지속적 말초신경 및 신경총 통증(자가)조절법 - 대퇴신경

Abstract

평가배경 ‘지속적 말초신경 및 신경총 통증(자기)조절법 – 대퇴신경(이하, 지속적 대퇴신경차단술)’은 수술 후 통증 관리를 목적으로 대퇴신경 주위에 카테터를 삽입하고 국소마취제를 지속적으로 주입하여 해당 신경을 차단하는 의료기술이다. 2009년, 2015년 신의료기술평가 후 비급여로 등재된 후 2019년 선별급여 80% 항목으로 변경되어 현재까지 사용 중이다. 해당 의료기술은 선별급여 항목의 적합성평가 주기를 고려하여 내부 모니터링으로 발굴된 주제로, 2022년 제4차 의료기술재평가위원회에서 평가계획서를 심의받아 재평가를 수행하였다.

평가방법 본 평가는 슬관절전치환술 및 전방십자인대재건술 환자에서 ‘지속적 대퇴신경차단술’의 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “지속적 말초신경 및 신경총 통증(자가)조절법 – 대퇴신경 소위원회(이하 ‘소위원회’라 한다)”의 논의를 거쳐 확정하였다. 소위원회는 마취통증의학과 2인, 정형외과 2인, 재활의학과 1인, 근거기반의학 1인의 전문가 6인으로 구성하였다. 평가의 핵심질문은 “지속적 대퇴신경차단술은 슬관절전치환술 및 전방십자인대재건술 환자에서 수술 후 통증 관리를 목적으로 단독사용시 안전하고 효과적인가?”였다. 문헌검색은 국외 3개, 국내 5개 데이터베이스에서 하였으며, 문헌선정은 선택배제 기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias를 사용하여 평가하였고, 자료추출은 미리 정해놓은 자료추출 양식을 활용하여 두 명의 검토자가 독립적으로 수행하였다. 의견 불일치가 있을 경우 제3자와 함께 논의하여 합의하였다. 자료 분석은 정량적 분석이 가능할 경우 메타분석을 수행하고 불가능한 경우 질적 검토를 수행하였다. 본 평가의 체계적 문헌고찰 결과는 GRADE (Grading of Recommendations Assessment, Development and Evaluation) 방법을 이용하여 근거 수준을 평가하고, 평가결과를 토대로 권고등급을 결정하였다.

평가결과 체계적 문헌고찰 결과, 핵심질문을 충족하는 총 47개(51편 문헌)의 무작위배정 비교임상시험을 선택하였다. 연구대상자는 슬관절전치환술 연구가 42개, 전방십자인대재건술 환자 연구가 5개였으며, 대조군은 일회성 관절강내주사, 대퇴신경차단술, 지속적 경막외 주입, 정맥내 통증자가조절법, 보존적 치료, 일회성 마약성진통제주사, 경막외 통증자가조절법, 간헐적 대퇴신경차단술로 다양하였다. 비뚤림위험은 ‘연구참여자, 연구자에 대한 눈가림’ 및 ‘민간연구비 지원’ 영역에서 다소 높았다.

안전성 지속적 대퇴신경차단술의 안전성은 45개의 무작위배정 비교임상시험에서 보고하였으며, 안전성 결과는 크게 시술 관련 합병증과 약물 부작용으로 구분하여 확인하였다. 시술 관련 합병증을 보고한 27개의 연구 대부분에서 지속적 대퇴신경차단술과 다른 통증조절법들간 시술 관련 합병증 발생에 유의한 차이가 없었으나, 정맥성 혈전 문제는 1개 연구에서 지속적 대퇴신경차단술이 보존적 치료(약물치료)보다 유의하게 적었다. 시술 관련 합병증은 카테터 문제(누출, 빠짐, 이탈, 분리 등), 장비 실패, 감염/출혈, 혈전성 문제가 대부분이었으며, 재수술 및 입원, 혈전성 문제 등은 드물게 보고되었다. 약물 부작용은 해당 시술에 사용되는 국소마취제로 인한 부작용 이외 통증 조절을 위해 추가 투여하는 마약성 진통제의 부작용이 혼재되었으나, 이를 구분하는 것이 쉽지 않아 관련 부작용들을 모두 포함하였다. 오심 및 구토, 가려움증은 마약성 진통제의 대표적 부작용으로 지속적 대퇴신경차단술과 직접적인 관련성은 낮다고 볼 수 있다. 약물 부작용을 보고한 44개의 연구 대부분에서 지속적 대퇴신경차단술과 대조군간 유의한 차이가 없었으나, 일부 연구에서는 해당 의료기술에서 오심 및 구토 발생이 유의하게 덜 발생하였고 감각이상(numbness)은 지속적 경막외 주입술보다 더 많이 발생하였다. 오심 및 구토, 가려움증, 신경학적 후유증(진정, 졸음, 어지러움 등), 감각이상(weakness, numbness)이 주요하게 나타났으며, 낙상 및 호흡 관련 증상 등이 드물게 보고되었다.

