SFLT-1/PLGF [정밀면역검사] (정량)

Abstract

평가배경 SFLT-1/PLGF [정밀면역검사] (정량)은 임신 20-34주 사이 전자간증이 의심되는 임산부를 대상으로 전자간증의 예측 및 진단을 위해 실시하는 검사이다. 해당 검사는 2016년 제5차 신의료기술평가위원회(2016.5.27.)에서 안전성 및 유효성이 있는 기술로 심의(보건복지부 고시 제2016-122호, 2016.7.5.)되었으며 2017년 선별급여(50%)로 등재되었다(보건복지부 고시 제2017-151호, 2017.9.1.). 본 기술은 선별급여 적용 후 적합성 평가주기(3년)이 도래하여 재평가를 통해 해당 의료기술의 임상적 안전성 및 효과성에 대한 최신 근거를 제공하고자 내부모니터링을 통해 재평가 안건으로 발굴되었으며, 임상자문 및 의료기술재평가위원회의 우선순위 심의를 거쳐 재평가가 필요한 기술로 선정되었다. 2022년 제6차 의료기술재평가위원회(2022.6.10.)에서 평가계획서를 심의한 후 재평가를 수행하였다.

평가 목적 및 방법 SFLT-1/PLGF [정밀면역검사] (정량)에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “SFLT-1/PLGF [정밀면역검사] (정량)에 대한 안전성 및 효과성 평가 소위원회(이하 ‘소위원회’라 한다)”의 심의를 거쳐 확정하였다. 소위원회 구성은 산부인과 2인, 진단검사의학과 2인, 근거기반의학 1인의 전문가 5인으로 구성하였다. 평가의 핵심질문은 ‘SFLT-1/PLGF [정밀면역검사] (정량)은 전자간증이 의심되는 20-34주 임산부를 대상으로 전자간증을 예측 및 진단하는 데 안전하고 효과적인 검사인가?’ 이다. 위 핵심질문을 토대로 국외 데이터베이스 3개(Ovid MEDLINE, Ovid EMBASE, Cochrane Central Register of Controlled Trials) 및 국내 데이터베이스 5개(KoreaMed, 의학논문데이터베이스, 학술데이터베이스, 한국교육학술정보원, 과학기술정보통합서비스)을 이용하여 문헌을 검색하였다. 문헌 선정기준 및 배제기준 적용을 통한 문헌선택과 자료추출, 비뚤림위험 평가는 모두 2명의 평가자가 독립적으로 수행하였고, 의견 불일치가 있을 경우 논의하여 합의하였다. 문헌의 비뚤림위험 평가는 Quality assessment of diagnostic accuracy studies-2 (QUADAS-2)을 사용하였다. 정량적 분석이 가능한 부분은 양적분석(메타분석)을 수행하였고, 불가능한 경우 질적 검토 방법을 적용하였다.

평가 결과 체계적 문헌고찰 결과 선택된 문헌은 총 38편(진단법평가연구 26편, 전향적 코호트연구 2편, 후향적 코호트 연구 2편, 경제성평가연구 8편)이었으며, 안전성 및 효과성 결과로 진단정확도, 질환과의 관련성을 보고하였다. 진단법 평가연구에 해당하는 문헌 29편에 대해 비뚤림위험을 평가한 결과 참고표준검사와 환자 선택 적용성 우려 부분에서는 모든 문헌에서 ‘낮음’으로 평가하였고, 중재검사의 적용성 우려 부분에서는 문헌의 69%에서 ‘낮음’, 31%에서 ‘불확실’로 평가하였다. 연구진행과 시점의 비뚤림위험은 모든 문헌에서 ‘낮음’으로 평가하였고, 참고표준검사의 비뚤림위험에 대해서는 중재검사에 결과 정보가 없이 참고표준검사 결과 해석을 하였는지 확실하지 않은 문헌 17.2%에서 ‘불확실’로, 나머지 82.8%에서는 ‘낮음’으로 평가하였다. 중재검사의 비뚤림위험에 대해서는 임계치가 사전에 명시되었는지 확실하지 않은 문헌 20.7%에서 ‘불확실’로 평가하였고, 환자선택 비뚤림위험에서는 연속적 표본 또는 무작위 표본이 확실하지 않아 ‘불확실’로 평가한 문헌이 전체 문헌의 51.7% 였다.

