당뇨병성 족부궤양에서 고압산소치료

Abstract

평가배경 고압산소치료는 현재 급여 항목으로, 신의료기술평가가 도입되기 전에 등재된 기술이다. 내부 모니터링을 통해 고압산소치료를 재평가 안건으로 발굴하였으며, 당뇨병성 족부궤양 환자 중 Wagner grade 3 이상이 건강보험 급여기준이다. 국외 임상진료지침에 따르면 그에 상충되는 주장 및 근거가 보고되고 있어 재평가가 필요하다는 의견이 있었다. 2023년 제6차 의료기술재평가위원회(2023.6.2.)에서 당뇨병성 족부궤양 환자를 대상으로 고압산소치료 기술의 안전성, 효과성에 대한 재평가가 필요하다고 심의하였다. 본 평가의 목적은 당뇨병성 족부궤양 환자에서 고압산소치료의 임상적 안전성 및 효과성 등에 대한 과학적 근거를 제공하여 의료기술의 적정사용 등 정책적 의사결정을 지원하고자 하였다.

평가방법 당뇨병성 족부궤양 환자에서 고압산소치료에 대한 안전성 및 효과성을 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “당뇨병성 족부궤양에서 고압산소치료 평가 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 내분비내과 2인, 성형외과 1인, 정형외과 1인, 근거기반의학 1인의 전문가 5인으로 구성하였다. 평가의 핵심질문은 “고압산소치료는 당뇨병성 족부궤양 환자에서 임상적으로 안전하고 효과적인가?”이었고, 안전성은 기압차에 의한 통증 등과 같은 부작용 및 합병증을 지표로 하였고, 효과성은 궤양 개선정도(궤양 부위 면적 감소율, 상처 치료율 등), 족부 절단율, 통증, 삶의 질을 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 3개 데이터베이스에서 검색하였으며, 문헌 선정과정은 문헌선택 및 배제기준에 따라 2명의 평가자가 독립적으로 수행하고, 의견의 불일치가 있는 경우 평가자간 합의를 통해 최종 논문을 결정하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias를 사용하여 최종 선택된 문헌을 대상으로 2명의 평가자가 독립적으로 평가를 실시하였으며, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 모든 자료는 연구단위로 추출하였다. 평가결과는 양적 분석(quantitative analysis)이 가능할 경우 메타분석을 수행하였고, 불가능할 경우 질적 검토(qualitative review) 방법을 적용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거 수준을 평가하고, 평가 결과를 토대로 권고등급을 결정하였다.

평가결과 본 평가에 최종 선택연구는 총 19편의 무작위배정임상시험연구(randomized controlled trials, RCT)이었다. RCT 연구 대상자는 총 1,112명이었고, 모든 연구대상자는 기본적으로 표준치료법(standard treatment, ST)을 받았다. 중재법은 표준치료에 고압산소치료를 병용하였고, 비교법은 표준치료와 모의대조를 같이 수행한 연구 5편(연구대상자 215명), 표준치료만 받은 연구 14편(연구대상자 897명)이었다. 비뚤림위험 수준은 불충분한 결과자료로 인해 11.8%에서 ‘높음’으로 평가되었다.

안전성 안전성 결과, 총 7편에서 합병증 및 이상반응을 보고하였다. 귀와 관련한 합병증 및 이상반응은 5편에서 보고하였고, 압력손상, 기압성 중이염, 이명 등의 발생률이 중재군에서 더 높았다. 시각 관련 이상반응을 다룬 2편에서 시각 변화, 백내장 악화 발생률이 중재군에서 더 높았다. 경련 및 발작은 1편에서 중재군에서만 1명(1/60명) 보고하였으며, 기흉과 같은 심각한 합병증은 발생하지 않았다. 모든 문헌에서는 두 군간 합병증 차이에 대한 통계적 유의성에 대해 언급하지 않았다.

효과성 효과성 결과, 총 19편에서 궤양 개선정도, 족부 절단율, 삶의 질을 보고하였다. 감소된 상처 면적 변화량(3편)은 메타분석 결과 중재군과 대조군간 유의한 차이가 없었으나, 감소된 상처 면적 비율(3편)은 메타분석 결과(3편) 중재군에서 더 유의하게 개선되었다(평균차(mean difference, MD) 24.45%, 95% CI 16.45, 32.46). 상처 치료율(9편)은 메타분석 결과, 비교군에 비해 중재군에서 상처가 치료된 환자 수가 유의하게 더 많았다(상대 위험도(relative risk, RR) 3.13, 95% CI 1.12, 8.75). 대절단 지표는 5편을 합성한 결과, 중재군이 비교군에 비해 유의하게 발생률이 낮았다(RR 0.56, 95% CI 0.37, 0.84). 소절단 지표는 6편을 합성한 결과, 두 군간 유의한 차이는 없었다(RR 1.34, 95% CI 0.95, 1.91). 통증 지표는 보고된 문헌이 없었으며, 삶의 질을 3편에서 보고하였는데 그 중 1편에서 비교군에 비해 중재군이 삶의 질이 개선되었고, 2편은 두 군간 차이가 없었다.

