무릎 골관절염에서 관절강내 약물 주사

Abstract

평가배경 관절강내 주사(Intraarticular injection, 이하 IA)는 관절염 환자를 대상으로 통증이 있는 관절 안으로 약물을 직접 주사하여 관절염 통증을 완화하고 관절 기능을 유지시키기 위한 기술이다. 신의료기술평가제도가 확립되기 전 급여로 등재되어 임상현장에서 오랫동안 사용한 기술이다. 2022년 내부 모니터링을 통해 발굴된 안건으로, 관절강내 주사 빈도가 높은 무릎 골관절염 환자를 대상으로 부신피질호르몬제, 히알루론산 등 약물을 이용한 관절강내 주사의 임상적 안전성 및 효과성에 대한 과학적 근거를 제공하고, 의료기술의 적정사용을 위한 대국민 정보를 제공하고자 하였다.

평가방법 무릎 골관절염에서 관절강내 주사 안전성 및 효과성 평가를 위해 체계적 문헌고찰의 검토(Overview of systematic review)를 수행하였다. 평가방법은 평가목적을 고려하여 「무릎 골관절염에서 관절강내 약물 주사」 에 대한 안전성 및 효과성 평가 소위원회(이하 소위원회) 논의를 거쳐 확정하였다. 소위원회는 정형외과 2인, 마취통증의학과 1인, 재활의학과 1인, 류마티스내과 1인, 근거기반의학 1인 총 6인의 전문가로 구성하였다. 핵심질문은 ‘관절강내 약물 주사는 무릎 골관절염 환자에서 임상적으로 안전하고 효과적인가?’이다. 주사약물은 국내에서 관절강내 주사로 사용할 수 있고, 임상진료지침에 포함된 부신피질호르몬(Corticosteroids, 이하 CS)와 히알루론산(Hyaluronic acid, 이하 HA), 국소마취제를 평가에 포함하였다. 안전성 지표는 시술 관련 이상반응이었고, 효과성 지표는 통증, 관절기능 장애, 관절강직, 관절연골 손상이었다. 핵심질문을 토대로 국외 3개, 국내 3개 데이터베이스에서 문헌검색을 수행하였고, 문헌선택 및 배제기준에 따라 두 명의 검토자가 독립적으로 문헌을 선택하였다. 비뚤림위험 평가는 A measurement tool to assess systematic reviews 2.0 (AMSTAR 2) 갱신판을 사용하였다. 문헌선택 및 비뚤림위험 평가과정에서 두 명의 검토자 의견이 불일치한 경우 검토자간 합의를 통해 일치된 결과를 도출하였다. 자료합성은 질적 검토방법을 적용하였으며, 체계적 문헌고찰 문헌에서 보고한 결과를 바탕으로 결론 및 대국민 정보문을 작성하였다.

평가결과 선택된 체계적 문헌고찰은 총 36편이었다. 관절강내 코르티코스테로이드 주사(Intraarticular corticosteroids injection, 이하 IA CS)의 안전성 및 효과성 결과는 20편에서, 관절강내 히알루론산 주사(Intraarticular hyaluronic acid injection, 이하 IA HA)의 안전성 및 효과성 결과는 30편에서 보고하였다. 그러나 국소마취제를 이용한 관절강내 주사의 안전성 및 효과성에 대한 체계적 문헌고찰은 없었다. 비뚤림위험 평가결과, 전반적 신뢰도는 ‘높음’이 4편이었고, ‘중등도’ 3편, ‘낮음’ 12편, ‘매우 낮음’ 17편이었다.

관절강내 코르티코스테로이드 주사

안전성 시술관련 이상반응은 위약과 비교한 9편에서 보고하였다. 시술관련 이상반응 발생률은 군간 유의한 차이가 없었다. 중대한 이상반응 발생률(2편)은 1편에서 발생하지 않았고, 나머지 1편에서는 군간 유의한 차이가 없었다. 주사 행위로 인한 국소 부작용(6편)은 주사부위 통증, Flare-up, 관절통, 관절부종, 삼출물, 인대염좌가 발생하였고, 2편에서 국소증상이 경미하게 단기간 발생하여 추가치료 없이 자연히 회복되었다고 보고하였다. 주사 약물로 인한 전신 부작용(4편)은 안면홍조, 가려움증 또는 홍반, 비특이적으로 오심, 구토, 발한, 두통 등이 발생하였다. 안면홍조(1편)는 IA CS군 1명에서 발생하였으며, Triamcinolone 40mg을 3개월 간격으로 8회 주사하였다.

