기관지경이용 폐용적축소술 - 일방향 기관지밸브 삽입

Abstract

평가배경 기관지경이용 폐용적축소술-일방향 기관지밸브 삽입 (이하 ‘일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술’)은 폐기종을 동반한 중증 만성 폐쇄성폐질환자를 대상으로 일방향 기관지밸브를 기관지내에 삽입하여 호기 시에는 공기가 시술한 폐엽에서 빠져나오고 흡기 시에는 들어가지 못하게 하여 시술부위의 폐용적을 감소시켜 전체적으로 폐기능을 향상시키고 호흡곤란을 개선하고자 수행하는 시술이다. 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술(bronchoscopic lung volume reduction using unilateral endobronchial valve, EBV)은 2013년 신의료기술평가를 받은 이후 2018년 2월부터 선별급여 50%로 적용되었고, 선별급여 도입 후 4년이 경과하여 해당 기술의 임상적 안전성 및 효과성을 확인하고 보건의료자원의 효율적 사용을 위한 근거를 제공하기 위하여 재평가를 하였다.

평가방법 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술에 대한 안전성, 효과성 및 경제성을 평가하기 위해 체계적 문헌고찰을 수행하였다. 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술의 경우 기관지경이용 폐엽측부환기 검사의 검사 결과를 반영하여 시술을 수행하므로 2개 기술 간 임상적 관련성이 높아 동일 소위원회를 구성하여 운영하기로 하였다. 모든 평가방법은 평가목적을 고려하여 “기관지경이용 폐엽측부환기 검사 및 기관지경이용 폐용적축소술 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 평가의 핵심질문은 “폐기종을 동반한 중증 만성 폐쇄성폐질환자에서 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술은 임상적으로 안전하고 효과적인가?”이다. 안전성은 시술 관련 합병증 지표로 보았고, 효과성은 4개로 범주화하여 폐기능 향상, 운동능력 향상, 삶의 질, COPD 사망위험인자 지표인 BODE 지표(체질량지수(body mass index), 기류제한(obstruction of airflow), 호흡곤란(dyspnea), 운동능력(exercise)으로 구성)로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 연구유형별로 무작위 배정 임상시험 연구(randomized controlled trial, RCT)는 cochrane의 risk of bias (RoB), 비무작위 연구(non randomized study, NRS)는 risk of bias assessment tool for nonrandomized studies (RoBANS)를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 추출하고, 의견 불일치가 있을 경우 제 3자와 함께 논의하여 합의하였다. 자료분석은 양적 분석(quantitative analysis)이 가능할 경우 메타분석을 수행하였으며, 양적 분석이 불가능할 경우 질적 검토(qualitative review) 방법을 적용하였다. 체계적 문헌고찰 결과의 근거수준은 grading of recommendations assessment, development and evaluation (GRADE) 접근 방법으로 평가하였다.

평가결과 최종 선택문헌은 총 13편(RCT 9편, NRS 2편, 경제성평가 연구 2편)이었다. 모든 문헌에서 zephyr endobronchial valve(Pulmonx Corp., Redwood, Calif., USA)를 이용하여 시술을 하였다. 비교검사는 약물 및 재활치료 등의 표준치료군 또는 모의대조군이 있었다. 비뚤림위험 평가결과, RCT의 경우 불충분한 자료 영역은 2편에서 높은 탈락률이 보고되어 비뚤림위험이 높은 것으로 판단하였다. 연구 참여자, 연구자에 대한 눈가림, 결과평가에 대한 눈가림, 선택적 보고 영역에서는 비뚤림위험을 ‘낮음’으로 평가하였다. 1편을 제외한 모든 문헌에서 민간 재정지원을 받아 연구를 수행하였기 때문에 기타 비뚤림(민간연구비지원) 영역은 비뚤림위험이 높은 것으로 판단하였다. NRS는 대상군 비교가능성, 대상군 선정, 평가자의 눈가림, 결과평가, 불완전한 결과자료, 선택적 보고 영역에서 비뚤림위험이 낮았다. 교란변수 영역은 설계단계나 분석단계에서 교란변수에 대한 언급이 없고, 노출측정 영역도 중재 및 대조군에 대한 시술방법을 구체적으로 제시한 내용이 없어 불확실로 평가하였다.

