레보노르게스트렐 방출 자궁내장치 삽입술

Abstract

평가배경 레보노르게스트렐 방출 자궁내장치(levonorgestrel releasing intrauterine device, LNG-IUD) 삽입술은 가임기 여성에서 프로게스틴(progestin)을 분비하는 피임장치를 자궁내에 삽입하는 기술이다. 동 기술은 치료목적으로는 급여 기술로 등재되어 있으나, 본인이 원하여 피임을 하고자 하는 경우에는 비급여로 적용하고 있다. 동 안건은 내부 모니터링을 통해 재평가 주제로 발굴하였고, 재평가 대상선별을 위한 임상자문회의와 우선순위 심의를 거쳐 2023년 제5차 의료기술재평가위원회에서 재평가 대상으로 최종 선정하였다. 본 평가의 목적은 가임기 여성에서 피임을 위해 레보노르게스트렐 방출 자궁내장치삽입술을 고려할 때, 합리적인 의료결정을 할 수 있도록 임상적 안전성 및 효과성에 대한 객관적인 정보를 국민들에게 제공하는 것이다.

평가방법 레보노르게스트렐 방출 자궁내장치삽입술은 기존의 체계적 문헌고찰의 검토(Overview of Systematic Reviews)를 통해 안전성 및 효과성 평가를 수행하였다. 모든 평가방법은 평가목적을 고려하여 “레보노르게스트렐 방출 자궁내장치삽입술 평가 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 산부인과 3인, 근거기반의학 1인의 전문가 4인으로 구성하였다. 평가의 핵심질문은 “가임기 여성에서 레보노르게스트렐 방출 자궁내장치삽입술은 피임목적으로 사용시 안전하고 효과적인가?”이었고, 안전성은 시술 관련한 부작용 및 이상반응(side-effects)과 시술로 인한 건강위험(health risk), 효과성은 피임효과 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 3개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우에는 평가자간 합의를 통해 최종 문헌을 결정하였다. 문헌의 비뚤림위험 평가는 체계적 문헌고찰 질 평가도구인 AMSTAR-2 (A Measurement Tool to Assess systematic reviews)를 사용하여 평가하였고, 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 자료분석은 질적 분석(qualitative review) 및 정량적 분석(quantitative analysis)을 수행하였다. 본 평가의 주요 목적을 고려하여 권고결정은 수행하지 않았고, 체계적 문헌고찰 결과를 바탕으로 대국민 정보문을 제공하였다.

평가결과 레보노르게스트렐 방출 자궁내장치삽입술 평가에 선택연구는 총 30편의 체계적 문헌고찰이였다. 선택연구의 비뚤림위험 수준은 ‘낮음’ 또는 ‘매우 낮음’으로 나타나 연구결과의 전반적 신뢰도는 낮았다.