안전성 SFLT-1/PLGF [정밀면역검사] (정량)의 안전성은 검사 관련 이상반응을 보고한 1편을 기준으로 평가하였고, 동 문헌에서 검사로 인한 이상반응이 발생하지 않았다고 보고하였다.

효과성 SFLT-1/PLGF [정밀면역검사](정량)의 효과성은 전자간증 발생 예측, 전자간증 진단정확성, 의료결과와의 관련성(출산 예측, 입원 예측)을 보고한 29편을 기준으로 평가하였다. 29편을 기준으로 평가하였다. 효과성 검토 시 전자간증 발생 예측과 전자간증 진단정확도는 NICE 가이드라인(2022)에서 제시한 임계치를 기준으로 하였으며, 합성이 가능할 경우 메타분석을 통한 통합값을 제시하였으며, 합성이 불가할 경우에는 진단정확도의 범위를 기술하였다. 1) 전자간증 예측 전자간증 예측을 보고한 18편의 문헌에서는 검사 후 발병시기 예측, 임신 주수를 기준으로 한 예측, 기간을 따로 명시하지 않은 전자간증 예측 결과를 구분하여 제시하였다. 검사 1주 이내 전자간증이 발생하지 않을 것을 예측한 결과는 11편에서 보고하였다. 이때 동 검사의 민감도 0.63~1.00, 특이도 0.63~0.85, 양성예측도 0.09~0.78, 음성예측도 0.80~1.00이었다. NICE 가이드라인에서 제시한 임계치 38을 기준으로 합성 가능한 6편을 메타분석 한 결과 통합 민감도 0.92 (95% CI 0.80-0.97, I2=61.7%), 통합 특이도 0.80 (95% CI 0.78-0.82, I2=0.0%), 통합 AUC 0.82 (95% CI 0.79-0.85) 였다. 검사 4주 이내 전자간증 발생 예측은 9편에서 보고하였다. 이때 동 검사의 민감도 0.5~1.00, 특이도 0.69~0.88, 양성예측도 0.18~0.59, 음성예측도 0.84~1.00 이었다. NICE 가이드라인에서 제시한 임계치 38을 기준으로 합성 가능한 4편을 메타분석 한 결과 통합 민감도 0.66 (95% CI 0.58-0.73, I2=0.0%), 통합 특이도 0.84 (95% CI 0.82-0.86, I2=0.0%), 통합 AUC 0.83 (95% CI 0.79-0.86) 였다. 임신 34주 이전 전자간증 발생예측을 보고한 3편에서 동 검사의 민감도는 0.35~1.00, 특이도 0.72~1.00, 양성예측도 0.50~0.83, 음성예측도 0.91~1.00 이었다. 전자간증 발생 예측 기간이나 임신주수를 제시하지 않고 전자간증 발생 예측 결과를 제시한 3편에서는 민감도는 0.59~0.92, 특이도 0.68~0.91, 양성예측도 0.32~0.63, 음성예측도 0.89~0.98 이었다. 또한 1편의 문헌에서는 임신 24주 임산부에서 SFLT-1/PLGF 수치가 17 이상일 때 전자간증 발생이 통계적으로 유의하게 증가했다고 보고하였다(OR 37.13 (95% CI 4.78-288.25), p=0.002). 2) 전자간증 진단정확성 전자간증 진단정확성을 보고한 10편에서 임신 34주를 기준으로 34주 이전에 발생하는 초기 전자간증의 진단(early-onset)과 34주 이후에 발생하는 후기 전자간증 진단(late-onset)으로 구분하여 결과를 제시하였다. 10편에서 보고한 초기 전자간증 진단 시 동 검사의 민감도 0.34~1.00, 특이도 0.6~0.98, 양성예측도 0.22~0.98, 음성예측도 0.68~1.00 이었다. NICE 가이드라인에서 제시한 임계치 85를 기준으로 할 때 진단정확도(4편)는 민감도 0.5~0.91, 특이도 0.7~0.98, 양성예측도 0.5~0.98, 음성예측도 0.7~0.99이었다. 5편에서 보고한 후기 전자간증 진단 시 동 검사의 진단정확도는 민감도 0~0.84, 특이도 0.73~1.00, 양성예측도 0.34~1.00, 음성예측도 0.81~0.97이었고, NICE 가이드라인에서 제시한 임계치 110을 기준으로 한 문헌은 없었다. 3) 의료결과와의 관련성 동 검사와 의료결과와의 관련성은 출산 및 입원 예측으로 평가하였다. 4편에서 검사 임계치 38 혹은 85를 기준으로 전자간증 검사 후 1주 혹은 2주 이내 출산 예측결과를 보고하였다. 1편에서는 초기 전자간증 진단 환자 중 검사 후 1주 이내 출산 할 환자가 약 22.1%로 보고하였고 나머지 3편에서는 1주 이내 출산 예측 시 민감도 0.96, 특이도 0.71, 양성예측도 0.42, 음성예측도 0.99로 보고하였고, 2주 이내 출산예측은 민감도 0.58~0.86, 특이도 0.76~0.94, 양성예측도 0.41~0.86, 음성예측도 0.68~0.99로 보고하였다. 검사 후 2주 이내 입원발생을 예측한 1편의 문헌에서 보고한 검사 민감도 0.6, 특이도 0.84로 보고하였다. 20-34주 임산부를 대상으로 NICE 지침에서 제시한 임계치를 적용한 문헌들에서 보고한 검사 후 1주 및 4주 이내 전자간증 발생 예측 시 동 검사의 통합민감도 0.66~0.92, 통합 특이도 0.80~0.84, 통합 AUC 0.82~0.83이었으며, 동 검사의 음성예측도가 0.8~1.00으로 높은 수준으로 확인되어 소위원회에서는 동 검사가 전자간증 발생 여부를 예측하는데 유용한 검사로 판단하였다. 또한 NICE 지침에서 제시한 임계치를 적용한 문헌에서 임신 34주 이전 초기 전자간증 진단 시 동 검사의 민감도는 0.5~0.91, 특이도가 0.7~0.98로 확인되어, 소위원회에서는 초기 전자간증 진단 시 동 검사의 진단정확도가 높은 수준은 아니지만 임상적 소견과 함께 동 검사를 보조적 검사로 사용 시 전자간증 진단에 도움을 줄 수 있는 유용한 검사로 판단하였다. 임신 34주 이후 후기 전자간증 진단에서 NICE 지침의 임계치를 적용한 문헌이 없어 소위원회는 동 검사의 유용성을 평가한 근거가 충분하지 않다고 판단하였다.

