기관지경이용 폐엽측부환기 검사

Abstract

평가배경 기관지경이용 폐엽측부환기 검사는 기관지내시경 폐용적축소술의 적응증인 폐기종을 동반한 중증 만성 폐쇄성폐질환자를 대상으로 기관지내시경 폐용적축소술의 반응을 예측하고자 측부환기 유무를 확인하는 검사이다. 기관지경이용 폐엽측부환기 검사는 2013년 신의료기술평가를 받은 이후 2018년 2월부터 선별급여 50%로 적용되었고, 선별급여 도입 후 4년이 경과하여 해당 기술의 임상적 안전성 및 효과성을 확인하고 보건의료자원의 효율적 사용을 위한 근거를 제공하기 위하여 재평가를 하였다.

평가방법 기관지경이용 폐엽측부환기 검사에 대한 안전성, 효과성 및 경제성을 평가하기 위해 체계적 문헌고찰을 하였다. 기관지경이용 폐엽측부환기 검사의 경우 검사 결과를 반영하여 기관지경이용 폐용적축소술-일방향 기관지밸브 삽입을 수행하므로 2개 기술 간 임상적 관련성이 높아 동일 소위원회를 구성하여 운영하기로 하였다. 모든 평가방법은 평가목적을 고려하여 “기관지경이용 폐엽측부환기 검사 및 기관지경이용 폐용적축소술 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 핵심질문은 “폐기종을 동반한 중증 만성 폐쇄성폐질환자 중 기관지내시경 폐용적축소술 대상선별에 기관지경이용 폐엽측부환기 검사가 안전하고 효과적인가?”이다. 안전성은 검사 관련 합병증을 보았고, 효과성은 기관지내시경 폐용적축소술(시술) 성공 예측도를 주요 결과지표로 하였다. 시술 성공 예측도 외에 측부환기 유무에 따른 폐용적축소술 이후의 폐용적 감소 관련 간접지표 및 폐용적축소술 이후 무기폐 발생과 기관지경이용 폐엽측부환기 검사결과 간 상관성을 통해 시술 성공을 간접적으로 확인하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선택한 후 합의과정을 거쳤다. 문헌의 비뚤림위험 평가는 quality assessment of diagnostic accuracy studies-2 (QUADAS-2)를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 추출하고, 의견 불일치가 있을 경우 제 3자와 함께 논의하여 합의하였다. 자료분석은 질적 검토(qualitative review) 방법을 적용하였다.

평가결과 최종 선택문헌은 총 7편의 코호트 연구였으며, 경제성 평가 문헌은 없었다. 모든 문헌에서 chartis 기기(ChartisⓇ System; Pulmonx Inc., Redwood, Calif., USA)를 이용하여 호흡기류, 압력, 저항치 자료를 통해 측부환기 유무를 측정하였다. 비교검사로는 컴퓨터 단층촬영을 이용한 경우가 선택문헌에 포함되었다. 비뚤림위험 평가결과, ‘환자선택’ 영역에서 기준이 명확히 제시되어 있지 않은 문헌 2편에 대해 불확실로 평가하였다. ‘중재검사’ 영역은 측부환기를 검사하는 데 있어 검사기기에서 바로 검사결과를 알 수 있고, 측부환기에 대해 검사하는 다른 검사법이 없는 점을 고려하여 타 검사결과와 관련 없이 검사결과가 해석되어 비뚤림위험을 낮게 평가하였다. ‘참고표준검사’와‘연구진행과 시점’ 영역은 참고표준검사가 없어 평가가 불가능하였다.

안전성 총 7편의 문헌 중 안전성 결과를 보고한 문헌은 3편으로 기관지경이용 폐엽측부환기 검사와 관련된 합병증이 발생하지 않았다고 보고하였다. 나머지 4편은 안전성 지표에 대한 언급이 없었다.