안전성 총 8편(RCT 7편, NRS 1편)의 문헌에서 안전성에 대해 보고하였고, 안전성 결과지표는 전체 이상반응 및 사망, 기흉, chronic obstructive pulmonary disease(COPD) 악화, 폐렴, 호흡부전 등 주요 합병증을 중심으로 안전성 여부를 판단하였다.

전체 이상반응 4편의 RCT를 메타분석한 결과, EBV군이 약물 및 재활치료 등의 표준치료군에 비해 전체 이상반응 발생위험비가 유의하게 더 높았다(발생위험비(relative risk ratio, (RR) 4.42, 95% 신뢰구간(confidence interval, CI) 2.74, 7.13, I2=0%). 1편의 NRS에서 전체(호흡기계) 이상반응을 중증도에 따라 보고하였다. 중증 합병증은 EBV군 20명 중 8명(40%), 약물 및 재활치료 등의 표준치료군에서는 18명 중 5명(27.7%)에서 발생하였고, 경증 합병증은 EBV군 20명 중 12명(60%), 약물 및 재활치료 등의 표준치료군 18명 중 12명(66.6%)에서 발생하였다.

주요 합병증 주요 합병증의 종류는 사망, 기흉, COPD 중증도 악화, 폐렴, 호흡부전, 밸브 관련 이상반응 등이 보고되었다. 사망을 보고한 총 6편의 RCT 중 5편에서는 EBV군이 대조군(약물 및 재활치료 등의 표준치료군 4편, 모의대조군 1편)에 비해 사망발생률이 더 높았다. 구체적으로는 2편에서는 EBV군에서만 사망 발생(1.5~2.9%)을 보고하였다. 다른 1편은 EBV군에서 6명(5.4%), 약물 및 재활치료 등의 표준치료군에서 4명(6.7%)이 사망하였다. 또 다른 1편의 연구에서는 EBV군에서 16명(7.5%), 약물 및 재활치료 등의 표준치료군에서 3명(3.4%)이 사망하였다. 1편의 연구에서는 EBV군에서 사망이 발생하지 않았으나, 약물 및 재활치료 등의 표준치료군 중 1명(2%)이 사망하였고, 나머지 1편에서는 EBV군 25명을 대상으로 2명(8%), 모의대조군 25명에서 1명(4%)에서 사망하였다. 기흉에 대해 보고한 7편의 RCT 중 5편에서는 EBV군에서만 기흉이 발생(5.1~29.2%)하였고 약물 및 재활치료 등의 표준치료군에서는 기흉이 발생하지 않았다. 나머지 2편의 연구 중 1편은 EBV군에서 11명(5.1%), 약물 및 재활치료 등의 표준치료군에서는 2명(2.3%)에서 기흉이 발생하였으며, 나머지 1편은 EBV군, 모의대조군 각각 2명(8%), 1명(4%)에서 기흉이 발생하였다. COPD 중증도 악화는 RCT 5편의 연구결과를 메타분석한 결과, 두 군간 유의한 차이가 없었다(RR 1.17, 95% CI 0.71, 1.94. I2=53%). 폐렴은 4편의 연구결과를 메타분석한 결과, 두 군간 유의한 차이가 없었다(RR 1.13, 95% CI 0.58,2.19, I2=0%). 호흡부전은 2편에서 보고되었다. 1편의 연구에서는 EBV군에서 10명(9%), 약물 및 재활치료 등의 표준치료군에서는 6명(10%)이 호흡부전이 발생하였다. 나머지 1편은 EBV군 중 6명(2.8%), 약물 및 재활치료 등의 표준치료군 중 2명(2.3%)에서 호흡부전이 발생하였다. 장비 관련 이상반응으로 밸브와 관련된 이상반응이 보고되었다. 밸브 관련 이상반응에 대해 보고한 6편의 문헌에서는 EBV군에서만 해당 이상반응이 발생(1.5~24%)하였다. 1편의 NRS에서 주요 합병증의 종류로 사망, 기흉 및 밸브 관련 이상반응이 보고하였다. 사망은 약물 및 재활치료 등의 표준치료군 18명 중 1명(5.6%) 발생하였고, 기흉은 EBV군 20명 중 2명(10%)에서 발생하였다. 밸브와 관련 이상반응은 EBV군 및 약물 및 재활치료 등의 표준치료군 모두에서 heimlich valve 이용은 없었으며 밸브 제거는 EBV군에서만 7명(35%)이 발생하였다.