경제성 SFLT-1/PLGF [정밀면역검사] (정량)의 경제성은 8편의 문헌을 기준으로 평가하였다. 모두 국외에서 수행된 연구들로, 공통적으로 SFLT-1/PLGF 검사로 전자간증 고위험 산모들을 감별해 불필요한 입원을 줄여 의료자원의 이용 및 비용 감소효과를 보고하였다. 이는 전자간증과 관련하여 과잉진단 및 치료를 줄이고, 신생아의 위허을 예방할 수 있다고 보았다. 이에 소위원회에서는 산모의 임상증상과 함께 동 검사 결과를 참고한 임상적 판단은 국내 임상상황에서도 전자간증으로 인한 불필요한 입원이나 진료횟수를 감소시켜 의료비용 절감효과를 기대할 수 있을 것이라고 판단하였다.

결론 소위원회에서는 SFLT-1/PLGF [정밀면역검사](정량)에 대해 현재 평가 결과 근거하여 다음과 같이 제언하였다. SFLT-1/PLGF [정밀면역검사](정량)은 임신 20-34주 전자간증 의심 임산부를 대상으로 전자간증을 예측 및 진단하는데 안전한 검사로 판단하였다. NICE 지침(2022)의 임계치를 기준으로 임신 20-34주 산모의 1주 및 4주 이내 전자간증 발생 예측을 위한 동 검사의 통합 민감도 및 통합 특이도와 음성예측도가 높은 수준으로 단기간 내 전자간증 발생 예측(rule-out/in)에 유용한 검사로 판단하였다. 초기 전자간증 진단에 있어, 문헌에서 보고한 진단정확도가 높은 수준은 아니지만 동 검사가 임상적 소견과 함께 보조적 검사로 전자간증 진단에 도움을 줄 수 있는 유용한 검사로 판단하였다. 또한 경제성 문헌을 검토하였을 때 산모의 임상증상과 동 검사결과를 참고한 임상적 판단을 통해 불필요한 입원이나 진료횟수를 감소시켜 국내 임상상황에서도 의료비용 절감을 기대할 수 있을 것이라는 의견을 제시하였다. 추가적으로 현재 동 기술 외에 잘 정립된 전자간증 예측검사가 없다는 점과 동 검사가 단기간 내 전자간증 미발생(rule-out)을 예측하여 산모의 불필요한 입원과 치료를 줄일 수 있고, 전자간증 발생이 예측되는 산모는 추적관찰을 통한 적절한 관리 및 조기치료를 위한 추가적인 정보를 제공함으로써 임상에서 전자간증을 예측하는 보완적 검사로서 도움이 된다는 의견이었다. 또한, 국내 35세 이상 고령 산모의 증가추세로, 산전 진찰에서 고위험 산모에 대한 전자간증 진단 및 관리를 위해 우리나라 산모에게 적합한 임계치 설정을 위한 임상연구가 필요하다고 제언하였다. 2023년 제3차 의료기술재평가위원회(2023.3.10.)에서는 소위원회 검토 결과에 근거하여 의료기술 재평가사업 관리지침 제4조제10항에 의거 “SFLT-1/PLGF [정밀면역검사] (정량)”에 대해 다음과 같이 심의하였다. 의료기술재평가위원회는 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 임신 20-34주 전자간증이 의심되는 임산부를 대상으로 전자간증 예측 및 진단하는데 SFLT-1/PLGF [정밀면역검사] (정량)의 사용을 ‘권고함’으로 심의하였다(권고등급: 권고함).

