빛산란 시광학 특성 분석검사[편측]

Abstract

평가배경 빛산란 시광학 특성 분석검사는 백내장 수술이 필요한 환자를 대상으로 백내장 치료 방침 결정에 보조하기 위한 검사로 이중경로광학분석(double-pass technique) 원리에 기반하여 안구 내 빛산란(Objective Scatter Index, OSI) 정도를 정량적으로 측정하는 객관적인 시력의 질을 제공하는 검사법이다. 일반적으로 OSI값이 1미만은 정상 눈으로 보고, OSI값이 1~3 범위는 초기 백내장이 있는 눈, 3~7 범위는 수술이 필요한 백내장에 해당되고, OSI값이 7보다 높으면 안구 내 산란이 높은 경우로 심각한 백내장과 관련있는 것으로 분류한다. 해당 검사는 2014년 제1차 신의료기술평가위원회에서 연구가 더 필요한 단계의 기술로 한 차례 심의한 바 있었다. 이후 2017년 제3차 신의료기술평가위원회에서 백내장 수술이 필요한 환자를 대상으로 수술 전 기존의 임상검사 후 보조적으로 시행시 안구 내 빛산란 정도를 정량화하여 환자의 주관적 불편감을 객관적으로 설명하는데 유용하고, 수술(치료) 여부 등 치료 방침을 결정하는데 도움을 주기 위한 보조적인 검사로 안전성 및 효과성이 있는 의료기술로 평가되었다. 빛산란 시광학 특성 분석검사는 2018년부터 선별급여 80%(본인 부담률 80%)로 사용되었다(보건복지부 고시 제2018-103호, 2018.5.31.). 본 검사는 의료기술재평가사업의 내부모니터링으로 발굴되었으며, 전문적·심층적 검토를 통해 임상적 안전성 및 효과성 등을 확인하고 의료기술의 적정 사용을 위한 정책적 의사결정에 지원하고자 2022년 제6차 의료기술재평가위원회(2022.6.10.)에서 의료기술재평가 안건으로 선정되어 재평가를 수행하였다.

평가방법 빛산란 시광학 특성 분석검사에 대한 안전성 및 효과성을 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “빛산란 시광학 특성 분석검사 소위원회(소위원회)”의 논의를 거쳐 확정하였고 소위원회는 총 5인(안과 3인, 근거기반의학 2인)으로 구성하였다. 평가의 핵심질문은 ‘백내장 환자에서 빛산란 시광학 특성 분석검사는 임상적으로 안전하고 효과적인가?’이며, 안전성은 검사관련 부작용, 효과성은 질병과의 관련성(OSI 수치), 진단정확도, 비교검사와의 상관성을 평가하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2)를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 추출 및 검토하였으며, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료분석은 정량적 분석(quantitative analysis) 및 정성적(qualitative review) 분석을 하였다.

평가결과 빛산란 시광학 특성 분석검사의 안전성과 효과성은 진단법 평가연구 총 19개(국내 3개, 국외 16개)를 대상으로 평가하였다. 연구 수행국가는 동 검사의 제조사 국가인 스페인 연구가 6편으로 가장 많았으며, 출판문헌은 2011~2022년 사이의 문헌이 포함되었다. 연구대상자는 백내장 진단을 받았거나 백내장 수술 예정인 환자 또는 수술 결정이 필요한 환자를 대상으로 하였고, 건강대조군과 비교한 연구도 포함하였다. 비교검사는 세극등현미경검사로 Lens Opacities Classification System Ⅲ (LOCS Ⅲ) 분류체계를 통해 백내장을 진단한 검사가 대부분이었고, 이외에 단일경로광학분석검사, 대비감도검사, 시력검사, 시각 증상, 샤임프러그 검사, 시기능 주관적 지표, 렌즈 혼탁도 검사 등이 있었다. QUADAS-2를 이용하여 비뚤림위험 평가를 수행한 결과, ‘환자선택(Patient selection)’ 항목에서는 주로 환자와 건강대조군을 비교한 연구(7편)들은 비뚤림위험 및 적용성에 대한 우려가 ‘높음’으로 평가하었다. ‘중재검사(Index test)’ 항목은 참고표준검사 전에 수행되었다는 기술이 대부분 보고되지 않아 ‘불확실’로 평가한 경향이 있었다. 또한, 진단정확도를 보고한 문헌(6편)에서 임계치를 사전에 명시하지 않은 경우 비뚤림위험 ‘높음’으로 평가하였고, 정확도를 보고하지 않은 나머지 문헌에 대해서는 해당 항목을 평가하지 않았으며 적용성에 대한 우려는 ‘불확실’로 평가하였다. ‘참고표준검사(Reference standard)’ 항목은 백내장 분류체계가 잘 확립되어 임상적 진단에서 사용되고 있기 때문에 비뚤림위험 및 적용가능성에 대한 우려는 ‘낮음’으로 평가하였다. 눈가림 측면에서 참고표준검사와 중재검사 간의 사전 정보없이 결과 해석이 되었는지 문헌에서 보고되지 않은 경우는 ‘불확실’로 비뚤림위험을 평가하였다.

