지속적 말초신경 및 신경총 통증(자기)조절법 - 좌골신경

Abstract

평가배경 ‘지속적 말초신경 및 신경총 통증(자기)조절법 – 좌골신경(이하, 지속적 좌골신경차단술)’은 수술 후 통증 관리를 목적으로 좌골신경 주위에 카테터를 삽입하고 국소마취제를 지속적으로 주입하여 해당 신경을 차단하는 의료기술이다. 2016년 신의료기술평가 후, 2019년 선별급여 80% 항목으로 등재되어 현재까지 사용 중이다. 해당 의료기술은 선별급여 항목의 적합성평가 주기를 고려하여 내부 모니터링으로 발굴된 주제로, 2022년 제4차 의료기술재평가위원회에서 평가계획서를 심의받아 재평가를 수행하였다.

평가방법 본 평가는 족부 및 족관절 수술 환자에서 ‘지속적 좌골신경차단술’의 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “지속적 말초신경 및 신경총 통증(자가)조절법 – 좌골신경 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 마취통증의학과 2인, 정형외과 2인, 재활의학과 1인, 근거기반의학 1인의 전문가 6인으로 구성하였다. 평가의 핵심질문은 “지속적 좌골신경차단술은 족부 및 족관절 수술 환자에서 수술 후 통증 관리를 목적으로 단독사용시 안전하고 효과적인가?”였다. 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 하였으며, 문헌선정은 선택배제 기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias를 사용하여 평가하였고, 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였다. 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료 분석은 정량적 분석이 가능할 경우 메타분석을 수행하고 불가능한 경우 질적 검토를 수행하였다. 본 평가의 체계적 문헌고찰 결과는 GRADE (Grading of Recommendations Assessment, Development and Evaluation) 방법을 이용하여 근거 수준을 평가하고, 평가결과를 토대로 권고등급을 결정하였다.

평가결과 체계적 문헌고찰 수행 결과, 핵심질문을 충족하는 총 12개의 무작위배정 비교임상시험(12편 문헌)을 선택하였다. 연구대상자는 발목 수술, 무지외반증 수술, 하지 원위부의 정형외과 수술을 받은 환자가 포함되었으며, 일회성 좌골신경차단술, 간헐적 좌골신경차단술, 지속적 경막외 주입이 대조군으로 포함되었다. 연구수행과 관련된 비뚤림위험은 대부분 낮았으나 ‘민간연구비 지원’과 관련된 비뚤림위험이 다소 높게 나타났다. 안전성 지속적 좌골신경차단술의 안전성은 모든(12개) 무작위배정 비교임상시험에서 보고하였으며, 안전성 결과는 크게 시술 관련 합병증과 약물 부작용으로 구분하여 확인하였다. 시술 관련 합병증을 보고한 9개의 연구 모두에서는 지속적 좌골신경차단술과 다른 통증조절법들간 시술 관련 합병증 발생에 유의한 차이가 없는 것으로 나타났다. 시술 관련 합병증은 카테터 문제(누출, 이탈, 분리 등), 장비 실패가 주요하게 보고되었다. 약물 부작용은 해당 시술에 사용되는 국소마취제로 인한 부작용 이외 통증 조절을 위해 추가 투여하는 마약성 진통제의 부작용이 혼재되었으나, 이를 구분하는 것이 쉽지 않아 관련 부작용들을 모두 포함하였다. 오심 및 구토, 가려움증은 마약성 진통제의 대표적 부작용으로 지속적 좌골신경차단술과 직접적인 관련성은 낮다고 볼 수 있다. 약물 부작용을 보고한 8개의 연구 대부분에서 지속적 좌골신경차단술과 대조군간 유의한 차이가 없었다. 일부 연구에서는 해당 의료기술에서 오심 및 구토, 무감각, 요페 발생이 유의하게 덜 발생하였으나, 또 다른 1개 연구에서는 감각이상(저림)이 일회성 좌골신경차단술보다 유의하게 더 많이 발생하는 것으로 나타났다. 약물 부작용으로는 오심 및 구토, 가려움증, 감각이상이 주요하게 보고되었다. 효과성 지속적 좌골신경차단술의 효과성은 모든(12개) 무작위배정 비교임상시험에서 보고하였으며, 효과성 결과는 크게 5개로 나누어 통증 정도, 수술 후 진통제 사용량, 기능(재활)지표, 환자만족도, 재원기간을 확인하였고, 결과지표는 대조군별로 나누어 확인하였다. 12개의 모든 연구에서는 통증 정도를 보고하였다. 해당 의료기술은 일회성 좌골신경차단술보다 적어도 한 시점에서 통증 감소의 효과가 더 좋았으나(5/5개), 간헐적 투여와 비교한 대부분의 연구(4/6개)에서는 두 군간 유의한 차이가 없거나 대조군의 통증 감소 효과가 유의하게 더 좋았다(2/6개). 지속적 경막외 주입과 비교한 1개 연구는 두 군간 유의한 차이가 없었다. 수술 후 진통제 소비량도 모든(12개) 연구에서 보고하였으며, 통증 정도와 유사한 결과가 확인되었다. 지속적 좌골신경차단술은 일회성 좌골신경차단술과 비교한 대부분의 연구(4/5개)에서 수술 후 진통제 소비량이 유의하게 낮았으나, 간헐적 투여와 비교한 연구에서는 일관성이 부족한 상반된 연구결과들이 혼재되어 보고되었다. 지속적 경막외 주입과 비교한 1개 연구는 두 군간 유의한 차이가 없는 것으로 나타났다. 지속적 좌골신경차단술의 기능(재활)지표를 보고하고 있는 연구는 없었다. 10개의 연구에서 환자만족도 결과를 보고하였다. 지속적 좌골신경차단술은 일회성 좌골신경차단술, 간헐적 투여, 지속적 경막외 주입술과 비교시 환자만족도에 유의한 차이가 없거나, 서로 상반된 연구결과가 혼재되어 있었다. 3개의 연구에서 재원기간을 보고하였다. 각각의 연구에서는 상반된 연구결과가 혼재되어 일관성이 부족하였다. 연구결과 지표별로 확인한 평가결과의 GRADE 근거수준은 대부분의 결과지표에서 중등도(Moderate)로 판단되었으나, 재원기간은 표본수가 400명 이하로 현저히 낮아 근거수준을 낮음(Low)으로 평가하였다.

