신장암에 실시하는 극초단파열치료술 - 개복술하, 복강경하

Abstract

평가배경 신장암 환자에서 수행하는 극초단파열치료술(Microwave ablation, MWA)은 수술적 절제가 어려운 5cm 미만의 원발성 및 전이성 신장암 환자에서, 신장 종양 내에 극초단파를 발생시켜 종양을 선택적으로 괴사시킴으로써 치료하는 기술이다. 접근 방법에 따라 경피적, 개복술하, 복강경하의 세 가지 항목으로 세분되어 2019년 급여로 등재되었다. 다양한 대체 의료기술이 활용되는 현시점에서, 비교기술 대비 동 기술의 안전성 및 유효성을 체계적으로 검토할 필요성이 제기되었으며, 내부 모니터링을 통해 재평가 대상 안건으로 발굴되었다. 이후 우선순위 심의 절차를 거쳐 재평가 대상 기술로 선정되었고, 이에 따라 2025년 제2차 의료기술재평가위원회(2025. 2. 14.)에서 재평가 계획서와 소위원회 구성(안)이 심의되었다. 본 평가는 이러한 절차적 근거에 기반하여 이 중 개복술하, 복강경하 MWA를 대상으로 수행되었다.

평가목적 본 평가의 목적은 신장암 환자에서 수행하는 개복술하, 복강경하 MWA의 임상적 안전성 및 효과성 등에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다.

평가방법 신장암 환자에서 개복술하, 복강경하 MWA에 대한 안전성, 효과성 및 경제성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “신장암의 극초단파열치료술 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 결정된 비뇨의학과 2인, 신장내과 1인, 영상의학과 1인, 혈액종양내과 1인, 근거기반의학 1인, 총 6인으로 구성하였다. 본 평가의 대상자는 신장암 환자, 비교치료는 부분 신절제술, 고주파열치료술(Radiofrequency, RFA), 냉동제거술(Cryoablation, CA), 방사선치료로 정의하였다. 결과변수로는 안전성과 효과성, 경제성을 확인하였으며, 안전성은 시술 관련 합병증을 결과 지표로, 효과성은 종양학적 결과(생존, 재발, 전이), 시술 성공, 신기능 관련 결과(혈액학적 지표, 만성 신장질환으로의 악화), 수술 관련 지표(시술시간, 재원기간, 혈액소실량)를 결과 지표로 평가하였다. 연구유형은 비교연구 이상인 경우를 평가에 포함하기로 하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 2개(KoreaMed, 한국교육학술정보원(RISS) 데이터베이스에서 검색하였다(최종검색일 2025. 4. 4.). 최종 선택된 연구의 비뚤림위험 평가는 Cochrane의 Risk of Bias (RoB)를 사용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 방법을 이용하여 근거 수준을 평가하였다. 사전에 정한 자료추출 서식을 활용해 자료를 추출하였고, 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다.

평가결과 체계적 문헌고찰 결과 1편의 무작위배정 임상시험연구(중재군 48평, 비교군 54명)가 평가에 최종 선택되었다. 2012년 중국에서 출판된 연구로, 작은 신장 종양 환자를 대상으로 개복술하, 복강경하 MWA와 부분 신절제술의 안전성과 효과성을 비교하였다. 비뚤림위험 평가에서는 배정순서 은폐 방법이 명시되지 않은 등의 이유로 일부 영역에서 비뚤림위험을 ‘불확실’로 평가하였다.

안전성 개복술하, 복강경하 MWA의 안전성은 시술 관련 합병증으로 평가하였다. 합병증이 발생한 환자는 MWA군에서 부분 신절제술군보다 통계적으로 유의하게 적었으며, 중대한 합병증으로 분류하는 Clavein-Dindo 3등급 합병증은 MWA군에서 1건, 부분 신절제술군에서 6건 발생하였고 두 군 모두 4 또는 5등급 합병증은 발생하지 않았다.

