누도내시경 검사[편측]

Abstract

평가배경 누도내시경 검사[편측](Dacryoendoscopy)는 누도질환 (의심) 환자를 대상으로 누도내시경을 이용하여 누도 내부를 직접 관찰하여 병변의 원인, 폐쇄 정도 및 위치를 확인하고자 수행하는 검사이다. 누도내시경 검사[편측]는 2018년 신의료기술평가를 받은 이후 2019년 8월부터 선별급여 80%로 적용되었고, 선별급여 도입 후 4년이 경과하여 해당 기술의 임상적 안전성 및 효과성을 확인하고 보건의료자원의 효율적 사용을 위한 근거를 제공하기 위하여 재평가를 수행하였다.

평가방법 누도내시경 검사[편측]에 대한 안전성, 효과성을 평가하기 위해 체계적 문헌고찰을 하였다. 모든 평가방법은 평가목적을 고려하여 “누도내시경 검사[편측]에 대한 안전성 및 효과성 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 평가의 핵심질문은 “누도질환(의심) 환자를 대상으로 병변및 의료결과에대한 영향 확인에 있어 누도내시경검사가 안전하고 효과적인가?”이다. 평가지표로 안전성은 검사 관련 합병증 지표로, 효과성은 2개로 범주화하여 병변 및 눈물길 확인, 의료결과에 대한 영향 확인 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 Risk of Bias for Nonrandomized Studies (RoBANS Ver.2)를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 질적 검토(qualitative review) 방법을 적용하였다. 안전성 결과지표는 검사 관련 합병증을 보았고, 효과성의 결과지표는 병변 및 눈물길 확인, 의료결과에의 영향으로 크게 2가지로 범주화하여 제시하였다. 병변 및 눈물길 확인에는 폐쇄정도, 폐쇄위치, 병변원인의 내용으로 제시하였고, 의료결과에의 영향은 치료율, 치료방법 결정의 내용으로 제시하였다.

평가결과 누도내시경 검사[편측]의 재평가에 포함된 문헌은 총 36편(비교연구 9편, 증례연구 27편)의 연구가 포함되었다. 비교연구 9편 중 누낭조영술과의 비교 6편, 관류검사와의 비교 6편, 탐침법과의 비교 1편이었다. 비뚤림위험 수준은 평가자의 눈가림에서 ‘높음’이 66.7%로 비뚤림위험이 높았다. 안전성, 효과성의 결과를 정리하면 다음과 같다.

안전성 누도내시경 검사[편측]의 안전성 결과를 보고한 문헌은 8편이었으며, 1편의 비교연구에서 부종, 찢김과 같은 합병증 비율에 대해 보고하였으며, 누도내시경 검사를 받은 군에서 일반 검사를 한 군(routine diagnosis)과 비교시 합병증(부종, 찢김) 비율이 유의하게 낮았다. 나머지 7편의 증례연구에서 동 검사 관련 합병증이 발생하지 않았다고 보고하였다.

효과성 효과성 결과지표는 병변 및 눈물길 확인, 의료결과에의 영향으로 크게 2가지로 범주화하였다. 병변 및 원인 확인은 폐쇄정도, 폐쇄위치, 병변원인의 내용등을 포함하였고, 의료결과에의 영향은 치료율, 치료방법 결정의 내용으로 평가하였다.

