Abstract

평가배경 신경발달중재치료(Neurodevelopmental Intervention Therapy)는 통상적인 인지행동치료가 불가능한 신경심리발달 문제를 보이는 소아·청소년을 대상으로, 지연되거나 손상된 인지, 언어 및 의사소통, 사회성․놀이, 주의력, 학습, 작업 능력 등 다양한 발달 영역을 증진시키기 위한 치료․교육적 접근 방법이다. 동 기술과 관련하여 임상현장에서 활용 가능한 정보를 제공하기 위하여 2025년 제5차 의료기술재평가위원회(2025. 5. 16.)에서 재평가 계획서 및 소위원회 구성안을 심의한 후 재평가를 수행하였다.

평가목적 본 평가는 ‘기타 행동치료-신경발달중재치료’의 사용대상 및 중재치료에 대한 근거를 검토하여, 임상현장에서의 활용에 도움이 될 수 있는 관련 정보를 제공하는 것을 목적으로 한다.

평가방법 ‘기타 행동치료-신경발달중재치료’의 근거를 검토하기 위한 세부 방법은 평가목적을 고려하여 “신경발달중재치료 소위원회(이하 ’소위원회‘라 한다)“의 논의를 통해 확정하였다. 소위원회는 재활의학과 2인, 신경과 1인, 정신건강의학과 1인, 소아청소년과 1인의 전문가 5인으로 구성하였다. 본 평가에서는 임상적으로 신경발달중재치료가 필요한 대상자와 중재 방법을 확인하기 위하여 국내외 문헌 및 교과서를 검토하였다. 먼저, 발달정밀평가 안내서, 관련 법령, 진단분류체계(Diagnostic and Statistical Manual of Mental Disorders, 5th Edition (DSM-5), International Classification of Diseases, 10th Revision (ICD-10)) 및 교과서를 토대로 신경발달중재치료의 주요 적용대상을 확인하였다. 다음으로, 신경발달중재치료의 중재 유형을 파악하기 위하여 교과서와 관련 전문서적을 검토하고, 소위원회 논의를 거쳐 대표적인 중재를 선정하였다. 이후 각 중재의 세부 특성을 확인하기 위하여 Google과 PubMed에서 체계적 문헌고찰 및 종설 등을 검색·검토하였다. 또한, 임상현장의 실제 사용 현황을 파악하기 위해 유관 전문학회(대한소아신경학회, 대한소아재활·발달의학회, 대한소아청소년정신의학회, 대한소아청소년과학회)로부터 중재유형 및 적용대상, 임상적 필요성 등에 대한 의견을 수렴하였다

평가결과 신경발달중재치료는 신경심리발달 문제를 가진 소아·청소년을 대상으로 개별 발달 단계와 특성에 맞춰 시행되는 맞춤형 집중재활치료를 포괄한다. 질병관리청의 「발달정밀평가 안내」, 「발달장애인 권리 보장 및 지원에 관한 법률」에 제시된 발달장애인 정의, DSM-5 및 ICD-10의 진단분류를 종합하여 검토한 결과, 본 치료의 주요 적용대상은 전반적 발달지연(Global Developmental Delay, GDD), 지적장애(Intellectual Disability, ID), 자폐스펙트럼장애(Autism Spectrum Disorder, ASD), 주의력결핍·과잉행동장애(Attention-Deficit/Hyperactivity Disorder, ADHD), 의사소통장애 및 협응성운동발달장애(Developmental Coordination Disorder, DCD) 등이었다. 또한 발달지연 및 조산아와 같은 발달지연 고위험군 영유아도 주요 개입 대상에 포함하였다. 각 질환들은 공통적으로 조기 발견과 다학제적 개입, 발달 단계에 맞춘 개별화된 치료 접근이 필수적이라는 점이 강조되었다.

