세포표지검사 - B 세포 검사

Abstract

평가배경 ‘세포표지검사’는 급여항목으로 급성 백혈병, 만성 골수성 백혈병의 보세포기, 비호지킨림프종의 진단 또는 추적관찰에 사용되고 있다. 2020년 리툭시맙(rituximab)이 소아 난치성 신증후군에 대하여 허가사항 초과항목으로 급여를 인정하였는데 이때 리툭시맙의 약제효과를 추적하기 위하여 필요한 세포표지검사(B 세포 검사)의 적응증에 신증후군이 포함되어 있지 않아 사용에 제한이 있다. 이에 의료기술재평가 전문가 수요조사를 통해 해당 안건이 제안되었으며 2023년 제3차 의료기술재평가위원회(2023.3.14.)에서 재평가 대상으로 최종 선정되었다. 본 평가의 목적은 소아 난치성 신증후군에서 세포표지검사(B 세포 검사)의 임상적 유용성에 대한 의과학적 근거를 제공하고 정책적 의사결정을 지원하고자 하였다.

평가방법 세포표지검사(B 세포 검사)의 임상적 효과성을 포함한 유용성을 평가하고자 평가목적을 고려하여 모든 평가방법은 “사구체병증 유전자패널 및 세포표지검사(B 세포 검사) 공동 소위원회(이하 ‘소위원회’라 한다)” 논의를 거쳐 확정하였다. 소위원회 구성은 소아청소년과 2인, 진단검사학과 2인, 근거기반의학 1인), 총 5인으로 구성하였다. 소위원회에서는 리툭시맙을 투여하는 소아 난치성 신증후군 환자에서 세포표지검사(B 세포 검사)를 사용하는 것은 관련 임상진료지침에서 권고하고 있으며 B 세포 검사의 비교검사 또는 임상적 결과지표를 설정하는 것이 적절하지 않아 체계적 문헌고찰을 통해 B 세포 검사의 유용성을 확인하기 어렵다고 판단하였다. 따라서 소위원회에서는 체계적 문헌고찰을 수행하지 않고 교과서 및 국내외 임상진료지침 검토, 문헌고찰을 통해 소아 난치성 신증후군 환자에서 리툭시맙을 투여할 때 B 세포 검사의 임상적 유용성을 확인하였다. 평가 핵심질문은 ‘리툭시맙을 투여하는 소아 난치성 신증후군 환자에서 세포표지검사(B 세포 검사)는 임상적으로 효과적인가?“이었다. 문헌고찰은 핵심질문을 토대로 Medline 데이터베이스를 검색하였으며 문헌선택 및 배제기준에 따라 문헌을 선정하였다. 리툭시맙을 투여하는 소아 난치성 신증후군 환자에서 약제 효과를 확인하기 위하여 B 세포 검사 또는 다른 검사를 사용하는지, B 세포 검사의 모니터링 시점 등을 추출하였다. 교과서는 신장학, 소아청소년학 분야의 국내외 교과서를 검토하였고 임상진료지침은 국내외 임상진료지침 데이터베이스를 검색하여 소위원회의 논의를 통해 평가의 적절성을 확인한 후, 포함여부를 확정하였다. 본 평가는 소위원회의 검토의견을 고려하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.

평가결과

문헌고찰 검색문헌을 검토하여 총 32편을 최종 선택하였고 무작위배정비교임상연구(radomized controlled trial, RCT) 6편, 코호트연구 23편, 증례연구 3편이었다. 32편 중 30편에서 리툭시맙의 약제효과를 확인하기 위하여 B 세포 검사를 수행하였다. CD19 또는 CD20과 같이 CD 항원을 특정지어 검사한 경우 21편, B 세포수를 검사한 경우 6편, 총림프구 또는 총세포수를 확인한 경우 3편이었다. B 세포 검사 수행 여부에 대한 언급이 없는 2편의 문헌에서도 CD19+ 기준으로 B 세포 고갈을 확인하거나 리툭시맙을 재투여한다고 보고하고 있어 B 세포 검사를 수행함을 간적접으로 확인할 수 있었다. B 세포 검사의 모니터링 시점은 리툭시맙 투여 후 1주일 간격, 또는 1~3개월 간격으로 약 3~5회 수행하는 등 임상진료지침에서 제안하는 모니터링 시점과 시행횟수가 유사하여 B 세포 검사의 임상적용에 대한 일관성을 확인하였다.

