치료적 저체온요법

Abstract

평가배경 치료적 저체온요법(Therapeutic Hypothermia, TH)은 급성 심정지, 외상성 뇌손상 등의 환자를 대상으로 뇌 혈류 및 뇌대사율을 낮추어 신경학적 증상을 개선하기 위한 보조 치료법이다. 동 기술은 2010년 6월부터 급여로 등재된 기술로, 심정지 이외 적응증(외상성 뇌손상, 허혈 뇌졸중, 뇌출혈) 대상에서 보건의료 자원 및 의료기술의 활용과 관련된 최신 근거자료를 제공하기 위하여 전문가 수요조사를 통해 재평가 주제로 발굴하였고, 2023년 제4차 의료기술재평가위원회(2023.4.14.)에서 평가대상으로 선정되어 재평가를 수행하였다.

평가방법 치료적 저체온요법의 안전성 및 효과성의 평가를 위해 체계적 문헌고찰을 수행하였으며 모든 평가방법은 평가목적을 고려하여 “치료적 저체온요법 평가 소위원회(이하 ‘소위원회’)”의 논의를 통해 확정하였다. 평가의 핵심질문은 치료적 저체온요법이 급성 두부 손상(외상성 뇌손상, 허혈 뇌졸중, 뇌출혈) 환자에서 안전하고 효과적인가이며, 안전성 측면으로는 시술 관련 합병증 및 중증 부작용을, 효과성 측면으로 장기(6개월 이상) 시점의 신경학적 결과 및 사망률, 인지 및 신경심리 기능, 조직손상개선, 뇌혈류 개선, 중환자실 재원기간을 확인하였다. 소위원회에서는 치료적 저체온요법의 효과성 및 안전성의 근거를 확인하는데 무작위배정임상시험연구(Randomized Clinical Trial, RCT)가 충분하다고 판단하여, 연구유형을 RCT로 제한하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개(Ovid MEDLINE, Ovid EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 의학논문데이터베이스, 한국교육학술정보원)) 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌 선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우 평가자간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험은 Cochran의 Risk of Bias(RoB)를 이용하였으며, 자료 분석은 대상 질환의 유형에 따라 구분하여 정량적 분석이 가능할 경우 메타분석을 수행하고 불가능한 경우 질적 검토 방법을 이용하였다. 평가결과의 근거수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법론을 사용하여 평가하였으며 동 평가는 소위원회의 검토의견을 반영하여 의료기술재평가위원회에서 최종심의 후 권고등급을 제시하였다.

평가결과 총 35개의 RCT 연구를 최종적으로 선택하였다. 적응증별로 외상성 뇌손상 28편, 허혈성 뇌졸중 5편, 뇌출혈 2편이었고, 비교 대상은 통상적 치료를 받은 정상체온군이었다. 비뚤림위험 수준은 연구진 및 연구 참여자에 대한 은폐 영역에서 ‘높음’이 11.4%, 결과평가에 대한 은폐 영역에서 2.9%, 불완전한 결과자료 영역에서 5.7%이었고, 대부분 영역에서 ‘불확실’이 50% 이상 차지하여 전반적으로 비뚤림위험이 높은 편이었다.

안전성 치료적 저체온요법의 안전성은 26편에서 보고되었으며, 시술 관련 합병증 및 중증 부작용을 주요 지표로 검토하였다. 시술 관련 합병증은 크게 4가지(감염증, 혈액응고이상, 부정맥, 전해질 장애)로 분류하였고 나머지 합병증은 부록 표에 제시하였다. 외상성 뇌손상 환자 대상으로 시술 관련 합병증은 19편에서 보고되었으며, 중증 부작용은 1편(Hergenroeder 등, 2022)에서 드물게 보고되었다. 메타분석 결과, 일부 합병증(폐렴, 혈액응고이상, 전해질 장애)이 중재군에서 유의하게 더 많이 발생하였으나 부정맥은 군간 차이가 없었다. 허혈 뇌졸중 환자 대상의 5개 연구에서는 모두 시술 관련 합병증을 보고했으며, 중증 부작용이 발생한 연구는 그중 2편이었다. 전반적으로 치료적 저체온요법이 사용된 환자군에서 합병증이 더 많이 발생하는 경향을 보였으나, 메타분석 결과 혈액응고이상, 부정맥 등의 주요 합병증은 대조군과 유사한 수준이었고 중증 폐렴은 군간 유의한 차이가 없었다. 뇌출혈 환자 대상은 2편에서 모두 안전성 결과가 보고되었다. 폐 감염을 포함한 주요 합병증(부정맥, 혈액응고이상, 전해질 장애)은 군간 유의한 차이가 없었으며, 중증 합병증은 모두 발생하지 않았다. 중증 부작용은 대부분 문헌에서 발생하지 않았으며, 외상성 뇌손상 대상에서 폐렴, 혈액응고이상 등의 일부 주요 합병증이 중재군에서 더 많이 발생하였으나, 허혈 뇌졸중 또는 뇌출혈 환자 대상에서는 주요 합병증이 대체로 정상체온군과 유사한 수준으로 나타났다. 소위원회에서는 저체온요법 후 발생한 합병증은 중재의 특성에 따라 예상가능한 부작용으로, 대상 환자가 모두 위중증 상태인 점을 고려하면 사망률이나 신경학적 기능이 개선되는 효과성 측면에 더 초점을 두어야 한다는 의견이었다. 이에 치료적 저체온을 세심한 주의와 관찰하에 수행하고 부작용이 발생하는 상황에 적절히 대처한다면 안전성은 수용가능한 수준으로 판단하였다.

