복강내 온열 항암화학요법-기타(악성 복막중피종, 결합조직형성 소원형세포종양)

Abstract

평가배경 복강내 온열 항암화학요법(hyperthermic intraperitoneal chemotherapy, HIPEC)은 복막암종증 환자를 대상으로 종양감축술(cytoreductive surgery, CRS)을 시행하여 육안적 병소 제거 후, 미세병소(잔류종양)를 제거하기 위해 고온의 항암제를 일정 시간 동안 복강내에 관류한 후 배액하는 치료이다. 동 기술은 2013년 신의료기술로 인정되어 고시된 이후, 2014년 사용 대상 및 방법의 확대를 반영하여 개정 고시되었으며, 2021년 선별급여 50%로 등재되었다. 이후 선별급여 적합성 평가주기(5년)가 도래되어 유관기관의 수요조사를 통해 발굴되었다. 본 평가는 다양한 대체 의료기술이 존재하는 현시점에서 악성 복막중피종, 결합조직형성 소원형세포종양 환자를 대상으로 기존기술 대비 동 기술의 안전성 및 효과성 등에 대한 최신 근거를 확인하고자, 2025년 제2차 의료기술재평가위원회(2025.2.14.)에서 의료기술의 평가계획서 및 소위원회 구성안에 대한 심의를 받고 재평가를 수행하였다.

평가목적 본 평가의 목적은 악성 복막중피종, 결합조직형성 소원형세포종양 환자에서 종양감축술 후 복강내 온열 항암화학요법(CRS+HIPEC)의 임상적 안전성 및 효과성 등에 대한 근거를 제공하고 동 기술 사용에 대한 의료기술재평가 권고등급을 결정하기 위함이다.

평가방법 CRS+HIPEC의 안전성 및 효과성 등을 평가하기 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “복강내 온열 항암화학요법에 대한 안전성 및 효과성 평가 공동 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 산부인과 1인, 대장항문외과 1인, 위장관외과 1인, 혈액종양내과 2인, 소화기내과 1인, 근거기반의학 1인, 총 7인의 전문가로 구성하였다. 본 평가의 핵심질문은 “악성 복막중피종, 결합조직형성 소원형세포종양 환자에서 CRS+HIPEC은 생존 향상 등의 목적으로 사용 시 임상적으로 안전하고 효과적인가?”이다. 대상환자는 건강보험심사평가원 암질환심의위원회(2025.3.18.)에서 고시한 HIPEC 시 허가초과 항암요법의 인정 범위에 따라 5개 복막암(난소암 및 결장·직장암 기원 복막암종증, 충수점액종양 기원 복막가성점액종, 악성 복막중피종, 결합조직형성 소원형세포종양)으로 한정하여 평가하였다. 중재법은 CRS+HIPEC로 정의하였으며, 항암제 종류는 제한하지 않았다. 비교법은 CRS, 전신 항암치료, 완화/보존요법(최선지지요법, 완화수술, 2차 추시개복술)을 포함하였다. 안전성은 수술 관련 부작용 및 합병증, 30일 이내 사망 지표로, 효과성의 경우 1차 의료결과로 전체생존, 무진행생존, 무질병생존, 무재발생존 등의 생존 관련 지표로, 2차 의료결과는 재수술, 삶의 질로 설정하였다. 경제성은 점증적 비용효과비(incremental cost effectiveness ratio) 등을 지표로 설정하였다. 연구유형은 제한하지 않았다. 연구문헌은 핵심질문을 토대로 국외 3개(Ovid-MEDLINE, Ovid-EMBASE, Cochrane Central Register of Controlled Trials), 국내 2개(KoreaMed, 한국교육학술정보원) 데이터베이스에서 검색하였다(최종검색일 2025.3.28.). 최종 선택된 연구의 비뚤림위험 평가는 연구설계에 따라 무작위배정 비교임상시험의 경우 Cochrane의 Risk of Bias (RoB)를, 그 외 연구는 비무작위 연구 비뚤림위험 평가도구 Risk of Bias for Nonrandomized Studies (RoBANS ver. 2.0)를 사용하였다. 모든 과정은 2명의 평가자가 독립적으로 수행하였으며 사전에 정한 서식을 활용해 자료를 추출하고 정량적 합성이 가능한 경우 메타분석을 수행하였다. 본 평가에서 수행한 체계적 문헌고찰 결과는 Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) 방법을 이용하여 근거수준을 평가하였다. 본 평가는 소위원회의 검토 결과를 바탕으로 재평가전문위원회에서 최종심의 후 의료기술재평가 권고등급을 결정하였다.

평가결과 악성 복막중피종 악성 복막중피종 환자에서 안전성 및 효과성을 보고한 연구는 43편(비무작위 비교연구 4편, 증례연구 29편, 증례보고 10편)이었으며, 경제성을 보고한 연구는 없었다. 대상환자는 악성 복막중피종의 미만성(diffuse) 유형이 41.9%를 차지하였다. 병리학적 아형은 상피형이 가장 많았으며, 양상형, 육종형 등 고등급 아형이 주를 이루었다. 복막암지수(peritoneal cancer index, PCI) 점수는 10-20점이 14편, 20점 이상 8편이었으나, 다수의 연구에서 보고하지 않았다. 중재 전 상태는 동일 연구 내에서도 환자 상태에 따라 치료 전략이 다양하게 적용되었다. 중재와 관련하여 CRS 후 종양감축(completeness of cytoreduction, CC) 정도는 완전 감축(CC0–1) 13편, 불완전 감축(CC2–3) 22편이었으며, 일부 환자는 중재 이후 보조 항암치료 또는 추가 복강내 항암요법을 병용하였다. 비교법은 CRS 단독, 전신 항암치료, 완화수술이 확인되었다. 비교연구 4편을 대상으로 비뚤림위험을 평가한 결과, 대상군 비교가능성(100.0%), 교란변수(25.0%), 노출 측정(25.0%), 불완전한 결과자료(25.0%) 영역의 비뚤림위험을 ‘높음’으로 평가하였다.

