복강내 온열 항암화학요법-난소암 기원 복막암종증

Abstract

평가배경 복강내 온열 항암화학요법(hyperthermic intraperitoneal chemotherapy, HIPEC)은 복막암종증 환자를 대상으로 종양감축술(cytoreductive surgery, CRS)을 시행하여 육안적 병소 제거 후, 미세병소(잔류종양)를 제거하기 위해 고온의 항암제를 일정 시간 동안 복강내에 관류한 후 배액하는 치료이다. 동 기술은 2013년 신의료기술로 인정되어 고시된 이후, 2014년 사용 대상 및 방법의 확대를 반영하여 개정 고시되었으며, 2021년 선별급여 50%로 등재되었다. 이후 선별급여 적합성 평가주기(5년)가 도래되어 유관기관의 수요조사를 통해 발굴되었다. 본 평가는 다양한 대체 의료기술이 존재하는 현시점에서 난소암 기원 복막암종증 환자를 대상으로 기존기술 대비 동 기술의 안전성 및 효과성 등에 대한 최신 근거를 확인하고자, 2025년 제2차 의료기술재평가위원회(2025.2.14.)에서 의료기술의 평가계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다.

평가목적 본 평가의 목적은 난소암 기원 복막암종증 환자에서 종양감축술 후 복강내 온열 항암화학요법(CRS+HIPEC)의 임상적 안전성 및 효과성 등에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다.

평가방법 CRS+HIPEC의 안전성, 효과성 및 경제성을 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “복강내 온열 항암화학요법에 대한 안전성 및 효과성 평가 공동 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 산부인과 1인, 대장항문외과 1인, 위장관외과 1인, 혈액종양내과 2인, 소화기내과 1인, 근거기반의학 1인, 총 7인의 전문가로 구성하였다. 본 평가의 핵심질문은 “난소암 기원 복막암종증 환자에서 CRS+HIPEC은 생존 향상 등의 목적으로 사용 시 임상적으로 안전하고 효과적인가?”이다. 소위원회는 대상환자와 관련하여 건강보험심사평가원 암질환심의위원회(2025.3.18.)에서 고시한 HIPEC 시 허가초과 항암요법의 인정 범위에 따라 5개 복막암(난소암 및 결장·직장암 기원 복막암종증, 충수점액종양 기원 복막가성점액종, 악성 복막중피종, 결합조직형성 소원형세포종양)으로 제한하여 평가하였다. 중재법은 CRS+HIPEC로 정의하였으며, 항암제 종류는 제한하지 않았다. 비교법은 CRS, 전신 항암치료, 완화/보존요법(최선지지요법, 완화수술, 2차 추시개복술)을 포함하였다. 안전성은 수술 관련 부작용 및 합병증, 30일 이내 사망 지표로, 효과성의 경우 1차 의료결과로 전체생존, 무진행생존, 무질병생존, 무재발생존 등의 생존 관련 지표로, 2차 의료결과는 재수술, 삶의 질로 설정하였다. 경제성은 점증적 비용효과비(incremental cost effectiveness ratio, ICER) 등을 지표로 설정하였다. 연구유형은 비교연구 이상의 연구만 평가에 포함하였다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, Cochrane Central Register of Controlled Trials), 국내 2개(KoreaMed, 한국교육학술정보원) 데이터베이스에서 검색하였다(최종검색일 2025.3.28.). 최종 선택된 연구의 비뚤림위험 평가는 연구설계에 따라 무작위배정 비교임상시험의 경우 Cochrane의 Risk of Bias (RoB)를, 그 외 연구는 비무작위 연구 비뚤림위험 평가도구 Risk of Bias for Nonrandomized Studies (RoBANS ver. 2.0)를 사용하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 사전에 정한 서식을 활용해 자료를 추출하고 정량적 합성이 가능한 경우 메타분석을 수행하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) 방법을 이용하여 근거수준을 평가하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다.

평가결과 체계적 문헌고찰 결과, 최종 선택연구는 43편이었다. 결과유형에 따라 안전성 및 효과성을 보고한 연구는 39편, 경제성을 보고한 연구는 4편이었다. 안전성 및 효과성 선택연구(39편)의 특성은 연구유형별로 무작위 비교임상시험 10편, 비무작위 비교연구 29편(전향 코호트 연구 3편, 후향 코호트 연구 19편, 환자대조군 연구 6편, 비동시적 과거대조군 연구 1편)이었다. 대상환자는 대부분이 상피성 난소암이었으며, 초치료 난소암 20편, 재발 난소암 16편, 초치료 및 재발 난소암 혼합 대상 3편이었다. 중재법과 관련하여 CRS는 적용 시점에 따라 난소암 진단 직후 1차 수술(primary cytoreductive/debulking surgery, P(C)DS), 선행항암화학요법 후 간격수술(interval cytoreductive/debulking surgery, I(C)DS), 초치료 이후 재발수술(secondary cytoreductive surgery, SCS)로 다양하게 수행되었다. CRS 이후 종양감축(completeness of cytoreduction, CC) 정도는 완전 종양감축(CC0-1 또는 R0-1) 25편, 불완전 종양감축(CC2-3 또는 R2 이상) 13편, 보고하지 않음 1편이었다. 비교법 유형은 CRS (PDS, IDS, SCS) 37편, 전신 항암치료 3편(2편은 중복 포함)이었다. 비뚤림위험 평가 결과, 전체 난소암 대상으로 RoB는 연구참여자에 대한 눈가림(50.0%), 결과평가에 대한 눈가림(25.5%), 불충분한 결과자료(16.7%) 영역의 비뚤림위험을 ‘높음’으로 평가하였다. RoBANS ver. 2.0은 대상군 비교가능성(24.1%), 대상군 선정(20.7%) 영역의 비뚤림위험을 ‘높음’으로 평가하였다. 경제성 선택연구의 특성은 4편 모두 초치료 난소암 stage III~IV 환자였으며, 이 중 1편은 국내 임상 및 비용자료를 활용하여 한국 보건의료체계 관점에서 수행되었다.

