급성 흉통 및 허혈성 심질환에서 혈중 마이오글로빈 검사

Abstract

평가배경 혈중 마이오글로빈 검사는 심근경색, 근이영양증, 다발성근염, 피부근염, 협심증, 갑상선기능저하증, 신부전, 당뇨병, 간질 등 환자를 대상으로 마이오글로빈을 정량하는 검사이다. 해당 의료기술은 내부 모니터링으로 발굴된 주제로 현존하는 검사 및 적응증을 고려하여 ‘급성 흉통 및 허혈성 심질환’을 대상으로 평가하였다. 2023년 제4차 의료기술재평가위원회에서 평가계획서를 심의받아 재평가를 수행하였다.

평가방법 급성 흉통 및 허혈성 심질환에서 혈중 마이오글로빈 검사에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “급성 흉통 및 허혈성 심질환에서 혈중 마이오글로빈 검사 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 평가의 핵심질문은 “급성 흉통 및 허혈성 심질환자에서 마이오글로빈 검사는 진단 또는 경과 추적 시 임상적으로 안전하고 효과적인가?”이었고 안전성은 검사관련 위해, 효과성은 진단정확성, 시술(수술) 후 경과 추적 시 진단정확성 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 3개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2)를 사용하였으며, 자료추출은 미리 정해놓은 자료추출 양식을 활용하였다. 문헌의 비뚤림위험 평가와 자료추출은 모두 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 논의하여 합의하였다. 정량적 분석이 가능한 부분은 양적 분석(메타분석)을 수행하였다.

평가결과 체계적 문헌고찰 결과, 핵심질문을 충족하는 총 66편을 선정하였다. 대부분의 문헌은 2000년대 초반에 발간되었고, 전체 문헌 중 5편이 국내에서 수행된 문헌이었다. 환자 선택과 관련된 비뚤림위험과 적용가능성에 대한 우려가 일부 확인되었으나, 그 외 다른 비뚤림위험은 대체로 낮았다.

안전성 급성 흉통 및 허혈성 심질환에서 혈중 마이오글로빈 검사로 인한 검사 관련 위해를 보고한 문헌은 없었지만, 해당 검사는 검체 채취 이외 인체에 직접적인 위해를 가하지 않아 안전한 기술로 평가하였다.

효과성 급성 흉통 및 허혈성 심질환에서 혈중 마이오글로빈 검사의 효과성은 진단정확성, 시술(수술) 후 경과 추적 시 진단정확성 지표로 평가하였다. 마이오글로빈 검사를 단독으로 사용하여 진단정확성을 보고한 문헌은 총 59편이었다. 최초 측정 시점 기준(미보고 포함)으로 통합민감도 0.62 (95% 신뢰구간(confidence interval, CI) 0.56 ~ 0.68), 통합특이도 0.85 (95% CI 0.81 ~ 0.88), 통합 AUC (area under the curve) 0.82 (95% CI 0.78 ~ 0.85)였다. 6시간 이내 측정 시점 기준으로 통합민감도 0.87 (95% CI 0.76 ~ 0.93), 통합특이도 0.88 (95% CI 0.81 ~ 0.93), 통합 AUC 0.94 (95% CI 0.91 ~ 0.96)였다. 비교검사인 트로포닌 단독 검사의 진단정확성을 보고한 총 59편의 메타분석 결과, 최초 측정 시점 기준 통합민감도는 0.54 (95% CI 0.46 ~ 0.63), 통합특이도는 0.94 (95% CI 0.91 ~ 0.96), 통합 AUC 0.89 (95% CI 0.84 ~ 0.90)였다. 24시간 이후 측정 시점 기준으로 통합민감도 0.89 (95% CI 0.69 ~ 0.97), 통합특이도 0.93 (95% CI 0.83 ~ 0.97), 통합 AUC 0.97 (95% CI 0.95 ~ 0.98)이었다. 마이오글로빈 검사와 트로포닌 검사를 병용하여 진단정확성을 보고한 문헌은 총 12편이었다. 11편의 메타분석 결과 최초 측정 시점 기준 통합민감도 0.73 (95% CI 0.53 ~ 0.86), 통합특이도 0.90 (95% CI 0.81 ~ 0.95), 통합 AUC 0.90 (95% CI 0.88 ~ 0.93)이었다. 시술(수술) 후 경과추적 시 마이오글로빈 단독 검사의 진단정확성을 보고한 문헌은 총 5편이었다. 메타분석 결과, 최초 측정 시점 기준(미보고 포함)으로 통합민감도 0.54 (95% CI 0.38 ~ 0.69), 통합특이도 0.72 (95% CI 0.59 ~ 0.82), 통합 AUC 0.68 (95% CI 0.63 ~ 0.71)이었다. 시술(수술) 후 경과추적 시 트로포닌 단독 검사의 진단정확성을 보고한 문헌은 총 5편이 확인되었다. 메타분석 결과, 최초 측정 시점 기준(미보고 포함)으로 통합민감도 0.56 (95% CI 0.40 ~ 0.71), 통합특이도 0.76 (95% CI 0.57 ~ 0.88), 통합 AUC 0.69 (95% CI 0.65 ~ 0.73)였다. 마이오글로빈 검사와 트로포닌 검사 병용 시 진단정확성은 1편에서 민감도 0.73 ~ 0.93, 특이도 0.55 ~ 0.56으로 보고하였다.