효과성 총 11편(RCT 9편, NRS 2편)의 문헌에서 효과성에 대해 보고하였고, 효과성 결과지표는 4개로 범주화하여 폐기능 향상, 운동능력 향상, 삶의 질, BODE 지표로 평가하였다.

폐기능 향상 총 9편 RCT에서 폐기능 향상에 대한 결과를 보고하였다. 메타분석한 결과, 폐기능 지표 중 1초간 노력성 호기량(6편; weighted mean difference (WMD) 17.09, 95% CI 9.02, 25.16, I2=80%), 잔기량(4편; WMD –0.65, 95% CI –0.82,-0.48, I2=41%)에 대해 대조군(약물 및 재활치료 등의 표준치료군 또는 모의대조군)보다 EBV군에서 유의하게 폐기능이 더 향상되었다. 이외 전폐용량, 노력성 폐활량, 기능성 잔류 용량, 총 폐용적 감소 지표들도 모두 EBV군이 대조군(약물 및 재활치료 등의 표준치료군 또는 모의대조군)에 비해 더 좋은 경향성을 보였다. 2편의 NRS에서 폐기능 향상에 대해 보고하였다. 1편은 추적관찰 3, 6개월 시점에서 폐기능 지표인 1초간 노력성 호기량, 잔기량, 및 기능 잔기용량 지표 모두가 EBV군에서 약물 및 재활치료 등의 표준치료군에 비해 유의하게 개선된 것으로 보고하였다. 나머지 1편의 연구에서는 시술 6개월 후 목표 폐엽(target lobe)의 용적이 EBV군에서 시술전보다 통계적으로 유의하게 감소하였으나(평균 -0.451, p<0.0001), 대조군인 약물 및 재활치료 등의 표준치료군에서는 시술 전과 유의한 차이가 없는 것으로 보고하였다(평균 –0.0051, p=0.70). 동일측 폐엽(ipsilateral lobe) 역시 EBV군에서는 유의하게 감소하였으나(rho=-0.68, p<0.0001), 약물 및 재활치료 등의 표준치료군에서는 유의하지 않았다(rho=0.10, p=0.28). 반대측 폐엽(contralateral lobe)는 두 군 모두에서 통계적으로 유의하게 감소하였다(EBV군: rho=-0.16, p<0.0001; 약물 및 재활치료 등의 표준치료군: rho=0.45, p=0.0056). 총 폐용적도 두 군 모두에서 통계적으로 유의하게 감소하였다(EBV군: rho=-0.53, p<0.0001); 약물 및 재활치료 등의 표준치료군: rho=0.60, p<0.0001).

운동능력 향상 운동능력 향상은 총 10편 RCT에서 보고되었다. 6분 도보거리(6 min walking distance, 6MWD)는 EBV군이 대조군(약물 및 재활치료 등의 표준치료군 또는 모의대조군)에 비해 유의하게 개선되었으며(6편; WMD 42.37, 95% CI 17.62, 67.13, I2=79%), 사이클 운동능력(cycle workload)을 보고한 2편에서는 EBV군이 약물 및 재활치료 등의 표준치료군에 비해 개선된 경향성을 보고하였다. 1편 NRS에서 추적관찰 3,6개월 시점에서 대조군인 약물 및 재활치료 등의 표준치료군에 비해 EBV군이 유의하게 운동능력이 개선되었다(3개월 EBV군: 50%, 약물 및 재활치료 등의 표준치료군: 5.6%, p=0.004; 6개월 EBV군: 55%, 약물 및 재활치료 등의 표준치료군: 5.6%, p=0.001).

