급성 흉통 및 허혈성 심질환에서 혈중 CK-MB [정밀면역검사]

Abstract

평가배경 급성 흉통 및 허혈성 심질환에서 혈중 CK-MB [정밀면역검사]는 환자의 혈액을 채취하여 CK-MB (creatine kinase-MB)를 정량 측정하여 심근경색을 진단 및 진단 후 추적관찰하는 검사로서, 신의료기술평가제도 도입 전 급여로 등재되었으며, 내부 모니터링을 통해 재평가 주제로 발굴하였다. 2023년 제4차 의료기술재평가위원회에서는 체계적 문헌고찰을 통해 급성 흉통 및 허혈성 심질환에서 혈중 CK-MB [정밀면역검사]의 안전성 및 효과성에 대한 의과학적 근거를 재평가하고, 권고등급 결정을 수행하는 것으로 심의하였다.

평가방법 급성 흉통 및 허혈성 심질환에서 혈중 CK-MB [정밀면역검사]의 안전성 및 효과성에 대한 근거를 확인하기 위하여 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 순환기내과 2인, 진단검사의학과 1인, 응급의학과 1인, 근거기반의학 1인으로 구성된 “급성 흉통 및 허혈성 심질환에서 혈중 CK-MB [정밀면역검사] 소위원회 (이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 평가의 핵심질문은 “급성 흉통 및 허혈성 심질환자에서 진단 또는 시술(수술) 후 경과추적 시 CK-MB [정밀면역검사]는 임상적으로 안전하고 효과적인가?”이다. 본 핵심질문을 토대로 국외 3개, 국내 3개 데이터베이스에서 검색하였으며, 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 Quality Assessment of Diagnostic Accuracy Studies -2 (QUADAS-2)도구를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료합성은 2X2 표 산출이 가능한 문헌을 대상으로 양적 분석(메타분석)을 수행하였다.

평가결과 최종 선택문헌은 총 73편이며, 출판연도 별로는 2020년대 8편, 2010년대 16편, 2000년대 26편, 1990년대 23편이었다. 연구수행 국가는 미국이 15편으로 가장 많았으며, 다음으로 한국 9편, 영국 6편 중국 5편 순이었다. 대부분의 연구에서 비뚤림위험이 낮은 것으로 판단하였다.

안전성 급성 흉통 및 허혈성 심질환에서 혈중 CK-MB [정밀면역검사]의 안전성은 검사 관련 부작용 및 이상반응, 위음성 및 위양성으로 인한 위해로 평가하고자 하였으나, 해당 지표를 보고한 문헌은 없었다. 소위원회에서는 동 검사는 혈액을 채취하여 체외에서 이루어지는 검사로, 검체 채취과정 외에는 환자에게 직접적인 위해를 가하지 않고, 급성심근경색 등 허혈성 심질환을 진단하는 여러 방법 중 하나로서 동 검사로 인한 치료 지연이나 과잉 진단이 발생할 우려는 낮아 안전한 기술로 평가하였다.

효과성 급성 흉통 및 허혈성 심질환에서 혈중 CK-MB [정밀면역검사]의 효과성은 진단정확성, 시술(수술) 후 경과추적 시 진단정확성으로 평가하였다. 급성심근경색 등 허헐성 심질환의 진단을 위한 진단정확성을 보고한 68편으로 메타분석한 결과, 최초 측정시점에서 CK-MB의 통합민감도 0.64, 통합특이도 0.90, 통합곡선하면적(area under the curve, AUC) 0.86이었으며, 비교검사인 트로포닌은 통합민감도 0.64, 통합특이도 0.92, 통합 AUC 0.89였다. 두 검사를 병용한 경우, 통합민감도 0.79, 통합특이도 0.86, 통합 AUC 0.84로, 트로포닌에 CK-MB를 추가하였을 때 민감도는 높아지나 특이도는 다소 낮아지는 것으로 확인되었다. 여러 시점의 결과를 보고한 9편에서 최종 시점 CK-MB의 진단정확성은 통합민감도 0.86, 통합특이도 0.94, 통합 AUC 0.96이었고, 트로포닌은 통합민감도 0.90, 통합특이도 0.94, 통합 AUC 0.96이었다. 관상동맥우회술 등 시술(수술)을 받은 환자에서 수술 시기 심근경색, 수술 후 심근경색 등의 진단정확성을 보고한 5편으로 메타분석을 수행하였으며, CK-MB는 통합민감도 0.81, 통합특이도 0.77, 통합 AUC 0.86이었고, 트로포닌은 통합민감도 0.86, 통합특이도 0.85, 통합 AUC 0.92였다. 시점별로 결과를 보고한 문헌 4편에 대하여 최종 측정시점의 결과를 메타분석한 결과, CK-MB는 통합민감도 0.70, 통합특이도 0.89, 통합 AUC 0.90이었고, 트로포닌은 통합민감도 0.77, 통합특이도 0.93, 통합 AUC 0.84였다.