효과성 지속적 대퇴신경차단술의 효과성은 47개의 무작위배정 비교임상시험에서 보고하였으며 효과성 결과는 크게 5개로 나누어 통증 정도, 수술 후 진통제 사용량, 기능(재활)지표, 환자만족도, 재원기간을 확인하였고, 결과지표는 대조군별로 나누어 확인하였다. 통증 정도는 47개의 연구 모두에서 보고하였다. 지속적 대퇴신경차단술은 보존적 치료보다(5/5개 연구) 최소 한 시점에서 통증 감소의 효과가 더 좋았고, 정맥내 통증자가조절법과 비교한 대부분의 연구(4/6개)에서도 해당 의료기술이 유의하게 통증을 감소시켰다. 일회성 대퇴신경차단술과 비교시 두 군간 유의한 차이가 없거나(6/10개) 해당 의료기술이 더 효과적(4/10개)이었다. 지속적 경막외 주입 또는 일회성 관절강내 주사와의 비교연구에서는 연구결과들이 혼재되어 일관성이 없었다. 지속적 경막외 주입과 비교한 연구에서는 지속적 대퇴신경차단술군(4/9개) 또는 대조군(2/9개)에서 더 효과적이거나 두 군간 유의한 차이가 없었으며(3/9개), 일회성 관절강내 주사와 비교한 연구에서도 유사하게 중재군(3/10개) 또는 대조군(4/10개)이 더 효과적이거나 두 군간 유의한 차이가 없었다(3/10개). 그 외 일회성 마약성진통제 주사, 경막외 통증자가조절법, 간헐적 대퇴신경차단술, 병용치료와 비교한 연구는 각 2개로 연구결과가 각각 달랐다. 46개의 연구에서 확인한 수술 후 진통제 사용량 결과는 통증 정도 결과와 유사하였다. 지속적 대퇴신경차단술은 보존적 치료(5/5개 연구) 및 정맥내 통증자가조절법(6/6개 연구)보다 최소 한 시점에서 수술 후 진통제 사용량이 유의하게 더 적었으며, 일회성 대퇴신경차단술과 비교시 두 군간 유의한 차이가 없거나(7/10개) 해당 의료기술이 더 효과적(3/10개)이었다. 그러나 간헐적 대퇴신경차단술과 비교한 2개의 연구에서는 대조군의 수술 후 진통제 사용량이 유의하게 더 적었다. 지속적 경막외 주입과 일회성 관절강내 주사와의 비교연구에서는 연구결과들이 혼재되어 일관성이 부족하였고, 두 군간 유의한 차이가 없거나 중재군 또는 대조군이 더 효과적이라는 연구가 비슷하게 보고되었다. 그 외 일회성 마약성진통제 주사, 경막외 통증자가조절법, 병용치료와 비교한 연구는 각 2개로 연구결과가 각각 달랐다. 37개의 연구에서 기능(재활)지표를 분석한 결과, 보존적 치료 및 정맥내 통증자가조절법과 비교한 연구의 대부분에서는(2/3개, 3/5개) 지속적 대퇴신경차단술의 수술 후 기능지표가 유의하게 개선되었다. 일회성 관절강내주사와 비교한 연구는 두 군간 유의한 차이가 없거나 중재군에서 기능지표가 더 개선된다는 연구가 각 5개, 대조군이 더 효과적이라는 연구가 1개였다. 그 외 일회성 대퇴신경차단술, 간헐적 대퇴신경차단술, 지속적 경막외 주입, 병용치료와 비교한 연구에서 기능지표는 두 군간 유의한 차이가 없었으며, 경막외 통증자가조절법과 비교한 1개의 연구에서는 지속적 대퇴신경차단술이 기능지표를 유의하게 개선시켰다. 23개의 연구에서 환자만족도 결과를 보고하였다. 지속적 대퇴신경차단술은 보존적 치료(3/3개 연구) 및 일회성 마약성진통제 주사(1/1개 연구)보다 환자만족도가 유의하게 더 높았으나, 병용치료(1/1개 연구)보다는 유의하게 더 낮았다. 그 외 일회성 대퇴신경차단술, 일회성 관절강내주사, 정맥내 통증자가조절법, 경막외 통증자가조절법, 간헐적 대퇴신경차단술, 지속적 경막외 주입과 비교한 연구의 대부분에서는 두 군간 유의한 차이가 없었다. 18개의 연구에서는 재원기간을 보고하였으며, 지속적 대퇴신경차단술은 대조군에 상관없이 모든 연구에서 중재군과 대조군간 재원기간에 유의한 차이가 없었다. 연구결과 지표별로 확인한 평가결과의 GRADE 근거수준은 대부분의 결과지표에서 중등도(moderate)로 판단되었으나, 통증 점수는 메타분석 결과 이질성이 증등도 이상으로 확인되어 근거수준을 낮음(low)으로 평가하였다.

