평가배경

자가 혈소판 풍부 혈장 치료술(Autologous Platelet Rich Plasma Application, PRP)은 보존치료에 반응하지 않는 상과염 환자에서 주관절의 통증 완화와 기능을 개선하기 위해 자가 혈소판 풍부 혈장을 상과염 병변에 주입하는 의료기술로 2019년 신의료기술평가를 통해 신의료기술로 인정되어 2023년 건강보험에 선별급여(본인부담률 90%)로 등재되었다. 동 기술은 2026년 선별급여 적합성 평가시기를 고려하여 유관기관의 요청에 따른 수요조사를 통해 평가대상 안건으로 발굴되었고 2024년 제12차 의료기술재평가위원회(2024.12.13.)에서 재평가 대상으로 선정되어 2025년 제3차 의료기술재평가위원회(2025. 3.14.)에서 재평가 계획서 및 소위원회 구성안을 심의한 후 임상적 안전성 및 효과성을 확인하기 위하여 의료기술재평가를 수행하였다.

평가목적 본 평가의 목적은 상과염 환자에서 주관절의 통증 완화 및 기능 개선을 목적으로 자가 혈소판 풍부 혈장 치료술이 임상적으로 안전하고 효과적인지에 대한 근거를 제공하고 동 기술의 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다.

평가방법 상과염 환자에서 자가 혈소판 풍부 혈장 치료술(이하, PRP)에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “자가 혈소판 풍부 혈장 치료술 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 의료기술재평가위원회에서 결정된 정형외과 2인, 재활의학과 1인, 마취통증의학과 1인, 진단검사의학과 1인, 근거기반의학 1인, 총 6인으로 구성하였다.

본 평가의 대상자는 상과염 환자, 비교시술은 위약 주입술, 스테로이드 주입술, 체외충격파 시술 등의 보존적 치료로 정의하였다. 결과변수로는 안전성과 효과성을 확인하였으며, 안전성은 주사부위 감염를 포함한 중재 관련 주요 이상반응으로 설정하였으며, 효과성은 6개월 시점에서의 통증점수, 기능점수, 조직재생, 환자 만족도, 삶의 질 개선으로 평가하였다. 연구유형은 무작위배정 임상시험연구(Randomized Controlled Trial, RCT)를 평가에 포함하기로 하였다.

연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, EBM Reviews- Cochrane Central Register of Controlled Trials), 국내 2개(KoreaMed, 한국교육학술정보원(Research Information Sharing Service, RISS) 데이터베이스에서 검색하였다(최종검색일 2025. 6. 9.).

최종 선택된 연구는 RCT로 비뚤림위험 평가는 Cochrane의 Risk of Bias (RoB)를 사용하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 GRADE (Grading of Recommendations Assessment, Development and Evaluation) 방법을 이용하여 근거수준을 평가하였다. 사전에 정한 자료추출 서식을 활용하여 자료를 추출한 후, 양적 분석이 가능한 경우에는 메타분석을 하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 의견이 불일치한 경우 평가자 간 합의를 통해 일치된 결과를 도출하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의를 진행한 후 의료기술재평가 권고등급을 결정하였다.

평가결과 체계적 문헌고찰 결과, 최종 선택된 RCT는 총 41편(40개 연구, 중재군 1,244명, 비교군 1,577명)이었다. 연구대상자는 기저시점에서 주관절 부위 통증 및 보존적 치료기간 등을 제한하지 않고 내·외측 상과염 환자를 모두 포함하였으며, PRP 중재술은 PRP 성분을 구분하지 않았으며 주입 용량, 횟수, 방법 등이 다양하였다. 비교중재는 상과염 병변 부위에 위약 주입(Sham 치료), 스테로이드 주입, 기타 보존적 치료를 단독 혹은 복합 적용하는 모든 중재를 포함하였다. 추적관찰시점은 3개월 이내 단기 시점부터 24개월 이상까지 다양하였으나, 본 평가에서는 연구에서 보고하는 시점별 자료추출을 진행하되 주요 결과변수의 비교는 6개월 시점에서 임상적 효과성을 확인하였다.