안전성 레보노르게스트렐 방출 자궁내장치삽입술의 안전성은 총 29편의 체계적 문헌고찰로 검토하였고, 시술 관련한 부작용 및 이상반응과 시술로 인한 건강 위험으로 구분하여 검토하였다. 1) 시술 관련 부작용 및 이상반응 부작용 및 이상반응의 지표로 출혈 관련한 부작용, 기기 방출, 삽입 지속률, 그 외 다양한 부작용 사례에 대해 검토하였다. 전체 부작용은 체계적 문헌고찰 1편에서 보고하였고, 청소년 및 젊은 여성에서 레보노르게스트렐 방출 자궁내장치를 삽입한 중재군은 다른 피임법(구리 자궁내장치삽입술, 피하이식제)을 사용한 대조군과 유의한 차이가 없었다. 출혈 관련 부작용 및 이상반응은 체계적 문헌고찰 6편으로 검토하였다. 레보노르게스트렐 방출 자궁내장치를 삽입한 대부분의 여성에서 출혈(점상 출혈, 불규칙 출혈 포함)이 감소하는 패턴, 무월경 비율이 증가됨을 확인하였다. 1편에서는 대부분 여성에서 출혈패턴은 수용가능한 수준으로, 출혈 장애는 주로 삽입 후 첫 몇 달동안 발생한 것으로 보고하였다. 시간이 경과함에 따라 월경곤란, 점상출혈, 불규칙 출혈의 비율은 감소하였고, 출산력에 따른 출혈패턴 및 무월경 비율은 유의한 차이는 없는 것으로 보고하였다. 자궁내장치 기기 방출은 체계적 문헌고찰 3편으로 검토하였다. 1편에서 보고한 미산부 여성에서 LNG-IUD 삽입 후 기기 방출률은 0.01 (expulsion in women-year, 약 11,365명 여성 중 144건 방출)이었다. 출산력이나 삽입시기에 따라 살펴볼 때, 미산부 여성에서 기기 방출이 유의하게 적었으며, 임신 중절 또는 분만 후 레보노르게스트렐 방출 자궁내장치를 삽입한 여성에서는 조기에 삽입한 경우가 비교적 기기 방출 위험이 높은 것으로 보고하였다. 자궁내장치 중단(제거)/지속율은 체계적 문헌고찰 8편으로 검토하였다. 중단(제거)율은 1편(추적관찰 1-3년)에서 4.3-35%, 출혈로 인한 제거율은 0-4%이었다. 지속율은 2편에서 1년 시점 84%, 73-93%, 3년 시점 50-58%, 5년 시점 11-54%로 시간이 경과함에 따라 감소하였고, 중단된 사유로는 출혈, 통증, 기기방출 등을 보고하였다. 다른 피임법과 비교한 대부분의 연구에서 중단율 또는 피임지속율은 유의한 차이가 없었다. 체중변화(증가)는 체계적 문헌고찰 2편에서 보고하였고, 모두 유의한 체중변화(증가)는 없었다. 자궁내장치 제거 후 생식력(임신/불임)은 2편에서 보고하였다. 1편에서는 출산능력에 관한 이용가능한 문헌은 적었으나 불임의 위험 증가는 없다고 보고하였고, 다른 1편에서는 다른 피임법(구리 자궁내장치삽입술)과 비교하여 삽입 중단 후 계획된 임신은 유의한 차이가 없었다. 성 기능은 체계적 문헌고찰 2편에서 보고하였다. 1편에서는 레보노르게스트렐 방출 자궁내장치 삽입 후 12개월까지 유의한 변화는 없었고, 다른 1편에서는 구리 자궁내장치삽입술과 비교한 대부분 연구들에서 유의한 차이는 없었고, 자연주기 피임법과 비교 연구는 중재군에서 좋은 결과를 보고하였다. 그 밖의 부작용 및 이상반응으로 부분천공 및 자궁근육 매몰, 자궁내막염, 복부/허리통증, 두통, 여드름, 유방압통, 세균성 질염 등의 사례가 확인되었고,다른 피임법과 비교하여 유의한 차이는 없었다. 2) 시술로 인한 건강 위험 시술로 인한 건강 위험은 레보노르게스트렐 방출 자궁내장치 사용 유무에 따른 유방암 발생, 정신과적 증상, 정맥혈전증 발생, 영아 성장의 관련성으로 검토하였다. 유방암 발생과의 관련성은 체계적 문헌고찰 6편에서 보고하였으나, 일부 연구 대상자 중 치료목적 사용자가 포함되어 체계적 문헌고찰에 선택된 일차연구들을 재분석하였다. 일차연구 8편 중 5편에서는 유의한 관련성이 없었고, 3편에서는 유방암 발생 위험과의 관련성이 있었다. 해당 일차연구들에서 효과추정치를 제시한 7편을 메타분석한 결과, 유방암 발생에 대한 통합 위험비는 1.1 (95% 신뢰구간(Confidence Interval, CI) 1.00, 1.21)로 높았으나 미사용군과의 유의한 차이는 없없다. 정신과적 증상 발생과의 관련성은 체계적 문헌고찰 3편에서 주로 우울증상에 대해 보고하였다. 각 체계적 문헌고찰에 포함된 개별 연구들에서는 우울증상과의 관련성이 있는 연구와 없다는 연구가 혼재되어 있었고, 최근의 체계적문헌고찰에서는 우울증 증가의 부정적 영향에 대해 상담 필요하고 추가 연구가 필요하다고 보고하였다. 정맥혈전증 발생과의 관련성은 체계적 문헌고찰 2편에서 보고하였다. 포함된 일차연구 수가 적었으나 레보노르게스트렐 방출 자궁내장치 사용은 정맥혈전증 발생 위험을 높이지 않았다. 영아 성장 및 모유 수유는 체계적 문헌고찰 2편에서 보고하였고, 삽입시기에 따른 수유 영향 및 비호르몬 자궁내장치삽입술과 비교하여 영아성장/모유수유에 유의한 차이는 없었다.