안전성 안전성 결과를 보고한 연구는 총 30편(무작위 비교임상시험 9편, 비무작위 비교연구 21편)이었다. 수술 관련 전체 이상반응을 보고한 연구는 21편이었다. 초치료 난소암(9편)에서 모든 연구는 CRS 단독과 비교하였다. 무작위 비교임상시험은 4편으로, 이 중 3편은 전체 이상반응 발생에서 군 간 차이가 없었다. 나머지 1편은 두 군 모두에서 100% 발생하였다. 비무작위 비교연구는 5편으로, 이 중 3편은 군 간 차이가 없었으며, 나머지 2편은 중재군에서 전체 이상반응 발생률이 높은 경향을 보였다. 메타분석 숲그림을 확인한 결과, 초치료 난소암에서 전체 이상반응 발생 위험은 무작위 비교임상시험과 비무작위 비교연구 모두에서 군 간 차이를 보이지 않았다. 재발 난소암(9편)에서 CRS 단독과 비교한 연구는 8편이었다. 무작위 비교임상시험은 2편이며, 이 중 1편은 전체 이상반응 발생에서 군 간 차이가 없었으며, 나머지 1편은 비교군에서 높은 경향을 보였다. 비무작위 비교연구는 6편이었다. 이 중 2편은 중재군에서 전체 이상반응 발생이 높았으며, 나머지 4편은 군 간 유사한 경향이었다. 메타분석 숲그림을 확인한 결과, 재발 난소암에서 전체 이상반응 발생 위험은 무작위 비교임상시험과 비무작위 비교연구 모두에서 군 간 차이를 보이지 않았다. 전신 항암치료와 비교한 연구는 비무작위 비교연구 1편이며, 중재군에서 전체 이상반응 발생이 낮았다. 초치료 및 재발 난소암 혼합 대상(3편)의 경우, 모두 CRS 단독과 비교하였다. 무작위 비교임상시험 1편과 비무작위 비교연구 2편 모두에서 군 간 차이가 없었다. 수술 관련 중대한 이상반응을 보고한 연구는 20편이었다. 초치료 난소암(11편)에서 모든 연구는 CRS 단독과 비교하였다. 무작위 비교임상시험은 4편으로, 이 중 3편은 군 간 차이가 없었다. 나머지 1편은 중재군에서 중대한 이상반응 발생이 높은 경향을 보였다. 비무작위 비교연구는 7편이었다. 이 중 5편은 군 간 차이가 없었으며, 다른 2편은 중재군에서 높은 경향을 보였다. 메타분석 숲그림을 확인한 결과, 초치료 난소암에서 중대한 이상반응 발생 위험은 무작위 비교임상시험과 비무작위 비교연구 모두에서 중재군이 높은 경향을 보였다. 일부 무작위 비교임상시험에서 보고한 주요 중대한 이상반응을 검토한 결과, 전해질 이상(12-80%), 빈혈(11-57%), 호중구 감소(34%), 감염(9-29%), 백혈구 감소(26%), 설사(21%), 상처벌어짐(15%) 순이었으며, 이 외에도 간효소 상승, 폐합병증, 고혈압(9%), 신부전(7%) 등을 보고하였다. 또한 장누공, 천공, 누출, 장폐색, 복강내 농양, 혈복강은 5% 이하로 보고하였다. 재발 난소암(8편)은 모두 CRS 단독과 비교하였다. 무작위 비교임상시험은 3편으로, 2편은 군 간 차이가 없었다. 나머지 1편은 중재군에서 높은 경향을 보였다. 비무작위 비교연구는 5편으로, 1편에서 군 간 차이가 없었으며, 다른 1편은 중재군에서 중대한 이상반응 발생이 높았다. 나머지 3편은 중재군에서 높은 경향을 보였다. 메타분석 숲그림을 확인한 결과, 재발 난소암에서 중대한 이상반응 발생 위험은 무작위 비교임상시험과 비무작위 비교연구 모두에서 중재군이 높은 경향을 보였다. 일부 무작위 비교임상시험에서 보고한 주요 중대한 이상반응을 검토한 결과, 빈혈(18-22%), 전해질 이상(12-15%)을 가장 빈번하게 보고하였고, 간독성 및 감염(4-9%), 신부전(7%), 장누출(5%)이 그 뒤를 이었다. 그 외 복막 출혈 및 농양, 누공, 출혈, 패혈증, 혈전증, 폐질환 등의 수술 관련 이상반응은 2% 이하로 보고하였다. 초치료 및 재발 난소암 혼합 대상(1편)의 경우, CRS 단독과 비교하였으며 무작위 비교임상시험 1편이었다. 중대한 이상반응 발생은 중재군에서 높은 경향을 보였다. 주요 중대한 이상반응을 검토한 결과, 복강내 농양(5.6%), 장폐색(4.6%), 출혈, 장 누공 또는 천공(3.7%), 복강내 누출, 폐합병증(2.8%) 순이었으며, 패혈증, 신부전, 복강내 출혈은 2% 이하로 보고하였다. 수술 관련 30일 이내 사망을 보고한 연구는 12편이었다. 초치료 난소암(7편)에서 모든 연구는 CRS 단독과 비교하였다. 무작위 비교임상시험은 3편으로, 1편은 두 군에서 사망이 발생하지 않았다. 나머지 2편은 두 군 모두 30일 이내 사망이 3.0% 미만이었다. 비무작위 비교연구는 4편이며, 이 중 3편은 두 군 모두 사망이 발생하지 않았다. 나머지 1편은 사망이 중재군에서 5% 발생하였고, CRS 단독군에서는 발생하지 않았다. 재발 난소암(4편)은 모두 CRS 단독과 비교하였다. 무작위 비교임상시험은 2편이며, 두 군에서 사망이 발생하지 않았다. 비무작위 비교연구는 2편이며, 30일 이내 사망은 중재군에서 발생하지 않았으며, CRS 단독군에서 4% 발생하였다. 초치료 및 재발 난소암 혼합 대상(1편)의 경우, CRS 단독과 비교한 무작위 비교임상시험 1편이었다. 두 군의 사망률은 6.5% 미만으로 군 간 차이가 없어 보였다.

