs (assessed using RoBANS ver. 2.0), more than half exhibited a high or uncertain risk of bias in concerning the comparability of study groups and participant selection, leading to an overall moderate risk.

Safety The safety of PAE in comparison with comparative procedures was assessed based on adverse events (AEs) and complications. Two retrospective cohort studies compared PAE with open prostatectomy. For overall AEs and complications, there were no significant difference in the incidence of Clavien–Dindo grade 3 events between the two groups (risk ratio [RR] 0.18, 95% confidence interval [CI] 0.02–1.44, p=0.11, I2=0%). However, for Clavien–Dindo grade 1–2 events, the incidence was lower in the PAE group than that in the open prostatectomy group (RR 0.46, 95% CI 0.24–0.89, p=0.02, I2=25%). The reported AEs and complications included hemorrhage-related complications, urinary-related complications, infections, and sexual function-related safety. Sexual function was assessed using the International Index of Erectile Function (IIEF-5). No significant difference was observed between the groups across most parameters. Eight studies (5 RCTs, 3 NRSs) compared PAE with TURP. The overall incidence of AEs and complications was reported in 3 RCTs and 1 NRS. In 2 out of 3 RCTs, the two groups did not significantly differ in the incidence of Clavien–Dindo grade 3 events (RR 0.58, 95% CI 0.21–1.60, p=0.29, I2=0%), as well as grade 1–2 events (RR 0.01, 95% CI 0.93–1.10, p=0.79, I2=0%). In 1 NRS, Clavien–Dindo grade 3 complications were not observed in the PAE group at 12 months but occurred in 2% (1/48) of patients in the TURP group. For grade 1–2 complications, the incidence was 33% (5/15) in the PAE group and 25% (14/48) in the TURP group, although statistical significance was not reported. The reported AEs and complications included hemorrhage-related complications, infections, urinary-related complications, pain, transurethral resection (TUR) syndrome or embolization syndrome, sexual function-related safety, fluoroscopy-related safety, reoperations, and readmissions. None of the studies reported significant differences in these parameters. Fluoroscopy-related safety was evaluated by radiation dose and fluoroscopy time. The studies mentioned that the radiation dose was within an acceptable range or that there were no AEs observed according to fluoroscopy time. One NRS compared PAE with PVP. Although Clavien–Dindo grade 3 complications were not observed in the PAE group, grade 1–2 complications occurred in 33% (5/15) of patients in the PAE group and 19% (12/62) of patients in the PVP group. Two NRSs compared PAE with HoLEP. There was no significant difference in the overall incidence of complications and AEs between the two groups (RR 0.99, 95% CI 0.69–1.43, p=0.98, I2=0%). The reported complications included hemorrhage-related complications, urinary-related complications, sepsis, sexual function-related safety, and fluoroscopy-related complications. Notably, urinary incontinence did not occur in the PAE group but was significantly more frequent in the HoLEP group with an incidence of 9.3% (46/490) (p<0.001). One NRS compared PAE with PUL. The reported complications included back pain, hematuria, urinary tract infections, unspecified abdominal pain, and readmissions; however, there was no significant differences between the groups. One NRS compared PAE with water vapor transurethral ablation. Embolization syndrome occurred in 27% (3/11) of patients in the PAE group. Furthermore, although hematuria and urinary tract infections were not reported in the PAE group, each complication occurred in 9% (1/11) of patients in the water vapor transurethral ablation group. One RCT compared PAE with a sham treatment. The overall incidence of complications was 35% (14/40) in the intervention group and 32.5% (12/40) in the sham treatment group, with no significant difference between the two groups. The reported complications included hemorrhage-related complications, infections, urinary-related complications, IIEF scores, and fluoroscopy-related safety. No significant differences were reported. One RCT compared PAE with pharmacological treatment. The PAE group experienced 51 complications, while the pharmacological treatment group had 42 complications. The reported complications encompassed hemorrhage-related complications, infections, urinary-related complications, pain, embolization syndrome, sexual function-related safety, and fluoroscopy-related safety. At nine months, impairment of sexual function in the PAE group was less than that reported in the pharmacological treatment group (p=0.0235). Fluoroscopy-related safety was reported in terms of excessive radiation exposure and occurred in 2.2% (1/45) of patients in the PAE group.

