Abstract

평가배경 갑상선암 수술, 위암 수술, 대장암 수술, 충수절제술, 자궁절제술 등의 수술은 관혈적, 내시경 또는 복강경에 의한 방식으로 수행되며, 해당 중재에는 1회용 절삭기가 이용되고 있다. 국내에서 이용되는 1회용 절삭기에는 초음파, 전파, 전파와 초음파, 다관절 다자유도 바이폴라 절삭기가 있으며 내시경, 복강경 또는 관혈적 수술에서 해당 에너지를 이용하여 조직의 지혈, 절개, 응고에 사용한다. 이 중 1회용 전파와 초음파 절삭기의 경우 관혈적 사용의 경우와 복강경 등 내시경하 수술용 각각 2015년 4월과 2014년 12월에 선별급여 80%로 최초 등재된 이후 현재까지 사용되고 있다. 건강보험심사평가원에서는 선별급여의 적합성 평가를 위해 4종의 1회용 절삭기 사용(1회용 초음파 절삭기, 1회용 전파 절삭기, 1회용 전파와 초음파 절삭기, 1회용 다관절 다자유도 바이폴라 절삭기)의 임상적 효과성에 대한 근거 확인을 위하여 의료기술재평가를 본 원에 의뢰하였고(2022.9.6.), 2022년 제10차 의료기술재평가위원회(2022.10.14.)에서 전문적·심층적 검토를 통해 동 기술의 임상적 안전성 및 효과성 평가가 필요한 안건으로 심의하였다. 이에 본 평가는 체계적 문헌고찰을 통해 1회용 전파와 초음파 절삭기의 임상적 안전성 및 효과성의 근거를 확인하여 정책적 의사결정을 지원하고자 수행하였다.

평가방법 동 기술에 대한 안전성 및 효과성 평가를 위해 체계적 문헌고찰을 수행하였다. 모든 평가방법은 평가목적을 고려하여 “1회용 절삭기를 이용한 수술 소위원회(소위원회)”의 심의를 거쳐 확정하였다. 평가의 핵심질문은 “갑상선암 수술, 위암 수술, 대장암 수술, 충수절제술, 자궁절제술 대상자에서 1회용 전파와 초음파 절삭기 사용은 임상적으로 안전하고 효과적인가?”이며, 안전성은 합병증 및 이상반응 지표로, 효과성은 혈액 소실량, 수술 시간, 재수술률, 배액량, 재원 기간을 지표로 평가하였다. 체계적 문헌고찰은 핵심 질문을 토대로 국외 3개, 국내 5개 데이터베이스에서 검색하여 문헌 선정 및 배제기준에 따라 두 명의 검토자가 독립적으로 선별하고 선택하였다. 선택 문헌의 비뚤림위험 평가는 Cochrane의 Risk of Bias (RoB)를 사용하여 두 명의 검토자가 독립적으로 수행하여 의견합의를 이루었다. 자료 분석은 정량적 분석(quantitative analysis)이 불가능한 경우 정성적(qualitative review) 분석을 적용하였다. 체계적 문헌고찰 결과의 근거 수준은 Grading of Recommendations Assessment, Development and Evaluation (GRADE) 접근 방법으로 평가하였다.

평가결과 1회용 전파와 초음파 절삭기 평가에 포함된 문헌은 총 1편(중재군 25명, 비교군 25명)의 무작위배정비교임상시험으로 부인과 종양에 대한 복강경하 자궁절제술의 수행에서 1회용 전파와 초음파 절삭기 이용군(중재군)과 양극성 겸자 및 냉/단극 가위를 사용하는 기존 기술(비교군)을 비교하였다. 해당 문헌의 안전성 및 효과성의 결과를 정리하면 다음과 같다.

안전성 안전성은 합병증 및 이상반응으로 살펴보았다. 중재군에서는 합병증 및 이상반응이 발생하지 않았고, 비교군에서는 4%(1/25)였으나 통계적으로 유의한 차이는 없었다. 비교군에서 보고된 복강 내 농양 1례는 배액 및 항생제 투여로 치료하였다.

효과성 1회용 전파와 초음파 절삭기 사용의 효과성은 1편의 선택문헌에서 혈액소실량, 수술시간, 재원기간이 제시되었으며, 재수술률 및 배액량은 다루지 않았다. 첫 번째 효과성 지표인 혈액소실량은 두 군 모두 평균 50ml 수준으로 유사한 수준이었다. 두 번째 효과성 지표인 수술시간은 중재군에서 평균 85분으로 비교군의 115분보다 약 30분 단축되는 효과를 보고하였다. 상세 수술별로는 자궁절제술에서 약 10분 단축, 림프절제술에서 약 15분 유의하게 단축되었다. 세 번째 효과성 지표인 재원기간은 두 군 모두 평균 3일로 군간 유의한 차이가 없었다.

