사구체병증 유전자패널

Abstract

평가배경 “사구체병증 유전자 패널”은 스테로이드 저항성 신증후군, 국소 분절 사구체 경화증 환자 및 가족성 사구체질환 환자를 대상으로 차세대 염기서열분석 기반의 사구체병증 유전자 패널검사를 통해 유전적 원인을 확인하는 검사이다. 현재 건강보험에서는 ‘유전성 유전자검사를 위해 실시하는 차세대 염기서열분석 기반의 유전자 패널검사’에 대해 본인부담률 80%의 조건부 선별급여로 적용하고 있다. 동 기술은 현재 보험수가가 높아 임상에서 사용하는데 제한이 있어 재평가가 필요하다는 전문가 수요조사를 통해 재평가 주제로 발굴되었고, 2023년 제4차 의료기술재평가위원회(‘23.4.14.)에서 재평가 대상으로 최종 선정하였다. 본 평가의 목적은 사구체병증 유전자 패널의 임상적 유용성에 대한 의과학적 근거를 제공하고 정책적 의사결정을 지원하고자 하였다.

평가방법 본 평가에서는 사구체병증 유전자 패널의 임상적 유용성을 평가하고자 하였으며 모든 평가방법은 평가목적을 고려하여 “사구체병증 유전자 패널 및 세포표지검사(B 세포 검사) 공동 소위원회(이하 ‘소위원회’라 한다)” 논의를 거쳐 확정하였다. 소위원회 구성은 소아청소년과 2인, 진단검사학과 2인, 근거기반의학 1인), 총 5인으로 구성하였다. 소위원회에서는 중재검사법인 차세대염기서열분석 기반 유전자 패널검사는 이미 임상에서 많이 시행되고 있는 검사이고, 대상질환은 발병률이 낮은 유전성 질환군으로 다양한 표현형과 유전적 이질성이 높은 질환이기 때문에 기존의 체계적 문헌고찰 방법으로 각각의 질환과 원인유전자 간의 연관성을 규명하기에는 제한점이 있을 것으로 판단하였다. 따라서 별도의 체계적 문헌고찰을 시행하지 않고 희귀질환 ․ 유전정보 데이터베이스, 의학교과서, 가이드라인의 검토를 통해 해당 질환에서 유전자 변이 검출 내용과 사구체병증 유전자 패널검사의 임상적 유용성을 검토하였다. 평가 핵심질문은 ‘스테로이드 저항성 신증후군 및 국소 분절 사구체 경화증, 가족성 사구체질환 환자 대상으로 사구체병증 유전자 패널검사는 임상적으로 효과적인가?’이었다. 국내외 희귀질환 ․ 유전정보 데이터베이스, 신장학, 소아청소년학 분야의 국내외 교과서 및 임상진료지침 데이터베이스를 검색하였으며 소위원회의 논의를 통해 평가의 포함 적절성 여부를 확인하였다. 본 평가는 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.

평가결과 희귀질환 유전정보 데이터베이스(database, DB) 및 교과서 검토결과, 유전성 스테로이드 저항성 신증후군(steroid-resistant nephrotic syndrome, SRNS)과 국소분절사구체경화증(focal segmental glomerulosclerosis, FSGS), 유전 가족성 사구체질환은 상염색체 열성 또는 우성, X염색체를 통해 유전되는 특징을 가진다. 유전성 SRNS의 경우, 발병은 출생부터 성인기에 걸쳐 발생되나 주로 젊은 연령층에서 발병되는 것으로 보고되고, 젊은 성인의 약 10-15%, 소아의 30%, 선천성 및 영아의 66% 이상에서 병원성의 유전자 변이가 확인되고 있으며, 현재까지 해당 질환을 일으키는 원인 유전자로 NPHS1, NPHS2, INF2, WT1 등 다수의(60여개 이상) 단일 원인 유전자가 관여되고 있다. FSGS의 발병률은 성인 신증후군 환자의 40%, 소아 신증후군 환자의 약 20%를 차지하며, 유전성 FSGS는 ACTN4, TRPC6, CD2AP 등 40여개 이상의 유전자가 관련되고 있다고 보고하고 있다. 해당 질환에서 유전성의 경우에는 대부분 면역치료에 반응하지 않고, 신 기능 저하로 말기 신부전으로 진행할 위험이 높다. 유전 가족성 사구체질환 중 대표적인 알포트증후군은 출생아 5만명 당 1명으로 발병하는 것으로 추정되며, COL4A3 (12-15%), COL4A4 (5-8%), COL4A5 (80-85%) 병원성 유전자 변이가 관여되는 것으로 보고된다. 유전적 변이 상태에 따라 치료받지 않을 경우 말기신장질환, 청각 장애 위험, 전방 원추 수정체 발생 위험 등과 관련이 있다. 스테로이드 저항성 신증후군을 포함한 해당 질환에서 사구체병증 유전자 패널검사의 임상적 유용성은 진단, 치료결과에의 영향, 유전상담 측면에서 살펴보았다.