효과성 통증은 16편에서 위약과 비교하였다. 주사 후 4~6주 통증을 보고한 5편에서 모두 IA CS군이 위약군보다 유의하게 낮았다. 주사 후 3개월 통증은 6편 중 2편에서, 주사 후 6개월 통증은 11편 중 5편에서 IA CS 군이 위약군보다 통증이 유의하게 낮았다. 주사 후 12개월 이상 통증은 군간 유의한 차이가 없었다. 관절기능 장애는 11편에서 위약과 비교하였다. 주사 후 4~6주 관절기능 장애를 보고한 4편 중 3편에서 IA CS군이 위약군보다 유의하게 낮았다. 주사 후 3개월 관절기능 장애는 6편 중 2편에서, 주사 후 6개월 관절기능 장애는 7편 중 2편에서 IA CS군이 위약군보다 유의하게 낮았다. 주사 후 12개월 이상 관절기능 장애는 군간 유의한 차이가 없었다. 주사 후 3개월(1편) 및 6개월(1편) 관절강직은 IA CS군과 위약군 간 유의한 차이가 없었다. 관절연골 손상과 관련하여 3편에서 영상검사를 이용하여 주사 후 6개월 이상 연골두께, 관절간격 등을 위약군과 비교하였다. Donovan 등(2022)에 포함된 일차연구 1편(140명)에서는 주사 후 24개월 관절연골 두께와 전체 연골손상지수(Cartilage damage index, 이하 CDI)가 위약군보다 유의하게 감소하였다. 나머지 2편에서 주사 후 6개월 이상 추적 시 IA CS군과 위약군의 관절 간격이 좁아진 정도는 유의한 차이가 없었다.

관절강내 히알루론산 주사

안전성 이상반응 전체 발생률은 위약과 비교한 8편에서 군간 유의한 차이가 없었다. 중대한 이상반응(7편)은 4편(위약과 비교 3편, 비스테로이드항염증제와 비교 1편)에서 군간 유의한 차이가 없었고 2편에서는 두 군에서 모두 중대한 이상반응이 발생하지 않았다. 나머지 1편(위약과 비교)에서 IA HA군 발생률이 위약군보다 유의하게 높았으나 시술과의 관련성은 없었다. 주사 행위로 인한 국소 부작용 발생률(3편)은 2편에서 IA HA군이 위약군보다 유의하게 높았고, 나머지 1편에서는 HA 약제 17개 중 14개의 발생률이 군간 유의한 차이가 없었다. 주사 약물로 인한 전신 부작용은 비스테로이드항염증제와 비교한 1편에서 소화기계 문제, 두통, 가려움증, 약물 알레르기, 하지부종, 비특이증상, 심혈관계 문제가 발생하였다. IA HA군에서 소화기계 문제(14.1%, 54/383명)가 가장 많이 발생하였고, 비스테로이드항염증제군보다 유의하게 발생률이 낮았다. 두통 발생률은 중재군 8.4% (32/383명)이었고, 비스테로이드항염증제군보다 유의하게 높았다. 심혈관계 문제를 포함한 나머지 전신증상 발생률은 군간 유의한 차이가 없었다.

효과성 통증은 위약과 비교한 27편에서 주사 후 1주(2편), 4~6주(3편), 3~6개월(15편 중 11편), 6~12개월(13편 중 10편) 추적 시 IA HA군이 위약군보다 유의하게 낮았다. 12개월 이상 장기 추적 시(5편) IA HA군과 위약군의 통증은 유의한 차이가 없었다. 관절기능 장애는 위약과 비교한 16편에서 주사 후 3~6개월(10편 중 8편) 추적 시 IA HA군의 기능장애가 위약군보다 유의하게 낮았다. 주사 후 6~12개월(10편 중 7편) 추적 시 관절기능 장애는 IA HA군이 위약군보다 낮았다. 12개월 이상 장기 추적 시(3편) IA HA군과 위약군의 기능장애는 유의한 차이가 없었다. 주사 후 3개월 이상~6개월 미만(1편) 및 6개월 이상~12개월 미만(2편) 추적 시 IA HA군의 관절강직은 위약군보다 낮았다. 관절연골 손상과 관련하여 2편에서 영상검사를 이용하여 주사 후 6개월 이상 추적 시 IA HA군과 위약군의 관절간격이 좁아진 정도는 유의한 차이가 없었다.

