요통 환자에서의 간섭파 전류치료

Abstract

평가배경 간섭파 전류치료(Interferential current therapy, ICT)는 신경근육계 질환자를 대상으로 두 개의 교류전류를 교차통전시켜 간섭현상을 일으켜 자극하는 전기치료법으로, 국내에서는 신의료기술평가제도 도입 전 급여항목으로 등재된 기술이다. 해당 의료기술은 내부 모니터링을 통해 발굴된 주제로, 본 평가에서는 신경 및 근골격계 질환 중 질병부담이 높은 질환이라는 점과 기존 국내외 임상진료지침에서 요통환자에게 간섭파 전류치료를 권고하지 않는 점 등을 고려하여 적응증을 요통 환자로 제한하여 평가하였다. 2023년 제4차 의료기술재평가위원회(2023.4.14.)에서 평가계획서를 심의받아 재평가를 수행하였고, 본 평가의 목적은 요통 환자에서 간섭파 전류치료의 임상적 안전성 및 효과성 등에 대한 과학적 근거를 제공하고 이를 통해 의료기술의 적정사용 등 정책적 의사결정을 지원하고자 하였다.

평가방법 요통 환자에서 간섭파 전류치료에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “요통 환자에서의 경피적 전기신경자극치료, 간섭파 전류치료 통합소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 재활의학과 2인, 마취통증의학과 2인, 정형외과 1인, 신경외과 1인, 근거기반의학과 1인, 총 7인으로 구성하였다. 평가의 핵심질문은 “요통 환자에서 간섭파 전류치료는 임상적으로 안전하고 효과적인가?”였으며, 안전성은 관련 부작용 및 이상반응을 결과지표로, 효과성은 통증점수, 기능장애, 삶의 질을 결과지표로 확인하였다. 평가결과는 급성과 만성요통 환자를 구분하여 확인하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 3개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 최종 선택문헌을 대상으로 Cochrane의 Risk of Bias를 사용하여 평가하였으며, 2명의 평가자가 독립적으로 수행하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE(Grading of Recommendations Assessment, Development and Evaluation)방법을 이용하여 근거 수준을 평가하고, 평가결과를 토대로 권고등급을 결정하였다.

평가결과 체계적 문헌고찰 결과, 핵심질문을 충족하는 총 18편의 무작위배정 비교임상시험연구를 선정하였다. 연구대상자는 모두 만성요통 환자로, 급성요통 환자 대상 연구는 없었으며, 대조군에 따라 거짓치료 및 무치료군과 비교한 연구가 14편, 보존적 치료(운동 및 마사지, 초음파치료)와 비교한 연구가 4편이었다. 비뚤림위험은 선택비뚤림, 불완전한 결과자료, 선택적 결과보고 영역에서 대체로 낮았으나(‘낮음’ 77.8%, 100%, 94.4%), 실행비뚤림과 결과평가 눈가림에서 다소 높은 수준이었다(‘높음’ 55.5%, 38.9%).

안전성 만성요통 환자에서 간섭파 전류치료의 안전성 결과를 보고한 문헌은 3편이었다. 간섭파 전류치료는 거짓치료 및 무치료군과 비교해 심각한 이상반응은 두 군 모두에서 발생하지 않았다. 시술 후 원인 미상의 통증 및 시술 중 부작용으로 인한 중단이 중재군과 대조군 모두에서 1~2명 보고되었으나 군간 유의한 차이는 없었다.