결합조직형성 소원형세포종양 결합조직형성 소원형세포종양 환자에서 안전성 및 효과성을 보고한 연구는 15편(비무작위 비교연구 3편, 증례연구 5편, 증례보고 7편)이었으며, 경제성을 보고한 연구는 없었다. 대상환자는 결합조직형성 소원형세포종양 환자로, 대부분 간·폐·림프절 전이를 동반하였으며 젊은 남성 환자 비중이 높았다. PCI 점수는 10–20점이 일부에서 보고되었으나, 연구별로 보고 방식이 다양하고 다수 연구에서 보고하지 않았다. 중재 전 대부분 선행 항암치료 시행 환자였으며, 일부 환자는 초기 오진 후 병리·유전자 검사(EWSR1-WT1 전좌)로 확진되었다. CRS 후 종양감축 정도는 완전 감축(CC0–1) 8편, 불완전 감축(CC2–3) 4편이었으며, 일부 환자는 중재 이후 보조 항암치료나 방사선치료를 병용하였다. 비교법은 CRS가 확인되었다. 비교연구 3편을 대상으로 비뚤림위험을 평가한 결과, 대상군 비교가능성(100.0%), 교란변수(66.7%) 영역의 비뚤림위험을 ‘높음’으로 평가하였다.

안전성 악성 복막중피종 안전성 결과를 보고한 연구는 단일군연구 26편(증례연구 23편, 증례보고 3편)이었다. 수술 관련 전체 이상반응은 단일군연구 19편(증례연구 16편, 증례보고 3편)에서 보고하였다. 증례연구 16편에서 수술 관련 전체 이상반응 발생은 9.5~65.0%였다. 경증(Grade I-II) 이상반응 발생은 9.5~39.3%였고, 대체로 약 25.0~40.0% 수준에서 보고하였다. 주요 경증 이상반응은 오심, 구토, 발열, 경미한 수준의 췌장염 등이었다. 소아를 대상으로 한 연구에서 이상반응 발생은 71.4%였으며, 주요 이상반응은 급성신손상, 고빌리루빈혈증, 호흡곤란(흉수 동반), 기흉, 빈혈/응고장애 등이 나타났고, 모두 흉관삽입 또는 보존적 치료를 통해 관리되었다. 증례보고 3편에서 전체 이상반응 발생은 0.0~100.0%였다. 1편은 이상반응 발생이 없었으며, 나머지 2편에서는 장루, 골반 내 농양이 발생하였으나, 배액, 항생제 등의 보존적 치료로 회복되었다. 수술 관련 중대한 이상반응을 보고한 연구는 증례연구 17편이며, 8.6~43.2%였다. 주요 중대한 이상반응은 수술 관련(장 누출, 창상 이개, 문합부 누출, 장 천공), 감염 관련(균혈증, 패혈증, 패혈성 쇼크, Clostridium difficile 감염, 농흉, 흉수 동반 감염), 호흡기 관련(흡인성 폐렴, 호흡부전) 및 신장 관련 이상반응(급성 신부전, 신독성)을 보고하였다. 이 외에도 혈액학적 독성, 화학성 또는 담즙 누출에 의한 복막염, 수술 후 사망 사례를 보고하였다. 수술 관련 30일 이내 사망을 보고한 연구는 증례연구 9편이며, 0.0~11.1%였다. 주요 사망 원인으로는 전신 염증반응 증후군 또는 패혈증으로 인한 다장기부전, 폐색전증으로 인한 심인성 쇼크, 출혈 또는 혈소판 응고 장애로 인한 신부전, 장피누공으로 인한 심정지 등을 보고하였다.

결합조직형성 소원형세포종양 안전성 결과를 보고한 연구는 단일군연구 11편(증례연구 4편, 증례보고 7편)이었다. 수술 관련 전체 이상반응은 단일군연구 8편(증례연구 2편, 증례보고 6편)에서 보고하였다. 증례연구 2편에서 전체 이상반응 발생은 각각 19.2%, 62.5%였다. 주요 합병증은 장폐색, 고아밀라아제혈증, 크레아틴 수치 상승, 창상이개, 일시적 위마비 등을 보고하였으며 진공보조장치, 보존적 치료 등으로 관리 및 회복되었다. 증례보고 6편 중 2편은 전체 이상반응 발생이 없었다. 나머지 4편에서 전체 이상반응 발생은 50.0~100.0%였다. 주요 이상반응은 요관 문합술로 인한 소변누출, 고출량 회장루, 급성신손상, 균혈증, 상처감염, 부분적 문합부 파열, 직장주위 농양, 흉막 농흉, 폐색전증, 장요근출혈, 장폐색 등을 보고하였고, 이러한 이상반응은 장기 정맥영양, 혈액투석, 항응고치료, 보존적 치료 등으로 관리 및 회복되었다. 수술 관련 중대한 이상반응을 보고한 연구는 단일군연구 4편(증례연구 3편, 증례보고 1편)이었다. 증례연구 3편 중 1편은 심각한 이상반응이 발생하지 않았다. 나머지 2편에서 각각 23.0%, 84.0%로, 투석이 필요한 Grade Ⅳ 사건이 발생하였다. 증례보고 1편에서 담낭 천공으로 인한 담관 복막염 발생을 보고하였다. 수술 관련 30일 이내 사망을 보고한 연구는 단일군연구 4편이며, 모든 연구에서 사망이 발생하지 않았다.

