M2BPGi[정밀면역검사]

Abstract

평가배경 ‘M2BPGi (Mac-2 binding protein glycosylation isomer) [정밀면역검사]’는 만성 간 질환에서 간섬유화 선별진단을 위한 검사이다. 2017년 ‘M2BPGi 간 섬유화 검사[화학발광효소면역측정법]’으로 신의료기술평가 결과, 만성 간 질환에서 간섬유화를 진단하는데 간생검 이전 선별검사로 안전성과 효과성이 있는 기술로 평가되었다. 이후 2018년 선별급여 80%로 등재되었으며(보건복지부 고시 제2017-57호, 2017. 3. 28.), 2023년 상반기 적합성평가위원회 평가 예정으로 내부 모니터링을 통해 발굴되었다. 2022년 제6차 의료기술재평가위원회(2022. 6. 10.)에서 동 기술의 평가계획서 및 소위원회 구성안에 대해 심의를 받아 재평가를 수행하였다. 본 평가목적은 만성 간 질환에서 M2BPGi[정밀면역검사]의 임상적 안전성 및 효과성에 대한 과학적 근거평가를 통해 보건의료자원의 효율적 사용을 위한 정책적 의사결정을 지원하고자 하였다.

평가방법 만성 간 질환에서 M2BPGi[정밀면역검사]의 안전성 및 효과성을 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “M2BPGi[정밀면역검사] 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 소화기내과 3인, 진단검사의학과 2인, 영상의학과 1인, 근거기반의학 1인의 전문가 7인으로 구성하였다. 핵심질문은 “M2BPGi[정밀면역검사]는 만성 간 질환에서 간섬유화 선별진단에 임상적으로 안전하고 효과적인가?”이었으며, 안전성은 검사 관련 합병증과 위음성으로 인한 치료지연 및 위양성으로 인한 과잉 진단을 지표로 하였고, 효과성은 진단정확성과 의료결과에 미치는 영향(치료 방법의 변화, 간생검 감소율)을 지표로 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하였으며, 문헌 선정과정은 선택 및 배제기준에 따라 두 명의 평가자가 독립적으로 수행하고, 의견이 불일치한 경우 평가자간 합의를 통해 최종 선택문헌을 결정하였다. 비뚤림위험 평가는 QUADAS-2 (Quality Assessment of Diagnostic Accuracy Studies-2)를 사용하여 최종 선택문헌을 대상으로 두 명의 평가자가 독립적으로 평가하였고, 의견이 불일치한 경우 평가자간 합의를 통해 일치된 결과를 도출하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하고, 참고표준검사 기준에 따른 간섬유화 단계를 구별하여 결과지표를 제시하고, 양적 분석이 가능한 경우 메타분석을 하였다.

평가결과 총 45편의 진단법평가연구를 최종 선택하였다. 비뚤림위험 평가 결과 절반 이상의 문헌(58.7%)이 후향적 연구로 환자선택 비뚤림위험은 ‘높음’으로 평가하였고, 대부분 문헌(95.7%)에서 ROC (Receiver Operating Characteristic) 곡선에 따른 최적 임계값을 사용하여 결과를 제시하여 중재검사 비뚤림위험을 ‘높음’으로 평가하였다. 선택적 보고의 비뚤림위험 평가결과, ‘높음’이 33.3% 였다.

안전성 안전성은 검사 관련 합병증과 위음성으로 인한 치료 지연 및 위양성으로 인한 과잉 진단을 지표로 평가하였는데, 이를 보고한 문헌은 없었다. 이에 소위원회에서는 M2BPGi[정밀면역검사]가 혈액을 채취하여 체외에서 이루어지는 검사로 환자에게 직접적인 위해를 가하지 않고, 간섬유화 진단의 보조검사인 점을 고려했을 때, 동 검사로 인한 치료 지연이나 과잉 진단이 발생할 우려는 낮아 안전한 기술로 평가하였다.

