현미경 관찰을 이용한 자동화된 항균제 감수성 검사

Abstract

평가배경 현미경 관찰을 이용한 자동화된 항균제 감수성 검사(Microscopic Imaging Based Automated Antimicrobial Susceptibility Test)는 감염 의심 환자를 대상으로 항균제 감수성 및 내성 여부를 확인하는 검사이다. 동 기술은 2018년도에 신의료기술평가로 인정되었으며 2023년 선별급여(50%)로 등재되었다. 이후 선별급여 적합성 평가시기(2026년)가 도래됨에 따라 수요조사(유관기관)를 통해 평가 필요성이 확인되어 재평가 대상 선정 절차를 거쳐 최종 선정되었다. 이에 따라 2025년 제3차 의료기술재평가위원회(2025.03.14.)에서는 동 기술에 대한 평가계획서 및 소위원회 구성안을 심의하였고, 이후 해당 기술의 임상적 안전성 및 효과성에 대한 평가가 수행하였다.

평가목적 본 평가의 목적은 현미경 관찰을 이용한 자동화된 항균제 감수성 검사의 임상적 안전성 및 효과성에 대한 근거를 제공하고 해당 기술의 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다.

평가방법 현미경 관찰을 이용한 자동화된 항균제 감수성 검사가 감염 의심 환자에게 임상적으로 안전하고 효과적인지 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “현미경 관찰을 이용한 자동화된 항균제 감수성 검사 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회 구성은 진단검사의학과 2인, 감염내과 2인, 소아청소년과 1인, 근거기반의학 1인 총 6인의 전문의로 구성하였다. 동 평가의 대상자는 감염 의심 환자이며, 중재검사는 현미경 관찰을 이용한 자동화된 항균제 감수성 검사이다. 참고표준검사는 유럽 항균제 감수성 시험 위원회(European Committee on Antimicrobial Susceptibility Testing, EUCAST) 및 임상 및 실험실 표준 연구소(Clinical and Laboratory Standards Institute, CLSI) 가이드라인에 의하여 수행된 검사이며, 비교검사는 디스크 확산법, 항균제 최소억제농도(액체배지미량희석법, 액체배지희석법, 한천희석법, 자동화 희석법)로 설정하였다. 평가의 핵심질문은 “현미경 관찰을 이용한 자동화된 항균제 감수성 검사는 감염 의심 환자에서 항균제 감수성 및 내성 여부를 확인하는데 임상적으로 안전하고 효과적인가?”이다. 대상환자는 감염 의심 환자이고, 참고표준/비교검사는 디스크 확산법, 항균제 최소억제농도이며, 결과지표는 안전성은 검사 관련 부작용, 효과성은 의료결과에의 영향, 참고표준/비교검사와의 일치도, 검사 소요시간, 검사 실패율로 평가하였다. 체계적 문헌고찰은 위의 핵심질문을 토대로 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials) 및 국내 데이터베이스 2개(KoreaMed, 한국교육학술정보원)를 이용하여 연구를 검색하였다. 연구 선택기준 및 배제기준 적용을 통한 연구 선택과 비뚤림위험 평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 비뚤림위험 평가는 선택연구 중 무작위배정 비교임상시험연구(randomized controlled trial, RCT) 3편을 대상으로 Risk of bias (RoB), 비무작위 대조연구(non randomized controlled trail, nRCT) 6편, 코호트연구 5편을 대상으로 비무작위 연구 비뚤림 위험 평가 도구(Risk of Bias Assessment tool for Non-randomized Studies 2.0, ROBANS 2.0)을 이용하여 평가하였다. 자료분석은 일치도의 보고 방식, 검사소요시간에서 측정 시점, 의료결과에서 항균제 스튜어드십 프로그램(Antimicrobial Stewardship Program, ASP)의 연구 설계 방식이 다양하여 질적으로 합성하여 제시하였다. 본 평가는 소위원회의 검토결과를 바탕으로 재평가전문위원회에서 최종심의 후 권고등급을 결정하였다.

평가결과 동 기술은 총 64편(진단적 코호트연구가 49편, RCT 3편, nRCT 6편, 코호트 연구 6편)의 연구로 평가하였으며, 경제성 평가 연구는 없었다. 비뚤림위험 평가 결과, RCT에서는 배정순서 은폐 및 결과 평가 눈가림의 보고 부족으로 ‘불확실’ 비율이 높았고, 연구참여자·연구자 눈가림 미실시, 불충분한 결과자료 및 연구지원과 관련된 영역에서 ‘높음’ 비뚤림위험이 관찰되었다. nRCT 및 코호트연구에서는 대상군 간 비교가능성 저하와 교란변수 통제 미흡으로 ‘높음’ 또는 ‘불확실’ 비뚤림위험이 다수 확인되었으며, 결과 평가자 눈가림에 대한 보고도 전반적으로 부족하였다.

