변실금에서의 천수신경조절술

Abstract

평가배경

천수신경조절술(sacral nerve modulation, 이하 SNM)은 배뇨반사의 척수 단계 중추인 천수에서 천수신경근이 나오는 부위에 지속적인 신경자극이 가능한 신경조절장치를 이식하고 이를 미세 조절하여 비정상적인 배뇨반사를 줄이는 치료방법이다. 변실금은 변 배설이 조절되지 않아 변을 적절한 시간이나 장소에서 배출하지 못하고 변이 새는 증상이 나타나는 질환이다. 천수신경조절술은 신의료기술평가 제도 시행 전 도입된 등재급여 기술로서(보건복지부고시 제2006-56호, 2006. 7. 25.), 2015년 해당 기술의 급여기준이 변실금으로 확대되었다.

동 기술은 국내ㆍ외 임상진료지침 검토와 내부 모니터링을 통해 재평가 안건으로 발굴되었다. 다양한 대체 의료기술이 존재하는 현 시점에서 비교기술 대비 동 기술의 효과성에 대한 근거를 확인하고자, 2023년 제9차 의료기술재평가위원회(2023. 9. 8.)에서 재평가 계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다.

평가목적

본 평가의 목적은 변실금 환자에서 천수신경조절술을 증상개선 목적으로 사용하는 것은 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술 사용에 대한 권고수준을 제시하기 위함이다.

평가방법

변실금 환자에서 천수신경조절술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 ‘변실금에서의 천수신경조절술 소위원회(이하 ‘소위원회’라 한다)’의 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 결정된 비뇨의학과 2인, 산부인과(비뇨부인과) 2인, 대장항문외과 2인, 근거기반의학과 1인, 총 7인으로 구성하였다.

본 평가의 대상자는 변실금 환자, 비교시술은 괄약근성형술로 정의하였다. 결과변수로는 안전성과 효과성을 확인하였으며, 안전성은 시술 및 기기 관련 이상반응, 그 외 이상반응을 결과지표로, 효과성은 증상개선, 항문직장기능, 삶의 질, 환자 만족도를 결과지표로 평가하였다. 연구유형은 비교연구 이상의 연구만 평가에 포함하기로 하였다.

체계적 문헌고찰은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 3개(KoreaMed, 한국의학논문데이터베이스(KMbase), 한국교육학술정보원(RISS)) 데이터베이스에서 검색하였다(최종검색일 2024. 2. 26.).

최종 선택된 연구의 비뚤림위험 평가는 Risk of bias assessment for nonrandomized studies (RoBANS Ver.2)를 사용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE (Grading of Recommendations Assessment, Development and Evaluation) 방법을 이용하여 근거수준을 평가하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하여 표로 제시하고, 양적 분석이 가능한 경우 메타분석을 하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다.

본 평가는 소위원회의 검토 결과를 바탕으로 의료기술재평가위원회에서 최종심의 후 권고등급을 결정하였다.

평가결과

체계적 문헌고찰 결과 최종 선택된 연구는 비무작위(NRS) 3편(98명, 치료군 38명, 대조군 60명)으로 모두 괄약근성형술과 비교하였다. 출판연도는 모두 2010년대였으며, 출판국가는 미국 2편, 이탈리아 1편이었다. 전반적인 비뚤림위험의 수준은 중등도인 것으로 판단하였다.

안전성

변실금에서의 천수신경조절술의 안전성은 시술 및 기기 관련 이상반응, 그 외 합병증 및 부작용으로 평가하였다.

선택연구 3편 중 2편에서 안전성 결과를 보고하였는데, 사망, 신경 손상과 같은 심각한 부작용 발생을 보고한 연구는 없었다. 1편(Rodrigues 등, 2017)은 천수신경조절술군과 괄약근성형술군에서 동일한 상처감염 발생률을 보고한 반면, 나머지 1편의 연구(Hong 등, 2014)에서는 괄약근성형술에 비해 천수신경조절술군에서 감염으로 인한 합병증과 재수술률의 발생률이 유의하게 더 높았다.

소위원회는 시술 및 기기 관련 이상반응은 경미한 수준이므로 변실금에서의 천수신경조절술은 안전한 기술이라고 판단하였다.

효과성

변실금에서의 천수신경조절술의 효과성은 증상개선, 항문직장기능, 삶의 질, 환자 만족도로 평가하였다.

증상개선은 선택연구 3편 모두에서 보고하였으며, 가장 널리 사용되는 도구인 Cleveland Clinic Florida-Wexner Fecal Incontinence Score (CCFFIS)를 포함한 대부분의 지표에서 중재군과 비교군 간 유의한 차이가 없었다. 항문직장기능은 1편에서 보고하였으며, 항문내압과 직장감각용적 등의 모든 지표에서 중재군과 비교군 간 유의한 차이가 없었다. 삶의 질은 1편에서 보고하였으며, Fecal Incontinence Quality of Life (FIQL) scale의 모든 세부영역에서 중재군과 비교군 간 유의한 차이가 없었다. 환자 만족도를 보고한 연구는 없었다.