효과성 레보노르게스트렐 방출 자궁내장치삽입술의 피임효과는 총 7편의 체계적 문헌고찰로 검토하였다. 2편의 체계적 문헌고찰에서 임신율 지수(Pearl Index, PI)는 각 0.09-0.11, 0.11로 보고하였고, 다른 1편의 체계적 문헌고찰에서 개별 연구들의 결과를 통합한 1년 시점의 임신율은 0.016 (95% CI 0.01, 0.02)로 임신율이 낮은 것으로 제시하였다. 그 외의 다른 피임법과 비교된 결과에서 레보노르게스트렐 방출 자궁내장치삽입술의 임신율이 낮거나 유의한 차이가 없었다.

결론 및 제언 체계적 문헌고찰을 중심으로 현재까지의 근거를 종합해 본 결과 “레보노르게스트렐 방출 자궁내장치삽입술”의 안전성과 효과성에 대하여 다음과 같이 제시하였다. 레보노르게스트렐 방출 자궁내장치삽입술의 시술관련 부작용 및 이상반응 관련하여 출혈감소 및 무월경 증가 등의 출혈변화가 확인되었으나 출혈로 인한 장치제거율은 0-4% 정도였고, 대부분 여성에서 출혈 패턴은 수용가능한 수준이었다. 자궁내장치 기기 방출률은 낮은 수준이었고, 체중변화나 자궁내장치 제거후 생식력(임신)에 있어 유의미한 부정적 영향은 확인되지 않았으며, 그 밖의 이상반응 사례들도 다른 피임법과 비교하여 유의한 차이는 없어 안전성의 우려는 크지 않다고 판단하였다. 동 기술로 인한 건강 위험으로 유방암 발생 위험이 증가하는 경향이 있고, 우울증이 증가한다는 일부 연구들도 있었다. 그 외 정맥혈전증 발생 증가 및 영아 성장이나 모유 수유에의 부정적 영향은 확인되지 않았으나, 결론을 내리기에는 문헌적 근거가 부족하다고 판단하였다. 이에 소위원회는 레보노르게스트렐 방출 자궁내장치삽입술로 인한 건강 위험에 대한 명확한 근거를 확인하기 위해 추가 연구가 필요하다고 제안하였다. 동 기술의 피임효과는 보고된 피임실패율이 불임수술에 견줄 수 있는 수준으로, 한 번의 시술로 장기간의 우수한 피임효과가 있는 기술로 판단하였다. 다만, 본 평가결과는 근거수준이 ‘낮음’, ‘매우 낮음’으로 제한점이 있어 해석에 주의가 필요하다고 보았다. 2023년 제12차 의료기술재평가위원회(2023.12.8.), 2024년 제1차 의료기술재평가위원회(2024.1. 12.)에서 의료기술재평가사업 관리지침 제4조제10항에 의거 “레보노르게스트렐 방출 자궁내장치삽입술”에 대해 소위원회 결론 및 대국민 정보문에 대해 다음과 같이 심의·의결하였다. 레보노르게스트렐 방출 자궁내장치삽입술은 가임기 여성에서 피임목적으로 사용 시 한 번의 시술로 장기간의 우수한 피임효과가 있는 기술로 평가하였다. 안전성과 관련하여 시술 관련 부작용 및 이상반응으로는 수용가능한 수준의 출혈양상 변화가 있었고, 장치 제거 후 생식능력에 부정적 영향은 확인되지 않았으며, 그 밖의 이상반응은 다른 피임법과 유사하여 안전성의 우려는 크지 않다고 평가하였다. 또한, 시술로 인한 건강 위험은 유방암 발생, 우울증상이 증가한다는 일부 연구들이 확인되었으나 명확한 근거확인을 위해서는 추가 연구가 필요하다고 제언하였다. 본 평가결과를 바탕으로 국민들의 합리적인 의료적 선택을 지원하고자 ‘레보노르게스트렐 방출 자궁내장치삽입술’에 대한 대국민 정보문을 별도로 제공하였다.

