언어전반진단검사-말장애(유창성 장애, 조음장애, 음성장애)

Abstract

평가배경 언어전반진단검사는 여러 질환으로 언어장애가 의심될 때, 발음장애, 언어학습장애 여부 및 장애의 유형을 감별진단하고, 손상의 정도를 판별하고 언어치료의 계획 수립과 언어구사능력 회복 여부에 대한 예후를 예측하기 위해 사용되는 검사이다. 본 검사는 신의료기술평가 제도 도입 이전인 2005년에 비급여로 등재되었다. 이후 이후 비급여 보고제도 항목으로 수요조사에서 유관기관으로부터 제안된 후 관련 선정 절차를 거쳐 최종 의료기술재평가 대상으로 선정되었다. 2025년 제5차 의료기술재평가위원회(2025.5.16.)에서는 본 기술을 신속검토 방법으로 평가하기로 결정하고, 소위원회를 구성하여 의료기술재평가를 진행하는 것으로 심의하였다.

본 평가는 언어전반진단검사의 적용 대상 및 활용되는 평가도구에 대한 근거를 검토하여, 임상 현장에서의 적정한 사용에 참고가 될 수 있는 정보를 제공하는 것을 목적으로 수행하였다. 특히, 말장애 중 유창성 장애, 조음장애, 음성장애를 주요 대상으로 포함하여 관련 근거를 중심으로 검토를 진행하였다.

평가목적 본 평가는 언어전반진단검사의 사용 대상과 평가도구에 대한 근거를 검토하되, 특히 말장애(유창성 장애, 조음장애, 음성장애)에 초점을 맞추어 임상 현장에서의 활용에 도움이 될 수 있는 관련 정보를 제공하는 것을 목적으로 한다.

평가방법 본 평가는 언어전반진단검사의 임상적 유용성을 검토하기 위해 문헌적 근거, 진단 관련 법령·지침, 임상전문가 의견을 종합하는 단계적 접근을 적용하였다. 먼저, 국내 의학·재활 관련 교과서와 정신질환 진단 및 통계 편람 제5판(Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, DSM-5), 국제질병분류 제10판(International Classification of Diseases, 10th Revision, ICD-10), 한국표준질병·사인분류(Korean Standard Classification of Diseases, KCD) 등 진단 분류체계 및 관련 법령을 검토하여 본 검사 적용이 가능한 주요 질환군을 범주화하였다. 이어, 언어재활 및 재활의학 분야의 전문서적과 연구문헌을 통해 질환별로 활용 가능한 평가도구의 종류와 구성요소, 적용범위를 확인하였으며, 장애 판정 및 언어장애 등록과 관련된 제도·지침을 함께 검토하여 국내 임상현장에서의 실제 사용 양상을 파악하였다.

또한, 평가도구의 신뢰도·타당도 등 과학적 근거를 확인하기 위해 국내 연구문헌을 우선적으로 검토하였고, 근거가 제한적인 경우 최신 해외 연구를 추가로 참고하여 질환별 평가도구의 중증도 관련성 및 평가대상자 선정기준의 활용 여부를 함께 살펴보았다. 마지막으로, 이러한 일련의 검토 결과와 법적·제도적 활용 근거 및 임상전문가 의견을 종합하여 언어전반진단검사의 실제 임상적 적용 가능성과 유용성을 평가하였다.

평가결과 본 평가는 언어전반진단검사의 임상적 적용 대상을 규명하기 위하여, 관련 교과서 및 전문서적 9편과 함께 DSM-5, ICD-10, KCD 등 주요 질환 분류체계, 보건복지부 고시 「장애정도판정기준」, 「장애인 등에 대한 특수교육법 시행령」의 특수교육대상자 선정기준 등을 검토하였다. 또한 국내 임상현황을 분석한 연구와 조음장애, 유창성장애 및 음성장애에서 사용되는 평가도구의 신뢰도·타당도 및 활용 근거를 확인한 문헌 14편을 추가로 검토하였다.