안전성 현미경 관찰을 이용한 자동화된 항균제 감수성 검사의 안전성을 보고한 연구는 없었다. 동 기술은 환자에게서 채취된 검체를 배양하여 얻은 양성균을 대상으로 체외에서 시행하는 검사로, 검사수행에 있어 대상자에게 직접적인 위해가 없어 안전성에는 문제가 없는 것으로 평가하였다.

효과성 현미경 관찰을 이용한 자동화된 항균제 감수성 검사의 효과성은 의료결과의 영향, 참고표준/비교검사와의 일치도, 검사 소요시간, 검사 실패율로 평가하였다. 의료결과에의 영향은 항균제 관련 결과와 임상 관련 결과로 구분하였으며, ASP 수행 여부에 따라 각각 제시하였다. 항균제 관련 결과 중 최적치료 관련 지표인 최적치료까지의 시간에 대해, ASP를 병행한 nRCT 5편을 포함한 총 7편 모두에서 중재군이 유의하게 짧았다. 첫 항생제 변경까지 시간은 2편(ASP 병행 RCT 1편, nRCT 1편)에서 중재군이 유의하게 짧았고(p=0.01, 0.002) 그 외 항균제 변경 시점은 군 간 유의한 차이가 없거나 이질적인 결과를 제시하였다. 항균제 사용 관련 결과로 최적치료 달성은 ASP 병행한 RCT 1편, nRCT 1편에서 중재군이 유의하게 높았고 ASP 병행하지 않은 RCT 1편에서 중재군이 유의하게 높으며(p=0.004), 코호트 1편에서 군 간 유의한 차이가 없었다(p=0.454). 광범위 그람양성/음성균 치료일수, 광범위 항균제 사용기간 및 사용량, 총 항균제 치료일수 및 치료량, 좁은 스펙트럼 β-lactam 사용기간 및 사용량, 글리코펩타이드 사용량, 정맥에서 경구 전환율 등의 지표는 일부 연구(1~2편)에서만 유의하거나 이질적인 결과가 보고되었다. 임상 관련 결과 중 사망 관련 결과는 총 10편(RCT 2편, nRCT 5편, 코호트 3편)에서 보고하였고, 30일 사망, 입원 중 사망, 병원 내 사망, 균혈증 관련 사망, 감염 관련 사망, 모두 군 간 유의한 차이가 없었다. 그 외 재원 관련 결과, 감염 관련 합병증, 기타 결과는 이질적이거나 군 간 유의한 차이가 없었다. 그람양성균의 참고표준검사와의 일치율은 CA 94~99%, VME 0~17.7%, ME 0~2.5%, mE 0~4%, EA 96~98.7%, 비교검사와의 일치율은 CA 92~97%, VME 1~2%, ME 3~11.4%, mE 0~4.5%, EA 96~97%이었다. 그람음성균의 참고표준검사와의 일치율은 CA 75~98%, VME 0~13.1%, ME 0~4.3%, mE 0~24%, EA 92~98%, 비교검사와의 일치율은 CA 89~97%, VME 0~12%, ME 0~4.2%, mE 0~7.4%, EA 90~98%이었다.

결론 및 권고결정 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같은 의견을 제시하였다. 동 기술은 환자 검체를 이용한 체외 검사로 검사 수행 과정에 있어 대상자에게 직접적인 위해가 없어 안전성에는 문제가 없는 것으로 평가하였다. 효과성 측면에서 최적 치료 도달 시간은 유의하게 감소되었으나, 사망률 개선, 재원기간 등 임상적 효과에 대한 근거는 제한적이라는 의견이었다. 참고표준/비교검사와의 일치율은 혈액배양 양성액을 직접 검사한다는 점을 고려할 때 전반적으로 수용 가능한 수준이며, 이에 따라 현 시점에서는 참고표준/비교검사를 대체하기보다는 보조적인 추가 검사로의 사용이 적절하다는 의견이었다. 또한 동 검사는 분리배양 과정을 거치지 않아 검사 소요시간을 단축할 수 있어 중환자 및 패혈증 환자의 초기 치료 전략 수립에 기여할 수 있으며, 항균제 스튜어드십 프로그램과 연계 시 신속한 임상 반영을 통해 항균제 최적화에 도움이 된다는 의견이었다. 2025년 제2차 재평가전문위원회*(2025.10.17.)는 소위원회 검토결과에 근거하여 다음과 같이 심의하였다. 재평가전문위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 감염 의심 환자를 대상으로 항균제 감수성 및 내성 여부를 확인하는데 보조적으로 사용할 수 있는 검사로 ‘약하게 권고함’으로 결정하였다.