효과성 만성요통 환자에서 간섭파 전류치료의 효과성을 보고한 문헌은 총 18편이었다. 효과성의 결과지표는 통증점수, 기능장애, 삶의 질로 확인하였다. 간섭파 치료 이후 통증점수를 메타분석한 결과, 간섭파 전류치료는 거짓치료 및 무치료군(13편), 또는 보존적 치료(4편) 보다 시술 후 통증점수가 유의하게 낮았다(거짓치료 및 무치료군 표준화된 평균치(Standardized Mean Difference, SMD) –0.90, 95% 신뢰구간(Confidence Interval, CI) –1.30~-0.50, I2=86%; 보존적 치료군 SMD –0.69, 95% CI –1.01~-0.36, I2=0%). 그러나 거짓치료 및 무치료군과의 비교결과는 이질성이 높아 해석에 주의가 필요하다. 또한 문헌마다 중재의 특성이 상이하여 측정시점별, 단독 및 병용 여부, 중재횟수별로 구분하여 하위군 분석을 수행한 결과, 전체 분석결과와 동일하게 간섭파 전류치료에서의 통증점수가 유의하게 낮았다. 기능장애점수를 보고한 연구를 메타분석한 결과, 간섭파 전류치료는 거짓치료 및 무치료군(6편)보다 기능장애점수가 유의하게 낮았으며(SMD –0.21, 95% CI –0.41~-0.02, I2=0%), 보존적치료와는 차이가 없었다(SMD –0.37, 95% CI –0.73~0.00, I2=0%). 삶의 질은 대조군보다 점수가 높은 경향을 보이나 통계적으로 유의한 차이는 없었다.

결론 소위원회는 현재 확인된 근거를 바탕으로 요통 환자를 대상으로 하는 간섭파 전류치료에 대해 다음과 같이 제언하였다. 만성요통 환자에서 간섭파 전류치료는 안전성 측면에서 심각한 이상반응이 발생하지 않았거나 원인 미상의 통증 또는 경미한 부작용이 일부 발생하는 수준으로 안전한 기술로 판단하였다. 효과성은 전체 및 하위군 분석 결과 모두에서 간섭파 전류치료가 거짓치료 및 무치료에 비해 통증이 개선되는 일관된 결과를 보여 효과적인 기술로 판단하였다. 급성요통 환자에서 간섭파 전류치료와 관련된 문헌을 확인할 수 없었으나, 만성요통 환자에서의 안전성 결과와 달라질 개연성은 크지 않을 것으로 생각되어 임상적으로 안전한 기술로 판단하였다. 효과성은 추가 연구가 필요하다는 의견이었다. 2024년 제1차 의료기술재평가위원회(2024.1.12.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조 제10항에 의거 “요통 환자에서의 간섭파 전류치료”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목을 종합적으로 고려하였을 때, 만성요통 환자 대상으로 통증 완화 목적으로 사용하는 간섭파 전류치료는 문헌적 근거를 토대로 안전성과 효과성이 입증되어 ‘권고함’으로, 급성요통 환자 대상으로는 안전성에는 문제가 없으나 근거가 불충분하고, 최근 해외 가이드라인에서도 권고하지 않는 경향 등을 고려하여 ‘권고하지 않음’으로 심의하였다.

Interferential Current Therapy for Low Back Pain

Background

Interferential current therapy (ICT) is an electrotherapy technique used for neuromuscular disorders, involving the application of two intersecting alternating currents to stimulate the target area. In South Korea, ICT was previously recognized for reimbursement before the New Health Technology Assessment system was introduced. Following internal monitoring, ICT was identified as needing reassessment. Given its application in neuromuscular and musculoskeletal disorders and the lack of recommendations for its use in low back pain (LBP) in existing clinical guidelines, this assessment focused solely on LBP. The assessment plan was reviewed and approved at the 4th 2023 Health Technology Reassessment Committee meeting on April 14, 2023. The objective of the assessment was to provide scientific evidence on the clinical safety and effectiveness of ICT for LBP to inform policy decisions on its appropriate use.

Methods

A systematic review was conducted to evaluate the safety and effectiveness of ICT for LBP. The assessment methods were finalized after discussions with the “Subcommittee for ICT for LBP” (hereafter referred to as the Subcommittee), which included two rehabilitation medicine specialists, two anesthesiologists, one orthopedic surgeon, one neurosurgeon, and one expert in evidence-based medicine.