효과성 효과성은 일차 지표(신경학적 결과, 사망률)와 이차 지표(인지 및 신경심리 기능, 조직손상개선, 뇌혈류 개선, 중환자실 재원기간)를 검토하였으며, 효과성 결과는 질환별로 구분하여 제시하였다. 첫째, 외상성 뇌손상(Traumatic Brain Injury, TBI) 환자 대상에서 신경학적 기능은 손상 후 장애 정도를 측정하는 도구로, 글래스고우 결과척도(Glasgow Outcome Scale (GOS) 또는 Glasgow Outcome Scale-Extended (GOSE))를 사용하여 평가하였다. TBI 환자 대상 연구 28개 중 21개에서 6개월 이상의 신경학적 결과를 보고하였다. GOS 또는 GOSE는 19편에서 보고하였으며, 메타분석 결과, 제처온요법군에서 신경학적 기능이 양호한 환자(%)가 유의하게 더 많은 것으로 나타났다. 6개월 이상 시점의 사망률(이하, ‘장기 사망률’)은 19개 연구에서 모두 제시하였으며, 중재군에서 유의하게 더 낮았다. 인지 및 신경심리 기능을 평가한 연구는 2개(Hutchison 등, 2008; Adelson 등, 2005)로 중재군과 정상체온군간 유의한 차이는 없었다. 또한 4개 연구에서 보고된 중환자실 입원 일수도 메타분석 결과 군간 유의한 차이가 없었다. 뇌혈류 지표로는 총 16개의 연구 중 5편에서 두개내압(Intracranial Pressure, ICP)을 제시하였으며, 냉각 종료시점의 ICP(mmHg)는 메타분석 결과 저체온요법군에서 유의하게 낮았다. 뇌관류압(Cerebral Perfusion Pressure, CPP)에 대한 평가는 9개 연구에서 수행되었으며, 그중 2편에서 보고한 냉각기 1일 또는 냉각종료 후 CPP (mmHg)는 메타분석 결과, 군간 유의한 차이가 없었다. 둘째, 허혈 뇌졸중(Ischemic Stroke, IS) 환자의 신경학적 기능 장기결과와 장기 사망률은 2편에서 보고되었으며, 메타분석 결과 신경학적 기능이 양호한 환자(%)는 저체온요법군에서 유의하게 더 많았으나, 사망률은 군간 유의한 차이가 없었다. 1편(De 등, 2004)에서 보고된 뇌 병변 크기는 군간 유의한 차이가 없었고, 뇌혈류 개선 및 중환자실 재원기간을 보고한 문헌은 없었다. 셋째, 뇌출혈(Hemorrhagic Stroke, HS) 환자 대상으로 한 2편에서 신경학적 기능 장기결과와 사망률은 보고되지 않았다. 1편(Yang 등, 2022)에서 뇌혈류 지표를 보고하였으며, 냉각이 끝난 직후 ICP (mmHg)는 치료적 저체온요법을 받은 환자군에서 유의하게 더 낮았다. 효과성 결과를 요약하면, 외상성 뇌손상(TBI) 환자의 경우 정상체온군과 비교하였을 때, TH군이 6개월 이상의 추적관찰에서 신경학적 기능이 양호한 환자(%)가 더 많았으며, 장기 사망률도 중재군에서 유의하게 더 낮았다. 또한 허혈성 뇌졸중(IS) 환자 대상으로 6개월 시점에서 신경학적 기능이 양호한 환자(%)도 중재군에서 더 많은 것으로 확인하였다. 뇌출혈(HS) 환자의 경우, 총 2개 연구 중 1편(Yang 등, 2022)에서 냉각 종료 직후 ICP는 중재군에서 유의하게 더 낮았으며, 신경학적 기능 및 사망률에 관한 장기결과를 보고한 문헌은 없었다. 소위원회에서는 뇌출혈(HS) 환자 대상에서 상대적으로 근거가 불충분하나, 결과의 방향성이 외상성 뇌손상(TBI) 및 허혈 뇌졸중(IS)과 일치하므로 ICP 상승을 완화하는 목적으로써 저체온요법의 효과는 세 질환에서 차이가 없다는 의견이었다. 이에, 외상성 뇌손상, 허혈 뇌졸중, 뇌출혈 후 급성기 환자 대상에서 치료적 저체온요법은 신경학적 기능 개선, 사망률 감소에 있어 일관되게 유의한 결과가 확인되어 효과적인 기술로 판단하였다.