효과성 악성 복막중피종 효과성 결과를 보고한 연구는 41편(비무작위 비교연구 4편, 증례연구 28편, 증례보고 9편)이었다. 중앙 전체생존기간(전체사망 위험)을 보고한 연구는 28편(비무작위 비교연구 4편, 증례연구 24편)이었다. CRS 단독과 비교한 연구는 3편이었다. 이 중 2편은 중앙 전체생존기간이 중재군에서 좋았으며, 나머지 1편은 군 간 차이가 없었다. 3편을 메타 분석한 결과, 전체사망 위험은 중재군에서 낮았다(hazard ratio, HR=0.53, 95% confidence interval, CI 0.41-0.68, p<0.0001, I2=0%). 전신 항암치료 단독과 비교한 연구는 2편이었다. 2편 모두 중앙 전체생존기간이 중재군에서 좋은 경향을 보였으며, 이 중 1편은 전체사망 위험이 중재군에서 낮았다(HR=0.54, 95% CI 0.43-0.69, p<0.001). 완화수술 단독과 비교한 연구는 1편이며, 중앙 전체생존기간이 중재군에서 좋은 경향을 보였으나, 군 간 유의성을 보고하지 않았다. 중앙 전체생존기간을 보고한 단일군연구는 증례연구 24편이었다. 개별 연구의 추적관찰 기간 동안 중앙값 미도달(not reached)로 보고한 연구 1편을 제외하고, 중앙 전체생존기간은 20~65개월 범위로 보고하였으며, 대부분의 연구에서 중앙 전체생존기간은 약 30~50개월 수준이었다. 특정 시점의 전체생존율을 보고한 연구는 25편(비무작위 비교연구 2편, 증례연구 23편)이었다. CRS 단독과 비교한 연구는 1편이었다. 5년 전체생존율은 중재군에서 높은 경향을 보였으나, 군 간 차이를 보고하지 않았다. 전신 항암치료 단독과 비교한 2편이었다. 2편 모두 특정 시점에서 전체생존율은 중재군에서 높은 경향을 보였으나, 군 간 차이가 없거나 유의성을 보고하지 않았다. 5년 전체생존율을 보고한 2편을 메타분석한 결과, 5년 시점에서 사망 위험은 중재군에서 낮았다(risk ratio, RR=0.62, 95% CI 0.60-0.64, I2=0%, p=0.003). 완화수술 단독과 비교한 연구는 1편이었다. 5년 전체생존율은 중재군에서 좋은 경향을 보였으나, 군 간 차이는 없었다. 증례연구 23편에서 전체생존율은 추적 기간에 따라 다양하게 보고하였다. 10년 전체생존율은 2편의 연구에서 보고하였으며 각각 26.0%와 44.6%였다. 5년 전체생존율은 20편의 연구에서 보고하였고, 28.9~100.0% 범위로 대부분의 연구에서 약 30~50% 수준을 보였다. 3년 전체생존율은 15편의 연구에서 보고하였으며, 33.3~91.8% 범위로 주로 40.0~60.0% 수준이었다. 2년 전체생존율은 3편의 연구에서 57.0~80.0%로 보고하였고, 대체로 약 65% 수준이었다. 1년 전체생존율은 14편의 연구에서 69.3~86.7% 범위로 보고하여 전반적으로 70.0~80.0% 이상을 나타냈다. 중앙 무진행생존기간은 증례연구 3편에서 보고하였고, 13.9~25.1개월이었다. 특정 시점의 무진행생존율을 보고한 연구는 증례연구 4편이었다. 10년 무진행생존율은 1편에서 35.9%로 보고하였다. 5년 무진행생존율은 4편에서 13.0~38.4%로 보고하였다. 3년, 2년, 1년 무진행생존율은 각각 1편으로 26.0%, 32.0%, 53.0%로 보고하였다. 중앙 무질병생존기간은 증례연구 3편에서 보고하였고, 8~17개월이었다. 특정 시점의 무질병생존율은 증례연구 1편에서 보고하였고, 4년 무질병생존율은 38.0%였다. 중앙 무재발생존기간은 증례연구 1편에서 보고하였고, 38개월이었다. 특정 시점에서 무재발생존율은 증례연구 1편에서 보고하였고, 5년 무재발생존율은 34.8%였다. 특정 시점에서 질병의 증거가 없는 상태는 단일군연구 11편에서 보고하였다. 증례연구 2편에서 질병의 증거가 없는 상태는 66.7%, 80.0%였다. 2편 중 1편은 수술 후 8년 이상 장기 시점에서 4명이 무병생존하였고, 15년 시점에서 2명이 사망하였다. 나머지 1편(5명)은 술 후 15개월 시점에서 4명이 무병생존하였고, 8개월 시점에서 1명은 전이가 의심되는 폐결절이 발견되었다. 증례보고는 9편에서 질병의 증거가 없는 상태는 0.0~100.0%로 보고하였다. 9편 중 5편은 수술 후 재발 없이 무병생존 상태를 유지하였으며, 나머지 4편에서는 질병 진행 또는 전이 등으로 사망하였다. 재수술률을 보고한 연구는 증례연구 10편이었다. 재수술 사건이 발생하지 않은 연구 1편을 제외하고, 9편에서 재수술률은 6.1~20.0%였다. 주요 재수술 사유는 재발 4편, 중대한 합병증 2편, 보고하지 않음 3편이었다. 삶의 질을 보고한 연구는 증례연구 1편이었다. 통증 관련 Brief Pain Inventory 점수는 술 전 대비 술 후 모든 측정시점에서 통증이 감소하였고, 최소 및 최대 통증 점수 또한 감소하였다. Functional Assessment of Cancer Therapy+Colon (FACT-C) 점수는 술 후 6개월부터 회복하여 24개월까지 증가 및 유지하였으나 군 간 차이는 없었다. FACT-C의 신체 영역의 경우, 술 전 대비 술 후 3개월 시점 감소하는 경향을 보였다가 6개월 시점 술 전 상태로 회복하여, 이후 시점에서 증가 추세를 유지하는 경향을 보였다. Short Form-36 점수는 신체기능 영역에서 술 전 대비 술 후 3개월 시점 감소하다가 6개월 시점 증가하기 시작하였고, 이후 증가 추세를 유지하였다. 일반건강 영역은 술 전 대비 감소하다가 술 후 6개월부터 회복하였으나 24개월 시점에서 감소하였다. 정서 영역에서는 술 전 대비 술 후 증가 추세를 보이다가 술 후 6개월, 12개월 시점에서 증가하였으나, 24개월 시점에서 술 전으로 감소하였다. 정서적 역할 제한 영역은 술 전 대비 술 후 12개월 시점부터 증가하여, 활동 제한 없이 역할을 수행하였다. 사회 기능 영역의 경우 술 전 대비 술 후 3개월부터 증가하는 추세를 보였으며, 술 후 24개월 시점에서는 이전의 모든 시점과 비교하여 감소하였다. 우울 증상 관련 Center for Epidemiologic Studies Depression Scale 점수는 술 전 대비 술 후 12개월 시점에서 감소하였으나, 24개월 시점에서는 술 전 대비 증가하였다. 추가로, 임상적 의미 있는 우울(≥16) 유병률은 술 전 31%였으나 술 후 증감을 보이다가 24개월 시점에서 37%로 보고하였다.