비뚤림위험의 평가항목은 무작위 순서 생성, 할당 은폐, 참여자 및 의료진의 눈가림, 불완전한 결과자료, 선택적 보고 등이었으며, 이 중 배정순서 은폐는 9편(22.5%), 연구참여자 및 연구자에 대한 눈가림은 29편(72.5%), 임상시험 프로토콜 등록 혹은 IRB 승인에 대한 언급은 9편(22.5%)에서 비뚤림위험을 ‘높음(High)’으로 평가하였으며, 그 외 비뚤림위험 평가의 대부분 항목은 대체로 낮거나 불확실로 평가하였다.

안전성 PRP의 안전성은 무작위배정 비교임상시험 41편 중 총 32편에서 동 기술과 관련된 이상반응을 보고하였으며, 주사부위 감염을 보고한 사례는 한 편도 없었다. 스테로이드 주입술과의 비교에서 PRP 중재군에서 주사부위 통증이 더 많이 발생하는 것으로 제시(6편, Risk Ratio (RR) 2.92, 95% Confidence Interval (CI) 2.16 ~ 3.94, I2 = 0%)되었으나, 일시적 증상으로 수일 내 소실되거나 회복된 사례들을 보고하였다. 주사부위 통증을 포함하여 경미한 국소 발적, 부종 등을 모두 포함하여 모든 이상반응을 포함한 통합결과 추정치는 통계적으로 유의한 차이는 없었다(10편, RR 1.23, 95% CI, 0.48 ~ 3.20, I2 = 50%). 일부 연구에서 PRP 중재군은 대조군에 비해 통증이 심하거나 긴 시간 동안 지속된다고 정성보고를 한 연구도 있었으나, 환자 수 및 지속기간 등을 보고하지 않아 결과를 합성할 수는 없었다.

효과성 상과염 환자에서 PRP 중재군의 효과성에 대한 주요 결과지표는 통증 개선을 확인하기 위해 비교중재 간 6개월 시점에서의 통증점수를 비교하기로 설정하였으며, 그 외 결과지표에 따라, 비교중재별로 메타분석한 결과들을 바탕으로 평가하기로 하였다.

PRP 중재군과 위약 주입술 간 통증점수를 비교한 결과, 9편 중 3편만 6개월 시점의 결과를 제시하여 메타분석이 가능하였는데 통합추정치는 두 군 간 유의한 차이가 없었고, 연구 간 이질성이 높았다(3편, Mean Difference (MD) -1.32, 95% CI –3.88 ~ 1.24, I2 = 92.0%). 또한, 두 군 간 기능점수를 메타분석한 결과도 유의한 차이가 없었고 연구 간 이질성은 높았다(3편, RR -8.71, 95% CI –33.73 ~ 16.31, I2 = 95.0%). 그 외 결과변수인 조직 재생, 환자 만족도, 삶의 질 등의 결과를 제시한 연구는 없었다.