검토 결과, 언어전반진단검사가 적용되는 말장애의 주요 범주는 조음장애, 유창성장애, 음성장애로 확인되었으며, 이들 모두 「장애정도판정기준」에서 언어장애 유형으로 규정되어 있었다.

조음장애는 구강 구조 또는 조음 근육 기능의 이상, 혹은 부적절한 조음 습득으로 인해 말소리를 정확하게 산출하지 못하는 운동 기반 장애로 정의된다. 언어전반진단검사에는 조음 기능 평가를 위해 그림자음검사와 그림음운변동검사가 포함되어 있다. 「장애정도판정기준」에서는 조음장애 판정을 위해 아동용 발음평가(Assessment of Phonology and Articulation for Children, APAC), 우리말조음·음운평가(Urimal Test of Articulation and Phonology, U-TAP), 그림자음검사를 활용 가능한 도구로 명시하고 있으며, 국내 임상에서는 U-TAP(U-TAP2), APAC, 한국어 조음·음운평가(Korean Standardized Phonology and Articulation Profile Test, KS-PAPT)가 표준화된 도구로 사용되고 있다.

유창성장애는 말의 흐름이 반복, 연장, 막힘 등으로 인해 부자연스럽게 되는 말산출 장애로 정의된다. 언어전반진단검사에는 유창성 평가를 위한 말더듬평가가 포함된다. 「장애정도판정기준」에서는 파라다이스 유창성검사(Paradise–Fluency Assessment, P-FA)를 기본 평가도구로 규정하고 있으며, 필요시 말더듬 심도 검사(Stuttering Severity Instrument, SSI)를 병행하도록 허용하고 있다. 또한 「장애정도심사규정」과 국민연금공단 장애등록심사 지침에서도 유창성장애 평가 시 P-FA 또는 SSI 결과 제출을 요구하고 있다.

음성장애는 성대의 구조적·기능적 이상으로 인해 음질, 강도, 음도 등의 음성 특성에 변화가 나타나는 상태를 말하며, 언어전반진단검사에는 ‘음성총괄평가’가 포함된다. 음성총괄평가는 면담, 구강구조 및 기동성 평가, 후두경·내시경·후두 스트로보스코피 검진, 음도·강도·음질 분석, 호흡조절 및 발성 효율성 평가 등으로 구성된다. 다만 이 중 후두 내시경 검사와 후두 스트로보스코피는 이미 보험급여 항목으로 등재되어 있으며, 음도·강도·음질 분석은 별도의 비급여 항목인 발음·발성 검사(노-688)와 중복되는 측면이 있어 본 평가는 중복 항목을 제외한 면담 및 구강구조·기동성 평가를 중심으로 검토하였다. 면담은 발병 시기, 증상 변화, 병력, 음성 사용 습관 및 환경적 요인 등을 파악하여 선별 및 치료계획 수립에 활용되며, 구강구조·기동성 평가는 발성 관련 구조적 이상 여부를 확인하고 필요시 전문 검사로의 연계를 판단하는 데 중요하다.

종합하면, 언어전반진단검사는 조음장애, 유창성장애 및 음성장애 환자를 대상으로 감별진단, 장애 정도 평가, 치료 계획 수립 및 예후 관련 평가에 활용되고 있으며, 이러한 목적을 위해 다양한 표준화 검사도구가 국내 임상에서 사용되고 있음을 확인하였다. 본 평가는 이러한 말장애 범주를 중심으로 언어전반진단검사의 적용 현황과 활용 근거를 제시하였다.

결론 의료기술재평가 소위원회에서는 현재 평가결과에 근거하여 다음과 같은 의견을 제시하였다.

