인터루킨-6 [정밀면역검사]_감염 및 패혈증 초기진단

Abstract

평가배경 인터루킨-6(interleukin-6, IL-6) [정밀면역검사]는 IL-6를 정량적으로 측정하여 감염, 패혈증의 초기 진단뿐만 아니라 종양 및 염증성 질환의 활동성 평가에 활용되는 검사로, 2001년 ‘인터루킨-6’라는 명칭으로 비급여 항목(노476)으로 등재되었다. 이후 2016년에는 본인부담률 차등적용(80%) 항목으로 전환되었고, 2018년에는 현재의 명칭과 분류체계로 변경되었으며, 2021년 12월에는 선별급여(본인부담률 90%)로 조정되었다. 본 기술은 2026년 적합성 평가가 예정된 항목으로, 수요조사와 관련 검토 절차를 통해 재평가 필요성이 인정되어 재평가 대상으로 선정되었으며, 2025년 3월 14일 개최된 제3차 의료기술재평가위원회에서 평가계획서 및 소위원회 구성을 심의·확정하였다.

「요양기관업무포털(건강보험심사평가원)」의 ‘행위설명’에 따르면, 본 검사는 감염·패혈증의 초기 진단 또는 IL-6 상승이 동반되는 종양·염증성 질환의 활동성 평가에 활용되는 것으로 정의된다. 이에 따라 본 평가는 두 임상적 목적을 구분하여 검토하였으며, 본 보고서에는 감염·패혈증의 초기 진단을 대상으로 한 평가결과를 제시하였다.

평가목적 본 평가는 IL-6 [정밀면역검사]가 감염이나 패혈증 초기 진단에 있어 임상적 효과성에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다.

평가 방법 본 평가는 IL-6 검사의 적응증이 건강보험심사평가원 행위설명에 제시된 정의 및 적응증에서 ‘감염이나 패혈증의 초기 진단 또는 IL-6가 증가하는 종양이나 염증성 질환의 활동성 평가’로 기술되어 있어, 특정 질환으로 한정하기 어려운 점을 고려하였다. 이에 따라 감염, 패혈증, 종양 및 염증성 질환 전반의 연구 경향을 정해진 기간 내에 포괄적으로 파악하기 위해 체계적 문헌고찰(Systematic review, SR) 대신 체계적 문헌고찰 개관(Overview of SR) 방법을 적용하였다. 다만, 본 평가에서는 이 중 ‘감염이나 패혈증의 초기 진단’에 대한 근거를 중심으로 분석을 수행하였다.

평가방법은 평가목적에 따라 IL-6 [정밀면역검사] 소위원회의 논의를 거쳐 확정하였으며, 소위원회는 감염내과 2인, 진단검사의학과 1인, 호흡기내과 1인, 소아청소년과(호흡기내과) 1인, 류마티스내과 1인, 근거기반의학 1인의 전문가 7인으로 구성하였다.

평가의 핵심질문은 ‘IL-6 [정밀면역검사]는 감염이나 패혈증 초기진단 목적으로 사용하는데 임상적으로 효과적인가?’이다. 비교검사는 감염진단의 경우 C-반응성 단백질(C-reactive protein, CRP), 적혈구침강속도(Erythrocyte sedimentation rate, ESR), 패혈증의 경우 CRP, 프로칼시토닌(Procalcitonin, PCT), 프리셉신을 포함하였다. 참고표준검사는 각 연구에서 감염 진단 및 패혈증에 사용되고 있는 참고표준검사를 제한 없이 포함하였으며, 의료결과는 효과성 측면에서 진단 정확성을 주요 평가 지표로 설정하였다.

연구검색은 핵심질문을 토대로 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials) 및 국내 데이터베이스 2개(KoreaMed, 한국교육학술정보원)에서 검색하였으며(최종검색일 2025. 5.1.), 최신의 연구 동향을 반영하고자 2010년 이후에 출판된 체계적 문헌고찰을 포함하였다. 연구 선정기준 및 배제기준 적용을 통한 연구선택과 비뚤림위험 평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치 시 제3자 합의를 통해 조정하였다. 최종 선택된 연구의 비뚤림위험 평가는 A measurement tool to assess systematic reviews-2 (AMSTAR-2)를 사용하였다. 사전에 정한 자료추출 서식을 활용하여 자료를 추출한 후 질적 검토(qualitative review)를 수행하였다. 감염과 패혈증을 구분하여 검토하되, 원저의 특성에 따라 신생아 패혈증, 성인 패혈증, 인공관절 감염, 그리고 감염과 패혈증을 구분하여 결과를 제시하지 않은 통합 보고 연구의 네 범주로 재분류하여 근거를 통합하였다.감염 및 패혈증을 함께 보고하여 각각의 결과를 구분할 수 없는 연구는 분석만 수행하고 별도의 결론은 도출하지 않았다. 결과는 비교검사 대비 IL-6의 진단적 유용성을 중심으로 제시하고, 필요 시 직접비교 결과를 함께 확인하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다.