PRP 중재군과 스테로이드 주입술 간 6개월 시점에서의 통증점수를 비교한 결과, PRP 중재군의 통증 점수가 유의하게 더 낮았으나, 연구 간 이질성은 높았다(11편, Standardized mean difference (SMD) -1.54, 95% CI –1.97 ~ -1.10, I2 = 84.3%). 또한, 효과성 결과치에 대한 과대 추정 여부와 근거수준을 고려하고자 IRB 승인 여부를 구분하여 하위분석을 진행하였으며, IRB 승인을 제시한 연구로 한정한 메타분석 결과도 스테로이드 주입술에 비해 PRP 중재군의 통증 점수는 유의하게 낮았다(7편, SMD -1.33, 95% CI –1.89 ~ -0.77, I2 = 87.4%). 이에 더불어 IRB 승인을 제시한 연구에서 3개월 이상의 만성 상과염 환자에서 PRP 중재군과 스테로이드 주입술 간 6개월 시점에서의 통증점수를 비교한 결과, 중재군의 통증점수가 유의하게 낮았으나, 연구 간 이질성은 높았다(5편, SMD -1.28, 95% CI –1.71 ~ -0.84, I2 = 71.5%). 그 외 결과지표로서 6개월 시점의 기능점수를 메타분석한 결과도 연구 간 이질성은 높았으나 스테로이드 주입술에 비해 PRP 중재군이 유의하게 더 낮아 통합추정치의 결과가 유사한 경향을 보였다(12편, SMD -1.22, 95% CI –3.98 ~ -0.50, I2=95.4%), IRB 승인 여부에 따른 하위분석에서도 IRB 승인을 제시한 연구에서 기능점수의 통합추정치 결과도 유의하게 낮았다(8편, SMD –0.56, 95% CI –1.04~-0.09, I2=86.3%). 또한, IRB 승인을 제시한 연구에서 3개월 이상의 만성 상과염 환자에서 PRP 중재군과 스테로이드 주입술 간 비교한 메타분석 결과도 PRP 중재군의 기능점수가 유의하게 더 낮아 일관된 결과를 보였다(5편, SMD -0.78, 95% CI –1.52~-0.04, I2=61.5%). 기타 결과변수인 조직 재생, 환자만족도, 삶의 질은 각 1편씩으로 이에 대한 문헌적 근거는 충분하지 않았다.

PRP 중재군과 보존적 치료군 간 6개월 시점에서의 통증점수를 비교한 결과는 1편이었으며. PRP 중재군의 통증점수가 유의하게 더 낮았다(1편, p<0.001). PRP 중재군과 보존적 치료 혹은 국소 마취제 주입술을 시행한 비교군의 경우, 6개월 시점에서의 기능점수를 비교한 결과는 2편이었으며, 체외충격파 치료술 및 운동치료를 동반한 보존적 치료한 비교군과 비교한 1편에서 PRP 중재군과 비교군보다 기능점수가 유의하게 좋았으며(p=0.007), 국소마취제를 사용한 보존적 치료군 간 비교결과도 PRP 중재군의 기능점수가 유의하게 좋았다(p<0.001). 그 외 연구에서는 6개월 시점에서 통증점수, 기능점수를 비교한 연구결과는 없었다.

결론 및 권고결정 의료기술재평가 소위원회에서는 상과염 환자에서 PRP의 안전성과 효과성에 대한 결과를 현재 확인된 근거를 바탕으로 다음과 같이 제시하였다.

소위원회는 상과염에서 PRP의 안전성은 시술 부위 감염사례를 보고한 연구가 없었고, 손상 부위에 주사를 주입하는 과정에서 통증 발현은 예상되는 경미한 수준의 이상반응으로, 안전성에 대한 우려는 낮다고 판단하였다. 또한, 효과성은 6개월 시점에서의 통증 및 기능점수는 표준치료법인 스테로이드 주입술보다 우수하였고, 만성 환자에서도 유사한 결과가 확인되었으나 효과성에 대한 통합추정치가 크지 않고 연구 간 이질성이 높았다. 다만, 임상진료지침에서 언급한 주요 지표들과의 결과 방향성이 일관되었고, 상과염 치료결과를 확인하기 위한 객관적 임상 진단법이 부재한 상황에서 환자 중심성 결과지표로서 타당성이 확보된 측정도구들(Visual Analogue Scale, Disabilities of the Arm, Shoulder and Hand 등)을 활용하였으므로 신뢰할 만하다고 판단하였다. 이에, 보존적 치료에 반응하지 않는 상과염 환자에서 통증 완화와 기능 개선을 목적으로 PRP는 임상에서 사용할 만한 치료옵션 중 하나라고 판단하였다.