결론 및 제언 소위원회는 현재 문헌에 근거하여 급성 흉통 및 허혈성 심질환자를 대상으로 하는 혈중 마이오글로빈 검사는 검체 채취 이외 인체에 직접적인 위해를 가하지 않아 안전한 기술로 평가하였다. 혈중 마이오글로빈 검사의 진단정확성은 트로포닌 검사보다 낮고, 트로포닌 검사와 병용 시 진단정확도가 높아졌지만 병용검사의 이득보다 비용이 더 크고 급성 허혈성 심질환의 질환 특성상 진단정확성보다 신속성이 더 중요할 뿐 아니라 혈액검사 결과로만 진단하는 것이 아니라는 점, 또한 트로포닌 검사와 비교하여 마이오글로빈 검사는 위양성이 많기 때문에 임상현장에서 트로포닌 검사가 더 유용한 검사라는 의견이었다. 이에 심근경색 등 허혈성 심질환의 진단(및 경과추적)을 위한 검사 중 하나로써 동 검사를 활용할 수 있다고 판단하였다. 추가로 임상진단은 시대에 따라 그 정의가 달라 “Universal Definition of Myocardial infarction”이 계속 개정되고 있는데 동 평가에 선택된 문헌들은 대부분 2000년대 초반의 문헌이 주를 이루고 있어 이러한 문헌을 토대로 분석한 문헌적 근거를 토대로 현재 임상에 적용하는 것은 제한점이 있다는 의견이었다. 2024년 제2차 의료기술재평가위원회(2024. 2. 16.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조 제10항에 의거 “급성 흉통 및 허혈성 심질환에서 혈중 마이오글로빈 검사”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 급성 흉통 및 허혈성 심질환의 진단에 있어 트로포닌 및 CK-MB 등 대체기술이 있어 혈중 마이오글로빈 검사의 유용성이 낮아 ‘권고하지 않음’으로 심의하였다.

Serum Myoglobin Test for Acute Chest Pain and Ischemic Heart Disease

Background The serum myoglobin test quantifies myoglobin in patients with myocardial infarction, muscular dystrophy, polymyositis, dermatomyositis, angina pectoris, hypothyroidism, renal failure, diabetes, epilepsy, etc. This medical technology was evaluated for “acute chest pain and ischemic heart disease,” a topic identified by internal monitoring, given existing tests and indications. In 2023, the 4th Medical Technology Reassessment Committee reviewed the assessment plan and conducted a reassessment.

Methods A systematic review was conducted to assess the safety and effectiveness of the serum myoglobin test for acute chest pain and ischemic heart disease. The “Subcommittee on the Serum Myoglobin Test for Acute Chest Pain and Ischemic Heart Disease” (the “Subcommittee”) discussed and finalized all methods, taking into account the objectives of the assessment. The key question of the assessment was: “Is the myoglobin test clinically safe and effective for diagnosis or follow-up in patients with acute chest pain and ischemic heart disease?” Safety was assessed based on test-related harms; effectiveness was assessed based on diagnostic accuracy; and diagnostic accuracy was assessed based on follow-up after an operation (surgery). A systematic review was conducted by searching three international and three national databases based on the key question, and two reviewers independently screened and selected studies according to the inclusion and exclusion criteria. The risk of bias in the literature was assessed using the Quality Assessment of Diagnostic Accuracy Studies-2, and data extraction was performed using a predetermined data extraction form. Both the assessment of the risk of bias in the literature and the extraction of data were performed independently by two reviewers, and any disagreements were discussed to reach a consensus. A quantitative analysis (meta-analysis) was performed.