평가결과 본 평가에 선택된 체계적 문헌고찰은 총 30편(원저 총 437편, 연구대상자 총 54,957명)으로 대상자 및 진단목적별로 신생아 패혈증 진단 10편, 성인 패혈증 진단 7편, 감염과 패혈증을 통합 보고한 연구 5편, 인공관절감염 진단이 9편이 포함되었다. 비뚤림위험 평가 결과, 제외 문헌의 사유가 불명확하거나, 비뚤림위험 및 출판 비뚤림에 대한 고려가 미흡한 문헌이 다수 확인되었다. 이에 따라 전체적인 근거의 신뢰도는 ‘매우 낮음’으로 평가하였다.

신생아 패혈증 진단과 관련하여 포함된 체계적 문헌고찰은 총 10편(원저 총 161편, 연구대상자 총 20,444명)으로, 이 중 5편은 신생아 조기발현형 패혈증(early-onset sepsis), 1편은 후기발현형 패혈증(late-onset sepsis), 나머지 4편은 조기 및 후기발현형 패혈증이 명확히 구분되지 않은 연구였다. 신생아 조기발현형 패혈증 관련 연구 중 2편은 IL-6와 비교검사(PCT, CRP, 프리셉신 등)를 함께 분석하였고, 나머지 3편은 IL-6 단독 검사 결과를 보고하였다.

신생아 조기발현형 패혈증 진단에서 IL-6의 진단정확성을 비교검사와 함께 평가한 2편의 연구에서는, IL-6가 CRP보다 높은 민감도를 보였으며, PCT와는 민감도가 유사하거나 PCT보다 다소 높은 수준, 프리셉신보다는 다소 낮은 정확성을 나타냈다. 그 외 신생아 조기발현형 패혈증 진단 목적으로 IL-6 검사 결과만을 보고한 연구는 총 3편으로, 이들 연구의 통합 민감도는 0.70~0.80, 통합 특이도는 0.70~0.80 수준으로 나타났다.

신생아 후기발현형 패혈증 진단은 IL-6 단독 검사 결과를 제시한 1편의 연구에서는, IL-6의 민감도 0.88, 특이도 0.78로 비교적 높은 진단정확성이 보고되었으나, 패혈증 의심 이후 시간이 경과함에 따라 민감도가 점차 감소하는 경향이 나타났다.

성인 패혈증 진단과 관련하여 포함된 체계적 문헌고찰은 총 7편(원저 총 128편, 연구대상자 총 20,732명)으로, 이 중 4편은 IL-6와 비교검사(CRP, PCT, 프리셉신)와의 진단정확도를 함께 평가하였고, 나머지 3편은 IL-6 단독결과만을 보고하였다. 비교검사를 함께 평가한 4편의 연구에서 IL-6의 민감도가 CRP와 유사한 수준으로 보고된 경우가 다수였으며(3편), 이 중 동일 연구 내에서 연구 내 직접 비교를 통해 재분석한 1편에서는 IL-6와 CRP의 민감도가 각각 0.76, 0.80으로 유사하게 나타났다. IL-6와 PCT는 대부분의 연구에서 유사하거나 IL-6가 더 높은 민감도를 보였으나(4편), 동일 연구 내 직접 비교를 수행한 2편에서는 IL-6가 PCT보다 낮은 경향을 보였다. IL-6와 프리셉신을 비교한 2편의 연구에서는 모두 프리셉신이 IL-6보다 높은 민감도를 나타냈다. 그 외 성인 패혈증 진단목적으로 IL-6 검사 결과만을 보고한 연구는 총 3편으로, 이들 연구의 통합 민감도는 0.66~0.85, 통합 특이도는 0.62~0.75 수준으로 나타났다.