결론 및 제언 급성 흉통 및 허혈성 심질환에서 혈중 CK-MB [정밀면역검사] 소위원회는 현재 평가결과에 근거하여 다음과 같이 제언하였다. 급성 흉통 및 허혈성 심질환 (의심)환자에서 혈중 CK-MB [정밀면역검사]는 체외 진단검사로 검체 채취 이외 인체에 직접적인 위해를 가하지 않으므로 안전한 기술로 평가하였다. 동 검사의 진단정확성은 트로포닌 검사와 유사한 수준이며 두 검사를 병용할 시 각 검사를 단독으로 시행할 때보다 민감도를 높이는 것으로 확인되었다. 이에 소위원회는 심근경색 등 허혈성 심질환의 진단 및 시술(수술) 후 경과추적 시 여러 검사 결과들을 종합하여 판단하며, 현행 급여기준에도 동 검사와 트로포닌의 병용이 인정되는 점을 고려할 때 동 검사는 임상진단에 도움이 되는 기술로 판단하였다. 추가적으로, 임상진단에 사용되는 “Universal Definition of Myocardial Infarction”이 계속 개정되고 가장 최근의 정의를 따르고 있지만, 동 평가에 선택된 문헌들은 2000년대 이전의 문헌이 주를 이루고 있어 이러한 문헌을 토대로 분석한 결과를 현재 임상에 적용하는 것은 제한점이 있다는 의견이 있었다. 2024년 제2차 의료기술재평가위원회(2024.2.16.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “급성 흉통 및 허혈성 심질환에서 혈중 CK-MB [정밀면역검사]”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, CK-MB [정밀면역검사]는 국내 임상상황에서 급성 흉통 및 허혈성 심질환 진단 시 트로포닌 검사와 진단정확성이 유사하고, 두 검사 결과를 임상에서 종합적으로 판단하여 진단 및 추적관찰에 사용하므로 CK-MB [정밀면역검사]의 사용을 ‘권고함’으로 심의하였다(권고등급: 권고함).

Serum CK-MB Test [Precision Immunoassay] for Acute Chest Pain and Ischemic Heart Disease

Background The serum creatine kinase muscle brain (CK-MB) test [precision immunoassay] for acute chest pain and ischemic heart disease is used in the diagnosis and follow-up of myocardial infarction. This test quantitatively measures CK-MB in blood samples and was listed as a benefit item before introducing the New Medical Technology Assessment System. Through internal monitoring, it was identified as a reassessment agenda. In 2023, the 4th Medical Technology Reassessment Committee deliberated to reassess the medical evidence for the safety and effectiveness of serum CK-MB [precision immunoassay] for acute chest pain and ischemic heart disease through a systematic review and to determine the level of recommendation.

Methods A systematic review was conducted to determine the evidence for the safety and effectiveness of serum CK-MB [precision immunoassay] for acute chest pain and ischemic heart disease. The “Subcommittee on the Serum CK-MB Test [Precision Immunoassay] for Acute Chest Pain and Ischemic Heart Disease” (the “Subcommittee”), comprising two members from cardiology, one from diagnostic laboratory medicine, one from emergency medicine, and one from evidence-based medicine, discussed and finalized all methods. The key question of the assessment was: “Is the CK-MB test [precision immunoassay] clinically safe and effective for diagnosis or post-operative follow-up in patients with acute chest pain and ischemic heart disease?” A systematic review was conducted by searching three international and three national databases based on the key question; two reviewers independently screened and selected studies according to the inclusion and exclusion criteria. The risk of bias in the literature was assessed based on a consensus reached by two independent reviewers using the Quality Assessment of Diagnostic Accuracy Studies-2. Data extraction was performed independently by two reviewers using a predetermined format, and any disagreements were discussed and agreed upon with a third reviewer. A quantitative analysis (meta-analysis) was performed on the literature that could be synthesized in 2 × 2 tables.

