섭식장애평가

Abstract

평가배경 섭식장애평가는 거식증, 폭식증 등 각종 섭식장애를 평가하기 위한 표준화된 척도검사로, 현행 비급여로 적용되고 있다.

동 행위는 신의료기술평가제도 도입 전 비급여로 등재된 기술로 국민건강보험공단의 비급여 보고제도 대상 항목으로, 수요조사(유관기관 평가 요청)를 통해 재평가 대상으로 발굴되었다. 2025년 제5차 의료기술재평가위원회(2025.5.16.)에서는 동 안건의 평가계획서 및 평가방법, 소위원회 구성을 심의·확정하였다.

평가목적 본 평가는 섭식장애평가에 활용 가능한 측정도구의 국내외 현황을 체계적으로 검토하여, 임상 적용에 유용한 정보를 제공하는 것을 목적으로 하였다. 특히 한국어로 번안되거나 국내 개발되어 신뢰도와 타당도가 검증된 표준화된 섭식장애평가 도구를 확인함으로써, 임상 현장에서의 적절한 도구 선택과 활용을 지원하고자 하였다.

평가방법 섭식장애평가에 대해 평가하기 위해 모든 평가방법은 평가목적으로 고려하여 “섭식장애평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다.

본 평가는 섭식장애평가에 활용 가능한 측정도구의 현황을 파악하고, 국내 임상에서 적용 가능한 도구를 확인하기 위해 국내외 체계적 문헌고찰과 타당화 연구(국외 번안 도구 및 국내 개발 도구 포함)를 검토하였다.

먼저, 체계적 문헌고찰을 통해 섭식장애평가 도구의 목록을 수집하였다. 국외 문헌은 Ovid-MEDLINE과 PsycINFO에서 ‘섭식장애’, ‘측정도구’, ‘체계적 문헌고찰’ 관련 검색어를 활용하여 검색하였으며, 국내 문헌은 KoreaMed에서 ‘섭식장애’ 관련 검색어를 사용하였다.

수집된 문헌을 바탕으로 전문가 자문을 통해 국내 임상 현장에서 활용 가능한 도구 목록을 확정하였다. 이후 각 도구의 세부 정보를 확인하기 위해 도구별 타당화 연구를 추가로 검색·검토하였다.

본 평가에는 (1) 한국어로 번안되어 신뢰도와 타당도가 검증된 표준화 도구, (2) 국내에서 개발된 도구를 포함하였다. 각 도구별로 ① 측정 목적 및 주요 개념, 하위 영역, 문항 수, 점수화 방법 등의 개요와 ② 신뢰도·타당도 검증 결과를 검토하여, 국내 임상에서의 활용 가능성과 근거 수준을 종합적으로 분석하였다.

평가결과 체계적 문헌고찰 검토 본 검토에서는 섭식장애평가 도구의 전반적 현황을 파악하기 위해 관련 체계적 문헌고찰 15편을 분석하였다. 주요 결과 및

현황은 다음과 같다.

첫째, 섭식장애평가 도구에는 자가보고식 설문뿐 아니라 면담형 도구도 포함되어 있어 평가도구의 유형과 적용 범위가 다양하였다.

둘째, 적용 대상 집단은 성인(Schaefer et al., 2021)과 아동·청소년(Jabs et al., 2025)을 대상으로 하는 경우 외에도 특정 진단에 초점을 둔 경우도 확인되었다. 예를 들어, 위장관 질환자(Soliman et al., 2025), 임산부(Bannatyne et al., 2021), 제1형 당뇨병 환자(Pursey et al., 2020), 비만수술 환자(Parker et al., 2014; Parker et al., 2015) 등 특정 집단을 대상으로 한 연구도 확인되었다.