인공관절감염 진단과 관련하여 포함된 체계적 문헌고찰은 총 9편(원저 총 122편, 연구대상자 총 11,185명 이상)으로, 이 중 5편은 IL-6와 비교검사(CRP, ESR)와의 진단정확도를 함께 평가하였고, 나머지 4편은 IL-6 단독결과만을 보고하였다. 비교검사와 함께 평가한 5편 모두에서 CRP와의 비교가 이루어졌으며, 민감도 범위는 IL-6가 0.52~0.97, CRP가 0.48~0.88로 IL-6의 민감도는 CRP와 유사하거나 더 높은 경향을 보였다. 이 중 1편에서는 동일 연구 내 직접 비교를 통해 IL-6와 CRP의 민감도가 각각 0.87, 0.84로 보고되었다. ESR과의 비교는 4편에서 수행되었으며, 이 중 3편에서는 IL-6(0.86~0.97)의 민감도가 ESR(0.75~0.79)보다 높았고, 1편에서는 두 지표 모두 0.52로 나타났다. 그 외 IL-6 단독 결과만을 보고한 4편의 연구에서는 통합 민감도 0.7~0.87, 통합 특이도 0.8~0.98, 통합 곡선하면적(area under the curve, AUC)는 0.83~0.91로 보고되었다.

결론 및 권고결정 의료기술재평가 소위원회에서는 현재 평가 결과에 근거하여 다음과 같은 의견을 제시하였다.

소위원회는 본 평가가 체계적 문헌고찰 개관 방식으로 수행되었으며, 이에 따른 몇 가지 한계를 제시하였다. 우선, 포함된 문헌의 검색 범위와 선정 기준, 원문헌 간 중복 여부 등에 따라 일부 근거가 누락되었거나 반복 포함되었을 가능성을 배제하기 어렵다는 점을 지적하였다. 특히 인공관절감염을 제외한 일반 감염 진단 관련 문헌은 감염과 패혈증을 통합하여 보고한 경우가 많아 감염 진단만을 독립적으로 평가하는 데 제약이 있었으며, 이에 따라 향후 감염 진단을 명확히 대상으로 한 체계적 문헌고찰이 필요하다는 의견을 제시하였다. 또한 IL-6와 비교검사(CRP, ESR, PCT, 프리셉신)의 진단정확성 비교는 대부분 서로 다른 연구의 결과를 바탕으로 한 간접비교에 근거하고 있어 해석에 주의가 필요하다는 점이 논의되었다. 다만 일부 문헌에서는 2×2 표를 활용하여 동일 연구 내에서 두 검사의 결과를 직접 비교하거나, 해당 연구들만을 모아 통합 분석을 시도하여 결과의 방향성이 일관적인지를 확인할 수 있었다. 아울러 문헌마다 상이한 임계값을 적용한 연구들이 통합되어 보고됨에 따라, 임상 현장에서 바로 활용하기에는 제약이 있으며, 일부 연구에서 특정 임계값을 기준으로 한 분석 결과가 제시되었으나 연구 수가 제한적이어서 이를 일반화하기는 어렵다고 판단하였다.

CRP는 6~8시간 이후에 상승하는 반면, IL-6는 감염이나 염증 발생 후 2~4시간 이내에 상승하는 특징이 있어, 감염 초기 진단에서 상대적으로 유용할 수 있다는 의견이 있었다. 실제로 신생아 조기발현형 패혈증에서는 이러한 특성이 잘 반영되어 IL-6가 CRP보다 높은 민감도를 보였으며, PCT와 유사하거나 더 높은 수준으로 보고되었다. 그러나 후기발현형 패혈증에서는 IL-6의 진단적 유용성이 낮아 관련 비교연구도 제한적으로만 확인되었다.

성인 패혈증의 경우, IL-6는 CRP와 유사하거나 PCT 및 프리셉신보다 낮은 민감도가 보고되었다. 이에 대해 응급실 내원 시 진단과 검체 채취가 지연되는 상황이 많아 IL-6가 이미 최고치를 지난 시점에서 측정되는 경우가 흔하기 때문으로 해석하였다. 반면, 중환자실과 같이 밀접한 모니터링이 가능한 환경에서는 조기 채혈이 가능하여 IL-6가 치료 판단에 유용할 가능성이 있어 추가 평가가 필요하다는 의견이 제시되었다. 따라서 성인 패혈증에서 IL-6의 진단적 가치는 측정 시점과 진료 환경에 따라 달라질 수 있으며, 특히 병원 내 획득 패혈증에서의 활용 가능성에 대한 후속 연구가 필요하다는 의견이었다. 다만 본 평가는 체계적 문헌고찰 개관 방식으로 수행되어 이러한 세부 차이를 충분히 반영하기 어려웠으므로, 향후 심층 문헌고찰을 통한 별도 평가가 필요하다는 점을 제시하였다.