효과성 기관지경이용 폐엽측부환기 검사에 대한 폐용적 축소술(시술) 성공 예측도는 총 6편에서 보고하였다. 대부분의 문헌에서 시술 성공을 폐용적이 350㎖ 이상 감소한 경우로 정의하였고, 기관지경이용 폐엽측부환기 검사를 통해 공기의 흐름을 파악하여 측부환기가 없는 경우를 음성으로 판단하였다. 기관지경이용 폐엽측부환기 검사의 시술 성공에 대한 양성예측도는 0.7~0.82, 음성예측도는 0.73~1, 검사정확도는 0.74~0.85로 보고하였다. 컴퓨터 단층촬영과 비교시 시술 성공 예측도를 보고한 문헌은 총 3편으로 해당 문헌들에서는 컴퓨터 단층촬영 결과에 대해 틈새보전(fissure integrity)을 기준으로 보고하였다. 컴퓨터 단층촬영의 시술 성공에 대한 양성예측도는 0.75~0.88, 음성예측도는 0.67~0.93, 검사정확도는 0.71~0.89였다. 시술 성공 예측도 외 측부환기 유무에 따른 폐용적축소술 이후 폐용적 감소 관련 간접지표 및 폐용적축소술 이후 무기폐 발생과 기관지경이용 폐엽측부환기 검사 결과 간 상관성을 검토하였다. 측부환기 유무에 따른 폐용적축소술 이후 폐용적 감소 관련 간접지표를 보고한 문헌은 총 2편으로 관련 지표는 1초간 노력성 호기량, 잔기량, 6분 도보거리, 세인트조지 호흡기 설문지 점수(St. Georges’s respiratory questionnaire, SGRQ) 등을 포함하였다. 2편 문헌에 대한 검토결과, 폐용적축소술 후 1초간 노력성 호기량은 측부환기가 없는(collateral ventilation negative, CV-) 군이 측부환기가 있는(collateral ventilation positive, CV+) 군에 비해 더 많고(1초간 노력성 호기량 평균변화량(2편) CV-군 0.14~0.16L vs. CV+군 –0.005~0.01L), 6분 도보거리는 더 길어 CV+군보다 폐기능 개선효과가 있었다(6분 도보거리 평균변화량(2편) CV-군 vs. CV+군 24~31.7m vs. 5~7m). SGRQ점수는 낮을 수록 삶의 질이 더 좋은 것을 의미하고, 1편에서 CV-군이 CV+군에 비해 점수가 더 낮았고(SGRQ점수 평균변화량 CV-군 -10점 vs. CV+군 -5점), 다른 1편에서는 반대로 CV-군이 점수가 더 높았다. 모든 지표에서 두 군간 유의성에 대해서는 보고되지 않았다. 폐용적축소술 후 무기폐 발생과 기관지경이용 폐엽측부환기 검사 결과(즉, 측부환기 유무) 간 상관성에 보고한 문헌은 총 3편이였다. 3편 중 2편에서는 폐엽 저항력이 높은 것은 측부환기가 없다는 것을 의미하고 측부환기가 없는(CV-) 환자에서 무기폐 발생이 높아, 이를 토대로 측부환기와 무기폐 간 유의한 상관성이 있다고 하였다. 나머지 1편에서는 측부환기가 없는(CV-) 군에서만 무기폐가 발생하여 측부환기와 무기폐 간 관련성이 있음을 확인하였다.

결론 및 제언 소위원회에서는 체계적 문헌고찰 결과를 바탕으로 기관지경이용 폐엽측부환기 검사의 안전성은 3편에서 관련 합병증이 발생하지 않았다고 보고하였으나, 기관지내시경 기반 센서를 통하여 공기흐름 및 압력을 측정하여 측부환기 유무를 측정하는 원리를 가지기 때문에 기관지내시경과 유사할 것으로 평가하였다. 또한 폐용적 축소술(시술) 성공 예측도가 컴퓨터 단층 촬영과 비슷한 수준이며, 폐용적축소술 후 측부환기가 있는 군에 비해 측부환기가 없는 군에서 폐기능이 향상되었고, 측부환기 유무와 무기폐 간 유의한 상관성을 보고한 근거를 바탕으로 기관지경이용 폐엽측부환기 검사를 통해 기관지내시경 폐용적축소술이 효과적일 것이라고 예상되는 측부환기가 없는 환자를 미리 선별하는데 동 검사가 효과적인 기술로 평가하였다. 2023년 제1차 의료기술재평가위원회(2023.1.13.)에서는 소위원회 검토 결과에 대하여 논의한 결과, 의료기술재평가사업 관리지침 제4조제10항에 의거 “기관지경이용 폐엽측부환기 검사”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황이나 가치에 따라 폐기종을 동반한 중증 만성 폐쇄성폐질환자 중 기관지내시경 폐용적축소술의 대상을 선별하는데 기관지경이용 폐엽측부환기 검사를 ‘권고함’으로 심의하였다.

Background The bronchoscopic lung lobal collateral ventilation (CV) test aims to predict the response to bronchoscopic lung volume reduction and check for CV in patients with severe chronic obstructive lung disease (COPD) and emphysema, which is an indication for bronchoscopic lung volume reduction.

After a new health technology assessment in 2013, a bronchoscopic lung lobal CV test was subsequently adopted as a selective benefit item (50% co-pay) starting in February 2018. Four years after the adoption of selective benefits, this technology has been reassessed to ensure clinical safety and effectiveness and to provide evidence for efficient use of public health resources.

Methods

A systematic review was performed to assess the safety, effectiveness, and cost-effectiveness of the bronchoscopic lung lobal CV test. Bronchoscopic lung volume reduction is performed using a unilateral endobronchial valve, based on the results obtained from the bronchoscopic lung lobal CV test. This results in a strong clinical correlation between the two technologies. Accordingly, the same subcommittee was formed and operated for these two technologies. All assessment methods were finalized through discussions with the “Bronchoscopic lung lobal CV test and bronchoscopic lung volume reduction subcommittee” (hereinafter referred to as “subcommittee”) with consideration of the purpose of the assessment. The key question was, “Is the bronchoscopic lung lobal CV test safe and effective for screening patients with severe COPD and emphysema for bronchoscopic lung volume reduction?” Safety was assessed based on test-related adverse events (AEs). For effectiveness, procedural (bronchoscopic lung volume reduction) success prediction was used as the key outcome indicator. In addition to procedural success prediction, indirect indicators related to lung volume reduction following the procedure, which included the presence/absence of CV and the correlation between atelectasis after lung volume reduction and bronchoscopic lung lobal CV test results, were used to indirectly identify the procedural success.

