고주파 자궁근종용해술

Abstract

평가배경 고주파 자궁근종용해술은 자궁근종, 자궁선근증 환자를 대상으로 고주파 전극을 삽입 후 열을 가해 근종을 괴사시켜 근종으로 인해 발생하는 월경과다, 부정기출혈, 하복부 통증, 복부 압박감과 같은 증상을 완화시키는 기술이다. 신의료기술평가 도입 전 2007년 비급여로 등재되었으며(보건복지부 고시 제2007-104호, 2007.11.22.), 내부 모니터링을 통해 재평가 안건으로 발굴되었다. 의료기술재평가위원회의 우선순위 심의를 거쳐 재평가가 필요한 기술로 선정되었다. 본 평가의 목적은 자궁근종 및 자궁선근증 환자에서 고주파 자궁근종용해술의 임상적 안전성 및 효과성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하고자 하였다.

평가방법 자궁근종 및 자궁선근증 환자에서 고주파 자궁근종용해술의 임상적 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “고주파 자궁근종용해술을 이용한 자궁근종 및 자궁선근증 평가 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 평가의 핵심질문은 ‘고주파 자궁근종용해술이 자궁근종 및 자궁선근증 환자를 대상으로 증상을 개선하는데 안전하고 효과적인 의료기술인가?’로 체계적 문헌고찰을 수행하였고, 의료결과는 안전성 지표로 시술 관련 부작용 및 이상반응, 임신 관련 합병증, 재입원율을 확인하였고, 효과성 지표로 자궁근종 및 선근증 크기(갯수) 감소, 증상개선, 증상중증도 점수, 건강 관련 삶의 질을 확인하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 3개 데이터베이스에서 검색하였으며, 문헌선정 과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림 위험 평가는 무작위배정 비교임상시험연구(Randomized controlled trial, RCT)는 Risk of bias (RoB)를 사용하여 평가하였으며, 비무작위연구(Non randomized study, NRS)는 Risk of bias assessment tool for non- randomized studies (RoBANS Ver 2.0) 도구를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 최종 선택한 문헌을 바탕으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 본 평가에서 모든 자료는 양적 합성이 불가능하여 질적 검토 방법을 적용하였으며, 수행한 체계적 문헌고찰 결과는 Grading of recommendations assessment, development and evaluation (GRADE) 방법을 이용하여 근거 수준을 평가하고, 평가결과를 토대로 권고등급을 결정하였다. 또한, 환자가치 조사는 자궁근종 및 자궁선근증 환자를 대상으로 초점집단면담조사를 통해 질환 경험 및 의료기술에 대한 이해도, 치료결정, 치료과정과 치료 후 환자가 느끼는 불편함 등을 평가하였다.

평가결과 체계적 문헌고찰 결과 선택문헌은 총 10편이었다. 이 중 자궁근종 환자 대상 문헌은 8편(6개 연구)으로 RCT 6편(4개 연구), 후향적 코호트 2편이었다. 비교시술별로는 비수술치료와 비교한 1편, 수술치료와 비교한 7편(5개 연구)이었다. 자궁선근증 환자 대상 문헌은 2편으로, RCT 1편, 후향적 코호트 1편이었고 모두 수술치료와 비교하였다. 질환별 특성이 상이하다는 소위원회 의견에 따라 자궁근종과 자궁선근증에 대한 평가결과를 별도로 작성하였고, 비교시술 또한 비수술치료와 수술치료를 구분하여 결과를 도출하였다. 비뚤림위험 평가결과, 자궁근종 대상 RCT 4개 연구(6편)에서 연구평가자의 눈가림 영역은 ‘불확실’, 민간 연구비 지원을 받은 연구의 비뚤림위험은 ‘높음’이었다. 자궁근종을 대상 후향적 코호트 연구 2편에서 평가자의 눈가림영역과 교란변수 영역은 모두 ‘불확실’이었다. 자궁선근증 대상 RCT 1편에서 무작위배정순서, 배정순서은폐, 연구참여자, 연구자에 대한 눈가림 등 영역은 ‘불확실’로 평가하였고, 후향적 코호트 1편에서 대상군 비교가능성, 불완전한 결과자료의 비뚤림위험은 ‘높음’으로 평가하였다.