효과성 효과성은 의료결과에 미치는 영향과 간섬유화 진행 단계를 평가하는 진단정확성을 평가하였다. 의료결과에 미치는 영향은 치료 방법의 변화와 간생검 감소율로 정의하였는데, 이를 보고한 문헌은 없었다. 간섬유화 단계 평가에 대한 진단정확성은 총 45편에서 보고하였으며, 섬유화 진행 단계에 따라 Fibrosis stage 2 이상(≥F2), Fibrosis stage 3 이상(≥F3), Fibrosis stage 4(F4)의 세 가지로 구분하여 평가하였다. F2 이상 상태를 진단한 문헌은 33편, F3 이상 상태를 진단한 문헌은 37편, F4 상태를 진단한 문헌은 28편이었다. 간섬유화 단계에 대한 임계값이 문헌별로 다양하였는데, F2 이상 진단의 임계값 범위는 0.25~2.2, F3 이상은 0.45~5.35, F4는 0.7~3.9였다. 제조사에서 제시한 임계값 1.0을 기준으로 결과를 제시한 문헌은 세 단계를 통틀어 4편으로 적었다. 섬유화 진행 단계별 메타분석 결과, M2BPGi[정밀면역검사]의 F2 이상 상태에 대한 진단정확성은 통합 민감도 0.71, 통합 특이도 0.75, 통합 AUC (area under the curve) 0.79였다. F3 이상 상태 진단정확성은 통합 민감도 0.74, 통합 특이도 0.75, 통합 AUC 0.81이었고, F4 상태 진단정확성은 통합 민감도 0.78, 통합 특이도 0.81, 통합 AUC 0.86이었다.

결론 및 제언 소위원회에서는 체계적 문헌고찰 결과를 바탕으로 M2BPGi[정밀면역검사]의 진단정확성이 기존 검사보다 우수하지 않았고, 대부분 연구들이 최적 임계값(다양한 임계값)을 활용한 결과로 동일한 임계값을 적용할 경우 진단정확성은 더 낮을 가능성이 있다고 평가하였다. 이에 따라 본 검사는 선별검사나 기존 검사의 대체검사로 임상에서 활용하기 어렵고, 기존 검사결과에서 명확하게 판단내리기 어렵거나 기존 검사가 불가능할 때 보조검사로 활용할 수 있다고 판단하였다. 아울러 대부분 문헌에서 다양한 임계값을 사용하였는데 이는 제조사가 제시한 임계값과도 차이가 있어 결과해석에 주의가 필요하며, 이부분이 한계점이라는 의견이 있었다. 2023년 제1차 의료기술재평가위원회(2023. 1. 13.)에서는 의료기술재평가사업 관리지침 제4조 제10항에 의거 “M2BPGi[정밀면역검사]”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황이나 가치에 따라 만성 간 질환의 간 섬유화 진단에서 M2BPGi[정밀면역검사]는 간 생검 이전 선별검사 목적으로 활용하기 어렵고 보조검사 목적 사용에 대해 ‘조건부 권고함’으로 심의하였다.

Background Mac-2-binding protein glycosylation isomer (M2BPGi) [Precision immunoassay] is a test for screening and diagnosing liver fibrosis among patients with chronic liver disease. When this test was evaluated as “M2BPGi liver fibrosis test [Chemiluminescent immunoassay]” in the new health technology assessment in 2017, it was considered to be a safe and effective technology suitable for use as a screening test prior to liver biopsy for diagnosing liver fibrosis. After being listed as a selective benefit item (80% co-pay) since 2018 (Ministry of Health and Welfare Notice No. 2017-57; March 28, 2017), it was identified through internal monitoring as being scheduled for assessment by the Conformity Assessment Committee during the first half of 2023. Accordingly, the reassessment was conducted following a review of the reassessment plan and the proposed subcommittee composition of this technology during the sixth session of the Committee of Health Technology Reassessment in 2022 (June 10, 2022). This assessment aimed to support policy decision-making for the efficient use of public health resources through the assessment of the scientific evidence for the clinical safety and effectiveness of M2BPGi [precision immunoassay] in patients with chronic liver disease.