In the literature review, three international and five domestic databases were searched based on the key question. Two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria to reach a consensus. Two reviewers independently performed a risk of bias (RoB) assessment using Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2) and reached a consensus. Data extraction was performed by two reviewers using a predetermined format. Any disagreements were resolved by discussing with a third person to reach a consensus. For data analysis, a qualitative review was used.

Results

A total of seven articles were selected, all of which were cohort studies, and no articles on cost-effectiveness were found. In all studies, the Chartis® System (Pulmonx Inc., Redwood, Calif., USA) was used to measure CV based on respiratory flow, pressure, and resistance data. The use of computed tomography (CT) as the comparator test was included in the selected articles. In the RoB assessment, the patient selection domain RoB was assessed to be uncertain in two articles that did not clearly present the criteria. The RoB for the index test domain was assessed to be low because the test results were interpreted independently, considering that the CV results can be directly obtained from the testing equipment and no alternative test methods exist for CV. Meanwhile, RoB for the reference standard and flow and timing domains could not be assessed due to the unavailability of a reference standard.

Safety

Among the seven articles that were selected, safety was reported in three articles. These three articles reported no complications associated with the bronchoscopic lung lobal CV test. The remaining four articles did not mention any safety indicators.

Effectiveness

The procedural (lung volume reduction) success prediction for the bronchoscopic lung lobal CV test was reported in six articles. Most articles defined procedural success as a lung volume reduction of ≥350 ml, while a bronchoscopic lung lobal CV test was used to identify air flow and determined the absence of CV as a negative result. The positive and negative predictive values for procedural success of the bronchoscopic lung lobal CV test were 0.7–0.82 and 0.73–1, respectively, while the test accuracy was reported to be 0.74–0.85. Three articles reported about the procedural success rate in comparison to CT based on fissure integrity. The positive and negative predictive values for procedural success of CT were 0.75–0.88 and 0.67–0.93, respectively, while the test accuracy was reported to be 0.71–0.89.

In addition to procedural success prediction, indirect indicators related to lung volume reduction after the procedure, including the presence/absence of CV and correlation between atelectasis after lung volume reduction and bronchoscopic lung lobal CV test results, were also reviewed.

Two articles reported indirect indicators related to lung volume reduction after lung volume reduction procedures according to the presence/absence of CV, which included forced expiratory volume at one second (FEV1), residual volume, 6-min walking distance (6MWD), and St. George's respiratory questionnaire (SGRQ) scores. A review of these two articles showed that FEV1 after lung volume reduction was higher in the group without CV (CV-) than the group with CV (CV+) [mean change in FEV1 (n=2) – CV- group: 0.14–0.16L vs. CV+ group: –0.005–0.01L)] and 6MWD was longer in the CV- group than in the CV+ group [mean change in 6MWD (n=2) – CV- group: 24–31.7 m vs. CV+ group: 5–7 m), indicating greater improvement in lung function in the CV- group than in the CV+ group. Lower SGRQ scores indicate better quality of life, and in one article, the CV- group showed lower SGRQ scores than the CV+ group (mean change in SGRQ score – CV- group: -10 points vs. CV+ group: -5 points), whereas the other article reported the opposite, with the CV- group showing a higher score. For all indicators, significance between the two groups was not reported.

The correlation between atelectasis after lung volume reduction and bronchoscopic lung lobal CV test results (meaning presence/absence of CV) was reported in three articles. In two of the three articles, a significant correlation between CV and atelectasis was reported based on higher lobar resistance, indicating no CV, and the incidence of atelectasis was higher in CV- patients. The remaining article also confirmed a correlation between CV and atelectasis based on atelectasis occurring only in the CV- group.

Conclusions and recommendations

Based on a review of the systematic review findings, the subcommittee determined that the safety of a bronchoscopic lung lobal CV test would be similar to that of bronchoscopy since the test is based on the principle of determining CV by measuring air flow and pressure through a bronchoscopy-based sensor, and no test-related complications were reported in three articles. Moreover, the subcommittee also determined that the bronchoscopic lung lobal CV test is effective for pre-screening CV- patients who may benefit from bronchoscopic lung volume reduction. This is supported by similar procedural success rates compared to CT, improved lung function in the CV+ group after lung volume reduction as compared to the CV- group, and a significant correlation between CV and atelectasis.

In the first session of the Committee of Health Technology Reassessment in 2023 (January 13, 2023), the following conclusions were made regarding the Bronchoscopic lung lobal CV test based on the subcommittee findings in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project.

Accordingly, the Committee of Health Technology Reassessment “recommends” the use of a bronchoscopic lung lobal CV test for screening patients who should undergo a bronchoscopic lung volume reduction among patients with severe COPD and emphysema according to the clinical situation or value in Korea based on a comprehensive consideration of the evidence of its clinical safety and effectiveness and other assessment items.