셋째, 측정 개념의 범위가 다양한 하위 영역을 포괄하고 있다. Hatoum 등(2023)은 섭식장애 관련 인지(cognition) 측정을, Burton 등(2016)은 폭식 증상, Hoetzel 등(2013)은 변화 동기, Tirico 등(2010)은 섭식장애 특이 삶의 질을 다룬 도구를 검토하는 등, 특정 개념에 초점을 둔 연구들이 확인되었다.

넷째, 일부 체계적 문헌고찰의 경우, 특정 도구 중심으로 검토하였다. 예를 들어, Berg 등(2012)과 Jenkins 등(2022)의 연구에서는 EDE (Eating Disorder Examination)와 EDE-Q (Eating Disorder Examination-Questionnaire)의 신뢰도 및 타당도를 평가하였으며, Jahrami 등(2025)과 Penzenstadler 등(2019)은 Yale Food Addiction Scale의 신뢰도 등에 대한 평가하였다.

섭식장애평가 도구 검토 본 평가에서는 섭식장애평가를 위한 도구 중 한국어로 번안되어 신뢰도·타당도가 검증된 표준화된 도구와 국내 개발 도구를 포함하였다. 이를 위해 관련 체계적 문헌고찰 검토를 통해 확인된 도구 목록 가운데 전문가 검토를 거쳐 정보 제공이 필요하다고 제안된 대표 도구들을 중심으로 검토하였다.

소위원회 검토 결과, 총 20개 평가 도구 중 국내 임상 현장에서의 적절한 도구 선택과 활용을 지원하기 위해 검토에 포함한 대표적인 도구들은 6개로 다음과 같다: EDE, EDI, EAT (Eating Attitudes Test), EDDS (Eating Disorder Diagnostic Scale), CIA (Clinical Impairment Assessment Questionnaire).

결론 및 제언 섭식장애평가 소위원회는 섭식장애평가에 활용 가능한 측정도구의 국내·외 현황과 한국어로 표준화되어 신뢰도·타당도가 검증된 도구를 종합적으로 검토한 결과, 다음과 같은 의견을 제시하였다.

섭식장애평가 도구에 자가보고식 설문뿐 아니라 면담형 도구도 포함되어 있어 평가도구의 유형과 적용 범위가 다양함을 확인하였다. 국내에서 신뢰도와 타당도가 연구된 도구로는 EDE, EDE-Q, EDI, EAT, EDDS, CIA 등이 확인되었으며, 검토 결과 각 도구의 신뢰도와 타당도가 전반적으로 양호하게 확인되어, 해당 도구의 임상적 적용 타당성을 뒷받침하는 것으로 판단하였다.

다만, 표본의 대표성, 문화적 적합성, 최신화 수준 등의 측면에서 한계가 존재하므로, 한국인을 대상으로 한 측정도구 개발과 기존 한국어판 도구의 표준화 및 임상적 유용성 검증을 위한 지속적인 연구가 필요하다. 또한, 환자의 임상 특성, 평가 목적, 적용 환경 등에 따라 적절한 도구의 선택이 달라질 수 있다. 따라서 실제 임상에서는 각 도구의 특성과 환자 상태를 종합적으로 고려하여, 임상전문가의 판단과 경험에 기반한 적절한 평가도구를 선택하는 것이 바람직하다.

2025년 제3차 재평가전문위원회(2025.11.14.)에서는 소위원회 검토 결과를 바탕으로 “섭식장애평가”에 대해 논의하였으며, 소위원회 결론을 원안대로 심의·의결하였다.

Background Eating disorder assessment refers to standardized, scale-based instruments used to evaluate various conditions, including anorexia nervosa and bulimia nervosa. These assessments are currently categorized as non-reimbursed services under the Korean national health insurance system. It is currently an item subject to the National Health Insurance Service's non-reimbursable reporting system and was identified as a target for re-evaluation through a demand survey. At the 5th Medical Technology Re-evaluation Committee meeting in 2025 (May 16, 2025), the evaluation plan, evaluation method, and subcommittee composition for this item were deliberated and finalized.