Levonorgestrel-releasing intrauterine device insertion

Background The insertion of a levonorgestrel-releasing intrauterine device (LNG-IUD) is used for the placement of a progestin-releasing contraceptive device in women of childbearing age. This technique is reimbursable when medically necessary but is not covered as an elective procedure. It was identified as a candidate for reassessment through internal monitoring and selected for review at the fifth 2023 Health Technology Reassessment Committee meeting, following a clinical advisory committee meeting and priority review. The objective of this assessment was to provide objective data on the safety and effectiveness of LNG-IUD insertion to support informed healthcare decisions regarding contraception for women.

Method The safety and effectiveness of LNG-IUD insertion were assessed through an overview of systematic reviews. All assessment methods were finalized through discussion among the "Subcommittee for LNG-IUD Insertion" (hereinafter the "Subcommittee") in consideration of the objective of the assessment. The Subcommittee consisted of four experts: three obstetrics-gynecology specialists and one evidence-based medicine expert. The key question of the assessment was: “Is LNG-IUD insertion safe and effective when used for contraception in women of childbearing age?” Safety was assessed based on procedure-related adverse events (AEs) and complications, as well as health risks. Effectiveness was assessed based on contraceptive efficacy. Literature searches were conducted using three international and three Korean databases. Literature selection was carried out independently by two reviewers according to predetermined inclusion and exclusion criteria. In cases of disagreement, consensus was reached through discussion. Risk of bias was assessed independently by two reviewers using the A Measurement Tool to Assess Systematic Reviews-2 tool, with discrepancies resolved by consensus. Both qualitative and quantitative analyses were performed. Considering the objective of this assessment, no recommendations were made, but public information was provided based on the results of the systematic review.

Results In total, 30 systematic reviews were selected to assess LNG-IUD insertion. The overall reliability of the study results was low, but the risk of bias was determined to be “low” or “very low.”

Safety The safety of LNG-IUD insertion was assessed based on 29 systematic reviews in terms of procedure-related AEs and complications, as well as health risks. 1) Procedure-related AEs and complications Bleeding-related AEs, device expulsion, device continuation rate, and other AEs were considered. The overall incidence of AEs, reported in one study, did not significantly differ between adolescents and young women with LNG-IUDs and those using other types of contraceptives (copper IUD and subcutaneous contraceptive implant). Bleeding-related AEs and complications were reported in six systematic reviews. Most women experienced a reduction in bleeding (including spotting and irregular bleeding) and an increase in amenorrhea rates after LNG-IUD insertion. One review noted that the bleeding pattern was acceptable for most women, with bleeding irregularities primarily occurring during the first few months after insertion. Over time, the incidence of dysmenorrhea, spotting, and irregular bleeding decreased, with no significant differences in bleeding patterns or amenorrhea rates based on parity. Device expulsion was assessed in three systematic reviews. One study reported an expulsion rate of 0.01 per woman-year (144 expulsions among 11,365 women). Device expulsion was significantly lower in nulliparous women and higher when the LNG-IUD was inserted early after abortion or delivery. Device discontinuation/continuation rates were assessed in eight systematic reviews. In one review, the discontinuation rate was 4.3–35% (1–3 years of follow-up), with the rate of removal due to bleeding being 0–4%. In two reviews, the continuation rate was 84% and 73–93%, respectively, at 1 year, 50–58% at 3 years, and 11–54% at 5 years, indicating a declining rate over time. Reasons for discontinuation included bleeding, pain, and device expulsion. Most studies comparing LNG-IUD with other contraceptives reported no significant differences in discontinuation or continuation rates. Weight changes (weight gain) were reported in two systematic reviews, both indicating no significant weight changes. Fertility after device removal (pregnancy/infertility) was reported in two reviews. One review showed no increase in infertility risk, while the other reported no significant differences in planned pregnancies after device removal compared with other contraceptive methods (e.g., copper IUD). Sexual function was evaluated in two systematic reviews. One review reported no significant changes in sexual function up to 12 months after LNG-IUD insertion, whereas another showed no significant differences compared with copper IUDs, with better outcomes reported with LNG-IUDs than with natural cycle contraceptive methods. Other AEs included partial perforation and uterine muscle embedment, endometritis, abdominal/lower back pain, headaches, acne, breast tenderness, and bacterial vaginosis, with no significant differences compared with other contraceptives.