결합조직형성 소원형세포종양 효과성 결과를 보고한 연구는 15편(비무작위 비교연구 3편, 증례연구 3편, 증례보고 9편)이었다. 중앙 전체생존기간(전체사망 위험)을 보고한 연구는 5편(비무작위 비교연구 2편, 증례연구 3편)이었다.이 중 CRS 단독과 비교한 연구는 2편이었다. 중앙 전체생존기간은 중재군에서 좋은 경향을 보였으나, 군 간 차이가 없거나 유의성을 보고하지 않았다. 증례연구는 3편이며, 중앙 전체생존기간은 44.3~63.1개월이었다. 진단 시점부터 측정되거나 완전 종양 절제일수록 중앙 전체생존기간이 더 좋았다. 특정 시점의 전체생존율을 보고한 연구는 3편(비무작위 비교연구 1편, 증례연구 3편)이었다. CRS 단독과 비교한 연구 1편에서 3년 전체생존율은 중재군에서 좋은 경향을 보였으나, 군 간 유의성을 보고하지 않았다. 증례연구 3편에서 5년, 3년, 2년 전체생존율은 각각 38.0%, 64.0%, 63.0%였다. 중앙 무질병생존기간(전체사망 위험)을 보고한 연구는 증례연구 1편이며, 10개월로 보고하였다. 특정 시점의 무질병생존율을 보고한 연구는 3편이었다. CRS 단독과 비교한 연구 1편에서 1년 무질병생존율은 중재군에서 좋은 경향을 보였으나, 군 간 차이는 없었다. 증례연구 2편에서 3년, 1년 무질병생존율은 각각 9.9%, 42.0%로 보고하였다. 복강 외 병변 환자의 1년 무질병생존율은 0%였다. 중앙 무재발생존기간을 보고한 연구는 증례연구 1편이며, 14.9개월로 보고하였다. 특정 시점에서 질병의 증거가 없는 상태는 증례보고 9편이며, 0.0~100.0%로 보고하였다. 9편 중 3편은 수술 후 재발 없이 무병생존 상태를 유지하였으며, 나머지 6편에서는 질병 재발(진행) 또는 다른 장기로의 전이 등을 보고하였다. 재수술률을 보고한 연구는 증례연구 3편이며, 33.3~100.0%로 보고하였다. 주요 재수술 사유로는 국소재발, 복막재발, 담낭천공으로 인한 담즙성 복막염을 보고하였다. 삶의 질을 보고한 연구는 없었다.

결론 및 권고결정

의료기술재평가 소위원회에서는 현재 평가결과에 근거하여 다음과 같이 제언하였다.

악성 복막중피종 악성 복막중피종 환자에서 CRS+HIPEC의 안전성은 모두 단일군연구 결과로, 중대한 이상반응 발생은 약 8.6~43.2%, 30일 이내 사망률은 약 11.0%로 다른 복막 전이암과 유사한 수준이었다. 효과성은 CRS 단독과 비교하여 전체사망 위험이 중재군에서 더 낮은 경향을 보였으나(근거수준: 매우 낮음), 그 외 생존 관련 결과의 군 간 유의성은 일관되지 않았다(근거수준: 매우 낮음). 그 외 다른 치료법(전신 항암치료, 완화수술)과 비교하여 전체사망 위험과 5년 전체생존율이 중재군에서 더 좋은 경향을 보였으나(근거수준: 매우 낮음), 그 외 생존 관련 결과에서는 군 간 유의성이 일관되지 않았다(근거수준: 매우 낮음). 소위원회는 안전성과 관련하여 중재가 중등도 이상의 합병증 위험을 내재한 고강도 치료인 점과 전반적인 합병증 발생 수준이 다른 복막 전이암과 유사한 점을 감안할 때, 적절한 환자 선정과 숙련된 술기, 관리 가능한 전문기관에서 시행될 경우에는 임상적으로 허용 가능한 범위 내의 안전성이 있는 치료로 평가하였다. 효과성 측면에서 악성 복막중피종은 전체 중피종(mesothelioma)의 약 10-15%를 차지하는 매우 드문 희귀질환으로, 병리학적 아형 및 분자생물학적 특성이 이질적이므로 대상자 선정에 제약이 있다. 또한 진단 및 병기 설정 기준과 치료 프로토콜이 기관 및 국가별로 상이하여 표준화된 임상데이터를 체계적으로 축적하는 데 본질적인 한계가 있는 질환으로 간주되고 있다. 이러한 한계에도 불구하고 최근 제시된 가이드라인에서는 의학적으로 수술이 가능하고, 상피형(epithelioid)이며, 복막암지수(PCI) ≤17로 완전 종양절제(CC0-1, 잔존 병변 ≤2.5mm)가 가능할 것으로 판단되는 저위험 환자군에서 중재를 1차치료로 제시하고 있다. 또한 진료지침을 대상으로 수행된 체계적 문헌고찰 및 권고강도 분석연구(2025)에서도 가이드라인 간 합의가 높았으며(90% 이상 긍정 권고), 이를 기반으로 중재를 표준치료로 고려하였다(GRADE 등급: I). 따라서, 악성 복막중피종의 질병의 특성상 나타나는 이질성, 연구 인프라 및 자원의 제한, 이에 따른 연구 설계상의 한계들을 종합적으로 고려할 때, 복막 생검을 통한 정확한 조직학적 진단을 기반으로 전신 상태가 양호하고 완전 종양절제가 가능할 것으로 판단되는 저위험 환자군에서 본 중재는 임상적으로 유용한 치료 옵션으로 평가하였다. 다만, 수술 자체가 불가능하거나 완전 종양절제가 불가능하거나 고위험 인자(병리학적 아형이 혼합형, 육종형, 림프절전이 양성 또는 Ki-67>9%, 혈소판 등)를 동반한 환자에서는 선행항암치료를 시행한 후 반응을 평가하여 선택적으로 중재를 고려해 볼 수 있겠다.