진단 국내 임상신장학, 국외 Nelson Textbook of Pediatrics, Brenner&Rector's the kidney, Pediatric Nephrology 등 교과서 검토결과, 유전적 신증후군의 명확한 원인 규명과, 유전자-표현형 관계를 식별하는데 유전자 검사는 도움이 되며, 발병연령, 혈족관계, 가족력, 증후군 특징의 여러 요소를 고려해서 유전자 검사를 수행할 것을 명시하였다. 국제신장병가이드라인기구(KDIGO), 국제소아신장학회(IPNA) 등 임상가이드라인에서는 ‘스테로이드 저항성 신증후군’, ‘1세 미만의 영아 발병 신증후군’, ‘신장외 증상/증후군적 특징이 있는 신증후군’, ‘SRNS, FSGS 가족력 있는 선천성 신증후군’, ‘신장이식을 고려 중인 환자’, 유전 가족성 사구체질환(알포트증후군)에서 유전자검사를 권고하고 있다.

치료결정에 영향 국내 임상신장학, 국외 Nelson Textbook of Pediatrics, Brenner&Rector's the kidney, Pediatric Nephrology 등 교과서 검토결과, NPHS2와 같은 특정 유전자 돌연변이가 있는 SRNS 환자는 면역억제치료에 반응하지 않고, CoQ10 생합성에 관여하는 유전자(COQ2, COQ6, ADCK4, or PDSS2)에 변이가 있는 환자에서는 CoQ10 치료에, CUBN 유전자 변이가 있는 경우에는 비타민 B12 치료에 반응한다고 보고된다. 또한 유전자 변이가 있는 환자에서 신장 이식을 할 경우 재발 위험이 낮아 신이식 결정 등 치료방침을 결정하는데 도움을 줄 수 있다고 제시하였다. 국제신장병가이드라인기구(KDIGO), 국제소아신장학회(IPNA) 등 임상가이드라인에서도 유전성 소아 스테로이드 저항성 신증후군 환자, 유전적 선천성 신증후군에서 면역억제치료가 효과가 없으므로 중단하고 비면역억제 치료로 관리할 것을 권고하고 있다.

유전상담 국내 임상신장학, 국외 Brenner&Rector's the kidney, Pediatric Nephrology 교과서 검토결과, 유전자 검사는 가족계획을 위한 유전상담 및 장기 기장자의 적합성 평가에 도움이 될 것으로 보고하였고, 가이드라인에서도 유전상담을 권고하고 있다.