병변 및 눈물길 확인 폐쇄정도는 총 6편(비교연구 4편, 증례연구 2편)에서 보고하였다. 1편의 비교연구에서 기초검사에 누도내시경 검사를 추가한 군이 기초검사군에 비해 완전폐쇄 발견율이 유의하게 높았다. 2편의 비교연구에서 누도내시경 검사는 누낭조영술과 유사한 수준의 부분 또는 완전폐쇄 발견율을 보고하였으며, 관류검사는 그에 비해 발견율이 낮았다. 단, 군간 통계적 유의성은 보고하지 않았다. 1편의 비교연구에서 누도내시경 검사가 관류검사보다 협착 또는 폐쇄 발견율이 더 높았으나, 두 군간 통계적 유의성은 언급이 없었다. 증례연구 2편은 누도내시경 검사를 이용하여 폐쇄 중증도를 판단하였고, 1편은 누소관에서 개통(정상) 36.7%, 협착 34.2%, 경도 폐쇄 9.2%, 중등도 폐쇄 20%였다. 비루관에서는 개통(정상) 56.7%, 협착 20%, 경도 폐쇄 3.3%, 중등도 폐쇄 20%였다. 나머지 1편은 선천성 비루관 폐쇄증 환자를 대상으로 폐쇄 중증도를 판단한 결과, 경증 3.8%, 중등도 48.1%, 중증 48.1%이었다. 폐쇄위치는 총 19편(비교연구 5편, 증례연구 14편)에서 보고하였다. 2편의 비교연구에서 누도내시경 검사의 누낭 폐쇄 발견율을 보고하였다. 누도내시경 검사와 누낭조영술은 누낭 폐쇄 발견율에 있어 유사한 수준이었으나, 두 군 차이에 대한 통계적 유의성은 보고되지 않았다. 3편의 비교연구에서 누소관 폐쇄 발견율과 비루관 폐쇄 발견율을 보고하였다. 1편에서는 기초검사에 누도내시경 검사를 추가한 군에서의 누소관 폐쇄 발견율이 기초검사군에 비해 낮았고 비루관 폐쇄 발견율은 기초검사군에 비해 높았다. 단 두 군 차이에 대한 통계적 유의성은 보고되지 않았다. 나머지 2편은 누도내시경 검사에서의 누소관 폐쇄 발견율이 누낭조영술과 유사한 수준이었으나, 두 군 차이에 대한 통계적 유의성은 보고되지 않았다. 증례연구 14편에서는 누도내시경 검사로 누낭(2편, 14%~37.5%), 누도관(2편, 12.5%~36.1%), 누소관(12편, 21.3%~89.2%), 눈물점(1편, 12.5%), 비루관(11편, 35.7%~100%), 코안(3편, 3.3%~12.5%)에서의 폐쇄를 보고하였다. 병변원인은 총 23편(비교연구 4편, 증례연구 19편)에서 보고하였다. 다만, 누낭조영술, 관류검사, 탐침법과 같은 비교검사는 병변원인을 확인하지 못하고, 누도내시경 검사에서만 확인 가능하였다. 4편의 비교연구에서는 누도내시경 검사로 확인된 병변원인으로 농(1편, 21.2%), 누석(4편, 9.1%~22.6%), 섬유화 막(1편, 19.2%), 육아조직(3편, 8.9%~25.8%), 점액(3편, 22.6%~39.9%), 협착(3편, 2.7%~32.3%)이 보고되었다. 증례연구 19편에서 누도내시경 검사로 확인된 병변원인으로 누석(8편, 0.8%~30.8%), 막(6편, 0.5%~60.7%; 1편(만성염증으로 인한 반흔), 100%), 육아조직(5편, 10.5%~23.5%), 점액(5편, 12.5%~44.2%), 협착(11편, 14.3%~100%)), 부종(2편, 1.2%~18.8%), 오경로(2편, 2%~2.2%) 등으로 보고하였다.

의료결과에의 영향 치료율은 총 2편의 비교연구에서 보고하였다. 1편에서는 누도내시경 검사군에서 일반 검사를 한 군에 비해 치료(효과)율이 유의하게 더 높았다. 나머지 1편은 기초검사에 누도내시경 검사를 추가한 군에서 기초검사군에 비해 실리콘관 삽입술 성공률이 높았으나 두 군간 유의한 차이가 없었다. 치료방법 결정은 총 7편(비교연구 1편, 증례연구 6편)에서 보고하였다. 7편 연구 모두 누도내시경 검사를 통해 병변원인(예: 점액 과다분비, 누석 등)을 확인하고 그에 맞는 치료방법(예: 누낭비강문합술 , 내시경하 실리콘관 삽입술)을 결정하였다.

결론 및 제언 해당 소위원회는 현재 평가결과에 근거하여 다음과 같이 제언하였다. 소위원회는 누도내시경 검사로 부종, 찢김과 같은 합병증이 발생할 수 있으나 실명, 미란과 같은 심각한 합병증은 보고되지 않아 누도내시경 검사를 안전한 기술로 판단하였다. 또한 동 검사는 누낭조영술과 같은 기존 검사와 유사하게 폐쇄위치, 정도를 확인할 수 있으나 본 평가에서 검토한 문헌을 기준으로 기존 검사들과 검사정확도의 정량적 비교는 어려웠다. 그러나 기존 검사에서 확인할 수 없는 병변 원인을 누도내시경 검사를 통해 임상의가 직접 병소를 확인할 수 있고 이를 토대로 적절한 치료행위를 선택할 수 있어 동 검사를 누도질환(의심)환자에서 진단검사방법 중 하나로 사용할 수 있다고 판단하였다. 2023년 제10차 의료기술재평가위원회(2023.10.13.)에서는 소위원회 검토 결과에 대하여 논의한 결과, 의료기술재평가사업 관리지침 제4조제10항에 의거 “누도내시경 검사[편측]”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 누도질환(의심)환자를 대상으로 병변 및 의료결과에 대한 영향을 확인하는 검사로 부종, 찢김과 같은 합병증이 발생할 수 있으나 기존 검사에서 확인할 수 없는 병변원인을 임상의가 직접 확인할 수 있고 이를 토대로 적절한 치료행위를 선택할 수 있어 누도내시경 검사[편측]의 사용을 ‘조건부 권고함’으로 심의하였다.