안전성 고주파 자궁근종용해술의 안전성은 시술 관련 부작용 및 이상반응, 임신 관련 합병증, 재입원율로 평가하였고, 9편의 문헌(7개 연구)에서 안전성 지표를 보고하였다. 안전성 결과는 대상자별, 비교시술별로 구분하여 제시하였다. 자궁근종 환자에서 시술 관련 부작용 및 이상반응은 비수술치료(고강도초음파집속술, HIFU)와 비교한(RCT 1편) 결과, 두 군 모두 심각한 이상반응은 보고되지 않았다. 미열(중재군 18% vs. 대조군 4%)과 골반강내액(중재군 10% vs. 대조군 2%)이 고주파 자궁근종용해술군에서 더 많이 발생하였으며, 하복부 통증(중재군 6% vs. 대조군 12%)은 비수술치료군에서 더 많이 발생하였다. 수술치료와 비교한(RCT 5편, 후향적 코호트 2편) 결과, 심각한 이상반응인 요관손상 등(0.8% ~ 3.3%)은 고주파 자궁근종용해술군에서만 발생하였고, 탈장, 심부정맥혈전증/폐색전증 등(3.7% ~ 4.3%)은 수술군에서만 발생하였다. 재입원율(2.4%)은 수술과 비교문헌(1편)에서 고주파 자궁근종용해술군에서만 발생하였다. 자궁선근증 환자에서 시술 관련 부작용 및 이상반응은 선근증감축술과 비교한 RCT 1편에서 심각한 이상반응은 보고가 없었고, 유착의심 발생은 중재군과 대조군 각각 10%씩 발생했다. 임신 관련 합병증은 중재군에서 자궁파열(1/2 명), 대조군에서 조기양막파열(1/3 명)이 발생하였다. 코호트연구 1편에서 통증은 중재군에서만 발생, 골반저질환은 대조군에서만 발생하였다.

효과성 고주파 자궁근종용해술의 효과성은 자궁근종 및 자궁선근증의 크기 감소, 증상개선 관련(출혈, 통증, 복부 불편감 등), 재수술률, 증상중증도 점수, 건강 관련 삶의 질을 주요 지표로 검토하였다. 효과성 결과는 질환별, 비교시술별로 구분하였고, 선택문헌의 근거수준은 RCT 연구는 ‘낮음’, 후향적 코호트 연구는 ‘매우 낮음’이었다. 자궁근종 환자에서 근종 완전절제율은 고주파 자궁근종용해술군이 비수술치료(고강도초음파집속술)에 비해 유의하게 높았고(중재군 90% vs. 대조군 58%, p<0.05), 수술치료(근종절제술)에 비해서도 근종절제율이 높았으나 통계적 유의성은 보고하지 않았다. 증상개선 관련 지표(출혈, 통증, 복부불편감)는 수술치료와 비교해 두 군간 유의한 차이가 없다는 결과와 고주파 자궁근종용해술이 개선율이 높다는 결과와 수술치료가 높다는 결과가 혼재되어 결과의 방향성이 일관되지 않았다. 재수술률은 모두 수술치료(근종절제술, 전자궁절제술)와 비교하였고(RCT 5편, 후향적 코호트 1편), 고주파 자궁근종용해술군에서만 재수술이 1.6~14.3%가 발생하였다. 증상중증도 점수와 삶의 질은 모두 수술치료(근종절제술)와 비교하였고(RCT 3편), 시술 전후비교에서 두 군 모두 증상이 개선되었으나, 고주파 자궁근종용해술과 수술치료간 비교결과는 이질하여 일관성이 없었다. 자궁선근증 환자에서 자궁선근증 크기(갯수) 감소는 선근증감축술과 비교하였고(RCT 1편), 두 군 모두 선근증의 크기가 더 커졌고, 두 군간 유의한 차이가 없었다(p=0.63). 한편, 전자궁절제술과 비교한 결과(후향적 코호트 1편), 선근증의 크기는 두 군 모두 감소했고 두 군간 차이는 보고하지 않았다. 증상개선은 선근증감축술과 비교하였고(RCT 1편), 월경과다 및 생리통 감소율은 두 군간 유의한 차이가 없었다.