효과성 효과성 결과를 보고한 연구는 총 38편(무작위 비교임상시험 11편, 비무작위 비교연구 27편)이었다. 중앙 전체생존기간(전체사망 위험)을 보고한 연구는 총 32편이었다. 초치료 난소암(14편)의 경우 모두 CRS 단독과 비교하였다. 무작위 비교임상시험은 3편이었다. 중앙 전체생존기간은 모두 중재군에서 좋은 경향을 보였으며, 이 중 1편에서만 군 간 차이가 있었다. 3편을 메타분석한 결과, 전체사망 위험에 대한 군 간 차이는 없었다(hazard ratio, HR=0.74, 95% confidence interval, CI 0.52-1.06, I2=37%, p=0.10). 비무작위 비교연구는 11편이었다. 이 중 5편은 중앙 전체생존기간이 중재군에서 좋았다. 다른 5편은 군 간 차이가 없었으며, 나머지 1편은 군 간 유의성을 보고하지 않았다. 3편을 메타분석한 결과, 중재군에서 전체사망 위험이 낮았다(HR=0.41, 95% CI 0.23-0.75, I2=39%, p=0.004). 재발 난소암(15편)에서 CRS 단독과 비교한 연구는 12편이었다. 무작위 비교임상시험은 3편이며, 2편은 중앙 전체생존기간이 중재군에서 좋았다. 나머지 1편은 CRS 단독군에서 좋았으나, 군 간 차이는 없었다. 2편을 메타분석한 결과, 전체사망 위험에 대한 군 간 차이는 없었다(HR=0.91, 95% CI 0.52-1.61, I2=64%, p=0.75). 비무작위 비교연구는 9편이며, 이 중 4편은 중앙 전체생존기간(전체사망)이 중재군에서 좋았다. 나머지 5편은 결과의 방향성이 혼재되어 있었다. 3편을 메타분석한 결과, 전체사망 위험이 중재군에서 낮았다(HR=0.52, 95% CI 0.34-0.79, I2=64%, p=0.002). 전신 항암치료 단독과 비교한 연구는 비무작위 비교연구 3편이었다. 이 중 1편은 중앙 전체생존기간이 중재군에서 좋았으며, 다른 1편은 전신 항암치료군에서 좋은 경향을 보였다. 나머지 1편은 전체사망 위험이 중재군에서 낮았다(HR=0.36, 95% CI 0.16-0.77, p=0.012). 초치료 및 재발 난소암 혼합 대상(3편)은 모두 CRS 단독과 비교한 무작위 비교임상시험은 1편으로, 중앙 전체생존기간의 군 간 차이는 없었다. 비무작위 비교연구는 2편이며, 중앙 전체생존기간과 전체사망 위험의 군 간 차이는 없었다. 특정 시점의 전체생존율을 보고한 연구는 22편이었다. 초치료 난소암(8편)의 경우 모두 CRS 단독과 비교하였다. 무작위 비교임상시험은 4편이며, 이 중 1편은 2년 전체생존율이 중재군에서 높았다. 나머지 3편은 5년 전체생존율을 보고하였고, 모두 중재군에서 높은 경향을 보였다. 3편을 메타분석한 결과, 5년 시점에서 사망위험은 중재군에서 낮았다(risk ratio, RR=0.79, 95% CI 0.66-0.95, I2=0%, p=0.03). 비무작위 비교연구는 4편이었다. 4편 중 3편은 특정 시점에서 전체생존율이 중재군에서 높은 경향을 보였으나, 2편에서만 군 간 차이가 있었다. 나머지 1편은 CRS 단독군에서 5년 전체생존율이 높았으나 군 간 차이는 없었다. 3편을 메타분석한 결과, 5년 시점에서 사망위험은 중재군에서 낮았다(RR=0.58, 95% CI 0.40-0.84, I2=0%, p=0.024). 재발 난소암(12편)에서 CRS 단독과 비교한 연구는 10편이었다. 무작위 비교임상시험은 3편으로, 이 중 1편은 3년 전체생존율이 중재군에서 높았다. 나머지 2편은 특정 시점에서 전체생존율의 방향성이 혼재되어 있었다. 비무작위 비교연구는 7편이었다. 이 중 6편은 특정 시점에서 전체생존율이 중재군에서 높았으며, 나머지 1편은 군 간 차이가 없었다. 4편을 메타분석한 결과, 5년 시점에서 사망위험은 중재군에서 낮았다(RR=0.74, 95% CI 0.57-0.97, I2=37%, p=0.04). 전신 항암치료 단독과 비교한 연구는 비무작위 비교연구 2편이었다. 2편 중 1편은 5년 전체생존율이 중재군에서 높았으며, 나머지 1편은 전신 항암치료군에서 높은 경향을 보였다. 초치료 및 재발 난소암 혼합 대상(2편)은 모두 CRS 단독과 비교하였다. 무작위 비교임상시험 1편과 비무작위 비교연구 1편 모두에서 5년 전체생존율은 중재군에서 높은 경향을 보였다. 중앙 무진행생존기간(질병진행 및 사망 위험)을 보고한 연구는 21편이었다. 초치료 난소암(12편)의 경우 모두 CRS 단독과 비교하였다. 무작위 비교임상시험은 3편이며, 이 중 2편은 중앙 무진행생존기간(질병진행 및 사망위험)이 중재군에서 좋았다. 나머지 1편은 중재군에서 좋은 경향을 보였으나 군 간 차이는 없었다. 2편을 메타분석한 결과, 질병진행 및 사망 위험은 군 간 차이가 없었다(HR=0.73, 95% CI 0.53-1.00, I2=54%, p=0.05). 비무작위 연구는 9편이었다. 중앙 무진행생존기간은 3편에서 중재군이 좋았으며, 다른 4편은 중재군에서 좋은 경향을 보였으나 군 간 차이는 없었다. 나머지 2편은 CRS 단독군에서 좋은 경향을 보였으나 군 간 차이는 없었다. 3편을 메타분석한 결과, 질병진행 및 사망 위험은 군 간 차이가 없었다(HR=0.76, 95% CI 0.47-1.24, I2=67%, p=0.27). 재발 난소암(9편)에서 CRS 단독과 비교한 연구는 6편이었다. 무작위 비교임상시험은 3편으로, 이 중 1편은 중앙 무진행생존기간이 중재군에서 좋았다. 나머지 2편은 군 간 차이가 없었다. 3편을 메타분석한 결과, 군 간 차이는 없었다(HR=1.02, 95% CI 0.72-1.44, I2=70%, p=0.91). 비무작위 비교연구는 3편이며, 이 중 2편은 중앙 무진행생존기간이 중재군에서 좋았다. 나머지 1편은 중재군에서 높은 경향을 보였으나 군 간 차이는 없었다. 2편을 메타분석한 결과, 질병진행 및 사망 위험이 중재군에서 낮았으나, 이질성이 커 해석에 주의가 필요하다(HR=0.47, 95% CI 0.26-0.87, I2=90%, p=0.016). 전신 항암치료 단독과 비교한 연구는 비무작위 비교연구 3편이었다. 이 중 1편은 중앙 무진행생존기간이 중재군에서 좋았다. 나머지 2편은 중재군에서 좋은 경향을 보였으나, 군 간 차이가 없거나 유의성을 보고하지 않았다. 특정 시점의 무진행생존율을 보고한 연구는 7편이었다. 초치료 난소암(4편)은 모두 CRS 단독과 비교하였다. 무작위 비교임상시험은 1편으로, 2년 무진행생존율이 중재군에서 높은 경향을 보였으나 군 간 차이는 없었다. 비무작위 비교연구는 3편이었다. 이 중 1편은 3년 무진행생존율이 중재군에서 높았다. 나머지 2편에서 특정 시점에서 무진행생존율은 군 간 차이가 없었다. 재발 난소암(3편)의 경우, CRS 단독과 비교한 무작위 비교임상시험은 2편이었다. 2년 무진행생존율은 연구 간 결과의 방향성이 혼재되어 있었고, 군 간 차이는 없었다. 비무작위 비교연구는 1편으로, 1-3년 무진행생존율은 중재군에서 높은 경향을 보였으나, 군 간 유의성을 보고하지 않았다. 중앙 무질병생존기간(재발 또는 사망 위험)을 보고한 연구는 6편이었다. 초치료 난소암(4편)에서 모든 연구는 CRS 단독과 비교하였다. 무작위 비교임상시험은 1편으로, 중앙 무질병생존기간의 군 간 차이는 없었으나, 재발 또는 사망 위험은 중재군에서 낮았다(HR=0.12, 95% CI 0.02-0.89, p=0.038). 비무작위 비교연구는 3편이었다. 3편 모두 중앙 무질병생존기간이 중재군에서 좋은 경향을 보였으나, 군 간 차이는 없었다. 이 중 1편에서는 재발 또는 사망 위험이 중재군에서 낮았다(HR=0.54, 95% CI 0.30-0.97, p=0.041). 재발 난소암(2편)에서 모든 연구는 CRS 단독과 비교한 비무작위 비교연구 2편이었다. 무질병생존기간은 연구 간 결과의 방향성이 혼재되어 있었고, 군 간 차이는 확인되지 않았다. 특정 시점의 무질병생존율을 보고한 연구는 4편이었다. 초치료 난소암(2편)에서 모든 연구는 CRS 단독과 비교하였다. 무작위 비교임상시험은 1편으로, 5년 시점에서 무질병생존율은 중재군에서 높은 경향을 보였으나, 군 간 유의성을 보고하지 않았다. 비무작위 비교연구 1편은 10년 및 5년 무질병생존율을 보고하였고, 모두 중재군에서 높은 경향을 보였으나, 군 간 차이가 없거나 유의성을 보고하지 않았다. 재발 난소암(2편)에서 모든 연구는 CRS 단독과 비교하였고, 비무작위 비교연구 2편이었다. 1편은 2년 무질병생존율을 보고하였고 CRS 단독군에서 높은 경향을 보였으나 군 간 차이는 없었다. 나머지 1편은 3년 및 1년 무질병생존율을 보고하였고, 3년 무질병생존율은 중재군에서 높은 경향을 보였으나 군 간 차이는 없었으며, 1년 무질병생존율은 두 군 간 77%로 동일하였다. 중앙 무재발생존기간을 보고한 연구는 2편이었다. 초치료 난소암(1편)에서 CRS 단독과 비교한 무작위 비교임상시험 1편이었다. 중앙 무재발생존기간은 중재군에서 좋았으며, 재발 위험 또한 중재군에서 낮았다(HR=0.72, 95% CI 0.55-0.894, p=0.015). 초치료 및 재발 난소암 혼합 대상(1편)에서 CRS 단독과 비교한 무작위 비교임상시험 1편이었다. 중앙 무재발생존기간은 중재군에서 좋은 경향을 보였으나, 군 간 차이는 없었다. 특정 시점에서 무재발생존율을 보고한 연구는 1편이었다. 초치료 및 재발 난소암 혼합 대상(1편)으로 CRS 단독과 비교한 무작위 비교임상시험 1편이었다. 3년 무재발생존율은 중재군에서 높은 경향을 보였으나, 군 간 유의성을 보고하지 않았다. 재수술률을 보고한 연구는 8편이었다. 초치료 난소암(3편)에서 CRS 단독과 비교한 연구는 무작위 비교임상시험 1편으로, 두 군 모두 수술 후 합병증으로 인한 재수술 발생이 없었다. 비무작위 비교연구는 2편이었으며, 재수술률은 각 군당 10% 미만으로 발생하였고, 군 간 차이는 확인되지 않았다. 재수술 사유는 문합부 누출, 절제단 부전, 장루괴사 등을 보고하였다. 재발 난소암(4편)에서 CRS 단독과 비교한 연구는 3편이었다. 이 중 무작위 비교임상시험은 1편이며, 재수술률은 두 군 모두 2% 미만으로 발생하였고, 군 간 차이는 없었다. 재수술 사유는 장염전, 상처봉합을 보고하였다. 비무작위 비교연구는 2편이었다. 2편 중 1편은 중재군에서 10.5%, CRS 단독군에서는 발생하지 않았다. 재수술 사유는 Clavien-Dindo 3등급 이상 합병증 발생으로 보고하였다. 나머지 1편에서는 중재군 29%, CRS 단독군 33%이었으며, 군 간 유의성을 보고하지 않았다. 재수술 사유는 3년 이내 발생한 국소 재발로 보고하였다. 전신 항암치료 단독과 비교한 연구는 비무작위 비교연구 1편이었다. 재수술률은 중재군에서 5.3%였으나, 전신 항암치료군에서는 재수술 발생이 없었고, 군 간 차이는 보고하지 않았다. 재수술 사유로는 Clavien-Dindo 3등급 이상 합병증으로 보고하였다. 초치료 및 재발 난소암 혼합 대상(1편)에서 CRS 단독과 비교한 비무작위 비교연구 1편이었다. 재수술률은 중재군 14.3%, CRS 단독군 14.4%로 군 간 차이는 없었다. 재수술 사유로는 수술 후 합병증으로 보고하였다. 삶의 질을 보고한 연구는 4편이었다. 초치료 난소암(3편)에서 모두 CRS 단독과 비교하였다. 모두 무작위 비교임상시험 3편이었다. 이 중 2편은 수술 후 모든 추적관찰 시점에서 세부 평가 항목의 군 간 차이는 없었다. 나머지 1편은 EORTC QLQ-C30의 증상 척도 중 식욕감소(appetite loss) 항목은 중재군에서 좋았으나, 그 차이는 임상적으로 유의하지 않았다. 재발 난소암(1편)에서 CRS 단독과 비교한 무작위 비교임상시험 1편으로, 수술 후 모든 추적관찰 시점에서 군 간 차이는 없었다.