Dacryoendoscopy [Unilateral]

Background Dacryoendoscopy (unilateral), a procedure used to directly visualize the nasolacrimal duct lumen, is performed in patients with (suspected) nasolacrimal duct disorders. It employs an endoscope to assess the cause and degree of obstruction and the location of any lesions.

Following the Health Technology Assessment in 2018, dacryoendoscopy (unilateral) was approved for selective reimbursement at an 80% copayment from August 2019. As four years have passed since its adoption, a reassessment was conducted to confirm its clinical safety and effectiveness and to provide evidence for the efficient use of healthcare resources.

Method The safety and effectiveness of dacryoendoscopy (unilateral) were assessed through a systematic review. All assessment methods were finalized following deliberations by the "Subcommittee for Dacryoendoscopy (unilateral) (hereinafter referred to as the "Subcommittee"). The key question of the assessment was: “Is dacryoendoscopy (unilateral) safe and effective in examining lesions and improving medical outcomes in patients with (suspected) nasolacrimal duct disorders?” Safety was evaluated based on complications related to the procedure, while effectiveness was assessed in two main areas: lesion identification and tear duct status and the procedure’s impact on medical outcomes.

To address the key question, a literature search of three international databases and five Korean databases were conducted. Two reviewers independently selected studies based on the inclusion and exclusion criteria. The risk of bias was assessed independently by two reviewers using the Cochrane Risk of Bias (RoB) tool, and a consensus was reached. Data extraction was conducted independently by two reviewers using predefined extraction forms, and disagreements were resolved through discussion with a third reviewer. Data were analyzed using a qualitative review method. Safety was assessed based on complications related to the procedure, while effectiveness was assessed based on lesion identification and tear duct status and the impact on medical outcomes. Lesion and tear duct status were assessed in terms of the degree of obstruction, location of the obstruction, and the cause of the lesion. The impact on medical outcomes was evaluated based on treatment success rates and subsequent treatment decisions.

Results A total of 36 studies (9 comparative studies and 27 case series) were included in the reassessment of dacryoendoscopy (unilateral). Among the 9 comparative studies, 6 compared dacryoendoscopy with dacryocystography, 6 with irrigation tests, and 1 with probing. The overall risk of bias was high, with 66.7% of the studies exhibiting a high risk of bias primarily due to lack of blinding of the assessors. The results for safety and effectiveness are summarized below.

Safety In total, 8 studies reported on the safety of dacryoendoscopy (unilateral). Among these, a comparative study found that complication rates, including edema and tearing, were significantly lower in the dacryoendoscopy group compared to the routine diagnosis group. In contrast, the other 7 case series reported no complications related to the procedure.

Effectiveness Effectiveness was assessed based on identification of the lesion and tear duct status and the impact on medical outcomes. Identification of the lesion and tear duct status were assessed in terms of degree of obstruction, location of obstruction, and cause of the lesion, and the impact on medical outcomes was assessed in terms of treatment success rates and treatment decisions.

Identification of lesion and tear duct The degree of obstruction was reported in 6 studies (4 comparative studies and 2 case series). Among them, 1 comparative study found that the addition of dacryoendoscopy to the routine tests significantly increased the detection rate of complete obstructions compared to the use of basic tests alone. Furthermore, 2 comparative studies reported similar detection rates between dacryoendoscopy and dacryocystography for partial or complete obstruction. Overall, irrigation tests had lower detection rates. However, no significant differences were reported between the groups. Another comparative study found that dacryoendoscopy had a higher detection rate of stenosis or obstructions than irrigation tests, though statistical significance was not mentioned. Furthermore, 2 case series assessed the severity of obstruction using dacryoendoscopy. In 1 study, the findings in the canaliculus were as follows: patent (normal) in 36.7% of cases, stricture in 34.2%, mild obstruction in 9.2%, and moderate obstruction in 20%. For the nasolacrimal duct, the results were: patent (normal) in 56.7% of cases, stricture in 20%, mild obstruction in 3.3%, and moderate obstruction in 20%. The remaining study, which involved patients with congenital nasolacrimal duct obstruction, found mild obstruction in 3.8% of cases, moderate obstruction in 48.1%, and severe obstruction in 48.1%.