결론 및 제언 소위원회는 동 평가결과에 근거하여 다음과 같이 결론 및 의견을 제언하였다. 치료적 저체온요법은 급성기 뇌손상 환자 대상으로 수행 시 장기 시점의 신경학적 기능 개선, 사망률 감소에 있어 유의한 결과가 확인되어 효과적인 기술로 판단하였다. 다만, 대상자가 모두 위중증 환자임을 고려하여 세심한 주의와 관찰하에 수행할 필요가 있으며, 부작용이 발생하는 상황에 적절히 대처한다면 안전성은 수용가능한 수준이라는 의견이었다. 2024년 제2차 의료기술재평가위원회(2024.2.16.)는 소위원회 결론 및 분과 의견에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “치료적 저체온요법”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목을 종합적으로 고려하였을 때, 치료적 저체온요법의 뇌압 상승 완화 및 신경학적 증상 개선 효과가 확인되어 국내 임상 상황에서 표준치료로 개선효과가 없는 중증 두부 손상 환자 대상으로 치료적 저체온요법의 사용을 ‘권고함’으로 결정하였다.

Therapeutic Hypothermia

Background Therapeutic hypothermia (TH) is an adjunctive treatment for patients with acute cardiac arrest, traumatic brain injury (TBI), and other conditions to reduce cerebral blood flow and cerebral metabolic rate to improve neurological symptoms. This technology has been covered since June 2010. It was selected as a topic for reassessment through an expert demand survey to provide the latest evidence related to the utilization of healthcare resources and medical technology for indications other than cardiac arrest (TBI, ischemic stroke (IS), and cerebral hemorrhage). It was selected for reassessment at the 4th Medical Technology Reassessment Committee in 2023 (April 14, 2023).

Methods A systematic review was conducted to assess the safety and effectiveness of TH. The Subcommittee on the Assessment of Therapeutic Hypothermia (the "Subcommittee") discussed and finalized the methods in light of the assessment's objectives. The key question of the assessment was, "Is TH safe and effective in patients with acute head injury (TBI, IS, cerebral hemorrhage)?" Safety was measured by procedure-related complications and serious adverse events, and effectiveness was measured based on long-term (≥6 months) neurologic outcomes and mortality, cognitive and neuropsychological function, tissue injury improvement, cerebral blood flow improvement, and length of stay in the intensive care unit. The subcommittee limited the types of studies to randomized clinical trials (RCTs) as they were deemed sufficient to assess the evidence of therapeutic hypothermia effectiveness and safety. The systematic review was searched in three international (Ovid MEDLINE, Ovid EMBASE, and EBM Reviews- Cochrane Central Register of Controlled Trials) and three domestic (KoreaMed, Korean Medical Database, and Korea Education and Research Information Service) databases based on the key question. The literature selection process was conducted independently by two reviewers according to the inclusion and exclusion criteria; in the event of disagreement, the final articles were selected by consensus among the reviewers. The risk of bias in the literature was assessed using Cochran's Risk of Bias, and data analysis was conducted based on the disease type, with meta-analysis performed when quantitative analysis was possible and qualitative review methods used when it was not. The quality of evidence was evaluated using the Grading of Recommendations Assessment, Development, and Evaluation methodology. The review was finalized by the Medical Technology Reassessment Committee, which provided a recommendation grade after review by the subcommittee.

Results A total of 35 RCT studies were ultimately selected. The indications included 28 TBIs, five ISs, and two cerebral hemorrhages, with a comparison group of normothermic individuals receiving usual care. The risk of bias was generally high, with 11.4% scoring “high” for concealment of researchers and participants, 2.9% for concealment of outcome assessment, and 5.7% for incomplete outcome data; more than 50% scored “uncertain” in most areas.