안전성 빛산란 시광학 특성 분석검사의 안전성 결과를 보고한 문헌은 없었다.

효과성 빛산란 시광학 특성 분석검사의 효과성은 질병과의 관련성(OSI 수치), 진단정확도, 비교검사와의 상관성에 대해 확인하였다. 빛산란 시광학 특성 분석검사의 측정값인 OSI 수치는 총 18편의 문헌에서 보고하였다. 건강대조군은 평균 OSI 수치가 1미만 이었고, 백내장 환자에서의 OSI 수치는 15편을 메타분석한 결과, 가중 평균 4.08 (95% Confidence Interval(CI) 3.43, 4.86)이었고, 각 문헌들의 평균 OSI 보고값 범위는 1.09~9.0이었다. 참고표준검사인 세극등현미경검사를 통한 백내장 분류체계인 LOCS Ⅲ 점수체계에 따라 4편을 메타분석한 결과, LOCS 1점으로 분류한 군의 평균 OSI 수치는 1.98 (95% CI 1.5, 2.46), 2점인 군 3.41 (95% CI 2.92, 3.98), 3점인 군 6.16 (95% CI 5.43, 6.89), 4점인 군 9.49 (95% CI 1.84, 17.14)로 질병 중증도에 따른 관련성을 보였다. 백내장 환자에서 빛산란 시광학 특성 분석검사의 진단정확도는 6편의 문헌에서 보고하였으며, 백내장 진단을 위한 동 검사의 임계값은 1.18~2.35로 민감도 83.9%~100%, 특이도 76.7~100%의 분포로 나타났다. 백내장 수술군과 비수술군을 구분하기 위한 임계값은 2.1~3.2로 민감도 80%, 특이도 80~84%의 분포를 보였다. 빛산란 시광학 특성 분석검사와의 상관관계를 보고한 문헌은 총 14편이었다. 세극등현미경검사(LOCS Ⅲ)와의 상관성을 보고한 8편을 메타분석한 결과, 평균 상관계수는 0.47 (95% CI 0.32, 0.62)로 상관성이 높지 않았다. 단일경로광학분석검사와의 상관성은 2편의 문헌에서 보고하였으며 상관계수 범위는 0.309~0.629이었다. 대비감도검사와의 상관성은 총 4편 중 1편의 문헌에서는 통계적 유의성이 없다고 보고하였고, 3편의 상관계수 범위는 0.219~0.682였다. 시력검사와의 상관성은 총 13편의 문헌에서 보고하였으며 메타추정치는 0.59 (95% CI 0.50, 0.68)이었고, 주관적 시기능 검사(VF-14)와의 상관성을 보고한 3편의 문헌의 상관계수 범위는 0.1~0.712, 샤임프러그 검사와 렌즈 혼탁도 검사와의 상관성은 각각 1편씩 0.764 (p<0.01), 0.55로 보고하였다.

