신장암에 실시하는 극초단파열치료술 - 경피적

Abstract

평가배경 신장암 환자에서 수행하는 극초단파열치료술(Microwave ablation, MWA)은 신장암 환자에서 신장 종양 내에 극초단파를 발생시켜 종양을 선택적으로 괴사시킴으로써 치료하는 기술이다. 접근 방법에 따라 경피적, 개복술하, 복강경하의 세 가지 항목으로 세분되어 2019년 급여로 등재되었다. 다양한 대체 의료기술이 활용되는 현시점에서, 비교기술 대비 동 기술의 안전성 및 유효성을 체계적으로 검토할 필요성이 제기되었으며, 내부 모니터링을 통해 재평가대상 안건으로 발굴되었다. 이후 우선순위 심의 절차를 거쳐 재평가 대상 기술로 선정되었고, 이에 따라 2025년 제2차 의료기술재평가위원회(2025. 2. 14.)에서 재평가 계획서와 소위원회 구성(안)이 심의되었다. 본 평가는 이러한 절차적 근거에 기반하여 경피적 MWA를 대상으로 수행되었다.

평가목적 본 평가의 목적은 신장암 환자에서 수행하는 경피적 MWA의 임상적 안전성 및 효과성 등에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다.

평가방법 신장암 환자에서 경피적 MWA에 대한 안전성, 효과성 및 경제성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “신장암의 극초단파열치료술 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 결정된 비뇨의학과 2인, 신장내과 1인, 영상의학과 1인, 혈액종양내과 1인, 근거기반의학 1인, 총 6인으로 구성하였다. 평가의 핵심질문은 ‘신장암 환자에게 경피적 MWA은 신장 종양 치료 목적으로 적용 시 임상적으로 안전하고 효과적인가?’이다. 본 평가의 대상자는 신장암 환자, 비교치료는 부분 신절제술, 고주파열치료술(Radiofrequency, RFA), 냉동제거술(Cryoablation, CA), 방사선 치료로 정의하였다. 결과변수로는 안전성과 효과성, 경제성을 확인하였으며, 안전성은 시술 관련 합병증을 결과지표로, 효과성은 종양학적 결과(생존, 재발, 전이), 시술 성공, 신기능 관련 결과(혈액학적 지표, 만성 신장질환으로의 악화), 수술 관련 지표(시술시간, 재원기간, 혈액소실량)를 결과지표로 평가하였다. 연구유형은 비교연구 이상인 경우를 평가에 포함키로 하였다. 체계적 문헌고찰은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 2개(KoreaMed, 한국교육학술정보원(RISS) 데이터베이스에서 검색하였다(최종검색일 2025. 4. 4.). 최종 선택된 연구의 비뚤림위험 평가는 Risk of bias Assessment Tool for Nonrandomized Studies (RoBANS) 2.0 국문판을 사용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거 수준을 평가하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하였고, 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다.

평가결과 체계적 문헌고찰 결과 총 27편(문헌 28편)의 연구가 평가에 최종 선택되었다. 27편 중 25편(총 대상자수: 중재군 2,390명, 비교군 5,751명)은 안전성과 효과성을 보고한 후향적 코호트연구였고, 나머지 2편은 비용-효과성을 분석한 경제성 연구였다. 대상자는 주로 작은 신장 종양 환자로, T1a 병기 이거나 종양 직경이 4cm 미만이었다. 비교치료는 부분 신절제술이 12편으로 가장 많았고, RFA와 비교한 연구가 7편, CA와 비교한 연구가 6편, MWA, RFA, CA의 세 가지 비수술적 종양제거술을 비교한 연구가 2편이었다. 비뚤림위험 평가 결과, 다수의 연구에서 군 간 기본 특성의 불균형, 대상자 수의 차이, 또는 추적관찰 기간의 불일치가 확인되어, 대상군 비교가능성 항목에서 비뚤림위험이 높게 평가되었다.