결론 및 제언 관절강내 약물주사의 안전성 평가결과, 관절강내 주사 행위로 인한 주사부위 통증, 부종, 삼출, 급성 발작과 같은 국소 부작용이 발생하였다. IA CS와 IA HA에서 모두 국소 부작용 발생률이 위약군보다 높았으나 대부분 경미하고 발생 후 단기간 회복되었다. 주사약물로 인한 전신 부작용으로 두 가지 약물에서 공통적으로 오심, 구토, 발한, 두통, 가려움증 등 비특이 증상이 발생하였고, IA CS에서 안면홍조가 발생하였다. 안면홍조는 체계적 문헌고찰 1편에 포함된 일차연구 1편에서 CS를 3개월 간격으로 8회 주사한 후 24개월 동안 환자 1명에게 발생하였다. CS는 반복적으로 사용하는 경우 안면홍조, 발한, 일시적인 혈압 상승, 혈당 상승, Adrenal insufficiency 등 부작용이 알려져 있다. 그러나 선행 체계적 문헌고찰 대부분 국소 부작용을 보고하고 있어 CS 장기사용, 반복주사로 인한 부작용을 문헌적 근거로 확인하는데 한계가 있었다. 관절강내 약물주사의 효과성 결과결과, 위약과 비교했을 때 IA CS는 통증감소 및 관절기능 개선에 있어 주사 후 4~6주 단기 효과가 있고, IA HA는 주사 후 3개월 이상, 최장 12개월까지 통증감소, 관절기능 개선, 관절강직 완화에 효과가 있다고 평가하였다. IA CS에 대한 선행 SR 1편에서 주사 후 24개월 관절연골 두께가 위약군보다 감소하였다. 이는 IA CS가 관절연골을 손상시켜 골관절염 진행을 촉진시킨다는 것을 시사한다. 그러나 일차연구 1편에서 총 140명을 대상으로 수행한 연구결과이므로 현재 문헌적 근거로는 불충분하다고 평가하였다. 또한 IA HA가 관절연골을 유지시켜 골관절염 진행을 늦추는지 확인하고자 하였으나 JSN을 위약군과 비교한 문헌에서 6개월 이상 추적 시 군간 유의한 차이가 없었다. 이에 소위원회에서는 무릎 골관절염에서 관절강내 주사의 안전성은 주사부위 통증, 부종, 삼출, 급성 발작과 같은 국소 부작용이 발생했으나 단기간 회복되어 안전한 기술로 판단하였으며, 통증감소 및 관절기능 개선에 있어 관절강내 코르티코스테로이드 주사는 단기 효과가 있고 관절강내 히알루론산 주사는 3개월 이상 장기 효과가 있다고 평가하였다. 그리고 코르티코스테로이드 장기사용, 반복주사로 인한 부작용, 관절강내 약물 주사가 골관절염 진행에 미치는 영향, 적정 용법용량 등 실제 임상에서 환자들이 궁금해 하는 점들은 문헌적 근거로 확인하는데 한계가 있었다. 2023년 제12차 의료기술재평가위원회(2023.12.8.)에서는 소위원회 검토 결과와 대국민 정보문에 대해 논의하였으며, 의료기술재평가사업 관리지침 제4조 제10항에 의거 “무릎 골관절염에서 관절강내 약물주사”에 대해 소위원회 결론은 원안대로 심의 의결하였다. 대국민 정보문은 2024년 제1차 의료기술재평가위원회 (2024.1.12.)에서 일부 수정하여 심의 의결하였다.

Intra-Articular Injection for Knee Osteoarthritis

Background Intra-articular (IA) injection is a technique used to alleviate pain and maintain joint function in patients with arthritis by directly injecting a drug into the affected joint. This method has been widely used in clinical practice and was covered by health insurance before the implementation of the new Health Technology Assessment system. Following this change, it was identified for reassessment through internal monitoring in 2022. This study aimed to provide trustworthy evidence on the clinical safety and effectiveness of IA injections of corticosteroids (CS) and hyaluronic acid (HA) in patients with knee osteoarthritis (OA), a group that frequently uses IA injections. Additionally, the study sought to provide public information on the appropriate use of this health technology.