결론 및 제언 소위원회에서는 교과서나 가이드라인에서 다양한 유전적 원인이 관여될 수 있는 선천성 신증후군, 스테로이드 저항성 신증후군, 국소분절사구체경화증, 가족성 사구체질환에 대해 진단(원인 규명)과 환자 관리, 유전상담에 있어 유전자 검사를 고려하도록 명시하고 있고, 사구체병증 유전자 패널은 한번에 여러 원인 유전자의 식별이 가능하며, 이를 통해 면역억제 요법 회피, 코엔자임 Q10 보충 등의 치료방침의 결정과 이식 후 질환의 재발위험 평가에 도움을 줄 수 있어 임상적으로 유용하다고 판단하였다. 다만, 사구체병증과 관련한 질환 및 원인 유전자는 매우 다양하고 지속적으로 새로운 유전자가 발견되고 있는 현 상황에서 유전자 목록을 지속적으로 업데이트할 필요가 있고, 국내에서 많이 발견된 유전자를 포함한 잘 고안된 유전자 검사 패널의 사용이 중요하다는 의견이었다. 또한, 우리나라 환자에게 흔히 발견되는 유전질환 및 유전자에 대한 분포 파악 등의 추가 연구가 필요하다고 제언하였다. 2024년 제1차 의료기술재평가위원회는(2024.01.12.)에서는 소위원회 검토 결과에 대하여 논의하고 의료기술재평가사업 관리지침 제4조제10항에 의거 ‘사구체병증 유전자 패널’에 대하여 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 사구체병증 질환과 관련한 원인 유전자의 식별은 치료방침의 결정 등에 도움을 줄 수 있어, 스테로이드 저항성 신증후군, 국소분절사구체경화증 및 가족성 사구체질환 환자를 대상으로 유전적 원인을 확인하는데 사구체병증 유전자 패널검사의 사용을 ‘권고함’으로 심의하였다(권고등급 : 권고함).

Glomerulopathy Gene Panel

Background Glomerulopathy gene panel testing is a next-generation sequencing (NGS)-based test used in patients with steroid-resistant nephrotic syndrome (SRNS), focal segmental glomerulosclerosis (FSGS), and familial glomerular diseases to identify genetic factors that may cause or contribute to these diseases. In South Korea, hereditary NGS-based gene panel for hereditary genetic tests is currently conditionally covered under the National Health Insurance with an 80% copayment. However, its use in clinical practice has been limited due to high costs. A survey of experts highlighted the need for a reassessment, leading to the test being selected for review at the 4th Health Technology Reassessment Committee (April 14, 2023). The objective of this assessment is to provide scientific evidence on the clinical utility of the glomerulopathy gene panel and to support evidence-based policies and decision-making.

Method The assessment focused on evaluating the clinical utility of the glomerulopathy gene panel test. All assessment methods were finalized through discussions with the Subcommittee on Glomerulopathy Gene Panel and B Cell Test (hereinafter referred to as the “Subcommittee”). The Subcommittee comprised five experts, including two pediatricians, two laboratory medicine specialists, and one expert in evidence-based medicine. Given that the index test, the NGS-based gene panel test, is widely used in clinical practice, and that the target conditions are rare genetic disorders characterized by diverse phenotypes and significant genetic heterogeneity, the Subcommittee determined that a traditional systematic review might be limited in establishing the associations between specific diseases and causative genes. Therefore, instead of conducting a separate systematic review, the Subcommittee reviewed rare disease and genetic information databases, medical textbooks, and nephrology and pediatrics guidelines from both Korean and international sources to examine the detection of genetic mutations in these conditions and to assess the clinical utility of the glomerulopathy gene panel test. The key question was: "Is glomerulopathy gene panel testing clinically effective for patients with SRNS, FSGS, and familial glomerular disease?" The inclusion criteria for the assessment were confirmed through discussions among the Subcommittee members. The Subcommittee assessment results were reviewed, and the final decision and grade of recommendation were presented by the Health Technology Reassessment Committee.

Results The review of rare disease genetic databases and textbooks revealed that hereditary SRNS, FSGS, and familial glomerular diseases can be inherited through autosomal recessive, autosomal dominant, or X-linked patterns. Hereditary SRNS can manifest at any age, from birth to adulthood, but typically occurs in younger individuals. Pathogenic genetic mutations have been identified in approximately 10–15% of young adults, 30% of children, and ≥66% of congenital and infantile cases. More than 60 different single-gene mutations, including NPHS1, NPHS2, INF2, and WT1, are associated with these conditions. FSGS accounts for approximately 40% of adult nephrotic syndrome cases and 20% of pediatric cases, with over 40 genes, including ACTN4, TRPC6, and CD2AP, involved. Most hereditary cases do not respond to immunotherapy and are highly likely to progress to end-stage renal disease (ESRD). An example of hereditary familial glomerular disease is Alport syndrome which is estimated to affect approximately 1 in 50,000 births and involves mutations in genes including COL4A3 (12–15%), COL4A4 (5–8%), and COL4A5 (80–85%). Without treatment, these conditions are associated with risks of ESRD, hearing loss, and anterior lenticonus. The clinical utility of the glomerulopathy gene panel test for hereditary SRNS, FSGS, and familial glomerular diseases, was assessed in terms of diagnosis, impact on treatment outcomes, and genetic counseling.