소위원회는 거의 모든 효과성 지표에서 천수신경조절술군과 괄약근성형술군 간 유의한 차이가 확인되지 않아 변실금에서의 천수신경조절술은 괄약근성형술과 유사한 효과성을 가진 기술이라고 판단하였다.

결론 및 권고결정

의료기술재평가 소위원회에서는 변실금에서 천수신경조절술의 안전성과 효과성 결과를 현재 확인된 근거를 바탕으로 다음과 같이 제시하였다.

체계적 문헌고찰 결과, 변실금 환자에서 천수신경조절술은 사망, 신경 손상과 같은 심각한 부작용이 발생하지 않았고, 시술 및 기기 관련 이상반응들이 일부 보고되었으나 경미한 수준으로 판단하여 동 기술을 안전한 기술로 평가하였다.

효과성의 경우, 증상개선, 항문직장기능, 삶의 질 등 대부분의 지표에서 천수신경조절술과 괄약근성형술 간 유의한 차이가 확인되지 않아 동 기술은 괄약근성형술과 유사한 효과성을 가진 기술로 평가하였다. 또한 천수신경조절술은 구조적인 괄약근 손상이 없는 환자에게도 적용할 수 있는 시술이므로, 전문의의 판단하에 보존적 요법으로 개선되지 않는 변실금 환자에서 사용할 수 있는 치료 대안이 될 수 있다고 제언하였다.

2024년 제7차 의료기술재평가위원회(2024. 7. 12.)는 ‘변실금에서의 천수신경조절술’에 대해 다음과 같이 심의하였다.

의료기술재평가위원회는 임상적 안전성과 효과성의 근거 등을 종합적으로 고려하였을 때, 국내 임상상황에서 천수신경조절술을 변실금 환자의 증상개선 목적으로 사용하는 것을 ‘약하게 권고함’으로 결정하였다(※ 동 시술은 보존적 요법으로도 효과가 없는 난치성 변실금 환자에 한하여 제한적 사용을 권고함).

Background

Sacral neuromodulation (SNM) is a therapeutic intervention that entails the implantation of a neuromodulation device, which provides continuous neurostimulation and precise modulation of the sacral nerve roots from the sacrum, the spinal cord's center of voiding reflex control, with the objective of reducing abnormal voiding reflexes. Fecal incontinence is defined as a condition in which uncontrolled fecal excretion results in the leakage of feces at an inappropriate time or place. SNM is a listed benefit technology introduced before the implementation of a new health technology assessment system (Ministry of Health and Welfare Notice No. 2006-56, July 25, 2006), and in 2015 the scope of benefit for this technology was expanded to fecal incontinence.

The technology has been identified for reassessment through a review of national and international clinical guidelines and internal monitoring. In order to confirm evidence of the effectiveness of the technology compared to the comparative technology at the present time when various alternative health technologies exist, the Ninth Health Technology Reassessment Committee (September 8, 2023) deliberated on the reassessment plan and subcommittee organization to conduct the reassessment.

Purpose

This assessment aimed to provide evidence on whether the use of SNM for symptomatic improvement in patients with fecal incontinence is clinically safe and effective and to present the level of recommendation for its use.

Methods

The safety and effectiveness of SNM for fecal incontinence were assessed through a systematic review. All assessment methods were finalized through discussions within the “Subcommittee for SNM for Fecal Incontinence” (hereinafter the“Subcommittee”). The subcommittee was composed of seven members: two from urology, two from gynecology (urogynecology), two from colorectal surgery, and one from evidence-based medicine, as determined by the Health Technology Reassessment Committee.

The subjects of this assessment were defined as patients with fecal incontinence and the comparative procedure as sphincteroplasty. Safety and effectiveness were identified as outcome variables, with safety being measured by procedure- and device-related adverse events (AEs) and other AEs, and effectiveness being measured by symptom improvement, anorectal function, quality of life, and patient satisfaction. In regard to the type of study, only comparative studies and above were included in the assessment.

The systematic review was conducted by using key questions to search three international (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews-Cochrane Central Register of Controlled Trials) and three domestic (KoreaMed, Korean Medical Article Database (KMbase), and Research Information Service of Korea (RISS)) databases (last retrieved on February 26, 2024).

The risk of bias assessment of the final selected studies was conducted using the Risk of Bias Assessment for Nonrandomized Studies (RoBANS v. 2) tool. The results of the systematic review conducted for this assessment were assessed for quality of evidence using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) method. Data were extracted using a pre-established format for data extraction. When quantitative analysis could be performed, meta-analyses were performed. All processes were conducted independently by two assessors, and in case of disagreement, a consensus was reached between the assessors.

The assessment was subsequently reviewed by a subcommittee, after which a recommended rating was determined by the Health Technology Reassessment Committee following a final review.

Results

The systematic review selected three non-randomized (NRS) studies (98 patients, 38 in the treatment group and 60 in the control group), all conducted in comparison to sphincteroplasty.