Background The SFLT-1/PLGF (Soluble fms-like Tyrosine kinase-1/Placental Growth Factor) [High quality immunoassay] (quantitative) represents a pivotal diagnostic tool for predicting and diagnosing preeclampsia in pregnant women between 20–34 weeks of gestation with suspected preeclampsia. This test was officially recognized as a safe and effective technology during the fifth session of the New Health Technology Assessment Committee in 2016 (on May 27, 2016) (Ministry of Health and Welfare Notice No. 2016-122; July 5, 2016). Subsequently, it was categorized as a selective benefit item with a 50% co-payment in 2017 (Ministry of Health and Welfare Notice No. 2017-151; September 1, 2017). Upon reaching the three-year conformity assessment cycle as a selective benefit item, the technology underwent a reassessment to update and provide the latest evidence of its clinical safety and effectiveness. This reassessment was initiated following internal monitoring, a review by the Clinical Advisory Board, and a prioritization by the Committee of Health Technology Reassessment. Consequently, a comprehensive reassessment was conducted, as reviewed in the sixth session of the Committee of Health Technology Reassessment in 2022 (on June 10, 2022).

Purpose and methods An extensive systematic review was undertaken to evaluate the safety and effectiveness of the SFLT-1/PLGF [High quality immunoassay] (quantitative). The review methodology was thoroughly finalized after in-depth discussions by the “SFLT-1/PLGF [High quality immunoassay] (quantitative) safety and effectiveness assessment subcommittee (hereafter referred to as the “Subcommittee”). This subcommittee comprised five experts, with two each from obstetrics/gynecology and laboratory medicine, and one specializing in evidence-based medicine. The pivotal question driving this assessment was: “Is the SFLT-1/PLGF [High quality immunoassay] (quantitative) a safe and effective diagnostic tool for predicting and diagnosing preeclampsia in pregnant women with suspected preeclampsia between 20–34 weeks of gestation?” To address this question, a comprehensive search was conducted across three international databases (Ovid MEDLINE, Ovid EMBASE, and Cochrane Central Register of Controlled Trials) and five domestic databases (KoreaMed, KMbase, KISS, RISS, and Science On). Two independent reviewers meticulously screened and selected relevant articles based on strict inclusion and exclusion criteria, performed data extraction, and conducted a risk of bias (RoB) assessment using the Quality Assessment of Diagnostic Accuracy Studies-2 (QUADAS-2). Any disagreements were resolved through collaborative discussions among the reviewers. Data analysis was executed via meta-analysis where quantitatively feasible, supplemented by qualitative reviews in other cases.

Results The systematic review culminated in the selection of 38 articles, encompassing 26 diagnostic test evaluation studies, two prospective cohort studies, two retrospective cohort studies, and eight cost-effectiveness assessment studies. These studies reported on diagnostic accuracy and disease association as key safety and effectiveness outcomes. In the RoB assessment of 29 diagnostic test evaluation studies, concerns regarding applicability in the reference standard and patient selection domains were uniformly categorized as “low”" across all articles. Concerns about applicability in the index test domain were rated as “low” in 69% of articles and “uncertain” in 31%. The RoB in the flow and timing domain was consistently assessed as “low”," whereas in the reference domain, 17.2% of articles had “uncertain” RoB due to ambiguity about whether reference standard test results were interpreted independently of index test results. The remaining 82.8% of studies had “low” RoB in this domain. Concerns in the index test domain were “uncertain” in 20.7% of articles that did not clearly pre-specify a threshold value, and the RoB in the patient selection domain was “uncertain” in 51.7% of articles due to ambiguities related to continuous or random samples.