인공관절감염에서는 IL-6가 CRP, ESR에 비해 민감도가 유사한 수준이거나 다소 높은 것으로 보고되었으나, 서로 다른 연구결과를 비교한 간접비교 결과로 해석에 주의가 필요하며, 체계적 문헌고찰 개관 방법의 한계로 인해 감염과 비감염성 염증을 구분하는 진단 정확성을 명확히 확인하기는 어려웠다. 또한 대부분의 연구에서 CRP, ESR 등 기존 비교검사와의 간접비교 결과를 제시하고 있어, 향후에는 감염과 염증을 구분할 수 있는 연구 설계를 기반으로 IL-6와 비교검사의 진단 정확성을 직접 비교하는 연구가 필요하다는 점을 제시하였다.

2025년 제2차 재평가전문위원회*(2025.10.17.)는 임상적 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 대상 질환에 따라 다음과 같이 권고등급을 결정하였다.

i) 신생아 패혈증이 의심되는 환자에서 조기발현형 패혈증 초기진단을 목적으로 하는 IL-6 [정밀면역검사]의 사용을 ‘약하게 권고’한다. ii) 성인 패혈증이 의심되는 환자에서 패혈증 초기진단을 목적으로 하는 IL-6 [정밀면역검사]의 사용은 ‘권고 보류’한다. iii) 인공관절감염이 의심되는 환자에서 감염 초기진단을 목적으로 하는 IL-6 [정밀면역검사]의 사용은 ‘권고 보류’한다.

Background Interleukin-6 (IL-6) [High Quality Immunoassay] is a quantitative assay used not only for the early diagnosis of infection and sepsis, but also for the assessment of disease activity in neoplastic and inflammatory conditions. It was first listed as a non-reimbursed item (No. 476) under the name “Interleukin-6” in 2001. In 2016, it was reclassified as an item subject to a differential patient co-payment rate of 80%, and in 2018, its nomenclature and classification were revised to the current designation. In December 2021, it was converted to a selective benefit item with a patient co-payment rate of 90%. As this technology is scheduled for suitability assessment in 2026, it was selected for reassessment after a demand survey and related review procedures identified the need for reassessment. The evaluation plan and subcommittee composition were reviewed and finalized at the 3rd Health Technology Reassessment Committee meeting held on March 14, 2025. According to the “Procedure Description” in the Healthcare Institution Business Portal of the Health Insurance Review and Assessment Service (HIRA), this test is defined as being used for the early diagnosis of infection or sepsis, or for the assessment of disease activity in neoplastic or inflammatory conditions associated with elevated IL-6 levels. Accordingly, the present assessment considered these two clinical purposes separately, and this report presents the findings for the early diagnosis of infection and sepsis.

Objective This assessment aimed to provide evidence on the clinical effectiveness of IL-6 [High Quality Immunoassay] for the early diagnosis of infection and sepsis and to determine the recommendation grade for this technology.