결합조직형성 소원형세포종양 결합조직형성 소원형세포종양 환자에서 CRS+HIPEC의 안전성은 모두 단일군연구 결과로, 중대한 이상반응 발생이 약 23.0~84.0%로 높은 편이었으며, 30일 이내 사망은 발생하지 않았다. 효과성은 CRS 단독과 비교하여 중앙 전체생존기간, 3년 전체생존율, 1년 무질병생존율이 중재군에서 더 좋은 경향을 보였으나, 군 간 차이가 없거나 유의성을 보고하지 않았다(근거수준: 매우 낮음). 그 외 다른 치료법(전신 항암치료 또는 완화수술)과 비교한 연구는 없었다. 소위원회는 안전성 측면에서 중재가 중등도 이상의 합병증 위험을 내재한 고강도 치료인 점과 전반적인 합병증 발생 수준이 다른 복막 전이암과 유사한 점을 감안할 때, 적절한 환자 선정과 숙련된 술기, 관리 가능한 전문기관에서 시행될 경우에는 임상적으로 허용 가능한 범위 내의 안전성이 있는 치료로 평가하였다. 결합조직형성 소원형세포종양은 EWSR1–WT1 융합유전자를 특징으로 하는 극히 드물고 공격성이 높은 복막 육종으로, 진단 및 치료 과정이 복잡하며, 매우 낮은 유병률로 인해 연구 수행 및 근거 축적에 근본적인 제약이 있는 희귀암으로 간주되고 있다. 이러한 특성으로 인해 현재까지 확립된 표준치료가 없으며, 국내외적으로 공식적인 가이드라인도 부재한 상황이다. 다만 복막표면종양 분야의 다학제 전문가 합의문에서는 전신 항암요법에 반응을 보이는 국한성 복막병변 환자에서 완전 절제가 가능한 경우에는 중재를 치료 대안으로 고려할 수 있다고 제시하였다. 이에 소위원회는 국내외 임상 근거가 제한적이고, 희귀암의 특성상 적용 가능한 치료 옵션이 매우 제한적이라는 점을 종합적으로 고려할 때, 전신 항암요법에 반응하고 완전 절제가 가능한 일부 환자에서 중재를 치료 대안으로 고려할 수 있을 것으로 제안하였다. 아울러, 희귀암의 특성상 진단 및 치료 과정에서 임상적 의사결정의 불확실성이 크고, 의료기관 간 진료 편차가 발생할 가능성이 높은 점 등을 고려하여, 향후 진료의 일관성과 예측 가능성을 제고하기 위해 학회 차원의 전문가 합의문 또는 진료 가이드라인 마련이 필요할 것으로 제언하였다. 2025년 제3차 재평가전문위원회*(2025. 11. 14.)는 소위원회 결론 및 분과의견을 검토하여 다음과 같이 심의·의결하고 권고등급을 결정하였다. 재평가전문위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 악성 복막중피종 환자 대상 생존 향상 등의 목적으로 종양감축술 후 복강내 온열 항암화학요법의 사용을 ‘약하게 권고함’으로 결정하였다. 재평가전문위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 결합조직형성 소원형세포종양 환자 대상 생존 향상 등의 목적으로 종양감축술 후 복강내 온열 항암화학요법의 사용을 ‘권고 보류’로 결정하였다.

Background Hyperthermic intraperitoneal chemotherapy (HIPEC) is a treatment for patients with peritoneal carcinomatosis in which cytoreductive surgery (CRS) is first performed to remove all visible lesions, followed by the circulation of heated chemotherapeutic agents within the peritoneal cavity for a specified period to eliminate microscopic residual disease, after which the solution is drained. This technology was first recognized and notified as a new health technology in 2013. It was subsequently revised in 2014 to reflect the expansion of its indications and methods of use, and was later listed for conditional reimbursement at a 50% coverage level in 2021. As the five-year reassessment cycle for conditional reimbursement has elapsed, the technology was identified through a demand survey conducted by relevant institutions. In this reassessment, given the availability of various alternative medical technologies, we aimed to review the most recent evidence on the safety and effectiveness of HIPEC compared with existing treatments in patients with malignant peritoneal mesothelioma (MPM) and desmoplastic small round cell tumor (DSRCT). The reassessment was conducted following deliberation on the evaluation plan and the composition of the subcommittee at the 2nd Healthcare Technology Reassessment Committee in 2025 (February 14, 2025).

Objective The objective of this assessment is to provide evidence on the clinical safety and effectiveness of CRS+HIPEC in patients with MPM and DSRCT, and to determine the recommendation grade for the use of this technology through the healthcare technology reassessment process.