소위원회에서는 동 기술에 대한 제언을 위해 다음의 사항들을 고려하였다.

소위원에서는 만성 상과염 환자에서 요양급여 세부 인정기준에 부합하는 PRP 적용대상자가 많은 편은 아니므로 실제 사용량이 과도하게 증가하지는 않을 것으로 예상하였으며, 스테로이드제나 다른 대체 치료법에 비해 PRP 주입을 위한 제반 과정이나 시술 적용 소요시간, 의료자원, 치료재료대, 행위 수가 등을 감안할 때, 동 기술에 대한 의료인의 선호도가 낮아 임상적 유용성을 판단하기에는 한계가 있다는 의견을 제시하였다.

2025년 제2차 재평가전문위원회(2025. 10. 17.)에서는 소위원회 결론 및 분과 의견을 검토하여 ‘상과염 환자에서의 자가 혈소판 풍부 혈장 치료술’에 대해 다음과 같이 심의 의결하고 의료기술재평가 권고등급을 결정하였다.

재평가전문위원회*는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 보존치료에 반응하지 않는 상과염 환자를 대상으로 통증 및 기능 개선을 위해 자가 혈소판 풍부 혈장 치료술을 ‘약하게 권고함’으로 결정하였다. 또한, 재평가전문위원회에서는 건강보험 요양급여의 적용기준 세부인정사항 중 ‘6개월 간격으로 시행하여야 하며’라는 문구를 ‘6개월 간격으로 시행할 수 있으며’로 변경하여, 환자 상태에 따라 추가 시행이 가능하도록 하는 방안을 제안하였다.

Background Autologous platelet-rich plasma (PRP) injection is a medical technology that delivers a patient’s own platelet-rich plasma into epicondylar lesions in order to relieve elbow pain and improve function in patients with epicondylitis who are unresponsive to conservative care. In the Republic of Korea, this technology was recognised as a new health technology through the new Health Technology Assessment (nHTA) in 2019 and was subsequently listed in the National Health Insurance benefits scheme as a selective benefit with a 90% patient co-payment in 2023. In view of the scheduled appropriateness review of selective benefits in 2026, the technology was identified as a candidate for reassessment through a stakeholder demand survey requested by relevant agencies. It was selected as a reassessment subject at the 12th Committee on Health Technology Reassessment (13 December 2024). At the 3rd Committee on Health Technology Reassessment (14 March 2025), the reassessment protocol and the composition of the subcommittee were reviewed and approved, after which a formal health technology reassessment was conducted to confirm the clinical safety and effectiveness of the technology.

Objective The objective of this reassessment was to provide evidence on whether autologous PRP injection is clinically safe and effective for the relief of elbow pain and the improvement of elbow function in patients with epicondylitis, and to determine the recommendation grade of this technology under the Korean Health Technology Reassessment framework.

Methods A systematic review was undertaken to evaluate the safety and effectiveness of autologous PRP injection (hereinafter, “PRP”) in patients with epicondylitis. All methodological aspects were finalised through deliberation of the Subcommittee on Autologous PRP Injection (hereinafter, “the Subcommittee”), the constitution of which was determined by the Committee on Health Technology Reassessment. The Subcommittee comprised six experts: two orthopaedic surgeons, one rehabilitation medicine specialist, one anaesthesiology and pain medicine specialist, one laboratory medicine specialist, and one methodologist in evidence-based medicine.

Population, Intervention, Comparator, Outcomes, and Study design (PICO-SD). The target population was patients diagnosed with epicondylitis. The comparators were defined as conservative treatments, including sham (placebo) injection, corticosteroid injection, and extracorporeal shockwave therapy (ESWT). Outcomes covered both safety and effectiveness. Safety was operationalised as serious intervention-related adverse events including injection-site infection. Effectiveness was assessed using the following outcomes measured at six months: pain score, functional score, tissue regeneration, patient satisfaction, and quality of life. Only randomised controlled trials (RCTs) were eligible for inclusion.