안전성 경피적 MWA의 안전성은 총 24편(총대상자수: 중재군 2,396명, 비교군 5,367명)에서 시술 관련 합병증으로 평가하였다. 부분 신절제술과 시술 관련 합병증 결과를 비교한 연구 8편 중 7편에서 중대한 합병증의 발생을 보고했는데, 모두 MWA에서 부분 신절제술보다 중대한 합병증의 발생이 적었으나 이 중 1편에서만 군 간 차이가 통계적으로 유의하였다. 또한 RFA와 시술 관련 합병증 발생을 비교한 연구 6편 중 4편, CA와 비교한 연구 중 5편 중 4편, RFA 및 CA와 비교한 연구 중 2편에서 중대한 합병증의 발생을 보고하였고, 모두 MWA와 비교치료 간 통계적으로 유의한 차이가 없었다.

효과성 경피적 MWA의 효과성은 생존, 재발, 전이를 포함하는 종양학적 결과와 시술 성공률을 1차 평가지표로, 신기능 관련 결과와 수술 관련 지표를 2차 평가지표로 분석하였다. 종양학적 결과 중 생존 결과로 전체 생존율, 암특이 생존율, 무질병 생존율, 무재발 생존율, 무전이 생존율, 무진행 생존율이 보고되었다. 전체 생존율의 경우 부분 신절제술과 비교한 1편(중재군 185명, 비교군 1,770명)에서 MWA군의 5년 전체 생존율이 수술군보다 통계적으로 유의하게 낮았으며, 나머지 부분 신절제술과 비교한 3편(중재군 174명, 비교군 278명) 및 RFA(1편, 중재군 13명, 비교군 23명), CA(2편, 중재군 107명, 비교군 229명)와 비교한 연구에서는 군 간 유의한 차이가 없거나 통계적 유의성이 보고되지 않았다. 암특이 생존율은 부분 신절제술(2편, 중재군 247명, 비교군 1,879명), RFA(1편, 중재군 27명, 비교군 244명) 및 CA(2편, 중재군 115명, 비교군 217명)와 비교한 모든 연구에서 군 간 유의한 차이가 없었다. 무질병 생존율의 경우, 부분 신절제술과 비교한 2편(중재군 247명, 비교군 1,879명)에서 각각 2년 및 5년 시점에서 MWA군의 생존율이 비교치료군보다 통계적으로 유의하게 낮았으며, RFA(1편, 중재군 27명, 비교군 244명), CA(2편, 중재군 115명, 비교군 217명)와 비교한 연구에서는 유의한 차이가 없었다. 무재발 생존율의 경우, 부분 신절제술과 비교한 1편(중재군 41명, 비교군 82명)에서 2년 시점의 MWA군 생존율이 유의하게 낮았으나, 나머지 부분 신절제술 2편(중재군 197명, 비교군 167명), RFA(1편, 중재군 373명, 비교군 158명), CA(2편, 중재군 104명, 비교군 237명)와 비교한 연구에서는 군 간 유의한 차이가 없었다. 무전이 생존율은 부분 신절제술과 비교한 2편(중재군 112명, 비교군 169명) 모두에서 군 간 유의한 차이가 없었다. 무진행 생존율의 경우에도, 부분 신절제술(1편, 중재군 12명, 비교군 80명), RFA(1편, 중재군 27명, 비교군 244명), CA(1편, 중재군 27명, 비교군 26명)와 비교 시 모두 군 간 유의한 차이가 없었다. 종양학적 결과 중 재발을 보고한 16편의 연구 중, 부분 신절제술과 비교한 2편(중재군 103명, 비교군 191명)에서는 MWA군의 재발률이 수술군보다 통계적으로 유의하게 높았다. 