Safety The safety of TH was reported in 26 studies, reviewing procedure-related complications and serious adverse events as key indicators. Procedure-related complications were categorized into four main categories (infections, coagulation abnormalities, arrhythmias, and electrolyte disturbances), with the remaining complications presented in the Appendix Table. Procedure-related complications in patients with TBIs were reported in 19 studies, and serious adverse events were reported rarely in one study (Hergenroeder et al., 2022). The meta-analysis found that certain complications (pneumonia, coagulation abnormalities, and electrolyte disturbances) were significantly more common in the intervention than in the non-intervention group; no difference in arrhythmias was detected between groups. All five studies in patients with IS reported procedure-related complications, with two studies reporting serious adverse events. Overall, there was a trend toward more complications in the TH group; however, the meta-analysis showed similar rates of major complications, such as coagulation abnormalities and arrhythmias, as in the control group; severe pneumonia did not significantly differ between the groups. Both studies reported safety results in patients with cerebral hemorrhage. Major complications (arrhythmias, blood coagulation abnormalities, and electrolyte disturbances), including lung infections, did not differ significantly between groups, and no serious complications occurred. In the literature, serious adverse events were primarily absent. Although some major complications, such as pneumonia and coagulation abnormalities in TBI, were more common in the intervention group, in patients with IS or cerebral hemorrhage, major complications were generally similar to those in the normothermic group. The subcommittee was of the opinion that complications following hypothermia were expected given the nature of the intervention and that the focus should be on effectiveness in terms of mortality or improvement in neurological function, given that the patients were all critically ill. The safety of TH is considered acceptable when performed with great care and observation and when adverse events are appropriately addressed.

Effectiveness Effectiveness was assessed using primary endpoints (neurologic outcome, mortality) and secondary endpoints (cognitive and neuropsychological function, tissue damage improvement, cerebral blood flow improvement, and length of intensive care unit (ICU) stay), with outcomes presented by disease. First, neurological function in patients with TBI was assessed using the Glasgow Outcome Scale (GOS) or Glasgow Outcome Scale-Extended (GOSE), measuring the degree of disability after injury. In 21/28 studies on patients with TBI, neurologic outcomes of ≥6 months were reported. GOS or GOSE was reported in 19 studies, and a meta-analysis showed significantly more patients (%) with good neurologic function in the TH group. Mortality at ≥6 months (hereafter, “long-term mortality”) was presented by all 19 studies and was significantly lower in the intervention group. Two studies (Hutchison et al., 2008; Adelson et al., 2005) assessed cognitive and neuropsychological functioning and found no significant differences between the intervention and normothermia groups. The length of ICU stay reported in four studies also did not differ significantly between groups in the meta-analysis. Intracranial pressure (ICP) was presented as a measure of cerebral blood flow in 5/16 studies, and ICP (mmHg) at the end of cooling was significantly lower in the hypothermia group in the meta-analysis. Assessment of cerebral perfusion pressure (CPP) was performed in nine studies, of which two reported CPP (mmHg) on day 1 of the cooler or at the end of cooling, with no significant differences between groups in the meta-analysis. Second, long-term outcomes of neurologic function and long-term mortality in patients with IS were reported in two studies, and meta-analysis showed that the percentage of patients with good neurologic function was significantly higher in the TH group; mortality did not differ significantly between groups. There were no significant differences in brain lesion size between groups, as reported in one study (De et al., 2004), and no studies reported improvements in cerebral blood flow and length of ICU stay. Third, long-term outcomes of neurologic function and mortality were not reported in the two studies of patients with hemorrhagic stroke (HS). Cerebral blood flow metrics were reported in one study (Yang et al., 2022), and ICP (mmHg) immediately after cooling was significantly lower in the TH group. To summarize the effectiveness results, in patients with TBI, more patients (%) in the TH group had good neurological function at ≥6 months of follow-up compared to in the normothermic group, and long-term mortality was significantly lower in the intervention group. Regarding patients with IS, the intervention group also had more patients (%) with good neurological function at 6 months. Regarding patients with HS, ICP was significantly lower in the intervention group immediately after cooling in one of two studies (Yang et al. 2022); no studies reported long-term outcomes of neurologic function and mortality. The subcommittee thought that although the evidence was relatively sparse in patients with HS, the direction of results was consistent with that for TBI and IS; therefore, the effectiveness of hypothermia to mitigate ICP elevation did not differ between the three conditions. TH in the acute phase after TBI, IS, and HS was deemed an effective technique, with consistently significant results in improving neurologic function and reducing mortality.

Conclusion and Suggestions Based on this assessment, the subcommittee made the following conclusions and recommendations. TH was deemed an effective technology due to its significant results in improving neurological function and reducing mortality at long-term outcomes in patients with TBI. However, considering that all patients were critically ill, TH must be performed with great care and under observation; the safety was deemed acceptable as long as adverse events were handled appropriately. The Second Medical Technology Reassessment Committee of 2024 (February 16, 2024) deliberated on “TH” in accordance with Article 4, Paragraph 10 of the Guidelines for the Management of Medical Technology Reassessment Projects based on the subcommittee conclusions and comments as follows. The Medical Technology Reassessment Committee considered the clinical safety and effectiveness evidence, among other evaluation criteria. It determined that TH was effective in relieving elevated cerebral pressure and improving neurological symptoms. The use of TH was “recommended” for patients with severe head injuries who did not respond to standard treatment in Korean clinical settings.