결론 및 제언 해당 소위원회는 현재 평가결과에 근거하여 다음과 같이 제언하였다. 체계적 문헌고찰 결과, 지속적 대퇴신경차단술은 시술 관련 합병증 및 약물 부작용 발생에 있어 카테터 관련 문제, 오심 및 구토, 감각이상 등이 보고되었으나, 대부분의 연구에서 다른 통증조절법(대조군)과 비교시 유의한 차이가 없어 안전한 의료기술로 평가하였다. 지속적 대퇴신경차단술은 통증 정도 및 진통제 사용량에 있어 보존적 치료, 정맥내 통증자가조절법보다는 더 효과적이었으나, 다른 통증조절법(일회성 대퇴신경차단술, 일회성 마약성 진통제 주사, 일회성 관절강내주사, 경막외 통증자가조절법, 간헐적 대퇴신경차단술, 지속적 경막외 주입, 병용치료)과 비교시 유의한 차이가 없거나 상반된 연구결과가 혼재되어 있었다. 기능(재활)지표, 환자만족도 및 재원기간은 대조군에 상관없이 대부분의 연구에서 두 군간 유의한 차이가 없었으며, 일부 연구에서 상반된 연구결과가 혼재되어 있었다. 따라서 소위원회에서는 본 평가결과를 바탕으로 슬관절전치환술 및 십자인대재건술 환자에서 수술 후 통증 관리를 목적으로 사용하는 ‘지속적 말초신경 및 신경총 통증(자기)조절법 – 대퇴신경’은 안전한 기술이며, 다른 통증조절법과 효과가 유사한 치료대안 중 하나로 판단하였다. 2023년 제2차 의료기술재평가위원회(2023.02.10.)에서는 의료기술재평가사업 관리지침 제4조제10항에 의거 ‘지속적 말초신경 및 신경총 통증(자가)조절법 – 대퇴신경’에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 슬관절전치환술 및 전방십자인대재건술 환자에서 ‘지속적 말초신경 및 신경총 통증(자가)조절법 – 대퇴신경’은 안전한 기술이며, 다른 통증조절법과 유사한 의료기술로 ‘조건부 권고함’으로 심의하였다.

Background Continuous (patient-controlled) analgesia of peripheral nerve or plexus – femoral nerve (hereafter referred to as “continuous femoral nerve block; CFNB”) is a medical technique that involves the placement of a catheter adjacent to the femoral nerve for ongoing administration of a local anesthetic to manage postoperative pain. After its designation as a non-beneficial service following health technology assessments in 2009 and 2015, CFNB was reclassified as a selective benefit with an 80% patient co-payment requirement in 2019, a status which it retains presently. This technique was earmarked for reassessment through routine internal surveillance, prompted by the scheduled review cycle for selective benefit compliance. Subsequently, the re-evaluation was carried out in accordance with the reassessment agenda discussed during the fourth meeting of the Committee of Health Technology Reassessment in 2022.