Methods A systematic review was conducted to evaluate the safety, effectiveness, and cost-effectiveness of CRS+HIPEC. All methods were determined through deliberation by the Subcommittee in consideration of the assessment objectives. The Subcommittee comprised seven experts in obstetrics and gynecology, colorectal surgery, gastrointestinal surgery, hematology-oncology, gastroenterology, and evidence-based medicine. The key question was whether CRS+HIPEC is clinically safe and effective for improving survival in patients with MPM and DSRCT. The target population was defined in accordance with the scope of approved off-label anticancer drug use for HIPEC, as specified by the Cancer Review Committee of the Health Insurance Review and Assessment Service (March 18, 2025). It included five types of peritoneal malignancies: peritoneal metastases from ovarian and colorectal cancers, pseudomyxoma peritonei arising from appendiceal mucinous tumors, MPM and DSRCT. The intervention was CRS+HIPEC without restriction on chemotherapeutic agents. Comparators included CRS alone, systemic chemotherapy, and palliative/supportive care. Safety outcomes included surgery-related adverse events, complications, and 30-day mortality. Effectiveness outcomes included overall survival, progression-free survival (PFS), disease-free survival (DFS), and recurrence-free survival (RFS) as primary outcomes, and reoperation and quality of life as secondary outcomes. Cost-effectiveness was assessed using indicators such as the incremental cost-effectiveness ratio (ICER). No restrictions were placed on study design. Literature searches were conducted in Ovid-MEDLINE, Ovid-EMBASE, Cochrane CENTRAL, KoreaMed, and KERIS (last search: March 28, 2025). Risk of bias was assessed using the Cochrane RoB tool for randomized controlled trials (RCTs) and RoBANS 2.0 for nonrandomized studies (NRS). Two reviewers independently performed study selection and data extraction using predefined forms, and meta-analysis was conducted where appropriate. The level of evidence was assessed using the GRADE approach. The final recommendation grade was determined by the Healthcare Technology Reassessment Committee based on the Subcommittee’s review.

Results MPM A total of 43 studies reported safety and effectiveness outcomes in patients with MPM, including 4 nonrandomized comparative studies, 29 case series, and 10 case reports. No studies evaluated cost-effectiveness. The study population was heterogeneous, with diffuse-type disease accounting for approximately 41.9% of cases. Epithelioid histology was the most common subtype, although a substantial proportion of patients had higher-grade variants, including biphasic and sarcomatoid subtypes. Tumor burden, as measured by the peritoneal cancer index (PCI), was variably reported: PCI scores of 10–20 were described in 14 studies and scores ≥20 in 8 studies, whereas many studies did not report PCI. Pre-treatment status and therapeutic strategies varied considerably both across and within studies, reflecting differences in disease extent and patient condition. Following CRS, the completeness of cytoreduction (CC) also varied, with complete cytoreduction (CC0–1) reported in 13 studies and incomplete cytoreduction (CC2–3) in 22 studies. In some cases, CRS+HIPEC was combined with additional therapies, including adjuvant systemic chemotherapy or repeat intraperitoneal chemotherapy. Comparators, when reported, included CRS alone, systemic chemotherapy, and palliative surgery. Risk of bias assessment for the four comparative studies indicated a high risk of bias across several domains, particularly in comparability of groups (100%). Additional concerns were identified in confounding (25%), exposure measurement (25%), and incomplete outcome data (25%).

DSRCT A total of 15 studies reported safety and effectiveness outcomes in patients with DSRCT, including 3 nonrandomized comparative studies, 5 case series, and 7 case reports. No studies evaluated cost-effectiveness. The study population consisted predominantly of young male patients, most of whom presented with advanced disease accompanied by metastases to the liver, lungs, or lymph nodes. Tumor burden, as assessed by the PCI, was inconsistently reported across studies; where available, values generally ranged from 10 to 20, although many studies did not provide this information. Most patients had received prior systemic chemotherapy before undergoing CRS+HIPEC. In some cases, the diagnosis was established after initial misclassification, subsequently confirmed through pathological and molecular testing, including identification of the EWSR1–WT1 translocation. Following CRS, the CC varied, with complete cytoreduction (CC0–1) reported in 8 studies and incomplete cytoreduction (CC2–3) in 4 studies. Additional treatments, such as adjuvant chemotherapy or radiotherapy, were administered in a subset of patients. Comparative evidence was limited to studies evaluating CRS-based approaches. Risk of bias assessment for the three comparative studies indicated a high risk of bias, particularly in comparability of groups (100%) and confounding (66.7%).

Safety MPM Safety outcomes were reported in 26 single-arm studies, including 23 case series and 3 case reports. Evidence on surgery-related overall adverse events was derived from 19 studies. Among the case series (n=16), the incidence of overall postoperative complications ranged from 9.5% to 65.0%. Mild (grade I–II) adverse events occurred in approximately 9.5%–39.3% of patients, most commonly reported in the range of 25%–40%. Frequently observed mild complications included nausea, vomiting, fever, and mild pancreatitis. In a pediatric study, the overall complication rate was higher (71.4%), with reported events including acute kidney injury, hyperbilirubinemia, respiratory distress (often accompanied by pleural effusion), pneumothorax, and hematologic abnormalities such as anemia and coagulopathy. These complications were generally manageable with conservative measures or supportive interventions, including chest tube insertion. Among case reports (n=3), the incidence of adverse events ranged from 0% to 100%. One report described no complications, whereas the others reported events such as stoma formation and pelvic abscess, which were successfully managed with drainage and antibiotic therapy. Serious adverse events (SAEs) were reported in 17 case series, with incidence rates ranging from 8.6% to 43.2%. These included surgical complications (e.g., bowel leakage, wound dehiscence, anastomotic failure, and bowel perforation), infectious complications (e.g., bacteremia, sepsis, septic shock, Clostridium difficile infection, empyema, and infected pleural effusion), respiratory complications (e.g., aspiration pneumonia and respiratory failure), and renal complications (e.g., acute kidney injury and nephrotoxicity). Additional reported events included hematologic toxicities, peritonitis related to chemical or bile leakage, and postoperative mortality. Short-term mortality (within 30 days) was reported in nine case series, with rates ranging from 0% to 11.1%. Reported causes of death included multi-organ failure secondary to systemic inflammatory response or sepsis, cardiogenic shock due to pulmonary embolism, renal failure associated with bleeding or coagulopathy, and cardiac arrest related to enterocutaneous fistula.

