자율신경계이상검사 -피부전도반응검사-

Abstract

평가배경 자율신경계이상검사는 체내의 생리학적 항상성을 유지하는 복합신경망인 자율신경계의 이상여부를 평가하는 검사이며 이 중 ‘피부전도반응검사’는 인체에 전극을 부착해 저주파 임펄스 자극을 주어 자율신경계의 전기생리적 반응을 자동측정하는 기술이다. 자율신경계이상검사[피부전도반응검사] (이하 ‘피부전도반응검사’라 한다)는 2006년에 등재 비급여로 고시되었으며, 이후 건강보험심사평가원 예비급여부에서 비급여의 급여화 추진을 위하여 자율신경계이상검사 7개 항목(교감신경피부반응검사, 자율신경계이상검사(기립성 혈압검사, 발살바법, 지속적 근긴장에 따른 혈압검사, 심박변이도검사, 피부전도반응검사), 정량적 발한 축삭 반사검사)에 대한 재평가를 한국보건의료연구원에 의뢰하였다(2022.1.). 이에 따라 2022년 제2차 의료기술재평가위원회(2022.2.18.)에서는 해당 기술들의 평가계획서 및 통합 소위원회 구성안을 심의한 후 임상적 안전성 및 효과성을 평가하였다.

평가방법 자율신경계 기능장애 의심환자 및 질환자를 대상으로 피부전도반응검사가 임상적으로 안전하고 효과적인지 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “통합 자율신경계이상검사 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 신경과 3인, 내분비대사내과 2인, 순환기내과 2인, 정신건강의학과 2인, 근거기반의학 2인의 전문가 총 11인으로 구성하였다. 평가의 핵심질문은 ‘자율신경계 기능장애 의심환자 및 질환자를 대상으로 자율신경계 기능을 평가하기 위한 피부전도반응검사는 임상적으로 안전하고 효과적인가?’이다. 안전성은 검사 관련 이상반응 또는 부작용, 효과성은 진단정확성, 질환과의 관련성, 의료결과의 영향으로 평가하였다. 체계적 문헌고찰은 위의 핵심질문을 토대로 국외 데이터베이스 4개(Ovid MEDLINE, Ovid EMBASE, APA PsyInfo, Cochrane Central Register of Controlled Trials), 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, 사이언스온)를 이용하여 문헌을 검색하였으며, 문헌 선정기준 및 배제기준 적용을 통한 문헌선택과 자료추출, 비뚤림위험 평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 문헌의 비뚤림위험 평가는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)를 사용하였다. 자료분석에서 진단정확성은 2×2 표 값이 추출되지 않아 질적으로 기술하였으며, 질환과의 관련성은 교차비로 제시하였다.

평가결과 체계적 문헌고찰 결과, 선택문헌은 총 2편으로 모두 진단법 평가연구였다. 비뚤림위험 평가결과, 선택문헌 2편 모두 환자선택 영역은 대상자 선정방식이 명확히 제시되지 않아 ‘불확실’로 평가하였고, 중재검사 영역 평가에서는 모바일기기를 이용하여 피부전도검사를 수행한 1편에서 임계치를 제시하지 않아 ‘불확실’로 평가하였다. 나머지 지표들은 모두 기준을 충족하여 ‘낮음’으로 평가하였다.

안전성 피부전도반응검사의 안전성은 검사 관련 이상반응 및 합병증, 위음성 및 위양성으로 인한 위해 등의 지표로 살펴보고자 하였으나 이를 보고한 문헌은 없었다