언어전반진단검사는 발음의 정확성, 발화의 유창성, 음성 사용과 관련된 기능에 장애가 있는 환자를 평가하는 데 필수적인 검사로, 임상적 의사결정 과정 전반에서 중요한 역할을 수행한다. 특히 조음장애, 유창성장애, 음성장애의 존재 여부를 감별하고 손상 정도를 판별하는 데 유용하여, 다양한 임상 상황에서 핵심적인 진단 도구로 활용될 수 있음이 확인되었다.

본 평가에서는 주요 대상 질환을 언어장애와 말장애로 구분하였으며, 말장애에는 조음장애, 유창성장애, 음성장애가 포함된다. 이러한 장애들은 모두 「장애정도판정기준」에서 언어장애 유형으로 규정되어 있으며, 실제 진단에는 조음장애의 경우 U-TAP, APAC, 유창성장애에는 P-FA, SSI, 음성장애에는 음성총괄평가가 활용된다. 이 가운데 음성총괄평가의 일부 항목은 기존 급여·비급여 검사와 중복되는 문제가 있어, 본 평가는 면담과 구강구조·기동성 검사를 중심으로 검토하였다. 검토 결과, 이들 도구는 진단뿐 아니라 치료계획 수립, 예후 예측, 장애 정도 판정의 근거 자료로서 중요한 기능을 수행하는 것으로 확인되었다. 다만, 언어전반진단검사에는 다양한 질환군과 검사 항목이 포함되어 있으며, 일부 항목은 다른 급여·비급여 항목과 중복되는 부분이 있어 향후 수가체계를 보다 합리적으로 재분류·세분화할 필요가 있다는 의견이 제시되었다.

2025년 제3차 재평가전문위원회*(2025.11.14.)는 소위원회 결론안 중 수가체계 재분류 관련 의견은 본 평가의 범위를 다소 벗어나는 것으로 판단되어, 소위원회에서 제시한 결론안을 검토·수정한 후, 아래와 같이 심의 의결하였다.

언어전반진단검사는 발음의 정확성, 발화의 유창성, 음성 사용 등 말 산출 기능에 장애가 있는 환자를 평가하는 데 필수적인 검사로서, 임상적 의사결정 과정 전반에서 중요한 역할을 수행한다고 판단하였다. 특히 동 검사는 조음장애, 유창성장애, 음성장애의 존재 여부를 감별하고 손상 정도를 평가하는 데 유용하여, 다양한 임상 상황에서 핵심적인 진단 도구로 활용될 수 있음을 확인하였다.

이번 평가는 주요 대상 질환 중 말장애에 초점을 두어 검토하였으며, 그 범주에는 유창성장애, 조음장애, 음성장애가 포함되었다. 이러한 장애는 모두 「장애정도판정기준」에서 언어장애 유형으로 규정되어 있으며, 실제 임상에서는 조음장애 평가에 U-TAP, APAC, 유창성장애 평가에 P-FA, SSI, 음성장애 평가에 음성총괄평가 등이 사용되고 있었다. 검토 결과, 이들 검사도구는 진단뿐 아니라 치료계획 수립, 예후 예측, 장애 정도 판정을 위한 근거자료로서도 임상적 유용성이 높다고 판단하였다.

Background The Comprehensive Language Assessment is used when a language disorder is suspected in association with various underlying conditions. It is intended to differentially diagnose the presence and type of speech sound disorder or language learning disorder, determine the severity of impairment, establish a speech-language therapy plan, and predict prognosis for recovery of communicative function. This assessment was listed as a non-covered service in 2005, prior to the introduction of the New Health Technology Assessment (nHTA) system. It was subsequently proposed by relevant organizations through the demand survey conducted under the non-covered service reporting system and, after undergoing the relevant selection procedures, was ultimately selected as a subject for health technology reassessment. At the 5th Health Technology Reassessment Committee meeting held on May 16, 2025, it was decided that this technology would be evaluated using a rapid review approach and that a subcommittee would be formed to conduct the reassessment. The present evaluation was undertaken to review the evidence regarding the target population and assessment tools used in the Comprehensive Language Assessment and to provide information that may support its appropriate use in clinical practice. In particular, the review focused on the following major categories of speech disorders: fluency disorder, articulation disorder, and voice disorder.