삶의 질 삶의 질은 8편 RCT에서 보고하였다. 메타분석한 결과, EBV군이 대조군(약물 및 재활치료 등의 표준치료군 또는 모의대조군)에 비해 세인트 조지 호흡기 설문지(St. George's respiratory questionnaire, SGRQ)로 측정된 삶의 질이 유의하게 개선되었다(7편; WMD –5.76, 95% CI –7.66,-3.86, I2=2%). 호흡곤란 정도 평가 척도(modified medical research council dyspnea scale, mMRC dyspnea scale)는 EBV군이 약물 및 재활치료 등의 표준치료군에 비해 유의하게 개선되었다(4편; WMD –0.41, 95% CI –0.57,-0.26, I2=0%). 1편 NRS에서 추적관찰 3, 6개월 시점에서 대조군인 약물 및 재활치료 등의 표준치료군에 비해 EBV군이 유의하게 삶의 질이 개선되었다(3개월 EBV군: 70%, 약물 및 재활치료 등의 표준치료군: 27.8%, p=0.02; 6개월 EBV군: 85%, 약물 및 재활치료 등의 표준치료군: 38.9%, p=0.006).

BODE 지표 BODE 지표를 보고한 3편의 RCT 및 1편의 NRS에서는 모두 EBV군이 약물 및 재활치료 등의 표준치료군에 비해 유의하게 개선되었다.

경제성 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술 관련 비용-효과성을 보고한 문헌은 2편이었다. 2편의 연구 모두에서 약물 및 재활치료 등의 표준치료군 대비 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술이 더 비용-효과적이라고 제시하였다. 1편에서는 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술 을 통해 사망률 감소 및 삶의 질 개선을 기대할 수 있고, 분석기간 5년 동안 1년의 수명(life-year)연장을 위해 동 기술이 약물 및 재활치료 등의 표준치료군에 비해 약 79,100 유로의 비용이 더 소요되며, 분석기간 10년 동안 1년의 수명연장 당 34,883 유로의 비용이 소요되는 것으로 보고하였다. 다른 1편은 분석기간 5년 일때 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술을 수행으로 1 질보정수명(quality-adjusted life year, QALY)를 개선하기 위해 약물 및 재활치료 등의 표준치료군에 비해 약 46,322유로가 더 소요되고, 분석기간이 10년일 때는 1 QALY 개선 당 25,142유로의 비용이 더 발생하는 것으로 보고하였다.

결론 및 제언 소위원회에서는 체계적 문헌고찰 검토 결과, 전체 이상반응과 사망, 기흉, COPD 중증도 악화, 폐렴 등과 같은 주요 합병증이 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술 군에서 더 많이 발생한 것으로 확인하여 동 기술의 안전성이 명확히 확보되었다고 보기 어렵다고 판단하였다. 다만, 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술 군에서 폐기능 및 운동능력의 향상과 삶의 질 개선 등 유의한 효과를 확인할 수 있어, 동 기술을 위해 대비 더 큰 이득을 기대할 수 있는 기술로 판단하였다. 또한 일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술의 시술환자 선택에 있어 신중한 고려가 필요하다는 의견이었다. 2023년 제1차 의료기술재평가위원회(2023.1.13.)에서는 소위원회 검토 결과에 대하여 논의한 결과, 의료기술재평가사업 관리지침 제4조제10항에 의거 “일방향 기관지밸브를 이용한 기관지내시경 폐용적축소술”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황이나 가치에 따라 폐기종을 동반한 중증 만성 폐쇄성폐질환자를 대상으로 한 일방향기관지밸브를 이용한 기관지내시경 폐용적축소술을 ‘조건부 권고함’으로 심의하였다.

Background Bronchoscopic lung volume reduction-insertion of unilateral endobronchial valve (hereinafter referred to as “bronchoscopic lung volume reduction using unilateral endobronchial valve; BLVR using unilateral EBV”) is a procedure used on patients with severe chronic obstructive pulmonary disease (COPD) and emphysema, in which a unilateral EBV is inserted into the bronchus to prevent outflow of air from the treated lobe during expiration and inflow of air during inspiration, consequently reducing lung volume in the treated area. This procedure aims to improve the overall lung function and breathing difficulties. After a new health technology assessment in 2013, BLVR using unilateral EBV was subsequently adopted as a selective benefit item (50% co-pay) starting in February 2018. Four years after the adoption of selective benefits, this technology has been reassessed for clinical safety and effectiveness and to provide evidence for the efficient use of public health resources.