환자가치 조사 자궁근종 및 자궁선근증 환자 대상 환자가치 조사결과, 환자들은 생리기간동안 통증 및 사회생활의 어려움뿐만 아니라 심리적 위축도 함께 겪고 있었다. 자궁근종 치료법을 선택하는데 있어서는 자궁 보존과 빠른 일상복귀를 가장 중요하게 고려하였으며, 관련 정보는 인터넷을 통해 정보를 검색하였으며, 시술 전 마취형태 및 특성, 시술 중 발생할 수 있는 변수, 시술 후 부작용과 통증에 대한 정보를 제공받기를 원했다.

결론 및 제언 소위원회에서는 고주파 자궁근종용해술에 대해 현재 평가결과에 근거하여 다음과 같이 제언하였다. 자궁근종 및 자궁선근증을 대상으로 고주파 자궁근종용해술은 비수술치료(고강도초음파집속술) 및 수술치료와 비교하여 안전성과 효과성 지표 중 근종 크기(갯수)감소 지표 및 일부 지표를 제외하고는 두 군간 유의한 차이를 확인할 수 없었다. 또한 평가에 포함된 연구들간 결과가 일관되지 않고, 비뚤림위험이 높아 자궁근종 및 자궁선근증에서 고주파 자궁근종용행술의 안전성 및 효과성에 대해 명확한 결론을 내리기에 문헌적 근거가 부족하다고 평가하였다. 환자가치 조사결과는 자궁근종 치료법을 선택하는데 있어 자궁 보존과 빠른 일상복귀를 가장 중요한 고려사항이며, 이에 대해 시술과정에 대한 정보, 통증 및 부작용에 대한 정보를 제공받기를 원했다. 2023년 제11차 의료기술재평가위원회(2023.11.10.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “고주파 자궁근종용해술”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 비교연구의 수가 적고 연구들간 결과가 일관되지 않아 임상적 안전성과 효과성을 판단할 현재까지의 근거가 부족하여 국내 임상상황에서 자궁근종 및 자궁선근증 환자를 대상으로 증상 완화를 위해 고주파 자궁근종용해술의 사용을 ‘불충분’으로 심의하였다(권고등급: 불충분).

Radiofrequency Myolysis Background Radiofrequency (RF) myolysis, used to alleviate symptoms of uterine fibroids and adenomyosis, such as menorrhagia, irregular bleeding, lower abdominal pain, and abdominal pressure, involves the insertion of RF electrodes to heat and necrotize fibroids. This technique was listed as a non-reimbursable procedure in 2007 (Ministry of Health and Welfare Bulletin 2007–104, November 22, 2007), before the introduction of the new health technology assessment system. It was identified as a candidate for re-assessment through internal monitoring and chosen for re-assessment after priority review by the Health Technology Re-Assessment Committee. The objective of this assessment is to provide trustworthy evidence on the clinical safety and effectiveness of RF myolysis in patients with uterine fibroids and adenomyosis to support policy decisions regarding the appropriate use of this medical technology.