신경발달중재치료에 활용되는 대표적 중재유형으로는 감각통합치료, 사회기술훈련, 놀이치료, 응용행동분석(Applied Behavior Analysis, ABA), Treatment and Education of Autistic and Related Communication Handicapped Children (TEACCH) 프로그램, 미술·음악치료, 뉴로피드백, 인지치료, 또래활용중재, 부모중재 등이 확인되었다. 감각통합치료는 감각처리 이상을 완화하고 사회성·운동기술을 향상시키는 데 활용되며, 일부 연구에서 긍정적 효과가 보고되었으나 근거는 제한적이었다. 사회기술훈련은 자폐스펙트럼장애 아동의 사회적 반응성과 상호작용 능력 향상에 유의한 효과를 보였으며, 부모참여형 프로그램이 특히 긍정적인 결과를 나타냈다. 놀이치료는 언어, 정서, 사회성 발달을 촉진하고 행동 문제를 완화하는 효과가 보고되었다. 응용행동분석(ABA)은 조기집중행동중재(Early Intensive Behavioral Intervention, EIBI)를 중심으로 인지 및 적응행동 향상에 효과가 있으나, 장기적 지속효과는 불확실한 것으로 평가되었다. TEACCH 프로그램은 ASD 핵심증상 개선, 사회적 기술과 인지·소근육 기능 향상에 도움을 주었으나, 언어 및 대근육 기능 향상 효과는 제한적이었다. 미술치료와 음악치료는 ASD 등에서 사회성·정서·인지 영역 개선에 유의한 결과를 보였고, 뉴로피드백은 ADHD 아동의 부주의 증상 완화에 효과가 있었다. 인지치료는 ADHD 소아·청소년에서 ADHD 증상, 수행기능, 학업성취, 과제이탈행동 감소, 사회적 기술 향상 등에서 긍정적 효과를 보였고, 또래활용중재는 지적·발달장애 아동의 사회적 상호작용 향상에, 부모중재는 부 모-자녀 관계와 아동의 의사소통 능력 개선에 긍정적 효과가 확인되었다.

유관 전문학회들은 신경발달중재치료의 임상적 필요성을 공통적으로 강조하였다. 특히 발달장애나 발달지연이 있는 아동 및 청소년에게 조기 개입은 장애의 최소화 및 사회적 적응 향상을 위한 핵심적 요인임을 강조하였다. 치료는 대체로 주 1~4회, 회당 30~50분 단위의 개별치료로 이루어지며, 아동의 연령, 발달 수준, 증상의 특성에 맞춘 통합적․다학제적 접근의 필요성을 제시하였다. 또한, 신경발달중재치료는 발달장애 및 정서․행동상의 어려움을 지닌 소아·청소년에게 필수적이고 불가결한 개입으로 평가하였고, 다양한 치료 중재 기법을 적용하기 위해서 적절한 수가체계의 필요성도 언급하였다. 그리고 치료효과의 극대화를 위해서는 의학, 심리, 교육, 재활치료를 포함한 다학제적 접근이 필요하고, 발달 관련 전문과의 관리하에 시행되는 것이 바람직하다는 의견을 제시하였다.

종합하면, 신경발달중재치료는 발달지연 및 발달장애 아동의 인지, 언어, 사회성, 정서 기능 향상에 도움이 되는 근거 기반 치료 접근으로 평가된다. 특히 조기 발견과 지속적 개입이 예후 향상에 결정적인 역할을 하며, 향후 중재 유형별로 근거 수준을 강화하기 위한 체계적 연구와 표준화된 치료 프로토콜 마련이 필요하다.