Safety The safety of the SFLT-1/PLGF [High quality immunoassay] (quantitative) was rigorously evaluated based on an article that specifically addressed test-related adverse events (AEs). This singular study provided a comprehensive analysis and reported no incidents of AEs attributable to the test.

Effectiveness The effectiveness of the SFLT-1/PLGF [High quality immunoassay] (quantitative) was thoroughly assessed through an analysis of 29 articles. These studies encompassed a broad range of aspects, including the prediction of preeclampsia, the diagnostic accuracy for preeclampsia, and the test's correlation with critical health outcomes, notably the prediction of childbirth and hospitalization. The evaluation of effectiveness, especially concerning the prediction of preeclampsia and its diagnostic accuracy, adhered to the threshold values delineated in the 2022 NICE(The National Institute for Health and Care Excellence) guidelines. In cases where data synthesis was not feasible, the studies provided a descriptive range of diagnostic accuracy.

1)Prediction of preeclampsia In the 18 articles that addressed the prediction of preeclampsia, the results varied based on the timing of preeclampsia onset after the test, prediction relative to gestational age, and predictions not specific to any period. Eleven articles focused on the prediction of no preeclampsia onset within one week following the test. In these studies, the test demonstrated a sensitivity range of 0.63–1.00, specificity between 0.63 and 0.85, positive predictive value from 0.09 to 0.78, and negative predictive value spanning 0.80 to 1.00. A meta-analysis of six of these articles, conforming to the NICE threshold of 38, revealed a pooled sensitivity of 0.92 (95% CI 0.80–0.97, I2=61.7%), pooled specificity of 0.80 (95% CI 0.78–0.82, I2=0.0%), and a pooled area under the curve (AUC) of 0.82 (95% CI 0.79–0.85). Prediction of preeclampsia onset within four weeks after testing was reported in nine articles. These studies indicated a sensitivity range of 0.5–1.00, specificity from 0.69 to 0.88, positive predictive value between 0.18 and 0.59, and negative predictive value from 0.84 to 1.00. In the meta-analysis of four of these articles, again based on the NICE threshold of 38, the pooled results showed a sensitivity of 0.66 (95% CI 0.58–0.73, I2=0.0%), pooled specificity of 0.84 (95% CI 0.82–0.86, I2=0.0%), and pooled AUC of 0.83 (95% CI 0.79–0.86). Three articles reported on the prediction of preeclampsia onset before 34 weeks of gestation, with sensitivities ranging from 0.35 to 1.00, specificities between 0.72 and 1.00, positive predictive values from 0.50 to 0.83, and negative predictive values of 0.91 to 1.00. Prediction of preeclampsia without specifying a particular period or gestational age was detailed in three articles, where the test showed a sensitivity of 0.59–0.92, specificity from 0.68 to 0.91, positive predictive value ranging from 0.32 to 0.63, and negative predictive value between 0.89 and 0.98. Additionally, one of these articles highlighted a statistically significant increase in preeclampsia risk when the SFLT-1/PLGF ratio was ≥ 17 at 24 weeks of gestation (OR 37.13 [95% CI 4.78–288.25], p=0.002).

2)Diagnostic accuracy for preeclampsia Ten articles examined the diagnostic accuracy for preeclampsia, categorizing results based on a 34-week gestation cutoff: early-onset (before 34 weeks) and late-onset (at or after 34 weeks). In assessing early-onset preeclampsia, the test demonstrated sensitivities ranging from 0.34 to 1.00, specificities between 0.6 and 0.98, positive predictive values from 0.22 to 0.98, and negative predictive values spanning 0.68 to 1.00. Employing the NICE threshold of 85, four studies presented a diagnostic accuracy with sensitivities of 0.5 to 0.91, specificities from 0.7 to 0.98, positive predictive values between 0.5 to 0.98, and negative predictive values from 0.7 to 0.99. For late-onset preeclampsia, five studies reported sensitivities up to 0.84, specificities as high as 1.00, positive predictive values up to 1.00, and negative predictive values ranging from 0.81 to 0.97. Notably, there were no studies evaluating outcomes based on the NICE threshold of 110 for late-onset preeclampsia.