Information sources and search. Based on the key questions, three international databases (Ovid-MEDLINE, Ovid-EMBASE, and EBM Reviews – Cochrane Central Register of Controlled Trials) and two domestic databases (KoreaMed and the Research Information Sharing Service [RISS] of the Korea Education and Research Information Service) were searched. The final search was conducted on 9 June 2025.

Risk of bias and certainty of evidence. The methodological quality of the included RCTs was appraised with the Cochrane Risk of Bias (RoB) tool. The certainty of the body of evidence from the systematic review was rated using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) framework. Data were extracted using a pre-specified data-extraction form, and meta-analyses were performed where pooling was feasible. All steps were undertaken independently by two reviewers, with discrepancies resolved by consensus. The Subcommittee’s findings were reviewed and finalised by the Specialist Committee on Health Technology Reassessment, which deliberated and determined the final recommendation grade.

Results A total of 41 RCTs (40 unique studies; 1,244 patients in the intervention arms and 1,577 patients in the comparator arms) were included in the systematic review. Eligibility was not restricted by baseline pain severity at the elbow or by duration of prior conservative therapy; both medial and lateral epicondylitis populations were included. PRP interventions were heterogeneous: studies did not standardise the PRP composition, and the injection volume, frequency, and technique varied. Comparator interventions included sham (placebo) injection, corticosteroid injection, and other conservative treatments delivered as monotherapy or in combination. Follow-up time points ranged from short-term (≤3 months) to ≥24 months. Although outcome data were extracted at every time point reported, the principal effectiveness comparison was conducted at six months.

Risk of bias. Risk of bias was appraised across the standard Cochrane domains: random sequence generation, allocation concealment, blinding of participants and personnel, incomplete outcome data, and selective reporting. Allocation concealment was rated as ‘high risk’ in 9 trials (22.5%); blinding of participants and personnel was rated ‘high risk’ in 29 trials (72.5%); and absence of trial registration or institutional review board (IRB) approval was identified in 9 trials (22.5%). The remaining domains were generally rated as low or unclear risk of bias.

Safety Of the 41 included RCTs, 32 trials reported on adverse events related to the technology. No case of injection-site infection was reported in any included study. In comparison with corticosteroid injection, injection-site pain was significantly more frequent in the PRP arm (6 trials; risk ratio [RR] 2.92, 95% confidence interval [CI] 2.16–3.94; I2 = 0%); however, this pain was transient and resolved spontaneously within a few days. When injection-site pain was pooled together with minor local erythema, swelling, and other adverse events, the combined estimate showed no statistically significant difference between groups (10 trials; RR 1.23, 95% CI 0.48–3.20; I2 = 50%). A small number of studies qualitatively reported greater intensity or longer duration of pain in the PRP arm, but the absence of patient counts and duration data precluded quantitative synthesis.

Effectiveness The principal indicator of effectiveness was the between-arm difference in pain score at the six-month time point. Other outcomes were assessed through meta-analyses stratified by the type of comparator.

PRP versus sham (placebo) injection. Of the nine eligible trials, only three reported six-month outcomes amenable to meta-analysis. The pooled estimate showed no statistically significant difference in pain score between the two groups, with substantial between-study heterogeneity (3 trials; mean difference [MD] −1.32, 95% CI −3.88 to 1.24; I2 = 92.0%). The pooled functional score also did not differ significantly between groups and was associated with high heterogeneity (3 trials; RR −8.71, 95% CI −33.73 to 16.31; I2 = 95.0%). No included study reported on tissue regeneration, patient satisfaction, or quality of life.