DSRCT Safety outcomes were reported in 11 single-arm studies, including 4 case series and 7 case reports. Evidence on surgery-related overall adverse events was available from 8 studies. Among the case series (n=2), the incidence of overall postoperative complications ranged from 19.2% to 62.5%. Reported complications included bowel obstruction, hyperamylasemia, elevated creatinine levels, wound dehiscence, and transient gastric stasis, most of which were successfully managed with supportive or conservative measures, including vacuum-assisted therapy. Among case reports (n=6), two reported no adverse events, whereas the remaining four described complication rates ranging from 50% to 100%. Reported events included urinary leakage following ureteral anastomosis, high-output ileostomy, acute kidney injury, bacteremia, wound infection, partial anastomotic disruption, perirectal abscess, pleural empyema, pulmonary embolism, psoas muscle bleeding, and bowel obstruction. These complications were generally managed with intensive supportive care, including total parenteral nutrition, hemodialysis, anticoagulation therapy, and other conservative treatments. SAEs were reported in 4 single-arm studies (3 case series and 1 case report). One case series reported no severe complications, whereas the remaining two case series reported rates of 23.0% and 84.0%, respectively, including grade IV events requiring dialysis. The case report described biliary peritonitis secondary to gallbladder perforation. 30-day mortality was reported in four studies, with no deaths observed across all studies.

Effectiveness MPM A total of 41 studies reported effectiveness outcomes, including 4 nonrandomized comparative studies, 28 case series, and 9 case reports. Median overall survival or mortality risk was reported in 28 studies. When compared with CRS alone (n=3), two studies suggested longer median overall survival with CRS+HIPEC, while one showed no difference. Pooled analysis of these studies demonstrated a significantly lower risk of death in the intervention group (hazard ratio [HR]=0.53, 95% confidence interval [CI] 0.41–0.68, I²=0%, p<0.0001). When compared with systemic chemotherapy alone (n=2), both studies reported a favorable trend toward longer survival with CRS+HIPEC, with one study demonstrating a significantly reduced mortality risk (HR=0.54, 95% CI 0.43–0.69, p<0.001). Evidence comparing CRS+HIPEC with palliative surgery alone was limited to a single study, which suggested improved survival in the intervention group without statistical significance. Among single-arm studies (n=24 case series), median overall survival ranged from 20 to 65 months, with most studies reporting values between 30 and 50 months. One study reported that median survival was not reached during follow-up. Overall survival at predefined time points was reported in 25 studies. Comparative evidence was limited and inconsistent. A single study comparing CRS+HIPEC with CRS alone suggested a higher 5-year survival rate with the intervention but without statistical significance. Similarly, two studies comparing with systemic chemotherapy reported higher survival rates with CRS+HIPEC, although results were not consistently significant. Pooled analysis of two studies reporting 5-year outcomes demonstrated a reduced mortality risk in the intervention group (risk ratio [RR]=0.62, 95% CI 0.60–0.64, I²=0%, p=0.003). In single-arm studies, survival rates varied widely depending on follow-up duration. 10-year overall survival was reported in two studies (26.0% and 44.6%). 5-year overall survival was reported in 20 studies, ranging from 28.9% to 100.0%, with most estimates clustering around 30%–50%. 3-year overall survival ranged from 33.3% to 91.8% (predominantly 40%–60%), while 2-year overall survival was approximately 57.0%–80.0%, and 1-year overall survival ranged from 69.3% to 86.7%. Evidence on PFS was limited. Median PFS was reported in three case series, ranging from 13.9 to 25.1 months. PFS at specific time points was reported in four studies. 10-year PFS was reported in one study (35.9%), while 5-year PFS ranged from 13.0% to 38.4%. Isolated estimates of 3-, 2-, and 1-year PFS were reported as 26.0%, 32.0%, and 53.0%, respectively. Median DFS was reported in three studies, ranging from 8 to 17 months. One study reported a 4-year DFS rate of 38.0%. Median RFS was reported in one study (38 months), and a 5-year RFS rate of 34.8% was reported in another. Disease status without evidence of recurrence was reported in 11 single-arm studies. Among case series (n=2), 66.7% and 80.0% of patients were reported to be disease-free during follow-up. In one study, four patients remained disease-free beyond 8 years, with two deaths reported at 15 years. In another small cohort, four of five patients were disease-free at 15 months, while one developed suspected pulmonary metastasis at 8 months. Among case reports (n=9), outcomes ranged from complete remission to disease progression. Five reports described sustained disease-free status without recurrence, whereas the remaining cases involved progression or death. Reoperation rates were reported in 10 case series. Excluding one study with no reoperations, rates ranged from 6.1% to 20.0%. The most common indications were tumor recurrence (n=4) and major complications (n=2), while some studies did not specify indications. Quality-of-life outcomes were reported in one case series. Pain improved at all postoperative time points. Functional and physical outcomes declined initially but recovered by 6 months and remained stable thereafter. Emotional and social domains improved early but showed some decline at longer follow-up. Depressive symptoms decreased at 12 months but increased again by 24 months, with the prevalence of clinically significant depression rising from 31% preoperatively to 37%. DSRCT A total of 15 studies (3 nonrandomized comparative studies, 3 case series, and 9 case reports) reported effectiveness outcomes. Median overall survival was reported in five studies. Comparative evidence was limited: two studies comparing CRS+HIPEC with CRS alone suggested a favorable trend for the intervention but did not demonstrate statistically significant differences. In case series (n=3), median overall survival ranged from 44.3 to 63.1 months, with longer survival observed when measured from diagnosis or following complete cytoreduction. Overall survival at specific time points was reported in three studies. A single comparative study showed a non-significant trend toward improved 3-year survival with CRS+HIPEC. In case series, reported survival rates were 38.0% at 5 years, 64.0% at 3 years, and 63.0% at 2 years. Evidence on DFS was sparse. One case series reported a median DFS of 10 months. Three studies reported DFS at specific time points; a comparative study showed a non-significant trend favoring the intervention at 1 year, while case series reported 3-year and 1-year rates of 9.9% and 42.0%, respectively. Notably, patients with extra-abdominal disease had no DFS at 1 year. Median RFS was reported in one case series (14.9 months). Evidence on disease status was limited to case reports, with outcomes ranging from no evidence of disease to disease progression; three reports described sustained disease-free status, whereas the remainder reported recurrence or metastatic progression. Reoperation rates were reported in three case series and ranged from 33.3% to 100%, most commonly due to local or peritoneal recurrence or complications such as biliary peritonitis secondary to gallbladder perforation. No studies reported quality-of-life outcomes.