반면, 부분신절제술과 비교한 나머지 5편(중재군 322명, 비교군 1,955명), RFA와 비교한 4편(중재군 492명, 비교군 277명), CA와 비교한 5편(중재군 414명, 비교군 736명)에서는 군 간 유의한 차이가 없거나 통계적 유의성이 보고되지 않았다. 또한 전이 발생을 보고한 연구에서는 부분 신절제술(3편, 중재군 241명, 비교군 1,870명) 및 RFA(1편, 중재군 101명, 비교군 87명)와 비교 시 모두 군 간 유의한 차이가 없었다. 시술 성공을 기술적 성공률(시술이 목표 병변에 정확히 도달하여 계획한 범위 전체를 완전히 소작했는지 의미) 또는 유효율(시술 후 일정기간-주로 1개월-내 영상에서 종양이 완전 제거된 것을 확인한 비율)로 평가한 결과, RFA와 비교한 7편 중 1편(중재군 373명, 비교군 158명)에서 MWA의 완전소작률이 RFA보다 통계적으로 유의하게 높았다. 반면, 나머지 RFA와 비교한 6편 (중재군 376명, 비교군 627명) 및 CA와 비교한 5편(중재군 377명, 비교군 571명)에서는 군 간 유의한 차이가 없거나, 통계적 유의성이 보고되지 않았다. 신기능 관련 결과 중 혈액학적 지표의 경우, 부분 신절제술과 비교한 5편 중 1편(중재군 185명, 비교군 1,770명)에서 MWA가 비교치료보다 추정 사구체여과율(estimated glomerular filtration rate, eGRF) 감소가 통계적으로 유의하게 적었다. RFA와 비교한 2편(중재군 198명, 비교군 337명), CA와 비교한 4편(중재군 302명, 비교군 346명)에서는 군 간 유의한 차이가 없거나 통계적 유의성이 보고되지 않았다. 혈청 크레아티닌 상승의 경우, 부분 신절제술과 비교한 2편(중재군 221명, 비교군 135명) 및 RFA와 비교한 1편(중재군 6명, 비교군 6명) 모두에서 군 간 유의한 차이가 없거나 통계적 유의성을 보고하지 않았다. 만성 신질환으로의 진행을 평가한 2편(중재군 63명, 비교군 99명)에서도 MWA과 부분 신절제술 간 유의한 차이는 없었다. 수술 관련 지표를 평가한 결과, 시술시간의 경우 부분 신절제술(5편, 중재군 358명, 비교군 2,034명), RFA(2편, 중재군 379명, 비교군 164명), CA(3편, 중재군 132명, 비교군 185명)와 비교했을 때, MWA에서 유의하게 짧은 시술시간이 보고되었다. 다만, RFA와 비교한 다른 3편(중재군 78명, 비교군 344명)에서는 군 간 유의한 차이가 없거나 통계적 유의성을 보고하지 않았다. 재원기간의 경우, 부분 신절제술(6편, 중재군 484명, 비교군 2,114명) 및 RFA 1편(중재군 6명, 비교군 6명)에서는 MWA에서 유의하게 짧은 재원기간이 보고되었으며, RFA와 비교한 다른 1편(중재군 27명, 비교군 16명)에서는 군 간 유의한 차이가 없었다. 한편, CA와 비교한 1편(중재군 50명, 비교군 116명)에서는 MWA의 입원기간이 더 긴 것으로 보고되었으나(2일 vs. 1일), 이는 병원별 치료 프로토콜 차이에 기인한 것으로 해석되었다. 혈액소실량의 경우, 부분 신절제술과 비교한 1편(중재군 185명, 비교군 1770명)에서 MWA의 혈액소실량이 유의하게 더 적은 것으로 보고되었다.