교과서 검토 교과서를 검토한 결과, 소아 신증후군 환자에서 리툭시맙 투여에 대한 기준은 제시하고 있으나 리툭시맙 투여 이후 모니터링에 대한 구체적인 기준과 방법은 확인할 수 없었다. 다만, 진단검사의학(대한진단검사의학회, 2021), 혈액학(대한혈액학회, 2018) 교과서에서 리툭시맙 치료 반응 및 모니터링이 필요한 경우 CD19, CD20 세포수 또는 세포비율을 확인할 수 있음을 명시하고 있었다.

임상진료지침 검토 사구체질환 또는 소아 신증후군과 관련하여 총 6개의 임상진료지침을 검토하였다. 2015년 이후로 발행된 5개의 지침에서 기존 약제로 치료되지 않은 SRNS 소아 환자 또는 다른 형태의 사구체병증과 유전적 원인을 배제한 SRNS 소아 환자에서 리툭시맙을 투여할 수 있으며 이 때, 리툭시맙이 충분한지 모니터링하기 위하여 CD19/20, B 세포수 등 B 세포 검사를 통해 B 세포 고갈을 확인하도록 제시하였다. 또한 기저시점, 리툭시맙 투여 1개월 후, 그리고 B 세포가 회복될 때까지 1~3개월마다 모니터링을 권장하였으며(IPNA, 2020) B 세포의 수치가 회복되면 리툭시맙의 재투여를 고려할 수 있다고 제시하였다(NHS, 2015l IPNA, 2020).

결론 및 제언 소위원회는 다수의 임상진료지침에서 소아 난치성 신증후군 환자에게 리툭시맙을 투여할 때, 세포표지검사(B 세포 검사)를 통해 약제효과를 모니터링할 필요가 있음을 명시하고 있으며 관련 임상문헌에서도 리툭시맙 투여 후, B 세포가 고갈되는 것을 모니터링하기 위하여 CD19, CD20, B 세포수 등 B 세포 검사를 이용하고 있음을 확인하였다. 소위원회는 B 세포 검사가 리툭시맙의 약제효과를 확인하는 검사로 이미 임상에 검사의 위치가 확립되어 있는 유용한 기술이라는 의견이었다. 현재 리툭시맙은 허가사항 범위를 초과하여 소아 난치성 신증후군에 요양급여를 인정하고 있으나 리툭시맙 투여 이후 B 세포의 고갈을 모니터링할 수 있는 세포표지검사(B 세포 검사)의 행위 정의에는 소아 난치성 신증후군이 적응증에 포함되어 있지 않다. 따라서 소위원회에서는 리툭시맙의 약제효과 모니터링에 세포표지검사(B 세포 검사)가 유용한 점을 고려하여 향후 동 검사의 행위 정의에 소아 난치성 신증후군이 반영될 필요가 있다고 제안하였다. 2024년 제1차 의료기술재평가위원회는(2024.01.12.)에서는 소위원회 검토 결과에 대하여 논의하고 의료기술재평가사업 관리지침 제4조제10항에 의거 ‘세포표지검사(B 세포 검사)’에 대하여 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 리툭시맙을 투여하는 소아 난치성 신증후군에서 약제효과를 모니터링하는 데 필요하여 ‘세포표지검사(B 세포 검사)’의 사용을 ‘권고함’으로 심의하였다(권고등급: 권고함).

Cell Marker Test-B lymphocyte Test

Background The cell marker test is a reimbursable test used for diagnosis or monitoring acute leukemia, the blast phase of chronic myelogenous leukemia (CML), and non-Hodgkin lymphoma. In 2020, rituximab was approved for reimbursement for off-label use in pediatric refractory nephrotic syndrome; however, its use has been limited since the indication for the cell marker test (B lymphocyte test) required to monitor the efficacy of rituximab did not include nephrotic syndrome. Therefore, this test was identified as a candidate for reassessment during an expert survey on the need for health technology reassessments, and was selected for review at the third 2023 Health Technology Reassessment Committee meeting (March 14, 2023). The objective of this assessment is to provide scientific evidence on the clinical utility of the cell marker test (B lymphocyte test) and to support evidence-based policies and decision-making.