Methods Because the indication for IL-6 testing, as described in the HIRA procedure definition, includes both the “early diagnosis of infection or sepsis” and the “assessment of disease activity in neoplastic or inflammatory conditions with elevated IL-6,” it was difficult to limit the assessment to a specific disease area. Therefore, rather than conducting a conventional systematic review, we applied an overview of systematic reviews to comprehensively examine research trends across infection, sepsis, neoplastic diseases, and inflammatory conditions within a predefined time frame. However, the present assessment focused specifically on evidence related to the early diagnosis of infection and sepsis. The assessment methodology was finalized through discussion within the IL-6 [High Quality Immunoassay] subcommittee in accordance with the study objective. The subcommittee comprised seven experts: two in infectious diseases, one in laboratory medicine, one in pulmonology, one in pediatric pulmonology, one in rheumatology, and one in evidence-based medicine. The key question of this assessment was: “Is IL-6 [High Quality Immunoassay] clinically effective when used for the early diagnosis of infection or sepsis?” Comparator tests included C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) for infection diagnosis, and CRP, procalcitonin (PCT), and presepsin for sepsis diagnosis. Reference standards used in the included studies for the diagnosis of infection and sepsis were accepted without restriction. Diagnostic accuracy was defined as the primary outcome measure from the perspective of clinical effectiveness. The literature search was conducted based on the key question in three international databases (Ovid MEDLINE, Ovid EMBASE, and the Cochrane Central Register of Controlled Trials) and two domestic databases (KoreaMed and the Korea Education and Research Information Service) (final search date: May 1, 2025). To reflect recent research trends, systematic reviews published from 2010 onward were included. Study selection based on the predefined inclusion and exclusion criteria, as well as risk-of-bias assessment, was independently performed by two reviewers, with disagreements resolved through consultation with a third reviewer. The methodological quality of the final included studies was assessed using AMSTAR 2 (A MeaSurement Tool to Assess systematic Reviews 2). Data were extracted using a pre-specified extraction form and qualitatively synthesized. The evidence was reviewed separately for infection and sepsis, but was reclassified into four categories according to the characteristics of the primary studies: neonatal sepsis, adult sepsis, periprosthetic joint infection (PJI), and combined reports that did not distinguish between infection and sepsis in the presentation of results. Studies reporting infection and sepsis jointly, without separable results, were analyzed descriptively only, and no independent conclusions were drawn from them. The results were presented primarily in terms of the diagnostic utility of IL-6 relative to comparator tests, and direct comparative findings were additionally examined where available. Based on the subcommittee’s review, the final recommendation grade was determined by the Health Technology Reassessment Expert Committee.

Results A total of 30 systematic reviews were included in this assessment, comprising 437 primary studies and 54,957 participants. By target population and diagnostic purpose, these included 10 reviews on neonatal sepsis, 7 on adult sepsis, 5 combined reports on infection and sepsis, and 9 on periprosthetic joint infection. Risk-of-bias assessment identified many reviews with unclear reasons for study exclusion and insufficient consideration of methodological bias and publication bias. Accordingly, the overall certainty of the evidence was judged to be very low. For neonatal sepsis, 10 systematic reviews were included (161 primary studies; 20,444 participants). Of these, 5 addressed early-onset sepsis (EOS), 1 addressed late-onset sepsis (LOS), and the remaining 4 did not clearly distinguish between EOS and LOS. Among studies on neonatal EOS, 2 analyzed IL-6 together with comparator tests (PCT, CRP, and presepsin), whereas the remaining 3 reported results for IL-6 alone. In the two studies that assessed the diagnostic accuracy of IL-6 alongside comparator tests for neonatal EOS, IL-6 showed higher sensitivity than CRP, sensitivity similar to or slightly higher than that of PCT, and somewhat lower accuracy than presepsin. In the remaining three studies that reported IL-6 alone for neonatal EOS, pooled sensitivity ranged from 0.70 to 0.80 and pooled specificity also ranged from 0.70 to 0.80. For neonatal LOS, one study reporting IL-6 alone found relatively high diagnostic accuracy, with a sensitivity of 0.88 and a specificity of 0.78; however, sensitivity tended to decrease over time after clinical suspicion of sepsis. For adult sepsis, 7 systematic reviews were included (128 primary studies; 20,732 participants). Of these, 4 assessed the diagnostic accuracy of IL-6 together with comparator tests (CRP, PCT, and presepsin), whereas 3 reported IL-6 alone. Among the four comparative reviews, most reported that IL-6 had sensitivity comparable to CRP. In one re-analysis based on a direct within-study comparison, the sensitivity of IL-6 and CRP was 0.76 and 0.80, respectively, indicating similar performance. Compared with PCT, IL-6 showed similar or higher sensitivity in most studies, although two direct within-study comparisons suggested that IL-6 tended to be less sensitive than PCT. In the two studies comparing IL-6 with presepsin, presepsin showed higher sensitivity than IL-6 in both. In the three studies reporting IL-6 alone for adult sepsis, pooled sensitivity ranged from 0.66 to 0.85 and pooled specificity from 0.62 to 0.75. For periprosthetic joint infection, 9 systematic reviews were included (122 primary studies; at least 11,185 participants). Of these, 5 assessed IL-6 together with comparator tests (CRP and ESR), whereas 4 reported IL-6 alone. All five comparative reviews included comparisons with CRP. Sensitivity ranged from 0.52 to 0.97 for IL-6 and from 0.48 to 0.88 for CRP, suggesting that IL-6 was generally comparable to or somewhat more sensitive than CRP. One direct within-study comparison reported sensitivities of 0.87 for IL-6 and 0.84 for CRP. Comparisons with ESR were reported in four studies; in three, IL-6 (0.86–0.97) showed higher sensitivity than ESR (0.75–0.79), while in one study both markers had a sensitivity of 0.52. In the four studies reporting IL-6 alone, pooled sensitivity ranged from 0.70 to 0.87, pooled specificity from 0.80 to 0.98, and pooled area under the curve (AUC) from 0.83 to 0.91.