경제성 총 2편의 연구에서 경피적 MWA와 로봇보조 부분 신절제술(robot-assisted partial nephrectomy, RAPN)의 비용-효과성을 평가하였다. 각각 호주와 미국에서 수행된 연구로, 마콥 모형을 이용하여 분석한 결과 두 연구 모두에서 MWA가 RAPN보다 낮은 비용과 더 높은 질 보정 생존연수(quality-adjusted life years, QALYs)를 보여, MWA가 보다 비용효과적인 치료법으로 제시되었다.

결론 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같은 의견을 제시하였다. 경피적 MWA의 안전성과 효과성에 대한 근거를 종합적으로 검토한 결과, 본 시술은 수술적 치료에 비해 안전성은 우수하나 종양학적 효과성은 다소 열등한 경향을 보였으며, RFA 및 CA와는 효과성이 유사한 수준으로 평가되었다. 이에 따라, 고령이거나 중증 동반질환으로 전신마취가 어려운 경우, 다발성 종양이 있는 경우, 한쪽 신장이 없어서 남은 신장의 기능 보존이 절대적으로 중요한 경우, 종양의 위치상 수술적 접근이 곤란한 경우 등 수술이 제한적인 상황에서 신장 종양의 크기가 작은 환자에게 본 기술은 비교적 안전하고 효과적인 치료 대안이 될 수 있다고 판단하였다. 한편, 일부 해외 연구에서 비용효과성 결과가 보고되었으나, 국내의 임상 상황 및 제도적 여건과는 차이가 있어 이 결과를 직접적으로 적용하기에는 제한이 있다고 판단하였다. 2025년 제1차 재평가전문위원회*(2025.9.12.)는 소위원회 결론 및 분과의견을 검토하여 다음과 같이 심의 의결하고 권고등급을 결정하였다. 재평가전문위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 수술이 제한적인 상황에서 신장 종양의 크기가 작은 환자를 대상으로 신장 종양 치료 목적으로 경피적 MWA의 사용을 ‘약하게 권고함’으로 심의하였다.

Background Microwave ablation (MWA) for renal cell carcinoma is a treatment modality that selectively induces coagulative necrosis of renal tumors by generating microwave energy within the tumor. According to the access approach, this technology was classified into three categories—percutaneous, open, and laparoscopic—and was listed for reimbursement under the National Health Insurance in 2019. Given the current availability of various alternative treatment modalities, the need for a systematic review of the safety and effectiveness of this technology relative to comparator interventions was identified. Through internal monitoring, it was selected as a candidate for reassessment. Following the priority-setting process, it was subsequently designated as a technology for reassessment. Accordingly, at the 2nd Health Technology Reassessment Committee meeting in 2025 (February 14, 2025), the reassessment protocol and the proposed composition of the subcommittee were reviewed. This evaluation was therefore conducted for percutaneous MWA based on these procedural grounds.

Objective The objective of this assessment was to provide evidence on the clinical safety and effectiveness of percutaneous MWA in patients with renal cell carcinoma and to determine the recommendation grade for its use in health technology reassessment.

Methods A systematic review was conducted to evaluate the safety, effectiveness, and cost-effectiveness of percutaneous MWA in patients with renal cell carcinoma. All assessment methods were finalized through discussions by the “Subcommittee on Microwave Ablation for Renal Cancer” (hereinafter referred to as the “Subcommittee”), taking into account the objective of the assessment. The Subcommittee consisted of six members appointed by the Health Technology Reassessment Committee: two urologists, one nephrologist, one radiologist, one hemato-oncologist, and one evidence-based medicine specialist. The key question of this assessment was: “Is percutaneous MWA clinically safe and effective when used for the treatment of renal tumors in patients with renal cell carcinoma?” The target population comprised patients with renal cell carcinoma, and the comparators were partial nephrectomy, radiofrequency ablation (RFA), cryoablation (CA), and radiation therapy. The outcomes of interest included safety, effectiveness, and cost-effectiveness. Safety was assessed based on procedure-related complications. Effectiveness was evaluated in terms of oncologic outcomes (survival, recurrence, and metastasis), procedural success, renal function-related outcomes (hematologic parameters and progression to chronic kidney disease), and surgery-related outcomes (procedure time, length of hospital stay, and blood loss). Comparative studies or higher levels of evidence were included in the assessment. The literature search was performed in three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews–Cochrane Central Register of Controlled Trials) and two domestic databases (KoreaMed and RISS), with the final search conducted on April 4, 2025. The risk of bias of the included studies was assessed using the Korean version of the Risk of Bias Assessment Tool for Nonrandomized Studies (RoBANS) 2.0. The certainty of evidence identified through the systematic review was evaluated using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Data were extracted using a pre-specified extraction form. All review processes were conducted independently by two reviewers, and any disagreements were resolved through consensus. Based on the findings reviewed by the Subcommittee, the final recommendation grade was determined by the Health Technology Reassessment Expert Committee after deliberation.