결론 및 제언 해당 소위원회는 현재 평가결과에 근거하여 다음과 같이 제언하였다. 체계적 문헌고찰 결과, 지속적 좌골신경차단술은 시술 관련 합병증 및 약물 부작용 발생에 있어 카테터 관련 문제, 오심 및 구토, 감각이상 등의 경미한 합병증이 보고되었으나 대부분의 연구에서 다른 통증조절법(대조군)과 비교시 유의한 차이가 없어 안전한 의료기술로 평가하였다. 지속적 좌골신경차단술은 통증 정도 및 진통제 소비량에 있어 일회성 좌골신경차단술보다는 더 효과적이었으나, 다른 통증조절법(간헐적 투여, 지속적 경막외 주입)과 비교시 유의한 차이가 없거나 상반된 연구결과가 혼재되어 있었다. 환자만족도 및 재원기간에 있어서는 대조군에 상관없이 두 군간 유의한 차이가 없거나 일관성 없는 상반된 연구결과 혼재되어 있었다. 따라서 소위원회에서는 본 평가결과를 바탕으로 족부 및 족관절 수술 환자에서 수술 후 통증 관리를 목적으로 사용되는 ‘지속적 말초신경 및 신경총 통증(자기)조절법 – 좌골신경’은 안전한 기술이며, 다른 통증조절법과 효과가 유사한 치료대안 중 하나로 판단하였다. 2023년 제2차 의료기술재평가위원회(2023.02.10.)에서는 의료기술재평가사업 관리지침 제4조제10항에 의거 ‘지속적 말초신경 및 신경총 통증(자가)조절법 – 좌골신경’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 족부 및 족관절 수술 환자에서 ‘지속적 말초신경 및 신경총 통증(자가)조절법 – 좌골신경’은 안전한 기술이며, 다른 통증조절법과 유사한 의료기술로 ‘조건부 권고함’으로 심의하였다.

Background Continuous (patient-controlled) analgesia of the peripheral nerve or plexus – sciatic nerve (hereafter referred to as “continuous sciatic nerve block," or CSNB), is a medical technique that involves the placement of a catheter adjacent to the sciatic nerve to provide a continuous infusion of a local anesthetic, thereby blocking the nerve to manage postoperative pain. Following a health technology assessment in 2016, CSNB was incorporated as a selective benefit item with an 80% patient co-payment requirement in 2019, and it continues to be available under this categorization. The need for re-evaluation of this technique was recognized via internal monitoring, prompted by the scheduled review cycle for selective benefit compliance. Subsequently, the re-evaluation was carried out in accordance with the reassessment plan discussed during the fourth meeting of the Committee of Health Technology Reassessment in 2022.

Methods A systematic review was conducted to evaluate the safety and efficacy of CSNB in patients who have undergone surgery on the foot and ankle or foot joints. The methodologies for this assessment were established following deliberations by the CSNB subcommittee (hereafter referred to as “Subcommittee”), with careful consideration of the evaluation process. This Subcommittee comprised six experts: two each from anesthesiology and pain medicine and orthopedic surgery and one each from rehabilitation medicine and evidence-based medicine. The principal question addressed was, “Is CSNB safe and effective as a sole modality for postoperative pain management in patients after foot and ankle or foot joint surgeries?” During the literature search phase, three international and five domestic databases were queried. Two independent reviewers screened the articles, applying predefined inclusion and exclusion criteria. The risk of bias (RoB) was assessed using the Cochrane RoB tool. Data extraction was independently conducted by the same reviewers, utilizing a pre-established template. Discrepancies between reviewers were resolved by consensus, facilitated by a third reviewer if necessary. A meta-analysis was carried out where quantitative synthesis was feasible; in cases where it was not, a qualitative synthesis was undertaken. The findings from the systematic review were appraised for their evidence level employing the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach. The final grade of recommendation was assigned based on the results of the evaluation.