Results The systematic review identified 66 studies that addressed the key question. Most of the literature was published in the early 2000s, and five were conducted in Korea. Concerns regarding the risk of bias and applicability related to patient selection were identified; other risks of bias were generally low.

Safety Although there were no literature reports of test-related harm from testing for serum myoglobin for acute chest pain and ischemic heart disease, the test was considered safe as it does not result in any direct harm to humans other than sample collection.

Effectiveness The effectiveness of the serum myoglobin test for acute chest pain and ischemic heart disease was evaluated in terms of diagnostic accuracy and post-operative (surgery) follow-up. A total of 59 studies reported diagnostic accuracy using the myoglobin test alone. At the time of the first measurement (including unreported), the overall sensitivity was 0.62 (95% confidence interval (CI) 0.56–0.68), overall specificity was 0.85 (95% CI 0.81–0.88), and overall area under the receiver operating characteristic curve (AUC) was 0.82 (95% CI 0.78–0.85). At time points within 6 h, the overall sensitivity was 0.87 (95% CI 0.76–0.93), overall specificity was 0.88 (95% CI 0.81–0.93), and overall AUC was 0.94 (95% CI 0.91–0.96). In the meta-analyses of 59 studies reporting the diagnostic accuracy of the troponin stand-alone test as a comparator, the overall sensitivity was 0.54 (95% CI 0.46–0.63), overall specificity was 0.94 (95% CI 0.91–0.96), and overall AUC was 0.89 (95% CI 0.84–0.90) at the time of first measurement. After 24 h, the overall sensitivity was 0.89 (95% CI 0.69–0.97), overall specificity was 0.93 (95% CI 0.83–0.97), and overall AUC was 0.97 (95% CI 0.95–0.98). A total of 12 studies reported diagnostic accuracy using a combination of the myoglobin and troponin tests. The meta-analysis of 11 studies found an overall sensitivity of 0.73 (95% CI 0.53–0.86), overall specificity of 0.90 (95% CI 0.81–0.95), and overall AUC of 0.90 (95% CI 0.88–0.93) at the time of first measurement. Five studies reported the diagnostic accuracy of myoglobin alone at post-operative (surgery) follow-up. The meta-analysis showed an overall sensitivity of 0.54 (95% CI 0.38–0.69), overall specificity of 0.72 (95% CI 0.59–0.82), and overall AUC of 0.68 (95% CI 0.63–0.71) at the time of first measurement (including unreported). Five studies reported the diagnostic accuracy of the troponin stand-alone test at post-operative (surgery) follow-up. The meta-analysis showed an overall sensitivity of 0.56 (95% CI 0.40–0.71), overall specificity of 0.76 (95% CI 0.57–0.88), and overall AUC of 0.69 (95% CI 0.65–0.73) at the time of first measurement (including unreported). The diagnostic accuracy of the combined myoglobin and troponin test was reported to be 0.73–0.93 in sensitivity and 0.55–0.56 in specificity in one study.

Conclusion and Suggestions Based on the current literature, the subcommittee has assessed the serum myoglobin test for acute chest pain and ischemic heart disease as a safe technique as it does not pose any direct harm to humans other than sample collection. The diagnostic accuracy of the serum myoglobin test is lower than that of the troponin test, although the diagnostic accuracy is increased when combined. However, the cost of the combined test outweighs the benefit. The nature of acute ischemic heart disease is such that promptness is more important than diagnostic accuracy; the diagnosis is not based solely on the blood test results. Additionally, the troponin test is more useful in clinical practice, given the higher rate of false positives produced by the myoglobin tests. Therefore, it has been determined that this test can be used as one of the tests for diagnosing (and follow-up) ischemic heart disease, such as myocardial infarction. Furthermore, the “Universal Definition of Myocardial Infarction” is constantly being updated as clinical diagnoses are redefined, and the literature selected for this evaluation was primarily published in the early 2000s, limiting its applicability to current clinical settings. In 2024, the 2nd Medical Technology Reassessment Committee (February 16, 2024) deliberated on “the serum myoglobin test in acute chest pain and ischemic heart disease” based on the results of the subcommittee review in accordance with Article 4, Paragraph 10 of the Guidelines for the Management of Medical Technology Reassessment Projects as follows. Based on the clinical safety and effectiveness evidence and other assessment criteria, the Medical Technology Reassessment Committee concluded that the serum myoglobin test was “not recommended” for acute chest pain and ischemic heart disease as the usefulness of the blood myoglobin test was low due to the availability of alternative technologies, such as troponin and CK-MB.