경제성 경제성을 보고한 연구는 총 4편이었다. 모두 초치료 난소암에서 CRS 단독과 비교하였다. 이 중 PDS 단독과 비교한 연구는 1편으로, 중국 다기관 후향적 코호트 연구와 미국 보건의료 비용 자료를 활용하여 미국 보건의료체계 관점에서 분석되었다. PDS 단독군 대비 중재군에서 비용은 높았으나, 중앙 생존년수(life year saved, LYs)가 1.3년 증가하여 점증적 비용효과비(incremental cost effectiveness ratio, ICER)는 7,854$/LYS였다. 잔존종양 정도에 따른 하위군 분석에서 최적감축군(＜1cm)은 LYs가 1.0년으로 유지되어 ICER가 9,789$/LYs였으며, 불완전감축군(≥1cm)에서는 LYs가 0.8년으로 감소하여 ICER가 18,164$/LYs로 높았다. 종합하면, PDS+HIPEC은 전체 환자군에서 비용 효과적인 치료법이며, 잔존종양이 적은 환자군에서 더 비용 효과적일 가능성이 높은 치료로 보고하였다. 다만, 해당 연구는 경제성 분석을 위한 효과 결괏값으로 중국 다기관 후향적 코호트 연구에서 시행된 다회 HIPEC(평균 2.8회) 결과를 활용하였으며, 비용 산출을 위한 분석 모형에서는 미국의 임상 관행을 반영하여 수술 시 1회 HIPEC 투여로 가정하였다. 이에 중재의 실제 시행 방식과 분석 모형의 가정 간에 차이가 있으므로, 결과 해석 시 주의가 필요하다. IDS 단독과 비교한 연구는 3편이었다. 이 중 1편은 국내 무작위 비교임상시험 및 비용 자료를 활용하여 한국 보건의료체계 관점에서 분석되었다. IDS 단독군 대비 중재군에서 비용은 $2,599.9 증가하였고, 질보정생존년(quality-adjusted life year, QALY) 또한 3.64 증가하였다. 그 결과, ICER는 약 708$/QALY로 산출되어, IDS+HIPEC은 한국의 지불의사금액(willingness to pay, WTP) 기준(30,496$/QALY) 대비 비용 효과적인 치료로 보고하였다. 다른 1편은 네덜란드 무작위 비교임상시험 및 비용 자료를 활용하여, 네덜란드 사회적 관점에서 분석되었다. IDS 단독군(€70,046) 대비 중재군에서 비용은 €85,791 증가하였고, LYs와 QALY 또한 모두 증가하였다. 그 결과, ICER는 LYs 기준 €15,746, QALY 기준 €28,299로 산출되어, IDS+HIPEC은 네덜란드 WTP 기준(30,496€/QALY) 대비 비용 효과적인 치료로 보고하였다. 나머지 1편은 네덜란드 무작위 비교임상시험 자료와 Medicare 및 문헌, 병원 재무자료 등의 미국 비용 자료를 활용하여, 미국 보험자 관점에서 분석되었다. 중재군의 비용은 $79,954, IDS 단독군은 $78,849로 두 군 간 비용 차이는 크지 않았다. 반면 QALY는 중재군에서 0.45 증가하여, ICER는 2,436$/QALY로 산출되었다. 확률적 민감도 분석결과, IDS+HIPEC은 미국 WTP 50,000$/QALY 기준에서는 91.5%, WTP 100,000$/QALY 기준에서는 94.1% 비용 효과적인 치료로 보고하였다.