PRP versus corticosteroid injection. In the comparison of pain scores at six months, PRP was associated with a significantly lower pain score, although heterogeneity was high (11 trials; standardised mean difference [SMD] −1.54, 95% CI −1.97 to −1.10; I2 = 84.3%). To explore possible overestimation of effect and to inform certainty of evidence, a subgroup analysis restricted to trials with documented IRB approval was performed; the direction and significance of the result were preserved (7 trials; SMD −1.33, 95% CI −1.89 to −0.77; I2 = 87.4%). Among trials with IRB approval that enrolled patients with chronic epicondylitis (≥3 months’ symptom duration), PRP also yielded a significantly lower pain score at six months, although heterogeneity remained high (5 trials; SMD −1.28, 95% CI −1.71 to −0.84; I2 = 71.5%). Functional scores at six months showed a consistent direction of effect: the PRP arm had a significantly lower (i.e. more favourable) functional score than the corticosteroid arm (12 trials; SMD −1.22, 95% CI −3.98 to −0.50; I2 = 95.4%). When restricted to trials with IRB approval, the pooled estimate remained significant (8 trials; SMD −0.56, 95% CI −1.04 to −0.09; I2 = 86.3%), as did the analysis of IRB-approved trials enrolling chronic patients (5 trials; SMD −0.78, 95% CI −1.52 to −0.04; I2 = 61.5%). Tissue regeneration, patient satisfaction, and quality of life were each reported in only one trial, so the literature base for these outcomes was insufficient.

PRP versus other conservative care. One trial compared pain scores at six months between PRP and conservative treatment, with the PRP arm demonstrating a significantly lower pain score (1 trial; p < 0.001). Two trials reported functional scores at six months between PRP and conservative care or local anaesthetic injection: one trial comparing PRP with combined ESWT and exercise therapy showed significantly better functional scores in the PRP arm (p = 0.007), and one trial comparing PRP with local anaesthetic injection likewise demonstrated significantly better functional scores in the PRP arm (p < 0.001). No additional studies reported pain or functional scores at the six-month time point.

Conclusion and Recommendation Based on the available body of evidence, the Subcommittee summarised its findings on the safety and effectiveness of PRP for epicondylitis as follows. With respect to safety, no included study reported injection-site infection, and the injection-site pain observed during the procedure was considered an expected, mild, and self-limited adverse event, indicating a low level of safety concern. With respect to effectiveness, PRP demonstrated superior pain and functional outcomes at six months compared with the standard of care (corticosteroid injection), with consistent results in patients with chronic disease. The pooled effect estimates were modest in magnitude and accompanied by high between-study heterogeneity. Nonetheless, the direction of effect was concordant with key outcomes recommended in clinical practice guidelines, and—given the absence of an objective diagnostic gold standard for epicondylitis—the use of validated patient-centred measures such as the Visual Analogue Scale (VAS) and the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire was deemed appropriate. Accordingly, PRP was judged to constitute a clinically reasonable treatment option for pain relief and functional improvement in patients with epicondylitis who are unresponsive to conservative therapy. In formulating its advisory remarks, the Subcommittee noted that, given the detailed reimbursement criteria, the eligible chronic epicondylitis population is not large, so a marked increase in real-world utilisation is not anticipated. Considering also the procedural complexity, time required, healthcare resources, consumables, and reimbursement fees relative to corticosteroid injection and other alternatives, clinician preference for PRP appears to be limited, which constrains the assessment of overall clinical utility. At the 2nd Specialist Committee on Health Technology Reassessment (17 October 2025), the Subcommittee’s conclusions and divisional opinions were reviewed and the following decision was made on “Autologous PRP Injection in Patients with Epicondylitis”. Taking into account the totality of the evidence on clinical safety and effectiveness as well as the other appraisal criteria, the Specialist Committee on Health Technology Reassessment* issued a ‘weak recommendation’ in favour of autologous PRP injection for pain relief and functional improvement in patients with epicondylitis who are unresponsive to conservative care, within the Korean clinical context. Furthermore, the Specialist Committee proposed amending the wording in the detailed reimbursement criteria of the National Health Insurance from “shall be administered at intervals of six months” to “may be administered at intervals of six months,” thereby allowing additional administration as clinically warranted by patient status.