Effectiveness The effectiveness of PAE compared to other comparative procedures was assessed using symptom scores, quality of life, urodynamic indices, prostate volume, PSA levels, catheter-related outcomes, reoperation rates, satisfaction or success rates, operation duration, and LOS. Regarding the effectiveness of PAE in comparison with open prostatectomy, 1 NRS reported on symptom scores, quality of life, urodynamic parameters, PSA levels, catheter-related effectiveness, reoperation, operation duration, and LOS. Among these parameters, symptom scores and quality of life were lower in the intervention group than those in the control group (p<0.05). Urodynamic parameters showed greater improvement in the open prostatectomy group than that observed in the PAE group, and PSA levels were significantly higher in the intervention group than those in the control group (p<0.05). For catheter-related effectiveness, the catheterization duration (in days) was significantly shorter in the intervention group (mean 0.03±0.16 days) than that in the control group (mean 6.1±2.97 days) (p<0.01). Eight studies (5 RCTs, 3 NRSs) compared PAE with TURP. Data on symptom scores and quality of life were synthesized from 4 of the 5 RCTs. There were no significant differences between the groups (symptom scores: mean difference [MD] 2.94 points, 95% CI -1.35–7.22, p=0.18, I2=81%; quality of life: MD 0.40 points, 95% CI -0.26–1.06, p=0.24, I2=92%), but the high heterogeneity suggests that caution is needed in the interpretation of these results. The remaining study reported a smaller change in symptom scores in the intervention group compared to that observed in the control group (p<0.05). Urodynamic parameters included peak urine flow rate, post-void residual volume, and detrusor pressure. A meta-analysis of 5 RCTs reporting peak flow rate showed that the peak urine flow rate was slower in the PAE group compared to that in the TURP group (MD -6.54 m/s, 95% CI -9.45 to -3.64, p<0.00001, I2=83%), and post-void residual volume was higher in the PAE group than that in the TURP group (MD -25.95 mL, 95% CI 4.67–47.23, p=0.02, I2=95%), with high heterogeneity requiring careful interpretation. For PSA levels, the data reported by 4 RCTs (based on the longest follow-up) were synthesized. The results showed that PSA levels were lower in the TURP group than those in the PAE group (MD 0.46 ng/mL, 95% CI 0.25–0.67, p<0.0001, I2=0%). For other parameters, such as detrusor pressure (1 RCT), peak urine flow rate (1 NRS), and post-void residual volume (1 NRS), inter-group differences were not reported. Regarding catheter-related outcomes, 1 NRS indicated a higher catheter-free survival rate in the control group (60.4%, 29/48) than that in the intervention group (13.3%, 2/15) among patients not treated pharmacologically (p<0.002). The duration of postoperative catheter use was significantly shorter in the PAE group (mean 1.3±1.4 days) compared to that in the TURP group (mean 3.3±1.4 days) (p=0.001). Reoperation rates were assessed in 1 RCT, with a reported rate of 29% (10/34) in patients in the PAE group, although there was no mention of reoperation rates in the TURP group. One NRS found a higher risk of reoperation at two years in the PAE group (28.5%) than that in the TURP group (3.4%). Furthermore, satisfaction was higher in the intervention group at one month post-procedure (mean 88.9±16.5 points) than in the control group (mean 65.9±12.2 points) (p<0.001). Another NRS compared the effectiveness of PAE with PVP. Effectiveness was assessed based on LOS (days) and catheter-related outcomes, specifically the number of patients with successful catheter removal and catheter-free survival rate (survival after catheter removal). No significant differences were reported in these parameters between the groups. Two NRSs compared PAE with HoLEP. At 12 months post-treatment, the mean symptom score was 9.55 in the intervention group and 2.93 in the control group, showing less improvement of symptoms in the intervention group (p<0.001). Additionally, the quality of life at 12 months was higher in the PAE group (mean score of 0.57) than that in the HoLEP group (mean score of 2.25) (p<0.001). Peak urine flow rate at 12 months was lower in the PAE group (mean of 15 m/s) than that in the HoLEP group (mean of 24 m/s) (p<0.001). There was no significant difference in prostate volume between the groups, but PSA levels