Objective This evaluation aimed to review the evidence regarding the target population and assessment tools of the Comprehensive Language Assessment, with particular focus on speech disorders, namely fluency disorder, articulation disorder, and voice disorder, and to provide clinically relevant information to support its use in practice.

Methods To evaluate the clinical utility of the Comprehensive Language Assessment, a stepwise approach was applied that integrated published evidence, diagnosis-related laws and guidelines, and expert clinical opinion. First, domestic textbooks in medicine and rehabilitation, major diagnostic classification systems—including the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5), the International Classification of Diseases, Tenth Revision (ICD-10), and the Korean Standard Classification of Diseases (KCD)—as well as relevant legislation were reviewed to classify the major disease categories for which the Comprehensive Language Assessment may be applicable. Next, professional texts and research literature in the fields of speech-language rehabilitation and rehabilitation medicine were reviewed to identify the types, components, and scope of assessment tools used for each disorder category. Relevant systems and guidelines concerning disability assessment and registration of language disability were also examined to identify actual patterns of use in Korean clinical practice. In addition, to examine the scientific evidence supporting the assessment tools, including their reliability and validity, domestic research literature was reviewed preferentially. When domestic evidence was limited, recent international studies were additionally consulted to examine the association between assessment tools and severity grading, as well as their use in determining eligibility for evaluation. Finally, the findings from these reviews were synthesized with legal and institutional evidence and expert clinical opinion to assess the practical applicability and clinical utility of the Comprehensive Language Assessment.

Results To identify the clinical target population for the Comprehensive Language Assessment, this evaluation reviewed nine textbooks and professional references together with major diagnostic classification systems (DSM-5, ICD-10, and KCD), the Ministry of Health and Welfare’s Disability Severity Rating Criteria, and the eligibility criteria for students requiring special education under the Enforcement Decree of the Act on Special Education for Persons with Disabilities, etc. In addition, 14 publications were reviewed, including studies analyzing the domestic clinical landscape and studies examining the reliability, validity, and evidence supporting the assessment tools used for articulation disorder, fluency disorder, and voice disorder. The review confirmed that the main categories of speech disorders to which the Comprehensive Language Assessment is applied are articulation disorder, fluency disorder, and voice disorder, all of which are classified as types of language disability under the Disability Severity Rating Criteria. Articulation disorder is defined as a motor-based disorder in which accurate production of speech sounds is impaired because of abnormalities in oral structure or articulatory muscle function, or because of inappropriate acquisition of articulation patterns. The Comprehensive Language Assessment includes the Picture Consonant Test and the Picture Phonological Process Test for the assessment of articulatory function. Under the Disability Severity Rating Criteria, the Assessment of Phonology and Articulation for Children (APAC), the Urimal Test of Articulation and Phonology (U-TAP), and the Picture Consonant Test are specified as applicable tools for determining disability severity in articulation disorder. In Korean clinical practice, U-TAP (U-TAP2), APAC, and the Korean Standardized Phonology and Articulation Profile Test (KS-PAPT) are used as standardized assessment tools. Fluency disorder is defined as a speech production disorder characterized by disruptions in the flow of speech, such as repetitions, prolongations, and blocks. The Comprehensive Language Assessment includes a stuttering assessment for the evaluation of speech fluency. The Disability Severity Rating Criteria specify the Paradise-Fluency Assessment (P-FA) as the primary assessment tool and allow the Stuttering Severity Instrument (SSI) to be used when necessary. In addition, the Disability Severity Review Regulations and the National Pension Service guidelines for disability registration also require submission of P-FA or SSI results when evaluating fluency disorder. Voice disorder refers to a condition in which characteristics of the voice, such as quality, intensity, and pitch, are altered because of structural or functional abnormalities of the vocal folds. The Comprehensive Language Assessment includes a Comprehensive Voice Evaluation (CVE), which consists of interview, assessment of oral structure and mobility, laryngoscopy, endoscopy, laryngeal stroboscopy, analysis of pitch, intensity, and voice quality, and assessment of respiratory control and phonatory efficiency. However, among these components, laryngeal endoscopy and laryngeal stroboscopy are already listed as covered services, while analysis of pitch, intensity, and voice quality overlaps with a separate non-covered service, namely the Speech and Voice Examination (No-688). Accordingly, the present evaluation focused on the non-overlapping components, namely the interview and the assessment of oral structure and mobility. The interview is used to identify information such as symptom onset, changes in symptoms, medical history, voice use habits, and environmental factors, and is useful for screening and treatment planning. Assessment of oral structure and mobility is important for identifying structural abnormalities related to phonation and for determining whether referral for specialized examination is needed.