Methods A systematic review was performed to assess the safety, effectiveness, and cost-effectiveness of BLVR using unilateral EBV. BLVR, using unilateral EBV, is performed based on the results obtained from the bronchoscopic lung lobal collateral ventilation test, resulting in a strong clinical correlation between the two technologies. Accordingly, the same subcommittee was formed and operated for these two technologies. All assessment methods were finalized through discussions with the “Bronchoscopic lung lobal collateral ventilation test and bronchoscopic lung volume reduction subcommittee” (hereinafter referred to as “subcommittee”) with consideration of the purpose of the assessment. The key question of the assessment was, “Is BLVR using unilateral EBV clinically safe and effective for patients with severe COPD and emphysema?” Safety was assessed based on test-related adverse events (AEs), while effectiveness was assessed based on four categories: improved lung function, improved workload, quality of life (QoL), and BODE (Body mass index, Obstruction of airflow, Dyspnea, and Exercise) index, which is an indicator of risk factors for COPD mortality. For the literature review, three international and five domestic databases were searched based on the key question, and two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria. Two reviewers independently performed the risk of bias (RoB) assessment and reached a consensus on the findings, using Cochrane’s Risk of Bias Tool for randomized controlled trials (RCTs) and the Risk of Bias Assessment for Non-randomized Studies (RoBANS) for non-randomized studies (NRSs). The data were extracted independently by two reviewers using a predetermined format. Any disagreements were resolved by discussing with a third person to reach a consensus. For data synthesis, a meta-analysis was performed if a quantitative synthesis was possible, and a qualitative review was performed if not. Subsequently, the systematic review results were assessed for their level of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach.

Results A total of 13 articles (nine RCTs, two NRSs, and two cost-effectiveness assessment studies) were selected. In all articles, the procedure was performed using Zephyr EBV (Pulmonx Corp., Redwood, Calif., USA). Comparator tests included standard treatments, such as drugs and rehabilitation, and sham treatments. Based on the RoB assessment results, a high RoB was determined in the incomplete data domain of two studies reporting elevated dropout rates. Meanwhile, the RoB in patient/assessor blinding, outcome blinding, and selective reporting was assessed to be “low.” Because all studies, except one, were conducted with private financial support, the RoB in the other bias (private research funding) domain was determined to be high. In NRSs, low RoB was observed in the domains related to target group comparisons, target group selection, blinding of assessors, outcome assessment, incomplete outcome date, and selective reporting domains. The confounders domain was assessed to be uncertain due to the absence of mentions of confounders in the design or analysis stages, while the exposure measurement domain was also assessed to be uncertain, as it lacked specific details about the procedures applied to the intervention and control groups.

Safety Safety was reported in a total of eight articles (seven RCTs and one NRS), in which it was determined based on safety outcome indicators such as overall AEs, mortality, pneumothorax, exacerbation of COPD, pneumonia, and respiratory failure.

Overall AEs In the meta-analysis of four RCTs, the EBV showed a significantly higher relative risk ratio (RR) for overall AEs than the standard treatment (drug and rehabilitation therapy) group [RR: 4.42, 95% confidence interval (CI): 2.74, 7.13, I2=0%]. One NRS reported overall (respiratory) AEs according to severity. Severe complications were found in 8 out of 20 patients (40%) in the EBV group and 5 out of 18 patients (27.7%) in the standard treatment (drug and rehabilitation therapy) group, while mild complications were found in 12 out of 20 patients (60%) in the EBV group and 12 out of 18 patients (66.6%) in the standard treatment (drug and rehabilitation therapy) group.