Objective The purpose of this evaluation was to systematically review the current domestic and international status of measurement tools used for assessing eating disorders and to provide useful information for clinical application. In particular, this study aimed to support appropriate selection and utilization of assessment tools in clinical practice by identifying standardized eating disorder assessment tools that have been translated into Korean or developed domestically and have established reliability and validity.

Methods In order to evaluate the eating disorder assessment tools, all evaluation methods were finalized after deliberation by the “Eating Disorder Evaluation Subcommittee.” This evaluation examined systematic reviews and validation studies (including translated and domestically developed tools) to identify available assessment tools and determine their applicability in Korean clinical settings.

Literature Search Strategy: • International literature was searched in Ovid-MEDLINE and PsycINFO using keywords related to “eating disorders,” “measurement tools,” and “systematic review.” • Domestic literature was searched in KoreaMed using keywords related to “eating disorders.”

Tool Selection and Review: Based on the collected literature, a list of tools applicable in domestic clinical practice was finalized through expert consultation. Additional searches and reviews of validation studies were conducted for each tool to obtain detailed information.

This evaluation included: 1. Standardized tools translated into Korean with established reliability and validity 2. Tools developed in Korea

For each tool, the following were reviewed: ① Overview (measurement purpose, key concepts, subdomains, number of items, scoring method) ② Evidence on reliability and validity Based on this, the applicability and level of evidence for clinical use in Korea were comprehensively analyzed.

Results Review of Systematic Reviews An analysis of 15 systematic reviews revealed the following trends: • Instrument Diversity: Assessments include both self-report questionnaires and clinician-administered interviews, offering a broad range of application scopes. • Target Populations: While many tools target adults or children/adolescents (Jabs et al., 2025), others are tailored for specific cohorts, such as patients with gastrointestinal diseases (Soliman et al., 2025), pregnant women (Bannatyne et al., 2021), patients with type 1 diabetes (Pursey et al., 2020), and bariatric surgery candidates (Parker et al., 2014; 2015). • Construct Specificity: Certain reviews focused on specific dimensions, such as ED-related cognitions (Hatoum et al., 2023), binge eating symptoms (Burton et al., 2016), motivation for change (Hoetzel et al., 2013), and ED-specific quality of life (Tirico et al., 2010). • Tool-Specific Evidence: Several reviews provided focused evaluations of specific instruments, such as the EDE and EDE-Q (Berg et al., 2012; Jenkins et al., 2022) or the Yale Food Addiction Scale (Jahrami et al., 2025; Penzenstadler et al., 2019).

Review of Assessment Tools Based on Subcommittee deliberation and expert review, six representative tools (out of 20 identified) were selected for detailed information provision to support clinical decision-making: • EDE (Eating Disorder Examination) • EDE-Q (Eating Disorder Examination Questionnaire) • EDI (Eating Disorder Inventory) • EAT (Eating Attitudes Test) • EDDS (Eating Disorder Diagnostic Scale) • CIA (Clinical Impairment Assessment Questionnaire)

Conclusion and Recommendations The Subcommittee concluded that the six tools validated in Korea (EDE, EDE-Q, EDI, EAT, EDDS, and CIA) generally demonstrate acceptable reliability and validity, affirming their clinical utility. However, several limitations were identified regarding sample representativeness, cultural nuance, and the frequency of normative data updates. Consequently, the Subcommittee recommends further research to: 1. Develop de novo measurement tools tailored to the Korean population. 2. Further standardize existing Korean versions of international tools. 3. Continuously validate clinical utility across diverse settings. Final Recommendation: The choice of an assessment tool should be dictated by patient characteristics, the specific purpose of the evaluation, and the clinical setting. Practitioners are encouraged to exercise clinical judgment and expertise to select the most appropriate tool for each individual case. On November 14, 2025, the 3rd Re-evaluation Expert Committee formally adopted these conclusions as proposed.