2) Health risks associated with the procedure Health risks associated with the procedure were assessed in terms of the incidence of breast cancer, psychiatric symptoms, venous thromboembolism (VTE), and infant growth. The association between LNG-IUD use and breast cancer was examined in six systematic reviews. Some studies included patients who underwent the procedure for medically necessary reasons, prompting a re-analysis of the primary studies included in these reviews. Of eight primary studies, five showed no significant association, while three did. Meta-analysis of seven studies presenting effect estimates showed a pooled relative risk of 1.1 (95% confidence interval: 1.00, 1.21) for breast cancer, but the difference from non-users was not significant. The relationship between LNG-IUD use and psychiatric symptoms, particularly depression, was assessed in three systematic reviews. The results of the primary studies were mixed, with some reporting an association and others not. Recent reviews emphasized the need for counselling on a potential increase in depression and further research on the topic. The association between LNG-IUD use and VTE was examined in two systematic reviews. Although the number of primary studies was limited, LNG-IUD use did not increase the risk of VTE. The association between LNG-IUD use and infant growth and breastfeeding was reported in two systematic reviews. The timing of LNG-IUD insertion had no significant effects on breastfeeding, and there were no significant differences in infant growth or breastfeeding between LNG-IUD insertion and non-hormonal IUDs.

Effectiveness The contraceptive efficacy of LNG-IUD was assessed based on seven systematic reviews. Two reviews reported the Pearl Index as 0.09–0.11 and 0.11. Another systematic review, which pooled results from individual studies, indicated a 1-year pregnancy rate of 0.016 (95% CI 0.01, 0.02), suggesting a very low pregnancy rate. Comparisons with other contraceptive methods showed that the pregnancy rate with the LNG-IUD was either lower or not significantly different.

Conclusion and Recommendations Based on the synthesis of current evidence from systematic reviews, the safety and effectiveness of the LNG-IUD can be summarized as follows: LNG-IUD was associated with changes in bleeding patterns, such as a reduction in bleeding and an increase in amenorrhea. The rate of removal due to bleeding was low, at 0–4%, and the bleeding patterns were acceptable in most women. The rate of device expulsion was low, and device removal had no significant effects on weight change or fertility. Other AEs were comparable to those of other contraceptive methods, suggesting minimal safety concerns for LNG-IUD insertion. However, some studies indicated an increased risk of breast cancer and depression with the use of LNG-IUD. Although there was no clear evidence linking LNG-IUD use to an increased risk of VTE or adverse effects on infant growth and breastfeeding, the Subcommittee determined that current evidence is insufficient to draw definitive conclusions. Thus, the Subcommittee recommended further research to clarify the health risks associated with LNG-IUD use. The contraceptive efficacy of the LNG-IUD was excellent even with one procedure, with a reported contraceptive failure rate comparable to that of sterilization. However, the level of evidence supporting these conclusions was “low” or “very low,” indicating the need for cautious interpretation. Based on Article 4–10 of the Health Technology Reassessment Project Management Guidelines, the twelfth 2023 Health Technology Reassessment Committee (December 8, 2023) and the first 2024 Health Technology Reassessment Committee (January 12, 2024) drew the following conclusions and finalized the public information document on “LNG-IUD insertion”: LNG-IUD is a highly effective long-term contraceptive method for women of reproductive age. Regarding safety, changes in bleeding patterns after the procedure were acceptable, with no adverse effects on fertility after device removal. Furthermore, the levels of other AEs were similar to those of other contraceptive methods, and no marked safety concerns were identified. Although some studies reported increased risks of breast cancer and depression, further research is needed to confirm these findings. To support informed medical decision-making among the public, a separate public information document on “LNG-IUD insertion” was provided based on these assessment results.