Major complications Major complications reported included mortality, pneumothorax, exacerbation of COPD, pneumonia, respiratory failure, and valve-related AEs. A total of six RCTs reported on mortality, and of these, five RCTs showed a higher mortality rate in the EBV group than in the control group [standard treatment group (drug and rehabilitation therapy): n=4, sham control: n=1]. Specifically, two studies reported mortalities only in the EBV group (1.5–2.9%). One study reported six mortalities in the EBV (5.4%) and four mortalities in the standard treatment (drug and rehabilitation therapy) group. Another study reported 16 mortalities (7.5%) in the EBV group and three mortalities (3.4%) in the standard treatment (drug and rehabilitation therapy) group. One other study reported no mortality in the EBV group and one mortality (2%) in the standard treatment (drug and rehabilitation therapy) group, while the remaining one study reported two mortalities among 25 patients (8%) in the EBV group and one mortality out of 25 patients (4%) in the sham control group. Among seven RCTs that reported on pneumothorax, five RCTs reported pneumothorax only in the EBV group (5.1–29.2%) and no pneumothorax in the standard treatment (drug and rehabilitation therapy) group. Of the remaining two RCTs, one RCT reported 11 cases (5.1%) in the EBV group and two cases (2.3%) in the standard treatment (drug and rehabilitation therapy) group, while the other RCT reported two cases (8%) in the EBV group and one case (4%) in the sham control group. A meta-analysis of five RCTs that reported COPD exacerbations showed no significant difference between the two groups (RR 1.17, 95% CI 0.71, 1.94, I2=53%). The meta-analysis of four RCTs that reported on pneumonia also showed no significant difference between the two groups (RR 1.13, 95% CI 0.58, 2.19, I2=0%). Respiratory failure was reported in two studies, with one reporting ten cases (9%) in the EBV group and six cases (10%) in the standard treatment (drug and rehabilitation therapy) group, and the other reporting six cases (2.8%) in the EBV group and two cases (2.3%) in the standard treatment (drug and rehabilitation therapy) group. Valve-related AEs were reported as equipment-related AEs. In six studies that reported on valve-related AEs, such AEs occurred only in the EBV group (1.5–24%). One NRS reported mortality, pneumothorax, and valve-related AEs as types of major complications. Mortality was found in 1 out of 18 patients (5.6%) in the standard treatment (drug and rehabilitation therapy) group and 2 out of 20 patients (10%) in the EBV group. Regarding valve-related AEs, no use of the Heimlich valve was found in either the EBV or standard treatment (drug and rehabilitation therapy) groups, while valve removal occurred in seven cases (35%) exclusively within the EBV group.

Effectiveness Effectiveness was reported in a total of 11 articles (nine RCTs and two NRSs), in which it was assessed based on four categories of effectiveness outcome indicators: improved lung function, improved workload, QoL, and BODE index.

Improved lung function Nine RCTs reported outcomes regarding improved lung function. Meta-analysis results showed a significantly greater improvement in the EBV group as compared to the control group [standard treatment (drug and rehabilitation therapy) or sham control group] with respect to forced expiratory volume at one second (FEV1; n=6; weighted mean difference (WMD) 17.09, 95% CI 9.02, 25.16, I2=80%) and residual volume (n=4; WMD –0.65, 95% CI –0.82,-0.48, I2=41%). Moreover, the EBV tended to show better outcomes than the control group [standard treatment (drug and rehabilitation therapy) or sham control group] with respect to total lung capacity (TLC), forced vital capacity, functional residual capacity (FRC), and total lung volume reduction indicators. Two NRSs also reported improved lung function. One study reported that the EBV group showed a significant improvement in lung function indicators such as FEV1, residual volume, and FRC at 3- and 6-month follow-up, as compared to the standard treatment (drug and rehabilitation therapy) group. The other study reported that the EBV group showed a statistically significant decrease in target lobe volume at 6 months after as compared to before the procedure (mean: -0.451, p<0.0001), whereas the control group [standard treatment (drug and rehabilitation therapy) group] showed no significant difference as compared to before the procedure (mean: –0.0051, p=0.70). The EBV group also showed a significant decrease in the ipsilateral lobe (rho=-0.68, p<0.0001), whereas the standard treatment (drug and rehabilitation therapy) group showed no significant difference (rho=0.10, p=0.28). Meanwhile, both groups showed a statistically significant decrease in the contralateral lobe (EBV group: rho=-0.16, p<0.0001; standard treatment (drug and rehabilitation therapy) group: rho=0.45, p=0.0056). Both groups also showed a statistically significant decrease in TLC (EBV group: rho=-0.53, p<0.0001); standard treatment (drug and rehabilitation therapy) group: rho=0.60, p<0.0001).