결론 및 제언 소위원회에서는 빛산란 시광학 특성 분석검사가 체외진단법으로 환자에게 직접적으로 위해를 가하지 않아 검사에 따른 안전성 문제는 없다고 평가하였다. 효과성 지표인 질병과의 관련성(OSI 수치), 진단정확도, 비교검사와의 상관성을 검토한 결과, 빛산란 시광학 특성 분석검사의 OSI 수치는 백내장 환자의 질병 중증도와 통계적으로 유의한 연관성이 있었다. 동 검사의 진단정확도는 민감도 83.9%~100%, 특이도 76.7~100%로 보고하였으나, 연구의 비뚤림위험이 있고 연구마다 임계값이 다르게 제시되어 해석에 주의가 필요하다고 보았다. 또한, 백내장 진단 및 백내장 수술을 결정할 때의 임계값 범위를 구분하기 어려운 제한점이 있었다. 빛산란 시광학 특성 분석검사와 세극등현미경검사(LOCS Ⅲ)와의 평균 상관계수는 0.47 (95% CI 0.32-0.62)로 연구 간 상관성의 범위가 넓었으며, 다른 검사와 일대일로 비교한 상관관계에서도 다수의 문헌에서 연관성이 있다고 보고하였으나 평균 상관성이 0.4~0.6으로 상관성이 높지 않고 상관계수의 범위가 넓었다. 이에 소위원회에서는 현재 빛산란 시광학 특성 분석검사는 백내장 진단보조 검사로 사용되고 있으며 안구 내 빛산란 정도를 정량화하여 환자의 주관적 불편감을 정량적으로 나타낼 수 있고 백내장 질병의 중증도를 측정할 수 있다는 측면에서 검사의 이점이 있다고 보았다. 그러나 단독검사로써 진단정확도가 높지 않고, 참고표준검사 및 기존 다른 검사와의 상관성도 높지 않아 백내장 수술 여부를 결정하는 역할로 사용할 수 있는지에 대해 문헌적 근거는 부족하다고 평가하여 이에 대한 추가 연구가 필요하다고 제언하였다. 2023년 제1차 의료기술재평가위원회(2022.1.13.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “빛산란 시광학 특성 분석검사”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 빛산란 시광학 특성 분석검사가 환자의 주관적 불편감을 정량적으로 측정하는 이점이 있으나, 백내장 진단 및 수술을 결정하는데 동 검사만으로 판단할 수 없고 백내장 수술 여부를 결정하는데 충분한 근거를 제공하지 못한다고 판단하였다. 이에 임상적 안전성과 효과성 등에 대해 판단할 임상연구가 부족하여 국내 임상상황에서 백내장 수술이 필요한 환자를 대상으로 백내장 진단 및 백내장 치료 방침 결정을 위한 보조 검사로 빛산란 시광학 특성 분석검사의 권고등급을 결정할 수 없어 ‘불충분’으로 심의하였다.

Background The Scatter Optical Quality Analysis System (OQAS) is an optical method to estimate the contribution of forward scattering that affects vision to characterize the severity of cataracts. It is a test method that provides objective quality of vision by quantitatively measuring objective scatter index (OSI) based on the principle of the double-pass technique. Generally, an OSI value <1 is considered normal; 1–3 is considered early cataract; 3–7 is considered mature cataract; and >7 is considered severe cataract with high intraocular light scattering. During the 3rd New Health Technology Assessment Committee in 2017, OQAS was considered a health technology with proven safety and effectiveness in patients who are subjectively uncomfortable due to cataracts. Currently, it is classified as a selective benefit item (80% out-of-pocket pay) in accordance with Ministry of Health and Welfare (Notice No. 2018-103, May 31, 2018). In order to promote efficient utilization of healthcare resources and induce optimal use of health technology, this technology was proposed as an item that required assessment of the latest evidence. This health technology was selected as a health technology reassessment agenda during the 5th Health Technology Reassessment Committee in 2022 (June 10, 2022) and performed accordingly.

Methods A systematic review was performed to assess the safety and effectiveness of OQAS. All assessment methods were finalized after discussions by the “Scatter Optical Quality Analysis Test subcommittee” (“Subcommittee”) with consideration for the purpose of the assessment. The Subcommittee consisted of five experts (three from ophthalmology and two from evidence-based medicine). The key question of the assessment was, “Is OQAS clinically safe and effective for patients with cataracts?” Safety included test-related adverse events (AEs), while effectiveness indicators included association with the disease (OSI values), diagnostic accuracy, and correlation with the other tests. For the systematic review, three international and five domestic databases were searched based on the key question. Studies were selected independently by two reviewers according to the inclusion and exclusion criteria. The risk of bias in the included studies was assessed two reviewers independently using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). All data were extracted by study unit. Any disagreements were resolved through a discussion with a third person to reach a consensus. Data were qualitatively analyzed if quantitative analysis was not possible.