결론 및 제언 의료기술재평가 소위원회에서는 현재 평가결과에 근거하여 다음과 같은 의견을 제시하였다. 신경발달중재치료는 조기 치료가 중요한 소아·청소년의 발달적 특성과 환경적 요인을 종합적으로 고려하여 포괄적으로 시행되는 특성으로 인해, 특정 질환이나 중재 유형을 명확히 한정하기 어려운 한계가 있다. 따라서, 본 평가에서는 신경발달중재치료의 주요 적용 대상 질환으로 전반적 발달지연, 지적장애, 자폐 스펙트럼 장애, 주의력결핍·과잉행동장애(ADHD), 의사소통장애, 협응성운동발달장애, 기타 발달지연 및 발달지연 고위험군(조산아 등)을 포함하였고, 주요 중재 유형으로는 감각통합치료, 놀이치료, 사회기술훈련, 응용행동분석(ABA), TEACCH, 미술치료, 음악치료, 뉴로피드백, 인지치료, 또래활용중재, 부모교육 및 부모중재 등이 있었다. 각 중재 유형별 근거를 검토한 결과, 아동의 특성에 따라 개별 중재 방법의 근거 수준이 상이하고, 다수의 문헌에서 연구 규모의 제한, 중재 시행 방법의 표준화 부족, 장기 추적 결과의 부재 등의 공통적인 한계가 지적되었다. 이에 따라 아동의 개별적 상황, 이용 가능한 진단 및 치료 옵션, 접근 가능한 자원 등을 종합적으로 고려하여 가장 적절한 치료를 선택하고 실행되어야 한다는 점을 강조하였으며, 특히, 소아·청소년의 발달과 관련된 전문과의 관리하에 신경발달중재치료가 시행될 필요성이 있다. 다만, 소위원회에서는 본 보고서에서 검토한 신경발달중재치료의 중재유형 외에도 다양한 치료가 있을 수 있으며, 개별 아동의 특성과 환경적 요인을 종합적으로 고려한 합리적 치료선택이 중요하다고 제언하였다.

Background Neurodevelopmental Intervention Therapy (NIT) is a therapeutic and educational approach aimed at enhancing various developmental areas, including cognition, language, communication, social/play skills, attention, learning, and occupational performance. This approach is designed for children and adolescents with neuropsychological developmental issues when conventional cognitive-behavioral therapy is not suitable. To provide valuable clinical insights, a reassessment was conducted following the established protocol and the composition of the Subcommittee at the 5th Health Technology Reassessment Committee Meeting on May 16, 2025.

Objective The objective of this reassessment was to review the evidence regarding the target populations and intervention modalities of “Other Behavioral Therapy – Neurodevelopmental Intervention Therapy” and to provide relevant information to support its use in clinical practice.

Methods The specific methodology for reviewing evidence was finalized through discussions by the Neurodevelopmental Intervention Therapy Subcommittee (hereafter "the Subcommittee"). The Subcommittee was composed of five experts: two in rehabilitation medicine, and one each in neurology, psychiatry, and pediatrics. The assessment involved a review of domestic and international literature and textbooks to identify clinically necessary target populations and intervention modalities. Primary target populations were identified based on developmental assessment guidelines, relevant legislation, diagnostic classification systems, including the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition (DSM-5), and the International Classification of Diseases, 10th Revision (ICD-10), and textbooks. Representative interventions were selected through Subcommittee discussions. Subsequently, systematic reviews and narrative reviews were searched via Google and PubMed to verify the characteristics of each intervention. Additionally, expert opinions on intervention types, target populations, and clinical necessity were collected from four major academic societies, including the Korean Child Neurology Society, the Korean Academy of Pediatric Rehabilitation and Developmental Medicine, the Korean Academy of Child and Adolescent Psychiatry, and the Korean Pediatric Society.

Results Neurodevelopmental Intervention Therapy encompasses individualized and intensive rehabilitation approaches tailored to the developmental stage and characteristics of children and adolescents with neuropsychological developmental problems.