Diagnosis Textbooks, including Korea’s Clinical Nephrology, as well as international texts such as Nelson Textbook of Pediatrics, Brenner & Rector's The Kidney, and Pediatric Nephrology, were reviewed. According to these sources, genetic testing is valuable for identifying the specific causes of genetic nephrotic syndromes and for determining gene-phenotype relationships. Furthermore, the textbooks recommend that genetic testing should be conducted taking into account factors such as age of onset, consanguinity, family history, and syndrome characteristics. Clinical guidelines, including those from Kidney Disease: Improving Global Outcomes (KDIGO) and the International Pediatric Nephrology Association (IPNA), recommend genetic testing to identify the specific cause and genotype-phenotype correlations in a range of conditions such as SRNS, congenital nephrotic syndrome with onset in infants under one year of age, nephrotic syndrome with extrarenal symptoms/syndromic features, familial history of SRNS or FSGS, patients under consideration for kidney transplantation, and hereditary glomerular diseases such as Alport syndrome.

Impact on treatment decisions According to medical textbooks, including Korea’s Clinical Nephrology, and international textbooks such as Nelson Textbook of Pediatrics, Brenner & Rector's The Kidney, and Pediatric Nephrology, patients with SRNS who have specific gene mutations such as NPHS2 do not respond to immunosuppressive therapy. Patients with mutations in genes involved in CoQ10 biosynthesis (COQ2, COQ6, ADCK4, or PDSS2) respond to CoQ10 treatment, while those with CUBN mutations respond to vitamin B12 therapy. Additionally, identifying genetic mutations can inform treatment decisions, such as opting for kidney transplantation, as patients with these mutations have a low risk of disease recurrence after kidney transplantation. Clinical guidelines such as those from KDIGO and IPNA also recommend discontinuing immunosuppressive therapy for children with genetic SRNS and congenital nephrotic syndrome, as these patients do not benefit from immunosuppressive treatment. Instead, management with non-immunosuppressive therapies is recommended.

Genetic counseling According to Korea’s Clinical Nephrology, and international textbooks, including Nelson Textbook of Pediatrics, Brenner & Rector's The Kidney, and Pediatric Nephrology, genetic testing is useful for genetic counseling related to family planning and assessing donor suitability. The KDIGO and IPNA guidelines also recommend genetic counseling for these purposes.

Conclusion and Recommendations The Subcommittee concluded that the glomerulopathy gene panel test is clinically useful. This conclusion is based on the fact that genetic testing is recommended for diagnostic purposes, patient management, and genetic counseling in congenital nephrotic syndrome, SRNS, FSGS, and familial glomerular diseases. The Subcommittee also noted that the test allows for the simultaneous identification of multiple causative genes, which helps avoid unnecessary immunosuppressive therapy, guides treatment decisions such as CoQ10 supplementation, and assesses the risk of disease recurrence after transplantation. However, the Subcommittee stated that given the diversity of conditions and causative genes, the gene list should be updated continuously. Furthermore, it recommended the use of a well-designed gene panel test that includes genes commonly found in the Korean population. In addition, the Subcommittee concluded that further research is required to understand the distribution of genetic diseases and genes commonly found in Korean patients. Based on the Subcommittee’s assessment and Article 4–10 of the Health Technology Reassessment Project Management Guidelines, the following conclusions were drawn for the glomerulopathy gene panel at the 1st 2024 Health Technology Reassessment Committee (January 12, 2024). Considering the evidence on the clinical safety, effectiveness, and other evaluation criteria, the Health Technology Reassessment Committee determined that the glomerulopathy gene panel test is “recommended” for identifying genetic causes in patients with SRNS, FSGS, and familial glomerular diseases (Grade of recommendation: Recommended).