결론 및 권고결정 의료기술재평가 소위원회에서는 현재의 평가 결과에 근거하여 다음과 같이 제언하였다. CRS 단독과 비교하여, CRS+HIPEC의 안전성은 초치료 및 재발 난소암 환자를 대상으로 전체 및 중대한 이상반응 발생이 중재군에서 전반적으로 높은 경향을 보였다(근거수준: 중간 ~ 매우 낮음). 중등도 이상의 이상반응은 오심, 피로, 구토, 혈액학적 이상(빈혈, 혈소판감소증) 등 항암제 관련 전신 대사이상이거나, 빈혈, 전해질 이상 등과 같이 암환자의 기저 전신상태에서 기인하는 가역적 합병증이 높은 비중을 차지하였다. 또한 출혈, 장폐색, 마비, 장누공, 문합부 누출, 복막내감염, 흉막삼출, 폐렴, 신손상 등 대규모 장기 절제 후 동반되는 합병증을 보고하였다. 30일 이내 사망률은 6.3% 이하로 CRS 단독과 유사한 경향을 보였다(근거수준: 중간 ~ 매우 낮음). 효과성은 초치료 및 재발 난소암 환자를 대상으로 전체사망 및 질병진행 위험과 5년 전체생존율이 중재군에서 긍정적인 경향을 보였으나, 비무작위 비교연구의 비중이 높아 결과의 불확실성이 존재하였다(근거수준: 낮음 또는 매우 낮음). 무진행생존율, 무질병 및 무재발 생존 관련 결과는 연구 간 통계적 유의성이 일관되지 않았으며(근거수준: 낮음 또는 매우 낮음), 연구 수가 부족하여 전반적인 결과의 방향성을 판단하기에는 근거가 불충분하였다. 그 외 재수술과 삶의 질은 군 간 유사한 경향을 보였다(근거수준: 중간 또는 매우 낮음). 경제성 4편은 초치료 난소암을 대상으로 CRS(PDS/IDS) 단독 대비 중재(PDS/IDS+HIPEC)가 모두 비용 효과적인 치료로 보고하였고, 이 중 1편은 국내 임상 및 비용 자료를 활용하여 한국 보건의료체계 관점에서 수행되었다. 전신 항암치료 단독과 비교하여, CRS+HIPEC의 안전성은 재발 난소암 환자를 대상으로 전체 이상반응 발생이 중재군에서 더 낮은 경향을 보였다(근거수준: 매우 낮음). 효과성은 전체 사망과 5년 전체생존율에 있어 과반수의 연구에서 중재가 좋은 경향을 보였으나, 결과의 방향성을 판단하기에는 근거가 불충분하였다. 그 외 중앙 무진행생존기간은 연구 간 통계적 유의성이 일관되지 않았다(근거수준: 매우 낮음). 소위원회는 광범위한 복부 장기 절제를 동반하는 종양감축술과 함께 고온 항암제 복강관류요법을 병행하는 중재의 특성상, 전신 대사이상(빈혈, 전해질 이상 등), 혈액학적, 소화기계, 감염성, 호흡기계, 요로계 관련 이상반응 발생 위험이 종양감축술 단독 또는 전신 항암치료 단독에 비해 높을 수 있다고 보았다. 이러한 이상반응 및 합병증은 보존적 치료만으로 회복되지 않은 경우, 수혈, 배액관삽입, 내시경적 시술, 투석 등과 같은 적극적인 처치나 중환자실 치료를 필요로 하는 중대한 수준의 합병증으로 진행될 수 있다. 다만, 비교적 높은 빈도로 보고된 중등도 이상의 주요 합병증은 항암제 독성에서 기인하는 혈액학적, 소화기계, 호흡기계 등에서 흔히 발생할 수 있는 이상반응이거나, 광범위한 장기 절제 후 일반적으로 동반될 수 있는 수술 관련 외과적·감염성 합병증으로 판단하였다. 이러한 합병증은 패혈증, 전신성 염증반응, 다장기부전 등으로 악화될 잠재적 위험이 있으나, 이를 HIPEC의 특이적 위해성으로 보기는 어렵다고 판단하였다. 따라서, 중재는 중등도 이상(grade II-IV)의 합병증 위험이 내재된 고강도 치료로서, 위험과 생존 이득 간의 균형적인 측면에서 신중한 적용이 필요하겠다. 다만 적절한 환자 선정과 숙련된 술기, 관리 가능한 전문기관에서 시행될 경우에는 임상적으로 허용가능한 범위 내의 안전성이 있는 치료로 평가하였다.

효과성과 관련하여, 초치료 및 재발 난소암 환자에서 중재는 임상사건 발생을 감소시키고, 장기 생존에 긍정적인 영향을 미칠 일부 가능성이 있는 치료로 보았다. 그러나 중재의 침습적 특성과 질환의 진행 양상으로 인해 무작위배정 비교임상시험 설계가 현실적으로 어렵고, 대부분의 연구가 비무작위 연구에 기반하여 선택편향, 교란 등의 잠재적 비뚤림위험을 내재하고 있어 결과의 불확실성이 존재하는 것으로 판단하였다. 그럼에도 불구하고 난소암으로 인한 복막 전이(stage III~IV)는 진단 시 70~80%가 이미 고위험 가능성이 높고, 치료하지 않으면 대부분 사망하는 치명적 질환인 점, 기존 치료만으로는 예후가 좋지 않은 점들을 고려할 때, 중재는 일정 수준의 합병증 위험을 감수하더라도 생존 이득을 기대할 수 있는 적극적인 치료로 평가하였다. 다만, 모든 환자에서 생존 이득이 보장되는 것은 아니므로, 종양부하(복막암지수), 잔존종양(절제) 정도, 환자의 전신상태 등을 종합적으로 고려하여 적절한 대상자를 선별하는 것이 필수적이다. 아울러, 숙련된 시술자에 의한 술기의 최적화와 수술 후 체계적 관리가 병행될 때, 합병증 위험 대비 생존 이득을 향상시킬 수 있을 것으로 제언하였다. 2025년 제3차 재평가전문위원회*(2025.11.14.)는 소위원회 결론 및 분과의견을 검토하여 다음과 같이 심의·의결하고 권고등급을 결정하였다. 재평가전문위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 난소암 기원 복막암종증 환자 대상 생존 향상 등의 목적으로 복강내 온열 항암화학요법의 사용을 ‘약하게 권고함’으로 결정하였다.

Background Hyperthermic intraperitoneal chemotherapy (HIPEC) is a treatment for patients with peritoneal carcinomatosis in which cytoreductive surgery (CRS) is first performed to remove all visible lesions, followed by the circulation of heated chemotherapeutic agents within the peritoneal cavity for a specified period to eliminate microscopic residual disease, after which the solution is drained. This technology was first recognized and notified as a new health technology in 2013. It was subsequently revised in 2014 to reflect the expansion of its indications and methods of use, and was later listed for conditional reimbursement at a 50% coverage level in 2021. As the five-year reassessment cycle for conditional reimbursement has elapsed, the technology was identified through a demand survey conducted by relevant institutions. In this reassessment, given the availability of various alternative medical technologies, we aimed to review the most recent evidence on the safety and effectiveness of HIPEC compared with existing treatments in patients with peritoneal metastases originating from ovarian cancer. The reassessment was conducted following deliberation on the evaluation plan and the composition of the subcommittee at the 2nd Healthcare Technology Reassessment Committee in 2025 (February 14, 2025).

Objective The objective of this assessment is to provide evidence on the clinical safety and effectiveness of CRS+HIPEC in patients with peritoneal metastases from ovarian cancer, and to determine the recommendation grade for the use of this technology through the healthcare technology reassessment.