Based on a comprehensive review of the Korea Disease Control and Prevention Agency’s “The National Screening Program Guideline on a Child with Developmental Delay”, the Act on Guarantee of Rights and Support for Persons with Developmental Disabilities, DSM-5, and ICD-10 classifications, the major target populations included Global Developmental Delay (GDD), Intellectual Disability (ID), Autism Spectrum Disorder (ASD), Attention-Deficit/Hyperactivity Disorder (ADHD), communication disorders, and Developmental Coordination Disorder (DCD). Infants and toddlers at high risk for developmental delay, such as preterm infants, were also included as key target populations. Across these conditions, early identification, multidisciplinary intervention, and individualized treatment approaches appropriate to developmental stages were emphasized as essential. Representative intervention modalities identified for Neurodevelopmental Intervention Therapy included sensory integration therapy, social skills training, play therapy, Applied Behavior Analysis (ABA), the Treatment and Education of Autistic and Related Communication Handicapped Children (TEACCH) program, art and music therapy, neurofeedback, cognitive therapy, peer-mediated intervention, and parent-mediated intervention. Sensory integration therapy was used to alleviate sensory processing abnormalities and improve social and motor skills, although evidence was limited despite some positive findings. Social skills training demonstrated significant effects on improving social responsiveness and interaction abilities in children with ASD, particularly in parent-participatory programs. Play therapy was reported to promote language, emotional, and social development while reducing behavioral problems. ABA, particularly Early Intensive Behavioral Intervention (EIBI), showed effectiveness in improving cognition and adaptive behavior, although long-term sustainability remained uncertain. The TEACCH program improved core ASD symptoms as well as social, cognitive, and fine motor skills, while evidence for improvements in language and gross motor function was limited. Art and music therapy demonstrated beneficial effects on social, emotional, and cognitive domains in ASD, and neurofeedback showed positive effects on inattentive symptoms in children with ADHD. Cognitive therapy demonstrated beneficial effects in children and adolescents with ADHD, including reductions in ADHD symptoms, executive dysfunction difficulties, academic difficulties, and off-task behavior, as well as improvements in social skills. Peer-mediated intervention improved social interaction in children with intellectual and developmental disabilities, while parent-mediated intervention positively affected parent-child relationships and children’s communication abilities. Related academic societies consistently emphasized the clinical necessity of Neurodevelopmental Intervention Therapy. In particular, they highlighted that early intervention is a critical factor in minimizing disability and improving social adaptation in children and adolescents with developmental disabilities or developmental delay. Treatment was generally provided as individualized therapy sessions conducted one to four times per week for 30–50 minutes per session, and the need for integrated and multidisciplinary approaches tailored to age, developmental level, and symptom characteristics was emphasized. The societies also regarded Neurodevelopmental Intervention Therapy as an essential and indispensable intervention for children and adolescents with developmental and emotional-behavioral difficulties and noted the need for an appropriate reimbursement system to support diverse intervention modalities. Furthermore, they suggested that multidisciplinary approaches involving medicine, psychology, education, and rehabilitation therapy are necessary to maximize treatment effectiveness and that these therapies should ideally be implemented under the supervision of specialists in developmental disorders. Overall, Neurodevelopmental Intervention Therapy was assessed as an evidence-based therapeutic approach that contributes to improvements in cognition, language, social skills, and emotional functioning in children with developmental delay and developmental disorders. Early identification and continuous intervention play decisive roles in improving prognosis, and further systematic research and standardized treatment protocols are needed to strengthen the evidence base for individual intervention modalities.

Conclusion and Recommendations Based on the current evidence, the Subcommittee of the Health Technology Reassessment Committee presented the following conclusions. Due to the comprehensive nature of Neurodevelopmental Intervention Therapy, which reflects the developmental characteristics and environmental factors of children and adolescents who require early treatment, it is difficult to limit the therapy to specific diseases or intervention modalities clearly. Accordingly, this reassessment included major target conditions such as Global Developmental Delay, Intellectual Disability, Autism Spectrum Disorder, ADHD, communication disorders, Developmental Coordination Disorder, and other developmental delays or high-risk groups for developmental delay (e.g., premature infants).