The key question addressed was: “Is ICT clinically safe and effective for patients with LBP?” Safety was assessed based on relevant adverse events (AEs) and complications, whereas effectiveness was evaluated based on pain score, functional impairment, and quality of life. The results of the assessment were presented separately for acute and chronic LBP.

Literature searches were conducted across three international and three Korean databases. Two reviewers independently selected studies based on the inclusion and exclusion criteria, resolving disagreements through discussion to reach a consensus. The risk of bias was assessed by two independent reviewers using the Cochrane Risk of Bias tool. The level of evidence for the systematic review results was graded using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method, and the grade of recommendation was determined based on these results.

Results

A total of 18 randomized controlled trials (RCTs) meeting the inclusion criteria were selected. All study participants were patients with chronic LBP, and no patients with acute LBP were enrolled. Fourteen studies compared ICT with sham or no treatment groups, whereas four compared ICT with conservative treatments such as exercise, massage, and ultrasound therapy. The risk of bias was low for selection bias, incomplete outcome data, and selective outcome reporting (77.8%, 100%, and 94.4%, respectively). However, performance bias and detection bias were somewhat high (55.5% and 38.9%, respectively).

Safety

Three studies reported safety data for ICT in patients with chronic LBP. No serious adverse events (SAEs) were reported in either the ICT or sham/no treatment groups. Some participants in both the intervention and control groups experienced unexplained pain after the procedure or discontinued due to adverse events during the procedure, but these occurrences were not significantly different between the groups.

Effectiveness

Eighteen studies evaluated the effectiveness of ICT in patients with chronic LBP, focusing on pain scores, functional impairment, and quality of life.

A meta-analysis was conducted on the post-treatment pain scores. The results indicated that ICT significantly reduced post-procedure pain scores compared with both sham/no treatment (13 studies) and conservative treatment (4 studies). The standardized mean difference (SMD) in pain scores was –0.90 (95% confidence interval [CI]: –1.30 to –0.50, I² = 86%) compared with sham/no treatment and –0.69 (95% CI: –1.01 to –0.36, I² = 0%) compared with conservative treatment. However, the results should be interpreted with caution due to the high heterogeneity in the data comparing with sham/no treatment. Additionally, due to the varying characteristics of the interventions across the included studies, a subgroup analysis was conducted based on the timing of measurements, whether the treatment was used alone or in combination with other treatments, and the frequency of the interventions. The results of these subgroup analyses were consistent with the overall analysis, showing significantly lower pain scores in the ICT group.

For functional impairment scores, ICT significantly lowered the scores compared with sham/no treatment (6 studies), with an SMD of –0.21 (95% CI: –0.41 to –0.02, I² = 0%). However, no significant differences were found in functional impairment scores when ICT was compared with conservative treatment (SMD –0.37, 95% CI: –0.73 to 0.00, I² = 0%).

Quality of life scores tended to be higher in the ICT group than in control groups, although the differences were not statistically significant.

Conclusions

Based on the current evidence, the Subcommittee found ICT to be a safe technology for patients with chronic LBP, with no cases of SAEs and only mild unexplained pain or AEs in some patients. The technique was also determined to be effective, with both overall and subgroup analyses consistently showing greater pain relief in the ICT group than in the sham or no treatment groups.

Although no studies on ICT for acute LBP were identified, the Subcommittee deemed the technique to be clinically safe for this population, as it is unlikely that its safety profile would differ between chronic and acute LBP. However, further research is needed to determine its effectiveness for acute LBP.

Based on the Subcommittee assessment results and Article 4-10 of the Health Technology Reassessment Project Management Guidelines, the following conclusions were drawn for ICT in LBP at the 1st 2024 Health Technology Reassessment Committee meeting on January 12, 2024.

Based on a comprehensive consideration of the current evidence on the clinical safety, effectiveness, and other assessment items, the Health Technology Reassessment Committee concluded that ICT is recommended for pain relief in patients with chronic LBP. However, the Health Technology Reassessment Committee decided that ICT is not recommended for patients with acute LBP because, despite the absence of safety concerns and recent international guidelines not supporting its use for this indication.