Results A total of 27 studies (reported in 28 publications) were included in the final assessment. Of these, 25 studies (total participants: 2,390 in the intervention group and 5,751 in the comparator group) were retrospective cohort studies reporting safety and effectiveness outcomes, and the remaining two were economic evaluations of cost-effectiveness. The study populations mainly comprised patients with small renal tumors, most of whom had stage T1a disease or tumors smaller than 4 cm in diameter. Partial nephrectomy was the most common comparator (12 studies), followed by RFA (7 studies), CA (6 studies), and two studies comparing all three nonsurgical ablative modalities (MWA, RFA, and CA). Risk-of-bias assessment indicated that many studies had a high risk of bias in the comparability domain because of imbalances in baseline characteristics between groups, differences in sample size, or discrepancies in follow-up duration.

Safety The safety of percutaneous MWA was assessed in 24 studies (2,396 patients in the intervention group and 5,367 in the comparator group) based on procedure-related complications. Among the eight studies comparing MWA with partial nephrectomy, seven reported major complications. In all seven studies, the incidence of major complications was lower in the MWA group than in the partial nephrectomy group, although the between-group difference reached statistical significance in only one study. Of the six studies comparing MWA with RFA, four reported major complications; of the five studies comparing MWA with CA, four reported major complications; and both studies comparing MWA with RFA and CA reported major complications. None of these studies found a statistically significant difference between MWA and the comparator interventions.

Effectiveness The effectiveness of percutaneous MWA was assessed using oncologic outcomes, including survival, recurrence, and metastasis, as well as procedural success as the primary outcomes, and renal function-related outcomes and surgery-related outcomes as the secondary outcomes. Among the oncologic outcomes, the reported survival endpoints included overall survival, cancer-specific survival, disease-free survival, recurrence-free survival, metastasis-free survival, and progression-free survival. For overall survival, one study comparing MWA with partial nephrectomy (intervention group, n=185; comparator group, n=1,770) reported that the 5-year overall survival rate was significantly lower in the MWA group than in the surgical group. The remaining three studies comparing MWA with partial nephrectomy (intervention group, n=174; comparator group, n=278), one study comparing MWA with RFA (n=13 vs. n=23), and two studies comparing MWA with CA (n=107 vs. n=229) reported no significant between-group difference or did not report statistical significance. Cancer-specific survival did not differ significantly between groups in any of the studies comparing MWA with partial nephrectomy (two studies; n=247 vs. n=1,879), RFA (one study; n=27 vs. n=244), or CA (two studies; n=115 vs. n=217). For disease-free survival, two studies comparing MWA with partial nephrectomy (n=247 vs. n=1,879) reported significantly lower survival in the MWA group at the 2-year and 5-year time points, respectively, whereas no significant difference was found in studies comparing MWA with RFA (one study; n=27 vs. n=244) or CA (two studies; n=115 vs. n=217). For recurrence-free survival, one study comparing MWA with partial nephrectomy (n=41 vs. n=82) reported significantly lower survival in the MWA group at 2 years; however, the remaining two studies comparing MWA with partial nephrectomy (n=197 vs. n=167), one study comparing MWA with RFA (n=373 vs. n=158), and two studies comparing MWA with CA (n=104 vs. n=237) found no significant between-group difference. Metastasis-free survival showed no significant difference in either of the two studies comparing MWA with partial nephrectomy (n=112 vs. n=169). Likewise, progression-free survival did not differ significantly between groups in comparisons with partial nephrectomy (one study; n=12 vs. n=80), RFA (one study; n=27 vs. n=244), or CA (one study; n=27 vs. n=26). Among the 16 studies reporting recurrence, two studies comparing MWA with partial nephrectomy (n=103 vs. n=191) found a significantly higher recurrence rate in the MWA group than in the surgical group. In contrast, the remaining five studies comparing MWA with partial nephrectomy (n=322 vs. n=1,955), four studies comparing MWA with RFA (n=492 vs. n=277), and five studies comparing MWA with CA (n=414 vs. n=736) reported no significant between-group difference or did not report statistical significance. Studies reporting metastasis found no significant difference between MWA and partial nephrectomy (three studies; n=241 vs. n=1,870) or RFA (one study; n=101 vs. n=87). Procedural success was assessed using either the technical success rate, defined as whether the procedure accurately reached the target lesion and completely ablated the entire planned area, or the technique efficacy rate, defined as the proportion of cases in which complete tumor ablation was confirmed on imaging within a specified post-procedural period, usually within one month. Among the seven studies comparing MWA with RFA, one study (n=373 vs. n=158) reported that MWA had a significantly higher complete ablation rate than RFA. However, the remaining six studies comparing MWA with RFA (n=376 vs. n=627) and the five studies comparing MWA with CA (n=377 vs. n=571) reported no significant between-group difference or did not report statistical significance. Regarding renal function-related outcomes, among five studies comparing MWA with partial nephrectomy, one study (n=185 vs. n=1,770) reported a significantly smaller decline in estimated glomerular filtration rate (eGFR) in the MWA group than in the comparator group. In the two studies comparing MWA with RFA (n=198 vs. n=337) and the four studies comparing MWA with CA (n=302 vs. n=346), no significant between-group difference was found or statistical significance was not reported. For serum creatinine elevation, both studies comparing MWA with partial nephrectomy (n=221 vs. n=135) and the one study comparing MWA with RFA (n=6 vs. n=6) found no significant between-group difference or did not report statistical significance. Likewise, in the two studies evaluating progression to chronic kidney disease (n=63 vs. n=99), there was no significant difference between MWA and partial nephrectomy. For surgery-related outcomes, procedure time was significantly shorter with MWA than with partial nephrectomy (five studies; n=358 vs. n=2,034), RFA (two studies; n=379 vs. n=164), and CA (three studies; n=132 vs. n=185). However, three other studies comparing MWA with RFA (n=78 vs. n=344) reported no significant between-group difference or did not report statistical significance. Length of hospital stay was significantly shorter with MWA than with partial nephrectomy (six studies; n=484 vs. n=2,114) and in one study comparing MWA with RFA (n=6 vs. n=6), whereas another study comparing MWA with RFA (n=27 vs. n=16) found no significant difference. In one study comparing MWA with CA (n=50 vs. n=116), hospital stay was longer in the MWA group (2 days vs. 1 day), which was interpreted as being attributable to differences in institutional treatment protocols. Estimated blood loss was significantly lower with MWA than with partial nephrectomy in one study (n=185 vs. n=1,770).