Conclusions and Recommendation Based on the current findings, the Health Technology Reassessment Subcommittee presented the following conclusions. The subcommittee noted several limitations related to the use of an overview of systematic reviews. First, depending on the search scope, eligibility criteria, and overlap among primary studies across the included reviews, it is difficult to exclude the possibility that some evidence may have been omitted or counted more than once. In particular, many studies on general infection diagnosis other than PJI reported infection and sepsis together, which limited the ability to independently assess infection diagnosis alone. Accordingly, the subcommittee suggested that a future systematic review specifically focused on infection diagnosis would be necessary. In addition, comparisons of diagnostic accuracy between IL-6 and comparator tests (CRP, ESR, PCT, and presepsin) were based largely on indirect comparisons across different studies and therefore should be interpreted with caution. However, in some reviews, direct within-study comparisons based on 2 × 2 tables, or pooled analyses restricted to such studies, made it possible to examine whether the direction of the findings was consistent. Furthermore, because studies using different cutoff values were synthesized together, the immediate applicability of the results in clinical practice is limited. Although some studies reported findings based on specific cutoff values, the number of such studies was too small to support generalization. Unlike CRP, which generally rises 6–8 hours after the onset of infection or inflammation, IL-6 increases within 2–4 hours. This feature suggests that IL-6 may have relative value in the early diagnosis of infection. This pattern was particularly evident in neonatal EOS, in which IL-6 showed higher sensitivity than CRP and sensitivity similar to or greater than that of PCT. In contrast, in LOS, the diagnostic utility of IL-6 appeared to be lower, and comparative evidence was limited. In adult sepsis, IL-6 was reported to have sensitivity similar to CRP and lower than that of PCT and presepsin. This was interpreted as reflecting the frequent delay in diagnosis and specimen collection after presentation to the emergency department, such that IL-6 may often be measured after its peak has already passed. In contrast, in settings where close monitoring is possible, such as intensive care units, earlier blood sampling may be feasible, and IL-6 may therefore have potential value in clinical decision-making. Additional evaluation in such settings was considered necessary. Thus, the diagnostic value of IL-6 in adult sepsis may vary according to the timing of measurement and the clinical setting, and further studies are needed, particularly regarding its potential role in hospital-acquired sepsis. Because the present assessment was conducted as an overview of systematic reviews, it was not able to fully capture these nuanced differences, and a separate in-depth systematic review was recommended. For periprosthetic joint infection, IL-6 was reported to have sensitivity comparable to or somewhat higher than that of CRP and ESR; however, these findings should be interpreted cautiously because they were derived largely from indirect comparisons across different studies. In addition, because of the limitations inherent in the overview-of-reviews approach, it was difficult to clearly determine the diagnostic accuracy of IL-6 for distinguishing infection from non-infectious inflammation. As most studies reported only indirect comparisons with conventional comparator tests such as CRP and ESR, future studies using designs that directly compare IL-6 with comparator tests in populations where infection can be differentiated from non-infectious inflammation are needed. The 2nd Health Technology Reassessment Expert Committee* (October 17, 2025), after considering the evidence on clinical effectiveness together with the other evaluation domains, determined the following recommendation grades for each target condition in the Korean clinical context: i) The use of IL-6 [High Quality Immunoassay] for the purpose of early diagnosis of early-onset sepsis in neonates with suspected sepsis is “Weakly recommended.” ii) The use of IL-6 [High Quality Immunoassay] for the purpose of early diagnosis of sepsis in adults with suspected sepsis is “Deferred recommendation.” iii) The use of IL-6 [High Quality Immunoassay] for the purpose of early diagnosis of infection in patients with suspected periprosthetic joint infection is “Deferred recommendation.”