Methods A systematic review was conducted to evaluate the safety and efficacy of CFNB in patients who had undergone total knee replacement (TKR) or anterior cruciate ligament (ACL) reconstruction. The methodology for the assessment was solidified following deliberations by the CFNB subcommittee (hereafter referred to as “Subcommittee”), with careful consideration for the evaluative process. This Subcommittee was comprised of six experts: two each from anesthesiology and pain medicine and orthopedic surgery and one each from rehabilitation medicine and evidence-based medicine. The central inquiry of the assessment was: “Is CFNB alone a safe and effective modality for managing postoperative pain in patients post-TKR or ACL reconstruction?” In conducting the literature review, three international and five domestic databases were queried. Two independent reviewers then screened and selected studies based on predefined inclusion and exclusion criteria. Additionally, the Cochrane risk of bias (RoB) tool was employed to appraise the potential bias of the studies, and data extraction was independently conducted by two reviewers using a standardized template. Any disagreements between reviewers were resolved through discussion, or if necessary, consultation with a third party. When feasible, a meta-analysis was undertaken to quantitatively synthesize the data; otherwise, a qualitative synthesis was utilized. The evidence from the systematic review was rated using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) approach, and the strength of the recommendations was established in accordance with these findings.

Results The systematic review included a total of 47 randomized controlled trials (RCTs), encompassing 51 articles, that met the criteria of the key question. Of these, 42 trials involved patients who had TKR surgery, and five focused on patients who had ACL reconstruction. The comparator interventions (controls) in these studies were varied, featuring single-shot intra-articular injections, single-shot femoral nerve blocks (FNBs), continuous epidural infusions, patient-controlled intravenous analgesia, conservative therapy, single-shot narcotic analgesic injections, patient-controlled epidural analgesia, and intermittent FNBs. The RoB was determined to be moderately high, particularly in the domains concerning patient/assessor blinding and the influence of private research funding.

Safety The safety of CFNB was documented in 45 RCTs. The evaluation of safety outcomes was based on procedure-related complications (PRCs) and adverse drug reactions (ADRs). In the 27 studies that provided data on PRCs, the majority found no significant differences in the incidence of PRCs between CFNB and other pain management techniques. However, one study indicated a notably lower incidence of PRCs in the CFNB group compared to the group receiving conservative treatment (medication therapy). Reported PRCs predominantly consisted of catheter-related issues (such as leakage, dislodgement, displacement and disconnection), equipment malfunctions, infections/bleeding, and thrombotic events. Incidences of reoperation, hospitalization, and thrombotic events were infrequent. As for ADRs, these encompassed adverse events (AEs) stemming from the local anesthetics used in CFNB, as well as those AEs linked to additional narcotic analgesics administered given pain management. Distinguishing the sources of AEs proved challenging; therefore, all pertinent AEs were considered. Typical AEs associated with narcotic analgesics included nausea, vomiting, and pruritus, which seemed minimally connected to CFNB. Out of the 44 studies examining ADRs, most did not find significant discrepancies in AEs between the CFNB and control cohorts. Nonetheless, some studies noted a lower frequency of nausea and vomiting in the CFNB group, while paresthesia (numbness) occurred more frequently compared to the continuous epidural infusion group. Commonly reported AEs were nausea, vomiting, pruritus, neurologic symptoms (such as sedation, lethargy and vertigo), and paresthesia (including weakness and numbness), while falls and respiratory issues were rarely reported.