3)Association with health outcomes The test's association with health outcomes, particularly in predicting childbirth and hospitalization, was assessed in four articles. These studies reported on childbirth prediction within one or two weeks post-testing, employing threshold values of 38 or 85. One study observed predicted childbirth within one-week post-testing in approximately 22.1% of patients diagnosed with early-onset preeclampsia. The other three studies reported prediction of childbirth within one week with a sensitivity of 0.96, specificity of 0.71, positive predictive value of 0.42, and negative predictive value of 0.99, and prediction of childbirth within two weeks with sensitivities ranging from 0.58 to 0.86, specificities between 0.76 and 0.94, positive predictive values from 0.41 to 0.86, and negative predictive values of 0.68 to 0.99. Additionally, these studies noted the prediction of hospitalization within two weeks post-testing with a sensitivity of 0.6 and specificity of 0.84. In the context of pregnant women between 20 and 34 weeks of gestation, the SFLT-1/PLGF high quality immunoassay (quantitative) demonstrated a pooled sensitivity ranging from 0.66 to 0.92, with a pooled specificity of 0.80 to 0.84, and a pooled Area Under the Curve (AUC) of 0.82 to 0.83 for predicting preeclampsia onset within one and four weeks post-testing. Additionally, the test reported a high negative predictive value of 0.8 to 1.00. Based on these robust findings, the subcommittee affirmed the utility of the test in effectively predicting the onset of preeclampsia. Moreover, when applying NICE threshold values, this test showed sensitivity for predicting early-onset preeclampsia before 34 weeks of gestation ranging from 0.5 to 0.91 and specificity from 0.7 to 0.98. Despite not exhibiting high diagnostic accuracy for early-onset preeclampsia, the subcommittee recognized its value as a supplementary diagnostic tool when used in conjunction with clinical findings. For the diagnosis of late-onset preeclampsia at or after 34 weeks of gestation, the subcommittee noted an insufficient evidence base due to the lack of studies incorporating the appropriate NICE threshold values.

Cost-effectiveness The cost-effectiveness of the SFLT-1/PLGF [High quality immunoassay] (quantitative) was analyzed through eight articles. These studies, conducted internationally, consistently indicated that using the SFLT-1/PLGF test for screening mothers at high risk of preeclampsia led to reductions in unnecessary hospitalizations, health resource use, and associated costs. These outcomes suggest the test's role in diminishing over-diagnosis and overtreatment related to preeclampsia, as well as reducing risks to neonates. Consequently, the Subcommittee concluded that clinical decisions informed by the test's findings, alongside the clinical symptoms of mothers, are likely to lead to decreased healthcare costs in Korea by reducing unnecessary hospitalizations and treatment visits for preeclampsia.

Conclusions The Subcommittee presented the following recommendations on the safety and effectiveness of SFLT-1/PLGF [High quality immunoassay] (quantitative), following a thorough review of the current assessment results. The Subcommittee determined that the test is safe for predicting and diagnosing preeclampsia in pregnant women with suspected preeclampsia between 20–34 weeks of gestation. In terms of predicting preeclampsia onset within one week and four weeks of testing for mothers at 20–34 weeks of gestation, based on the NICE threshold values (2022), the test's high pooled sensitivity, specificity, and negative predictive value render it a valuable tool for short-term preeclampsia prediction. Although the diagnostic accuracy for early-onset preeclampsia as reported in the literature was not high, the test was still considered useful as an adjunct tool alongside clinical observations. Additionally, the Subcommittee noted that the test could potentially help reduce healthcare costs in Korea by minimizing unnecessary hospitalizations and treatment visits for preeclampsia, based on its predictive ability in conjunction with clinical findings. The test was also viewed as beneficial in clinical practice as an add on for preeclampsia prediction, especially given the lack of other established alternatives. The Subcommittee suggested the need for clinical trials to establish appropriate threshold values for high-risk mothers, particularly in light of the increasing number of older mothers aged ≥ 35 years in Korea. In the third session of the Committee of Health Technology Reassessment in 2023 (on March 10, 2023), conclusions were drawn regarding the SFLT-1/PLGF [High quality immunoassay] (quantitative) in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment. The Committee “recommends” the use of the SFLT-1/PLGF [High quality immunoassay] (quantitative) for pregnant women with suspected preeclampsia at 20-34 weeks of gestation, based on a comprehensive evaluation of its clinical safety, effectiveness, and other relevant assessment factors (Grade of recommendation: Recommended).