Improved workload Ten RCTs reported an improved workload. The results showed a significant improvement in 6-min walking distance (6MWD) in the EBV group as compared to the control group [standard treatment (drug and rehabilitation therapy) or sham control group] (n=6; WMD 42.37, 95% CI 17.62, 67.13, I2=79%). In two studies that reported on cycle workload, the EBV group exhibited a tendency for greater improvement than the standard treatment (drug and rehabilitation therapy) group. In one NRS, the EBV group showed a significant improvement in workload at 3- and 6-month follow-up, as compared to the standard treatment (drug and rehabilitation therapy) group (3-month EBV group: 50%, the standard treatment (drug and rehabilitation therapy) group: 5.6%, p=0.004; 6-month EBV group: 55%, the standard treatment (drug and rehabilitation therapy) group: 5.6%, p=0.001).

QoL Eight RCTs were reported on QoL. Meta-analysis results showed that QoL measured by St. George's respiratory questionnaire (SGRQ) was significantly improved in the EBV group as compared to the control group [standard treatment (drug and rehabilitation therapy) or sham control group] (n=7; WMD –5.76, 95% CI –7.66, -3.86, I2=2%). Moreover, the EBV group also showed a significant improvement in modified Medical Research Council dyspnea scale (mMRC dyspnea scale) scores than the standard treatment (drug and rehabilitation therapy) group (n=4; WMD –0.41, 95% CI –0.57, -0.26, I2=0%). One NRS reported a significant improvement in QoL in the EBV group at 3- and 6-month follow-up, as compared to the standard treatment (drug and rehabilitation therapy) group (3-month EBV group: 70%, the standard treatment (drug and rehabilitation therapy) group: 27.8%, p=0.02; 6-month EBV group: 85%, the standard treatment (drug and rehabilitation therapy) group: 38.9%, p=0.006).

BODE index In three RCTs and one NRS that reported on BODE index, the EBV group showed a significant improvement as compared to the standard treatment (drug and rehabilitation therapy) group.

Cost-effectiveness Two studies reported cost-effectiveness related to BLVR using unilateral EBV. In both studies, BLVR using unilateral EBV was presented as being more cost-effective than standard treatment (drug and rehabilitation therapy). One study reported that a lower mortality rate and improved QoL can be expected with BLVR using unilateral EBV. The study also reported that for extending the life-year by one year, this technology cost approximately 79,100 and 34,883 euros more than standard treatment (drug and rehabilitation therapy) over a 5- and 10-year analysis period, respectively. Another study reported that for improving quality-adjusted life year by one year (1 QALY), BLVR using unilateral EBV cost approximately 46,322 and 25,142 Euros more than standard treatment (drug and rehabilitation therapy) over a 5- and 10-year analysis period, respectively.

Conclusions and recommendations Based on a review of the systematic review findings, the subcommittee confirmed that major complications, such as overall AEs, mortality, pneumothorax, exacerbation of COPD, and pneumonia, occurred more frequently in the BLVR using the unilateral EBV group and determined that regarding BLVR using unilateral EBV, viewing it as a safe technology is challenging. However, since significant improvement in lung function, workload, and QoL was found in the BLVR using unilateral EBV group, greater benefits can be expected from this technology. Moreover, the subcommittee stated that patients should carefully consider when choosing BLVR using unilateral EBV. In the first session of the Committee of Health Technology Reassessment in 2023 (January 13, 2023), the following conclusions were made with regard to BLVR using unilateral EBV based on the subcommittee findings in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project. Accordingly, the Committee of Health Technology Reassessment “conditionally recommends” the use of BLVR using unilateral EBV for patients with severe COPD and emphysema in Korea, according to the clinical situation or value. This is based on a comprehensive consideration of its clinical safety and effectiveness, and other assessment items.