Results The systematic review identified 12 randomized controlled trials (RCTs), encapsulated in 12 articles, that met the criteria of the key question. The selected studies encompassed a population of patients who underwent surgeries on the foot and ankle joints, hallux valgus corrective procedures, or other orthopedic interventions on the distal lower extremity. The comparative interventions (controls) examined alongside CSNB were single-shot sciatic nerve block (SNB), intermittent SNB, and continuous epidural analgesia. The RoB) associated with the execution of the studies was generally low. However, there was a moderately higher RoB detected in the context of studies funded by private entities.

Safety The safety profile of CSNB was addressed in all 12 RCTs). This assessment took into account procedure-related complications (PRCs) and adverse drug reactions (ADRs). The nine RCTs that provided data on PRCs found no substantial difference in the incidence of PRCs when comparing CSNB to other pain management techniques. The PRCs primarily consisted of issues related to the catheter, such as leakage, dislodgement, and disconnection, as well as failures of the equipment used. ADRs encompassed adverse events (AEs) from the local anesthetics utilized in CSNB, combined with those arising from supplemental narcotic analgesics used for pain management. Distinguishing the specific source of AEs was challenging; hence, all associated AEs were included in the analysis. Common AEs linked to narcotic analgesics are nausea, vomiting, and pruritus, which seemed to be indirectly related to CSNB. Out of eight studies that examined ADRs, the majority indicated no significant disparity in the occurrence of ADRs between CSNB and the controls. Some studies noted a significantly lower frequency of nausea, vomiting, numbness, and and anuria in the CSNB group. Conversely, one study observed that paresthesia (numbness) was significantly more prevalent in the CSNB group compared to the group receiving a single-shot SNB). The most frequently reported ADRs were nausea, vomiting, pruritus, and paresthesia.

Effectiveness The effectiveness of CSNB was evaluated in all 12 randomized controlled trials (RCTs). The assessment of effectiveness was based on five criteria: pain severity, postoperative analgesic consumption, functional (rehabilitation) outcomes, patient satisfaction, and duration of hospital stay. These outcome measures were analyzed in comparison with each respective control group. All 12 studies reported on pain severity. Findings indicated that CSNB generally provided better pain relief at various time points compared to the single-shot SNB) in all five studies assessing this comparison. In contrast, when CSNB was compared with intermittent administration, most studies (4 out of 6) found no significant difference in pain relief, and two studies reported superior pain reduction in the control group. One study comparing CSNB to continuous epidural analgesia found no notable difference in pain outcomes between the two groups. Postoperative analgesic requirements were documented in all studies, with outcomes paralleling those for pain severity. The majority of studies comparing CSNB to single SNB (4 out of 5) observed a significant reduction in analgesic usage in the CSNB group. However, results were variable and lacked consistency when CSNB was compared with intermittent administration. One study examining CSNB against continuous epidural analgesia noted no substantial difference. No studies addressed functional (rehabilitation) outcomes in relation to CSNB. Patient satisfaction was reported in 10 studies. Those comparing CSNB to single SNB, intermittent administration, and continuous epidural infusion found either no significant difference in patient satisfaction or presented conflicting evidence. Only three studies reported on the length of hospital stay, yielding inconsistent results. The GRADE approach rated the quality of evidence as moderate for most outcomes but low for the length of hospital stay due to the very small sample size (under 400 patients).

Conclusions and recommendations The Subcommittee, informed by the results of the systematic review, put forth the following recommendations: The review highlighted that complications associated with CSNB, such as catheter issues, nausea, vomiting, and paresthesia, were generally mild. CSNB was deemed a safe medical modality since the majority of studies found no significant difference in PRCs and ADRs when compared with other analgesic techniques (controls). In terms of pain management, CSNB was more effective in reducing pain severity and postoperative analgesic consumption compared to a single-shot sciatic nerve block (SNB). Against other pain management strategies, such as intermittent administration and continuous epidural infusion, CSNB did not demonstrate significant differences, or the results were mixed. Furthermore, CSNB did not significantly affect patient satisfaction or the length of hospital stay compared to any control group, with some studies reporting variable outcomes. Considering these findings, the Subcommittee concluded that CSNB represents a secure option for managing postoperative pain in patients undergoing foot and ankle or foot joint surgery, offering an efficacy comparable to other pain management modalities. During the initial meeting of the Committee of Health Technology Reassessment in 2023 (on January 13, 2023, conclusions were drawn about CSNB following Article 4-10 of the Guidelines for the Management of Health Technology Reassessment. The Committee “conditionally recommends” the use of CSNB, recognizing it as a safe option that provides similar effectiveness to other pain management approaches for patients post foot and ankle or foot joint surgeries. This recommendation is contingent upon the clinical context in Korea, taking into account the totality of evidence regarding its clinical safety and effectiveness, as well as other evaluative factors.