Cost-Effectiveness Two studies evaluated the cost-effectiveness of percutaneous MWA compared with robot-assisted partial nephrectomy (RAPN). These studies, conducted in Australia and the United States, used Markov models and found that MWA was associated with lower costs and higher quality-adjusted life years (QALYs) than RAPN, suggesting that MWA is a more cost-effective treatment option.

Conclusion Based on the findings of the current assessment, the Health Technology Reassessment Subcommittee reached the following conclusions. A comprehensive review of the evidence on the safety and effectiveness of percutaneous MWA indicated that, compared with surgical treatment, MWA showed superior safety but a tendency toward somewhat inferior oncologic effectiveness. In comparison with RFA and CA, MWA demonstrated a similar level of effectiveness. Accordingly, in clinical situations where surgery is limited—such as in elderly patients, patients with severe comorbidities who are not suitable for general anesthesia, patients with multiple tumors, patients with a solitary kidney in whom preservation of residual renal function is critically important, or patients in whom surgical access is difficult because of tumor location—this technology may serve as a relatively safe and effective treatment alternative for patients with small renal tumors. Although cost-effectiveness results were reported in some international studies, their direct applicability to the domestic clinical and institutional context was considered limited. At the 1st Health Technology Reassessment Expert Committee* meeting in 2025 (September 12, 2025), the Committee reviewed the conclusions of the Subcommittee and the panel opinions, and determined the recommendation grade as follows. After comprehensively considering the evidence on clinical safety and effectiveness, as well as other evaluation criteria, the Health Technology Reassessment Expert Committee determined that the use of percutaneous MWA for the treatment of small renal tumors in patients for whom surgery is limited should be weakly recommended in the domestic clinical setting.