Results The final selection included 73 studies: eight from the 2020s, 16 from the 2010s, 26 from the 2000s, and 23 from the 1990s. Most studies were conducted in the United States (15), followed by South Korea (9), the United Kingdom (6), and China (5). Additionally, most studies had reported a low risk of bias.

Safety The safety of CK-MB [precision immunoassay] for acute chest pain and ischemic heart disease was to be assessed based on harm from test-related adverse events and adverse reactions and false negatives and false positives; however, no literature reported these indicators. Hence, the subcommittee considered the test, performed in vitro on blood samples, to be a safe technology as it did not pose any direct harm to the patient other than the sample collection process. Moreover, given that it is one of several modalities to diagnose ischemic heart disease, including acute myocardial infarction, the risk of delayed treatment or overdiagnosis was considered low.

Effectiveness The effectiveness of serum CK-MB [precision immunoassay] for acute chest pain and ischemic heart disease was assessed based on diagnostic accuracy and diagnostic accuracy at post-operative follow-up. In a meta-analysis of 68 studies reporting diagnostic accuracy for diagnosing ischemic heart disease, including acute myocardial infarction, CK-MB had an overall sensitivity of 0.64, overall specificity of 0.90, and overall area under the receiver operating characteristic curve (AUC) of 0.86 at the time of first measurement, compared to an overall sensitivity of 0.64, overall specificity of 0.92, and overall AUC of 0.89 for troponin. When the two tests were combined, the sensitivity was 0.79, specificity was 0.86, and AUC was 0.84. Hence, combining CK-MB with troponin increased sensitivity but slightly decreased specificity. Of the nine studies reporting results at multiple time points, CK-MB had an overall sensitivity of 0.86, overall specificity of 0.94, and overall AUC of 0.96, while troponin had an overall sensitivity of 0.90, overall specificity of 0.94, and overall AUC of 0.96. In a meta-analysis of five studies reporting diagnostic accuracy for perioperative myocardial infarction and postoperative myocardial infarction in patients undergoing operations, such as coronary artery bypass grafting, CK-MB had an overall sensitivity of 0.81, overall specificity of 0.77, and overall AUC of 0.86, and troponin had an overall sensitivity of 0.86, overall specificity of 0.85, and overall AUC of 0.92. In a meta-analysis of the four studies that reported outcomes by time point, CK-MB had an overall sensitivity of 0.70, overall specificity of 0.89, and overall AUC of 0.90, and troponin had an overall sensitivity of 0.77, overall specificity of 0.93, and overall AUC of 0.84.

Conclusion and Suggestions Based on the current evaluation, the Subcommittee on the Serum CK-MB Test [Precision Immunoassay] for Acute Chest Pain and Ischemic Heart Disease recommended the following: In patients with acute chest pain and suspected ischemic heart disease, the serum CK-MB test [microimmunoassay] is an in vitro diagnostic test that does not cause direct harm to humans other than sample collection and is, therefore, considered safe. The diagnostic accuracy of this test is similar to that of troponin, and combining the two tests is more sensitive than either test alone. Therefore, the subcommittee considered the test a useful technique for clinical diagnosis of ischemic heart diseases, such as myocardial infarction and post-operative follow-up, considering the combination of the test and troponin is recognized by the current coverage criteria. Furthermore, the “Universal Definition of Myocardial Infarction” is constantly being updated as the clinical diagnoses are redefined; although the most recent definition was applied in this analysis, the literature selected for this evaluation was primarily published before 2000, limiting its applicability to current clinical settings. The 2nd Medical Technology Reassessment Committee in 2024 (Feb. 16, 2024) deliberated on “the CK-MB rest [precision immunoassay] for acute chest pain and ischemic heart disease” based on the results of the subcommittee review in accordance with Article 4, Paragraph 10 of the Guidelines for the Management of Medical Technology Reassessment Projects based on the subcommittee review results. Based on comprehensive consideration of the evidence of clinical safety and effectiveness and other evaluation items, the Medical Technology Reassessment Committee concluded that the use of CK-MB [precision immunoassay] was “recommended” as the diagnostic accuracy of CK-MB was similar to that of the troponin test for diagnosing acute chest pain and ischemic heart disease in Korean clinical settings; the results of both tests were comprehensively judged in clinical practice for diagnosis and follow-up (level of recommendation: recommended).