Results The safety and effectiveness of OQAS were assessed based on 19 studies. The highest number of studies were from Spain (n=6), which is the country where the manufacturer of the test is from, and the selected articles were published from 2011–2022. As a result of the QUADAS-2, “patient selection” domain had a “high” risk of bias, mainly in case-control studies (7 studies). The “index test” domain was evaluated as having a risk of bias of “uncertain” or “high” when the timing of the reference standard test was not mentioned or the threshold value of test was not specified in the diagnostic accuracy study. For the “reference standard” domain, concerns for applicability were assessed to be “low” since the classification of cataract is well established and is being used in clinical diagnosis. Regarding blinding, prior information between the reference standard and index test was not reported so it was evaluated as “uncertain”.

Effectiveness The effectiveness was assessed based on association with the disease (OSI values), diagnostic accuracy, and correlation with the comparator test. OSI values, the measure of OQAS, were reported 18 articles. The mean OSI value of health control groups was <1, and a meta-analysis of 15 articles showed that the weighted mean OSI value was 4.08 (95% confidence interval (CI) 3.43, 4.86) for patients with cataracts. The mean OSI value reported in each article ranged from 1.09–9.0. Meta-analysis of four articles according to Lens Opacities Classification System Ⅲ (LOCS III) scores based on slit lamp microscopy, the reference standard, showed mean OSI values of 1.98 (95% CI 1.5, 2.46), 3.41 (95% CI 2.92, 3.98), 6.16 (95% CI 5.43, 6.89), and 9.49 (95% CI 1.84, 17.14) in LOCS 1, 2, 3, and 4 groups, respectively, indicating an association according to disease severity. Diagnostic accuracy of OQAS in patients with cataract was reported in six articles, which showed sensitivity of 83.9%–100% and specificity of 76.7%–100% at a threshold value of 1.18–2.35 for diagnosing cataract. The threshold value for differentiating between cataract surgery and non-surgery groups was 2.1–3.2, with a sensitivity of 80% and specificity of 80–84%. The correlation with OQAS was reported in 14 articles. Meta-analysis of eight articles that reported on correlation with slit lamp microscopy (LOCS Ⅲ) showed that correlation was not very high with a mean correlation coefficient of 0.47 (95% CI 0.32, 0.62). Correlation with the single-pass technique was reported in two articles, in which correlation coefficients ranged from 0.309–0.629. Among the four articles that reported on correlation with contrast sensitivity tests, one article reported no statistically significant correlation, whereas three reported correlation coefficients ranging from 0.219–0.682. A total of 13 articles reported on the correlation with the visual acuity test, in which the meta-estimation was 0.59 (95% CI 0.50, 0.68). In three articles that reported on correlation with subjective measures of visual function, correlation coefficients ranged from 0.1–0.712. In one article each on correlation with Scheimpflug examination and lens opacity test, the correlation coefficient was reported to be 0.55-0.764.

Conclusions and recommendations The Subcommittee concluded that OQAS is an in vitro diagnostic method, and there were no test-related safety problems. OSI values of OQAS were statistically significantly associated with the disease severity of patients with cataract. The diagnostic accuracy of this test was reported to have a sensitivity of 83.9–100% and specificity of 76.7–100%, but caution should be taken when interpreting the results since each study used a different threshold value. Moreover, a limitation existed in differentiating the range of threshold values for diagnosing cataracts and deciding on cataract surgery. While many articles reported correlations in one-on-one comparisons with other tests, correlations were not strong, with the mean correlation coefficients varying widely. The Subcommittee considered that OQAS has some benefits of being able to quantitatively demonstrate the subjective discomfort the patient. Conversely, the diagnostic accuracy was not very high when used as a stand-alone test, and correlations with reference standards and other conventional tests were found to be low. Accordingly, the subcommittee determined that the existing literature lacks sufficient evidence to determine whether OQAS effectively assess the need for cataract surgery and recommended that additional studies on these factors are needed. The 1st Health Technology Reassessment Committee in 2023 (January 13, 2023) reviewed findings based on the results of Subcommittee review.

The Committee determined to “insufficient” the use of scatter optical quality analysis for diagnosis and decision on surgery in patients with cataract who experienced subjective discomfort, based on the lack of clinical studies supporting its clinical safety and effectiveness (recommendation level: insufficient).