효과성 개복술하, 복강경하 MWA의 효과성은 일차 지표로 생존, 재발, 전이를 포함하는 종양학적 결과와 시술 성공률로 평가하였고, 이차 지표로 혈액학적 지표와 만성 신질환으로의 악화를 포함하는 신기능 관련 결과와 수술 관련 지표로 평가하였다. 생존 결과로 MWA군과 부분 신절제술군의 3년 시점 무재발 생존율은 유의한 차이가 없었고(91.3% vs. 96%), 암특이 생존율의 경우 두 군 모두 100%였다. 재발은 MWA군과 부분 신절제술군 모두에서 각각 2건(4.2% vs 3.7%) 발생하였는데, 군 간 재발률 차이는 통계적으로 유의하지 않았다. 신기능 변화 평가 결과, 수술 직후에는 MWA군에서 혈청 크레아티닌 수치가 유의하게 낮고 eGFR 감소율도 부분 신절제술군보다 적었으나(p=0.0092), 추적관찰 이후에는 두 군 간 eGFR 감소율에 유의한 차이는 확인되지 않았다(p=1.0000). 수술 관련 지표로 시술시간과 입원기간은 군 간 유의한 차이가 없었고, 혈액소실량은 MWA군에서 부분 신절제술군보다 통계적으로 유의하게 적었다. 핵심적인 결과 지표인 시술 관련 전체 및 중대한 합병증, 종양학적 결과, 시술 성공 지표를 중심으로 GRADE 평가결과, 전반적인 근거수준은 ‘낮음(low)’으로 평가하였다.

결론 및 제언 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같은 의견을 제시하였다. 신장암에서 시행된 개복술하 및 복강경하 극초단파열치료술의 근거를 검토한 결과, 관련 연구는 2012년에 출판된 1편으로 확인되었으며, 이는 신장암을 대상으로 한 극초단파열치료술 연구 중 가장 오래된 것으로 파악되었다. 임상현장에서는 경피적 접근이 상대적으로 용이한 반면, 복강경 접근은 기술적 난이도가 높아 실제 활용이 거의 없고, 개복 접근의 경우에는 수술적 절제가 더 적절하다는 전문가 의견이 제시되었다. 이러한 점을 종합할 때, 개복술하 및 복강경하 극초단파열치료술의 임상적 효과성을 입증할 만한 근거는 현재로서는 충분하지 않다고 판단하였다. 2025년 제1차 재평가전문위원회*(2025.9.12.)는 소위원회 결론 및 분과의견을 검토하여 다음과 같이 심의 의결하고 권고등급을 결정하였다. 재평가전문위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 수술이 제한적인 상황에서 신장 종양의 크기가 작은 환자를 대상으로 신장 종양치료 목적으로 개복술하 및 복강경하 극초단파열치료술의 사용을 ‘권고보류’로 심의하였다.

Background Microwave ablation (MWA) for renal cell carcinoma (RCC) is a thermal ablation technique that induces selective tumor necrosis by delivering microwave energy directly to the renal tumor. It is used to treat primary or metastatic renal tumors smaller than 5 cm in patients for whom surgical resection is difficult. According to the route of access, MWA is categorized into three approaches—percutaneous, open, and laparoscopic—and all three were reimbursed under the Korean National Health Insurance in 2019. Given the current availability of various alternative treatment modalities, the need for a systematic review of the safety and effectiveness of this technology relative to comparator interventions was identified through internal monitoring. Following the priority-setting process, open and laparoscopic MWA were selected as technologies for reassessment. Accordingly, the reassessment protocol and the proposed composition of the subcommittee were reviewed by the 2nd Health Technology Reassessment Committee of 2025 (February 14, 2025). Based on these procedural grounds, the present assessment was conducted for open and laparoscopic MWA.

Objectives The objective of this assessment was to provide evidence on the clinical safety and effectiveness of open and laparoscopic MWA in patients with RCC and to determine the recommendation grade for these procedures within the health technology reassessment framework.