결론 및 제언 소위원회에서는 현재 문헌적 근거를 바탕으로 다음과 같이 결론을 제시하였다. 갑상선암 수술, 위암 수술, 대장암 수술, 충수절제술, 자궁절제술 등에서 1회용 전파와 초음파 절삭기를 이용한 수술을 기존 지혈법을 이용한 수술과 비교하였을 때, 합병증 및 이상반응 발생률은 유사한 수준이므로 안전한 의료기술로 판단하였다. 그리고 기존 장비 및 방법과 비교하여 1회용 전파와 초음파 절삭기 사용은 혈액소실량 및 재원기간이 동등한 수준이었고, 수술시간은 다소 감소하는 경향을 보여 1회용 전파와 초음파 절삭기의 사용은 안전하고 효과적인 것으로 판단하였다. 동 절삭기 관련 문헌이 1편에 불과한 점에 대하여 소위원회는 에너지를 이용한 절삭기 이용은 기술적 발전에 따른 자연스러운 세대이전으로 보아야 하며, 수술 시 절삭기는 환자 및 혈관상태에 맞게 술자의 선택에 따른 사용이므로 절삭기 간의 안전성과 효과성은 유사할 것으로 보았다. 따라서 1회용 초음파 절삭기와 1회용 전파 절삭기의 평가근거를 1회용 전파와 초음파 절삭기의 평가근거로 고려하는 것에 문제가 없다는 의견이었다. 이에 수술 시 1회용 전파와 초음파 절삭기의 사용은 효과적인 것으로 평가하였다. 2023년 제5차 의료기술재평가위원회(2023.5.12.)에서는 소위원회 검토 결과에 근거하여 의료기술재평가사업 관리지침 제4조제10항에 의거 “1회용 전파와 초음파 절삭기”에 대해 다음과 같이 심의하였다. 임상적 안전성과 효과성의 근거 및 그 외 평가항목 등을 종합적으로 고려하였을 때, 국내 임상상황에서 수술 시 1회용 전파와 초음파 절삭기의 사용을 ‘조건부 권고함’으로 심의하였다.

Background In the evolving landscape of surgical practices, a diverse array of procedures including thyroid cancer surgery, gastric cancer surgery, colorectal cancer surgery, appendectomies, and hysterectomies are conducted utilizing open, endoscopic, or laparoscopic techniques. A pivotal element in these procedures is the disposable scalpel. Within the Korean medical landscape, the spectrum of disposable scalpels encompasses ultrasonic, bipolar, and ultrasonic and bipolar models, alongside multi-degree of freedom articulating laparoscopic instruments. These instruments, characterized by their specific energy profiles, play a critical role in hemostasis, precision in incision, and tissue coagulation across the various surgical modalities. Notably, the disposable ultrasonic and bipolar energy device, designed for both open and endoscopic surgeries, including laparoscopic applications, was first introduced as a selective benefit item with an 80% co-payment in April 2015 for open surgery and in December 2014 for endoscopic surgery, and continues to be widely utilized. On September 6, 2022, the Health Insurance Review and Assessment Service tasked the National Evidence-based Healthcare Collaborating Agency with the reassessment of four distinct types of disposal scalpels (disposable ultrasonic surgical scalpel, disposable energy-based vessel sealing system, disposable ultrasonic and bipolar energy device, and disposable multi-degree of freedom articulating laparoscopic instrument) . This initiative forms part of the conformity assessment of selective benefit items, aiming to ascertain their clinical efficacy. Subsequently, in the tenth session of the Committee of Health Technology Reassessment in 2022 (on October 14, 2022), this specific health technology was designated as a priority agenda item for a detailed assessment of its clinical safety and effectiveness. A systematic review was thus conducted to gather robust evidence to inform policy decision-making regarding the clinical utility of the disposable ultrasonic and bipolar energy device.