Methods An overview of systematic reviews was conducted to assess the safety and effectiveness of IA injection in knee OA. The assessment methods were finalized following discussions with the “Subcommittee for IA Injection for Knee OA” (hereafter referred to as the Subcommittee), which included two orthopedic surgeons, one anesthesiologist, one rehabilitation medicine specialist, one rheumatologist, and one evidence-based medicine expert. The key question was “Is IA injection clinically safe and effective for patients with knee OA?” The assessment considered injections of CS, HA, and local anesthetics, which can be used for IA injections and are included in clinical practice guidelines. Safety was assessed based on procedure-related adverse events (AEs), whereas effectiveness was assessed based on pain, joint dysfunction, joint stiffness, and joint cartilage injury. Literature searches were conducted in three international and three Korean databases. Study selection was performed independently by two reviewers based on the inclusion and exclusion criteria. The risk of bias was assessed using the updated version of A Measurement Tool to Assess Systematic Reviews 2.0 (AMSTAR 2). Disagreements during study selection and risk of bias assessment were resolved through discussion to reach a consensus. A qualitative review method was used for data synthesis, and conclusions and public information documents were prepared based on the results reported in the systematic reviews.

Results A total of 36 systematic reviews were selected. The safety and effectiveness of IA-CS injections were reported in 20 reviews, whereas IA-HA injections were covered in 30 reviews. However, none of the reviews reported safety and effectiveness data for IA injections of local anesthetics. Regarding the risk of bias, 4 reviews had a "high" risk, 3 had a "moderate" risk, 12 had a "low" risk, and 17 had a "critically low" risk. IA-CS injection Safety Procedure-related AEs were reported in nine reviews comparing IA-CS injection with a placebo, with no significant difference in the incidence of AEs between the groups. Serious AEs (SAEs) were reported in two reviews: one reported no SAEs, and the other found no significant differences between the two groups. Local AEs caused by the injection, reported in 6 reviews, included pain at the injection site, flare-ups, joint pain, joint swelling, exudate, and ligament sprain. Two studies noted that local symptoms were mild, short-term, and resolved spontaneously without additional treatment. Systemic AEs caused by the injected medication (4 reviews) included facial flushing, itching or erythema, and nonspecific symptoms such as nausea, vomiting, sweating, and headache. Facial flushing (1 review) occurred in one patient in the IA-CS group who received triamcinolone (40 mg) injections every 3 months for a total of eight doses. Effectiveness Pain was reported in 16 reviews comparing IA-CS injection with a placebo. In the five reviews reporting pain at 4–6 weeks post-injection, pain was significantly lower in the IA-CS group than in the placebo group. Of the six reviews that reported pain at 3 months post-injection, two showed significantly lower pain in the IA-CS group. At 6 months post-injection, five out of eleven reviews reported significantly lower pain in the IA-CS group than in the placebo group. Pain levels did not significantly differ between the groups at ≥12 months post-injection. Joint dysfunction was reported in 11 reviews comparing IA-CS injection with a placebo. In three of the four reviews reporting joint dysfunction at 4–6 weeks post-injection, joint dysfunction was significantly lower in the IA-CS group. Of the six reviews reporting joint dysfunction at 3 months post-injection, two showed significantly lower joint dysfunction in the IA-CS group. At 6 months post-injection, two out of seven reviews reported significantly lower joint dysfunction in the IA-CS group than in the placebo group. Joint dysfunction did not significantly differ between the groups at ≥12 months post-injection. Joint stiffness was not significantly different between the IA-CS and placebo groups at 3 months (1 review) and 6 months (1 review) post-injection. To assess joint cartilage damage, three reviews used imaging studies to compare cartilage thickness and joint space between the IA-CS and placebo groups after ≥6 months post-injection. In one primary study (n=140) included in the review by Donovan et al. (2022), cartilage thickness and the overall cartilage damage index was significantly lower in the IA-CS group than in the placebo group at 24 months post-injection. In the other two studies, no significant differences were found in the degree of joint space narrowing between the IA-CS and placebo groups at ≥6 months post-injection. IA-HA injection Safety The incidence of AEs (8 reviews) did not significantly differ between the IA-HA and placebo groups. Of the seven reviews reporting incidence of SAEs, four found no significant differences in SAEs between the IA-HA and control groups (three compared with placebo and one with NSAIDs). Two reviews reported no SAEs in either group, whereas one review found that the incidence of SAEs was significantly higher in the IA-HA group than in the placebo group, though these SAEs were not related to the procedure itself. The rate of local AEs caused by the injection was reported in three reviews; two found a significantly higher rate in the IA-HA group than in the placebo group, whereas one reported that the incidence of AEs for 14 of 17 HA products did not significantly differ between the two groups. Regarding systemic AEs caused by the injected medication, one study comparing IA-HA with NSAIDs reported cases of gastrointestinal issues, headache, itching, drug allergies, lower extremity edema, non-specific symptoms, and cardiovascular symptoms. The most common systemic AE in the IA-HA group was gastrointestinal issues (14.1%, 54/383), which occurred at a significantly lower rate than in the NSAID group. Headache occurred in 8.4% (32/383) of the IA-HA group, which was significantly higher than that in the NSAID group. No significant differences in the incidence of cardiovascular and other systemic symptoms were found between the groups. Effectiveness In 27 reviews reporting pain post-injection, pain was significantly lower in the IA-HA group than in the placebo group at 1 week (2 reviews), 4–6 weeks (3 reviews), 3–6 months (11/15 reviews), and 6–12 months (10/13 reviews). During long-term follow-up of ≥12 months (5 reviews), no significant differences in pain were found between the IA-HA and placebo groups. In 16 reviews reporting joint dysfunction at 3–6 months post-injection, joint dysfunction was significantly lower in the IA-HA group than in the placebo group (8/10 reviews). Joint dysfunction was also significantly lower in the IA-HA group than in the placebo group at 6–12 months post-injection (7/10 reviews). However, no significant differences in joint dysfunction were reported between the IA-HA and placebo groups at a long-term follow-up of ≥12 months (3 reviews). Join stiffness was significantly lower in the IA-HA group than in the placebo group at 3–6 months (1 review) and 6–12 months (2 reviews) post-injection. Two reviews assessed cartilage joint damage using imaging studies and found no significant differences in joint space narrowing between the IA-HA and placebo groups at ≥6 months post-injection.