Method The clinical safety and effectiveness of RF myolysis in patients with uterine fibroids and adenomyosis were assessed through a systematic review. All assessment methods were finalized after discussions with the “Subcommittee for RF Myolysis for Uterine Fibroids and Adenomyosis” (hereinafter “Subcommittee”) in consideration of the objective of the assessment. The key question was: "Is RF myolysis a safe and effective health technology for improving symptoms in patients with uterine fibroids and adenomyosis?" Safety was assessed based on procedure-related adverse events and complications, pregnancy-related complications, and re-admission rates. Effectiveness was assessed based on reductions in the size and number of fibroids and adenomyosis, symptom improvement, severity of symptoms scores, and health-related quality of life. Based on the key question, literature searches were conducted using three international and three Korean databases. Literature selection was performed independently by two reviewers according to the inclusion and exclusion criteria. Disagreements were resolved through discussion. Randomized controlled trials (RCTs) were assessed using the Risk of Bias tool, whereas non-randomized studies were assessed using the Risk of Bias Assessment Tool for Non-Randomized Studies (Ver 2.0). Two reviewers independently conducted these evaluations and reached a consensus. All data were qualitatively reviewed, as quantitative synthesis was not feasible. The results of the systematic review were graded using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method to determine the level of evidence. The grade of recommendation was determined based on these results. Additionally, a patient value survey was conducted through focus group interviews with patients with uterine fibroids and adenomyosis to evaluate their experience with the disease, understanding of the health technology, decision-making in treatment, and post-treatment discomfort.

Results In total, 10 articles were selected for the systematic review. Eight articles (six studies) involved patients with uterine fibroids, with six RCTs and two retrospective cohort studies. Comparators included non-surgical treatment (one article) and surgical treatment (seven articles; five studies). Two articles (one RCT and one retrospective cohort study) involved patients with uterine adenomyosis. Both studies compared RF myolysis with surgical treatment. Based on the opinion that the characteristics of uterine fibroids and adenomyosis are different, assessment results were presented separately for each condition. Additionally, the results were presented separately for comparison with non-surgical and surgical treatments. For four RCTs (six articles) on uterine fibroids, the risk of bias was "unclear” for the blinding of outcome assessors, and studies funded by private sources were rated as having a "high" risk of bias. For the two retrospective cohort studies on uterine fibroids, the risk of bias for the blinding of assessors and confounding variables was "unclear.” For one RCT on uterine adenomyosis, the risk of bias for random sequence generation, allocation concealment, and blinding of participants and researchers was "unclear." For one retrospective cohort study on adenomyosis, the risk of bias for the comparability of the study groups and incomplete outcome data was "high."

Safety The safety of RF myolysis was assessed based on procedure-related adverse events and complications, pregnancy-related complications, and readmission rates. Nine articles (seven studies) reported safety data. Safety results were presented separately for patient population and comparator. In patients with uterine fibroids, no serious adverse events were reported for the intervention and control groups (non-surgical treatment: high-intensity focused ultrasound, HIFU) in one RCT. Mild fever (18% in the intervention group vs. 4% in the control group) and pelvic fluid accumulation (10% in the intervention group vs. 2% in the control group) were more common in the intervention group than in the control group. Lower abdominal pain was more frequent in the control group than in the intervention group (6% in the intervention group vs. 12% in the control group). Comparing RF myolysis with surgical treatment (five RCTs and two retrospective cohort studies), serious adverse events such as ureteral injury (0.8%–3.3%) were observed only in the RF myolysis group, whereas hernia and deep vein thrombosis/pulmonary embolism (3.7%–4.3%) occurred only in the surgical group. Readmission rates (2.4%) were reported only in the RF myolysis group in one comparative study. In patients with adenomyosis, there were no cases of serious adverse events in one RCT that compared RF myolysis with adenomyosis reduction surgery. Suspected adhesion was reported in 10% of patients in both the intervention and control groups. Pregnancy-related complications included one case of uterine rupture in the intervention group (1/2 patients) and one case of premature rupture of membranes in the control group (1/3 patients). In one cohort study, pain was reported only in the intervention group, whereas pelvic floor disorders were reported only in the control group.