Methods A systematic review was conducted to evaluate the safety, effectiveness, and cost-effectiveness of CRS+HIPEC. All methods were determined through deliberation by the Subcommittee in consideration of the assessment objectives. The Subcommittee comprised seven experts in obstetrics and gynecology, colorectal surgery, gastrointestinal surgery, hematology-oncology, gastroenterology, and evidence-based medicine. The key question was whether CRS+HIPEC is clinically safe and effective for improving survival in patients with peritoneal metastases from ovarian cancer. The target population was defined in accordance with the scope of approved off-label anticancer drug use for HIPEC, as specified by the Cancer Review Committee of the Health Insurance Review and Assessment Service (March 18, 2025). It included five types of peritoneal malignancies: peritoneal metastases from ovarian and colorectal cancers, pseudomyxoma peritonei arising from appendiceal mucinous tumors, malignant peritoneal mesothelioma, and desmoplastic small round cell tumor. The intervention was CRS+HIPEC without restriction on chemotherapeutic agents. Comparators included CRS alone, systemic chemotherapy, and palliative/supportive care. Safety outcomes included surgery-related adverse events, complications, and 30-day mortality. Effectiveness outcomes included overall survival, progression-free survival, disease-free survival, and recurrence-free survival as primary outcomes, and reoperation and quality of life as secondary outcomes. Cost-effectiveness was assessed using indicators such as the incremental cost-effectiveness ratio (ICER). Only comparative studies were included. Literature searches were conducted in Ovid-MEDLINE, Ovid-EMBASE, Cochrane CENTRAL, KoreaMed, and KERIS (last search: March 28, 2025). Risk of bias was assessed using the Cochrane RoB tool for randomized controlled trials (RCTs) and RoBANS 2.0 for nonrandomized studies (NRS). Two reviewers independently performed study selection and data extraction using predefined forms, and meta-analysis was conducted where appropriate. The level of evidence was assessed using the GRADE approach. The final recommendation grade was determined by the Healthcare Technology Reassessment Committee based on the Subcommittee’s review.

Results A total of 43 studies were included in the systematic review. Of these, 39 studies reported safety and effectiveness outcomes, and 4 studies reported cost-effectiveness outcomes. Among the 39 studies on safety and effectiveness, 10 were RCTs and 29 were NRS, including 3 prospective cohort studies, 19 retrospective cohort studies, 6 case-control studies, and 1 non-concurrent historical control study. Most patients had epithelial ovarian cancer. Regarding treatment setting, 20 studies included patients with primary ovarian cancer, 16 with recurrent ovarian cancer, and 3 with a mixed population. With respect to the intervention, CRS was performed at different time points: primary cytoreductive/debulking surgery (P(C)DS) at initial diagnosis, interval cytoreductive/debulking surgery (I(C)DS following neoadjuvant chemotherapy), and secondary cytoreductive surgery (SCS) for recurrent disease. The completeness of cytoreduction (CC) after CRS was reported as complete cytoreduction (CC0–1 or R0–1) in 25 studies, incomplete cytoreduction (CC2–3 or ≥R2) in 13 studies, and was not reported in 1 study. Comparators included CRS (PDS, IDS, SCS) in 37 studies and systemic chemotherapy in 3 studies (including 2 overlapping studies). The risk of bias assessment showed that, for RCTs, a high risk of bias was identified in blinding of participants (50.0%), blinding of outcome assessment (25.5%), and incomplete outcome data (16.7%). For NRS assessed using RoBANS ver. 2.0, a high risk of bias was identified in comparability of groups (24.1%) and selection of participants (20.7%). Among the four cost-effectiveness studies, all included patients with stage III–IV primary ovarian cancer. One study was conducted from the perspective of the Korean healthcare system using domestic clinical and cost data.

Safety Safety outcomes were reported in 30 studies (9 RCTs and 21 NRS). Surgery-related overall adverse events were reported in 21 studies. In primary ovarian cancer, studies comparing CRS+HIPEC with CRS alone were 9. Among RCTs (n=4), no between-group differences were observed in three studies, while a 100% incidence was reported in both groups in one study. Among NRS (n=5), no differences were observed in three studies, whereas a higher incidence in the intervention group was reported in two studies. Meta-analysis showed no significant between-group differences in the risk of overall adverse events in both RCTs and NRS. In recurrent ovarian cancer, studies comparing CRS+HIPEC with CRS alone were 8. Among RCTs (n=2), no difference was observed in one study, while a higher incidence in the comparator group was reported in one study. Among NRS (n=6), a higher incidence in the intervention group was reported in two studies, while similar trends between groups were observed in four studies. Meta-analysis indicated no significant between-group differences. Studies comparing systemic chemotherapy were 1 (NRS), in which a lower incidence was reported in the intervention group. In mixed populations of primary and recurrent ovarian cancer, studies were 3, all comparing with CRS alone, and no between-group differences were observed.

Surgery-related serious adverse events (SAEs) were reported in 20 studies. In primary ovarian cancer, studies comparing CRS+HIPEC with CRS alone were 11. Among RCTs (n=4), no between-group differences were observed in three studies, while a higher incidence in the intervention group was reported in one study. Among NRS (n=7), no differences were observed in five studies, whereas a higher incidence in the intervention group was reported in two studies. Meta-analysis indicated a higher risk of SAEs in the intervention group in both RCTs and NRS. Common SAEs reported in some RCTs included electrolyte imbalance (12–80%), anemia (11–57%), neutropenia (34%), infection (9–29%), leukopenia (26%), diarrhea (21%), and wound dehiscence (15%). Other events included elevated liver enzymes, pulmonary complications, hypertension (9%), and renal failure (7%), while gastrointestinal fistula, perforation, leakage, bowel obstruction, intra-abdominal abscess, and hemoperitoneum were reported in ≤5%. In recurrent ovarian cancer, studies comparing CRS+HIPEC with CRS alone were 8. Among RCTs (n=3), no between-group differences were observed in two studies, while a higher incidence in the intervention group was reported in one study. Among NRS (n=5), no difference was observed in one study, a higher incidence in the intervention group was reported in one study, and a higher trend in the intervention group was observed in three studies. Meta-analysis indicated a higher risk in the intervention group. Common SAEs included anemia (18–22%) and electrolyte imbalance (12–15%), followed by hepatotoxicity and infection (4–9%), renal failure (7%), and bowel leakage (5%). Other events, including peritoneal bleeding, abscess, fistula, hemorrhage, sepsis, thrombosis, and pulmonary disease, were reported in ≤2%. In mixed populations, studies were 1 (RCT), in which a higher incidence of SAEs in the intervention group was observed. Frequently reported SAEs included intra-abdominal abscess (5.6%), bowel obstruction (4.6%), bleeding, bowel fistula or perforation (3.7%), intra-abdominal leakage, and pulmonary complications (2.8%), while sepsis, renal failure, and intra-abdominal bleeding were reported in ≤2%.

Surgery-related 30-day mortality was reported in 12 studies. In primary ovarian cancer, studies comparing CRS+HIPEC with CRS alone were 7. Among RCTs (n=3), no deaths in either group were reported in one study, while mortality rates of <3.0% in both groups were reported in two studies. Among NRS (n=4), no deaths in either group were reported in three studies, while a 5% mortality rate in the intervention group and none in the CRS-alone group were reported in one study. In recurrent ovarian cancer, studies comparing CRS+HIPEC with CRS alone were 4. Among RCTs (n=2), no deaths in either group were reported. Among NRS (n=2), no deaths in the intervention group and a 4% mortality rate in the CRS-alone group were reported. In mixed populations, studies were 1 (RCT), in which mortality rates of <6.5% in both groups with no apparent between-group differences were reported.