The location of the obstruction was reported in 19 studies (5 comparative studies and 14 case series). Among them, 2 comparative studies reported the rates of nasolacrimal duct obstruction detection between dacryoendoscopy and dacryocystography. Although the rates were similar, no significant difference was reported between the groups. Additionally, 3 comparative studies reported on the detection rates of canalicular and nasolacrimal duct obstruction. In 1 study, the addition of dacryoendoscopy to routine tests decreased the detection of canalicular obstruction but increased the detection of nasolacrimal duct obstruction compared to routine tests alone. However, no significant difference was observed between the groups. In the 2 remaining studies, the rates of detection of canalicular obstruction were similar between dacryoendoscopy and dacryocystography; however, no significant difference was reported between the procedures. In 14 case series, dacryoendoscopy was used to detect obstructions in the lacrimal sac (2 studies, 14%–37.5% of cases), nasolacrimal duct (2 studies, 12.5%–36.1%), canaliculus (12 studies, 21.3%–89.2%), punctum (1 study, 12.5%), nasolacrimal duct (11 studies, 35.7%–100%), and nasal cavity (3 studies, 3.3%–12.5%).

The cause of the lesion was reported in 23 studies (4 comparative studies and 19 case series). However, the cause of the lesion could not be identified through comparative procedures such as dacryocystography, irrigation tests, and probing; the cause of obstruction was only identified with dacryoendoscopy. The 4 comparative studies identified the following causes of obstruction through dacryoendoscopy: pus (1 study, 21.2% of cases), dacryoliths (4 studies, 9.1%–22.6%), fibrous membranes (1 study, 19.2%), granulation tissue (3 studies, 8.9%–25.8%), mucus (3 studies, 22.6%–39.9%), and stenosis (3 studies, 2.7%–32.3%). The 19 case studies reported the following causes: dacryoliths (8 studies, 0.8%–30.8% of cases), membranes (6 studies, 0.5%–60.7%, with 1 study reporting 100% due to chronic inflammation-induced scarring), granulation tissue (5 studies, 10.5%–23.5%), mucus (5 studies, 12.5%–44.2%), stenosis (11 studies, 14.3%–100%), edema (2 studies, 1.2%–18.8%), and false passages (2 studies, 2%–2.2%).

Impact on medical outcomes Treatment success rate was reported in 2 comparative studies. In 1 study, the dacryoendoscopy group exhibited a significantly better treatment success rate than that in the routine diagnosis group. The other study found that adding dacryoendoscopy to routine examination increased the success rate of silicone tube insertion compared to that with routine examination alone, although the difference between the two groups was not significant.

The impact on treatment decisions was reported in 7 studies (1 comparative study and 6 case series). All 7 studies utilized dacryoendoscopy to identify the cause of the obstruction (e.g., excessive mucus secretion, dacryoliths), which informed the choice of the appropriate treatment modality (e.g., dacryocystorhinostomy, endoscopic silicone tube insertion).

Conclusion and Recommendations Based on the current assessment results, the Subcommittee made the following recommendations.

Considering that although dacryoendoscopy may lead to minor complications such as edema and tearing, no severe complications such as blindness or erosion were reported, the Subcommittee determined that the technique is safe. Additionally, the Subcommittee noted that dacryoendoscopy effectively identifies the location and severity of obstruction, similar to conventional tests such as dacryocystography. However, the included literature did not permit a quantitative comparison of the diagnostic accuracy between dacryoendoscopy and other tests. Importantly, dacryoendoscopy allows clinicians to directly observe lesions that are not detectable by other methods, enabling them to select the most appropriate treatment. Therefore, the Subcommittee recommended that dacryoendoscopy be considered as one of the diagnostic methods for patients with (suspected) lacrimal duct disorders.

Based on the subcommittee assessment and Article 4-10 of the Health Technology Reassessment Project Management Guidelines, the following conclusion were drawn for “Dacryoendoscopy [Unilateral]” at the 10th 2023 Health Technology Reassessment Committee (October 13, 2024):

Based on the evidence for clinical safety, effectiveness, and other assessment criteria, the Health Technology Reassessment Committee concluded that dacryoendoscopy [unilateral] can be “conditionally recommended” for use among patients with (suspected) nasolacrimal duct disorders as a test for identifying lesions and impact on medical outcomes in the Korean clinical setting. This recommendation is due to its ability to directly observe and diagnose lesions that are not detectable by other methods, thus guiding appropriate treatment decisions despite the potential for minor complications such as edema and tearing.