Conclusion and Recommendation Based on the available evidence, the Subcommittee on Healthcare Technology Reassessment makes the following conclusions.

MPM In patients with MPM, evidence on safety was derived exclusively from single-arm studies. SAEs occurred in approximately 8.6%–43.2% of patients, and 30-day mortality was reported at up to 11%, broadly comparable to rates observed in other peritoneal malignancies. With respect to effectiveness, limited comparative evidence suggested a trend toward reduced mortality with CRS+HIPEC compared with CRS alone; however, the certainty of evidence was very low and findings across other survival outcomes were inconsistent. Similarly, comparisons with alternative treatments, including systemic chemotherapy and palliative surgery, indicated a tendency toward improved overall survival and 5-year survival with the intervention, although these results were not consistently statistically significant and remained subject to substantial uncertainty (certainty of evidence: very low). The Subcommittee considered that CRS+HIPEC is an intensive treatment modality associated with a non-negligible risk of moderate-to-severe complications. Nevertheless, given that the overall complication profile is comparable to that of other peritoneal malignancies, the intervention may be regarded as clinically acceptable when performed in carefully selected patients, by experienced surgical teams, and within appropriately equipped centers. MPM is a rare disease, accounting for approximately 10%–15% of all mesothelioma cases, and is characterized by substantial histopathological and molecular heterogeneity. Variability in diagnostic criteria, staging systems, and treatment protocols across institutions and countries further limits the availability of standardized clinical data. Despite these constraints, recent clinical guidelines recommend CRS+HIPEC as a first-line treatment in selected low-risk patients—specifically those who are medically fit for surgery, have epithelioid histology, a PCI ≤17, and are considered candidates for complete cytoreduction (CC0–1). A recent systematic review of guidelines (2025) also demonstrated a high level of consensus (>90% favorable recommendations), supporting the role of CRS+HIPEC as a standard treatment option (GRADE I). Taken together, considering the inherent limitations in the evidence base and the biological characteristics of the disease, CRS+HIPEC may represent a clinically meaningful treatment option in carefully selected patients. Appropriate candidates include those with confirmed histological diagnosis, good performance status, and a high likelihood of achieving complete cytoreduction. In contrast, for patients who are not surgical candidates, are unlikely to achieve complete cytoreduction, or have high-risk features (e.g., biphasic or sarcomatoid histology, nodal involvement, elevated proliferative indices), neoadjuvant systemic therapy followed by selective consideration of CRS+HIPEC may be a more appropriate approach.

DSRCT In patients with DSRCT, evidence on safety was derived exclusively from single-arm studies. Rates of SAEs were relatively high, ranging from approximately 23.0% to 84.0%, whereas no 30-day mortality was reported. With respect to effectiveness, limited comparative evidence suggested a trend toward improved outcomes with CRS+HIPEC compared with CRS alone, including longer median overall survival, higher 3-year overall survival, and improved 1-year DFS. However, these findings were not statistically significant and remain highly uncertain (certainty of evidence: very low). No studies directly compared CRS+HIPEC with other treatment modalities, such as systemic chemotherapy or palliative surgery. The Subcommittee considered that CRS+HIPEC is an intensive treatment associated with a substantial risk of moderate-to-severe complications. Nevertheless, given that the overall complication profile is broadly comparable to that observed in other peritoneal malignancies, the intervention may be regarded as clinically acceptable when performed in carefully selected patients, by experienced multidisciplinary teams, and within appropriately resourced specialized centers. DSRCT is an extremely rare and aggressive peritoneal sarcoma characterized by the EWSR1–WT1 fusion gene. Its low incidence and biological complexity impose fundamental limitations on clinical research and evidence generation. To date, no universally accepted standard of care or formal clinical guidelines have been established. However, multidisciplinary expert consensus in the field of peritoneal surface malignancies suggests that CRS+HIPEC may be considered as a treatment option in selected patients with localized peritoneal disease who demonstrate a favorable response to systemic chemotherapy and are candidates for complete cytoreduction. Taken together, given the scarcity of evidence and the limited availability of effective treatment options for this rare malignancy, CRS+HIPEC may be considered a potential therapeutic option in carefully selected patients—particularly those with chemosensitive disease and a high likelihood of achieving complete resection. In addition, considering the inherent uncertainty in clinical decision-making and the potential variability in practice across institutions, the development of expert consensus statements or clinical guidelines is warranted to improve consistency and predictability in care. At the 3rd Healthcare Technology Reassessment Expert Committee in 2025 (November 14, 2025), the Committee reviewed the Subcommittee’s conclusions and expert opinions and reached the following decision. MPM After considering the available evidence on clinical safety, effectiveness, and other relevant factors, the Healthcare Technology Reassessment Expert Committee issued a weak recommendation for the use of HIPEC following CRS in patients with MPM to improve survival in the Korean clinical setting.

DSRCT After considering the available evidence on clinical safety, effectiveness, and other relevant factors, the Healthcare Technology Reassessment Expert Committee determined that the use of HIPEC following CRS in patients with DSRCT should be deferred (recommendation withheld) in the current Korean clinical context.