Effectiveness Effectiveness outcomes were reported in 38 studies (11 RCTs and 27 NRS). Median overall survival, including the risk of death, was reported in 32 studies. In patients with primary ovarian cancer (n=14), all studies compared CRS+HIPEC with CRS alone. Among the RCTs (n=3), all demonstrated a trend toward improved median overall survival in the intervention group, although statistical significance was achieved in only one study. When these trials were pooled, no significant difference in the risk of death was observed (hazard ratio [HR]=0.74, 95% confidence interval [CI] 0.52–1.06, I²=37%, p=0.10). Similarly, among the NRS (n=11), five studies reported longer survival in the intervention group, while five showed no meaningful difference; one study did not report statistical significance. Pooled analysis of three NRS, however, suggested a significantly lower risk of death in the intervention group (HR=0.41, 95% CI 0.23–0.75, I²=39%, p=0.004). In recurrent ovarian cancer (n=15), 12 studies compared CRS+HIPEC with CRS alone. Among the RCTs (n=3), two favored the intervention, whereas the remaining study showed a non-significant trend in favor of CRS alone. Meta-analysis of two RCTs showed no significant difference (HR=0.91, 95% CI 0.52–1.61, I²=64%, p=0.75). Among the NRS (n=9), four studies reported improved survival with CRS+HIPEC, while the remaining studies showed inconsistent findings. Nevertheless, pooled analysis demonstrated a significantly reduced risk of death in the intervention group (HR=0.52, 95% CI 0.34–0.79, I²=64%, p=0.002). When compared with systemic chemotherapy alone (n=3, all NRS), the results were mixed: one study favored CRS+HIPEC, another favored chemotherapy, and the remaining study reported a significantly lower mortality risk in the intervention group (HR=0.36, 95% CI 0.16–0.77, p=0.012). In studies including mixed populations (n=3), no significant differences between groups were observed. Overall survival at predefined time points was reported in a total of 22 studies. In primary ovarian cancer (n=8), all studies compared CRS+HIPEC with CRS alone. Among the RCTs (n=4), one study demonstrated improved 2-year survival, while the remaining three reported higher 5-year survival rates in the intervention group. Meta-analysis confirmed a significantly lower risk of death at 5 years (risk ratio [RR]=0.79, 95% CI 0.66–0.95, I²=0%, p=0.03). Among the NRS (n=4), three studies also favored the intervention group, although statistical significance was observed in only two. Pooled analysis again showed a reduced 5-year mortality risk (RR=0.58, 95% CI 0.40–0.84, I²=0%, p=0.024). In recurrent ovarian cancer (n=12), 10 studies compared CRS+HIPEC with CRS alone. While RCT findings were inconsistent, most NRS (6 of 7) suggested improved survival with CRS+HIPEC. Meta-analysis supported this trend, demonstrating a significantly lower mortality risk at 5 years (RR=0.74, 95% CI 0.57–0.97, I²=37%, p=0.04). Comparisons with systemic chemotherapy (n=2) yielded inconsistent results. Similarly, studies in mixed populations (n=2) showed a consistent but non-significant trend favoring CRS+HIPEC. Median progression-free survival (PFS), including progression or death events, was reported in 21 studies. In primary ovarian cancer (n=12), all studies compared CRS+HIPEC with CRS alone. Among RCTs (n=3), two showed longer PFS in the intervention group, whereas one did not demonstrate a significant difference. Meta-analysis of two RCTs confirmed no statistically significant difference (HR=0.73, 95% CI 0.53–1.00, I²=54%, p=0.05). Among the NRS (n=9), findings were variable: three studies reported improved PFS, four showed favorable trends without significance, and two favored CRS alone. Pooled analysis showed no significant difference (HR=0.76, 95% CI 0.47–1.24, I²=67%, p=0.27). In recurrent ovarian cancer (n=9), six studies compared CRS+HIPEC with CRS alone. RCT results were largely neutral, and meta-analysis confirmed no difference (HR=1.02, 95% CI 0.72–1.44, I²=70%, p=0.91). Among NRS, two studies reported improved PFS with CRS+HIPEC. Although pooled analysis suggested a benefit (HR=0.47, 95% CI 0.26–0.87, p=0.016), the high heterogeneity (I²=90%) warrants cautious interpretation. When compared with systemic chemotherapy alone (n=3, all NRS), one study demonstrated longer median PFS in the intervention group, whereas the remaining two showed a favorable trend without statistical significance or did not report statistical testing. PFS at predefined time points was reported in 7 studies. In primary ovarian cancer (n=4), all studies compared CRS+HIPEC with CRS alone. The single RCT showed a non-significant trend toward improved 2-year PFS in the intervention group. Among the NRS (n=3), one study demonstrated higher 3-year PFS with CRS+HIPEC, whereas the remaining studies found no meaningful between-group differences. In recurrent ovarian cancer (n=3), two RCTs yielded inconsistent findings for 2-year PFS, with no significant differences observed overall. The remaining NRS reported a favorable trend for CRS+HIPEC at 1–3 years, although statistical significance was not assessed. Median disease-free survival (DFS) was reported in 6 studies. In primary ovarian cancer (n=4), all studies compared CRS+HIPEC with CRS alone. The single RCT showed no difference in median DFS; however, the risk of recurrence or death was significantly lower in the intervention group (HR=0.12, 95% CI 0.02–0.89, p=0.038). Among the NRS (n=3), all reported a consistent trend toward longer DFS with CRS+HIPEC, although none demonstrated a statistically significant difference; notably, one study reported a reduced risk of recurrence or death (HR=0.54, 95% CI 0.30–0.97, p=0.041). In recurrent ovarian cancer (n=2), both studies were NRS and showed heterogeneous results, with no clear between-group differences. DFS at predefined time points was reported in 4 studies. In primary ovarian cancer (n=2), all studies compared CRS+HIPEC with CRS alone. The RCT reported a non-significant trend toward improved 5-year DFS in the intervention group. Similarly, the NRS reported higher 5-year and 10-year DFS with CRS+HIPEC, although these differences were not statistically significant. In recurrent ovarian cancer (n=2), both studies were NRS. One study reported higher 2-year DFS with CRS alone, whereas the other showed a non-significant trend favoring CRS+HIPEC at 3 years; 1-year DFS was identical between groups (77%). Median recurrence-free survival (RFS) was reported in 2 studies. In primary ovarian cancer (n=1), the RCT demonstrated longer RFS and a significantly lower risk of recurrence in the intervention group (HR=0.72, 95% CI 0.55–0.894, p=0.015). In a mixed population (n=1), the RCT showed a favorable trend toward longer RFS with CRS+HIPEC, although no significant difference was observed. Only one study reported 3-year RFS, showing a non-significant trend favoring the intervention group. Reoperation rates were reported in 8 studies. In primary ovarian cancer (n=3), no reoperations were observed in the RCT, while the NRS reported low rates (<10%) with no meaningful differences between groups. In recurrent ovarian cancer (n=4), findings were somewhat variable. Although the RCT reported similarly low reoperation rates in both groups, one NRS showed a higher rate in the intervention group, whereas another found comparable rates without statistical significance. Additionally, when compared with systemic chemotherapy, reoperations occurred only in the intervention group, although no formal comparison was provided. In mixed populations, reoperation rates were nearly identical between groups. Quality of life was reported in 4 studies. In primary ovarian cancer (n=3), all RCTs found no differences between groups across follow-up time points. Although one study reported improvement in appetite loss, this was not considered clinically meaningful. Similarly, in recurrent ovarian cancer (n=1), no differences in quality of life were observed at any time point.