Method The assessment focused on evaluating the clinical utility of the cell marker test (B lymphocyte test). All assessment methods were finalized through discussions with the Subcommittee on Glomerulopathy Gene Panel and B lymphocyte Test (hereinafter referred to as the “Subcommittee”). The Subcommittee comprised five experts, including two pediatricians, two laboratory medicine specialists, and one expert in evidence-based medicine. The Subcommittee noted that the use of the cell marker test (B lymphocyte test) in pediatric patients with refractory nephrotic syndrome treated with rituximab is recommended in clinical practice guidelines. However, the Subcommittee determined that the utility of the B lymphocyte test cannot be assessed through a systematic review, as it is inappropriate to use comparative tests or establish clinical outcome measures for the B lymphocyte test. Thus, a systematic review was not conducted. Instead, the Subcommittee conducted a review of the literature, including textbooks and Korean and international clinical practice guidelines, to confirm the clinical utility of the B lymphocyte test in pediatric patients with refractory nephrotic syndrome treated with rituximab. The key question of the assessment was “Is the cell marker test (B lymphocyte test) clinically effective in pediatric patients with refractory nephrotic syndrome treated with rituximab?” To address this question, a literature search of Medline was conducted and studies were selected according to the inclusion and exclusion criteria. To assess the efficacy of rituximab in pediatric patients with refractory nephrotic syndrome, information on the use of the B lymphocyte test and other relevant tests, as well as the timing of B lymphocyte monitoring, was extracted. Additionally, the Subcommittee reviewed both Korean and international textbooks in nephrology and pediatrics. Korean and international clinical practice guidelines were also searched, assessed for eligibility, and selected accordingly. The Subcommittee’s assessment results were reviewed, and the final decision and grade of recommendation were presented by the Health Technology Reassessment Committee.

Results

Literature review A total of 32 articles were selected, including 6 randomized controlled trials (RCTs), 23 cohort studies, and 3 case studies. In 3 out of the 32 articles, the B lymphocyte test was used to verify the efficacy of rituximab. Specifically, 21 studies tested CD antigens such as CD19 or CD20, 6 studies measured B lymphocyte counts, and 3 studies determined total lymphocyte or cell counts. In the remaining 2 articles, while the use of the B lymphocyte test was not explicitly mentioned, the depletion of B lymphocytes was monitored based on CD19+ counts or additional doses of rituximab were reported. This indirectly suggests the use of the B lymphocyte test. Regarding the timing of the B lymphocyte test, the test was generally conducted 3–5 times at weekly intervals or every 1–3 months. The timing and frequency aligned with the clinical practice guideline recommendations, thereby confirming consistency in the clinical application of the B lymphocyte test.

Textbook review A review of textbooks identified the criteria for administering rituximab in pediatric patients with nephrotic syndrome. However, the specific criteria and methods for post-administration monitoring were not detailed. Nonetheless, textbooks on Laboratory Medicine (Korean Society for Laboratory Medicine, 2021) and Hematology (Korean Society of Hematology, 2018) specified that assessing CD19 and CD20 cell counts or ratios could be useful for evaluating the therapeutic response or monitoring the effects of rituximab.

Review of clinical practice guidelines In total, 6 clinical practice guidelines on glomerular diseases or pediatric nephrotic syndrome were reviewed. Among these, 5 guidelines published since 2015 state that rituximab can be administered to pediatric patients with SRNS who do not respond to conventional treatment or those with SRNS without a genetic cause. These guidelines recommend confirming adequate B lymphocyte depletion using CD19/20 and other B lymphocyte tests to assess the effectiveness of rituximab. Additionally, they advise monitoring patients at baseline, one month after rituximab administration, and then every one to three months until B lymphocyte recovery (IPNA, 2020). The guidelines also suggest considering the re-administration of rituximab once B lymphocyte levels recover (NHS, 2015; IPNA, 2020).

Conclusion and Recommendations The Subcommittee noted that numerous clinical practice guidelines emphasize the need to use cell marker tests (B lymphocyte tests) to monitor the effects of rituximab in pediatric patients with refractory nephrotic syndrome. The clinical literature also confirmed the use of B lymphocyte tests, such as CD19, CD20, and B lymphocyte counts, to monitor B lymphocyte depletion after rituximab administration. Consequently, the Subcommittee concluded that the B lymphocyte test is an established and valuable clinical tool for verifying the effectiveness of rituximab therapy. Rituximab has been approved for reimbursement for off-label use in pediatric refractory nephrotic syndrome. However, pediatric refractory nephrotic syndrome is included in the indications for the cell marker test (B lymphocyte test) that is used to monitor the efficacy of rituximab. Given the test’s utility in monitoring the effects of rituximab, the Subcommittee recommended including pediatric refractory nephrotic syndrome in the indications for the test. Based on the Subcommittee assessment and Article 4–10 of the Health Technology Reassessment Project Management Guidelines, the following conclusions were drawn for the cell marker test (B lymphocyte test) at the 1st 2024 Health Technology Reassessment Committee (January 12, 2024): Considering the evidence for the clinical safety, effectiveness, and other evaluation criteria, the Health Technology Reassessment Committee determined that the use of the cell marker test (B lymphocyte test) is “recommended” (Grade of recommendation: Recommended).