결론 및 제언 소위원회에서는 당뇨병성 족부궤양에서의 고압산소치료와 표준치료를 병용한 군이 표준치료 단독군에 비해 귀와 관련한 합병증 및 이상반응의 발생률은 중재군에서 더 높게 나타나는 경향이 있었으나, 기흉과 같은 심각한 합병증이 발생하지 않아 동 기술을 안전한 기술로 판단하였다. 또한, 중재군에서 표준치료 단독군과 비교하여 상처 치료 및 대절단 발생률의 유의한 개선효과가 있는 것을 바탕으로 동 기술을 통해 환자의 상처 치료에 도움을 줄 수 있는 효과적인 기술로 판단하였다. 2024년 제2차 의료기술재평가위원회(2024.02.16.)에서는 소위원회 검토 결과와 관련하여 논의를 하였고, 의료기술재평가사업 관리지침 제4조제10항에 의거 “당뇨병성 족부궤양 환자에서 고압산소치료”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 고압산소치료는 당뇨병성 족부궤양 환자를 대상으로 압력과 관련한 안전성의 우려가 있으며 족부궤양의 중증도(Wagner grade)에 따른 평가는 할 수 없었으나 상처치료, 대절단 발생률에 개선효과가 확인되어 국내 임상상황에서 중증 당뇨병성 족부궤양 환자에서 표준치료에 추가적으로 고압산소치료의 사용을 고려해 볼 수 있다고 판단하여 ‘조건부 권고함’으로 심의하였다.

Hyperbaric Oxygen Therapy for Diabetic Foot Ulcers

Background Since its registration before introducing the New Technology Assessment, hyperbaric oxygen therapy has been a covered item. Internal monitoring identified hyperbaric oxygen therapy as an agenda item for reassessment, with a Wagner grade 3 or higher for diabetic foot ulcer being the threshold for health insurance coverage. International clinical guidelines have reported conflicting claims and evidence, requiring reassessment. In 2023, the 6th Medical Technology Reassessment Committee (June 2, 2023) determined that it was necessary to reassess the safety and effectiveness of hyperbaric oxygen therapy technology for patients with diabetic foot ulcers. This assessment aimed to provide scientific evidence on the clinical safety and effectiveness of hyperbaric oxygen therapy in patients with diabetic foot ulcers to support policy decisions, including the appropriate use of medical technology.

Methods A systematic review was conducted to assess the safety and effectiveness of hyperbaric oxygen therapy in patients with diabetic foot ulcers. In light of the study's objectives, the “Subcommittee on the Assessment of Hyperbaric Oxygen Therapy for Diabetic Foot Ulcers” (the “Subcommittee”) discussed and finalized all methods. The subcommittee comprised five experts: two from endocrinology, one from plastic surgery, one from orthopedics, and one from evidence-based medicine. The key question of the assessment was: “Is hyperbaric oxygen therapy clinically safe and effective in patients with diabetic foot ulcers?” Safety was assessed based on adverse events and complications, such as pain due to barotrauma, and effectiveness was assessed based on ulcer improvement (ulcer area reduction, wound healing rate, etc.), foot amputation rate, pain, and quality of life. The systematic review used three international and three domestic databases based on the key questions. The literature selection process was conducted independently by two reviewers according to the inclusion and exclusion criteria. In the event of disagreement, the final articles were selected by consensus among the reviewers. The risk of bias in the literature was assessed using Cochrane's risk of bias by two reviewers for the articles finally selected. In the event of disagreement, consensus was reached by inter-reviewer agreement. All data were extracted on a per-study basis. Meta-analyses were conducted when quantitative analysis could be performed, and qualitative review methods were applied when quantitative analysis could not be performed. The results of the systematic reviews conducted for this assessment were assessed for quality of evidence using the Grading of Recommendations Assessment, Development, and Evaluation methodology, and the level of recommendation was determined based on the assessment results.

Results A total of 19 randomized controlled trials (RCTs) were selected for analysis. The RCTs included 1,112 participants, all receiving standard treatment (ST) by default. The interventions were ST combined with hyperbaric oxygen therapy, compared with ST and sham control in five studies (215 patients) and ST alone in 14 studies (897 patients). Due to insufficient results, the risk of skewness was assessed as “high” at 11.8%.

Safety For safety, a total of seven studies reported complications and adverse events. Ear-related complications and adverse events were reported in five studies, with higher rates of pressure injury, barotrauma, barotraumatic otitis media, and tinnitus in the intervention group. In the two studies that included vision-related adverse events, the incidence of vision changes and cataract worsening was higher in the intervention group. Convulsions and seizures were reported only in the intervention group in one study (1/60), and no serious complications, such as pneumothorax, occurred.