Cost-effectiveness Four studies evaluated cost-effectiveness, all of which compared CRS+HIPEC with CRS alone in patients with primary ovarian cancer. When compared with PDS alone, PDS+HIPEC was associated with higher costs but also improved survival outcomes. Specifically, life-years gained increased by 1.3 years, resulting in an ICER of $7,854 per life-year saved (LY). Subgroup analyses further indicated that cost-effectiveness varied according to residual disease. In patients with optimal cytoreduction (<1 cm), life-years gained were maintained at 1.0 year, with an ICER of $9,789/LY, whereas in those with suboptimal cytoreduction (≥1 cm), life-years gained decreased to 0.8 years, yielding a higher ICER of $18,164/LY. Overall, CRS+HIPEC in combination with PDS was considered cost-effective, particularly among patients with minimal residual disease. However, these findings should be interpreted with caution. The effectiveness estimates were derived from a Chinese multicenter retrospective cohort study in which multiple HIPEC sessions were administered (mean 2.8 cycles), whereas the cost model assumed a single intraoperative HIPEC procedure based on US clinical practice. This discrepancy between the observed intervention and the modeled assumptions may limit the validity of the results. When compared with IDS alone, three studies consistently suggested that IDS+HIPEC improved health outcomes with acceptable incremental costs across different healthcare settings. In a study conducted from the Korean healthcare system perspective, IDS+HIPEC increased costs by $2,599.9 but also improved quality-adjusted life years (QALYs) by 3.64, resulting in an ICER of approximately $708/QALY. This was well below the Korean willingness-to-pay (WTP) threshold ($30,496/QALY), indicating that IDS+HIPEC is a cost-effective strategy in this setting. Similarly, a Dutch study adopting a societal perspective found higher costs for IDS+HIPEC (€85,791 vs. €70,046) alongside gains in both life-years and QALYs. The ICERs were €15,746 per life-year gained and €28,299 per QALY, both below the Dutch WTP threshold (€30,496/QALY), supporting the cost-effectiveness of IDS+HIPEC. Finally, a US-based analysis from a payer perspective reported only a modest increase in cost with IDS+HIPEC ($79,954 vs. $78,849), while QALYs improved by 0.45. This corresponded to an ICER of $2,436/QALY. Probabilistic sensitivity analyses further demonstrated that IDSS+HIPEC was cost-effective in 91.5% of simulations at a WTP threshold of $50,000/QALY and in 94.1% at $100,000/QALY.

Conclusion and Recommendation Based on the available evidence, the Subcommittee on Healthcare Technology Reassessment makes the following conclusions. Compared with CRS alone, the addition of HIPEC appears to be associated with a higher incidence of both overall and SAEs in patients with primary and recurrent ovarian cancer (certainty of evidence: moderate to very low). These adverse events were largely driven by systemic toxicities related to chemotherapy—such as nausea, fatigue, vomiting, and hematologic abnormalities—as well as reversible complications reflecting the patients’ underlying clinical condition. Procedure-related complications following extensive CRS were also reported, including gastrointestinal, infectious, pulmonary, and renal events. Importantly, short-term mortality remained low and comparable between groups, with 30-day mortality rates not exceeding 6.3% (certainty of evidence: moderate to very low). With regard to effectiveness, CRS+HIPEC demonstrated a consistent trend toward improved overall survival and reduced risks of death and disease progression across both primary and recurrent settings; however, the predominance of nonrandomized evidence substantially limits the certainty of these findings (certainty of evidence: low to very low). Moreover, outcomes related to progression-free, disease-free, and recurrence-free survival were heterogeneous and lacked consistent statistical significance, making it difficult to draw firm conclusions (certainty of evidence: low to very low). Other clinically relevant outcomes, such as reoperation and quality of life, were broadly similar between groups (certainty of evidence: moderate to very low). Economic evaluations conducted in patients with primary ovarian cancer consistently suggested that CRS+HIPEC is a cost-effective strategy when added to CRS (either PDS or IDS). These findings were observed across different healthcare settings, including an analysis based on Korean clinical and cost data. When compared with systemic chemotherapy alone in recurrent ovarian cancer, CRS+HIPEC may be associated with a lower incidence of overall adverse events, although the certainty of evidence is very low (certainty of evidence: very low). While several studies suggested potential survival benefits, the overall body of evidence remains insufficient and inconsistent to establish a clear direction of effect. Similarly, findings for progression-free survival were variable across studies (certainty of evidence: very low). Given the nature of the intervention—combining extensive cytoreductive surgery with hyperthermic intraperitoneal chemotherapy—the Subcommittee considered that the risk of adverse events is inherently higher than with CRS alone or systemic chemotherapy. In particular, patients undergoing CRS+HIPEC may experience a broad spectrum of complications, including metabolic disturbances (e.g., anemia and electrolyte imbalance), hematologic toxicity, and gastrointestinal, infectious, respiratory, and genitourinary events. While many of these complications are manageable, some may progress to serious conditions requiring intensive interventions, such as blood transfusion, drainage procedures, endoscopic management, renal replacement therapy, or admission to the intensive care unit. Nevertheless, the majority of commonly reported moderate-to-severe complications were judged to be either predictable toxicities of chemotherapy or typical postoperative complications associated with extensive abdominal surgery. Although such events may, in some cases, escalate to severe systemic conditions—such as sepsis, systemic inflammatory response, or multi-organ failure—they were not considered specific to HIPEC itself. Taken together, CRS+HIPEC should be regarded as an intensive treatment modality with an inherent risk of moderate-to-severe (grade II–IV) complications. Accordingly, its use requires careful consideration of the balance between potential risks and anticipated survival benefits. However, when applied in appropriately selected patients and performed in experienced centers with adequate perioperative management, the intervention was considered to have an acceptable safety profile in clinical practice. From an effectiveness perspective, CRS+HIPEC may offer potential benefits in reducing disease progression and improving long-term survival in both primary and recurrent ovarian cancer. However, the invasive nature of the intervention and the clinical course of the disease limit the feasibility of randomized controlled trials, and much of the available evidence is derived from nonrandomized studies. As a result, the risk of bias—particularly selection bias and confounding—remains a concern, and the overall certainty of evidence is limited. Despite these limitations, peritoneal metastasis from ovarian cancer (stage III–IV) represents a highly lethal condition, with a large proportion of patients presenting with advanced disease at diagnosis and poor outcomes with conventional treatment alone. In this context, CRS+HIPEC may be considered an aggressive therapeutic option with the potential to improve survival, even at the cost of increased treatment-related morbidity. Nevertheless, survival benefits are not guaranteed for all patients, underscoring the importance of careful patient selection. Factors such as tumor burden (e.g., peritoneal cancer index), completeness of cytoreduction, and overall patient condition should be taken into account when determining eligibility. Furthermore, optimization of surgical technique by experienced clinicians and structured postoperative care are likely to be critical in maximizing survival benefits while minimizing complications. At the 3rd Healthcare Technology Reassessment Expert Committee in 2025 (November 14, 2025), the Committee reviewed the Subcommittee’s conclusions and expert opinions and reached the following decision. After considering the available evidence on clinical safety, effectiveness, and other relevant factors, the Committee issued a weak recommendation for the use of HIPEC in patients with peritoneal metastases from ovarian cancer, with the aim of improving survival in the Korean clinical context.