Methods A systematic review was performed to assess the safety and effectiveness of M2BPGi [precision immunoassay] in patients with chronic liver disease. All assessment methods were finalized after discussions by the “M2BPGi [precision immunoassay] subcommittee” (hereinafter referred to as “subcommittee”) with consideration for the purpose of the assessment. The subcommittee consisted of seven members (three from gastroenterology, two from laboratory medicine, and one from radiology and evidence-based medicine, respectively). The key question was, “Is M2BPGi [precision immunoassay] clinically safe and effective for screening and diagnosing liver fibrosis in patients with chronic liver disease?” Safety was assessed based on test-related complications, treatment delay due to false negatives, and overdiagnosis from false positives. The effectiveness was assessed based on the diagnostic accuracy and impact on health outcomes (change in treatment modality and reduction in the rate of liver biopsies). For the literature review, three international and five domestic databases were searched based on the key question. Two reviewers independently screened and selected the articles in accordance with the inclusion and exclusion criteria. Additionally, two reviewers independently performed a risk of bias (RoB) assessment using Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). In case of any disagreement, consensus was reached between the reviewers to select the final articles and ensure consistent results. Data extraction was performed using a predetermined format, and the outcome indicators were presented by differentiating liver fibrosis according to the reference standard test criteria. Data were analyzed by meta-analysis if quantitative analysis was possible.

Results A total of 45 articles on diagnostic test evaluation studies were selected. The RoB assessment results showed that RoB in the patient selection domain was “high,” since more than half (58.7%) of the studies were retrospective studies. Moreover, RoB in the index test domain was assessed as “high,” since most of the articles (95.7%) represented the results using optimal threshold values according to the receiver operating characteristic (ROC) curve. With respect to the RoB of selective reporting, the assessment showed a “high” RoB of 33.3%.

Safety Safety was assessed based on test-related complications, treatment delay due to false negatives, and overdiagnosis due to false positives; however, no articles reported on these factors. Accordingly, the subcommittee determined that M2BPGi [precision immunoassay] is a safe technology, as the concern for treatment delay or overdiagnosis due to this test is low, considering that it is an in vitro test using collected blood that does not cause direct harm to the patient and is used as a supplementary test for diagnosing liver fibrosis.

Effectiveness Effectiveness was assessed based on the impact on health outcomes and diagnostic accuracy for assessing the stages of liver fibrosis progression. Impact on health outcomes was defined as change in treatment modality and rate of reduction in liver biopsies, but no articles reported on these factors. Diagnostic accuracy for assessing the stages of liver fibrosis progression was reported in 45 articles, and the assessment was based on three stages of fibrosis progression: Fibrosis stage ≥ 2 (≥ F2), stage ≥ 3 (≥ F3), and stage 4 (F4). There were 33 articles on diagnosis of ≥ F2; 73 articles on diagnosis of ≥ F3; and 28 articles on diagnosis of F4. The threshold values for fibrosis stages varied between studies, with the threshold values showing ranges of 0.25–2.2, 0.45–5.35, and 0.7–3.9 for ≥ F2, ≥ F3, and F4, respectively. The number of articles that presented the results based on the manufacturer’s suggested threshold value of 1.0 was very low, with only four for all three stages. Meta-analysis by fibrosis stages showed that the diagnostic accuracy of M2BPGi [precision immunoassay] had a pooled sensitivity, specificity, and area under the curve (AUC) of 0.71, 0.75, and 0.79 for ≥ F2, respectively; 0.74, 0.75, and 0.81 for ≥ F3, respectively; and 0.78, 0.81, and 0.86 for F4.

Conclusions and recommendations Based on the systematic review findings, the subcommittee concluded that M2BPGi [precision immunoassay] did not demonstrate superior diagnostic accuracy compared to conventional tests, particularly since most studies reported results using optimal threshold values (different threshold values). The aforementioned results suggested that uniform threshold values could potentially yield lower diagnostic accuracy. Accordingly, the subcommittee determined that using this test in clinical practice as a screening test or an alternative to conventional tests would be challenging. However, it may be used as a supplementary test when conventional tests either yield inconclusive results or cannot be employed. The subcommittee also advised exercising caution when interpreting the results, since varying threshold values used in most studies differed from the manufacturer's recommended threshold, which was a limitation of the current study. In accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment Project, the following conclusions were made during the first session of the Committee of Health Technology Reassessment in 2023 (January 13, 2023) regarding M2BPGi [precision immunoassay]. The Committee of Health Technology Reassessment concluded that M2BPGi [precision immunoassay] may not be used as a screening test prior to liver biopsy but “conditionally recommends” its use as a supplementary test for diagnosing liver fibrosis in patients with chronic liver disease according to the clinical situation or value in Korea based on comprehensive consideration of the evidence of its clinical safety and effectiveness and other assessment items.