Methods A systematic review was meticulously conducted to evaluate the safety and effectiveness of this innovative health technology. The methodology was thoroughly refined and finalized following extensive discussions by the “surgery using disposable scalpel subcommittee” (hereafter referred to as the “Subcommittee”), with a focus on aligning with the objectives of the assessment. The pivotal question posed for assessment was: “Do disposable ultrasonic and bipolar energy devices exhibit clinical safety and efficacy when employed in thyroid cancer surgery, gastric cancer surgery, colorectal cancer surgery, appendectomy, and hysterectomy?” The safety evaluation encompassed an analysis of complications and adverse events (AEs), while effectiveness was gauged based on parameters such as blood loss, operative duration, reoperation rate, drainage volume, and length of hospital stay. In executing the literature review, a comprehensive search was conducted in three international and five domestic databases, guided by the central research question. Two independent reviewers meticulously screened and selected relevant articles, adhering to pre-established inclusion and exclusion criteria. The risk of bias (RoB) within these studies was rigorously assessed using the Cochrane RoB tool, achieving consensus between the reviewers. For data synthesis, qualitative analysis methodologies were employed in instances where quantitative approaches were not feasible. The results of the systematic review were subsequently evaluated for their evidence level using the Grading of Recommendations Assessment, Development, and Evaluation (GRADE) methodology.

Results The systematic review process culminated in the inclusion of only one randomized controlled trial (RCT), which encompassed 25 patients in each of the intervention and control groups, in the assessment of disposable ultrasonic and bipolar energy devices. This singular study embarked on a comparative analysis of the use of disposable ultrasonic and bipolar energy devices (intervention group) against the conventional surgical method employing bipolar forceps and cold/monopolar scissors (control group) during laparoscopic hysterectomy for gynecologic tumors. The study reported on the safety and effectiveness outcomes.

Safety The assessment of safety was meticulously conducted by examining the incidence of complications and AEs. In the intervention group, there were no recorded complications or AEs. Conversely, the control group exhibited a complication rate of 4% (1 out of 25 participants). This single incidence, an intra-abdominal abscess, was clinically managed with drainage and antibiotic therapy. However, it is important to note that the difference in complication rates between the two groups did not achieve statistical significance.

Effectiveness The effectiveness of the disposable ultrasonic and bipolar energy device was evaluated based on the only study selected for this review. This study reported on three key effectiveness indicators: the amount of blood loss, operative time, and length of hospital stay, while omitting data on reoperation rate and drainage volume. The first indicator, mean blood loss, was comparable between the two groups, averaging around 50 mL. As for the second indicator, operative time, the study found a notable reduction in the intervention group. The average operative time was approximately 30 minutes shorter in the group using the device (85 minutes) compared to the control group (115 minutes). Specifically, in surgeries like hysterectomy and lymphadenectomy, the time reduction was significant, amounting to about 10 and 15 minutes, respectively. Regarding the third indicator, the mean length of hospital stay was identical for both groups, at three days, demonstrating no significant difference between them.

Conclusions and recommendations Following a thorough evaluation of the available literature, the Subcommittee formulated the following conclusions: The use of disposable ultrasonic and bipolar energy devices in surgeries such as thyroid cancer surgery, gastric cancer surgery, colorectal cancer surgery, appendectomy, and hysterectomy is deemed a safe health technology. This conclusion is based on the observation that these devices exhibit a similar rate of complications and AEs as compared to conventional hemostatic methods. Furthermore, in terms of effectiveness, the use of these devices demonstrated comparable levels of blood loss and lengths of hospital stays, with a tendency towards slightly reduced operative times compared to conventional equipment and methods. Therefore, it is established that the disposable ultrasonic and bipolar energy device is both safe and effective for surgical use. Given the limited number of studies specifically reporting on this device, the Subcommittee recognizes the adoption of energy-utilizing scalpels and dissectors as a natural progression within the landscape of technological advancements in surgery. The choice of surgical instruments, be it a scalpel or dissector, is dictated by the surgeon based on patient-specific factors and vascular considerations. Consequently, it is reasonable to infer that the safety and effectiveness of different types of scalpels and dissectors are comparable. Thus, it is deemed appropriate to consider the evidence from assessments of other similar surgical instruments, such as disposable ultrasonic scalpels and energy-based vessel sealing systems, as applicable evidence for the disposable ultrasonic and bipolar energy device. During the fifth session of the Committee of Health Technology Reassessment in 2023 (on May 12, 2023), conclusions regarding the disposable ultrasonic and bipolar energy device, based on the subcommittee findings, were formulated in accordance with Article 4-10 of the Guidelines for Management of Health Technology Reassessment. Based on an integrative analysis of clinical safety, effectiveness, and other relevant assessment criteria, the disposable ultrasonic and bipolar energy device is “conditionally recommended” for surgical use in the clinical setting in Korea.