Background The Microscopic Imaging-Based Automated Antimicrobial Susceptibility Test is performed to determine antimicrobial susceptibility and resistance in patients with suspected infection. This technology was approved as a new health technology assessment in 2018 and was listed under selective coverage at 50% in 2023. As the reassessment period for selective coverage (2026) approached, the need for evaluation was identified through a demand survey of relevant institutions, and the technology was subsequently selected as a subject for re-evaluation after a priority review process. Accordingly, the 3rd Health Technology Reassessment Committee of 2025 (March 14, 2025) deliberated on the evaluation plan and the composition of the subcommittee for this technology, after which an assessment of its clinical safety and effectiveness was conducted.

Objective The objective of this assessment was to provide evidence on the clinical safety and effectiveness of the Microscopic Imaging-Based Automated Antimicrobial Susceptibility Test and to determine the recommendation grade for health technology reassessment regarding its use.

Methods A systematic review was conducted to assess whether the Microscopic Imaging-Based Automated Antimicrobial Susceptibility Test is clinically safe and effective in patients suspected of infection. All assessment methods were finalized through discussion by the “Subcommittee for Microscopic Imaging-Based Automated Antimicrobial Susceptibility Test” (hereinafter referred to as the “Subcommittee”) in consideration of the assessment objective. The Subcommittee comprised six specialists: two in laboratory medicine, two in infectious diseases, one in pediatrics, and one in evidence-based medicine. The target population for this assessment was patients suspected of infection, and the index test was the Microscopic Imaging-Based Automated Antimicrobial Susceptibility Test. The reference standard test was a test performed in accordance with the guidelines of the European Committee on Antimicrobial Susceptibility Testing (EUCAST) and the Clinical and Laboratory Standards Institute (CLSI), and the comparator tests were disk diffusion and antimicrobial minimum inhibitory concentration methods, including broth microdilution, broth dilution, agar dilution, and automated dilution methods. The key question of the assessment was: “Is the Microscopic Imaging-Based Automated Antimicrobial Susceptibility Test clinically safe and effective for determining antimicrobial susceptibility and resistance in patients suspected of infection?” The target population was patients suspected of infection; the reference standard/comparator tests were disk diffusion and antimicrobial minimum inhibitory concentration methods; and the outcome measures were adverse events related to testing for safety, and effects on clinical outcomes, agreement with the reference standard/comparator tests, turnaround time, and test failure rate for effectiveness. A systematic literature search was conducted using three international databases (Ovid MEDLINE, Ovid EMBASE, and the Cochrane Central Register of Controlled Trials) and two domestic databases (KoreaMed and RISS) based on the above key question. Study selection and risk-of-bias assessment using the inclusion and exclusion criteria were independently performed by two evaluators, and disagreements were resolved through discussion with a third party. Risk of bias was assessed using the Risk of Bias tool for randomized controlled trials (RoB) for three selected randomized controlled trials (RCTs), and using the Risk of Bias Assessment tool for Non-randomized Studies 2.0 (ROBANS 2.0) for six non-randomized controlled trials (nRCTs) and five cohort studies. Because the reporting methods for agreement, the timing of outcome measurement for turnaround time, and the study design of antimicrobial stewardship programs (ASPs) for clinical outcomes varied, the data were synthesized qualitatively. This assessment was finalized after deliberation by the Health Reassessment Expert Committee based on the review results of the Subcommittee.

Results A total of 64 studies were included in the assessment of this technology, comprising 49 diagnostic cohort studies, 3 RCTs, 6 nRCTs, and 6 cohort studies. No economic evaluation studies were identified. Risk-of-bias assessment showed a high proportion of “unclear” risk in RCTs due to insufficient reporting of allocation concealment and blinding of outcome assessment, while “high” risk of bias was observed in domains related to lack of blinding of participants and investigators, incomplete outcome data, and study support. In nRCTs and cohort studies, many studies were judged to have “high” or “unclear” risk of bias due to limited comparability between groups and inadequate control of confounding variables, and reporting on blinding of outcome assessors was generally insufficient.