were higher in the PAE group (4.32 ml) than those in the HoLEP group (2 ml) at 12 months (p<0.001). For catheter-related outcomes, the duration of catheter use was significantly shorter in the HoLEP group (mean 1.36±1.39 days) than that in the PAE group (mean 4.73±7.03 days) (p<0.001). No statistical significance was mentioned for catheter removal success rate and catheter-free survival. One NRS comparing PAE with PUL reported on reoperation rates. The risk of reoperation at two years 28.5% (95% CI 23.7–34.2%) in the PAE group and 8.5% (95% CI 5.6–12.9%) in the PUL group. One NRS compared the effectiveness of PAE with water vapor transurethral ablation based on post-void residual volume, catheter-related outcomes, and operation duration (minutes). At 12 months, the catheter removal success rate was lower in the PAE group (45.4% (5/11)) than that in the water vapor ablation group (100% (11/11)) (p=0.01). The median operation duration was significantly longer in the PAE group (median 148 minutes, range 123–161) than that in the water vapor ablation group (median 8 minutes, range 5–13) (p<0.001). There was no significant difference in post-void residual volume. One RCT compared PAE with sham treatment. Effectiveness was assessed based on symptom scores, quality of life, urodynamic parameters, PSA, prostate volume, and surgery duration. At 6 months, the PAE group had significantly lower symptom scores than the sham treatment group (p<0.0001), with a significantly higher quality of life (p<0.0001). The PAE group also showed a higher peak urine flow rate than that in the sham treatment group (p=0.005), with a lower post-void residual volume at six months (p=0.03). There were no significant differences between the groups in PSA levels and prostate volume. One RCT compared PAE with pharmacological treatment. Effectiveness was assessed based on symptom scores, quality of life, urodynamic parameters, PSA, prostate volume, and LOS. At nine months, the PAE group showed a greater improvement in symptom scores and the quality of life compared to the pharmacological treatment group (p<0.05). No significant differences were observed in other outcomes between the groups.

Conclusion and Recommendations Based on the current assessment results, the Subcommittee made the following recommendations regarding PAE for patients with BPH: Systematic review results indicate that PAE has fewer minor complications compared to open prostatectomy. Although some instances of urinary retention and embolization syndrome were observed, most studies reported no significant differences in the incidence of AEs and complications between PAE and comparative procedures. Additionally, radiation exposure during PAE was not at levels considered harmful to the human body. Taken together, the Subcommittee determined that the technology is safe. In terms of effectiveness, PAE was more effective than sham treatment and pharmacological treatment in improving symptom scores and the quality of life. However, symptoms scores were higher for PAE than for open prostatectomy and HoLEP. Additionally, the PAE group exhibited higher symptom scores compared to TURP; however, some studies found no significant differences. Quality of life was lower in patients undergoing PAE than that in patients undergoing open prostatectomy but higher than that in patients undergoing HoLEP. Comparisons with TURP regarding quality of life yielded mixed results. Therefore, the Subcommittee concluded that prostate arterial embolization (PAE) is a safe technology for patients with benign prostatic hyperplasia (BPH), particularly when considering individual patient conditions and preferences. The Subcommittee suggested that PAE could be considered for patients who do not wish to undergo surgery or have limitations regarding anesthesia. Based on the Subcommittee assessment and Article 4-10 of the Health Technology Reassessment Project Management Guidelines, the following conclusions were drawn for prostate arterial embolization at the 1st 2024 Health Technology Reassessment Committee (January 12, 2024): Based on the evidence on the clinical safety, effectiveness, and other evaluation criteria, the Health Technology Reassessment Committee determined that PAE is “conditionally recommended” as an alternative treatment option for patients with BPH who are not responsive to pharmacological treatment or other non-surgical or surgical treatments.