효과성 효과성은 진단정확성, 질병과의 관련성(질병예측 등), 의료결과의 영향의 지표로 살펴보고자 하였다. 진단정확성은 2편의 연구 모두에서 임상진단과 비교한 결과를 다루었다. 주요우울장애 환자 대상 연구는 임상진단 대비 피부전도반응검사 결과, 민감도 65%, 특이도 75%, 정확성 70%로 보고하였다. 외상 후 스트레스장애 환자를 대상으로 한 연구에서는 임상진단과 비교해 피부전도반응검사 결과를 곡선하 면적(Area Under the Curve, AUC) 0.79 (95% CI: 0.66-0.91)로 제시하였다. 질환과의 관련성을 다룬 연구는 1편으로 외상후 스트레스 장애 진단 여부에 따른 환자 간 피부전도반응검사 결과, 1.76 (95% CI: 1.11-2.78)의 오즈비를 보고하였고, 이는 외상에 대한 반응으로 피부전도반응검사 결과가 1μS 증가할 수록 외상후 스트레스 장애가 발생할 오즈가 1.76배 높음을 의미한다. 피부전도반응검사로 인한 의료결과의 영향을 다룬 연구는 없었다. 소위원회에서는 피부전도반응검사와 관련한 안전성에 대한 문헌적 근거는 확인할 수 없었으나, 동 검사가 환자 피부에 전극을 부착하여 이루어지는 비침습적인 검사이므로 안전성에는 문제가 없다고 판단하였다. 그러나 동 검사의 효과성 판단에 대해서는 전문가 간 이견이 있었다. 즉 임상에서 진단목적으로 활용되고 있지는 않으나 바이오피드백 검사 중 하나로 환자상태를 판단함에 있어 도움을 줄 수 있는 검사로 볼 수 있다는 의견과 효과성을 판단할 문헌적 근거가 부족하다는 의견으로 나뉘었다.

결론 및 제언 통합 자율신경계이상검사 소위원회는 현재 평가 결과에 근거하여 다음과 같이 제언하였다. 피부전도반응검사는 자율신경계 기능장애 의심환자 및 질환자를 대상으로 자율신경계기능 평가목적으로 사용시 안전한 기술로 판단하였다. 피부전도반응검사의 효과성은 i) 피부전도반응검사는 바이오피드백 등 환자상태 평가에 도움을 줄 수 있는 기술, ii) 임상적 유용성 등의 효과성에 대한 결론을 내리기에 근거가 불충분한 기술로 전문가 간 이견이 있었다. 2023년 제1차 의료기술재평가위원회(2023.1.13.)에서는 피부전도반응검사에 대한 여러 전문가 간 이견을 포함한 소위원회 결론을 검토하여 다음과 같이 심의 의결하고 권고등급을 결정하였다. 자율신경계이상검사[피부전도반응검사]는 환자 피부에 전극을 부착하여 이루어지는 비침습적인 검사로 안전한 검사로 판단하였다. 효과성은 자율신경계 기능장애 평가시 여러 자율신경계검사들을 조합하여 종합적으로 판단한다는 점을 고려했을 때 임상진단에 도움이 되는 기술로 판단하였다. 이에 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황이나 가치에 따라 자율신경계 기능장애 의심환자 및 질환자를 대상으로 자율신경계 기능 평가목적으로 사용시 자율신경계이상검사[피부전도반응검사]를 ‘조건부 권고함’으로 심의하였다.

Background Autonomic neuropathy tests are designed to assess abnormalities in the autonomic nervous system, a complex neural network that maintains physiological homeostasis in the body. Among these tests, the skin conductance test is a technology that automatically measures the electrophysiological response of the autonomic nervous system when low-frequency impulse stimulation is applied through electrodes attached to the body. Autonomic neuropathy test [skin conductance test] (hereinafter referred to as “SCT”) was listed as a registered non-benefit item in 2006, and subsequently, the Preliminary Coverage Division of the Health Insurance Review and Assessment Service (HIRA) commissioned the National Evidence-based Healthcare Collaborating Agency to conduct a reassessment of seven autonomic neuropathy test items i.e., sympathetic skin response test, autonomic neuropathy [orthostatic blood pressure test, Valsalva maneuver, sustained handgrip test, heart rate variability test, and SCT], and quantitative sudomotor axon reflex test for expansion of insurance coverage to include non-benefit items (January 2022). Accordingly, the reassessment of clinical safety and effectiveness was conducted after a review of the reassessment plan and the proposed integration subcommittee composition of the applicable technologies during the 2nd session of the Committee of Health Technology Reassessment in 2022 (February 18, 2022).