Conclusions and Recommendations Regarding the safety of IA injection, local AEs reported include injection site pain, edema, exudate, and acute flare-ups post-injection. Although the incidence of local AEs was higher in the IA-CS and IA-HA groups than in the placebo group, most cases were mild and resolved quickly. Systemic AEs from the injected medication included nausea, vomiting, sweating, headaches, and itching for both CS and HA. Facial flushing was specifically observed in the IA-CS group; one systematic review reported facial flushing in a single patient who received IA-CS eight times at 3-month intervals over 24 months. Repeated use of IA-CS is known to cause facial flushing, sweating, transient increases in blood pressure and blood glucose, and adrenal insufficiency. However, most systematic reviews primarily reported local AEs, with limited data on AEs resulting from long-term and repeated use of CS. Regarding the effectiveness of IA injection compared with placebo, IA-CS provided short-term benefits, reducing pain and improving joint function for 4–6 weeks post-injection. IA-HA was effective in reducing pain, improving joint function, and alleviating joint stiffness for more than 3 months and up to 12 months post-injection. One systematic review reported a reduction in cartilage thickness at 24 months post-injection in the IA-CS group compared with placebo, suggesting that IA-CS might facilitate progression of OA by accelerating cartilage damage. However, this finding was based on a single study with only 140 participants. Consequently, the Subcommittee determined that no definitive conclusion can be drawn from the currently available data. In addition, the Subcommittee sought to assess if IA-HA delays OA progression by maintaining cartilage, but no significant differences in joint space narrowing were found between the IA-HA and placebo groups at ≥6 months post-injection. Thus, the Subcommittee concluded that IA injections for knee OA are generally safe, as local AEs resolved quickly. IA-CS was found to be effective for short-term pain relief and joint function improvement, whereas IA-HA provided longer-term benefits lasting >3 months. However, evidence was limited regarding optimal dosages and administration, as well as addressing patients’ concerns about AEs from long-term CS use, repeated injections, and the overall impact of IA injections on OA progression. At the twelfth Health Technology Reassessment Committee meeting on December 8, 2023, the Committee reviewed the Subcommittee’s findings and the public information document. In accordance with Article 4-10 of the Health Technology Reassessment Project Management Guidelines, the Committee deliberated and approved the Subcommittee’s conclusions on IA injections for knee OA. The public information document was subsequently revised and approved at the first Health Technology Reassessment Committee meeting on January 12, 2024.