Effectiveness The effectiveness of RF myolysis was assessed based on fibroid and adenomyosis size reduction, symptom improvement (bleeding, pain, and abdominal discomfort), reoperation rates, symptom severity scores, and health-related quality of life. The effectiveness results were presented separately for each condition and comparator. The level of evidence of the selected studies was “low” for RCTs and “very low” for retrospective cohort studies. In patients with uterine fibroids, the complete fibroid resection rate was significantly higher in the RF myolysis group than in the non-surgical treatment (HIFU) group (intervention group 90% vs. control group 58%, p<0.05). The resection rate was also higher in the RF myolysis group than in the surgical treatment (myomectomy) group, but statistical significance was not reported. Regarding symptom improvement (bleeding, pain, and abdominal discomfort), the results were inconsistent; some studies reported no significant difference between surgical treatment and RF myolysis, some reported higher improvement rates for RF myolysis, and some reported higher improvement rates with surgical treatment. For reoperation rates, the intervention was compared only to surgical treatment (myomectomy and total hysterectomy; five RCTs and one retrospective cohort study). Reoperation occurred only in the RF myolysis group, at a range of 1.6%–14.3%. For symptom severity scores and quality of life, the intervention was compared only to surgical treatment (myomectomy; three RCTs). Although both groups showed improvement after the procedure compared with the baseline, the comparison between RF myolysis and surgical treatment was inconsistent. In patients with adenomyosis, the reduction in adenomyosis size (number) was compared between RF myolysis and adenomyosis reduction surgery (one RCT). Both groups showed an increase in adenomyosis size, with no significant difference between the groups (p=0.63). When compared to total hysterectomy (one retrospective cohort study), both groups showed a reduction in adenomyosis size after the procedure, but the difference between the groups was not reported. Symptom improvement was compared between RF myolysis and adenomyosis reduction surgery (one RCT). There were no significant differences in the rates of heavy menstrual bleeding and dysmenorrhea between the groups.

Patient value survey A patient value survey was conducted on patients with uterine fibroids and adenomyosis. Patients experienced not only pain and difficulties in social life during their menstrual periods but also psychological distress. When choosing a treatment for uterine fibroids, patients prioritized preserving the uterus and quick return to daily life. They sought information primarily through the internet. Additionally, patients wanted to be informed about the type and characteristics of anesthesia before the procedure, potential variables that could occur during the procedure, and post-procedural complications and pain management.

Conclusion and Recommendations Based on the current assessment results, the Subcommittee made the following recommendations for RF myolysis: For patients with uterine fibroids and adenomyosis, there are no significant differences in the safety and effectiveness outcomes, with the exception of a reduction of fibroid size (number) and a few other measures, between the RF myolysis group and non-surgical treatment (HIFU) and surgical treatments. Furthermore, the results are inconsistent across included studies, and the risk of bias is high. Therefore, the Subcommittee concluded that there is insufficient evidence to determine the safety and effectiveness of RF myolysis for uterine fibroids and adenomyosis. The patient value survey revealed that preserving the uterus and a quick return to daily life are the most important considerations for patients when choosing a treatment for uterine fibroids. Patients also desired comprehensive information about the procedure, including the type of anesthesia, potential intraoperative variables, and postoperative side effects and pain. Based on the Subcommittee results and Article 4–10 of the Health Technology Re-Assessment Project Management Guidelines, the eleventh 2023 Health Technology Re-Assessment Committee drew the following conclusions for “RF Myolysis” (November 10, 2023): The current evidence is insufficient to assess the clinical safety and effectiveness of RF myolysis for symptom relief in patients with uterine fibroids and adenomyosis owing to the limited number of comparative studies and inconsistent results among them. Consequently, the evidence for the use of RF myolysis for these conditions was rated as “insufficient” (Grade of recommendation: Insufficient)