Effectiveness The effectiveness of CFNB was reported in 47 RCTs. These RCTs measured effectiveness across five domains: pain severity, consumption of postoperative analgesics, functional rehabilitation metrics, patient satisfaction, and hospital stay duration. Outcome measures were analyzed separately across various control interventions. All 47 studies reported on pain severity, with findings indicating that CFNB was generally more effective at reducing pain at certain time points compared to conservative treatment, as shown in all five studies addressing this comparison. When CFNB was contrasted with patient-controlled intravenous analgesia, the majority of studies (4 out of 6) noted a significant reduction in pain with CFNB. Against a single-shot FNB, no significant difference was reported in the majority of studies (6 out of 10), while the remainder (4 out of 10) favored CFNB. Results were mixed and inconclusive when CFNB was compared to continuous epidural infusion or single-shot intra-articular injections. In the case of continuous epidural infusion, some studies (4 out of 9) found CFNB more effective, a minority (2 out of 9) reported the opposite, and the rest (3 out of 9) detected no significant difference. Similar variability was seen with single-shot intra-articular injection comparisons: CFNB was more effective in some studies (3 out of 10), less effective in others (4 out of 10), and equivalently effective in the remainder (3 out of 10). There were two studies each on comparison to the single-shot narcotic analgesic injection, patient-controlled epidural analgesia, intermittent FNB, and concomitant treatment, which reported different results. Regarding the use of postoperative analgesics, reported in 46 studies, the trends paralleled those observed for pain severity. The CFNB group required significantly fewer analgesics at various postoperative stages compared to the conservative treatment group (in all 5 studies) and the patient-controlled intravenous analgesia group (in all 6 studies). When compared to single-shot FNB, the majority of studies (7 out of 10) found no significant difference, while a few (3 out of 10) found CFNB more effective. For the two studies comparing CFNB with intermittent FNB, the control group used less postoperative analgesia. Mixed and inconsistent results were also reported for comparisons with continuous epidural infusion or single-shot intra-articular injections. As with pain severity, studies reported no clear trend, with some indicating no significant difference, and an equal number suggesting one group was more effective than the other. There were two studies each on comparison to the single-shot narcotic analgesic injection, patient-controlled epidural analgesia, and concomitant treatment, which reported different results. In the analysis of functional (rehabilitation) outcomes from 37 studies, the majority comparing CFNB with both conservative treatment and patient-controlled intravenous analgesia (in 2 out of 3 and 3 out of 5 studies, respectively) indicated significant improvement in postoperative functional metrics with CFNB. Regarding comparisons with the single-shot intra-articular injection, five studies found either no significant difference or reported better functional outcomes with CFNB, while one study favored the control. Other comparisons, including those with single-shot and intermittent FNB, continuous epidural infusion, and concomitant therapies, generally showed no significant difference in functional outcomes. However, in one study where CFNB was measured against patient-controlled epidural analgesia, CFNB was noted to significantly improve these metrics. Patient satisfaction, assessed in 23 studies, was significantly higher in the CFNB group compared to those receiving conservative treatment (in all three relevant studies) and the single-shot narcotic analgesic injection group (1 out of 1 study). However, satisfaction was lower compared to the concomitant treatment group (1 out of 1 study). The majority of studies found no significant differences in patient satisfaction when CFNB was compared with single-shot FNB, single-shot intra-articular injection, patient-controlled intravenous or epidural analgesia, intermittent FNB, and continuous epidural infusion. Hospital stay duration, evaluated in 18 studies, revealed no significant differences between groups receiving CFNB or any other interventions. The GRADE methodology rated most outcome indicators' evidence as moderate. However, the evidence level for pain scores was considered low due to moderate-to-high heterogeneity in the meta-analysis results.

Conclusions and recommendations Upon reviewing the results of the systematic review, the Subcommittee offers the following recommendations: The systematic review indicated that CFNB is associated with certain PRCs and ADRs such as catheter issues, nausea, vomiting, and paresthesia. Despite these findings, CFNB is deemed to be a safe medical modality, as the majority of studies found no significant difference in safety compared to other pain management methods. In terms of pain severity and analgesic consumption, CFNB proved more effective than conservative treatment and patient-controlled intravenous analgesia. Comparisons with other pain management modalities, including single-shot FNB, single-shot narcotic analgesic injection, single-shot intra-articular injection, patient-controlled epidural analgesia, intermittent FNB, continuous epidural infusion, and concomitant treatments, revealed no significant differences or yielded inconsistent results. Similarly, CFNB did not show significant differences in functional outcomes, patient satisfaction, or length of hospital stay against most control methods, although some studies did report variable outcomes. Based on these findings, the Subcommittee concludes that CFNB is a viable and safe option for managing postoperative pain in patients following TKR and ACL reconstruction. It offers comparable efficacy to other pain management strategies. During the first session of the Committee of Health Technology Reassessment in 2023 (on January 13, 2023, in alignment with Article 4-10 of the Guidelines for the Management of Health Technology Reassessment, the Committee “conditionally recommends”the use of CFNB, recognizing it as a safe option that provides similar effectiveness to other pain management approaches. This endorsement is contingent on the clinical context in Korea, considering the aggregate evidence of clinical safety, efficacy, and other evaluative criteria.