Methods A systematic literature review was conducted to assess the safety, effectiveness, and cost-effectiveness of open and laparoscopic MWA in patients with RCC. All methodological aspects of the assessment were finalized through deliberation by the Subcommittee on Microwave Ablation for Renal Cancer (hereafter, “the Subcommittee”). The Subcommittee consisted of six members appointed by the Health Technology Reassessment Committee: two urologists, one nephrologist, one radiologist, one hemato-oncologist, and one evidence-based medicine specialist. The target population comprised patients with RCC. Comparator interventions were partial nephrectomy, radiofrequency ablation (RFA), cryoablation (CA), and radiotherapy. The outcomes of interest were safety, effectiveness, and cost-effectiveness. Safety was evaluated based on procedure-related complications. Effectiveness outcomes included oncological outcomes (survival, recurrence, and metastasis), procedural success, renal function outcomes (hematologic parameters and progression to chronic kidney disease), and surgery-related outcomes (operative time, length of hospital stay, and blood loss). Comparative studies or those with a higher level of evidence were eligible for inclusion. The literature search was conducted in three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews-Cochrane Central Register of Controlled Trials) and two Korean databases (KoreaMed and the Research Information Sharing Service [RISS]). The final search date was April 4, 2025. Risk of bias in the included studies was assessed using the Cochrane Risk of Bias (RoB) tool. The certainty of evidence from the systematic review was evaluated using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. Data were extracted using a predefined extraction form. All review processes were conducted independently by two reviewers, and any disagreements were resolved by consensus. Based on the Subcommittee’s review, the Reassessment Expert Committee conducted the final deliberation and determined the recommendation grade.

Results The systematic literature review identified one randomized controlled trial for final inclusion (intervention group, n=48; comparator group, n=54). The study was published in China in 2012 and compared the safety and effectiveness of open and laparoscopic MWA with partial nephrectomy in patients with small renal tumors. In the risk-of-bias assessment, some domains were judged as unclear risk, including allocation concealment, because the relevant methods were not explicitly described.

Safety The safety of open and laparoscopic MWA was assessed in terms of procedure-related complications. The number of patients who experienced complications was statistically significantly lower in the MWA group than in the partial nephrectomy group. Regarding major complications, one Clavien-Dindo Grade III complication occurred in the MWA group and six occurred in the partial nephrectomy group. No Grade IV or V complications were reported in either group.

Effectiveness The effectiveness of open and laparoscopic MWA was assessed using oncological outcomes (survival, recurrence, and metastasis) and procedural success as primary outcomes, and renal function outcomes and surgery-related outcomes as secondary outcomes. For survival outcomes, the 3-year recurrence-free survival rate did not differ significantly between the MWA and partial nephrectomy groups (91.3% vs. 96%), and cancer-specific survival was 100% in both groups. Recurrence occurred in two patients in each group (4.2% vs. 3.7%), with no statistically significant between-group difference in recurrence rate. With respect to renal function, immediately after surgery the MWA group had significantly lower serum creatinine levels and a smaller decline in estimated glomerular filtration rate (eGFR) than the partial nephrectomy group (p=0.0092). However, at follow-up, no significant between-group difference was observed in the decline in eGFR (p=1.0000). For surgery-related outcomes, there were no significant between-group differences in operative time or length of hospital stay, whereas blood loss was significantly lower in the MWA group than in the partial nephrectomy group. According to the GRADE assessment of the key outcomes, including overall and major procedure-related complications, oncological outcomes, and procedural success, the overall certainty of evidence was rated as low.

Conclusions and Recommendations Based on the findings of the current assessment, the Health Technology Reassessment Subcommittee concluded as follows. A review of the evidence on open and laparoscopic MWA for RCC identified only one relevant study published in 2012, which was also the earliest study of MWA for renal tumors. Expert opinion indicated that, in current clinical practice, the percutaneous approach is relatively easy to perform, whereas the laparoscopic approach is technically demanding and rarely used in practice; for the open approach, surgical resection was considered more appropriate. Taken together, the available evidence was judged insufficient to establish the clinical effectiveness of open and laparoscopic MWA. The 1st Health Technology Reassessment Expert Committee of 2025* (September 12, 2025) reviewed the Subcommittee’s conclusion and the opinions of the working group, and made the following decision. After comprehensively considering the evidence on clinical safety and effectiveness, along with other evaluation factors, the Reassessment Expert Committee decided that, in the current Korean clinical setting, the use of open and laparoscopic MWA for the treatment of small renal tumors in patients with RCC when surgery is limited should be assigned a recommendation grade of “Deferred recommendation.”