Methods A systematic review assessed the clinical safety and effectiveness of SCT for assessing patients with or suspected of having autonomic dysfunction. All assessment methods were finalized through discussions with the “Integration subcommittee for autonomic neuropathy tests” (hereinafter referred to as the “subcommittee”) with consideration of the purpose of the assessment. The subcommittee consisted of 11 experts: three from neurology and two each from endocrinology and metabolism, cardiology, psychiatry, and evidence-based medicine. The key question of the assessment was, “Is SCT clinically safe and effective for assessing the autonomic nervous system function in patients with or suspected of having autonomic dysfunction?” Safety was assessed based on test-related adverse events (AEs) or complications, while effectiveness was assessed based on diagnostic accuracy, association with the disease, and impact on health outcomes. In the systematic review, four international databases (Ovid MEDLINE, Ovid EMBASE, APA PsyInfo, and Cochrane Central Register of Controlled Trials) and five domestic databases (KoreaMed, KMbase, KISS, KERIS, and ScienceON) were searched based on the key question. Article selection based on inclusion/exclusion criteria, data extraction, and risk of bias (RoB) assessment were performed independently by two reviewers, and in cases of disagreement, a consensus was reached through discussions with a third person. RoB assessment was performed using Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). In the data analysis, diagnostic accuracy was described qualitatively, as 2 × 2 tables of values could not be extracted, while association with the disease was presented as odds ratios (ORs).

Results A total of two articles (both diagnostic test evaluation studies) were selected for the systematic review. In the RoB assessment, both articles did not clearly describe the subject selection methods. Consequently, the RoB in the patient selection domain was assessed as “uncertain.” Regarding the index test domain, one study that conducted SCT using a mobile device received an “uncertain” RoB assessment because it did not present a threshold value. All other indicators satisfied the criteria, resulting in a “low” RoB assessment.

Safety The safety of SCT was to be assessed based on test-related AEs, complications, and harm from false negative/positive. However, no articles reported on these factors.

Effectiveness Effectiveness was assessed based on diagnostic accuracy, association with the disease (disease prediction, etc.), and impact on health outcomes as indicators. Two studies on diagnostic accuracy reported a comparison to clinical diagnosis. The study on patients with major depressive disorder reported that SCT results showed a sensitivity of 65%, specificity of 75%, and accuracy of 70%, as compared to clinical diagnosis. The study on patients with post-traumatic stress disorder (PTSD) reported an area under the curve (AUC) of 0.79 (95% CI: 0.66-0.91) for SCT as compared to clinical diagnosis. One study on association with the disease reported an OR of 1.76 (95% CI: 1.11-2.78) for SCT results between patients according to the diagnosis of PTSD, which indicated a response to trauma where an increase of 1 μS in the SCT results increased the OR of developing PTSD by 1.76. Meanwhile, no studies have explored the impact of SCT on health outcomes. The subcommittee determined that although evidence of safety related to SCT could not be found in the literature, this technology is safe since it is a non-invasive test with electrodes attached to the patient’s skin. However, experts had disagreements on the effectiveness of this test. In other words, some opined that although this test is not used in clinical practice for diagnostic purposes, it can be a test that can assist in the determination of the patient’s condition as a biofeedback test. Conversely, some experts argued that the available literature lacks sufficient evidence to determine its effectiveness.

Conclusions and Recommendations Based on the current assessment results, the subcommittee presented the following recommendations. SCT was determined to be a safe technology when used for the purpose of assessing autonomic nervous system function in patients with or suspected of having autonomic dysfunction. Varying expert opinions on the effectiveness of SCT include: i) SCT is a technology that can assist in the patient’s condition assessment, such as biofeedback; and ii) this technology has insufficient evidence for making any conclusions about its clinical effectiveness. During the first session of the Committee of Health Technology Reassessment in 2023 (January 13, 2023), the following determinations and grades of recommendation were made on SCT based on a review of the subcommittee findings, including various expert opinions. SCT was determined to be a safe test since it is a non-invasive test with electrodes attached to the patient’s skin. Regarding its effectiveness, this technology was determined to be helpful for clinical diagnosis, considering that assessment of autonomic dysfunction is based on a combination of various autonomic nervous system tests in clinical practice. Accordingly, the Committee of Health Technology Reassessment “conditionally recommends” the use of SCT for assessing autonomic nervous system function in patients with or suspected of having autonomic dysfunction according to the clinical situation or value in Korea based on comprehensive consideration of the evidence of its clinical safety and effectiveness and other assessment items.