In summary, the Comprehensive Language Assessment is used in patients with articulation disorder, fluency disorder, and voice disorder for differential diagnosis, assessment of severity, treatment planning, and prognostic evaluation. This evaluation confirmed that various standardized assessment tools are used in Korean clinical practice for these purposes. The present review therefore provides evidence on the current use and clinical basis of the Comprehensive Language Assessment, with a particular focus on these categories of speech disorders.

Conclusion Based on the findings of the present evaluation, the Health Technology Reassessment Subcommittee presented the following opinion. The Comprehensive Language Assessment is an essential assessment for patients with impairments related to speech sound accuracy, speech fluency, and voice use, and it plays an important role throughout the clinical decision-making process. In particular, it was confirmed to be useful for differentiating articulation disorder, fluency disorder, and voice disorder and for determining the severity of impairment, thereby serving as a key diagnostic tool in a wide range of clinical settings. In this evaluation, the major target conditions were classified into language disorders and speech disorders, and the category of speech disorders included articulation disorder, fluency disorder, and voice disorder. All of these disorders are classified as types of language disability under the Disability Severity Rating Criteria. In actual clinical practice, U-TAP and APAC are used for the assessment of articulation disorder, P-FA and SSI for fluency disorder, and CVE for voice disorder. Because some components of CVE overlap with existing covered and non-covered examination items, this evaluation focused primarily on the interview and oral structure/mobility assessment. The review confirmed that these tools serve important functions not only in diagnosis, but also in treatment planning, prognostic prediction, and determination of disability severity. However, because the Comprehensive Language Assessment encompasses a broad range of disease categories and assessment items, some of which overlap with other covered and non-covered services, it was suggested that the fee schedule may need to be reclassified and subdivided more rationally in the future. At the 3rd Health Technology Reassessment Expert Committee* meeting held on November 14, 2025, it was determined that the Subcommittee’s opinion regarding reclassification of the fee schedule extended somewhat beyond the scope of the present evaluation. After reviewing and revising the Subcommittee’s draft conclusion, the Committee deliberated and resolved as follows. The Comprehensive Language Assessment was judged to be an essential assessment for patients with impairments in speech production functions, including speech sound accuracy, speech fluency, and voice use, and to play an important role throughout the clinical decision-making process. In particular, the assessment was confirmed to be useful for differentiating articulation disorder, fluency disorder, and voice disorder and for evaluating the degree of impairment, and it was considered a key diagnostic tool applicable across a wide range of clinical settings. This evaluation focused on speech disorders among the major target conditions, including fluency disorder, articulation disorder, and voice disorder. All of these disorders are classified as types of language disability under the Disability Severity Rating Criteria. In actual clinical practice, U-TAP and APAC are used to assess articulation disorder, P-FA and SSI to assess fluency disorder, and CVE to assess voice disorder. The review found that these assessment tools have substantial clinical utility not only in diagnosis, but also as supporting evidence for treatment planning, prognostic prediction, and determination of disability severity.