Safety No studies reported the safety of the Microscopic Imaging-Based Automated Antimicrobial Susceptibility Test. As this technology is an in vitro test performed on positive bacterial isolates obtained by culturing specimens collected from patients, it poses no direct harm to the patient during testing and was therefore considered safe.

Effectiveness The effectiveness of the Microscopic Imaging-Based Automated Antimicrobial Susceptibility Test was evaluated based on effects on clinical outcomes, agreement with the reference standard/comparator tests, turnaround time, and test failure rate. Effects on clinical outcomes were classified into antimicrobial-related outcomes and clinical outcomes and presented according to whether an antimicrobial stewardship program (ASP) was implemented. Among antimicrobial-related outcomes, all seven studies, including five nRCTs conducted with ASP, showed that the intervention group had a significantly shorter time to optimal therapy. Time to first antibiotic change was significantly shorter in the intervention group in two studies (one RCT with ASP and one nRCT with ASP) (p=0.01, 0.002). For other timing of antimicrobial change, there was no significant between-group difference or heterogeneous results. Regarding antimicrobial use outcomes, achievement of optimal therapy was significantly higher in the intervention group in one RCT with ASP and one nRCT with ASP, and in one RCT without ASP the intervention group also showed a significantly higher rate (p=0.004), whereas one cohort study showed no significant between-group difference (p=0.454). Outcomes such as days of broad-spectrum gram-positive/gram-negative coverage, duration and amount of broad-spectrum antimicrobial use, total days and amount of antimicrobial therapy, duration and amount of narrow-spectrum beta-lactam use, glycopeptide use, and intravenous-to-oral switch rate were reported as significant only in a small number of studies (1–2 studies) or heterogeneous. Among clinical outcomes, mortality-related outcomes were reported in a total of 10 studies (2 RCTs, 5 nRCTs, and 3 cohort studies), and no significant between-group differences were found for 30-day mortality, in-hospital mortality, hospital mortality, bacteremia-related mortality, or infection-related mortality. Other outcomes related to length of stay, infection-related complications, and other outcomes were heterogeneous or showed no significant between-group differences. For gram-positive bacteria, agreement with the reference standard test showed CA 94–99%, VME 0–17.7%, ME 0–2.5%, mE 0–4%, and EA 96–98.7%; agreement with the comparator test showed CA 92–97%, VME 1–2%, ME 3–11.4%, mE 0–4.5%, and EA 96–97%. For gram-negative bacteria, agreement with the reference standard test showed CA 75–98%, VME 0–13.1%, ME 0–4.3%, mE 0–24%, and EA 92–98%; agreement with the comparator test showed CA 89–97%, VME 0–12%, ME 0–4.2%, mE 0–7.4%, and EA 90–98%.

Conclusion and Recommendation Based on the results of the assessment, the Subcommittee for Health Technology Reassessment presented the following opinion. This technology is an in vitro test using patient specimens and does not pose direct harm to the patient during the testing process; therefore, it was judged to have no safety issues. In effectiveness, the time to optimal therapy was significantly reduced; however, evidence for clinical benefits such as mortality reduction and shorter length of stay was considered limited. The agreement with the reference standard/comparator tests was generally at an acceptable level, considering that the test directly examines positive blood culture broth. Accordingly, at present, it was considered appropriate to use this technology as an adjunctive additional test rather than as a substitute for the reference standard/comparator tests. It was also noted that, because the test does not require an isolation culture process, it can shorten turnaround time and may contribute to the establishment of early treatment strategies in critically ill and septic patients. Furthermore, when combined with an antimicrobial stewardship program, it may help optimize antimicrobial use through rapid clinical implementation. The 2nd Health Reassessment Expert Committee of 2025 (October 17, 2025) deliberated based on the Subcommittee’s findings as follows. Considering the evidence on clinical safety and effectiveness, along with other evaluation criteria, the Health Reassessment Expert Committee decided to issue a “weak recommendation” for this test as an adjunctive test that can be used to determine antimicrobial susceptibility and resistance in patients suspected of infection in the domestic clinical setting. Following the revision of the Regulations on New Health Technology Assessment (Ministry of Health and Welfare Ordinance No. 1098, partially revised and effective September 7, 2025), Health Reassessment Expert Committee was established and has been in operation since September 2025.
